Abstract
Полезная модель относится к медицине, в частности к ортопедии, и может быть использована для хирургического лечения врожденного вывиха головки бедренной кости, а именно для осуществления шевронной укорачивающей остеотомии бедренной кости при тотальном эндопротезировании тазобедренного сустава. Нами впервые разработано устройство в виде V-образной пластины, выполненной из гибкого металла для возможности ее моделирования по окружности бедренной кости. Пластина представляет собой по форме два одинаковых параллелограмма, соединенных по короткой стороне. Между двумя верхними длинными сторонами, а также между двумя нижними длинными сторонами параллелограммов образован угол в 70-80°. Величина этого угла диктуется необходимостью создания более обширной площади контакта остеотомированных фрагментов бедренной кости. Длина коротких сторон параллелограммов равна высоте удаляемого костного фрагмента, определяемой в предоперационном периоде по рентгенограммам тазобедренных суставов в переднезадней проекции. На общей короткой стороне параллелограммов имеются два отверстия диаметром 2 мм для фиксации пластины к бедренной кости и обеспечения ее неподвижности при выполнении остеотомии. Разработанное устройство для шевронной укорачивающей остеотомии бедренной кости при тотальном эндопротезировании тазобедренного сустава у пациентов с врожденным вывихом бедра позволяет максимально точно сопоставить остеотомированные костные фрагменты, что увеличивает их площадь контакта и ротационную стабильность, ускоряя репаративные процессы и снижая риск несращений, тем самым сокращая сроки реабилитации пациентов и улучшая результаты оперативного лечения.The invention relates to medicine, in particular to orthopedics, and can be used for the surgical treatment of congenital dislocation of the femoral head, namely, for the implementation of chevron shortening osteotomy of the femur with total hip arthroplasty. The length of the short sides of the parallelograms is equal to the height of the bone fragment to be removed, which is determined in the preoperative period by radiographs of the hip joints in the anteroposterior projection. On the common short side of the parallelograms there are two holes with a diameter of 2 mm for fixing the plate to the femur and ensuring its immobility when performing osteotomy. The developed device for chevron shortening osteotomy of the femur with total hip joint arthroplasty in patients with congenital dislocation of the femur makes it possible to accurately compare osteotomized bone fragments, which increases their contact area and rotational stability, accelerating reparative processes and reducing the risk of non-joints, thereby reducing the periods of contact and improving the results of surgical treatment.

На фиг. 1 представлено предлагаемое устройство, где 1 - верхние длинные стороны параллелограммов, 2 - нижние длинные стороны параллелограммов, 3 - короткие стороны параллелограммов, 4 - отверстия для фиксации пластины, 5 - угол между длинными сторонами параллелограммов. Пластина представляет собой по форме два одинаковых параллелограмма, соединенных по короткой стороне. Между двумя верхними длинными сторонами, а также между двумя нижними длинными сторонами параллелограммов образован угол в 70°-80°. Величина этого угла диктуется необходимостью создания более обширной площади контакта остеотомированных фрагментов бедренной кости. Длина коротких сторон параллелограммов равна высоте удаляемого костного фрагмента, определяемой в предоперационном периоде по рентгенограммам тазобедренных суставов в переднезадней проекции. На общей короткой стороне параллелограммов имеются два отверстия диаметром 2 мм для фиксации пластины к бедренной кости и обеспечения ее неподвижности при выполнении остеотомии. Производят установку вертлужного компонента и полиэтиленового вкладыша в истинную вертлужную впадину. Осуществляют опил головки бедренной кости и вскрытие костномозгового канала, после чего производят его обработку рашпилями до необходимого размера. На расстоянии 1,5-2 см дистальнее малого вертела накладывают отмоделированную по окружности бедренной кости предлагаемую пластину, фиксацию которой проводят путем введения двух спиц Киршнера диаметром 1,5 мм или 1,8 мм в фиксационные отверстия. При помощи пилы осуществляют укорачивающую остеотомию бедренной кости. Удерживая костные фрагменты костодержателем, осуществляют повторную обработку костномозгового канала рашпилем необходимого размера и установку феморального компонента. При помощи пробной головки осуществляют подборку необходимого офсета с проверкой сустава на стабильность, после чего производят ее замену на установочную и вправление. Полость сустава промывают большим количеством антисептического раствора. Производят ушивание капсулы и дренирование полости сустава трубкой.
