Description
Foreliggende oppfinnelse angår galliumforbindelser som er lett absorberbare ved peroral administrasjon.
Salter av metaller fra 3. hovedgruppe har i en tid vært kjent for å ha antitumor-aktivitet. Relativt nylig har det vært vist at gallium reduserer serumkalsium hos pasienter med hyperkalsemi. Gallium bevirker sistnevnte effekt ved å hemme resorpsjon av kalsium fra benmateriale. Det øker også ben-styrken slik at det ville være egnet for behandling av for-styrrelser som har sammenheng med aksellerert tap av benvev og nedsatt styrke i benmaterialet (se f.eks. US-patent 4.704.277 og US-patent 4.529.593.
I praksis har det vært vanskelig å benytte galliumterapi ved hyperkalsemi. Det har vært angitt at nyretoksisiteten er dose-begrensende når gallium administreres som en i.v.-bolus. En 7 dagers kontinuerlig i.v.-infusjon av gallium oppviste ingen renal toksisitet ved behandling av cancer-assosiert hyperkalsemi, men selv om terapien er effektiv, er den tung-vint. For å forenkle administrasjonen ved galliumterapi både for cancerkjemoterapi og hyperkalsemi, og for å utvide bruks-området for galliumterapi til aktuelle benlidelser, er en peroral doseringform for gallium sterkt ønskelig.
Medikamentabsorpsjon fra den gastrointestinale trakt skjer ved pH 4,5-7. I dette pH-området hydrolyseres gallium(III)-akvaionet i stor grad til uløselige hydroksyder og absorberes meget dårlig. Daglige perorale doser på 400 mg GaCl^ til pasienter med lungekreft, ga gjennomsnittlige galliumkonsentrasjoner i serum på 371 ± 142 ug/ml. Gallium i et passende koordinasjonsmiljø er imidlertid stabilt overfor hydrolyse i vandig miljø ved biologisk relevante pH.
Foreliggende oppfinnelse tilveiebringer nye hydroksamsyre-komplekser av gallium(III) som sammenlignet med gallium-salter, fører til høye serumnivåer av gallium ved peroral administrasjon. Strukturelt kan disse kompleksene gjengis med den følgende formel I:
hvor R1er C^-Cg-n-alkyl og R2er H eller e^-C^-alkyl eller R±og R2 sammen danner tetra- eller pentametylen.

