Abstract

본 발명의 내시경을 겸하는 케이지 핸들은 렌즈와 광케이블을 구비하여 케이지 전방의 영상을 정확하고 선명하게 확인하면서 수술할 수 있다. 종래와 같은 별도의 내시경이나 엑스레이를 이용하지 않으므로 수술이 간편하고 효율적이며 경제적이다.

Description

골유합술용 케이지 수술 시스템{Interbody Fusion cage operation system}
본 발명은 골유합술용 케이지 수술 시스템에 관한 것이다. 더욱 구체적으로 본 발명은 케이지 핸들에 내시경 기능을 구현하여 척추에 케이지를 수술할 때 삽입 경로와 주변 환부를 명료히 확인할 수 있도록 한 골유합술용 케이지 수술 시스템에 관한 것이다.
척추의 병든 디스크를 제거한 자리 또는 척추 분리증인 경우 이에 케이지를 끼워 척추뼈를 고정시키는 수술이‘척추체간 골유합술’이다. 케이지의 몸통에는 나사못과 같이 나선형 돌기가 있고, 작은 구멍들이 뚫려 있다. 케이지를 만드는 재료는 티타늄 또는 레티놀이고 가공 및 3D printer로 만든다. 케이지를 끼울 때는 디스크를 중심으로 위아래에 있는 척추 뼈들을 조금 갈아낸 다음 케이지에 본칩을 삽입하고 나사못처럼 돌려서 핸들에 고정 하거나 전용핸들에 손쉽게 부착 후 단단하게 끼운다. 처음에는 케이지가 위아래 척추뼈들을 고정시키지만 3~4개월 정도 지나면 케이지 속의 뼈들이 자라서 자연스럽게 서로 붙게 된다.
골유합술 시행 시 기존에는 등을 개복하였지만, 최근에는 수술 도구를 사용할 수 있는 일정한 공간을 절개한 후 이 공간을 통해 케이지를 삽입한다. 그런데, 케이지를 넣을 때 경로를 내시경으로 보거나 확인할 수 없어 수술자의 느낌에 의존하여 케이지를 넣고 있다.
도 6(a)는 케이지를 삽입하는데 이용되는 케이지 핸들(H’)을 도시하고 있다. 도 6(b)는 케이지 핸들(H’)의 선단을 확대한 것이다. 도 6(c)는 케이지 핸들(H’)을 이용하여 디스크에 케이지(C’)를 위치시킨 것을 보인 도면이다.
종래 케이지 핸들(H’)을 이용하는 경우, 케이지를 환부에 삽입하면서 엑스레이 화면을 보면서 수술자가 삽입하거나 허리 쪽에 하나의 구멍을 만들어 그쪽으로 내시경을 삽입하여 케이지를 관찰하는 방법을 이용한다. 최초에 케이지를 삽입 시 척추 신경이 있는지 확인이 불가능하여 매우 주의가 요구되고, 엑스 레이 영상에만 의존하는 문제가 있다.
도 7의 단면도의 내시경(1‘)은 출원인의 선특허 제10-1681451호에 개시한 것이다. 수술 도구가 통과하는 워킹채널(130’)이 튜브(100‘) 대부분의 공간을 차지하고, 워킹채널(130’)의 하부에 카메라 또는 렌즈(110‘)와 세정채널(120’)이 형성되고, 남은 공간에 광원으로 기능하는 광섬유(140‘)를 배치하고 있다.
본 발명은 케이지 핸들에 내시경의 기능을 결합하여 케이지 핸들의 구조를 획기적으로 개선한 것이다. 또는 다른 관점에서는, 케이지를 바로 부착하여 사용할 수 있는 새로운 내시경에 관한 발명이라고도 할 수 있다.
따라서 본 발명은 척추 골유합술 수술 시 케이지의 앞쪽에 소형의 내시경 카메라를 장착하여 케이지 삽입 시 어떠한 신경 또는 장애물이 있는지 시술자가 보면서 환부에 삽입할 수 있는 전용 케이지 핸들 또는 내시경과 이를 구비한 시스템을 제공하는 것을 목적으로 한다.
