【発明の詳細な説明】

【発明の属する技術分野】本発明は、バイポーラ代用シ
ェルをユニポーラ代用大腿骨頭に変換するためのアダプ
ターと、代用大腿骨頭において頸部長さを可調とするた
めのアダプターと、大腿代用システムおよび代用大腿骨
頭に関するものである。
【０００２】
【従来の技術】バイポーラ股関節部人工器官移植に現在
使用されている手法において、代用整復手法は典型的に
４つの代用品システムを用いて実施されている。このシ
ステムには、代用大腿ブローチ、該ブローチに取り付け
た頸部、該頸部に取り付けられた代用大腿骨頭、および
該代用大腿骨頭を受けるとともに患者の寛骨臼内に固定
するためのシェルが含まれている。これら４つの部品を
用いる代用手法は、医師にとって、しばしば扱いにくい
場合がある。というのは、医師が前記代用大腿骨頭を固
定しようとするときに、この代用品は動きやすくて前記
シェルから外れ易いからである。
【０００３】バイポーラ股関節部移植器官の代用整復を
実施するいくつかの場合に、対応のユニポーラ代用品を
用いることができる。というのは、バイポーラおよびユ
ニポーラの両移植手法において代用整復を実施する際の
主も重要なことの一つは、寛骨臼に適合する部品を決定
することであるからである。また、多くの場合、バイポ
ーラ移植部品の動く範囲は、ユニポーラ代用品とほぼ同
様であることがあるからである。ユニポーラ股関節部の
移植に用いられるユニポーラ代用品システムは、典型的
に、ブローチ、頸部、および代用大腿骨頭を有する。

用シェル中に挿入可能であると同時にバイポーラシェル
から取り外し可能なバイポーラ・ユニポーラ代用大腿骨
頭アダプターを提供ことにある。さらに、このアダプタ
ーは、異なった可能な各代用骨頭サイズに対応する多重
ユニポーラ代用大腿骨頭の必要性をなくす調節可能な頸
部を備えている。

