Description
Foreliggende oppfinnelse vedrører fremstilling av
nye forbindelser for behandling av inflammatoriske tarm-
sykdommer (IBD), nærmere bestemt et farmakologisk aksepta-
belt polymert middel med formelen:
hvor R er en ikke-nedbrytbar anionisk utvekslingsharpiks som er tilstrekkelig basisk til å deprotonere et benzosyre-
derivat, hvilket middel kan frigi en terapeutisk virkende forbindelse.. Den forbindelse som frigis, er 5,5'-azobis-
salicylsyre med formelen:
IBD er en kronisk, ikke-spesifikk, inflammatorisk
og sårdannende sykdom i tynntarmen og/eller tykktarmen, og kan være kjennetegnet ved blodig diare. Et eksempel er ulcerøs kolitt. Ved ulcerøs kolitt begynner sykdommen i den øvre del av endetarmen og kan spre seg, eventuelt til å
omfatte hele tykktarmen, eller den kan omfatte hele tarm-
systemet på en gang. Se Cecil, Textbook of Medicine, 1568-

Behandling av IBD har vært utført med forskjellige farmasøytiske forbindelser. Verdt å nevne er adrenokorti-kosteroider, belladonnaalkaloider, belladonnaderivater, vismuttbikarbonat, kaolin og sulfasalazin, som er vanlig brukt. Adrenokortikosteroidene kan skjule symptomer på
perforering av tarmen og bukhinnebetennelse, og brukes vanligvis bare for korttidsterapi (Goodman og Gilman 4.
utgave, side 1634 (1970)), og store komplikasjoner kan opp-
stå på tross av kortikosteroidterapi. Belladonnaalkalidene og derivatene derav er stort sett ansett som ineffektive mot IBD. (Goodman og Gilman 4. utgave, side 544 (1970)). Vismuttbikarbonat er et mekanisk beskyttelsesmiddel og forhindrer bare ytterligere irritasjon av tilstanden uten noen direkte virkning på tilstanden. Kaolin er et absorberings-middel som absorberer bakterier og toksiner i tykktarmen, men det er tvilsomt om noen merkbar virkning er i behold når det når den nedre delen av tarmen. (Goodman og Gilman, 4. utgave, side 990 (1970)). Sulfasalazin (SS) er det mest brukte legemiddel mot IBD. Dets struktur er vist i formel

SS er et forløperlegemiddel, dvs. at etter administrering, virker biologiske prosesser på SS slik at legemidlet som har den ønskede biologiske virkning, fremstilles. Etter otsl administrering absorberes ca. en tredjedel av den gitte mengde SS fra tynntarmen. Den gjenværende totredjedel spal-tes i tykktarmen med azo-reduktase fra bakteriefloraen til sulfapyridin (SP), formel III og 5-aminosalicylsyre (5-ASA) med formel IV
(Physican's Desk Reference 31. utgave, side 1250 (1977), se også Klotz, New Eng. J. of Med. 303, 1499 (1980)). Det er fastslått at virkningen av SS kommer fra den fremstilte 5-ASA. SS er effektiv som et forløperlegemiddel fordi dens relative uoppløselighet forhindrer dens fullstendige absorbsjon i tynntarmen, noe som gjør at SS når frem til virkningsstedet, dvs. tykktarmen. Når de gis hver for seg, absorberes både SP og 5-ASA nesten fullstendig fra tynntarmen. Selv om SS er effektiv, har den flere alvorlige bivirkninger inkludert sykelig sammensetning av blodet og overfølsomhets-reaksjoner. Denne giftvirkningen av SS skyldes nesten ute-lukkende det fremstilte SP.
Den farmakologiske behandlingen av IBD er velkjent som angitt ovenfor. SS er beskrevet i US-PS nr. 2.396.145
(1946). 5,5<1->azobis-salicylsyre er beskrevet i GB-PS nr-408.676 (1934). Den eneste beskrevne anvendelse er som et fargestoff. En rekke artikler har beskrevet den terapeutiske effektiviteten av 5-ASA og SS, virkningen av såvel som toksisitetsproblemene med SS. Nevnes skal Kah, et al.,
The Lancet, 292, oktober 29, (1977); Hees, et al, Gut 21, 632-1502 (1980) og Klotz, et al, N. Engl. J. Med. 303, 1499-1502 (1980). US-PS nr. 4.190.716 åpenbarer en polymer som består av et organisk polymerskjelett som inneholder aro-matiske ringer som er kovalent bundet via azobindinger til en mengde salicylsyre- eller salicylatsaltgrupper. Den foreliggende oppfinnelse omfatter derimot en ionisk bundet 5,5'-azobis-salicylsyre.
