本発明は、少なくとも1種の診断薬または治療薬を炎症性または感染性病変部に
到達させるキメラ抗体とその使用方法とに係わる。キメラ抗体は、ヒト顆粒球上
のエピトープに特異的に結合する抗原結合可変領域と、ヒト単核リンパ球上のレ
セプターに高い親和性を示すFc部分を有する定常領域とを含む、このようなキ
メラ抗体を、感染または炎症部位に到達させるために適当な診断ラベルまたは治
療薬に結合する。
潜在感染及び炎症を検出するために顆粒球を放射性核種で標識することの重要性
はここ数年にわたり理解されている。顆粒球、即ち単核血球及び血小板は、種々
のIn−111塩または単純なIn−111有機リガンドと一緒にインキュベー
トすることにより標識されている。この方法では標識前に血液から細胞を単離す
ることが必要であり、これは労力を要するとともに時間もかかる。
より最近では、前標識顆粒球の代替として、放射性標識ネズミ抗顆粒球抗体もま
た潜在感染を画像化するのに有効であることが示されている。標識抗体は血管系
に直接注入することができ、労力を要する細胞単離作業の必要は排除される。参
照により本明細書の一部を構成するものとする米国特許第4.634.586号
(Goodwin etal、)には、患者に、抗白血球単−特異性抗体とγ放
射金属キレートの免疫反応性非白血球結合体を注入し、結合体が白血球上に局在
するのを待ち、結合体に対する抗体を患者に注入して局在しなかったバックグラ
ウンド結合体を血液から除去し、シンチレーションスキャンニングによって白血
球を可視化することにより、白血球を放射性免疫画像化(radioimmun
oimage)する。
放射性標識ヒトポリクローナルIgGを使用して潜在感染及び炎症を画像化し得
ることもよく知られている。これらの抗体は、部分集団をなす放射性標識IgG
のFc部分が疾患部位に存在する単核細胞と相互作用することによりかかる部位
に局在するらしい。
潜在感染または炎症を検出するために顆粒球に対する抗体を最適に使用すること
を制限する問題点は、疾患が進行するにつれて病変部の顆粒球集団が減少し、高
親和性Fcレセプターを担うと思われる単核リンパ球(MLC)、即ち単球、T
細胞、B4a胞及び/または非特異的キラー細胞(NK細胞)によって置き換え
られることである。顆粒球対MLCの比は、病変を惹起する感染因子のタイプの
関数として著しく変化し得る。従って抗顆粒球抗体は、感染または炎症のより初
期の急性段階ではなくて後期段階を画像化したり、他の理由により顆粒球レベル
が低い病変を画像化するには有効でない。
更に、非特異的IgGは所定のタイプの癌において局在しく例えばRubin

Med、、321:935−940.1989参照)、本発明の結合体の顆粒球
特異性は癌性病変と非癌性病変とをより明確に区別し得ることが判明している。
白血球の画像化は、従来技術においてはターゲット対バックグラウンド比が乏し
いが故に厳しく制限されていた。例えば第2抗体クリアランスを使用することに
より、局在比は増大し得ることが判明している。しかしながら、各到達用抗体が
通常は特定の白血球タイプ、即ち顆粒球、単球、Bリンパ球またはTリンパ球の
いずれかに結合するため、抗白血球抗体の混合物を使用する場合にはターゲット
対バックグラウンド比が依然として問題である。つまり、特定の白血球に高度に
反応性であり且つ特異性を示す抗体は、その特定の白血球タイプが感染または炎
症部位に有意なレベルで存在しないと、その多くがターゲ・ント部位に結合しな
いでバックグラウンド中に存在することになる。
上記問題に対する1つの解決策は、本出願人らの同時係属及び一般譲渡特許出願
であるHansenらの1988年7月29日付は米国特許第07./226,
180号に開示されており、この特許明細書は参照により本明細書の一部を構成
するものとする。この明細書は、1種以上の白血球に親和性を示す多特異性抗体
結合体(polyspecific antibody composite

nj ugate)を開示している。かかる結合体を用いて到達される診断また
は治療薬は、病変部位において優勢を占める特定タイプの白血球に対してより少
ない依存性で病変部に局在する。
上述の問題に対する別の解決策または改善策が必要とされている。

