Abstract
다축성 육경 스크류 조립체는 볼 조인트를 형성하기 위해 스크류의 헤드 상에서 볼록면과 결합하는 리시버 상의 오목부를 포함한다. 상기 오목면의 적어도 일부의 반경은 결합하는 볼록부의 반경보다 작으며, 억지끼워맞춤을 제공한다.
Description
본원에 참고로 하고 있는 비더만(Biedermann) 등에 의한 미국 특허 제 5,443,467호에 공개된 바와 같은 다축성 육경 스크류들은 척추 수술시 척추골들을 로드에 연결하기 위해 사용된다. 이들은 외과의사들이 스크류들을 위치시키는 데 있어서 약간의 유연성을 허용하기 위해 상기 로드의 연결시 볼 조인트를 포함한다. 스크류 상에 너트를 조임으로써 상기 볼 조인트의 각위치를 체결하기 위해 볼 조인트 요소들을 압축한다.
본 발명은 다축성 육경 스크류(polyaxial pedicle screw)에 관한 것이다.

본 발명은 볼 조인트를 체결할 때 성취될 수 있는 체결력을 개선하기 위한 것이다.
본 발명에 따른 육경 스크류 조립체는 볼록부를 갖는 헤드와 상기 헤드를 수용하는 리시버를 구비한 스크류를 포함한다. 상기 리시버는 또한 척추 고정 로드와 같은 연장 부재를 수용한다. 상기 리시버는 상기 헤드의 볼록부의 곡률 반경보다 작은 곡률 반경을 갖는 오목부를 가지며, 상기 헤드의 볼록부와 리시버의 오옥부 사이에 억지끼워맞춤을 제공한다.
적합하게도, 상기 리시버 상의 너트는 상기 헤드의 볼록부를 상기 리시버의 오목부 안으로 압축한다. 한 가지 편리한 점은, 상기 리시버가 연장 부재를 수용하기 위한 U-형상부를 포함한다는 사실이다. 적합하게도, 상기 리시버의 오목부는 티타늄으로 제조된다. 비록 다른 형상들이 채용될지라도, 한나의 적합한 실시예에 있어서는 각각의 오목부 및 볼록부는 구형 형상을 갖는다. 표면의 결합 전에 상기 헤드와 리시버의 자유 회전을 허용하는 어떠한 형상도 허용될 수 있다.
하나의 적합한 실시예에 있어서, 상기 스크류는 그 위에 골질 나사산(bone thread)들을 갖는 연장 섕크와 그의 한 단부에 위치하는 헤드를 포함하며, 상기 리시버는 상기 섕크를 수용하기 위해 관통하는 구멍 및 상기 구멍에 위치한 오목부를 갖는 몸체를 포함한다. 상기 리시버는 또한 상기 구멍의 반대쪽을 관통하는 채널을 부가로 포함한다.
상기 육경 스크류는 상기 연장 부재와 헤드 사이에 압축 부재를 부가로 포함한다; 상기 헤드는 상기 압축 부재와 마주하는 제 2 볼록부를 가지며, 상기 압축 부재는 상기 헤드와 마주하는 제 2 오목부를 가지며, 상기 제 2 오목부는 제 2 볼록부의 곡률 반경보다 작은 곡률 반경을 가지며, 상기 헤드와 압력 부재 사이에 억지끼워맞춤을 제공한다.
한 실시예에 있어서 상기 오목부와 볼록부 사이의 곡률 반경의 차이는 약0.05㎜이다.
도 1 및 도 2는 본 발명에 따른 다축성 스크류 조립체(10)를 설명한다. 상기 조립체는 특히 육경을 통해 척추골의 해면뼈(cancellous bone) 안으로 삽입하기 위한 해면체 나사산(14)을 갖는 스크류(12)를 포함한다. 구형 헤드(16)는 볼록면(18) 및 6각 드라이버나 또는 기타 공구(도시하지 않음)를 수용하기 위한 공구 리세스(20)를 갖는다. 상기 헤드(16)는 내부 오목면(24) 및 근접 개구부(26)를 갖는 관상 리시버(22) 내에 수용된다. 상기 헤드(16)의 볼록면(18)은 상기 오목면(24)과 결합한다. 상기 개구부(26)는, 스크류(12)가 상기 리시버(22)로부터 빠지는 일 없이 상기 개구부(26) 밖으로 돌출할 수 있도록, 상기 헤드(16)보다 작게 된다.
압력 디스크(28)는 상기 헤드(26)의 꼭대기에 위치하며, 상기 헤드(26)의 것과 맞물리 형상의 표면(30)을 갖는다. 상기 리시버는 또한 연장 로드(34)를 수용하는 U-형상부(32)를 갖는다. 상기 로드(34)는 당업계에서 공지된 바와 같은 인접하는 척추골들을 연결하기 위해 사용된다. 숫나사(36)와 암나사(38)는 차례로 상기 리시버 오목부(24) 안으로 헤드 볼록부(18)를 압축하는 압력 디스크(28)에 대항하여 상기 로드(34)를 압축하며, 상기 스크류(12)에 대해 상기 리시버(22)의 각위치를 체결한다.
