Abstract
본 발명은 약학분야에 관한 것으로 특히 크론씨병, 궤양상대장염 및 1형당뇨병(인슐린의존성당뇨병)의 치료를 위한 약학적조성물 제조를 위해 60kDa 분자량의 인간 열충격단백질로부터 추출한 APL(altered peptide ligand)펩티드 또는 그 유사물의 사용에 관한 것이다. 이 펩티드는 위장관에서 바이오분배되고 크론씨병에 감염된 활성화된 위장관 고유막 및 환자의 말초혈액 T세포의 세포자살유도를 증폭시킨다. 더욱이, 이 펩티드는 1형당뇨병(인슐린의존성당뇨병)환자의 단핵세포들의 세포자살을 유도한다.

본 발명은 의약 분야에 관련된 것으로 특히 그 염증성 장질환(크론씨병 및 궤양성 대장염과 같은) 과 1형진성당뇨병(인슐린의존성당뇨병)의 치료를 위한 APL 펩티드(Altered Peptide Ligand: APL 이하 칭함) 또는 그 유사물의 사용에 관한 것이다.
크론씨병이나 궤양성 대장염같은 염증성 장질환은 공생박테리아에 대해 강력한 효과기 기능을 가진 장 고유막 T 세포의 활성화에 기인한다. 그러나, 이러한 림프구의 만성적인 활성화에 이르는 메카니즘은 아직 그 원인이 명확하게 알려지지 않고있다.(Balfour R (2006) 질병메카니즘(mechanism of Disease), 크론씨병 및 궤양성대장염의 병인(pathogenesis of Crohn's desease and ulcerative colitis). nature clinical practice 저널지. 위장병학 및 간장학 3 (7):390-407).

대장의 염증 세포의 수는 세포의 보충, 세포사멸 또는 세포자살에 의한 증식 또는 죽음으로 결정된다. 정상적인 장점막의 고유막 T 세포에서는 림프구의 확산을 제어할 수 있는 활성화에 의해 유도되는 세포자살 (FAS/ FasL 시스템을 통해)이 쉽게 일어 난다. (Bu P et al. 특히, 종양괴사인자-알파(TNFα)항체에 대한 크론씨병을 가진 스테로이드 내성을 가진 환자에게 장기간 병을 완화시키는데 있어 아주 중요한 옵션이다.
인플릭시맵(Infliximab)은 생쥐단일클론항체가변영역 과 인간 면역글로블린 G1분자의 불변영역을 가진 키메릭 단클론항체(AcM) 로서 결합되어 높은 친화성과 특이성을 가진 자유 및 결합 종양괴사인자-알파(TNFα)의 효과를 중화시킨다(Knight DMK et al. 에타너셉트(Etanercept)는 인간면역글로블린 G1분자의 Fc 부위에 접합된 인간 종양괴사인자 수용기 2(TNFR2)의 용액부위의 두개의 모노메트린사슬을 포함하는 재조합된 단백질이다 IgG1 (Mohler KM et al. (1993) Soluble tumor necrosis factor (TNF) receptors are effective therapeutic agents in lethal endotoxemia and function simultaneously as both TNF carriers and TNF antagonists. 에타너셉트는 그의 자살촉진효과에 기인한 크론씨병에서 유효한 인플릭시맵과 같은 그러한 세포자살을 유도하지 않는다.
T-세포 자살을 유도하는 인플릭시맵(infliximab)및 기타약제를 사용하여 크론씨병환자를 치료한후 얻어진 임상결과는 점막T세포 구획에서 세포자살 기제의 부활이 크론씨병을 치료하는데에 가장 중요한 요소라는 것을 암시하고 있다.
현재까지 인플릭시맵은 크론씨병 환자를 치료하는데 있어 가장 성공적인 치료약이다. 이 치료는 궤양성대장염을 치료하는데 있어서도 사용되어 왔다. 그러나 이러한 치료약의 사용은 부작용을 일으켰는데 이로 인해 발생하는 광범위한 면역억제로 인한 결핵과 마이코 플라스마 감염증과 같은 아픈 증상의 증가 같은 것이었다(Kooloos WM. 결과적으로, 현재 주요한 과제는 광범위한 면역억제를 하지않고도 특이성을 가진 병원성 세포를 제거할 수 있는 치료전략의 개발이다.

