Abstract

임플랜트는 평평한 상부 뼈-접촉면(3), 평평한 하부 뼈-접촉면(4) 및 두 개의 측면(5)을 포함하는 외부 표면(2)을 가지고, 중공실린더, 중공원추대, 끝이 잘린 중공 피라미드 혹은 끝이 잘린 중공 쐐기 형태의 평평한 형상의 중공 엘리먼트(1)로 구성된다. 상기 상부 및 하부 뼈-접촉면(3;4)은 중공엘리먼트(1)의 내부공간(6)을 향해 탄력적으로 압축가능하여, 상기 상부 및 하부 뼈-접촉면(3;4) 사이의 최대 거리(H)는 0.5 - 5.0mm 정도 변형가능하다.

Description

압축가능한 형상의 중공 엘리먼트를 구비한 추간 임플랜트
그런 추간 임플랜트는 척추몸체의 융해의 경우에 사용되고, 특히 척주(SPINAL COLUMN)의 요추부의 지역에 사용된다. 추간 공간마다 하나에서 두 개의 임플랜트가 사용된다.
종래 기술로부터 다음과 같은 문제점을 보이는 다른 타입의 그런 추간 임플랜트가 이미 있다:
- 추간 공간에 임플랜트를 삽입하기 위해 각각의 척추가 적절한 도구에 의해 확장되어야 한다; 그리고
- 상기 임플랜트는 각각의 척추의 엔드 플레이트에서 가라앉게 될 위험이 있다.
본 발명은 청구범위 1항에 정의된 것에 따른 추간 임플랜트에 관한 것이다.
도 1은 속이 차있지 않은 결합립을 가진 압축가능한 상태의 본 발명에 의해 임플랜트의 사시도.
도 2는 결합된 결합립을 가진 압축되지 않은 상태에서 도 1에 따른 임플랜트의 단면도.
여기서 본 발명은 구제책을 제공한다. 본 발명은 확장도구 없이 조절가능한 방식으로 치워진 추간 공간으로 삽입가능한 추간 임플랜트를 제조하는 과업에 기반을 둔다.
이 문제점의 해법으로, 기재된 것에 묘사된 구성은 독립항 1 항의 특징부의 특징에 의해 더 형성된다.
평평하게 된 중공 엘리먼트는 중공 실린더, 끝이 잘린 중공 원추, 끝이 잘린 중공 피라미드 혹은 끝이 잘린 중공 쐐기의 형태를 가질 수 있다.
치수, 결합구조 및 재질은 선택될 수 있는데, 상부 및 하부 뼈접촉면 사이의 최대 길이 H가 0.5 - 5.0mm 바람직하게는 1 - 3mm정도 줄어들 수 있다.
상부 및 하부 뼈접촉면 상에 12뉴턴의 압력의 힘이 작용되는 경우에, 최대길이 H는 15% 에서 30% 바람직하게는 20%에서 25% 정도 줄어들 수 있게 된다. 상기 임플랜트의 압축성은 확장도구를 적용함이 없이 그것의 삽입을 허용한다.
게다가 장점은 삽입을 위해 필요로 하는 힘의 최소 소비에 도달할 수 있다는 것과 압축된 임플랜트의 조절가능한 삽입이 가능하다는 것이다. 최소의 침해와 개복 수술 기술이 거기에 적용될 수 있다.
임플랜트의 큰지지(bearing) 면적은 그것의 엔드플레이트로의 가라앉음을 방지한다.
본 발명의 바람직한 실시예가 그점에서 구성되는데, 중공 엘리먼트의 내부 공간에서 결합립이 체결수단에 결합되게 제공된다. 결합된 결합립(lip)의 경우에 중공 엘리먼트의 압축성이 보통의 단단한 임플랜트가 되도록 다시 중화된다.
뼈 파편 혹은 뼈 대체물질은 개방된 중공 엘리먼트의 전방 그리고 후방으로부터 내부의 공간으로 쉽게 채워질 수 있고 상기 임플랜트는 아주 적은 작동단계로서 안전하게 장착될 수 있다.
뼈의 신속한 성장을 촉진하기 위해 상기 임플랜트는 바람직하게는 길이방향 통공의 형태의 구멍을 적절하게 구비한다. 상기 길이 방향통공은 X선이 투과할 수 없는 재질의 경우에 X선 이미지에 의해 뼈의 안에서 자람을 제어할 수 있다.
임플랜트의 위치의 안정성을 개선하기 위해 상부 및/혹은 하부 뼈 접촉면이 바람직하게는 길이방향 홈의 형태의 3차원 구조로 제공된다.
종래 기술과 비교하면 본 발명에 따른 임플랜트의 다음과 같은 장점이 생긴다.:
-미끄러짐에 대한 안전성;
-X선에 대한 개선된 투과성; 및
-임플랜트의 증대된 탄성.
본 발명 및 본 발명의 실시예는 본 발명 실시예를 부분적인 도식적 표현에 의해 다음에서 더 상세하게 논의 된다.
도 1 및 도 2에 도시된 추간 임플랜트는 그 외부 표면(2)이 평평한 상부 뼈 접촉면(3), 평평한 하부 뼈접촉면(4)과 두 개의 측면(5)으로 구성된 중공 실린더 형태인 중공 엘리먼트(1)로 본질적으로 구성된다.
상기 임플랜트는 적절한 금속(즉, 티탄(Titan))으로 제조될 수 있으나, 특별히 폴리에틸렌과 같은 인체가 견딜 수 있는 합성물질로 만들어 질 수 있다.
상기 상부 및 하부 뼈-접촉면(3,4)은 중공 엘리먼트(1)의 내부 공간(6)을 향해 탄력적으로 압축될 수 있어, 상기 상부 및 하부 뼈-접촉면(3,4) 사이의 최대 거리(H)는, 전형적을 2mm 까지 줄어들 수 있다. 이것은 상기 임플랜트의 추간 공간에 대한 용이한 삽입을 가능하게 한다.
상기 측면(5)은 길이방향 홀의 형상의 홈을 구비하는데, 합성물질을 사용할 때에는 생략될 수 있다. 상기 상부 뼈-접촉면(3) 및/혹은 상기 하부 뼈-접촉면(4)은 중앙홈(9)뿐 아니라 구멍(8)을 구비한다.
중공 엘리먼트(1)의 내부공간(6)에는 결합립(10)이 제공되는데, 이는 체결수단(11)에 결합가능하다.
다음에서 의료적 적용이 상세하게 기술된다. 도 1에 도시된 상기 중공 엘리먼트(1)는 적절한 압축도구에 의해 파지되는데, 압력의 힘으로, 대표적으로 12 뉴턴이 상기 중공 엘리먼트(1)를 압축하게 상기 상부 및 하부 뼈-접촉면(3,4)에 가해질 수 있다. 이 상태에서 상기 임플랜트는 분리 도구 없이 최대한 가능한 빈 추간공간으로 삽입되는데, 길이방향 홈의 형상인 상기 3차원 구조(7)가 임플랜트의 진행을 돕고 또 발생할 수 있는 미끄러짐에 대한 보호를 제공한다.
압축툴은 제거되고 결합립(10)이 수직으로 세워지고 상기 중공 엘리먼트(1)가 더 이상 압축되지 않게 상기 체결수단(11)에 결합된다. 상기 임플랜트는 이제 뼈 파편 혹은 뼈 대체물질로 채워질 수 있다.

