Description

【発明の詳細な説明】 「産業上の利用分野」 本発明は直接固定型人工股関節に関するものである。
「従来の技術」 一般的に知られているように、人工股関節は本質的に二
つの種類に分類される。
第１の種類は、膠着物により固定されるタイプの人工股
関節である。しかし、このタイプの人工股関節は二つの
問題を呈示する。
一つは膠着物自体によって引き起こさ、れる麻酔ショッ
ク等の手術中の問題である。
また、もう一つは骨が膠着物と接合することによる汚染
の危険、あるいは、穿孔された骨がもろくなって人工股
関節の交換が必要になった場合に取り外し等の再手術が
困難であること等の手術後の問題である。
一方、第２の種類は、現在広まりつつある「直接固定型
人工股関節（ｐｒｏｓｔｈｅｓｉｓ　ｗｉｔｈ　ｐｒｉ
ｍａｒｙｆ’１ｘａｔｉｏｎ　）　Ｊと呼ばれるタイプ
である。このタイプの人工股関節では、調整されたピン
が骨に強制的に嵌め合わされ、また、ピンの特殊な形状
により骨の矯正がなされる。このタイプの人工股関節は
三つの主要部分、すなわち、順に、大腿骨に挿入される
大腿ピン、患部股関節の寛骨臼に嵌め込まれる杯体、お
よび、球状部を戦きかつピンを杯体に結合して関節接合
を適正に保持する関節首を必須構成としている。
このような人工股関節を提供するにあたって、これまで
に多くの提案がなされている。
例えば、大腿ピンをねじのように大腿骨にねじ込む場合
に、前記ピンを多少切り詰めることによりねじ込み操作
を簡単にする提案である（欧州特許第０（１１０５２７
号および仏閣特許第２２９５７２９号参照）。
「発明が解決しようとする問題点」 しかしながら、上記ようなピンは、しばしば大腿骨の形
状にふされしくない状態にねじ込まれて調整不良を起こ
し、ひいては不具合な応力分布を発生させる。さらに、
これまで使用されていたピンは、長さ不足あるいはフラ
ンジにおける引き戻し現象（ｄｒａｗｉｎｇ−ｂａｃｋ
　ｅｆｆｅｃｔ　）等に起因して、取り外し時に危険を
伴うことが指摘されている。
このような欠点は、ねじの取り付は位置で生じる最大応
力によるところの手術後の痛みを引き起こす可能性があ
る。
一方、欧州特許第０００５４９号および仏”国特許第２
４８１５９６号の両者は、ピンを大腿骨に嵌め込まない
代わりに、ピンの頭部に同軸フランジを設け、その下面
を大腿骨に係合させ、かつ、上面に位置決め固定手段を
設けて、ピン組立時に関節首の傾斜姿勢の設定と保持と
を行なうことを提案している。この提案は多くの効果を
奏するものであるが、実際にはフランジが整復部位に係
合するから、骨の切除が必要であり、大腿骨の上端、特
に、「転子」として知られる部分に破損を与える。さら
に、このフランジの構造そのものから考えて、関節首の
萌傾姿勢があらゆる状況において理想的な向きになり得
ない。
本発明は上記の事情に鑑みてなされたもので、その目的
とするところは、上記全ての欠点を排除した直接固定型
人工股関節を提供することにより、上記の不利を克服す
ることにある。
「問題点を解決するための手段」 かかる目的を達成するために、本発明に係る直接固定型
人工股関節は、次の三つの部分、すなわち、 １、大腿骨にねじ込まれるべく設計されたねじ山付き大
腿ピン、 Ｉｉ、患部股関節の杯状の窪みに嵌まり込む形状の杯体
、 ｉｉｉ　、上端に球状部を載くとともにピンの頭部を杯
体に連結して関節接合を適正状態に保持する関節首 を主要構成とし、しかも、ねじ付き大腿ビンが、その末
端部から頭部に向けて漸次半径を増大させる如くテーバ
付けされたチューリップ形の縦断面を有することを特徴
とするものである。
言い替えれば、本発明は直接固定型人工股関節に係り、
そのねじ付き大腿ピンは半径が漸次変化する特殊なデユ
ーリップ形の解剖学的な輪郭形状を有するものである。
ここで「半径が漸次変化する輪郭形状」とは、末端部か
ら頭部に向かって曲率半径が連続的に漸次減少する縦断
面輪郭を意味する。従って、横断面の半径は末端部から
頭部に向けて漸次増加することになる。
この場合、実際には、ねじ付き大腿ピンの横断面形状が
円形であること、また、ねじ付き大腿ピンのねじ山が、
棒体のねじ山間様に台形断面、好ましくは角が丸まった
台形断面を有することが有利である。
また、前記ねじ山において、ねじの方向に沿−う横断面
に対するねじ山上面の傾きは、同横断面に対するねじ山
