Abstract
본 발명은 공중합체-1의 섭취 또는 흡입에 의한 다발성 경화증의 치료 및 그 치료에 사용되는 약학적 조성물에 관한 것이다.
Description
글라티라머 아세테이트(glatiramer acetate)로 또한 공지되어 있으며, 코팍손(Copaxone)이라는 상표명으로 시판되는 공중합체-1은 L-글루탐산, L-알라닌, L-티로신 및 L-리신을 함유하는 폴리펩티드의 아세테이트 염을 포함한다. 아미노산의 평균 몰분율은 각각 0.141, 0.427, 0.095 및 0.338이며, 공중합체-1의 평균 분자량은 4,700 내지 11,000 달톤이다. 이것은 다양한 종에 있어서 모든 마이엘린 항원, 예컨대 마이엘린 염기성 단백질 (MBP)[Sela M et al., Bull Inst Pasteur (1990) 88 303-314], 프로테오리피드 단백질 (PLP)[Teitelbaum D et al., J Neuroimmunol (1996) 64 209-217] 및 마이엘린 올리고덴드로사이트 당단백질 (MOG)[Ben-Nun A et al., J Neurol (1996) 243 (Suppl1) S14-S22]을 포함하는 마우스 척수의 균질물 (mouse spinal cord homogenate; MSCH)을 포함하는 다양한 뇌염발생원에 의해 유발된 실험적인 알레르기성 뇌척수염(experimental allergic encephalomyelitis; EAE)을 억제하는 것으로 입증된 비-자가항원이다. 공중합체-1은 피하로, 복강내로, 정맥으로 또는 근육내 주사시 활성이 있는 것으로 입증되었다 [D. Teitelbaum et al., Eur. J. Immunol. (1971) 1:242-248; D. Teitelbaum et al., Eur. J. Immunol. (1973) 3:273-279].
제Ⅲ상(phase Ⅲ) 임상 시험에서, 매일 공중합체-1을 피하로 주사하면, 악화-완화되는 다발성 경화증에서 장애가 느리게 진행하고 재발율이 감소한다는 것이 발견되었다 [K.P. Johnson, Neurology (1995) 1:65-70]. 현재 공중합체-1 치료법은 그것의 일일 피하 투여로 제한되어 있다.
현재, 다발성 경화증의 특히 인정된 치료법은 활성 물질의 자가-주사를 포함한다. 주사-부위에서 자주 관찰되는 문제점은 자극, 과민성 반응, 염증, 통증, 심지어 괴사(하나 이상의 인터페론 β1-B 치료의 경우) 및 낮은 수준의 환자 순응도를 포함한다. 따라서, 대안적인 투여법이 바람직하다.
유럽 특허 제 359,783호는 자가항원을 경구 투여하여 자가면역 질환을 치료하는 것을 개시하고 있다. 이것은 다발성 경화증의 치료를 위한 MBP의 경구 투여를 개시한다. 자가항원의 경구 투여를 “경구 내성(oral tolerance)”이라고 명명했다.
PCT 국제 출원 공보 No. WO 91/12816, WO 91/08760 및 WO 92/06704 모두는 다양한 자가항원을 가지고 “경구 내성”법을 이용하여 다른 자가면역 질환을 치료하는 것을 개시하고 있다. 그러나, 이들 참고 문헌 중 어느 것도 비-자가항원 공중합체-1의 경구 투여에 의한 다발성 경화증의 치료법을 개시하고 있지 않다. 전술한 모든 이들 특허의 내용 및 모든 참고 문헌은 전체로서 본원에 참고로 인용된다.
따라서, 본 발명의 목적은 섭취 또는 흡입을 통해 공중합체-1을 경구 투여함으로써 다발성 경화증을 치료하는 방법을 제공하는 것이다.
본 발명은 공중합체-1(copolymer-1)(아래에 정의됨)의 섭취 또는 흡입에 의한 다발성 경화증의 치료에 관한 것이다. 또한, 본 발명은 다발성 경화증의 치료에 사용되는 공중합체-1을 포함하는 약학적 조성물에 관한 것으로서, 상기 약학적 조성물은 섭취 또는 흡입에 의한 투여의 목적으로 제제화된다.
도1 및 도2는, 비장 세포 증식으로 평가했을 때, 공중합체-1이 래트(도1) 및 마우스(도2)에서 기니아 피그 마이엘린 염기성 단백질(GPBP)에 대한 면역 반응에 미치는 효과를 도시한다.
도3 및 도4는 공중합체-1의 사이토킨(cytokine) 방출에 대한 효과를 도시한다.
