Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は、目的物に対して正確な位置関係をもって容易
に挿置し得るようにした移植骨片に関する。
〔従来技術と問題点〕
頚部の体内関節固定を実施する外科的方法を記載した研
究論文が、スミスロビンソン、プリーメーカやクロツク
ドにより、30年前に発表されて以来この技術分野の変
化はきわめて少ない。これは彼らの技術的方法の明白な
優秀性、即ち比較的実施容易で、危険性が少なく、また
すぐれた結果が得られることによるものである。
クロツクドの技術(円筒形移植骨片による前部体内関節
固定法)はスペイン国の病院のみならず世界中の多くの
外傷治療機関および神経外科機関においてもつとも広く
使用されていることを考慮して、出願人はこの技術を用
いる出願人自身の実験により臨床的および実験的研究を
開始した。これは過去5年間に手術された約300人の
患者に及ぶものである。この意味において、移植骨片お
よびクロツクドの基本的手術器具の一部の要素について
大きな変更が加えられ、このような変更くより、医学文
献にしばしば見られるような手術中および手術後の多く
の合併症を除くことができた。
この外科技術に対する出願人の貢献は下記のコ点にまと
められる。
a)  対応の体内開腔の中に植込まれる平滑面円筒形
移植骨片の代わりに、ねじ山またはコイルを備えた移植
骨片を用いること。
b)前記移植骨片のねじ込み挿入を実施するだめの二種
類の器具の設計。
〔発明の目的および効果〕
本発明による技術工程を下記に略説する。移植骨片を得
たのち患者自身の腸骨稜の同一組織抽出により、または
同種組織移植骨片を使用し、あるいはキールサージボー
ン異性組織移植骨片を用いることKよって、特に設計さ
れあらかじめ滅菌された小型旋盤を用いて移植骨片の上
にねじ山を外科的に加工する。この方法によれば、ねじ
山を非常に容易Kまた急速に備えることができ、みぞは
所要の深さに成すことができる。異性組織移植骨片の場
合には、好ましくはこの移植骨片の製造工程中にコイル
をねじ切る。
クロウワドの標準技術によって頚椎部の前側が準備され
ている間に、前記のようにして得られた螺旋形移植骨片
が十分な滅菌状態に保持される。
対応の椎骨間腔の中に円筒形受容床が穿孔されたのち、
特にその目的に設計された器具を用いて移植骨片をねじ
込む。
〔実施例〕
以下本発明を図面に示す実施例について詳細に説明する
第9図において符号/は、移植骨片であって、この移植
骨片lは、全体形状を円柱状をなし、外周面にねじ部コ
を有して構成されている。上記ねじ部コのねじ山高さは
必要に応じて可変する。また移植骨片lの一端面にはね
じ孔2aが形成されている。そして上記移植骨片/は取
付器具3により保持されるようになっている。
上記取付器具3は、前端部分に保持ヘラドグを有し、こ
の保持ヘッド弘の端面には中心位置に中央ねじ夕および
この中央ねじりの対称位置に保持ピン6.6が設けられ
ている。上記中央ねじりに一体に連接された案内ロッド
7の端部にはねじgを有するつまみ?が設げられており
、ねじjを取付器具Jに設けたねじ10に螺合すること
で案内ロッド7が可動になっている(第1図)。
保持ヘッド弘の寸法は移植骨片の大きさに応じて取替可
能である。
しかして第1図に示すように、取付器具Jの中央ねじ!
を、移植骨片lのねじ部コに合わせ、つまみデを回わす
と、取付器具3に移植骨片lが保持される(第一図)。
使用に際しては、第一図に示すように移植骨片/を保持
した取付器具3を、予め椎骨間腔に穿孔された移植床(
図示せず)に対して移植床に設ゆたねじ部に移植骨片/
が対向する に置き、取付器具Jを回わすことで移植骨
片lを移植床に固着する。
移植骨片/の固着が終ったら、つまみりを回わすと、中
央ねじ5が取付器具Jより引込む方向に動き、中央ねじ
りの先端が保持ピン6、乙の先端面より上方に位置する
と、中央ねじ5が移植骨片lから離れることになる。
ついで取付器具3を動かすと、取付器具3は移植骨片/
から離れ、移植骨片lの移植床への固着が完了する。
第7図ないし第6図は本発明の他の実施例を示すもので
あって、この実施例においては、取付器具mに設けた保
持ヘッド21は、長手方向に設けたスリット二により形
成される複数の保持片n、n・・・を有し、移植骨片/
を保持片n、n・・・で囲むようにした後、取付器具J
に設けたつまみコタを回わすことで、保持片nを半径方
向に動かし、保持片刃、3・・・により移植骨片/の外
周面を挟着せしめるようにしている。保持ヘッドコlは
、ねじを有する中心軸コによってつまみ評に対して連結
されている。移植骨片tを椎骨問答植床の中にねじ込み
移植したのち、つまみ評を逆方向に回転させるだけで取
付器具除去され、移植骨片/が先に穿孔された移植床の
穴の中にねじ込まれて残る。
標準技術に比べて、本発明の目的を成す移植骨片を使用
する利点は下記の通りである。すなわち螺旋形移植骨片
を緩やかに挿入することができるので下記を避けること
ができる。
/  &)移植骨片の基部に対して直接に反復して加え
られる、場合によっては乱暴なハンマー衝撃による背髄
挫傷の可能性。このような重大な神経性合併症が背髄症
患者に加えられ、これと共に背髄脆弱性が生じる場合が
ある。この点に関しては文献および出願人の個人的経験
は幸にして少ないのであるが、対応の髄質部分の前面に
おける反復された無分別な外傷状態ののちに、四肢不全
麻痺、呼吸不整および転帰の症例さえもが報告されてい
る。
b)ハンマーの衝撃による移植骨片の破断。
C)移植骨片が背髄管に侵入する可能性も避けられる。
なぜかならば、この場合、受容床の中への骨片の深さを
完全に計算することができるからである。従来の方法の
場合はこれと異なり、ハンマーの衝撃を計算することが
常に困難であるから、当然(不幸な結果をもたらす。
ユ 骨片の挿入深さを完全に計算することができるので
、骨片を椎骨体の後側の自由縁まで導入することができ
、これは椎骨間腔の低下と二次的彎曲とを防止するのく
役立つ。
ユ 手術中または手術後の合併症が現われても(血腫、
背髄圧縮、呼吸変調など)、移植骨片の抽出が容易に実
施できる。
偶ねじ込み方法の故に受容床よりも広い直径の移植骨片
を挿入することができるので、椎骨間腔の中の固定度が
強くなる。なぜかならばこれら両方の要素(ねじ山と直
径)が関節固定の強さを増大し、従って滑りが困*にな
るからである。
よ頚部骨折脱臼患者を出願人は現在まで手術したことは
ないが、強く脱臼した頚椎を有する死体について広く研
究した結果、螺旋形移植骨片は負傷部の固定のために平
滑面骨片よりも明らかKすぐれていることが明かになっ
た。
本発明は前記の説明のみに限定されるものでなく、その
主旨の範囲内において任意に変更実施できる。  ゛

    【特許請求の範囲】
  1. 同一組織、同種組織または異質組織の移植骨片において
    、上記移植骨片は外周面にねじ部を有する同筒形をなし
    、対応の椎骨間腔に穿孔された円筒形受容床に螺着する
    ようしたことを特徴とする移植骨片。
