Oppfinnelsens område
Foreliggende oppfinnelse vedrører kombinasjoner av farma-søytisk aktive substanser for anvendelse i behandlingen av respiratoriske sykdommer.
Substansene anvendt i kombinasjonene ifølge oppfinnelsen er kjent aktive forbindelse fra PDE4- og PDE3/4-inhibitor-klassen, nemlig 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST],eller et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav, og aktive forbindelser fra histaminreseptorantagonistklassen, nemlig (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazinyl]etoksy]-eddiksyre[INN:CETIRIZIN] eller et farmasøytisk akseptabelt salt,solvat, N-oksid, eller solvat av ett salt eller N-oksid
Kjent teknisk bakgrunn
I den internasjonale søknaden WO03/000289 er det beskrevet sammensetninger som omfatter en PDE4-inhibitor og en Hl-re-septorantagonist og anvendelsen av disse sammensetningene for fremstilling av et medikament for behandlingen av respiratoriske sykdommer.
Beskrivelse av oppfinnelsen
Astma er en vanlig inflammatorisk lidelse i det respiratoriske system, som utgjør 1 - 3% av alle legebesøk, 500 000 sykehusinnleggelser per år og flere pediatriske sykehusinnleggelser enn noen annen enkelt sykdom i USA. Årlig dør mer enn 5000 barn og voksne av astmaanfall i USA (William E. S.; Goodmann Gilmann A.: The pharmacological Basis of Therapeutics, 9. utgave, s. 152 og 659-682, Mc Graw Hill, New York 1996).
Astma kan ikke lenger ses på bare som en reversibel luft-veisobstruksjon. Sykdommen bør i stedet først og fremst be- traktes som en inflammatorisk sykdom med bronkial hyperak-tivitet og bronkospasme som resultat. Allergen spesifikk immunoglobulin E (IgE) bindes til mastcellene via Fc-resep-torer. Det er et fragment oppnådd ved papainnedbryting av immunoglobulinmolekyler og inneholder de fleste antigen determinanter. Når et allergen kommer i kontakt med IgE, aktiveres mastcellene og frigjør en mengde inflammatoriske mediatorer, som inkluderer granulainnhold som histamin, proteaser, heparin og tumornekrosefaktor (TNF), mange forskjellige lipidmembran avledede molekyler som prostaglan-diner, levkotriener og plateaktiverende faktor (PAF) og en mengde cytokiner som interlevkin (IL)-l, -3, -4, -5, -6 og -8 og kemokiner. Et enormt mangfold av mediatorer som har mer enn en potent effekt på luftveisinflammasjon frigjøres.
Som et resultat av vasodilasjon, fører økt vasopermeabi-litet og økt endotelial adhesjon mot levkocytter videre til en innstrømning av inflammatoriske celler som lymfocytter, eosinofiler og makrofager fra blodsirkulasjon inn i vevene. Dette førerer deretter til frigjøringen av mediatorer med ytterligere inflammatoriske effekter (Rao A. R. et al. Recent Perspectives in the design of antiasthmatic agents, Pharmazie, 55, 7, 475-482, 2000).
Det kan følgelig forstås at det er usannsynlig at legemidler som påvirker en enkel mediator alene kan behandle lidelsen tilfredsstillende. Ettersom astma er en av hoved-lidelsene som påvirker mennesket, er det et behov for å ut-vikle legemidler som kan påvirke et stort mangfold av mediatorer .
Formålet med den foreliggende oppfinnelse er derfor å gjøre terapeutika for det respiratoriske system som oppfyller følgende betingelser tilgjengelige: Gunstig simultan innvirkning på flere av de

Det har nå blitt funnet at kombinert anvendelse av en PDE4-eller en PDE3/4-inhibitor og en histaminreseptorantagonist utmerket oppfyller de ovennevnte betingelsene.
Heri beskrives den kombinerte anvendelsen av en PDE4- eller en PDE3/4-inhibitor og en histaminreseptorantagonist i behandlingen av respiratoriske sykdommer.