Claims (10)

  1. 평평한 상부 뼈-접촉면(3), 평평한 하부 뼈-접촉면(4) 및 두 개의 측면(5)을 포함하는 외부 표면(2)을 가지고, 중공실린더, 끝이 잘린 중공원추, 끝이 잘린 중공 피라미드 혹은 끝이 잘린 중공 쐐기 형태의 평평한 형상의 중공 엘리먼트(1)로 구성되는 추간 임플랜트에 있어서,
    상기 상부 및 하부 뼈-접촉면(3;4)은 중공엘리먼트(1)의 내부공간(6)을 향해 탄력적으로 압축가능하여, 상기 상부 및 하부 뼈-접촉면(3;4) 사이의 최대 거리(H)는 0.5 - 5.0mm 정도 변형가능함을 특징으로 하는 추간 임플랜트.
  2. 제 1 항에 있어서, 상부 및 하부 뼈-접촉면(3;4) 사이의 최대 거리(H)는 1 - 3mm 정도 변형가능함을 특징으로 하는 추간 임플랜트.
  3. 제 1 항 또는 제 2 항에 있어서, 상기 상부 및 하부 뼈-접촉면(3;4)에 12뉴턴의 압력의 힘이 작용되는 경우에 상기 최대 거리(H)는 15%에서 30% 정도, 바람직하게는 20%에서 25% 정도 변형가능함을 특징으로 하는 추간 임플랜트.
  4. 제 1 항 내지 제 3 항중 어느 한 항에 있어서, 상기 중공 엘리먼트(1)의 내부 공간(6) 내에 결합립(10)이 제공되는데 중공 엘리먼트(1)의 압축성의 진압하에서 체결수단(11)에 체결가능함을 특징으로 하는 추간 임플랜트.
  5. 제 1 항 내지 제 4 항중 어느 한 항에 있어서, 상기 측면(5)은 연장된 통공(12)을 구비함을 특징으로 하는 추간 임플랜트.
  6. 제 1 항 내지 제 5 항중 어느 한 항에 있어서, 상기 상부 뼈-접촉면(3) 및/혹은 상기 하부 뼈-접촉면(4)은 바람직하게는 길이방향 홈 형상의 3차원 구조(7)를 구비함을 특징으로 하는 추간 임플랜트.
  7. 제 1 항 내지 제 6 항중 어느 한 항에 있어서, 상기 상부 뼈-접촉면(3) 및/혹은 상기 하부 뼈-접촉면(4)은 통공(8)을 구비함을 특징으로 하는 추간 임플랜트.
  8. 제 1 항 내지 제 7 항중 어느 한 항에 있어서, 상기 상부 뼈-접촉면(3) 및/혹은 상기 하부 뼈-접촉면(4)은 하나 혹은 몇 개의 중앙 홈(9)을 제공함을 특징으로 하는 추간 임플랜트.
  9. 제 1 항 내지 제 8 항중 어느 한 항에 있어서, 생친화적(Biocompatible) 합성물질, 바람직하게는 폴리에틸렌으로 제조됨을 특징으로 하는 추간 임플랜트.
  10. 제 1 항 내지 제 9 항중 어느 한 항에 있어서, 트리칼슘포스패이트(tricalciumphosphat)(TCP)의 수산화인회석(hydroxylapatit)으로 코팅됨을 특징으로 하는 추간 임플랜트.
