方法も短時間で行なえる合理的な方法とは言えず、改良
法を求める声が多かった。

スコープENT30SII(図3)用として製作した。
このファイバースコープを構造的に、検査に使用すると
きに手で保持し操作する保持部分と、患者の鼻内から喉

外径は4.5mm、長さは300mmである。図4の様
に内径16mmの塩化ビニル製パイプ(1)にT字型パ

た。ステンレス製の、洗浄用ファイバースコープ固定具

を選んで取り付けた。図に示すようにT字型パイプに接
続パイプ(7)(8)を用いてネジ込み式ホース(9)
を接続し、さらに水道栓と接続した。水道栓を開いてい
くと流水は流入口である(2)から(1)に入り、ファ
イバースコープ挿入部分に沿って流れ、L字型パイプの
一端(10)から流出される。そして流水量が一定量を
越えるとパイプ内に充満し、上端(11)から溢れるよ
うになる。この流水量で洗浄することにより、ファイバ
ースコープ挿入部分(300mm)の全てが流水で洗浄
され、また先端のレンズ面においては、パイプが直角に
曲がる部分に位置することにより、水流がレンズ面に当
たり洗浄されると考えられる。またこの洗浄器に並べて
消毒薬浸漬用パイプを取り付けた。これは、内径19m
mの塩化ビニル製パイプ(12)の上端に、一回り太
い、短いパイプ(13)を取り付けて取り付け具(1
4)の引っ掛かりとし、下端にキャップ(15)を付け
て全体として400mmとしたものである。ファイバー

水洗浄効果を検討した1)。102例についてファイバ
ースコープ使用直後(洗浄前)と洗浄直後に挿入部先端
の細菌検査を行なったところ、洗浄前に認められた細菌
が残らず消失し、完全な消毒効果が認められたと考えら
れた例(菌消失例)は85例83.3%であった。洗浄
前の菌種数が洗浄により減少した例は6例5.9%、菌
量が減少した例は1例1.0%、洗浄によって菌種、菌
量が全く変わらなかった例は10例9.8%であった。
つぎにこの消毒器の消毒薬浸漬用パイプに2%グルタル
アルデヒドを満たし、前述の一連の消毒行程を行なった
場合の消毒効果2)について述べる。消毒薬浸漬用パイ
プに入れる前の肉眼的な有機物の除去には塩化ベンザル
コニウムを浸した綿花による軽い清拭を行なった。洗浄
前に認められた細菌が残らず消失した菌消失例は34例
中31例97.1%に達し、不変例は1例2.9%であ
った。HB肝炎ウイルスなどウイルスについての消毒効
果を見るために鼻出血患者に使用したファイバースコー
プを洗浄用パイプにて流水洗浄を行ない、挿入部先端の
潜血反応を見た2)ところ15例中14例93.3%が
3分間の洗浄で陰性となり、残り1例も1分間の洗浄を
追加することで陰性となった。Bondら3)はファイ
バースコープによるB型肝炎ウイルス感染の危険性の検
討のなかで潜血反応がHBs抗原検出の目安すなわちH
B肝炎ウイルス存在の目安となるとし、神木ら4)の提
唱した内視鏡消毒の基準でもHBs抗原測定の代用とし
て洗浄した液の潜血反応が陰性であることを挙げてい
る。これらの報告は消化管用のファイバースコープにつ
いてのものであるが、鼻咽喉ファイバースコープについ
ても当然同じことが言えるはずであるから、上記の潜血
反応の結果から鼻咽喉ファイバースコープに付着した肝
炎ウイルスについても消毒効果が期待できる。また消毒
薬浸漬用パイプにいれる消毒薬の選び方によってはこれ
によるウイルスの消毒効果も期待できることになる。2
%グルタルアルデヒドを使用する場合を考えてみると、
一般に食器に付いたウイルスの消毒には60分を要する
とされる5)が、大倉ら6)の報告によればHBs抗原
をグルタルアルデヒドと反応させると0.5%以上のグ
ルタルアルデヒドでは3分以内に抗原の失活が見られた
としており、流水洗浄の効果と合わせることによりウイ
ルスの消毒効果も多いに期待できると考えられる。なお
消毒薬浸漬用パイプにいれる消毒薬は、消毒器を使用す
る医師の選択に任せるものであるからその浸漬時間も薬
剤により適宜選択するべきものと考える。消毒薬に浸漬
する場合、ファイバースコープに残存すると鼻・咽頭・
喉頭粘膜に有害な薬品もある。グルタルアルデヒドはそ
の代表的なものであり、強力な蛋白凝固作用により触れ
た粘膜の損傷を生ずる7)。そこでファイバースコープ
を消毒薬浸漬用パイプにて2%グルタルアルデヒドに3
分間浸漬した後、洗浄用パイプにて3分間洗浄すること
によりグルタルアルデヒドが残存するか否かを検討8)
した。その結果、挿入部分の平滑性が失われている老朽
化したファイバースコープ以外では1分以内の洗浄でグ
ルタルアルデヒドは2ppm以下という問題とならない
量となった。他の粘膜毒性のある消毒薬(グルコン酸ク
ロルヘキシジンなど)を使用する場合について考える
と、一般に手術用器具を浸漬した場合も水洗いを行なっ
て使用することで問題がないことから、消毒薬浸漬用パ
イプから出した後に洗浄用パイプにて洗浄を行なうこと
で、消毒薬の人体への影響は除けるものと考える。これ
らの結果からこの鼻咽喉ファイバースコープ消毒器は細
菌学的、ウイルス学的に十分な消毒効果が期待でき、使
用した消毒薬の洗浄効果も合わせ持っているといえ
る。。

の流入口(4)を設け、ホース(またはパイプ)(5)
にて水道栓に接続する。 (ニ)上記の流水洗浄装置に加え、消毒液を貯留し、鼻
咽喉ファイバースコープの挿入部分全体を浸漬できる消
毒薬浸漬用の底付きパイプ(6)とファイバースコープ
保持部分を固定する固定具(7)を備えている。 以上のごとく構成された簡易型鼻咽喉ファイバースコー
プ消毒器。(図1)