本発明の1つの目的は、顆粒球と更に異なるタイプのリンパ球とに選択的に結合
し、炎症または感染性病変部をり−ゲットとするのに使用し得るキメラ抗体−作
用物質結合体を提供することである。
本発明の別の目的は、増強されたターゲット対バックグラウンド比で、診断また
は治療薬を感染または炎症性病変部に到達させる方法を提供することである。
本発明の他の目的は、以下の説明によって当業者にはより明らかとなろう。

本発明の上記目的及び他の目的は、顆粒球に特異的に結合する抗原結合超可変額
域と、ヒト単核リンパ球上のレセプターに高い親和性を示すFc部分を有するヒ
ト免疫グロブリンの定常領域とを有する組換えキメラを含む、白血球癒着巣をタ
ーゲットとするためのキメラ抗体−作用物質結合体であって、前記キメラが少な
くとも1種の診断薬または治療薬に結合されている結合体を提供することにより
達成される。
本発明は更に、哺乳動物に、到達させるのに有効な量の1種以上の前記キメラ抗
体−作用物質結合体を非経口的に注入することからなる、診断または治療薬を炎
症または感染性病変部に到達させる方法をも提供する。
更に本発明は、上記方法を実施する上で使用される無菌注射用製剤及びキットを
も提供する。

本発明は、白血球癒着巣をターゲットとするないしそれに到達するためのキメラ
抗体−作用物質結合体を使用することで、Goodwinらの従来の画像処理法
を改良し、またHanseらの゛ 180号の方法を拡張する。
感染または炎症性病変の発症に関与するT細胞、B細胞。
顆粒球及び単球は、病変を発症させる因子の特性及び/または病変の期間に従っ
て、炎症または怒染性病変中に著しく異なる比で存在することが多い、単一特異
性抗体を使用すると、G o o d w i nによって教示されているよう
に、病変部位にある白血球集団の一部しか到達用抗体に結合しないので到達は無
効となり、ターゲット対バックグラウンド比(所謂“局在比”)は小さくなる8
種々の白血球特異性を有する抗体の混合物を使用すると、特異性の比が正しけれ
ば、注入量のうちターゲット部位に到達する割合はわずかに向上し得るが、病変
が主に単一の白血球タイプを含むならば非ターゲット白血球への結合も更に増大
する。
本発明は、少なくとも2種の異なる白血球タイプに結合し得るキメラ到連用抗体
を使用することにより、上記ジレンマを解消する。抗体−薬剤結合体のイメージ
ング剤(1maging agent)成分は、種々のタイプの白血球の比にか
かわらずより高い効率で且つ高いターゲット対バックグラウンド比で、ターゲッ
ト部位に局在する。
本発明の抗体−薬剤結合体のキメラ到連用抗体成分は、これまでに当分界におい
てよく知られている多数の種々の方法によって製造することができる。
抗顆粒球モノクローナル抗体(Mab)はヒト以外の哺乳動物、例えばマウス、
ラット、ウサギ、ヤギなどにおいて産生され得、かかるMabの超可変ドメイン
の全部、好ましくは必須抗原結合領域のみをコードする単離DNAを、ヒト免疫
グロブリンの残りの部分をコードするDNA中に組込むことができる。このよう
なキメラヒトMabの製造は、哺乳動物MabのHiill及びLM可変領域の
一方または両方の全部または一部をコードするDNA配列を切り出し、それを公
知の方法で、その部分を欠くヒト免疫グロブリンDNA中に導入することを含む

顆粒球エピトープを認識し結合するMabは市販されてもいるし、最初はKoh

こともできる。ヒト顆粒球抗原に対する抗体は、宿主、例えばマウス、ウサギ、
ヤギ、ヒトまたは他の哺乳動物種に、ヒト顆粒球またはそれらから調製した膜調
製物を接種することにより製造することができる。免疫宿主から婢細胞を採取し
、上述の体細胞ハイブリダイゼーション法と使用して適当な骨mm細胞系と融合
し、抗顆粒球抗体を産生ずるハイブリドーマを製造する。かかるハイブリドーマ
を単離し、サブクローニングし、培養して、モノクローナル抗体を製造すること
ができる。

USAを含み世界的に展開している)のカタログは市販の抗顆粒球Mabを列挙
しており、それらの多くが、本発明のキメラを製造する際の使用に適している。
所定の抗体は、1種以上のタイプの白血球、例えば単球と顆粒球、B4aIm!