도5는 경구 투여된 공중합체-1에 의한 마우스에서의 EAE의 억제를 도시한다. (SJL/JxBALB/c) F1마우스에게 PBS( ■), 0.1 mg의 공중합체-1( □), 0.25 mg의 공중합체-1( ▲) 또는 0.5 mg의 공중합체-1(△)을 공급했다. 각 투여량을 7회 -7; -5; -3; 0; 2; 4 및 6일째 되는 날에 공급했다. 0일에 MSCH을 주사하여 EAE를 유도했다.
도6은 공중합체-1을 공급한 래트의 비장에서 유래된 T-세포주에 의한 증식 및 사이토킨 분비를 도시한다. 세포를 공중합체-1(50 μ/ml) 또는 콘카발린 A(Con A)(5 ㎍/ml)의 배지에서 배양했다. 이들 항원에 대한 증식 및 사이토킨 분비 반응을 측정했다.
도7은 공중합체-1을 공급한 래트의 비장에서 유래된 T-세포에 의한 EAE의 억제를 도시한다. 공중합체-1로 자극한 후 3일이 되었을 때, 세포(20 x 106/래트)를 복강내 주사하여 EAE를 유도했다.
도8은 공중합체-1을 공급한 마우스의 비장에서 유래된 T-세포주에 의한 EAE의 저해를 도시한다. 공중합체-1로 자극한 후 3일이 되었을 때, 세포(15 x 106/마우스)를 정맥 주사하여 EAE를 유도했다.
도9는 3마리의 레서스(Rhesus) 원숭이에서 임상 점수(clinical score) 대 EAE 질병 유도 이후 공중합체-1의 일수를 도시한다.
도10은 장용 코팅 대 비코팅된 약학적 제형을 비교하는 시험에서 6마리 레서스 원숭이에서의 임상 점수 대 EAE 질병 유도 이후 공중합체-1의 일수를 도시한다. 결과를 더 잘 도시하기 위해, 0에서의 값은 y-축상에서 분리되었다.
따라서, 본 발명은 다발성 경화증의 치료를 위한 약물의 제조에서 공중합체-1을 사용하는 것에 관한 것이다. 이 약물은 섭취 또는 흡입을 통해 투여된다.
또한, 본 발명은 치료적 유효량(therapeutically effective amount)의 공중합체-1을 섭취 또는 흡입을 통해 투여하는 것을 포함하는, 다발성 경화증의 치료 방법에 관한 것이다.
또한, 본 발명은 약학적 허용 담체 및 치료적 유효량의 공중합체-1을 포함하는, 섭취 또는 흡입을 통해 투여되는 약학적 조성물에 관한 것으로서, 상기 약학적 조성물은 다발성 경화증의 치료에 사용된다.

공중합체-1은 면역계 감작의 1차적인 원천으로 여겨지는 점막 내부(mucosal lining)의 임파 조직과 접촉하게 된다. 이들 점막 내부는 동(洞)(sinus), 기관(trachea), 기관지 통로(이들은 BALT 또는 기관지 연관 임파 조직으로 알려져 있음) 및 위장의 내막(GALT 또는 내장 연관 임파 조직으로 알려져 있음)에서 발견될 수 있다(반드시 그러한 것은 아니다). 그러므로, 공중합체-1의 투여는 공중합체-1이 섭취 또는 흡입에 의해 체내로 도입되는 방법을 포함하는 것으로 이해된다. 예를 들어, 공중합체-1이 섭취를 통해, 위관(stomach tube)을 통해 경구로 투여될 수 있고, 기관지 통로로의 흡입 또는 비강 흡입에 의해 투여될 수 있다.
본 발명의 예시적인 구체예 중에서 공중합체-1은 하루 0.1 내지 1000 mg 의 양으로 경구 투여되며, 이는 단일 투여량 또는 다중 투여량으로 투여될 수 있다. 당업자가 이해하는 바와 같이, 치료적 유효량은 일반적으로 환자의 연령, 성별 및 신체적 조건의 함수일 뿐 아니라 투여되는 기타 동반 치료 요법들의 함수이다. 최적의 치료적 유효량은 당업자가 의도하는 범위내에서 적절히 결정된다.