【０００８】ここで、前記結合部は、前記代用シェルと
係合するばね付勢結合部材を有してもよい。また、前記
結合部は、複数のボールプランジャーを有してもよい。
さらに、前記頸部部材は、前記環状部に移動可能に結合
されている調節可能な頸部を有してもよい。
【０００９】また、前記課題を解決するために、本発明
にかかる大腿代用システムは、大腿骨の空所に挿入する
ための大腿茎部と、該大腿茎部に連結された頸部と、寛
骨臼内に挿入するための代用シェルと、代用シェルと代
用大腿骨頭に変換するアダプターとを有してなり、前記
代用シェルが、開口と、該開口内に形成されている内面
が球面状の凹部とを有し、前記アダプターが、前記シェ
ルの前記開口によって受けられる環状部と、前記環状部
から延出する頸部部材と、前記環状部を前記代用シェル
に固定する結合部とを有し、前記頸部部材が前記代用大
腿茎部の前記頸部を受けるための開口を有することを特
徴とする。
【００１０】ここで、前記結合部は前記代用シェルと係
合するばね付勢された結合部材を有し、前記代用シェル
は前記結合部材を結合するための表面を有し、該結合部
材は前記代用シェルとの係合方向にばねにより付勢され
ていてもよい。また、前記頸部部材は、前記環状部に移
動可能に結合された調節可能な頸部を有し、該調節可能
な頸部は、前記環状部に基づく複数の頸部長さの対応す
る複数の位置を有してよい。さらに、本代用システムで
は、複数の代用シェルを有し、該複数の代用シェルは複
数のシェル外径を有してよい。
【００１１】さらに、前記課題を解決するために、本発
明にかかる代用大腿骨頭は、球状当接表面と、末端と、
該末端から先端に延びる頸部部材とを有し、前記頸部部
材が、代用大腿茎部の頸部回動軸部を受けるための開口
と、前記末端と移動可能に結合する調節可能な頸部とを
有し、前記調節可能な頸部が複数の頸部長さに対応した
複数の位置を有することを特徴とする。
【００１２】
【発明の実施の形態】図１ないし５に見るように、図示
の代用アダプター１０は、貫通開口１２を有する円筒部
（環状部）１１、ばねで付勢されて前記円筒部１１の外
周１３から突出しているボールプランジャー（代用シェ
ルへの結合部）１４、および前記円筒部１１の外周１３
を突き抜けて突出している爪部材１５とを有している。
【００１３】この代用アダプターは、さらに、図４に示
すように、このアダプター１０がバイポーラ代用シェル
３０中に挿入された場合にユニポーラ代用骨頭の調節可
能な頸部部材として動作するバレル（アダプターの頸部
部材）１６を有している。このバレル１６は、このバレ
ル１６の外周に設けられた３つの溝１７を有している。
このバレル１６は、さらに、このバレル１６の外周２４
に長手方向に設けられたスロット１８と、このバレルの
円筒状の壁２０を貫通する開口１９とを有する。このバ
レル１６は、このバレルの底部２６からこのバレルの頂
部壁２２に延びる長手開口２１を有している。前記頂部
壁２２には、前記開口２１と連通した小開口２３が設け
られている。
【００１４】合い釘ピン２８は円筒部１１の側部に押入
されて固定され、該合い釘ピン２８は前記側部を貫き延
びてバレル１６のスロット１８に填め込まれている。該
ピン２８は、前記円筒部１１を前記バレル１６に変位可
能に固定しており、前記スロット１８は、前記バレル１
６を前記溝１７によって決められた３つの頸部長さ位置
の間を上下動可能にしている。
【００１５】前記爪部材１５は、ボタン部４０、溝係合
部４２、および圧縮ばね４１を受けるめくら穴４６を有
している。前記爪部材１５が組み立てられると、該爪部
材１５は開口２５内に挿入され、合い釘ピン４７を円筒
部１１の貫通孔５１に押入し、爪部材１５の貫通孔４８
を貫通させて固定することにより、該爪部材１５は円筒
部１１に回動可能に組み付けられる。前記圧縮ばね４１
は前記円筒部１１の穴４９と爪部材の穴４６とに挿入さ
れ、円筒部１１の穴４９に配置される。合い釘ピン５０
は、円筒部１１内に延びて穴４９を横切り、前記ばね４
１を圧縮状態で穴４６、４９に閉じこめる。前記爪部材
１５は、ばね４１によってバレル１６に向かって付勢さ
れており、その結果、バレル１６が固定位置にある時に
は、前記溝係合部４２が前記溝１７の一つに係合する。
前記ボタン部４０は前記円筒部１１の外周１３に露出さ
れており、その結果、使用者は、該ボタン部４０を動か
すことによって、係合部４２を溝１７との係合から解除
し、前記バレル１６を所望の位置に移動させることがで
きる。
【００１６】使用に当たっては、前記円筒部１１をバイ
ポーラ代用シェル３０の開口３１内に挿入し、バレル１
６をアダプター１０から下方に延ばす。前記円筒部１１
の頂部２７は、前記シェル３０の開口３１内の棚部３３
に当接し、ばねが付勢されたボールプランジャー１４が
前記バイポーラ代用シェル３０内に形成されている凹部
３４に勘合する。その結果、アダプター１０は、バイポ
ーラ代用シェル３０内に勘入され、ばね付勢されている
ボールプランジャーが凹部３４に係合することによっ
て、シェル３０に固定される。
【００１７】前記バレル１６は、代用大腿５４の頸部５
５の大腿頸部回動軸部５６との結合部としての役割を果
たす。前記大腿頸部回動軸部５６は、該大腿頸部回動軸
部５６をバレル１６の開口２１に挿入することによっ
て、アダプター１０に結合することができる。
【００１８】組立が完了したら、バレル１６を円筒部１
１の開口１２内に挿入する。前記溝１７は、前記爪部材
１５と係合して、所望の頸部長さに対応して選択した位
置にバレル１６を固定する。その結果、アダプター１０
は前記代用大腿の頸部長さに調節される。前記３つの溝
１７は、３種類の異なった頸部長さ、例えば、ゼロ、５
および１０ミリメータの頸部長さに、対応する。
【００１９】前記爪部材１５のボタン部４０が動かされ
ると、爪部材１５と係合している溝１７から係合部４２
が解除される。バレル１６は、前記代用大腿５４の頸部
５５がバレル１６に取り付けられた時に、代用大腿５４
が所望の長さとなるように、伸張する。次に、前記ボタ
ン部４０が解除されて、爪部材５が該爪部材１５と当接
している溝１７と係合すると、バレル１６は前述の所望
位置に固定される。
【００２０】前記調節可能な頸部構成は、ユニポーラシ
ステムであろうと、バイポーラシステムであろうと、そ
の他のシステムであろうと、あらゆる代用システムに、
組み込むことができる。この調節可能な頸部は、所望の
アダプターや、代用システムのあらゆる代用骨頭または
頸部を、含むことができる。前記ユニポーラアダプター
１０は、代用シェルの頂部孔３５から径の小さな円筒状
器具を用いて手で押し出すことによって、前記バイポー
ラ代用シェル３０から取り外すことができる。
【００２１】本発明の具体的な実施態様は以下の通りで
ある。 （１） 前記結合部が前記代用シェルと係合するばね付
勢結合部材を有することを特徴とする請求項１に記載の
アダプター。 （２） 前記結合部が複数のボールプランジャーを有す
ることを特徴とする上記実施態様１に記載のアダプタ
ー。 （３） 前記頸部部材が前記環状部に移動可能に結合さ
れている調節可能な頸部を有することを特徴とする請求
項１に記載のアダプター。 （４） 前記結合部が前記代用シェルと係合するばね付
勢された結合部材を有し、前記代用シェルが前記結合部
材を結合するための表面を有し、該結合部材は前記代用
シェルとの係合方向にばねにより付勢されていることを
特徴とする請求項２に記載の代用システム。 （５） 前記頸部部材が、前記環状部に移動可能に結合
された調節可能な頸部を有し、該調節可能な頸部が、前
記環状部に基づく複数の頸部長さの対応する複数の位置
を有することを特徴とする請求項２に記載の代用システ
ム。 （６） 複数の代用シェルを有し、該複数の代用シェル
は複数のシェル外径を有することを特徴とする請求項２
に記載の代用システム。