1と顆粒球、T細胞と顆粒球に共通なエピトープに結合する。
Immunotechによって製造・販売されて〜旭る抗体は、他から入手可能
なりローン由来の他の抗体と類似である。市販の抗顆粒球Mabは典型的にはネ
ズミまたはう・ソト由来のものであって、典型的にはIgGまたはIgMである
が1本発明の結合体を製造する際の使用に適した抗体は、種またはIgクラスに
関しては制限されな(1゜所定の抗T細胞抗体、特に単球及び顆粒球抗原の両方
に結合する抗体、即ちCDW14抗原に結合するモノクローナルは本発明に有効
である。CD抗原は、特定の白血球特異性を有する抗体を規定する白血球決定基
である。同じCD抗原上の同じエピトープに結合する1対の抗体は、同じ白血球
タイプに結合するのをクロスプロ・ンクする。顆粒球と、例えばT細胞、Bm胞
、NK細胞及び/または単球とに共通な抗原に特異的に結合する抗体の超可変領
域を使用し、多特異性キメラを製造し得ると考えられる。しかしながら、単一キ
メラ中に顆粒球(及び必要によっては単球、T細胞、B細胞及び/またはNK細
胞)をターゲ・ントとする抗体由来の特異的抗原結合領域と、MLC上のレセプ
ターに高い親和性3示すFc部分またはそのタンパク質断片とを組込むことの原
理は同じである。得られた到達用キメラは、単一到達用分子中の複数結合特異性
の故に、更に多種で且つ可変量の顆粒球を含む病変部がターゲントとされるが故
に、怒染または炎症の巣により効果的に到達するだけでなく、病変部位近傍の正
常組織中に白血球が存在するために非病変バックグラウンドに保持される、到達
用結合体の注入量のうちの割合は、より低い。
上述の組換えキメラのために遺伝子のより小さな部分を切り出し、顆粒球に対し
て有志な結合親和性を保有するセグメントとMLC上のレセプターに対して高い
親和性を示すセグメントとを組み合わせ、キメラと実質的に等価な到達機能を有
するより小さなタンパク質として発現させることも当技術内にある。このような
“整形(trimmed)”キメラタンパク質も、本発明において使用される“
キメラ抗体”なる用語の範囲内に含まれる。
顆粒球及び他の白血球に高い結合親和性を示す免疫グロブリン様タンパク質フラ
グメントをコードする遺伝子セグメントを、ヒト免疫グロブリンイディオタイプ
またはイソタイプの1つ以上のレセプター結合Fcタンパク質配列をコードする
セグメントと一緒に、診断及び/または治療薬を怒染または炎症の巣に運搬する
多特異性(即ち三種以上の特異性)到達用分子の発現を誘導する単一組換えDN
A中に組込むことができる。
キメラを製造するのに使用されるDNAは、例えば蛍光免疫測定法によって試験
すると、ヒトMLC上のレセプターに対して窩い親和性を示す免疫グロブリンを
産生ずることが知られている任意のヒト免疫グロブリン産生細胞由来のものとす
ることができる。MLCに対して高い親和性を示すポリクローナルヒト抗血清が

な抗血清は、まず結合MLCを含むアフィニティーカラムを通し、次いで回収及
び単離することにより更に精製することができる。
本発明のキメラ“ヒト”または“ヒト化(humanized)”抗体の使用は
、所定のヒト免疫グロブリンイソタイプのFc部分上に、ヒト単核リンパ球の所
定の集団または部分集団における“Fcレセプター”領域に高い結合親和性を示
す領域が存在することがきっかけとなっている。
理想的には、全ヒトモノクローナル抗顆粒球抗体がこの目的に有用であるが、当
技術の現状では、安定ヒトモノクローナル抗体産生細胞系の製造は難しく、信頼
性のないことが多い、しかしながら将来の開発によってこのようなヒトモノクロ
ーナルの製造が可能になりしかも信頼性がおけるようにもなり得ると、このよう
なヒトモノクローナルは明らかに本発明の結合体の製造のためのキメラと等価と
考えられる。
ヒトMabを製造する1つの方法によれば、全ヒト顆粒球または好ましくはこの
ような細胞由来の膜成分フラクションのいずれかを、ヒトリンパ球を用いて再集
団構成(repopu Iated)しておいた重症複合免疫不全症(SCID
)のマウスに、マウス中に存在するヒトリンパ球を高度免疫(hyperimm