Ifølge foreliggende oppfinnelse fremstilles nevnte Dolymere middel med formel I ved at man omsetter 5,5'-azobis-salicylsyre med formelen:
med nevnte utvekslingsharpiks.
Harpiksen R har en molekylstørrelse som utelukker dens absorpsjon til tarmkanalen. Derfor, ved å binde 5,5'-azobis-salicylsyre ionisk til harpiksen, fremstilles et avleveringssystem for legemiddel som unngår absorbsjon i magesekken og tynntarmen. Polymerkomplekset minsker absorbsjonen av 5,5'-azobis-salicylsyre inntil den når den nedre del av tarmen. Frigivelse på denne måten muliggjør levering av 5,5'-azobis-salicylsyren til virkningsstedet. Forbindelsene fremstilt ifølqe foreliggende oppfinnelse er nyttige på den måten at 5,5'-azobis-salicylsyren frigitt derved lett reduseres til 5-ASA
i tykktarmen ved hjelp av tarmbakterier. Polymerdelen av komplekset har en størrelse som utskilles i avføringen uten absorbsjon. 1 mol 5,5<1->azobis-salicylsyre som frigjøres fra polymerkomplekset splittes bakterielt til 2 mol aktiv 5-ASA. Dette betyr at en lavere molar konsentrasjon av forbindelse kan brukes enn hva som var tilfellet ved de: tidligere forbindelser og frigivelsessystemer (f.eks. sulfasalazin). Dertil oppnås reduksjonen uten at en potensielt giftig forbindelse fremstilles.
Generelt kan en hvilken som helst anionbytterhar-piks som er tilstrekkelig basisk til å deprotonere et benzosyrederivat, brukes for å lage forbindelsene ifølge oppfinnelsen. En foretrukket serie er en polystyrenpolymer kryssbundet med 1-5% divinylbenzen, og funksjonalisert på benzenringen med en metylentrialkylammoniumrest, som gir harpiksen en permanent positiv ladning. En annen serie består av polyetyleniminer med ca. 1-5% kryssbinding mellom nitrogenatomene, de resterende nitrogenatomene er kvaterni-sert og omdannet til hydroksydformen. En foretrukket an-ionbytterharpiks er den hvor polystyrenet er kryssbundet med 2% vinylbenzen (AG1-X2). Anionbytterharpiksen AG1-X2, som er ekvivalent med "Dowex 1-X2", leveres fra Bio-Rad som 200-400 mesh kuler i kloridformen. Den omdannes til hydrok-
sydformen ved å bruke vandig NaOH og skylles med destillert vann.
I kraft av anti-IBD-virkningen er 5,5'-azobis-salicylsyre-polymerbytterkomplekser nyttige ved behandling av IBD hos mennesker og dyr. En lege med alminnelig erfaring vil lett kunne påvise at et individ oppviser IBD-symptomer. Uansett hvilken administrasjonsmåte som velges, bringes forbindelsene fremstilt ifølge oppfinnelsen i farmasøytisk akseptable doseringsformer ved hjelp av vanlig kjente metoder.
Forbindelsene kan administreres i slike orale enhets-former som tabletter, kapsler, piller, kuler, pulvere, suspen-sjon eller oppløsning eller granulater. De kan også administreres rektalt i slike former som stikkpiller, kremer, salver, klyster og lignende. En effektiv, men ikke-giftig mengde an-vendes ved behandling. Mengdene som gis for beskyttelse mot eller behandling av IBD med forbindelsene fremstilt ifølge oppfinnelsen, velges i overensstemmelse med en rekke faktorer om-fattende typen, alderen, vekten, kjønnet og de medisinske til-stander hos pattedyret, alvorligheten av IBD og administrasjons-måten. En lege eller veterinær med alminnelig erfaring ville anvende relativt lave mengder til å begynne med, for gradvis å øke mengden inntil en maksimal virkning oppnås.
De første mengder av forbindelsene er vanligvis i om-rådet fra 5 mg/kg opp til i det minste 100 mg/kg oralt.
Forbindelsene fremstilt ifølge oppfinnelsen er nyttige ved behandlingen av IBD som vist i den følgende prøve.
Eksempel 1 In vitro bakteriell inkubering
Det finnes ingen dyremodell for utprøving av virkningen av terapeutiske former for behandlingen av IBD. Den følgende prøve påviser reduksjonen av azobindinger i 5,5'-azobis-salicylsyre ved hjelp av bakterier fra tykktarmen for å frigjøre 5-ASA.