Disse kompleksene ér ikke tidligere fremstillet i farmasøytisk akseptabel form. Forbindelsen med formel I hvor R er Cg-n-alkyl og R2 er CH3, har vært omtalt i US-patent 4.741.887 som et ekstrakt ved ekstraksjon av gallium fra vandige oppløsninger som også inneholder aluminium, jern og sink.
Som representative for forbindelsene i henhold til oppfinnelsen, kan følgende nevnes:
Kompleksene i henhold til oppfinnelsen og de nødvendige utgangsmaterialer, kan fremstilles analogt med kjente frem-gangsmåter, illustrert nedenfor. De gallium-holdige utgangs-materialene er enkle salter av gallium, så som Ga(N03)3eller GaCl3, eller suspensjoner av nylig utfelt Ga(OH)3<Hydroksam-syrene er kommersielt tilgjengelig eller kan fremstilles ved omsetning av et passende hydroksylamin med en karboksylsyre-ester eller -klorid for å gi den fri hydroksamsyre eller saltet av denne. Som nevnt gir kompleksene i henhold til oppfinnelsen, god peroral absorpsjon av gallium i forhold til kommersielt tilgjengelige preparater benyttet for behandling av cancer-relatert hyperkalsemi ved in vivo undersøkelser på rotter i henhold til beskrivelsen nedenfor.Forbindelsene er anvendelige for å øke kalsiuminnholdet i benvev og for å nedsette benresorpsjon når de administreres i en effektiv mengde.
De virksomme kompleksene i henhold til foreliggende oppfinnelse, kan administreres i form av et farmasøytisk preparat formulert i henhold til velkjente prinsipper. Preparatene omfatter således virkestoffet, fortrinnsvis som en enhetsdose, i blanding med et farmasøytisk akseptabelt fortynnings- eller bæremiddel. De virksomme kompleksene i henhold til oppfinnelsen, er fastslått å ha særlig virkning når de tas peroralt, og foretrukne preparater er derfor slike som er formulert i form av kapsler, tabletter, drasjeer eller andre faste tilberedninger, eller som en oppløsning eller suspensjon, for eksempel som en sirup for peroral administrasjon. Passende fortynnings- og bæremidler og andre komponenter, samt formuleringsmetoder, er i alminnelighet kjent.
Selv om de virksomme kompleksene i henhold til oppfinnelsen, er særlig egnet for peroral administrasjon, anses ikke oppfinnelsen som begrenset til behandlingsmetoder og preparater utelukkende for peroral administrasjon.
Preparater for injeksjon, suppositorier, former med forsinket frigjøring eller for implantasjon og lignende, kan formuleres på konvensjonell måte og kan gi fordeler ved bestemte behandlingsforløp eller ved kombinasjonsterapi.
Foreliggende oppfinnelse tilveiebringer dessuten en fremgangsmåte for behandling av betydelig kalsiumtap fra benvev hos pasienter som har behov for slik behandling, og andre anvendelser som her er nevnt, som består i å administrere pasienten en effektiv dose av et aktivt kompleks med formel I. Fortrinnsvis er administrasjonsmåten peroral.
Doseringen kan hensiktsmessig ligge i området fra 0,1 til 100 mg/kg legemsvekt. Fortrinnsvis er doseringen tilstrekkelig til å opprettholde et nivå på 1 til 1,5 ug gallium per ml blod, og dosen kan hensiktsmessig ligge i området 0,5 til 1,5 g galliumforbindelse per dag. En slik dose kan administreres som en enkelt enhetsdose eller i en rekke mindre enhetsdoser. Andre virkestoffer kan administreres separat eller sammen med galliumkomplekset. Supplementær terapi kan dessuten inngå i løpet av behandlingen av en pasient.
Eksempel 1
Syntese av CH3 CON( OH) CH3
Til 5,27 g CH3NH0H.HC1 i 35 ml MeOH ble det ved 0°C under omrøring dråpevis tilsatt 22 mlEt3/N. Etter omrøring av suspensjonen i 0,5 timer, ble 5,78 gCH3COCl dråpevis tilsatt i løpet av 5-10 minutter under kraftig omrøring. Etter at suspensjonen hadde fått oppvarmes til romtemperatur, ble det utfelte Et3/N.HC1 frafiltrert og vasket med eter. Eter-vaskevæskene ble kombinert med filtratet som deretter ble inndampet til tørrhet på rotasjonsfordamper og omrørt med 150 ml Et20 i 10 minutter. Suspensjonen ble filtrert og filtratet inndampet for å etterlate 4,3 g gul olje. Den gule oljen ble destillert under oppsamling av alt lavere enn 90°C ved 50 pm Hg.
Syntese av [ CH3 CON( 0) CH3]3Ga
En klorid-fri suspensjon av nylig utfelt Ga(OH)3fra
20 ml 1,1M vandig GaCl3i 120 ml deionisert vann ble omrørt med 3,6 g CH3CON(OH)CH3 i 16 timer. Suspensjonen ble sentrifugert ved 10.000 rpm. x 30 minutter og supernatanten inndampet til tørrhet på rotasjonsfordamper. Residuet ble omrørt med varm absolutt etanol og suspensjonen sentrifugert ved 15.000 rpm. x 40 minutter. Supernatanten ble avdekantert og volumet redusert på rotasjonsfordamper.
Tilsetning av eter fullførte utfellingen. Faststoffet ble filtrert, vasket med aceton og tørket.

Syntese av CH3 CON( OH) CH2CH3
En blanding av 5 g N-etylhydroksylamin.HCl og 5,4 g Na2C03 ble omrørt ved 0°C i 1 time. 4,02 g acetylklorid ble tilsatt dråpevis under omrøring mens temperaturen ble holdt ved 0°C. Suspensjonen ble filtrert og filtratet inndampet til en olje som ble destillert ved 1 mm Hg 85°C.
Syntese av [ CH3 CON( 0) CH2 CH3J3Ga
Til en vandig suspensjon av kloridfri Ga(0H)3i 40 ml H20, fremstillet fra 1,6 g Ga(N03)3.<9H>2<0>, ble det tilsatt 1,18 g CH3CON(OH)CH2<CH>3. Suspensjonen ble omrørt over natten, filtrert og inndampet til en olje. Oljen ble oppløst i aceton og tilsatt eter for å utfelle et hvitt faststoff som ble frafiltrert, vasket og tørket.