unize)するために導入する0例えばMo5ier et al、Natu
re。
335 : 256−9 (1988)を参照されたい。次いで高度免疫細胞を
単離し、例えばEpste i n−Barrウィルス(EBV)感染によって
無限増殖細胞として培養し、次いでヒト骨髄腫細胞に融合してハイブリドーマと
製造した0例えばJames et al、、J、Immunol、Meth、
、100:5 40(1987)参照。
使用してスクリーニングし、ヒト顆粒球に結合する抗体を産生ずるハイブリドー
マを選択する。一旦有効量の抗顆粒球Mabを産生ずるハイブリドーマを選択し
たならば、医薬的に容認可能なMabを公知の方法によって精製及び回収するた
めの資源として培養液上清を使用する。このような方法またはその改変法によっ
て安定なヒトMabが製造され得る限りは、それは本発明の結合体の適当な到達
用成分となろう。
本発明のキメラ結合体を製造するのに使用される抗体の免疫学的特性は、感染ま
たは炎症性病変への結合が最適で且つ非ターゲット部位への結合が最小となるこ
とを保証するよう調整することができる。結合体が投入される診断または治療用
途に従って、白血球タイプの特異性、抗原特異性、及び特定の細胞タイプに存在
する抗原上のエピトープに対する特異性、並びにターゲット抗原及び/または細
胞タイプに対する結合定数は、全てが、本発明の試薬の選択性及び到達効率をF
R調整するのに使用することができる。
これは、患者のコストと、放射性免疫療法においては重要な配慮事項である患者
が受けねばならない放射線量とを最小化する上で有利である。
一般には、比較的高い免疫反応性、即ち約10’11モル以上、好ましくは10
’11モル以上の結合定数と、顆粒球エピトープに対して高い免疫特異性、即ち
約40%、好ましくは約60%以上、より好ましくは約70〜95%以上の免疫
特異性とを有するキメラ抗体を使用することが望ましい。
所定の用途に対しては、本発明において幾分低い結合定数を有する抗体を使用す
ることが望ましくなり得る。高い結合定数を有する抗体は、炎症または感染部位
にある白血球のみならず、循環系、骨髄または正常組織中に存在する白血球にも
しっかりと結合し易い、他方で、より低い結合定数を有する抗体は、一種の集団
効果(mass action effect)によって病変部位にある白血球
タイプに主に癒着する傾向にある。これによって、イメージングラベル及び後述
する治療用途においては治療薬の時期尚早のクリアランス及び非ターゲット部位
への癒着が削減され、従って病変部に到達させるのに有効な量が増大される6
本発明のキメラ抗体結合体は、例えば骨髄炎の診断及び治療のためには単球及び
顆粒球、慢性感染の診断及び治療のためにはT細胞、B細胞、単球または顆粒球
、原因不明の発熱、例えば肉芽腫性感染、結節性病変、真菌性感染などの治療の
ためにはIa(dr)組織適合性抗原及び顆粒球に有利に到達し得る。
ターゲット部位は、比較的濃度の高い集中に存在する任意の感染性病変、炎症性
沈着物または白血球を有する潜在琳物曙結合体の、非経口投与した時点に病変部
に存在する白血球との反応性、及びIIm白血球の病変部への進入によって直接
に行われる。
キメラ到連用抗体は、診断イメージング剤として使用するための、シンチグラフ
イックイメージング用の放射性同位元素または磁気共鳴像増強剤で標識すること
もできるし、それに結合するかまたは結合に適合させたりすることもできる。適
当なイメージング放射性同位元素としては例えばきる、抗体をW識するのに適し
ており且つ都合の良い放射性同位元素としては、限定的ではないが、ヨウ素−1
31、テクネチウム−99m及びフッ素−18を挙げることができる。
in vivoで使用するためにタンパク質を標識するのに適した通常の放射性
標識方法はいずれも一般にキメラを標識するのに適している。これは、通常の技
術またはより精巧な方法を使用してハロゲンまたは金属イオンの放射性同位元素
で直接標識するか、または放射性金属または常磁性イオンのためのキレート化剤
を結合させることにより行なうことができる。かかるキレート化剤及びそれを抗
