Abstract

(57)【要約】 【課題】 本発明は、整形外科用補綴インプラントのス
テムを、機械で調整した骨の空隙に挿入するのを補助す
る装置を提供することを目的とする。 【解決手段】 好ましい態様においては、大腿骨内側部
分の調整した管に挿入する臀部関節プロテーゼにおける
大腿骨・股関節幹の縦襞のついたステムを心合わせする
配列ガイドが提供される。インプラント上の他のくさび
栓状付属物も、インプラント挿入時の心合わせに用いる
ことができる。

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、適切な配列（心合
わせ）をして骨に補綴用インプラントの挿入するを補助
する装置に関する。
【０００２】
【従来の技術】補綴用関節インプラントにおいては、イ
ンプラントを骨の空隙に据え付けるのに、ステムが用い
られる。ステムを受け入れる骨は、典型的には、ドリル
で穴を開け、インプラントのステムを収められる大きさ
と形状の開口部を作成し、準備する。ステムは、骨の調
整した空隙に挿入し、このステムに取り付けないし連結
される関節ベアリングの表面は、空隙から外に出る。ス
テムは多くが、縦襞、すなわち調整した空隙に入れたと
きステムに安定性と据え付け勝手をよくするための末端
側に延びる細長い溝を付けられている。
【０００３】ところで、通常、縦襞の付いたステムを調
整した空隙に挿入すると、ステムを適当な方向に向ける
ために回転させるのは、きわめて難しくなる。すなわ
ち、適当な方向に向けるには、ステムを取り外して再度
挿入しなければならないが、こうすると骨を損傷し、手
術時間が長くなることがある。また、この方法は、配列
が不正確になるおそれがある。ステムと骨の目標点は互
いに近接してはいないため、視差その他の眼による配列
に伴う問題が起こる。また、ステムは、挿入されるにつ
れて、その心合わせした位置から動くことがある。

【発明が解決しようとする課題】そこで、本発明は、整
形外科用インプラントの縦襞の付いたステムを、用意し
た骨の空隙に挿入する前および挿入中に、心合わせする
手段を提供することを目的とする。
【０００５】
【課題を解決するための手段】本発明は、くさび栓（ke
y)または縦襞の付いた補綴具を、調整した骨の空隙に配
列した状態で挿入する方法および装置を提供する。骨
は、骨内側部分の管から骨材を取り出し、できた空隙に
機械加工で所定形状の開口部を形成して調整する。そし
て、この空隙に、機械加工した開口部の形状に合った外
形の配列（alignment)ガイドを挿入する。配列ガイドは
また、好ましくはステムの小さな表面積に接触するよう
にステムに係合するインプラントの縦襞の形状に対応す
る内形またはくさび栓（key)要素を有する。好ましい態
様においては、配列ガイドの内形には、ステムの縦襞を
収めるノッチまたは溝を設ける。好ましい態様のもう一
つの様相によれば、配列ガイドには、挿入する補綴具の
一定のくさび栓要素と接触する突起を設ける。
【０００６】好ましい態様においては、配列ガイドは、
配列ガイドを取り外すときに可撓性を与えるよう、一般
に途中で途切れた円形状または周縁を有する。配列ガイ
ドはさらに、ステムを配列ガイドを通して挿入した後、
配列ガイドを骨の空隙から引き上げる手段を備える。こ
の手段は、例えばタブや突起、または配列ガイドをほじ
くり出す道具を挿入するための開口部とすることができ
る。
【０００７】好ましい態様においては、骨の空隙が調整
され、配列ガイドが挿入されたら、補綴用インプラント
の縦襞付きステムを、その軸を、縦襞が配列ガイドのノ
ッチに入り込むように合わせ、配列ガイドに挿入する。
ステムを配列ガイドに挿入したら、ステムが骨の空隙に
当るまで、その位置または方位を正しく保つ。ステムが
挿入されると、縦襞は、骨の内側部分に、ステムの最終
的なセグメントを挿入する間、ステムが進む際に従う道
筋を形成する。そして、この工程が終わったら、タブを
引っ張って配列ガイドを取り外す。挿入は、ステムの近
位側が機械加工した骨の開口部内に位置するまで、イン
プラントを開口部の中に突き進めることによって完了す
る。このステムの近位側の形状は、インプラントが骨の
空隙に嵌合するよう、機械加工した骨の開口部形状に合
致したものにする。

