Usage: in medicine, namely in cardiovascular surgery. The purpose of the invention is to improve venous circulation. The essence of the invention is that anastomosis is additionally applied between the distal coronary artery and the proximal large vein of the heart. The advantage is improved functional results.
Description
The invention relates to medicine, in particular to experimental surgery of coronary vessels, and can be used to connect the systemic artery to a heart vein.
The aim of the invention is to improve venous circulation
Поставленна цель достигаетс тем, что в известном способе торацико-коронарного шунтировани путем анастомозировани большой вены сердца и внутренней грудной артерии согласно изобретению дополнительно накладывают анастомоз между дистальным отделом коронарной артерии и проксимальным отделом большой вены сердца .The object is achieved by the fact that in the known method of thoracic-coronary artery bypass grafting by anastomosis of the large vein of the heart and the internal thoracic artery according to the invention, anastomosis is additionally applied between the distal coronary artery and the proximal section of the large vein of the heart.
The method is carried out as follows. To create a model of acute myocardial ischemia, we used the generally accepted technique for ligation of the anterior interventricular branch of the left coronary artery at the border of its upper and middle third. A dog under morphine-hexanol anesthesia with artificial ventilation of the lungs carried torocogomy in the 4th interstitial tract and pericardotomy. Mobilized at a distance of 10 12 cm from connecting the artery, the internal thoracic artery was ligated and intersected. A large vein of the heart stood out at the border of its upper and middle third, mobilized over a length of 1.5-2 cm. Bandaged and intersected. The proximal end of the internal thoracic artery was anastamosed with the distal end of the large vein of the heart. Then they started to isolate and mobilize the anterior interventricular branch of the left coronary artery. At the border of its upper and middle third, after which a second anastomosis was placed between the distal (from the ligation site) section of the coronary artery and the proximal end of the large vein of the heart. Anastomosis performed end-to-end

Рана послойно ушивалась, пунктировалась плевральна полость и удал лс воздух.blood flow from the coronary artery to the distal section of the large vein of the heart, and then a retrograde blood flow from the intravenous thoracic artery into the proximal section of the large vein of the heart was switched on, thereby completely reversing the blood flow. The wound was sutured in layers, the pleural cavity punctured and air removed.
2 series of experiments were carried out on 30 outbred dogs weighing from 7 to 20 kg with a model of acute myocardial ischemia caused by ligation of the anterior ventricular branch of the left coronary artery. In the main group (15 dogs), an anastomosis was applied between the internal thoracic artery and the large vein of the heart, its distal end, as well as between the distal end of the coronary artery and the proximal part of the large vein of the heart. In the control group (15 dogs), only one anastomosis was performed between the internal thoracic artery and the large vein of the heart.

При этом за счет анастомоза между дистальным концом коронарной артерии и проксимальным концом большой вены сердца обеспечиваетс снижение давлени в большой вене сердца и создаютс оптимальные услови дл оттока крови от тканей пораженного миокарда в венозную систему, улучшаетс дренаж тканей миокарда, улучшаетс их ок- сигенаци .blood through the internal thoracic artery through the large vein of the heart retrograde in the direction of the ischemic myocardium, which improves its blood supply. Moreover, due to the anastomosis between the distal end of the coronary artery and the proximal end of the large vein of the heart, pressure is reduced in the large vein of the heart and optimal conditions are created for the outflow of blood from the tissues of the affected myocardium into the venous system, the drainage of myocardial tissues is improved, and their oxygenation is improved.