상술한 목적을 달성하기 위하여 본 발명은, 척추 골유합술용 케이지 핸들로서, 상기 케이지 핸들은 중앙에 회전 가능한 회전노브를 포함하며, 회전노브의 전방에는 튜브가 전방으로 길게 연장되고, 튜브의 선단에는 케이지와 결합하기 위한 결합부가 제공되며, 결합부의 외면에는 케이지와 결합하기 위한 나사산이 형성되고, 케이지 핸들은 케이블을 통하여 카메라 커넥터 및 LED 커넥터 또는 광원장치 광케이블에 접속되며, 튜브의 적어도 상기 결합부의 내부 중앙에는 렌즈가 설치되고, 렌즈의 외부를 둘러 광섬유가 배치되며, 광섬유를 둘러 바깥에서 케이지가 탈부착 가능하게 장착되는, 척추 골유합술용 케이지 핸들을 제공한다.
케이지는 중앙 상면에 본칩을 장착하기 위한 관통홀이 형성되며, 전후를 관통하여 통로가 형성되고 이 통로에 결합부가 결합될 수 있다.
케이지는 오픈 타입으로, 전방 중심을 향하여 테이퍼진 케이지선단부, 케이지선단부와 연속하여 연장되며 외면에 나사산이 형성된 케이지체결부 및 케이지체결부에 연속하여 형성되며 나사산이 없는 외면을 가진 케이지슬리브로 이루어지며, 이들 각각에 대응하도록 케이지 핸들의 결합부는, 전방 중심을 향하여 테이퍼 진 선단부, 선단부와 연속하여 연장되며 외면에 나사산이 형성된 체결부 및 체결부에 연속하여 형성되며 나사산이 없는 외면을 가진 슬리브로 이루어지며, 케이지 핸들의 선단부를 통로로 밀어 넣고 케이지 핸들을 회전시켜 점점 전진 이동시켜 그 나사산을 상기 케이지체결부의 나사산과 체결할 수 있다.
또한, 본 발명은 케이지 핸들과 케이블을 통하여 연결되며, 카메라 커넥터 및 LED 커넥터를 구비한 콘솔 및 케이지 핸들로부터 전송되는 영상을 보기 위한 디스플레이를 포함하는 척추 골유합술용 케이지 수술 시스템을 제공한다.
본 발명에 의하면, 척추의 수술 시 환부를 정확하고 선명하게 볼 수 있으며, 케이지의 탈부착이 간편하고 경제적인 척추 골유합술용 케이지 수술 시스템을 제공한다는 효과를 발휘한다.

본 발명의 목적 및 효과, 그리고 그것들을 달성하기 위한 기술적 구성들은 첨부되는 도면과 함께 상세하게 뒤에 설명되는 실시예들을 참조하면 명확해질 것이다. 본 발명을 설명함에 있어서 공지 기능 또는 구성에 대한 구체적인 설명이 본 발명의 요지를 불필요하게 흐를 수 있다고 판단되는 경우에는 그 상세한 설명을 생략할 것이다.
명세서 전체에서, 어떤 부분이 어떤 구성요소를 "포함"한다고 할 때, 이는 특별히 반대되는 기재가 없는 한 다른 구성요소를 제외하는 것이 아니라 다른 구성요소를 더 포함할 수 있는 것을 의미한다. 한편, 본 발명의 실시예에 있어서, 각 구성요소들, 기능 블록들 또는 수단들은 하나 또는 그 이상의 하부 구성 요소로 구성될 수 있다.
도 1은 본 발명의 골유합술용 케이지 수술 시스템(1)의 전체 구성도이다.
