Subject: Tissue adhesive and method for its preparation.
The invention relates to a tissue adhesive based on human or animal proteins with a content of factor XIII, fibrinogen and an antibiotic and a method for its preparation.
There are already known from Austrian patents 359 - 652 and 359 * 653 methods for preparing a tissue adhesive containing fibrinogen and factor XIII, in which certain concentration ratios of factor XIII to fibrinogen and optionally albumin 10 are adjusted and the preparations frozen or lyophilized. These preparations had quite satisfactory properties, i.e. a high load-bearing capacity of the adhesive and good resorbability; however, it is desirable to improve these preparations in terms of antimicrobial activity.
Whey has already been proposed in U.S. Pat. No. 2,533,001 as well as by Fellinger et al. In the magazine "Der Tuberkulosearzt," (6/11, 1952) to provide fibrinogen solutions with antibiotics and to use them as wound adhesives. solutions to be prepared on the sick bed do not have sufficient shelf life and loadability of the fibrin clot formed therefrom.
Furthermore, from an article by Bosch et al., Archiv fiir orthopadische und Unfall-Chirurgie, part 90 (1977) "biz. 63-75, in combination with bone grafts for filling the bone defects, it is known to use a fibrin pasting system, the fibrin being locally formed at the bone cavity by the addition of thrombin to a fibrinogen solution. Where necessary, 00k commercial combination preparations of the powdered neomycin were added.
Finally, in PCT application 80 / OOO83, it has been proposed to prepare a fibrizene antibiotic gel, using a sick bed mixture of an eryoprecipitate with tobramycin and gentamycin as an antibiotic.
Own tests have shown that the disclosed and known 8202982 -2-4 '* # 41 tissue adhesives containing fibrinogen, factor XIII and a. antibiotic, the desired combination of properties, viz. do not have a high tackiness of the adhesive and an antimicrobial activity, but that an unfavorable interaction occurs between the antibiotics and the factor XIII, with the result that the crosslinking capacity of the fibrinogen decreases sharply and the coagulation capacity is adversely affected. . As a result, a low strength and adhesive capacity of the adhesive occurs. wound · resp. tissue surfaces.
A further drawback of the known preparations consists in that the release of the antibiotic to the tissue takes place too quickly, so that the antibiotic is delayed. is insufficient to. be active for a long time and achieve a high active substance release.
The object of the invention is to avoid these drawbacks and difficulties and it relates to a tissue adhesive of human or animal origin, which satisfies the said combination properties and a better activity of. the antibiotic guarantees.
This object is met by a tissue adhesive of the aforementioned type, wherein the ratio of factor XIII to fibrinogen, expressed in units of factor XII per g of fibrinogen, is at least 500 and an antibiotic from the group of the aminoglycosides, beta lactams, polypeptides or tetra cyclins are used.
With a frozen tissue adhesive it is advantageous to use the factor XIII 25 in an amount of at least 0 0 h / cm.
In another embodiment, at least 33 weight percent fibrinogen is present in a lyophilized tissue adhesive, factor XIII being present in an amount of at least 170 unit / g lyophilisate.
A plasmin inhibitor, e.g. plasminogen activator inhibitor from the group of aprotinin, C 1 -antiplasmin, c / g macroglobulin, 1-antitrypsin, 1-aminocaproic acid and tranexamic acid.
Most preferably, the tissue adhesive is in the form of a two-component composition, in the first component of factor XIII, 8202982-3% c%.
fibrinogeen en de plasmineremstof resp. plasminogeen-activator-rem-* stof aanwezig is, terwijl in de tveede component het antibioticum, thrombine en tweewaardig calcium vervat zijn.fibrinogen and the plasmin inhibitor resp. plasminogen activator inhibitor is present, while the second component contains the antibiotic, thrombin and divalent calcium.
Preferably, the antibiotic is present in the form of a sparingly soluble derivative. A variant of this embodiment consists in that,. in addition to the sparingly soluble derivative, a readily soluble derivative is also used, optionally divided into the two components of the tissue adhesive. This embodiment trembles.
• voordeel, dat bet goed oplosbare derivaat snel wordt afgegeven en 10 een boge beginactiviteit garandeert, terwijl bet moeilijk oplosbare derivaat een langdurige activiteit doet optreden.• advantage that the highly soluble derivative is released quickly and guarantees a high initial activity, while the sparingly soluble derivative causes a long-lasting activity.
The invention further comprises a method for preparing this tissue adhesive, a modification of which comprises, in a fibrinogen-containing blood plasma fraction, a concentration ratio of factor XII to fibrinogen, expressed in units of factor ΧΙΙΙ / g of fibrinogen of at least 500 by a addition of factor XIII is adjusted, then the antibiotic is added and the preparation is deep frozen or lyophilized.
According to another modification,. .in a fibrinogen-containing blood plasma fraction, set a concentration ratio of factor XIII to fibrinogen, expressed in units of factor g / g of fibrinogen of at least 500 by adding factor XIII, after which. the preparation is deep frozen or lyophilized and after thawing resp.
In this embodiment, the antibiotic can be added after thawing or added to the reconstitution solution.
However, care should be taken in these embodiments that a minimum concentration of factor XIII is not undercut; 3 it must always be above one bed / cm.
In a preferred embodiment, wherein the fibrinogen-containing blood plasma fraction is washed with a buffer solution, this becomes. washing process until reaching a factor XIII concentration of 200 units of factor ΧΙΙΙ / g fibrinogen is performed, after which factor XIII · .in. an amount of at least 300 units / g of fibrinogen 35 in the form of a concentrate or lyophilisate is added.
