Description
The proposed utility model relates to the field of medical instrumentation and can be used in intensive care to provide respiratory support for newborns with acute respiratory failure.
Non-invasive artificial lung ventilation (hereinafter referred to as mechanical ventilation) devices are known and considered to be the most effective in the treatment of newborn respiratory failure using respiratory circuits containing devices designed to interface with the patient, as well as pneumatic elements that contribute to the creation of an excess compared to atmospheric pressure in the respiratory Patient pathways (Continuous positive airway pressure), denoted by the widespread abbreviation CPAP (see, for example, WO 2012047903, 2012.04.12; WO 2012047900, 2012.04.12; US 2009165799, 2009.07.02). These pneumatic elements, (hereinafter referred to as CPAP pneumatic elements), are a variety of pneumatic devices, the pneumatic resistance of which towards the patient is significantly less than in the opposite direction. Their sizes range from very miniature to large, and designs - from pneumatic accumulators of the breathing mixture, to asymmetric venturi and non-return pneumatic valves.
In well-known non-invasive mechanical ventilation devices, the CPAP miniature pneumatic element can be integrated directly into patient interface devices, which are, for example, nasal cannulas, nasal masks, etc., and can be used either as an integral part of the mechanical ventilation device, or as an interacting element, including one-time.
A known device designed for non-invasive mechanical ventilation (see US 2012304995 A1, 2012.12.06), interacting with the respiratory circuit, including the CPAP pneumatic element and a device for interfacing with the patient.
The known apparatus comprises a pneumatic injection channel, a respiratory mixture flow control module equipped with a pneumatic inlet for oxygen supply, a pneumatic air inlet and a pneumatic outlet for connecting to the pneumatic discharge channel, a data input module, a control unit electrically connected to a control input of the respiratory mixture flow control module, and a pressure sensor installed at the outlet of the breathing circuit and electrically connected to the control unit. The control unit of this device is designed to store the set pressure value entered into it via a bi-directional communication line from the data input module and compare the actual pressure value measured by the pressure sensor with the set value, and if they diverge, to calculate the corresponding correction, and then, based on the actual pressure value and the calculated correction - to calculate the required value of the constant flow rate of the respiratory mixture and the issuance of the corresponding control signals in fashion To control the flow rate of the respiratory mixture, which is designed to form the flow of the respiratory mixture necessary to maintain the required constant flow rate. The generated flow of the respiratory mixture enters through the injection channel into the patient's respiratory circuit.
A disadvantage of the known apparatus is that the creation of a constant flow rate of the respiratory mixture cannot guarantee a constant excess pressure in the patient's respiratory circuit, as a result of which it is possible to increase this pressure above the required value with a prerequisite for pulmonary barotrauma, as well as insufficiency causing adhesion of the alveoli.
US 2002 2078958 А1, 2002.06.27). This drawback is eliminated in the closest to the claimed utility model and adopted for the prototype device, designed for non-invasive artificial ventilation of the lungs of newborns with pressure control in the respiratory circuit (see US 2002 2078958 A1, 2002.06.27). The known device interacts with the respiratory circuit, including the CPAP pneumatic element, which is built into the device for interfacing with the patient, and contains a pressure pneumatic channel, a respiratory mixture flow control module, pneumatically connected with its inputs to an oxygen source and an air source and connected with its output to the input of the pressure pneumatic channel, the output which is designed to be connected to the breathing circuit, data input module, at least one pressure sensor, at least one of which copulating output breathing circuit, and a control unit, whose respective inputs are connected, each with a corresponding electrical output of one of said pressure sensors, and the output control input c module regulating breathing gas flow. The control unit of this device is designed to compare the actual value of the pressure of the respiratory mixture at the outlet of the respiratory circuit, measured by the pressure sensor, with the set pressure value entered into this unit from the data input module via the bi-directional information communication line, and the continuous generation of control signals to the control input of the regulation module the flow rate of the respiratory mixture designed to continuously regulate the flow of the respiratory mixture in order to maintain an almost constant ass pressure in the respiratory tract of the newborn during the entire mechanical ventilation procedure.
A disadvantage of the known device is the inability to measure the pneumatic parameters of the specific respiratory circuit interacting with it, such as, for example, the time constant of the CPAP pneumatic element and the pneumatic resistance of the respiratory circuit, which makes it difficult, and in some cases makes it impossible to interact with certain types of respiratory circuits.
The objective of the proposed utility model is to expand the operational capabilities of the proposed device through the use of non-invasive mechanical ventilation of newborns of any type of respiratory circuit containing various CPAP pneumatic elements and various devices for interfacing with the patient, as well as to increase the safety of newborns when using them.
The solution to this problem is achieved by the fact that the apparatus of non-invasive artificial ventilation of the lungs of newborns, interacting with the respiratory circuit, including the CPAP pneumatic element and a device for interfacing with the patient, containing a respiratory mixture flow control module, a control unit, a data input module, a pneumatic injection channel, as well as the first and second pressure sensors, and the respiratory mixture flow control module is equipped with a pneumatic inlet for air supply, a pneumatic inlet for oxygen supply and a pneumatic outlet ohm for connecting to the input of the discharge pneumatic channel, the output of which is intended to be connected to the respiratory circuit, while the first pressure sensor is equipped with a pneumatic inlet for connecting to the output of the respiratory circuit and contains an electrical output connected to the corresponding input of the control unit containing an electrical output connected to the control input respiratory mixture flow control module, and the corresponding input connected to the output of the data input module by a bi-directional information line tie, supplemented with new elements and connections.