上面の傾きより小さいことが望ましい。
前記横断面に対するねじ山上面の傾斜角は１５ないし２
５°、好ましくは２０°であり、一方、同横断面に対す
るねじ山上面の傾斜角はＩＯないし２０°、好ましくは
１５°である。
また、ねじ付き大腿ピンはその軸線に沿う複数の溝を備
えており、谷溝の切り込み角は、ねじ込み、および抜き
出し操作を補助する両方向に傾斜していることが好まし
い。
さらに、関節首とねじ付き大腿ピン頭部との嵌め合いお
よび関節首と球状部との嵌め合いは、側面の傾斜が小さ
い二つの円錐状部分、すなわち円錐状凸部および円錐穴
の嵌め合いによりなされるのがよい。
「作用」 本発明の直接固定型人工股関節は大腿骨の形状に適応し
ているので、順応性が優れている。
また、ねじの取り付は位置における最大応力を低減させ
、かつ、大腿骨の上端の切除を少なくして直接固定法を
改善する。
そして、結果的に関節首の前傾姿勢を理想的な向きにす
る。
「実施例」 以下、本発明の一実施例を図面に基づいて説明する。
本発明に係る人工股関節は、横断面円形をなすねじ付き
大腿ピン（以下単にピンと称す）ｌを第１の構成要素と
している。このピンｌは、その頭部２が末端部３より太
く形成されており、また、本発明によれば、末端部３か
ら頭部２に向けて半径を漸次増大させるチューリップ形
の縦断面４を備えている。この外側に向って広がる独特
の解剖学的輪郭の曲率は、連続的に累進的に変化してい
る。従って、ピン１の半径も変化している。このピンｌ
の輪郭形状は、三次多項式に近似できる。
好ましい実施例では、例えば、ピンｌの全長を９５ｍｍ
とした場合、末端部３の直径は１８ｍｍ（ねじ山も含む
）、頭部２の直径は３０　ｍｍ％これに対し、中央部の
直径は２０．５ｍｍ足らずである。
前記ピンＩは在来の一般物質、例えばステンレス、合金
、あるいはセラミック等により形成されている。好まし
くはピンｌはチタニウムで形成され、表面にセラミック
、特に酸化アルミニウムの被覆層を有するのがよい。
前記ピン１には、第３図および第５図に詳細に示すよう
に、角が丸まった台形状の断面を有するねじ山５が設け
られている。
また、前記ピン１にはその軸線に沿って三本の溝６．７
．８が各々設けられている。これら溝は１２０°毎にず
れて配置されており、その切り込み角９．１０は、ピン
ｌのねじ込み操作と抜き出し操作を補助する両方向に傾
斜している。実際には、このような切り込み角は鋭い角
度をなしている。
従って、切り欠き刃を形成する三本の溝６．７．８はタ
ッピング作用（ねじを切る作用）により、ねじ込み操作
、あるいは必要に応じて抜き出し操作を容易にするので
、このとき使用される膠着物等の量を少なくできる利点
がある。
本発明の他の実施例によれば（第３図および第５図参照
）、ピンｌのねじ山５の断面は、角が丸まった台形状を
なしている。このねじ山５の上面１１はねじ山の方向、
すなわち、ねじ込み方向に対して傾斜している。この上
面１１の傾斜角はねじ山５の下面１２の傾斜角よりも小
さく形成されている。
上面１１と水平面Ｈとのなす角度（傾斜角）αは、１０
ないし２０°、好ましくは１５°前後である。
これは傾斜角αを切り込み角の一部となし、かつ、大腿
骨内におけるピンｌの固定状態を確実にするためである
。水平面Ｈと下面１２とのなす角度（傾斜角）βは、■
５ないし２５°、好ましくは２０°である。これは効果
的な切り込み角を形成するためである。第５図に示すよ
うに傾斜面１１．１２間の角部１３．１３°は丸められ
ている。
ピンｌに好適な頭部２の形状は円錐状であり、この頭部
（円錐状凸部）２の側面（軸受面）１５は約６゛程度に
傾斜している。この頭部２はこれに符合する形状の雌型
、つまり関節首１７の基部１６内に形成された円錐穴１
８に嵌まり合う。
前記関節首１７はピンｌと同様の物質、例えばチタニウ
ムで形成されている。この関節首１７はその基部１６に
円錐穴１８を備えている。また、その頭部■９は円錐状
をなし、この頭部（円錐状凸部）１９の側面（上部軸受
面）２０は僅かに傾斜している。この円錐状凸部１９は
球・状部２１内に形成された符合する形状の円錐穴２５
の中に嵌まり込むようになっている。前記上部軸受面２
０の傾斜角度は、軸受面１５の傾斜角度にほぼ近く、約
３°程度（モールス円錐）である。このような嵌め合い
構造は、機械的分野では既に周知であり、かつ、人工股