실험적으로 펩타이드리간드의 고유속성을 조정하는 능력은 면역세포반응의 속성,과정 그리고 힘을 적절히 변화시킬 수 있도록 해주고있다..
특허번호 WO2006/032216의 국제출원에서는 인간 열충격단백질 분자량 60kDa(Hsp60으로 칭함) 로부터 추출한 APL 펩티드가 류마티스성관절염의 치료를 위한 그러한 펩티드의 약학적제재의 사용과 함께 특허청구범위로 주장되었다.
1형당뇨병(인슐린의존형당뇨병)은 인슐린을 생산하는 이자의 베타세포(βcells )를 파괴하여 포도당 대사의 철폐에 이르게 하는 T세포에 의해 매개되는 자기면역반응의 특수한 질병이다(Brown L and Eisenbarth GS (1990) 1형당뇨병: 인간,생쥐,쥐에서의 만성적인 자가면역질환(Type 1 diabetes: A chronic autoimmune disease of human, mouse, and rat). Annu Rev Immunol 8: 647-79).

이 펩티드는 크론씨 병을 가진 환자의 활성화된 대장고유막 그리고 말초혈액 T세포 의 세포자살의 유도를 촉진하여 종양괴사인자-알파(tumor necrosis factor alpha ; TNFa) 항체의 사용시 흔하게 나타나는 비특이적인 면역 억제을 유발하지 않고서도 이 질병의 병인에 포함된 T 세포 클론이 억제되도록 한다. 염증성장질환의 치료를 위해 면역 조절물질인 APL 펩티드와 그 유사물의 사용은 병의 특징적인 염증성진행에 기여하는 T세포의 클론을 중화시키는 데 향해져 있다.

염기서열 아이디 번호 1로 염기서열목록 식별되는 APL(Altered Peptide Ligand) 은 국제특허출원 WO 2006/032216 에서 류머티즘성 관절염(RA)의 치료를 위한 펩티드의 약학적제제의 사용을 위해 주장되었다. 그러나 이 특허는 이 펩티드가 염증성장질환 그리고 1형당뇨병(인슐린의존성당뇨병)의 치료를 위해 사용될 수 있다는 것을 주장하거나 제안하지 못하였다.
염증성장질환은 자가면역질환으로 생각되지 않는다. 왜냐하면 자기항원에 대한 면역반응은 염증의 시작과 유지에 책임이 없기 때문인데 적어도 현재까지는 통상적으로 관련된 자기항원은 알려져 있지 않으며 이는 자기면역질환과 대비되는 것이다.

아이런 코헨미 그 동료들에게 권리가 부여된 미국 특허 6682897호는 1형당뇨병 (인슐린 의존성 당뇨병) 진단과 치료를 위해 인간 열충격단백질(Hsp60) 펩티드의 사용을 기술하고 있다. 이 염기서열 아이디 번호 1로 식별되는 염기서열은 이 특허에는 속하지 아니하였으며 또한 염기서열아이디 번호1의 펩티드에 대한 유사한 생리활동도 고려되지 아니하였다.
이들 작자들과 달리, 본원 출원에서 출원인은 병의 발전과정에 개입된 병원성 T세포의 세포자살을 유도하기 위해서 염기서열 번호 1 , 즉 인간 열 충격단백질 (Hsp60) 으로 부터 추출한 APL 펩티드의 사용을 밝히고자 하는 것이다.
본 발명의 실시 예는 처음으로 위장관으로의 바이오분배와 관련된 염기서열 아이디 번호 1 펩티드의 속성과 크론씨병, 궤양성대장염 그리고 1형 당뇨병(인슐린의존성당뇨병)의 치료를 위해서 이 펩티드의 사용을 가능케하는 T세포의 병원성 클론의 세포자살을 유도하는 능력을 보여준다. 동종업계에 근무하는 일반기술자 시각에서 ,국제특허출원번호 WO 2006/032216에서 제시한 요소에 기초하여 본 발명에서 주장하는 펩타이드의 신규한 사용을 예견하는 것은 가능하지 않다.
염기서열 아이디 번호 1 로 식별되는 염기서열의 펩티드 또는 그 유사물은 펩티드 합성의 통상적인 방법으로 생산될 수 있으며 후술하는 바 실시 예에서 설명되는 것과 같은 실험에서 유도된 수준과 면역 반응의 품질로 평가될 수 있다.