Uttrykket "PDE4-inhibitor" betyr en selektiv fosfodiesteraseinhibitor, som foretrukket hemmer type 4 fosfodiesterase sammenlignet med andre kjente typer av fosfodiesterase, for eksempel type 1, 2, 3, 5 etc., hvorved forbindelsen har en lavere IC50(mer potent) for type 4 fosfodiesterasen, slik at der IC50for PDE4-hemming er omkring faktor 10 lavere sammenlignet med IC50for hemming av annen kjent type av fosfodiesterase, for eksempel type 1, 2, 3, 5 etc.
Uttrykket "PDE3/4-inhibitor" er analogt definert. Fremgangsmåter for å bestemme aktiviteten og selektiviteten til en fosfodiesteraseinhibitor er kjent for fagmannen. Eksempler på salter med baser som kan nevnes er alkalimetall-(litium, natrium, kalium) eller kalsium-, aluminium-, mag-nesium-, titan-, ammonium-, meglumin- eller guanidinsalter, hvor basene her også anvendes i saltfremstilling i et ekvimolart mengdeforhold eller ett som atskiller seg derfra.
Visse av de aktive ingrediensene anvendt i den foreliggende oppfinnelse er i stand til å eksistere i stereoisomerfor-mer. Oppfinnelsen omfatter alle stereoisomerer av de aktive ingrediensene og blandinger derav inkludert racemater. Tautomerer og blandinger derav av den aktive ingrediensen er også del av den foreliggende oppfinnelse.
I overensstemmelse med den foreliggende oppfinnelse, til-veiebringes det i et første aspekt en farmasøytisk sammensetning omfattende, i kombinasjon, en første aktiv ingrediens som velges fra gruppen bestående av 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST],og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav,og en andre aktiv ingrediens som velges fra gruppen bestående av (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazinyl]etoksy]-eddiksyre[INN:CETIRIZIN] og et farmasøytisk akseptabelt salt,solvat, N-oksid, eller solvat av ett salt eller N-oksid derav.
I et annet aspekt tilveiebringer oppfinnelsen et farma-søytisk produkt omfattende, i kombinasjon, et preparat av en første aktiv ingrediens som velges fra gruppen bestående av 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST], og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav,, og et preparat av en andre aktiv ingrediens som velges fra gruppen bestående av (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazinyl]etoksy]-eddiksyre [INN: CETIRIZIN] og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav, for simultan, sekvensiell eller separat anvendelse i terapi.
Det har blitt funnet at administrasjonen av aktive ingredienser ifølge oppfinnelsen er fordelaktig da det resulterer - sammenlignet med administrasjon av en enkel aktiv ingrediens fra PDE4- og PDE3/4-inhibitorene eller histaminreseptorantagonistklassen - i en redusert tidlig allergisk respons og/eller i en redusert sen inflammatorisk luftveisrespons.
Den farmasøytiske sammensetningen ifølge den foreliggende oppfinnelse kan fremstilles ved å blande den første aktive ingrediensen med den andre aktive ingrediensen. I et annet aspekt av foreliggende oppfinnelse er det derfor tilveiebrakt en fremgangsmåte for fremstillingen av en far-masøytisk sammensetning som omfatter å blande den første aktive ingrediensen med den andre aktive ingrediensen.
Den første og den andre aktive ingrediensen kan alternativ (andre enn i tilsetning som beskrevet over) administreres simultant, sekvensielt eller separat for å behandle respiratoriske sykdommer. Sekvensielt betyr at den første og den andre aktive ingrediensen administreres med en rett etter den andre. Separat betyr en administrasjonstidsdifferanse på opp til 24 timer, foretrukket opp til 12 timer.
Den foreliggende oppfinnelse tilveiebringer ytterligere an-vendelser av en farmasøytisk sammensetning eller et farma-søytisk produkt ifølge oppfinnelsen ved fremstilling av et medikament for profylaksen og/eller behandling av en respi-ratorisk sykdom.
Respiratoriske sykdommer som kan nevnes er spesielt allergen- og inflammasjonsinduserte bronkiale forstyrrelser (bronkitt, obstruktiv bronkitt, spastisk bronkitt, allergisk bronkitt, allergisk astma, bronkialastma, KOLS, allergisk, sesongmessig og varig rhinitt), som kan behandles ved kombinasjonen ifølge oppfinnelsen også i betydningen av en langtidsterapi (hvis ønsket med passende justering av dose- behov, for eksempel behov etter sesongrelaterte variasjo-ner) .