体に結合させる態様は当業者には公知であり、特に、例えばChilds et
al、、J、Nuc、Med、、26 : 293 (1985)並びにGo
ldenbergの米国特許第4.331,647号、第4.348.376号
、第4.361.544号、第4.468.457号、第4,444.744号
及び第4.624.846号に記載されている。
典型的なものは、エチレンジアミン四散(EDTA)及びジエチレントリアミン
五酢酸(DPTA>の誘導体である。
これらは典型的には、キレ−1・止剤が抗体に結合し得る基を側頭上に有する。
或いは、キレート止剤上のカルボキシまたはアミン基を活性化し、次いでそれを
公知の方法でキメラ抗体に結合することもできる1例えばGa−67に対するキ
レート化剤であるデフェロキシアミンは遊離アミン基を有しており、このアミン
基を、活性化カルボキシル、イソチオシアネートなどの基を含むように適当なリ
ンカ−によって活性化し、次いで、抗体上のアミンに結合することができる。
キレート化剤はキメラ抗体に、直接にまたは短いもしくは長いリンカ−鎖部分を
介して、抗体上の1つ以上の官能基、例えばアミン、カルボキシル、フェニル、
チオールまたはヒドキンル基によって結合することができる0例えばジイソシア
ネート、ジイソチオシアネート、カルボジイミド、ビス−ヒドロキシスクシンイ
ミドエステル、マレイミド−ヒドロキシスクシンイミドエステル、グルタルアル
デヒドなどの種々の通常のリンカ−を使用することができるが、米国特許第4,
680,338号に記載の無水物−インチオシアネートリンカーのような順次選
択リンカ−が好ましい。
ヨウ素−131([131)またはヨウ素−123(I−123)のいずれかに
よる標識は、放射性ヨウ化カリウムまたはヨウ化ナトリウムと抗体の混合物をク
ロロアミン−Tを用いて、例えばGreenwood et al。
Biochem、J、、89.114 (1963>に報告されており且つMc
Conahey et al、IntArch、AIlergy Appl、r
mmunol、。
29.185(1969)によって改変されているように処理する酸化方法を使
用して容易に行われる。この結果、抗体分子上、即ち試薬の割合と反応条件とに
従って恐らくはチロシン残基上、場合によってはトリプトファン上、或いはフェ
ニルアラニン残基上の水素原子がヨウ素原子で直接1き換えられる。或いは、F
eteanu、前出、page303及びそこに引用されている参考文献によっ
て記載されているように、ラクトペルオキシダーゼヨウ素化を使用することもで
きる。
標識の更に高度の方法は、係属米国特許出願第742゜436号(6−7−85
)、第084,544号(8−12−87>及び第176.421号(4−18
8)に記載されている。前記特許及び特許出願の全ての開示内容は参照により本
明細嘗の一部を構成するものとする。広範の標識技術がFeteanu、Lab

る。種々の金属放射性同位元素の導入は、Wagner et al、J、Nu
c 1.Med、、20,428 (1979);Sundberg et a
l、、J、Med、Cheml、17.1304 (1974);及び5aha
et al、、J、Nuc 1.Med、、6.542 (1976)の方法
に従って行なうことができる。これらは単に、当分野には公知のタンパク質を標
識する多数の方法の例示にすぎない。
MRI像増強剤に有効な化合物の例としては、常磁性イオン、例えばGd (■
11) 、Eu (III) 、Dy (DI)、Pr (III) 、Pa
(IV) 、Mn (II) 、Cr (III)、Co (III) + F
e (III) 、Cu (II) 、Ni (■■)、Tj(IN)及びV(
IV)イオン、またはラジカル、例えばニトロオキシドを挙げることができ、こ
れらを、常磁性イオンキレート化剤を担う基質に結合するか、またはイオンに対
しては例えばSH,Nl2、C0OHといったキレート化官能基、ラジカル付加
体(radical addends)に対してはリンカ−に暴露する。MHI
