Description
The present invention relates to a sustained-release combination formulation containing a statin-based drug and cilostazol, an HMG-CoA reductase inhibitor, as an active ingredient.
Cilostazol inhibits the primary aggregation of platelets induced by ADP, epinephrine, etc. in platelets isolated from mice, rats, rabbits, dogs and humans, and dissociates platelet aggregation. In addition, when administered orally to beagle dogs, it inhibits the aggregation of platelets induced by ADP and collagen, and when administered orally to patients with chronic arterial obstruction (Berger's disease, obstructive arteriosclerosis, diabetic peripheral angiopathy, etc.) It inhibits the aggregation of platelets induced by ADP, collagen, arachidonic acid, and epinephrine in the isolated platelets. The platelet aggregation inhibitory effect of cilostazol is rapidly expressed after administration, and the effect is maintained even with repeated administration. When the administration is stopped, the inhibited platelet aggregation capacity is restored to the pre-administration level with a decrease in plasma concentration, and rebound phenomenon (hyperaggregation) does not appear.
The mechanism of action of cilostazol is as follows. It inhibits the release of serotonin from platelets in rabbits, but does not affect the entry of serotonin and adenosine into platelets. It does not affect the arachidonic acid metabolism of platelets and inhibits platelet aggregation by thromboxane A2 (TXA2). This is due to the inhibition of cAMP-PDE (cyclic AMP phosphodiesterase) activity of platelets and vascular smooth muscle, and eventually exerts antiplatelet and vasodilating effects.
Existing cilostazol formulations are taken twice a day and have the disadvantage of poor patient compliance. In the case of cilostazol immediate-release formulations, rapid and uneven drug release when administered orally results in a rapid increase in blood concentration. It is known that it can cause side effects such as headache, weight loss, and tachycardia (see Am J Cardiol 2001;87(suppl):28D-33D and US Patent Publication No. 2002/0058066). In addition, since cilostazol is not only poorly soluble but also has a lower absorption rate toward the lower part of the small intestine, there is a concern that the overall bioavailability may decrease in the case of a conventional release-controlled formulation. PCT Patent Publication No. WO 2000/57881 is a fine powder form Disclosed is a method of increasing the absorption rate of cilostazol in the lower intestine by using a formulation in a form in which the sol is dispersed and/or dissolved together with a dispersing agent and/or a solubilizing agent.
The present invention improves the shortcomings of the conventional cilostazol sustained-release formulation with a large dissolution deviation, and significantly increases the productivity by manufacturing it in a tablet form, and further improves the bioavailability by adding an HMG-CoA reductase inhibitor as an active ingredient. will be.
The present invention is an HMG-CoA reductase inhibitor mixing unit containing an HMG-CoA reductase inhibitor or a pharmaceutically acceptable salt thereof as an active ingredient; And it provides a sustained-release combination formulation comprising a cilostazol mixture containing cilostazol or a pharmaceutically acceptable salt thereof as an active ingredient. On the other hand, the cilostazol mixing part includes a release controlling agent to control the amount of the cilostazol active ingredient eluted in the body, and the content of the release controlling agent is 3 to 9 parts by weight based on 100 parts by weight of the cilostazol active ingredient. desirable.
The pharmaceutical composition according to the present invention is a combination formulation containing cilostazol and an HMG-CoA reductase inhibitor, and has excellent bioavailability, and the content of the release regulator in the cilostazol mixture containing cilostazol is adjusted to 1 day 1 It has sufficient sustained-release properties with only one dose, and exhibits excellent effects of reducing side effects of cilostazol.
In this case, when the content of the release controlling agent is less than 3 parts by weight based on 100 parts by weight of the active ingredient of cilostazol, the initial release amount of cilostazol is excessive, so that sufficient sustained release may not appear, and side effects such as headache may occur. On the other hand, when the content of the release controlling agent exceeds 9 parts by weight based on 100 parts by weight of the cilostazol active ingredient, the dissolution of the cilostazol active ingredient is excessively delayed, making it difficult to achieve a sufficient pharmacological effect.
The HMG-CoA reductase inhibitor used in the present invention may be used by selecting one or two or more from the group consisting of rosuvastatin, simvastatin, atorvastatin, pitavastatin, pravastatin, fluvastatin, and lovastatin. , The content of the active ingredient in the HMG-CoA reductase inhibitor mixture portion is preferably 3 to 40 mg, and the content of the active ingredient in the cilostazol mixture portion is preferably 160 to 240 mg.
If the active ingredient content of the HMG-CoA reductase inhibitor mixture is less than 3 mg, or the active ingredient content of the cilostazol mixture is less than 160 mg, sufficient pharmacological activity does not appear, and the active ingredient content of the HMG-CoA reductase inhibitor mixture is 40 mg. When the content of the active ingredient in the cilostazol mixture exceeds 240 mg, the blood concentration of the active ingredient is excessively increased, and side effects may occur.
