Abstract

部に孔抜き先端を持つ遠方に伸びている螺旋部材を有す
る。螺旋部材は螺旋を形成する。本発明の鋲は組織、縫
合糸または医療用具を組織と結び付けるために使用され
うる。
Description
用具、特に外科用鋲およびその使用方法に関する。

ステープルは慣用の縫合糸の代わりに開業医が入手でき
る有用な代替物であることがわかっている。

鼠蹊ヘルニア部分に生体適合性の外科用メッシュを貼る
ことである。これは典型的に外科用メッシュをヘルニア
部分の周囲の組織および筋肉にステープルで留めること
により行われる。この処置において使用されるステープ
ルは典型的にステンレス鋼、チタン、タンタルなどから
作られた慣用の金属ステープルである。このような処置
における金属ステープルの使用には幾つかの欠点が伴
う。ある明らかな欠点は金属ステープルが患者の体内に
ずっと残留しうることである。さらに、神経が金属ステ
ープルにより捉えられ、圧迫されることは珍しくない。
このことは患者に痛みを残しうると考えられている。こ
のような痛みを軽減するために、ステープルを除去およ
び/または交換するその後の手術が必要である。
慣用のステープルの保持力がステープルワイヤの大きさ
とその材料の堅さおよび機械的性質により制限されるこ
とである。より大きなそして/またはより堅い金属ステ
ープルは慣用の外科用装置のアンビルの周りに曲げるこ
とがより難しく、そのためその強度が制限される。幾つ
かの場合において、慣用の金属ステープルは強くない、
またはあまり大きくないため組織から引き抜くことがで
きる。
克服する手助けになり、また例えば吸収性ステープルは
しばらくすると吸収され、それにより圧迫された神経に
かかる圧力が緩和されるため追加の外科的処置を行う必
要がない。吸収性ステープルは当該技術分野において知
られているが、このようなステープルは典型的にクラウ

技術分野において知られている吸収性ステープルは典型
的に組織の片側に近づくだけの鼠蹊ヘルニアの治療のよ
うな外科的処置に使用することができない。下部の受け
口を必要としない他のタイプの吸収性ステープルは19
93年11月2日に出願され、一般譲渡された共同未決
米国特許出願第146,755号(参考文献として本明
細書に組込まれる)に記載されている3個構成ステープ
ルである。

されると組織を押しつぶすステープルの使用はある場合
において虚血をひき起こし、その結果ステープルが加え
る圧力に応じて壊死が生じることが知られている。さら
に、殆どのステープルは非吸収性の材料から製造され、

易に特定用途のそれぞれについて最適な螺旋の長さ、コ
イルの主な寸法およびコイルの数が決定されることを理
解するであろう。

外科用メッシュを鼠蹊ヘルニア部分の上に載せる。本発
明の吸収性外科用鋲10は鋲10を適当な外科用つかみ
具でつかみ、そしてその鋲10を留める対象の組織に近
い位置に配置することにより施す。鋲10は慣用の吸収
性ポリマー材料から製造される。螺旋部材50の孔抜き
先端58は組織の表面と実質的に垂直に位置する。次
に、鋲10を先端58が組織を突き通すまでメッシュを
通して深く押し進める。次に、鋲10をベース部材20
の底面40がメッシュと接触するまで組織の奥深くまで
移動するように回転させる。

た後、鼠蹊ヘルニアの処置を慣用の方法で終了し、そし
て腹腔壁の切開部を慣用の縫合糸を使用して閉じる。外
科用鋲10は鼠蹊ヘルニア部分の上に外科用メッシュを
保持し、時間がたつと患者の体内に吸収される。

外科的処置において使用される鋲が提供される。本発明
の鋲はベース部材、その底から伸びている螺旋部材、お
よび螺旋部材の遠端部から伸びている孔抜き手段で構成
される。慣用のステープルと異なり、本発明の鋲はそれ
を組織に保持するための追加の部材を必要とせず、使用
の際は組織の片側に挿入するだけでよく、しかも組織を
圧迫しない。また、本発明の鋲は吸収性材料で製造する
ことができるため体内に吸収される。

Claims (5)
- 【請求項1】 上面および底面を有するベース部材;ベ
ース部材の底から伸びており、少なくとも1つの外縁と
縁端部を有し、そして少なくとも1つのコイル、軸線お
よび内部通路からなる螺旋を形成する螺旋部材;および
螺旋部材の縁端部から伸びている孔抜き手段を包含す
る、外科的処置において使用される鋲。 - 【請求項2】 さらに、組織への鋲打ちを容易にするた
めベース部材に取付けられた手段を包含する請求項1記
載の鋲。 - 【請求項3】 さらに、それに取付けられた縫合糸を包
含する請求項1記載の鋲。 - 【請求項4】 さらに、ベース部材の上面から伸びてい
る縫合糸取付け手段を包含する請求項1記載の鋲。 - 【請求項5】 上面および底面を有する近位のベース部
材;ベース部材の底から伸びており、少なくとも1つの
外縁と縁端部を有し、そして少なくとも1つのコイル、
軸線および内部通路からなる螺旋を形成する遠位の螺旋
部材;および螺旋部材の縁端部から伸びている孔抜き手
段を包含する外科用鋲を組織の断面の片側に挿入するこ
とからなる、組織を外科的に留める方法。