【発明の効果】医師は、手術中に、ユニポーラ代用アダ
プター１０を挿入することによって、前記バイポーラ代
用シェルをユニポーラ代用骨頭に変換することを選択す
ることができる。このことによって、医師は、慣用の扱
いにくいバイポーラ代用システムを用いることなく好適
な頸部長さを選択しながら、前記代用大腿骨頭またはバ
イポーラシェルの固定を調節し、大腿を整復することが
できる。この代用システムは、好適には、代用整復手術
中に術者が好適な寸法を選択可能とするバイポーラシェ
ルの様々な寸法と、大腿茎部寸法を、有する。以上、本
発明を、特定の実施例、構成および用途に関して説明し
たが、本発明は、これらに限定されるものではなく、本
発明の特許請求の範囲の思想および観点から逸脱するも
のでない限り、様々な変形や置き換えが可能である。

    することを特徴とする代用大腿骨頭アダプター。
  2. 【請求項２】 大腿骨の空所に挿入するための大腿茎部
    と、 該大腿茎部に連結された頸部と、 寛骨臼内に挿入するための代用シェルと、 代用シェルと代用大腿骨頭に変換するアダプターとを有
    してなり、 前記代用シェルが、開口と、該開口内に形成されている
    内面が球面状の凹部とを有し、 前記アダプターが、 前記シェルの前記開口によって受けられる環状部と、 前記環状部から延出する頸部部材と、 前記環状部を前記代用シェルに固定する結合部とを有
    し、 前記頸部部材が前記代用大腿茎部の前記頸部を受けるた
    めの開口を有することを特徴とする大腿代用システム。