공중합체-1이 경구 투여되면, 이는 다른 식이 형태와 혼합되며, 고형, 반-고형, 현탁액 또는 유제 형태로 소모될 수 있고, 이는 물, 현탁화제, 유화제, 향미제 등을 포함하는 약학적 허용 담체와 혼합될 수 있다. 하나의 구체예에서, 경구 조성물은 장용 코팅된다. 장용 코팅(enteric coating)의 용도는 당해 기술 분야에 잘 공지되어 있다. 예를 들어, 참고 문헌[K. Lehman, Acrylic Coatings in Controlled Release Tablet Manufacturer, Manufacturing Chemist and Aerosol News, p. 39 (1973년 6월), 및 K. Lehman, Programmed Drug Release From Oral Program Forms; Pharma. Int., vol. ISS 3 1971, p. 34-41]은 유드라짓(Eudragit) S 및 유드라짓 L과 같은 장용 코팅에 대해 교시하고 있다. 또한, 참고 문헌[Handbook of Pharmaceutical Excipients, 2nd ed.,]도 유드라짓 S 및 유드라짓 L의 적용에 대해 교시하고 있다. 본 발명에 사용될 수 있는 하나의 유드라짓은 L30D55이다.
또한, 공중합체-1은 흡입 또는 비강 점적에 의해 전술한 특정 형태로 비강 투여될 수 있다. 게다가, 공중합체-1을 경구 흡입시켜 기관 또는 기관지 통로의 점막 내부로 전달되도록 할 수 있다.
공중합체-1은 당해 기술 분야에 공지된 방법, 예를 들어 미국 특허 번호 제 3,849,550호에 개시된 방법으로 제조될 수 있으며, 상기 특허에서 티로신, 알라닌, 감마-벤질 글루타메이트 및 ε-N-트리플루오로아세틸리신의 N-카르복시무수물은 주변 온도에서 개시제로서 디에틸아민을 함유하는 무수 디옥산 중에서 중합된다. 글루탐산의 감마-카르복실기의 탈블록킹(deblocking)은 빙초산 중의 브롬화수소에 의해 수행되며, 이어서 1M 피페리딘에 의하여 리신 잔기로부터 트리플루오로아세틸 기가 제거된다.
PCT/WO95/31990에 기재된 것과 같이, 약 7 ±2 킬로달톤의 소정의 평균 분자량을 가진 공중합체-1은, 보호된 공중합체-1과 브롬화수소산을 반응시켜 트리플루오로아세틸 공중합체-1을 형성하는 단계, 트리플루오로아세틸 공중합체-1을 피페리딘 수용액으로 처리하여 공중합체-1을 형성하는 단계 및 공중합체-1을 정제하여 소정의 평균 분자량을 가진 공중합체-1을 생성하는 단계를 포함하는 방법에 의해 제조되는 것이 바람직하다.
하기 실시예는 본 발명을 더 자세히 설명할 것이다, 그러나, 본 발명이 실시예에 의해 제한되는 것으로 해석되어서는 안된다. 다른 지시가 없는 한, 모든 부(part), 백분율 등은 중량 기준이다.
실시예 1
항원
PCT/WO95/31990에 기재된 방법으로 제조된 공중합체-1은 이스라엘의 테바 파마슈티칼 인더스트리 리미티드로부터 입수되었다. GPBP는 참고 문헌[Hirshfeld H et al., Febs Lett (1970) 7 317-320]에 기재된 것과 같이, 기니아 피그 척수로부터 산 추출 및 황산 암모늄 침전에 의해 제조되었다.
동물
잭슨 연구소(Jackson Laboratory; Bar Harbor, ME)로부터 (PL/JxSJL/J) F1암컷 마우스(8-10주 된 것)를 입수했다. 암컷 루이스(Lewis) 래트(8-12주 된 것)는 할란-올락(Harlan-Olac, Bicester, G.B.)으로부터 입수되었다.
EAE의 유도 및 평가
4 mg/ml의 마이코박테리움 투베르쿨로시스(H37Ra)(미시간주 디트로이트의 디프코 연구소)를 함유하는 동일한 부피의 프로인트 완전 보조액(CFA) 중에 유화시킨 GPBP 200 ㎍을 마우스에 주사했다. 총부피 0.1 ml의 유제를 4개의 모든 발바닥에 주사했다. 주사 후 즉시 및 24시간 경과후에, 백일해(pertussis) 독소(250 ng/마우스)(시그마)를 정맥으로 주사했다.
4 mg/ml H37Ra를 함유하는 CFA 중에 1:1로 유화시킨 GPBP 25 ㎍으로 래트를 면역화시켰다. 이 유제를 총부피 0.1 ml로 2개의 뒷 발바닥에 주사했다.
질병의 징후에 대해 유도후 10일째 되는 날로부터 매일 동물을 검사했다. EAE의 점수는 하기와 같다:
0 - 질병이 없음
1 - 기운이 없는 꼬리
2 - 2개의 뒷다리 마비
3 - 4개의 모든 다리의 마비
4 - 빈사 상태

경구 내성의 유도
마우스에 인산염 완충 염수(PBS) 중에 용해시킨 GPBP 또는 공중합체-1 250 ㎍을 18-게이지의 스테인레스 강 공급 바늘(토마스)로 위 삽관(gastric intubation)하여 -7, -5, -3, 0, 2, 4 및 6일째 되는 날에 공급했다. EAE는 0일에 유도되었다.