According to the utility model, the control unit of the apparatus proposed additionally includes a correction coefficient module containing the first and second calculators, and a pressure stabilization module containing a memory of preset pressure values and a stabilization circuit, both modules being interconnected by a corresponding bi-directional communication line, and a pneumatic input the second pressure sensor is connected to the output of the discharge pneumatic channel, while all of the above elements of the proposed device: control unit Manifestations, a module for regulating the flow rate of the respiratory mixture, a data input module, an injection pneumatic channel, and both pressure sensors are located in a common housing. Both of these calculators are designed to calculate and store the values of correction factors characterizing the pneumatic parameters of the interacting respiratory circuit, which are automatically determined by the said calculators when testing the respiratory circuit before the start of the ventilator procedure. The first calculator is designed to determine and store the value of the first correction factor characterizing the time constant of the CPAP pneumatic element, equal to the period of time for which at a given value of the constant flow rate of the respiratory mixture, the specified pressure value is reached at the output of this pneumatic element. The second computer is designed to determine and store the value of the second correction factor characterizing the pneumatic resistance of the respiratory circuit in the forward direction of the flow of the respiratory mixture, proportional to the pressure difference between the input and output of this circuit, equal to the difference in the readings of the second and first pressure sensors. The pressure stabilization module is designed to calculate the flow rate of the respiratory mixture required to create and maintain a predetermined value of the constant pressure of the respiratory mixture at the outlet of the respiratory circuit throughout the ventilation procedure and generate a control signal for supplying the respiratory mixture flow control module to the control input through the pressure stabilization module . The calculation of the required flow rate in the control unit is based on the readings of the first pressure sensor taking into account the set pressure value and the values of the first and second correction factors. Thanks to the proposed technical solution, a constant overpressure is established in the respiratory tract of the newborn when using any type of respiratory circuit, which corresponds to the set value necessary for effective respiratory support of the patient, which reliably ensures the effectiveness of mechanical ventilation and protection of the newborn during the procedure.

The proposed device operates as follows.
14.Before starting the ventilator procedure, the pneumatic outlet 18 of the injection pneumatic channel 4 is connected to the input of the breathing circuit 5, the pneumatic inlet 19 of the first pressure sensor 8 is connected to the output of the respiratory circuit 5, and the pneumatic inputs 2 and 3 of the flow control module 1 are connected to the sources of compressed gas: oxygen O 2 and air B , respectively, after which, using the data input module 10, the value of the set pressure at the output of the respiratory circuit 5, which is stored in the cell, is entered into the control unit 9 via a bi-directional information line 15 yke memory module 14 stabilize the pressure.
After performing the above preliminary operations, the first and second calculators 12, 13, respectively, of the correction factor module 11 automatically calculate the values of the first and second correction factors, respectively, characterizing the pneumatic parameters of the connected respiratory circuit 5. To determine the value of the first coefficient in the first calculator 12, the time interval is automatically measured establishment of excess pressure at the entrance to the respiratory circuit, based on the measured value is calculated by toyannaya time Pneumoelements CPAP 7 and its value is determined by the numerical value of the first coefficient. The numerical value of the second coefficient is automatically determined in the second calculator 13 based on the difference between the readings of the second 17 and the first 8 pressure sensors, characterizing the pressure difference between the input and output of the respiratory circuit 5; each of the two numerical values found is stored in the corresponding calculator 12, 13. After the automatic adjustment of the proposed device is completed, the coupler 6 is connected to the patient’s airways and, if necessary, depending on the depth of inhalation-exhalation, the value of the preset pressure is slightly adjusted, after which ventilation procedure. During the operation of the proposed device in the pressure stabilization module 14 using the stabilization circuit 16 continuously during each inhalation-exhalation cycle, the value of the flow rate of the respiratory mixture is automatically determined, which is necessary to create a constant value of the preset pressure at the output of the respiratory circuit 5, adjusted taking into account the first and second correction factors characterizing the connected respiratory circuit 5. The values of these coefficients enter the pressure stabilization module 14 from the correction module coefficients 11 on the connecting bidirectional information line. The pressure stabilization module 14 continuously issues the corresponding control signals to the flow control module 1 during the breathing cycle. The flow control module 1 supplies the breathing mixture to the breathing circuit 5 through the air supply channel 4, the flow rate of which is determined by a continuous sequence of the said control signals. In the process of breathing a newborn, the pressure at the outlet of the respiratory circuit changes cyclically: during inhalation, it decreases, and during exhalation it rises. The first pressure sensor 8 captures the pressure changes that occur at the output of the breathing circuit 5 and with high speed gives information signals to the control unit 9, which generates and issues the corresponding control signals to the flow control module 1 to change the flow rate of the breathing mixture so that the pressure value on the output of the respiratory circuit during the respiratory cycle remained essentially constant and equal to the specified value.
Maintaining a constant value of the pressure of the respiratory mixture at the outlet of any breathing circuit connected to the proposed device significantly increases the safety of the ventilator procedure, makes it more efficient and expands the operational capabilities of the device.
Thus, the task of the utility model, which consists in providing the possibility of using non-invasive mechanical ventilation of newborns of any type of respiratory circuit with various CPAP pneumatic elements and various coupling devices, to increase the safety of the newborn and to expand the operational capabilities of the proposed product, has been successfully solved using the proposed technical solutions.
