Description
The invention relates to an aqueous preparation for absorbing and stabilizing microgas bubbles for use as an echo contrast agent, as well as a method for producing such an agent.
Teknikkens stand State of the art
Because ultrasound of gas bubbles suspended in liquid is strongly reflected, aqueous preparations containing stabilizing microscopic gas bubbles were already proposed as a contrast agent for ultrasound diagnostics at an early point. Microgas bubbles can be stabilized in aqueous preparations by lowering the surface tension, i.e. by adding suitable surfactants.
In EP-B-0077752 it is stated that aqueous solutions containing a surfactant or a surfactant mixture and in addition a viscosity-increasing substance have advantageous contrast-producing properties. Among the surfactants mentioned are, among other things, non-ionic lecithins and lecithin fractions as well as polyoxyethylene polyoxypropylene polymers. In the six production examples in EP-B-0077752, preparations are indicated which contain as surfactant a polyoxyethylene polyoxypropylene polymer and as viscosity-increasing substance glucose or dextran or the polyoxyethylene polyoxypropylene polymer itself. A check of the manufacturing examples has shown that the contrast-giving effect is not satisfactory. Thus, the preparations according to EP B-0077752 are not suitable for imaging left hearts.
It was now surprisingly found that aqueous preparations which, in addition to polyoxyethylene polyoxypropylene polymers, contain negatively charged phospholipids are surprisingly suitable for absorbing and stabilizing microgas bubbles.
Beskrivelse av oppfinnelsen Description of the invention
The subject matter of the invention is thus aqueous preparations for absorbing and stabilizing microgas bubbles for use as echo contrast agents, polyoxyethylene polyoxypropylene polymers containing negatively charged phospholipids with the exception of aqueous solutions of antibodies or of physostigmine containing polyoxyethylene - polyoxypropylene - polymers and phospholipids as well as with the exception of aqueous solutions -nings containing 0.23% (weight/volume) polyoxyethylene - polyoxypropylene - polymers. Further items appear in the patent claims.
As polyoxyethylene polyoxypropylene polymers, those with an average molecular weight of 8,350 to 14,000 are preferred. Polyoxyethylene polyoxypropylene polymers are referred to as poloxamers and are commercially available, e.g. under the trademark "Pluro-nics" (Wyandotte Chemicals Corp.). The preparations according to the invention contain 0.1 to 10%, preferably 1 to 5% polyoxyethylene polyoxypropylene polymers. The negatively charged phospholipids are present in an amount of 0.01 to 5%, preferably 0.5 to 2%. The percentages refer to weight/volume.
Negatively charged phospholipids include phosphatidylglycerols, phosphatidylinositols, phosphatidylethanolamines and phosphatidylserines and their lysoforms. By lysoforms of negatively charged phospholipids is meant the negatively charged phospholipids that contain only one acyl residue. Preferred are lysoforms of negatively charged phospholipids where the acyl groups are bound to the oxygen of carbon atom 1 of the glycerol molecule. Particularly preferred negatively charged phospholipids are dipalmitoylphosphatidylglycerol (DPPG) and distearoylphosphatidylglycerol (DSPG), whereby distearoylphosphatidylglycerol (DSPG) is particularly preferred.
The preparations according to the invention stand out compared to the state of the art in that, with small mechanical devices, echocontrast agents containing microgas bubbles can be produced, which, due to their great stability, produce a long-lasting contrast and which are excellently suitable for imaging the left heart. In particular, it should be emphasized that the preparations according to the invention are excellently suited for imaging internal surface structures, as the microgas bubbles apparently stick firmly to the surface and thus, even after flushing the microgas bubbles which are located in the lumen of vessels, produce an informative contrast. Thus, it is e.g. possible to produce a clearer picture of the heart's dynamics even after the washout of the contrast medium.
The preparation of the preparations according to the invention is unproblematic and can be done by adding the individual components together or one after the other to water and, if necessary, dissolving them while heating and stirring. If desired, they can also be sterilized, e.g. by heat irradiation.
Glycerin, mannitol and ammonium salts of amino acids, preferably glycine, have been found to be particularly suitable for adjusting the isotonicity of the preparations according to the invention.
The production of microgas bubbles conveniently takes place shortly before administration to the patients to be examined, and is carried out in a manner known per se. If e.g. the preparation according to the invention is made available in a puncture bottle, then the solution together with the desired amount of air can be drawn up into an ordinary syringe and injected again with the highest possible pressure via a narrow cannula into the puncture bottle. If necessary, the pulling up and spraying is repeated several times. Alternatively, the preparations according to the invention can also be pressed back and forth between two syringes via a connecting piece with a narrow cross-section or a mixing chamber which is connected between the two syringes. The latter method leads to particularly informative ultrasound images, whereby the result is further improved at the same time.
All physiologically compatible gases can be used as gas in the production of microgas bubbles. The preparations according to the invention are foamed with 0.01 to 0.1, preferably 0.04 to 0.06 ml of gas per 1 ml. They are preferably administered intravenously after the production of the microgas bubbles. Depending on the intended use, 1 to 20 ml, preferably 2 to 8 ml, and particularly preferably 5 ml of the preparations according to the invention are administered.
It should be emphasized in particular that the preparations according to the invention can be dosed lower due to their increased effectiveness in relation to the state of the art.

Solutions A, C, D, E and F are drawn up air-free in a first syringe. This syringe is then connected with the free end of a mixing chamber containing 0.18 ml of air and which is fixedly connected to a second syringe. Immediately before application, the solutions are pumped five times from the first syringe via the mixing chamber into the second syringe and back again.
The commercially available contrast medium B is prepared according to the instructions on the package leaflet.
The echocardiographic ultrasound recordings were carried out with an ultrasound device Sonoscope 4 with a mechanical head at 3.5 MHz. The video prints of the obtained ultrasound image were assessed densitometrically with regard to the contrast intensity. The used densitometer (Gretag D182) determines the changes in brightness in 100 steps in a range of 0.00 to 2.50 density units. The calibration takes place with the help of the calibration card according to DIN 16536 (calibration reference) which was made available by the manufacturer, as the value of 1.64 applies to the lightest white and the value 0.00 applies to the darkest black. The mean from four individual determinations on an area of 1 cm x 1 cm gives the value for each animal for the applied preparation.
The results obtained are shown in the following table.
From the results it appears that the echo contrast agent according to the invention, in contrast to the echo contrast agents according to the state of the art, is unavailable and is thus excellently suited for left heart diagnostics. The applicability of ultrasound in heart diagnostics is considerably increased by means of the echo contrast agents according to the invention.
Moreover, it was found that the microbubbles of the echo contrast agent according to the invention apparently have a considerable affinity to the inner surfaces of vessels and cavities in the body. This results in the outline of vessels and cavities being shown much better and are thus more informative than was possible with the contrast agents according to the state of the art. It is particularly advantageous here that this greatly improved presentation of the surface of vessels and cavities also remains, when the lumen in vessels and cavities is already free of echo contrast agent. This surprising contrast of surfaces can be used e.g. by observing the endocar diet.
In fig. 1 and 2 show the result of an experiment when laying this new contrasting surface structure. Fig. 1 shows the echocardiographic image of the endocardium of an awake beagle dog in the so-called four-chamber view immediately before the appearance of the first contrast after administration of 1 ml of echo contrast medium according to example 1. Fig. 2 shows the endocardium of the animal after the echo contrast medium again is washed out of the heart.
When comparing the two figures, it appears that with the echo contrast agent according to the invention, an unexpected drawing of the endocardium is made possible, which means a high level of information for diagnostic purposes.