PBS중에 용해시킨 GPBP 또는 공중합체-1 1 mg을 멸균 공급 튜브(우노 플라스트, 덴마크)를 사용하는 위 삽관에 의해 공급했다. 질병이 유도되기 전에 2-3일 간격으로 래트에게 5회(총투여량 5 mg) 공급했다. EAE는 최종 공급 후 2일째 되는 날에 유도되었다. 대조 마우스 및 래트에게는 위약(僞藥)으로 PBS를 공급했다.
증식 분석
전술한 것과 같이, 비장 세포의 증식 반응을 EAE 유도 이후 10-11일경에 시험했다. 각 군에 있는 3마리의 동물로부터 얻은 세포를 모아서, 최종 부피 0.2 ml중에 다양한 항원 농도(GPBP)를 가진 마이크로타이터 플레이트(microtiter plate) 중에 3중으로(5 x 105마우스 세포 및 2 x 105래트 세포) 배양했다. 마이크로타이터 플레이트는 1% 자가혈청을 보충한 RPMI 1640(미주리주 세인트 루이스의 시그마 바이오케미칼스에서 시판) 배양 배지를 함유했다. 72시간의 배양 후, 18시간동안 1μCi {3H}-티미딘을 사용하여 세포를 펄스화한 후, 여과지 상에서 수확하고, 방사능을 계산했다.
사이토킨 분비 분석
EAE 유도 후 10-11일째 되는 날에 비장을 제거하고, 각 군으로부터 3마리 마우스의 세포를 모았다. 세포(5 x 106/ml)는 항원(GPBP 100 Fg/ml)의 존재 또는 부재시, 10% FCS(태아 송아지 혈청)를 보충한 RPMI 1640 중의 24 웰 플레이트 중에서 이중으로 배양되었다. 24 내지 40시간 배양한 후, 상청액을 수확했다. IL-2, IFN-감마, IL-4, IL-6 및 IL-10에 대한 정량 ELISA는, 제조자의 지시에 따라 해당 사이토킨(Pharmingen, LA Jolla, CA)에 대해 특이적인 mAb의 쌍을 사용하여 수행되었다.
결과
GPBP에 의한 경구적인 억제를 유도하는 것으로 이미 보고된 조건하에서 평가되었을 때[PJ Higgins & HL Weiner, J Immunol (1988) 140 440-445], 루이스 래트에서 EAE의 임상적인 징후를 방지하는 데 있어서 경구 투여된 공중합체-1의 효능을 GPBP의 효능과 비교하였다. 아래의 표 1에 제시된 결과는, 대조군으로 작용하는 PBS를 공급한 래트에 비해서 공중합체-1이 GPBP보다 더 효과적이며 EAE의 발병(54% 저해) 및 심각도(57%의 저해) 모두를 유의성 있게 감소시켰다는 것을 입증한다.

GPBP에 대한 면역 반응에 있어서 항원 공급의 효과
질병을 유도하는 항원-GPBP에 대한 면역 반응에 있어서 공중합체-1 및 GPBP의 경구 투여 효과를 마우스 및 래트에서 시험했다. 그 결과는 도1에 제시되어 있는데, 이는 GPBP로 자극한 래트의 비장 세포 현탁액 중에 경구 투여된 화합물(공중합체-1 또는 GPBP) 각각에 의해 세포의 증식이 감소되었다는 것을 나타낸다(도2는 마우스에서의 결과와 유사한 결과를 나타낸다). 도시된 것과 같이, 공중합체-1의 경구 투여는 이들 2종에서 GPBP에 대한 증식적 반응을 거의 완전히 저해했다. 2종 모두에서 공중합체-1은 GPBP에 대한 반응을 저해하는 데 있어서 GPBP보다 더 유효했다.
마우스에서 유래된 비장 세포 배양물의 상청액중 사이토킨 함량을 측정했다(도3 및 도4). PBS를 공급한 대조 마우스는 GPBP에 대한 반응에서 IL-2, IFN-γ 및 IL-6(도시되지 않음)을 분비했다. 공중합체-1 또는 GPBP를 공급한 마우스에서, GPBP 자극에 대한 응답으로 생성되는 Th1 전염증성(proinflammatory) 사이토킨 IL-2 및 IFN-γ의 양은 대조군에서보다 낮았다. (공중합체-1은 보다 더 효과적인 억제제이다.) IL-4 및 IL-10은 어떤 군의 치료에서도 검출되지 않았다.