상기 압력 디스크(28)는 적합하게도, 상기 리시버(22) 상의 재료가 리시버(22) 내에 압력 디스크(28)를 보유하고 그 안에서 약간의 운동을 허용하도록, 스웨이지(swage)(42)를 구부리는 측면 함입부(40)를 갖는다.
도 3은 종래의 유사한 스크류 조립체와 비교할 때 상기 스크류(12)에 대해 리시버(22)의 체결을 개량한 특징에 대해 설명한다. 2개가 함께 압축될 때 재료는 상기 표면들을 억지끼워맞춤으로 결합하도록 다소 변형되고 따라서 표면들 사이의 그립을 강화하도록, 상기 오목면(26)은 상기 볼록면(18)보다 약간 작은 곡률 반경을 갖는다.
종전 스크류에 대한 토크 부하 하에서의 스크류(12)의 편향 테스트는 종전 스크류에 대한 편향의 큰 감소를 나타내며, 따라서 미끄러짐은 줄어들고 체결성은 개선된다. 표 1은 억지끼워맞춤을 갖는 스크류와 억지끼워맞춤을 갖지 않는 스크류의 테스트 결과를 나타낸다. 표 1에서는 7개의 스크류가 스테인레스강으로 제조되고, 표 2에서는 7개의 스크류가 티타늄으로 제조되었다. 매그넘(Magnum)으로 분류된 스크류들은 억지끼워맞춤을 포함하고, 다른 것들은 포함하지 않는다. 상기 스크류들은 유사한 치수를 갖는다; 수자들은 스크류가 로드 사이즈와 관련된 후 기입되었다. 상기 테스트들은 스크류들을 균일한 토크로 체결하고 헤드(16)에서 토크를 유도하기 위해 스크류(12)에 횡력을 제공하는 것으로 구성된다. 0.5㎜의 오프셋에서의 부하와 강도가 각각 샘플용으로 부과되었다. 본 발명의 억지끼워맞춤을 갖는 스크류들은 양쪽 매개변수 모두에서 개선되었다.

적합한 일 실시예에 있어서, 상기 헤드 볼록면(18)은 6.995㎜의 직경을 가지며, 결합 리시버 오목면(24)은 6.88㎜의 직경을 갖는다. 상기 압력 디스크면(30)과 헤드(16)의 결합 접촉면에도 또한 유사한 계면 치수가 제공될 수 있다.
본 발명은 특정 실시예에 대해 설명하고 있으나, 당업자라면 본 발명이 그와 같은 특정 실시예에 의해 제한받지 않으며, 상술된 기술 내용에 비추어 변경이 가능할 수 있음을 당연히 이해할 수 있을 것이다. 본 발명의 정신에서 벗어나지 않는 한도 내에서, 본 발명은 상술된 기술 내용의 합리적인 변경과 수정이 가능함을 밝혀둔다.
Claims (8)
- 볼록부를 갖는 헤드를 구비한 스크류와;상기 헤드와 연장 부재를 수용하는 리시버를 포함하며,상기 리시버는 상기 헤드의 볼록부의 곡률 반경보다 작은 곡률 반경을 갖는 오목부를 가지며, 상기 헤드의 볼록부와 리시버의 오목부 사이에 억지끼워맞춤을 제공하는 육경 스크류 조립체.
- 제 1항에 있어서, 상기 헤드의 볼록부를 상기 리시버의 오목부 안으로 압축하는 리시버 상의 너트를 추가로 포함하는 육경 스크류 조립체.
- 제 2항에 있어서, 상기 리시버는 상기 연장 부재를 수용하는 U-형상부를 포함하는 육경 스크류 조립체.
- 제 2항에 있어서, 상기 리시버의 오목부는 티타늄으로 제조되는 육경 스크류 조립체.
- 제 1항에 있어서, 상기 각각의 오목부 및 볼록부는 구형 형상을 갖는 육경 스크류 조립체.
- 제 1항에 있어서, 상기 스크류는 그 위에 골질 나사산(bone thread)들을 갖는 연장 섕크와 그의 한 단부에 위치하는 헤드를 포함하며;상기 리시버는 상기 섕크를 수용하기 위해 관통하는 구멍 및 상기 구멍에 위치한 오목부를 갖는 몸체를 포함하고;상기 리시버는 또한 상기 구멍의 반대쪽을 관통하는 채널을 추가로 포함하며, 상기 채널은 상기 연장 부재를 수용하는 육경 스크류 조립체.
- 제 6항에 있어서, 상기 연장 부재와 헤드 사이에 압축 부재를 추가로 포함하며;상기 헤드는 상기 압축 부재와 마주하는 제 2 볼록부를 가지며, 상기 압축 부재는 상기 헤드와 마주하는 제 2 오목부를 가지며, 상기 제 2 오목부는 제 2 볼록부의 곡률 반경보다 작은 곡률 반경을 가지며, 상기 헤드와 압력 부재 사이에 억지끼워맞춤을 제공하는 육경 스크류 조립체.
- 제 1항에 있어서, 상기 오목부의 곡률 반경은 상기 볼록부의 곡률 반경보다 약 0.05㎜ 작은 육경 스크류 조립체.