関節として既に提案されているとうり（例えば仏閣特許
１０１７９２７および欧州特許００００５４９参照）、
容易に得られるものであり、関節首１７の前傾姿勢を理
想的な向きにする。
関節首１７は、取り付けねじ２３によりピン１の上に取
り付けられている。
関節首１７の軸線とピン１とのなす角度は、１３５°前
後である。
球状部２１は、周知技術により棒体３２に嵌まり合い、
また、棒体３２は患部股関節の杯状の窪みに嵌まり合う
ようになっている。
面記杯体（第１図参照）３２は、高密度ポリエチレン製
の在来部材（図示略）を第１の構成要素としている。こ
の在来部材は、球状部２１の外面（接触部）２２に係合
するよう空洞部を有する形状とされており、この在来部
材の外部は卵形を呈し、かつ、それ自身を棒体３２の中
に適応状態に埋設して、受は具を形成している。有利な
実施例では、前記接触部２２は気相で付着させられた窒
化チタニウムの層、例えば３ないし１０ミクロン以上の
厚さを有する層で構成されている。また、チタニウム製
の棒体３２は、前記受は具の形状に実質的に符合する外
形を持っている。このことは骨盤への力の伝達状態を改
善する。また、前記棒体３２には、ねじ山５に似た台形
状のねじ山３３、およびタッピング作用を持つ一連のＷ
Ｉｔ３４，３５が設けられている。
また、棒体３２のねじ山３３はピンｌのねじ山５と同じ
特徴、すなわち角が丸まった台形断面を有するという特
徴を持っているのが好ましい。そして、本発明の好まし
い実施例では、ピンＩのねじ山５と杯状体３２のねじ山
３３は６本の突条により構成されている。
上記のように構成された人工股関節において、ピンｌの
形状、すなわち、半径が漸次変化するチューリップ形の
特徴ある輪郭形状は、予期した以上に大腿骨上端の皮質
の形状にほぼ完全に適合する。
この結果、ピンｌの調整をほどよく行うことが可能とな
り、人工股関節と骨との間の歪分布を改善することがで
きる。
また、前記ねじ山５は人工股関節に適用された場合に、
予想以上に抜群の強度を発揮し、かつ、応力分布を改善
して最大応力を低減させる素晴しい効果を奏する。
「発明の効果」 以上説明したように、本発明の人工股関節は従来のもの
を上回る次のような多くの効果を奏する。
■ねじ山のピッチに与えられた特殊な形状、すなわち、
角が丸まった台形状の断面形状により、ねじ山の取り付
は位置での最大応力を消し去るか、あるいは完全に消し
去れなくとも、低減させることができる。従って、手術
後の痛みを緩和することができる。
■半径を漸次変化させながら外側に向かって広がるチュ
ーリップ形状のピンが、従来の円錐形のピンよりも良好
に大腿骨の形に適合する。この結果、ピンの挿入操作時
に解剖学的に優れた調整を行うことが可能となる。
■ビンにフランジが無いので大腿骨上端の骨の切除が重
要でなくなるから、転子が破損する危険性を小さくする
ことができる。
■機械的分野およびこの種の出願でよく知られている低
傾斜の円錐状部分の嵌め合いにより、結果的に、これま
で得ることができなかった関節首の理想的な前傾姿勢を
確保することができる。
■ビンのタッピング作用により、少量の膠着物で簡単に
位置調整ができる。

  1. 【特許請求の範囲】 １）大腿骨に挿入されるねじ山付き大腿ピンと、患部股
    関節の寛骨臼に嵌め込まれる杯体と、球状部を載くとと
    もに前記ねじ山付き大腿ピンを杯体に連結して関節接合
    を適正状態に保持する関節首とを必須構成とし、しかも
    、前記ねじ山付き大腿ピンは、半径が漸次変化する特殊
    なチューリップ形の解剖学的な輪郭形状を有することを
    特徴とする直接固定型人工股関節。 ２）前記ねじ山付き大腿ピンの横断面は、円形であるこ

    面の傾斜が小さい二つの円錐状部分、すなわち円錐状凸
    部および円錐穴の嵌め合いによりなされていることを特
    徴とする特許請求の範囲第１項記載の直接固定型人工股
    関節。 ８）前記ねじ山付き大腿ピン、関節首、球状部、および
    杯体はチタニウムで形成されていることを特徴とする特
    許請求の範囲第１項記載の直接固定型人工股関節。 ９）前記球状部の外面は、杯体の空洞部内の高密度ポリ
    エチレンと協同する窒化チタニウムの層で覆われている
    ことを特徴とする特許請求の範囲第８項記載の直接固定