【発明の実施の形態】以下は、股関節ステムインプラン
トを用いた、本発明の好ましい実施の形態の説明であ
る。図１乃至図３には、配列ガイド１０を示す。配列ガ
イド１０は、本体１１、ガイド部１２および開口部部１
４を備える。本体１１とガイド部１２は、骨２０の機械
加工した空隙２１の少なくとも一部に対応する外形を形
成する。ガイド部１２は、インプラント５０のステム３
０の縦襞３１を収容する溝またはノッチ１６を有する内
縁１５を具備する。ガイド部１２にある突起１３は、イ
ンプラント５０のステム３０に接触する。この実施の形
態においては、溝１６と突起１３は、ステム３０の骨の
空隙への挿入を案内するくさび栓（key)を形成する。こ
の実施の形態の変形例においては、くさび栓の付いたス
テム３０の挿入を案内するため、溝１６、突起１３等を
単独で、あるいは一緒に用いる。
【０００９】図１は、機械加工した空隙２１と骨の内管
に形成した細長い空隙２２を有する調整済の骨２０を示
す。機械加工した空隙２１の少なくとも一部は、配列ガ
イド１０の形状に対応する形状を有する。図１はさら
に、ステム３０の軸に沿って末端側に延びる縦襞３１を
有するステム３０と、骨を機械加工した空隙２１に嵌合
する形状をした突起部または近位配列３５、およびボー
ル型およびソケット型関節のボール部を収容するインプ
ラント近位端から延びるネック(neck)３３を有する。
【００１０】配列ガイドの内縁１５に設けるノッチ１６
は、ステム３０が配列ガイド１０に挿入されたときに、
骨空隙内の適当なあるいは整列した位置に保持または案
内するため、ステム３０の縦襞３１が配列ガイド１０に
あるノッチ１６の中に嵌合するように位置設定される。
【００１１】この実施の形態においては、機械加工した
骨の空隙２１は、通常、楕円形の端部２３を有するよう
にテーパ付けされる。そして、配列ガイド１０も、本体
１１の近位側端部がこれに合った楕円形部分１８を有す
る形状にされる。インプラント５０は、楕円形部分３４
を有する同様の形状の近位部分を有する。空隙２１はさ
らに、配列ガイド１０とインプラント５０のそれぞれの
テーパ１９，３５に対応する切り落し部２４を有する。
骨の空隙２１の切り落し部２４と配列ガイド１０のテー
パ部１９は、配列ガイド１０が骨の空隙２１，２２にあ
まり深く入り込まないようにするためのものである。骨
の空隙の切り落し部２４とインプラント５０のテーパ部
３５も同様に、インプラント５０が骨の空隙２１，２２
に深く入り過ぎて、ネック３３の位置が不適当にならな
いようにするためのものである。
【００１２】使用する際は、配列ガイド１０を、その楕
円形端部１８が、機械加工した骨の空隙２１の楕円形端
部２３と心が合うようにして空隙２１に挿入する。イン
プラント５０のステム３０は、その楕円形端部３４が配
列ガイド１０の楕円形端部１８を横断する溝９と嵌るよ
うにして、配列ガイド１０におけるガイド部１２の内縁
１５に配置される。インプラント５０を適切に心合わせ
しながら挿入するためには、インプラントの近位側形状
（geometry）３２を、これが機械加工した骨の空隙２１
の中に嵌合するように心合わせする。縦襞３１は、ガイ
ド部１２のノッチ１６の中に配置され、インプラント５
０は配列ガイド１０に案内されて、骨の空隙２２の中に
押し込められる。図２に示す様に、インプラント５０
が、完全に挿入されたわけではないが、骨の空隙２１，
２２内にあるときは、配列ガイド１０は、この開口部
に、これを骨の空隙２１とインプラント５０のステム３
０から取り外すための装置を挿入することによって取り
外される。インプラント５０はついで、楕円形端部３４
が、骨の楕円形端部２３内に嵌合し、テーパ３５が骨の
空隙２１の切り落し部２４に隣接して当接する定位置ま
でさらに押し進める。
【００１３】以上、本発明の配列ガイドを、くさび栓あ
るいは参照用にステムの縦襞を用い縦襞の付いた股関節
幹に使用する特別な態様に則して説明してきたが、当業
者にとっては、本発明の範囲を逸脱することなく種々の
変更を加えられることは明らかであろう。例えば、本発
明に係る挿入ガイドは、適切に選択した心合わせ位置で
ステムを挿入させるために、外表面にくさび栓を有する
ステムならば、どのようなものでもその挿入を補助する
のに用いることができる。配列ガイドには、インプラン
トのステムを適切に挿入させるためのくさび栓となるく
ぼみまたは突起は、何個設けてもよい。
【００１４】本発明の具体的な実施態様は以下の通りで
ある。 （１）前記整形外科用補綴具は、関節補綴具のステムを
含む請求項１記載の配列ガイド。 （２）前記くさび栓要素は、少なくとも一個の溝を含む
上記実施態様（１）記載の配列ガイド。 （３）前記くさび栓要素は、少なくとも一個の突起を含
む上記実施態様（１）記載の配列ガイド。 （４）前記くさび栓要素は、前記ステムの軸方向に延び
る少なくとも一個の縦襞を含む上記実施態様（２）記載