내시경 기능을 겸하는 케이지 핸들(2)이 케이블(22)을 통하여 콘솔(4)에 연결된다. 콘솔(4)에는 카메라 커넥터(42)와 LED 커넥터(44)가 설치되며, 콘솔(4)은 다시 케이블(62)을 통하여 수술 모니터링을 위한 디스플레이(6)에 연결된다. 케이지 핸들(2)은 LED 커넥터(44) 대신 광원장치의 광케이블에 연결될 수 있다.
도 2는 본 발명의 케이지 핸들(2)의 전체 구성도이다.
중앙의 컨트롤 노브는 회전노브(26)와, 회전노브(26)에 바로 인접한 고정노브(24)로 이루어진다. 회전노브(26)에서는 실린더형의 튜브(28)가 좌측, 즉 전방으로 길게 연장된다. 튜브(28)의 선단에는 케이지(C)와 결합하기 위한 결합부(30)가 제공된다. 결합부(30)의 외면에는 케이지(C)와 결합하기 위한 나사산(32)이 형성된다. 고정노브(24)의 하부로부터는 케이블(22)이 연장된다. 케이블(22)의 경로에는 손으로 케이지 핸들(2)을 잡기 위한 그립부(22c)가 설치되며, 케이블(22)은 그립부(22c)를 통과하여 카메라 커넥터(42)에 접속되는 카메라 케이블(22a)과 LED 커넥터(44)에 접속되는 LED케이블(22b)로 분기된다.
확대 단면도에 도시한 것과 같이, 결합부(30)의 내부 중앙에는 렌즈(34)가 설치되고, 렌즈(34)의 외부를 둘러 광섬유(36)가 배치된다. 본 발명의 케이지 핸들(2)은 케이지(C)를 척추 내에 장착하기 위한 도구이므로, 일반적인 내시경이 구비하는 워킹채널과 세정 채널은 필요하지 않다. 따라서 렌즈(34)가 정 중앙에 그리고 그 주위를 둘러 비교적 빈틈없이 광섬유(36)를 배치하여 수술 시 환자의 신체 내부를 더욱 크고 선명한 화면으로 모니터링 할 수 있는 장점이 있다.
본 발명에서는 “케이지 핸들”로 지칭하지만, 다른 관점에서는 케이지의 탈부착이 가능한 골유합술 전용의 내시경이라고도 할 수 있다.
도 3은 본 발명의 케이지 핸들(2)의 기능을 설명하기 위한 도면이다.
케이지(C)는 중앙 상면의 사각형 관통홀(Ca)을 통하여 인체의 뼈와 부착되기 위한 본칩(bone chip)을 장착하며, 전후를 관통하여 통로(Cb)가 형성되고 이 통로(Cb)에 결합부(30)가 결합된다. 회전노브(26)는 좌우로 회전가능하며 예를 들어 우회전하면 결합부(30)가 케이지(C) 내부의 통로(Cb)에 점점 진입하여 두 부재가 결합되며, 좌회전하면 결합부(30)가 케이지(C) 내부의 통로(Cb)에서 점점 벗어나서 두 부재가 분리된다.
도 4는 케이지(C)가 장착된 것을 도시한 결합부(30)의 단면도이다. 케이지(C)는 광섬유(36)를 둘러 가장 바깥에 장착된다. 렌즈(34)와 광섬유(36)가 최대한 케이지(C)에 인접하여 그 내부 중앙에 큰 면적을 차지하도록 배치되고 두 부재가 케이지(C)의 내부를 통과하여 전면에 배치되므로, 어둡고 긴 신체 내부를 크고 선명하게 조명하면서 촬영할 수 있다.
도 5는 본 발명의 케이지 핸들(2)의 결합부(30)와 이에 장착되는 케이지(C)의 일례를 구체적으로 설명하기 위한 도면이다.