増強剤は、合理的に達成し得て且つ患者の安全性及び器具の態様に適合し得る磁
場強度を使用し、外部カメラによって検出し得るのに十分な量で存在せねばなら
ない、このような薬剤に要求されることは、媒体中の水分子に影響を及ぼす薬剤
の分野においてよく知られており、特に、例えばPykett、5cienti
fic American、246 : 78 (1982):及びRunge
et al、。
Am、J、Radiol、、14ユニ1209 (1987)に記載されている
。
抗体中に直接にまたはキレートの形態で金属を導入する同じ方法の多くは、本発
明のキメラ抗体中にMRI剤を導入して、感染病変のためのイメージング剤を形
成するのに適していることはよく理解される6MRI剤は、画像化増強のために
は多数の常磁性イオンまたはラジカルと有するのが有利である。多数のこのよう
なイオンを導入する1つの方法は、担体ポリマーにキレートを付加し、担体を抗
体に、好ましくはキメラの抗原結合部位及びFcレセプター結合部位から遠く離
れたところに部位特異的に結合する。
これは、より多数のキレート化剤がより少ない部位で抗体に結合し得、従って免
疫反応性が大幅には弱められないという利点と有する。抗体にキレート化剤を付
加するのに有効なポリマーの例としては、例えばポリオール、多糖想、ポリペプ
チドなどを挙げることができる。米国特許第4゜699.784号(Sh i
nら)及び第4.046.722号(Rowland)を参照されたい、1つの
タイプの多糖想はデキストランである。キレート化剤は、デキストランヒドロキ
シル、例えば無水物、インシアネートまたはインチオシアネートなどに対する反
応基を含むように官能化することができる。或いは、例えばアミノデキストラン
に変換するなど多数の方法で、デキストランを誘導することができる。同様の方
法が多数の薬剤分子を抗体または抗体結合体上に付加するのに有効であることを
理解されたい。
これについてはあとで十分に検討する。
アミノデキストラン(AD)担体を含む抗体結合体を製造する方法は一般にデキ
ストランポリマー、有利には平均分子量(MW)が約10.000〜100,0
00.好ましくは約10.000〜40,000、より好ましく約15.000
のデキストランから出発する。次いでデキストランを酸化剤と反応させ、その炭
水化物環の一部を調節酸化してアルデヒド基を生成する0wi化は、解糖化学試
薬、例えばNal0,5:用い、通常の方法に従って通常に行なう。
酸化剤の量は、MWが約40,000のデキストランに対して約50〜150個
、好ましくは100個のアルデヒド基が生成され、他のMWのデキストランに対
してもおおよそ同じ割合のアルデヒド基が生成されるように調整するのが都合が
良い。より多くのアルデヒド基、ひいてはアミン基は、ポリマーがよりポリリシ
ンのように振舞うためやや有利でない、より少ないと、キレート化剤またはホウ
素付加体が付加されるというやや望ましくない結果となり、これは不利となり得
る。
次いで酸化デキストランをポリアミン、好ましくはジアミン、より好ましくはモ
ノまたはポリヒドロキシジアミンと反応させる。適当なアミンとしては、例えば
エチレンジアミン、プロピレンジアミンまたは想縁ポリメチルエンジアミン、ジ
エチレントリアミンなどのポリアミン、1.3−ジアミノ−2−ヒドロキシプロ
パンなどのヒドロキシル化ジアミンまたはポリアミンなどと挙げることができる
。
アルデヒド官能基をシック塩基(イミン)基に実質的に完全に変換するのを保証
するために、アルデヒド基に対して過剰なアミンを使用することができる。
シッフ塩基中間体を例えばN a B H4、N a B H) CNなどの還
元剤と反応させることにより、得られた中間体を還元安定化する。イミン基を第
2級アミン基へ実質的に完全に還元し且つ全ての未反応アルデヒド基をヒドロキ
シル基へ還元するのを保証するために、過剰な還元剤を使用する。
得られた付加物を、通常のサイズ排除カラムに通して架橋デキストランを除去す
ることにより更に精製することができる。AD上の第1次の使用可能なアミノ基