펩티드의 효과
크론씨병 환자와 건강한 기증자로부터의 정맥주사로 혈액을 추출하여 항응고제 솔루션이 (나트륨 구연 산염 123 mM, 인산 나트륨 일염기 18.5 mM, 구연산 17 mM과 포도당 141.5 mM) 포함된 멸균 튜브에 수집되었다. 각 환자의 혈액은 인산염완충식염수(Phosphate Buffered Saline solution ) 1X ( PBS1X 으로 약칭함) 에서 1:2로 희석되었고 이 희석액의 5mL 가 15mL의 원심분리튜브 에서 Ficoll - PaqueTM 플러스용액 (애머스햄, Biosciences AB, 스웨덴)3 mL에 추가되어 1,200 rpm으로 30분간 원심분리되었다.

MTT는 조직배양매체에 용해되지 않는 포르마잔(formazan)생성물에 활발한 대사활동을 가지는 세포에서 발견되는 미토콘드리아의 탈수소 효소에 의해 줄어든다. 562nm에서 흡광도로 측정 되는 포르마잔 생성물의 양은 배양액의 살아있는 세포의 수에 비례한다 72 시간 5 % CO2의 습한 분위기에서 섭씨 37도 에서 72시간 배양한 후에, MTT (5mg /ml) 의 20 ul이 각 웰에 추가되었다. 다음에 플레이트는 동등한 배양조건에서 4시간 동안 배양되었다.

활성 크론씨병 환자로부터의 말초 혈액 단핵 세포에서 APL의 펩타이드 (본 발명에서 염기서열 아이디 번호 1로 식별된)에 의해 유도된 세포 죽음의 메커니즘이 세포자살에 의해 매개된다는 것을 식별할 목적에서 샘플이 투과전자현미경(TEM)에 의해 분석되었다.

비활성 크론씨병환자로부터 말초혈액단핵세포(혈액내 백혈구 세포)는 5 %의 CO2의 습한 환경에서 섭씨 37 도로 72 시간동안 항-CD3항체(e-Biosciences)로서 활성화되었다. 항-CD3 항체의 부가는 세포개체군에서 T세포의 다중클론 활성화를 발생시킨다. 활성화된 림프구는 PBS1X 솔루션으로 세척되고 그런 다음 APL 펩티드(염기서열 아이디 번호 1)의 각각 다른 농도(10,40 그리고 160ug/mL)로 1시간 동안 배양되었다(1X105세포). 항-CD3 항체없이 72시간 동안 배양된 말초혈액단핵세포(혈액내백혈구세포)는 활성화분석의 대조군으로서 사용되었다(비활성세포). 이 시간후에, 이들 세포는 APL 펩티드의 동일한 농도로 배양되었다.

실시 예 1에서 기재된 대로 1 형당뇨병 (인 슐린의존성당뇨병) 환자의 말초 혈액단핵 세포가 원심분리에 의해 PaqueTM PLUS 용액 위에 분리되었다. 그렇게 하기 위해 10x106 의 세포가 72 시간 동안 APL 펩티드 (40 ug/ mL)로 처리되었다. 처리되지 않은 세포는 분석을 위한 대조군으로 사용되었다. 제5도에 도시된 바와 같이 이 펩티드로 처리된 세포는 앞서 기술한 세포자살(패널 C-D) 의 세포형태(실시 예2)를 가지고 있었다. 한편, 처리되지 않은 세포(패널 A-B)는 일반적인 형태를 가지고 있다. 이러한 결과는 APL 펩티드가 1형 당뇨병(인슐린의존성당뇨병) 환자의 단핵세포에서 세포자살을 유도하고 있음을 시사하고 있다.

APL 펩티드는 (염기서열 아이디 번호 1 식별) I125 동위원소로 라벨이 붙었으며 그것은 정맥과 피부 경로를 통해 0.25 mg 과 1mg / Kg 의 개체 중량비로 루이스 쥐에게 투여되었다.