케이지(C)는 오픈 타입으로, 전방 중심을 향하여 테이퍼진 케이지선단부(C1), 케이지선단부(C1)와 연속하여 연장되며 외면에 나사산이 형성된 케이지체결부(C2) 및 케이지체결부(C2)에 연속하여 형성되며 나사산이 없는 외면(54)을 가진 케이지슬리브(C3)로 이루어진다. 이들 각각에 대응하도록 케이지 핸들(2)의 결합부(30)는, 전방 중심을 향하여 테이퍼 진 선단부(30a), 선단부(30a)와 연속하여 연장되며 외면에 나사산(32)이 형성된 체결부(30b) 및 체결부(30b)에 연속하여 형성되며 나사산이 없는 외면(64)을 가진 슬리브(30c)로 이루어진다. 두 부재의 결합시에는 케이지 핸들(2)의 선단부(30a)를 통로(Cb)로 밀어 넣고 케이지 핸들(2)을 예를 들어 우회전하여 점점 전진 이동시켜 나사산(32)을 케이지체결부(C2)의 나사산과 완전히 잠금 결합한다. 그러면 렌즈(34)와 광섬유(36)가 케이지선단부(C1)에 수용되면서 전면이 노출되어 조명 및 촬영 준비를 마치게 된다. 튜브(20)의 내면과 외면(64) 사이의 갭에는 케이지슬리브(C3)가 삽입되어 케이지(C)가 케이지핸들(2)에 고정된다.
결합부(30)는 튜브(28)와 일체로 성형할 수도 있고, 튜브(28)와 별도로 제작되어 공급될 수도 있다. 케이지(C)의 타입이 다르면 이에 맞도록 결합부(30)의 형상도 변경될 수 있다.
본 발명의 케이지 수술 시스템(1)은 허리, 등뿐만 아니라 목 디스크의 관절에도 두루 적용된다.

Claims (4)

  1. 척추 골유합술용 케이지 핸들로서, 상기 케이지 핸들은 중앙에 회전 가능한 회전노브를 포함하며, 회전노브의 전방에는 튜브가 전방으로 길게 연장되고, 튜브의 선단에는 케이지와 결합하기 위한 결합부가 제공되며, 결합부의 외면에는 케이지와 결합하기 위한 나사산이 형성되고, 케이지 핸들은 케이블을 통하여 카메라 커넥터와 LED 커넥터에 접속되거나 또는, 카메라 커넥터와 광원장치의 광케이블에 접속되며, 튜브의 적어도 상기 결합부의 내부 중앙에는 렌즈가 설치되고, 렌즈의 외부를 둘러 광섬유가 배치되며, 광섬유를 둘러 바깥에서 케이지가 장착되는, 척추 골유합술용 케이지 핸들.
  2. 제 1항에 있어서,
    케이지는 전후를 관통하여 통로가 형성되고 이 통로에 결합부가 결합되는, 척추 골유합술용 케이지 핸들.
  3. 제 2항에 있어서,
    케이지는 오픈 타입으로, 전방 중심을 향하여 테이퍼진 케이지선단부, 케이지선단부와 연속하여 연장되며 외면에 나사산이 형성된 케이지체결부 및 케이지체결부에 연속하여 형성되며 나사산이 없는 외면을 가진 케이지슬리브로 이루어지며, 이들 각각에 대응하도록 케이지 핸들의 결합부는, 전방 중심을 향하여 테이퍼 진 선단부, 선단부와 연속하여 연장되며 외면에 나사산이 형성된 체결부 및 체결부에 연속하여 형성되며 나사산이 없는 외면을 가진 슬리브로 이루어지며, 케이지 핸들의 선단부를 통로로 밀어 넣고 케이지 핸들을 회전시켜 점점 전진 이동시켜 그 나사산을 상기 케이지체결부의 나사산과 체결하도록 한, 척추 골유합술용 케이지 핸들.
  4. 제 1항 내지 제 3항 중의 어느 한 항의 케이지 핸들;
    상기 케이지 핸들과 케이블을 통하여 연결되며, 카메라 커넥터 및 LED 커넥터를 구비한 콘솔; 및
    케이지 핸들로부터 전송되는 영상을 보기 위한 디스플레이를 포함하는 척추 골유합술용 케이지 수술 시스템.
