Abstract
Description
The invention relates to a covering device based on gel-like polymers, for treating the skin, especially for burns, wounds and skin defects.
Den For the therapy of large-surfaced wounds, for example in the case of heavier burns, very diverse, partially opposite properties are required of the covering material, which have so far been insufficient. Thus, on the one hand, the covering material must be suitable for absorbing wound secretions, on the other hand, however, keep the patient's water and protein loss as low as possible. It should enable heat and gas development, but prevent the access of bacteria and viruses. It
Videre forlanges at medikamenter kan appliseres epicutant. should be able to be changed as painlessly as possible, however, stimulate the granulation of the skin with the help of a moist medium, and when applied enable a dilution of secretions and toxins. It is also required that drugs can be applied epicutaneously.
Therefore, in practice, for wound treatment, for example in the case of major burns, natural material such as pig skin or autologous grafts is used, albeit only after removal of necrotic tissue. Of the grafts, it is true enough to prefer pig skin, however, they are not usable in the first phase of wound sanitation, namely for cleaning the wounds. In addition: they are usually not available in the necessary quantity.
It is also known to use net-formed gel-like polymers, i.e. aqueous polymer masses such as e.g. net-formed polyacrylamide gel or agar gel for wound treatment and in dermatology. However, these polymer masses are difficult to produce in a continuous manner in the form of layers or foil-like structures, and also show significant shortcomings in clinical application, as they are difficult to handle.
The task of the invention is therefore to provide a further improved agent based on gel-like polymers for the treatment of the skin, especially for the treatment of burns, wounds, but also skin defects and dermatological problems. The expert must be given the opportunity to treat specific skin lesions, for which the previous covering devices are less suitable.
The invention thus relates to a covering device for treating skin, in particular for treating skin burns, wounds and skin defects, consisting of a selectively permeable membrane layer with an adjacent aqueous gel of polymer, the device being characterized by the fact that it consists of a water vapor and gas permeable, flat membrane tube sealed at its ends with pores of an average size of 3 x 10 4 to 10 µm, the cavity of which contains easily deformable and easily flowable mass of polymer gel containing polymers selected from a group comprising carbohydrates, carbohydrate derivatives, polyvinylpyrrolidone, polyvinyl - alcohol, polyacrylic acid, derivatives of polyacrylic acid, polymethacrylic acid and derivatives of polymethacrylic acid with a high water content in relation to their total weight.
In terms of definition, the term "gel-like polymer" is to be understood as a pre-resistant, easily deformable mass of polymer, which has a high amount of water relative to its weight.
Further embodiments of the device are mentioned in the sub-claims.
The membrane, which at least partially covers the surface of the polymerizate facing away from the skin, and is permeable to water vapor and gas, is in particular a membrane with an average pore size in the range 3 x 10 to 10 pm, and consists, for example, of regenerated cellulose (DE-AS 27 36 569) or an asymmetric plastic membrane of polyamide (DE-AS 51 910) or polycarbonate (DE-OS 25 10 337). These membranes were previously used in dialysis (pore size 10 -3 to 10 -2 µm) or hemofiltration, and show a thickness from 10 to 150 (µm. The membrane shows a sheet-like shape. It is either sheet-shaped or has the shape of a sheet-like snake, which is sealed at one end or
i lukket, f.eks. i form av en pose som omgir det samlede gelaktigei polymerisatet. in closed, e.g. in the form of a bag that surrounds the overall gel-like polymer.
If the membrane only partially envelops the gel-like polymer, the non-enveloped part of the gel-like polymer is
dekket av en bladformet vevnad eller florstoff, f.eks. av polyetylen-, polyamid-, polyester- eller polypropylenfiber, eller med et åpenporet kunststoffskumsjikt, f.eks. av polyuretan-skum eller med et papir eller en porøs kunststofffilm. covered by a leaf-shaped tissue or flor material, e.g. of polyethylene, polyamide, polyester or polypropylene fibre, or with an open-pore plastic foam layer, e.g. of polyurethane foam or with a paper or a porous plastic film.
The gel-like polymer is particularly easily deformable or flowable in contrast to the net-formed polymers used for wound treatment up to now. Its non-liquid part consists of e.g. of a cellulose or starch ether, especially of hydroxyethyl cellulose, carboxymethyl cellulose or methyl cellulose or a corresponding starch derivative.
Suitable cellulose ethers are furthermore alkyl hydroxyalkyl ether, alkyl carboxymethyl ether, hydroxyalkyl carboxymethyl ether and alkyl hydroxyalkyl carboxymethyl ether.
Furthermore, gel-like polymers are suitable, e.g. gels based on acrylic acid and methacrylic acid or their derivatives, as well as polyvinylpyrrolidone, polyvinyl alcohol, agar or agarose.
However, it is also possible to use chemically modified cellulose ethers as described in DE-OS 19 12 740, 23 57 079, 23 58 150 and 25 19 927. Cellulose ethers modified with mono, di or trivalent ion-containing compounds are also suitable gel-like polymers.
The thickness of the gel-like polymer can vary within wide limits depending on the purpose of use and is, however, suitably at approx. 1 to 5 mm. When using a device with a transparent gel-like polymer and a transparent membrane for the treatment of skin damage, the healing process can advantageously be followed and controlled optically.
I en videre utførelsesform står membranen i forbindelse med In a further embodiment, the membrane is in connection with
This reservoir is formed e.g. by means of a liquid-tight foil, which covers the membrane, and forms a cavity with it. The surface of the membrane bordering this cavity is likewise covered with a liquid-tight foil with openings, or nominal breaking points, which are only opened immediately before the covering device is used. This cavity is filled with, for example, a bactericidal or therapeutically effective solution, which enters the gel-like polymer through the membrane and then hits the skin site to be treated. This reservoir thus enables a quick filling and/or a continuous re-dosing of liquid and medication in the cover-in rething. The dosing rate is thereby controlled by the type and pore size of the membrane. It is also possible to mix the gel-like polymer already when filling in a membrane tube, with the therapeutically effective solution.
The covering device according to the invention fulfills the initially mentioned tasks. They can also be manufactured economically and are easy to handle in clinical use.
The figure shows an embodiment in section. It consists of a membrane in the form of a hose bag 1 and a gel-like polymer 2.