이 결과는 공중합체-1이 경구 투여되었을 때 EAE를 억제하는 데 효과적이라는 것을 입증한다. 경구 투여된 공중합체-1의 임상적 보호 효과는 GPBP에 대한 T 세포 면역 반응, 예컨대 증식 및 전염증성 사이토킨(IL-2 및 IFN-γ) 방출의 억제조절(down regulation)과 관련된다.
실시예 2
공중합체-1을 경구 투여함으로써 EAE를 억제하는데 대한 추가적인 연구는 래트 및 마우스에서 수행되었다. 이들 연구는 각 종을 치료하기 위한 최적의 투여량을 확립했다. 공중합체-1에 의한 EAE의 경구적 저해의 근본 메커니즘을 이해하기 위해, 공중합체-1을 공급한 동물의 비장에서 공중합체-1 특이성 T-세포주를 분리했다. 이 세포주의 시험관내(in vitro) 반응성 및 질병 유도에 대한 생체내(in vivo) 효과를 연구했다.
물질 및 방법
공중합체-1 특이성 T 세포주의 분리
루이스 래트에게 공중합체-1 1 mg을 5회 공급하고, (SJL/J x BALB/c) F1마우스에게 공중합체-1 250 ㎍을 2-3일 간격으로 7회 공급했다. 최종 공급 후 4 내지 12일이 되었을 때, 동물을 치사시키고 이들의 비장을 제거했다.
3마리 동물의 비장 세포를 모아서, 1% 자가혈청을 함유하는 배지중에 4일간 공중합체-1(500 ㎍)과 배양했다(50 x 106/플레이트). 14-21일마다 세포(4-6 x 106/플레이트)를, 조사된(3000 rad) 선천성(syngeneic) 래트 흉선세포(100 x 106/플레이트) 또는 마우스 스플레노사이트(50 x 106/플레이트)에 주어진 공중합체-1(500 ㎍)에 3일간 노출시켜 재자극했다. 자극 후, T세포 성장 인자(TCGF)와 같이 Con A에 의해 활성화된 정상 마우스 비장 세포의 10% 상청액 중에서 증식시켰다.
증식 분석
T 세포주(1 x 104세포수)를 조사된 (3000 R) 흉선세포(래트- 1 x 106) 또는 스플레노사이트(마우스- 5 x 105) 및 표시된 항원(10 ㎍의 공중합체-1; 1 ㎍의 Con A)을 사용하여 마이크로타이터 플레이트 중에서 최종 부피 0.2 ml로 배양했다. 48시간의 배양 종반부에 배양물을3H-티미딘을 사용해 펄스화하고, 6-12시간 경과 후에 수확했다.
사이토킨 분석
래트의 T 세포주(0.5 x 106/ml)는 표시된 항원(50 ㎍의 공중합체-1, 5 ㎍의 Con A)이 있거나 또는 없는, 조사된 흉선세포(10 x 106)와 함께 배양하였다. IL-2, TNFα, IL-4 및 IL-10을 측정하기 위해 10% FCS를 보충한 RPMI 1640 중에 세포를 24시간동안 배양하고, TGFβ를 측정하기 위해 세포를 72시간동안 무혈청 배지-DCCM-1(Biological Industries, Kibbutz Beit Haemek, Israel)중에 배양했다.
상청액 중의 사이토킨 함량은 해당 사이토킨에 특이적인 모노클로날 항체쌍을 사용하여 정량 ELISA로 측정되었다.
EAE의 유도
(SJL/xBALB/c)F1마우스의 4개의 발바닥 모두에 1 mg/ml의 H37Ra(미시간주 디트로이트의 디프코 연구소)를 함유하는 CFA중에 1:1의 비율로 유화시킨 마우스 척수 균질물(MSCH) 2 mg을 주사했다. 백일해 독소(250 ng/마우스, 시그마)를 한번은 그 직후 및 두번째는 48시간 경과 후에 정맥 주사했다.
결과
1. 래트 및 마우스에서 투여량 반응 연구
확립된 프로토콜에 따라(상기 물질 및 방법 참조), 래트에게 공중합체-1 0.5, 1 또는 2 mg을 5회 공급한 후, 래트는 EAE 유도를 위해 공격(challenge)되었다. 그 결과는 표 2에 제시되어 있는데, 이는 EAE를 억제하는 데 공중합체-1 1 mg이 가장 유효한 투여량이고, 공중합체-1 0.5 mg 또는 2 mg은 덜 유효하다는 것을 나타낸다.