の数は、計量試料をトリニトロベンゼンスルホン酸と反応させ、420nmにお
ける光密度を標準と相関させることによって推定することができる。上記方法は
一般に、推定数のアルデヒド基をAD上の第1級アミン基へ実質的に完全に変換
することになる。
或いは、例えば臭化シアンと反応させ、次いでジアミンと反応させることでアミ
ン基を導入するための通常の方法により、デキストランを誘導することができる
。ADは、通常の手段、例えばジシクロへキシルカルボジイミド(DCC)また
はその種々の水溶性形態を使用することにより、調製された活性化形態の特定の
薬物またはキレート化剤の誘導体、好ましくはカルボキシル活性化誘導体と反応
させるべきである。
本発明のシンチグラフイックイメージング法は、哺乳動物、好ましくはヒトに、
シンチグラフイックイメージングに有効な量の放射性標識キメラ抗白血球抗体を
非経口的に注射することにより実現される。非経口的とは、例えば靜る。患者は
、1回の投与で約1mC1〜50mC1の放射性W4識結合体を受容するが、こ
の量は、特定の放射性同位元素及び投与の形態の関数である。適当なγ放射同位
体としては例えばl−131、l−123、Tc 99m、■n−111及びG
a−67を挙げることができるが、これらは50〜500KeVを放射する放射
性同位元素の例である。静脈内注射においては、>131の量は通常は約2〜1
0mCi 、好ましくは約2〜5mCiであり、丁−123の量は通常は約5〜
10mCi 、好ましくは約8mC1であり、Tc−99mの量は通常は約10
〜40mC1、好ましくは約20mC1であり、In−111またはGa−67
の量は通常は約2〜5mC1,好ましくは約4mC1である。
放射性標識キメラ抗白血球抗体は、シンチグラフイックイメージング割分、白血
球を含む感染または炎症性病変部に到達させるための、医薬的に容認可能な無菌
注射ビヒクル、好ましくは生理学的PH及び濃度のリンBM衝溶液(PBS)中
に有効量の放射性標識組成物を含む、哺乳動物用途のための注射用裂開、好まし
くはヒト用途のための無菌注射用製剤として都合良く提供される。非経口投与の
部位の要求に応じて、他の通常の医薬的に容認可能なビヒクルを使用することも
できる。
本発明の非経口的に投与される代表的な製剤は、有利には例えば09%塩化ナト
リウムを含む0.04Mリン61衝液(pH7,4Bioware)1ml当た
り約10mgのヒト血清アルブミン(1%USP : Parke−Davis
)を含む無菌液中に、通常は約01〜20mg、好ましくは約2mgの放射性キ
メラ抗体を含む。
一旦十分な同位体がターゲット部位に蓄積したならば、通常のプラナ−及び/ま
たは5PECTγカメラを用いて、も内部的にも使用される手支持γプローブ3
使用することにより走査を行なう。シンチグラムは通常は、50〜500 K、
e Vのエネルギーを検出するために1つ以上のウィンドウP有するγイメー
ジングカメラによって撮影される。
高エネルギーのβまたは陽電子を放射する放射性同位元素を使用することも可能
であり、そうならば適当な検出装Wを備えたイメージングカメラを使用すること
になるが、これら全ては当分野においては一般的である。
磁気共鳴映像法(MRI)は、イメージング剤が放射性同位元素ではなくてMR
I増強種を含む点を除いて、シンチグラフイックイメージングと頂似の方法にお
いて行なわれる。磁気共鳴現象はシンチグラフィーとは異なる原理で動作するこ
とは理解されたい。通常は、生成されるシグナルは、映像化されるべき領域中の
水分子の水素原子の核内にある陽子の磁気モーメントの緩和時間に相関する。磁
気共鳴像増強剤は、緩和の速度を増大し、それによって体内のイメージング剤が
癒着している領域中の水分子と他所の水分子との対比を高めることにより作用す
る。しかしながら、イメージング剤の作用はT、及びT2の両方を高めることに
なり、前者はより大きな対比ともたらすことになるが後者はより小さな対比3も
たらすことになる。従ってこの現象は濃度依存性であり、一般には最大効果に対
して最適な常磁性種濃度が存在する。最適濃度は、使用する特定のイメージング