  1. 【請求項１】 整形外科用補綴具のくさび栓付き部品を
    準備した骨の空隙に挿入する際に案内する配列ガイドで
    あって、 前記準備した骨の空隙の少なくとも一部に対応する外形
    を有する本体と、 前記くさび栓付き部品からなる整形外科用補綴具を収容
    するための前記本体の内側部分であって、前記整形外科
    用補綴具のくさび栓付部品を係合しながら収納するくさ
    び栓要素を具備する内側部分を備える配列ガイド。
  2. 【請求項２】 整形外科用補綴具を準備した骨の空隙に
    挿入する際に案内する配列ガイドであって、 前記準備した骨の空隙の少なくとも一部に対応する外形
    を有する本体と、 前記整形外科用補綴具の縦襞の付いたステムを収容する
    ための前記本体の内側部分であって、前記縦襞付きステ
    ム部品の少なくとも一本の縦襞を係合しながら収容す
    る、内部に形成されたくさび栓要素を具備する内壁を有
    する内側部分を備える配列ガイド。

    ントシステムであって、 近位部、末端部および、ステムに沿って延びる少なくと
    も一本の縦襞を有するステムであって、前記近位部は、
    前記補綴インプラントを収納する骨に形成された近位側
    空隙と一致する近位形状を有する、ステムを具備するプ
    ロテーゼと、 前記骨の近位側空隙に対応する外形を有する本体と、 前記補綴インプラントの縦襞付きステムを収容するため
    の、前記本体に設けられた開口部であって、前記ステム
    の少なくとも一本の縦襞を収容する溝を有する内壁を形
    成する前記開口部を備える配列ガイドを備えるシステ

    具を配列する方法であって、 前記縦襞付きステムを収容する細長い空隙を準備し、か

    うに、前記ステムの近位側部分を前記骨の近位側空隙と
    心合わせする工程と、 前記ステムを、前記配列ガイド内側部分に隣接させなが
    ら、前記骨の空隙に挿入する工程と、 前記ステムを、前記骨の細長い空隙内に少なくとも一部
    押し進める工程と、 前記配列ガイドを取り外す工程と、 前記ステムを、前記補綴具の近位側端部が前記骨の近位
    側空隙内に隙間なく入り込むまで、さらに押し進める工
    程を含む方法。