Syntese av Br( CH2)4 C( 0) NH( OCH2C6H5)
En suspensjon av 18,75 g NH2OCH2(CgH^).HCl i 200 ml CH2C12 ble avkjølt til 0°C i et is/NaCl-bad. 30,35 ml trietylamin ble tilsatt. En oppløsning av 5-bromvalerylklorid i 70 ml CH2C12ble dråpevis tilsatt mens temperaturen i den kalde suspensjon ble holdt mellom 0 og 5°C. Suspensjonen ble omrørt under avkjøling i 20 minutter, deretter fjernet fra badet og omrørt ved romtemperatur i 3 timer. Reaksjonsblandingen ble vasket med 3 x 100 ml IN HCl, 3 x 100 ml mettet NaHC03og 3 x 100 ml mettet NaCl-oppløsning. Det organiske lag ble tørket over MgS04og filtrert og oppløsningsmidlet fjernet for å gi 27,38 g av en grønngrå olje.
Syntese av CHrCH2 C( 0) N( OH) CH2CH2
Til 5 g Br(CH2)4<C>(0)NH(OCH2C6H5) ble det tilsatt 19,2 ml IM NaOH. Tofase-systemet ble omrørt i 30 minutter og deretter ekstrahert med 3 x 25 mlCH2C12. De organiske lagene ble kombinert og tørket over MgSO4 . filtert og oppløsningsmidlet fjernet, hvilket etterlot 3,23 g hvitt faststoff. Faststoffet ble blandet med 70 ml 95%EtOHog 0,323 g 10% Pd på kull. Blandingen ble hydrogenert i 1 time ved 3,45. Reaksjonsblandingen ble filtrert gjennom kiselgur og oppløsningsmidlet fjernet, hvilket etterlot 1,70 g av et gulaktig, fettlignende faststoff.
Syntese av [ CH2CH2 C( Q) N( 0) CH2CH2]3Ga
Til en suspensjon av nylig utfelt Ga(0H)3fra 5,9 ml 1,1M GaCl3i 40 ml H20 ble det tilsatt en klar oppløsning av 1,5 g CH2CH2CON(OH)CH2CH2i 60 ml H20 og omrørt i 72 timer ved romtemperatur. Oppløsningsmiddelvolumet ble redusert med 25% på rotasjonsfordamper og sentrifugert ved 15.000 rpm. i 40 minutter. H20 i den klare supernatanten ble fordampet, hvilket etterlot et gummiaktig residuum.Residuet ble oppløst i 50 ml H2<D og filtrert gjennom Celite, hvorpå oppløsningsmidlet ble fjernet. Det gjenværende gule lakkaktige materialet ble omkrystallisert fra absolutt etanol/eter for å gi 100 mg orange faststoff.