(SJL/JxBALB/c) F1마우스에게 위 삽관법으로 공중합체-1 0.1, 0.25, 또는 0.5 mg을 -7; -5; -3; 0; 2; 4 및 6일째 되는 날에 7회 공급했다. EAE는 MSCH의 주사에 의해 0일에 유도되었다. 도5에 제시된 결과는, 공중합체-1의 경구 투여로 마우스에서 질병을 억제할 수 있었다는 것과 가장 유효한 투여량은 공중합체-1 0.1 mg이었다는 것을 입증한다. 공중합체-1 0.25 mg은 덜 유효했으며, 0.5 mg의 투여량은 완전히 불활성 상태였다. 그러므로, 래트 및 마우스 양자에서의 결과는, 경구 공중합체-1은 최적의 투여량 반응 곡선을 가지며, 경구 유효량을 초과하는 것은 EAE를 억제하는 효과가 없다는 것을 입증한다.
2. 공중합체-1을 공급한 동물로부터 확립된 공중합체-1 특이성 Ts-세포주에 대한 연구
공중합체-1 특이성 T 억제(suppressor) 세포주는, 공중합체-1을 공급함으로써 EAE에 비반응적으로 된 래트 및 마우스의 비장으로부터 분리되었다. 래트에서 분리된 상기 세포주의 증식 및 사이토킨 분비 반응은 도6에서 나타낸다. 이 세포주는 공중합체-1에 대한 응답으로 증식했고, TNFα또는 IL-4를 제외한 IL-2, 일부의 IL-10 및 TGFβ를 분비했다. 이 사이토킨 프로파일(profile)은 경구 MBP에 의해 유도되는 것으로 나타난 Th3 유형의 세포와 양립가능하다(Chen et al. Science 265, 1237, 1994).
공중합체-1 특이성 세포주가 생체 내에서 EAE를 저해하는 능력을 연구했다. 공중합체-1로 생체 내에서 자극한 후 3일째 되는 날에 세포주를 주사했다(복강내 주사된 20 x 106세포수/래트 및 정맥 주사된 15 x 106세포수/마우스). 동물은 세포 이동에 뒤이어 즉시 EAE 유도를 위해 상기 동물을 공격하였다. 도7 및 도8에 도시된 결과는 질병이 수용체 동물에서 상당히 저해되었다는 것을 증명한다. 따라서, 래트 및 뮤린(murine)의 공중합체-1 T-세포주 모두는 공중합체-1의 경구 투여에 의해 유도된 EAE에 대해 무반응성을 제공하였다. 이들 세포는 생체 내에서 병적인 면역 반응을 활발히 억제조절한다.

실시예 3
레서스 원숭이에서 EAE의 유도에 대한 공중합체-1의 경구 투여 효과를 연구했다.
물질
공중합체-1은 테바 파마슈티칼 인더스트리 리미티드에서 2가지 투여량 레벨(1 mg의 공중합체-1 및 20 mg의 공중합체-1)을 포함하는 장용-코팅된 경질 젤라틴 캡슐중에 제공되었다. 각 투여량 레벨은 만니톨을 사용해 제제화되며, 유드라짓 L30D55로 코팅되었다. 플라시보(placebo) 또는 대조 캡슐은 5 mg의 당을 함유한 캡슐로 이루어졌다.
소의 MBP는 뉴욕 주 그랜드 아일랜드의 라이프 테크놀로지에서 구입했다. 이 물질은 SDS-PAGE 및 은 염색(Silver staining) 이후에, 18.5 Kd에서 단일 밴드(band)를 형성하는 고도로 정제된 제제를 나타낸다.
공급 프로토콜
3마리의 레서스 원숭이를 하기와 같이 처리했다: 한마리의 원숭이는 대조군의 역할을 하고, 플라시보 캡슐(5 mg의 글루코오스만을 함유)을 공급하였다. 제2 및 제3의 원숭이에게는 각각 1 mg/공급물 및 20 mg/공급물의 투여량으로 공중합체-1을 함유하는 캡슐을 공급했다. 동물들에게 격일로 총 10회 공급했다. 즉 5회는 질병 유도 전(0일에 면역화) 그리고 5회는 면역화 이후에 공급했다:
질병 유도
질병은 0일에 FCA 중에 8 mg의 소의 MBP 및 3 mg의 H37Ra M. 투베르쿨로시스를 뒷 발바닥에 피내 주사하여 유도되었으며, 총주사 부피는 발 하나당 0.1 내지 0.15 ml였다. 동물들을 EAE의 임상적 징후, 다양한 혈청 및 CSF 면역학적 마커 및 척수 및 두개골의 MRI에 대해 조사하였다.