1. Prøvefremstilling: To rene bakteriekulturer som vanligvis finnes i tykktarmen ble brukt: Klebsiella pneumo-r nia (ATCC 4352) i triptikase-soyadyrkningsvæske, og Bacter-oides fragilis (ATCC 23745) i flytende tioglykollatmedium. Forbindelser oppløst eller suspendert i DMSO ble tilsatt
til 5 ml sterilt medium eller til 5 ml medium som var blitt
inokulert med bakterier 24 timer tidligere. For hver gang en forbindelse ble inkubert, ble andre inokulater inkubert med 5-ASA og SS. Hver 100 ml DMSO inneholdt den samme molare ekvivalent av 5-ASA. Kulturer og kontrollmedia ble inkubert i 45-48 timer ved 37°C.
2, Prøveanalyse: Bakteriekulturer og kontrollmedia ble filtrert med en sluttfilterstørrelse på 0,4 5 mikron. Filtratene ble overført direkte til "Merck GF-254"-silisium-dioksyd-TCL-plater sammen med 5-ASA-standardoppløsninger med forskjellige konsentrasjoner fra 50 ng til 450 ng/5 1. Dess-uten ble standardoppløsninger av SS og hver forbindelse også overført til TLC-platen. Plater ble fremkalt til 16 cm i fulle beholdere av tre typer:
a) Etanol:NH^OH (99:1, volum/volum)
b) Butanol:Butanon:H20 (40:40:20, volum/volum)
c) Isopropanol:H20:NH4OH (70:20:10, volum/volum).
Platene ble undersøkt innen 10 minutter etter frem-kalling under langbølge UV-lys (36 5 nm) og styrken på den grønne fluorescensen frembragt av 5-ASA (frembragt av azo-reduksjonen av forbindelser fremstilt ifølge oppfinnelsen ) ble sam-menlignet med 5-ASA-standardoppløsninger, 5-ASA inkubert med bakterier og SS inkubert med bakterier.
I denne prøven er mengden 5-ASA fremstilt etter in-kuberingen i kontakt med bakterier et mål på graden av azoreduksjon hos forbindelser fremstilt ifølge oppfinnelsen til 5-ASA ved hjelp av bakterier. Sammenligning av disse forbindelser med dataene utledet fra SS ville avsløre de forbindelser som frigir ekvivalente mengder 5-ASA under identiske beting-elser. Kontrollene virket som en undersøkelse for å vise at uten bakterier fant ingen azoreduksjon sted, og at for-bindelsen eller eventuelle nedbrytningsprodukter ikke fluor-escerte grønt ved den samme Rf som 5-ASA. Disse data ut-gjorde følgelig bevis for at frigivelse av 5-ASA fra 5,5'-azobis-salicylsyre ved reduksjon finner sted.
E ksempel 2 (AG 1-X2)2 5,5'-azobis-salicylsyrekompleks (Formell:R er AG1-X2).
Biorad AG 1-X2 (kloridform), 200-400 mesh (0,8 meq/ml) suspenderes i vann og fylles over på en 2,5 cm i diameter glasskolonne. Ialt 220 mm fuktig harpiks brukes. Kolonnen gjennomstrømmes med destillert vann. Deretter føres NaOH i H20, 10% vekt/volum, sakte gjennom (ca. 1 ml/min.). Ialt 500 ml NaOH-oppløsning føres igjennom. Kolonnen vaskes deretter med destillert vann. Vaskingen fortsettes inntil pH i eluatet (indikatorpapir) er under 7. 4 7,5 ml av denne harpiks blandes med 5,7 g 5,5<1->azobis-salicylsyre i 250 ml destillert vann. Denne blandingen tilbake-løpskokes i 16 timer, filtreres deretter mens den ennå er varm og harpiksen vaskes med destillert vann. Harpiksen tørkes i en vakuum- (avsugnings-) ovn ved 60°C i"16 timer hvorved man får tittelforbindelsen. Tittelforbindelsen er ca. 40 vekt-% 5,5<1->azobis-salicylsyre.

- Analogifremgangsmåte for fremstilling av et farmakologisk akseptabelt polymert middel som kan frigi en terapeutisk virkende forbindelse, og som har formelen:hvor R er en ikke-nedbrytbar anionisk utvekslingsharpiks som er tilstrekkelig basisk til å deprotonere et benzosyrederivat, karakterisert ve'd at man omsetter 5,5'-azobis-salicylsyre med formel:med nevnte utvekslingsharpiks.