Syntese av BrCH2( CH2)4 C( 0) NH( OCH2C6H5)
Til 4,43 g NH2OCH2(CgH^).HCl i 50 ml CH2Cl2ble det ved 0°C i et is/NaCl-bad tilsatt 7,17 ml trietylamin. Temperaturen ble holdt mellom 0-5°C mens en oppløsning av 3,58 ml 6-brom- heksanoylklorid i 15 ml CH2C12 ble tilsatt dråpevis. Blandingen ble omrørt under avkjøling i 20 minutter, hvoretter isbadet ble fjernet og omrøringen fortsatt i 3 timer til ved romtemperatur. Blandingen ble ekstrahert med 3 x 25 ml IN HCl-oppløsning og 3 x 25 ml mettet NaCl-oppløsning. Det organiske lag ble tørket over MgS04, filtrert og inndampet til 6,56 g olje som størknet etter henstand i luft.
Syntese av Ci H2CH2 C( 0) N( OH) CH2CH2CiH2
En oppløsning av 18,7 ml IM NaOH og 5,1 g BrCH2(CH2)4CH2NHOCH2-CgH5 ble blandet sammen og oppvarmet til 80°C. Etter 10 minutter utfeltes et hvitt bunnfall fra den svakt blakkede oppløsningen. Dette ble omrørt ved 80°C i ytterligere 10 minutter. Blandingen ble ekstrahert med 3 x 20 ml CH2C12. De organiske lagene ble tørket over MgS04og filtrert, hvoretter oppløsningsmidlet ble fjernet, hvilket etterlot 3,42 g gul olje. Oljen ble oppløst i 50 ml MeOH. 0,342 g 10% Pd/C ble tilsatt og blandingen redusert i en Parr-reaktor under 3,45 bar H2i 2 timer. Reaksjonsblandingen ble filtrert gjennom kiselgur og oppløsningsmidlet fordampet, hvilket etterlot 1,91 g gul olje, som størknet etter henstand i luft. Faststoffet ble sublimert i etKugelrohr-apparat ved 50 pm Hg 60°C for å gi 1,15 g.
Syntese av ( Ci H2( CH2)3 C( 0) N( 0) C1 H2)3Ga
En klorid-fri suspensjon av nylig utfelt Ga(OH)3fra
1,8 ml 1,1M vandig GaCl3 i 20 ml H20 ble omrørt med en filtrert oppløsning av 0,5 g CH2(CH2)3C(0)N(OH)CH2 i 20 ml H20. Suspensjonen ble omrørt i 3 timer, deretter oppvarmet til 50°C i 1,5 timer og tilslutt omrørt ved romtemperatur i 15 timer. Den blakkede oppløsning ble sentrifugert ved 15.000 rpm. i 15 minutter og supernatanten befridd for H20. 450 mg hvitt faststoff ble oppsamlet.

Syntese av [ CH3( CH2)2 CON( 0) H]3Ga
Til fritt hydroksylamin i metanol, utviklet fra 20 g hydroksylamin-hydroklorid og 24,11 gKOHsom i Eksempel 6 nedenfor, ble det tilsatt 16,47 g etylbutyrat. Det faste KCl som oppsto, ble frafiltrert og vasket med metanol. Etter flere timer ble mer utfelt KCl frafiltrert og filtratet inndampet til et fuktig krystallinsk faststoff. Dette ble omkrystallisert fra varm 8:1 aceton/etanol, vasket med etylacetat og tørket for å gi 8,9 g CH3(CH2)2<C>ON(OK)H.
Til 2 g CH3(:'H2)2CON(OK)H i 50 ml metanol ble det tilsatt 1,97 g Ga(N03)3.9H20 i 50 ml metanol. Fast KN03 ble fjernet ved filtrering og filtratet inndampet. Residuet ble utgnidd med 10:1 metylenklorid/metanol for å felle ut KN03som ble fjernet ved filtrering. Filtratet ble inndampet, hvilket etterlot en rosa olje. Oljen ble omrørt i etylacetat for å gi 1,34 g hvitt faststoff.

Syntese av ( CH3( CH2)4 C0N( 0) H)3Ga
To blandinger av 8,4 g hydroksylamin-hydroklorid i 60 ml metanol og 10,2 g KOH i 30 ml metanol ble oppvarmet til koking for å oppnå fullstendig oppløsning. Til den avkjølte (40°C) oppløsning av hydroksylamin-hydroklorid ble det under inn-ledning av N2og omrøring tilsatt varm metanolisk oppløsning av KOH, hvorunder det oppsto et bunnfall av KCl. Etter avkjøling ble blandingen omrørt i 5 minutter og det hvite faststoff frafiltrert. Det oppsto mer KCl i filtratet og dette ble fjernet ved filtrering. Volumet av filtratet ble redusert til 100 ml ved inndampning og holdt ved -20°C i 16 timer. Det hvite gummiaktige krystallinske faststoff som oppsto, ble omkrystallisert fra 100 ml varm absolutt etanol. I alt 3 fraksjoner førte til 3,7 g CH3(CH2)4CON(OK)H.
Til 0,9 ml 1,1M vandig GaCl3i 80 ml vann ble det ved 80°C under omrøring tilsatt 0,5 g (CH3(CH2)4C0N(0K)H i 10 ml vann. Volumet av oppløsningen ble redusert til 50 ml ved inndampning, hvorved det oppsto en klebrig substans. Blandingen fikk stå ved romtemperatur i 16 timer, 40 ml 50/50 metanol/vann ble tilsatt til blandingen som så ble oppvarmet for å oppnå fullstendig oppløsning. 0,25 g hvitt faststoff ble oppsamlet etter 3 dager.