Meanwhile, in the cilostazol mixing part of the sustained-release composite formulation according to the present invention, hypromellose and carbomer may be mixed as a release controlling agent, and in this case, hypromellose and carbomer having a viscosity of 75,000 to 140,000 cps may be used. It is preferable to mix and use in a weight ratio of 1:1 to 2.5:1. When the mixing ratio of the release controlling agent is outside the above range, the stability of the sustained-release matrix may be deteriorated.
In addition, the cilostazol mixing unit may include an immediate release granule containing cilostazol and a disintegrant and a binder as active ingredients; And a sustained-release granule comprising cilostazol and a release controlling agent and a binder as an active ingredient, and the active ingredient content of the sustained-release granule is 50 to 90% by weight, more preferably, based on the active ingredient content of the immediate-release granule. It is preferably 60 to 80% by weight.
As described above, if immediate-release granules and sustained-release granules containing cilostazol containing cilostazol are prepared, and then mixed and used, the rapid pharmacological activity of the immediate-release granules and the sustained effect of the sustained-release granules are improved. It can reduce side effects even though all are expressed. In this case, the ratio of the active ingredient content of the immediate-release portion and the sustained-release portion is very important. If the active ingredient content of the immediate-release portion is excessive, side effects may occur due to excessively rapid dissolution. On the other hand, in the case of the Western Ministry, the content of the active ingredient must be adjusted while taking into account the dissolution characteristics of the release controlling agent so that the pharmacological effect can last for a long time. After repeated research, the applicant was able to find the optimal content ratio of the active ingredients in the immediate release portion and the sustained release portion as described above, and in order to uniformly control the dissolution rate, the active ingredient in the mixing portion containing each of the above active ingredients The types and contents of the disintegrant, binder, and release control agent were optimized in consideration of the content. Through this, it was possible to achieve the effect of reducing side effects while maintaining a long-term effect along with rapid pharmacological activity.
As the disintegrant, one or two or more selected from the group consisting of calcium carboxymethylcellulose, sodium carboxymethylcellulose, 1-hydroxypropylcellulose, and crosslinked polyvinylpyrrolidone may be used, and as the binder Hydroxypropyl cellulose, hydroxy cellulose, hydroxypropyl methyl cellulose, hydroxypropyl starch, polyvinyl alcohol, hydroxy methyl cellulose, polyvinyl pyrrolidone selected from the group consisting of one or two or more can be used. have. In addition, as the release controlling agent, hypromellose and carbomer may be used alone, or they may be used in combination. However, as described above, when hypromellose and carbomer were mixed and used, the most desirable result was obtained.
The active ingredient contained in the cilostazol mixing part of the sustained-release combination formulation according to the present invention is obtained from the 0.4% sodium lauryl sulfate (SLS) eluate according to the second method (paddle method) of the Korean Pharmacopoeia dissolution test method, after 30 minutes. 20% or less of the total weight of the active ingredient of the cilostazol mixture portion, after 90 minutes, 20 to 50% of the total weight of the active ingredient of the cilostazol mixture portion, after 300 minutes, elution of 80% or more of the total weight of the active ingredient of the cilostazol mixture portion It has a characteristic that satisfies the dissolution profile, which shows rapid pharmacological activity, reduces side effects caused by excessive dissolution, and completes a formulation in which most of the total active ingredients are dissolved after 300 minutes.
In addition, the immediate-release granules and the sustained-release granules according to the present invention use those having a diameter of 350 to 450 μm, which affects the dissolution properties, so when out of the above range, the dissolution may be too rapid or sufficient pharmacological effects may not appear. I can.
On the other hand, the sustained-release combination formulation of the present invention is preferably prepared with a diameter of 5 to 10 mm and a total weight of 350 to 550 mg, and if the diameter is less than 5 mm, there is a possibility that the formulation may collapse upon tableting, and if it exceeds 10 mm, the patient Convenience of taking medication may decrease. In addition, if the total weight is less than 350mg, the stability may be deteriorated due to insufficient addition of excipients, and if the total weight is more than 550mg, the formulation may become too large and the patient's convenience in medication may be reduced.
The sustained-release composite formulation of the present invention preferably has a hardness of 10 to 30kg/cm2, and if the hardness is less than 10kg/cm2, the formulation may collapse or decrease stability during tableting, and if it exceeds 30kg/cm2, it is more than intended. The problem of further delay in dissolution may occur.

Experiment 1> Measurement of dissolution rate of cilostazol active ingredient
According to the eleventh revised dissolution test method of the Korean Pharmacopoeia, the dissolution rates over time were measured for each of the Examples and Comparative Examples, and are shown in Table 2 below. A 0.4% SLS aqueous solution was used as the eluate, and the elution method was performed using the paddle method, the eluate was 900ml, the stirring speed was 75rpm, and the elution temperature was 37±0.5℃.