Den aktive ingrediensen kan og vil som del av den farmasøy-tiske sammensetningen, det farmasøytiske produktet eller preparatet, anvendes i tilsetning med en eller flere farma-søytisk akseptable hjelpestoffer og/eller eksipienser.
Innenfor betydningen ifølge den foreliggende oppfinnelse forstås "anvendelse" foretrukket som oral administrasjon av begge aktive ingredienser. Den aktive ingrediensen kan også administreres som en nasalspray, en aerosol eller som et inhalert pulver. Ytterligere fremgangsmåter for administrasjon som kan nevnes er parenteral (for eksempel intravenøs) og transdermal administrasjon av den aktive ingrediensen.
Oppfinnelsen omfatter på den ene siden å administrere begge legemidlene samtidig i en leveringsform slik som fast oral kombinasjon (ved å anbringe begge de aktive ingrediensene i en tablett), som en inhalator (ved å anbringe begge de aktive ingrediensene i den samme inhalatoren) eller som en fri oral kombinasjon (ved å anbringe begge de aktive ingrediensene i to separate tabletter). På den andre siden omfatter oppfinnelsen også administrasjon av legemidlene i to forskjellige leveringsformer slik som å anbringe PDE4-inhi-bitoren i tabletter og pakke dem med en inhalator som inneholder histaminreseptorantagonisten eller vice versa.
Fagmannen er med utgangspunkt i hans/hennes ekspertkunnskap kjent med hvilke eksipienser eller hjelpestoffer som er passende for den ønskede farmasøytiske sammensetningen, produktet eller preparatet. I tillegg til løsemidler, gel-dannende midler, tabletteksipienser og andre aktive forbin-delsesbærere er det mulig å anvende for eksempel antioksi-danter, dispergeringsmidler, emulgatorer, skumdempingsmid-ler, smakskorrigeringsmidler, konserveringsmidler, løselig-hetsformidlere, fargestoffer eller gjennomtrengingsfremmere og kompleksdannere (for eksempel cyklodekstriner).
De farmasøytiske sammensetningene eller preparatene ifølge oppfinnelsen for oral administrasjon er foretrukket i form av tabletter, belagte tabletter, kapsler, emulsjoner, sus-pensjoner eller løsninger, det aktive ingrediensinnholdet er fordelaktig mellom 0,1 og 95 % og ved passende valg av eksipiensene og hjelpestoffene er det mulig å oppnå en far-masøytisk administrasjonsform nøyaktig tilpasset til de aktive ingrediensen(e) og/eller til den ønskede begynnelse av virkningen (for eksempel en vedvarende frigjøringsform eller en enterisk form).
Den aktive ingrediensen ifølge oppfinnelsen administreres ved inhalering foretrukket i form av en aerosol, aerosol-partiklene av fast, flytende eller blandet sammensetning har foretrukket en diameter på 0,5 til 10 pm, foretrukket fra 2 til 6 pm.
Aerosolfrembringelse kan utføres ved, for eksempel trykk-drevne jetforstøvere eller ultrasoniske forstøvere, men foretrukket ved drivgassdrevne tilmålte aerosoler eller drivgassfri administrasjon av mikroniserte aktive forbindelser fra inhalasjonskapsler.
Avhengig av inhalatorsystemet som anvendes, i tillegg til de aktive forbindelsene, inneholder administrasjonsformene de krevde eksipiensene, slik som for eksempel drivgasser (for eksempel Frigen i tilfellet av tilmålte aerosoler), overflateaktive substanser, emulgatorer, stabilisatorer, konserveringsmidler, smaksstoffer, fyllstoffer (for eksempel laktose i tilfellet av pulverinhalatorer) eller, hvis passende, ytterligere aktive forbindelser.