Til en omrørt suspensjon av 1 g CH3(CH2)gC0N(0K)H i
120 ml vann ble det tilsatt 2 ml 1,1M vandig GaCl3. Etter ca. 3 timer ble pH justert til 6. Et hvitt bunnfall ble frafiltrert, vasket med vann og tørket. Det hvite faststoffet ble omkrystallisert ved forsiktig tilsetning av vann til en metanolisk oppløsning av det hvite faststoff for å gi 550 mg hvitt faststoff.

I henhold til oppfinnelsen ble forbindelser undersøkt for oral absorpsjon i rotter.SpragueDawley hannrotter med en vekt på 150-225 g ble anskaffet fraHarlanSprague Dawley Inc.
(Indianapolis, IN,U.S.A.).Gallium-standardoppløsningen er fra Aldrich Chemical Co (Milwaukee, WI, U.S.A.). Metofane er et produkt fra Pitman-Moore (Mundelein, IL, U.S.A.) og de øvrige kjemikalier er kommersielt tilgjengelige. Gallium-test-forbindelser ble oppløst 18 megaohm vann (Millipore, Bedford, MA, U.S.A.) eller suspendert i 0,5% karboksymetylcellulose i 5% etanol dersom forbindelsen ikke var oppløselig i vann. Suspensjonene ble ultralydbehandlet ved romtemperatur i ca. 5 minutter.
For administrasjon til mave og tarm, ble rottene anestetisert med metofane og et snitt på 2-3 cm ble foretatt for å blottlegge maven og en del av tynntarmen. Underbinding ble lagt umiddelbart nedenfor pylorus og en annen underbinding ble lagt 1 cm nedenfor for å sikre mot lekkasje. For oral administrasjon med sonde, ble det benyttet dyre-foringsnåler nr. 18 med kull-spiss (Popper & Sons. Inc. New Hyde Park, NJ, U.S.A.). For injeksjoner i mavesekken ble nåler ført inn i midten av pylorusdelen, som har en ugjennomsiktig tykk muskel-vegg, og intestinale injeksjoner ble foretatt ca. 0,5 cm nedenfor den andre underbinding med nålen pekende nedover og bort fra mavesekken.
Sutureringen ble foretatt med 3-4 sting med 3-0 kirurgisk silketråd (Ethicon Inc.Somerville,NJ, U.S.A.). Halevenen ble benyttet for intravenøse injeksjoner. Med unntak av perorale administrasjoner med sonde, ble alle injeksjoner foretatt med nåler nr. 30 for å redusere muligheten for lekkasje. Dosen var 0,067 mmol/kg. Ca. 300 ul blod ble uttatt 0,17, 0,5, 1,0, 2,0, 4,0 timer etter administrasjon av testforbindelsen. Blodet ble anbragt i 1 ml Eppendorf rør som på forhånd var dekket med
50 ul heparin (1000 U/ml) og lufttørket slik at det ikke fant sted noen fortynning av blodet.Plasmaet ble uttatt etter at blodet var sentrifugert i 2 minutter i en Fischer mikro-sentrifuge, Model 235B, og galliuminnholdet i dette målt med et flammeløst atomabsorpsjonsspektrometer (Varian). Standard- kurven var lineær for galliumkonsentrasjoner på 5-100 ng/ml. Arealet under konsentrasjon versus tid-kurven (AUC) i 0-4 timer ble bestemt.

En oppløsning av galliumnitrat i citratbuffer ble gitt som kontroll for å vise den intestinale absorpsjon av et kommersielt preparat.
4-timers AUC-verdiene tydet på god absorpsjon av gallium fra tarmen ved peroral administrasjon og at passende formulering av galliumforbindelser vil gi en egnet doserings-form av gallium for cancerbehandling, hyperkalsemi i forbindelse med malignitet og andre sykdommerkarakterisertved stort av tap av benvev og bensvekkelse.

1. Farmasøytisk preparat for galliumadministrasjon, som omfatter et gallium(III)kompleks med formel I
hvor R1 er C^-Cg-n-alkyl og R2 er H eller C^-C^-alkyl eller R1 og R2 sammen danner tetra- eller pentametylen,
i en farmasøytisk akseptabel form, i blanding eller sammen med et farmasøytisk akseptabelt bære- eller fortynningsmiddel.