고찰Review
In Examples 1 to 3, after 30 minutes of administration of cilostazol to the eluate, less than 20% of the active ingredient is eluted, and thereafter, a certain amount of the drug is continuously eluted. On the other hand, in the case of Comparative Example 1, since the active ingredient dissolves too rapidly due to poor sustained release, there is a concern that side effects may occur due to a rapid rise in blood concentration. In the case of Comparative Example 2, the elution is too slow, so that sufficient pharmacological activity is not expressed during stay in the body, and the effect may be reduced.
In addition, in the case of Examples 1 to 3, about 20 to 50% of the active ingredient is eluted after 90 minutes, and 80% or more of the active ingredient is eluted after 300 minutes. In this case, the environment is different from the eluate containing a surfactant. When administered orally into the body, a desirable dissolution pattern is expected for a long time.
Experiment 2> Measurement of dissolution rate of statin active ingredient
Since the pharmaceutical composition according to the present invention is a sustained-release combination formulation containing two different active ingredients, there is a possibility that the effective ingredient dissolution rate may change depending on the combination characteristics of the active ingredients having different properties or the characteristics of the formulation.
Accordingly, the applicant of the present invention selected rosuvastatin, simvastatin, and atorvastatin among statin drugs that can be used as controls in order to confirm whether the formulation characteristics of the combination formulation with cilostazol or sustained release affect the elution of the statin component. A single tablet according to Comparative Examples 3 to 5 containing was prepared. On the other hand, Examples 4 and 5 having the same composition as Example 2 were prepared except for the type of statin, and the elution patterns of statin components of the composite formulations of Examples 2, 4, and 5 and the single tablets of Comparative Examples 3 to 5 were Compared.

As the eluent, pH 6.6, 7.0 aqueous buffer and water were used, and the elution method was performed using the paddle method, the eluate was 900ml, the stirring speed was 50rpm, and the elution temperature was 37±0.5℃.
고찰Review
As shown in Tables 5 to 7 above, the dissolution rates of the sustained-release combination formulation and the statin single tablet according to the present invention are very similar and thus exhibit substantially the same dissolution rate. Accordingly, it can be seen that the sustained-release combination preparation according to the present invention has no effect on the dissolution of the contained statin component in the body despite the difference in formulation and composition from the control group containing a single statin component, and thus the effect is rapidly expressed.
As a result, when the dissolution rate of cilostazol shown in Table 2 and the dissolution rate of rosuvastatin shown in Tables 5 to 7 are summarized, the sustained-release combination formulation according to the present invention has an optimal dissolution profile of both different active ingredients. It is suitable as a sustained-release combination formulation taken once a day.

고찰Review
In the sustained-release composite formulation of the present invention, all active ingredients exhibit excellent initial dissolution rate, whereas in the case of a simple mixed single tablet, there is a problem that the statin active ingredient is hardly eluted at the beginning due to the viscosity of the sustained-release matrix. Accordingly, it can be seen that the sustained-release combination formulation of the present invention composed of a double layer containing statin and cilostazol in different layers, respectively, exhibits excellent initial dissolution rates of the active ingredients contained therein, and is an excellent formulation having sustained-release properties.
Experiment 4> Experiment of dissolution deviation of cilostazol active ingredient from control drug
On the other hand, in order to confirm the effect of minimized dissolution deviation of cilostazol, the sustained-release active ingredient of the sustained-release combination preparation according to the present invention, the dissolution rate deviation between the formulation of Example 2 of the present invention and the reference drug, Pretal SR capsule, was compared. It is shown in Table 10.
고찰Review
As shown in Table 10, it can be seen that the formulation according to Example 2 of the present invention exhibited significantly less variation than the conventional protal SR, thereby showing excellent pharmacological activity, while significantly reducing side effects. .

고찰Review
As in Experiment 3, in the case of a simple mixed single tablet, the statin active ingredient was hardly eluted at the beginning due to the viscosity of the sustained-release matrix.
Experiment 8> Experiment of dissolution deviation of cilostazol active ingredient from control drug
On the other hand, in order to check the minimized dissolution deviation effect of cilostazol, the sustained-release active ingredient of the sustained-release combination preparation according to the present invention, the dissolution rate deviation between the formulation of Example 7 of the present invention and the reference drug, Pretal SR capsule, was compared. It is shown in Table 20.
고찰Review
As shown in Table 20, it can be seen that the formulation according to Example 7 of the present invention exhibited significantly less deviation than the conventional protal SR, thereby showing excellent pharmacological activity, while significantly reducing side effects. .