For inhaleringsformål er et stort antall apparater tilgjengelige med hvilke aerosoler med optimal partikkelstør-relse kan genereres og administreres ved anvendelse av en inhalasjonsteknikk som er så riktig som mulig for pasien-ten. I tillegg til anvendelse av adaptorer (avsstandsstyk- Nebulator®, Volumatic®) og automatiske anordninger som sen-der ut en spray (Autohaler®) , for tilmålte aerosoler, spesielt i tilfellet av pulverinhalatorer, er en mengde tek-niske løsninger tilgjengelige (for eksempel Diskhaler®, Rotadisk®, Turbohaler® eller inhalatoren beskrevet i euro-peisk patentsøknad EP 0 505 321), ved anvendelse av hvilken en optimal administrasjon av aktiv forbindelse kan oppnås.
Typiske sammensetninger for nasal levering inkluderer de nevnt over for inhalering og inkluderer ytterligere ikke-trykksatte sammensetninger i form av en løsning eller en suspensjon i en inert vehikkel slik som vann eventuelt i kombinasjon med konvensjonelle eksipienser slik som buf-fere, antimikrobielle midler, tonisitetsmodifiserende midler og viskositetsmodifiserende midler som kan administreres med nasalpumpe.
Typiske transdermale formuleringer omfatter en konvensjo-nell vandig eller ikke-vandig vehikkel, for eksempel en krem, salve, losjon eller pasta eller er i form av et anti-septisk plaster, plaster eller membran.
For de ovennevnte terapeutiske anvendelsene vil de admi-nistrerte dosene selvsagt variere med den første og andre aktive ingrediensen anvendt, fremgangsmåten for administrasjon, den ønskede behandlingen og den indikerte forstyrrel-sen .
Generelt vil tilfredsstillende resultater imidlertid oppnås når den totale daglige doseringen av de(n) første aktive ingrediensen(e), henholdsvis PDE4- og PDE3/4-inhibitorene, når tatt oralt er i området fra 1 - 2000 ug/kg kroppsvekt.
I tilfellet av den PDE4-inhibitor ROFLUMILAST, er den daglige doseringen i et område fra 1-20 ug/kg kroppsvekt.
Den totale daglige doserigen av de(n) andre aktive ingrediensen (e), histaminreseptorantagonistene kan også variere innen et vidt område (0,1 - 1500 ug/kg kroppsvekt).
Claims (6)
1. Farmasøytisk sammensetning karaterisertve d at sammensetningen omfatter, i kombinasjon, en første aktiv ingrediens som velges fra gruppen bestående av 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST],og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav,og en andre aktiv ingrediens som velges fra gruppen bestående av (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazinyl]etoksy]-eddiksyre[INN:CETIRIZIN] og et farmasøytisk akseptabelt salt,solvat, N-oksid, eller solvat av ett salt eller N-oksid derav.
2. Farmasøytisk sammensetning ifølge krav 1, som er en fast oral kombinasjon.
3. Anvendelse av 3 cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST], et farmasøytisk akseptabelt salt, solvat eller N-oksid derav eller solvat av et salt eller et N-oksid derav, og (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazi-inyl] etoksy]-eddiksyre [INN: CETIRIZIN], et farmasøytisk akseptabelt salt, solvat, eller N-oksid derav, eller solvat av ett salt eller N-oksid i fremstillingen av et medikament egnet for behandlingen av respiratoriske sykdommer.
4. Anvendelse ifølge krav 3, hvori den respiratoriske sykdommen velges fra bronkitt, obstruktiv bronkitt, spastisk bronkitt, allergisk bronkitt, allergisk astma, bronkialastma, KOLS, og allergisk, sesongmessig og varig rhinitt.
5. Fremgangsmåte for fremstillingen av en farmasøytisk sammensetning som definert i et hvilket som helst av kravene 1-2, karaterisertvedat den første aktive ingrediensen blandes med den andre aktive ingrediensen.
6. Et farmasøytisk produkt omfattende, i kombinasjon, et preparat av en første aktiv ingrediens som velges fra gruppen bestående av 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-diklorpyrid-4-yl)-benzamid [INN: ROFLUMILAST], og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav, og et preparat av en andre aktiv ingrediens som velges fra gruppen bestående av (pluss/minus)-[2-[4-(p-klor-alfa-fenylbenzyl)-1-piperazinyl]etoksy]-eddiksyre [INN: CETIRIZIN] og et farmasøytisk akseptabelt salt, solvat, N-oksid, eller solvat av ett salt eller N-oksid derav, for simultan, sekvensiell eller separat anvendelse i terapi.