임상 점수
증상 점수는 하기와 같다:

항원에 의해 유도된 림프구 증식
헤파린처리된 혈액 샘플은 5% 열 불활성화시킨 태아 송아지 혈청(FCS)을 함유한 행크스의 평형 염 용액(Hanks Balanced salt solution; BSS)을 사용하여 1:1로 희석되었고, 히파크-피콜(Hypaque-ficol) 구배로 층을 이루었다. 원심분리(실온에서 20분간 2000 rpm)하여 희석된 혈장을 회수하고 계면에서 림프구를 분리시켰다. 회수된 림프구를 행크스 BSS-5% FCS중에 3회 세척하고, RPMI 1640 배지, 10% FCS 및 1%의 시약들(비필수 아미노산, 나트륨 피루베이트, L-글루타민, 2-메르캅토에탄올 및 페니실린/스트렙토마이신)을 함유하는 RPMI 1640 완전 배지중에 재현탁시켰다. 회수된 림프구를 계수하고, 최종 농도 2 x 106/ml로 재현탁시켰다. 100 ㎕의 세포 현탁액 및 100 ㎕의 MBP(20 ㎍/웰), 100 ㎕의 공중합체-1(10 ㎍/웰) 또는 100 ㎕의 Con A(1 ㎍/웰)를 함유하는 배양물을 둥근 바닥 96 웰 마이크로타이터 플레이트 중에 설치했다. 이 배양물을 5% CO2중 37℃에서 6일동안 유지시켰다. 배양 5일째 되는 날에, 각 웰을 16 내지 18시간동안3H-티미딘 1μCi를 이용해서 펄스화했다. 6일째 되는 날에 배양물을 자동 세포 수확기로 수확하고, 액체 신틸레이션으로 계수했다. 자극 지수(stimulation index)는 하기와 같이 정해진다.

임상 결과
어떤 동물도 24일동안 임상적 증상을 나타내지 않았다. 플라시보-처리된 동물 18374는 25일째 되는 날에 발병했으며, EAE의 심각한 징후(점수= 5등급중 4+)에 기인하여 28일째 되는 날에 치사되었다. 20 mg의 고-용량 공중합체-1로 처리한 동물 18498은 60일의 관찰 기간동안 어떤 유의성 있는 임상적 증상도 나타내지 않았다. 저-용량의 공중합체-1로 처리한 동물 18497은 25일째 되는 날에 최소한의 증상을 나타내기 시작하였으나, 플라시보-처리한 동물 18374에 비해, 18497은 임상적 증상의 매우 느린 증가를 나타내었으며, 28일에서 33일째 되는 날 사이에 2 내지 3+으로 안정상태가 되었다. 이 시점에서, 동물 18497에게 20 mg의 캡술을 5회에 걸쳐 격일로 총 10일간 공급했다. 도9에서 도시된 것과 같이, 3일이내에 동물의 임상적 증상이 0으로 감소되었고, 공중합체-1을 공급한 두 마리의 동물 18497 및 18498은 조직학 연구를 위해 치사되는 60일째 되는 날까지 그 상태를 유지했다.
플로우 시토메트리(Flow Cytometry)
에픽스(Epics) C 유동 세포계수기(flow cytometer)를 사용하여, 각 원숭이에서 수집한 말초 혈액 림프구(PBL) 및 뇌척수액(CSF) 양자를 분석했다. 적혈구를 용해시키고, 남아 있는 백혈구(WBC)를 세척하여, 적절한 시약으로 표준 방법을 이용해 염색했다. 아래 표 3 및 표 4는 이 실시예의 3마리 동물로부터 얻은 염색한 PBL 및 CSF WBC로부터 얻은 데이터를 나타낸다.
염색한 CD4+ 세포의 수에 있어서는, 공중합체-1을 공급한 동물 18497 및 18498이 대조 동물 18374에 비해 약간 많았다. 또한, 대조 동물 18374 및 저용량 공중합체-1을 공급한 동물 18497 모두가 임상적 증상을 나타낸 때(+27 내지 +34일째) CD45RA-인 CD4+ 세포의 수는 두 동물에서 증가된 듯이 보였으나, 고용량 공중합체-1을 공급한 동물 18498에서는 거의 일정하게 유지되었다. 염색한 세포 CD8+DC45RA+의 수는 지속적으로 대조 동물 18374 및 저용량 공중합체-1을 공급한 동물 18497에서 감소했으나, 고용량 공중합체-1을 공급한 동물 18498에서는 약간 증가했다(새로운 CD4+ 세포의 생성을 의미함).
대조 동물 18374의 CSF 중에서 발견된 세포의 수는 20+ 일에서부터 동물이 치사한 28+일까지 지속적으로 증가했다. 분석 결과, 대부분의 CD4+ 세포는 CD45RA-였다. 동물 18497의 CSF로부터 수집된 세포수는 극소량이라서 계수 또는 염색이 불가능했다. 동물 18498에서 수집한 세포수는 +27일째 되는 날을 제외하고는 계수 또는 염색이 불가능했다. 그 때 수집한 CD4+ 세포는 주로 CD45RA-였다.

항원 특이성 T 세포 억제 인자 유도제(T cell suppressor factor inducer; T csfi)의 분석
본 실시예에서의 3마리 원숭이의 혈장내 MBP-특이성 Tsfi의 분석 결과는 아래 표 5 및 표 6에 나타낸다. 어떤 동물도 MBP에 의한 EAE 유도 후 +13일이 되는 날까지, MBP-특이성 Tsfi를 생성하지 않았다. 대조 동물 18374는 +20일이 되는 날까지 어떤 Tsfi도 생성하지 않았고, 이후에 생성된 레벨은 단지 백그라운드를 상회하는 수준이었다. 공중합체-1을 공급한 동물 18497 및 18498은 +13일되는 날로부터 종료되기 직전인 +41일까지 유의성 있는 레벨의 Tsfi를 일관되게 생성했다.
표 6은, Tsfi를 함유하지 않는 혈장 샘플은 항-TGF-베타 항체와 반응하지 않았다는 것을 나타낸다. 또한, 3C9 항체(항-Tsfi)와 MBP-결합을 나타내는 혈장은 항-TGF-베타 항체와 반응했다. 재조합 사람 TGF-베타는 항-TGF-베타와 반응하였지만, 3C9 항체와는 반응하지 않았다.

공급 계획, 질병 유도 및 추적 조사는 실질적으로 전술한 실시예 3에서와 동일했다.
결과
도10에 참고로 도시된 것과 같이, 대조 원숭이 18746은 +21일째 되는 날에 발병한 급성 질병이 진행되어 +23일 되는 날에 질병의 징후로 치사했다. 위(stomach)에서 개방되는 “갈라진”장용 코팅으로 처리된 동물 18586 및 18639는 질병으로부터 보호받지 못하고 질병의 징후가 있은 지 2-3일내에 치사했다. 장용 코팅된 캡슐중에 공중합체-1을 공급한 모든 원숭이는 질병으로부터 완전히 보호받았고, 그들이 조직학 연구를 위해 치사되는 60일째 되는 날까지 EAE의 어떤 징후도 발현하지 않았다.
Claims (15)
- 치료적 유효량의 공중합체-1의 섭취 또는 흡입을 포함하는, 다발성 경화증의치료 방법.
- 제1항에 있어서,상기 공중합체-1의 치료적 유효량이 0.1 내지 1000 mg/일인 방법.
- 제1항에 있어서,상기 공중합체-1이 경구적으로 투여되는 방법.
- 제1항에 있어서,상기 공중합체-1이 비강으로 투여되는 방법.
- 제3항 또는 제4항에 있어서,상기 공중합체-1이 흡입되는 방법.
- 제3항에 있어서,상기 공중합체-1이 장용-코팅된 형태로 경구 투여되는 방법.
- 섭취 또는 흡입을 통해 투여되는, 다발성 경화증을 치료하기 위한 약물을 제조하기 위한 공중합체-1의 용도.
- 제7항에 있어서,상기 약물은 0.1 내지 1000 mg/일의 공중합체-1을 포함하는 용도.
- 제7항 또는 제8항에 있어서,상기 약물이 경구 또는 비강 투여용으로 제제화되는 용도.
- 제9항에 있어서,상기 투여가 흡입에 의한 용도.
- 제9항에 있어서,상기 약물이 장용-코팅된 용도.
- 치료적 유효량의 공중합체-1 및 약학적 허용 담체를 포함하고 경구 또는 흡입 투여용으로 제제화되는, 다발성 경화증의 치료를 위한 약학적 조성물.
- 제12항에 있어서,상기 약학적 조성물이 고형, 액형, 에어로솔(aerosol) 또는 흡입가능한 분말인 약학적 조성물.
- 제13항에 있어서,상기 고형이 장용-코팅된 형태인 약학적 조성물.
- 제12항 내지 제14항 중 어느 한 항에 따른 약학적 조성물의 치료적 유효량을 피검체에게 투여하는 것을 포함하여, 피검체에 있어서 다발성 경화증을 치료하는 방법.