剤、映像化の場所、映像化のモード、即ちスピン−エコー、飽和−回収、反転−
回収や、また種々の他の強力にT1依存性またはT2依存性のイメージング技術
に対しては、イメージング剤が溶解または懸濁される媒質の組成によって変化す
る。これらの因子及びそれらの相対的重要性は当分野において公知である。例え
ばPykett。

本発明のMRI法は、哺乳動物、好ましくはヒトに、磁気共鳴映像法に有効な量
の本発明のキメラ抗白血球抗体とMRI増強剤との結合体を非経口的に注射する
ことにより実現される。患者は、1回の投与で、病変部位においてMRIシグナ
ルを約20%以上、好ましくは50〜500%増強するのに十分な量の@識語合
体を受容するが、この量は、特定の常磁性種及び投与形態の関数であることが考
えられる。
ここでも、標識結合体は、白血球を含む感染または炎症性病変部にMRI剤を到
達させるための、医薬的に容認可能な無菌注射ビヒクル、好ましくは生理学的p
H及び濃度のリン酸緩衝溶液(PBS)中に有効量の放射性標諸組成物を含む、
哺乳動物用途のための注射用製剤、好ましくはヒト用途のための無菌注射用製剤
として都合良く提供される。非経口投与の部位の要求に応じて、他の通常の医薬
的に容認可能なビヒクルを使用することもできる。
本発明の非経口的に投与される代表的な製剤は、有利には例えば09%塩化ナト
リウムを含む0.04Mリン[新液(pH7,4,Bioware)1ml当た
り約10mgのヒト血清アルブミン(1%USP : Parke−Davis
)を含む無菌液中に、通常は約0.1〜20mg、好ましくは約2mgの標識キ
メラ抗体を含む、一旦十分なMRI剤がターゲット部位に蓄積したならば、通常
のMRrカメラご使用して走査を行ない、病変部分映像化する。
本発明の好ましい実施態様においては、第1擦識キメラ抗体−作用物質結合体の
局在比は、参照により本明細書の一部を構成するものとするGol、denbe
rgの米国特許第4.624.846号に記載の関備イメージング剤のごとく、
到達せずに循環している結合体を捕捉してそのクリアランスを促進する非標識第
2抗体を使用することにより増強される。この方法は、治療薬に結合させたキメ
ラ抗白血球抗体にも同様に適用可能であり、これについては後述する。′局在比
(localization ratiO)”なる用語は通常の意味、即ち到達
対非到達抗体結合体の比という意味で使用されている。一般に第2抗体は、第1
抗体結合体の局在比を約20%以上、典型的には50%以上増強する量で使用さ
れる。
第2抗体は、全IgGもしくはI、gM、または第1抗体結合体を結合して、第
1抗体だけよりも迅速に循環系及び非ターゲット場所から除去される結合物を形
成し得る限りは、IgGもしくはIgMのフラグメントとすることができるが、
第2抗体は全IgGまたはIgMであるのが好ましい。第1抗体がIgGまたは
rgMのフラグメントであplement cascade>を活性化する能力
を保有するように、第2抗体は全IgGまたはIgMであるのが好ましい。これ
とは逆に第1抗体が全IgGである場合、g)能力分保有するならば第2抗体は
フラグメントでもよい、第1/第2抗体対の少なくとも一方はIgGまたは1g
Mであることが好ましい。IgMを使用することの1つの利点は、それが第1抗
体または結合本分Ill物、即ち薬物、キレート化剤、放射性核種などの診断及
び/または治療薬とより高分子量の結合物を形成することである。これは、到達
しなかった第1抗体及び/または前記素の、特に血液からのクリアランスの速度
及び効率を高める。第2抗体は、前出のGoldenbergの°846号特許
に記載されている方法によって製造することができる。モノクローナル抗種Ig
Gもまた使用可能であり、本発明の方法における第2抗体として有利に使用され
る。非金属結合体、例えば放射性ヨウ素化結合基またはニトロキシドのような有
機常磁性種も、第2抗体が特異性を示すハプテンとすることができる。
第2抗体は、第1抗体−作用物質結合体の非経口投与のあと十分な時間がたって
その白血球による取込みが最大となってから、典型的には初期投与の2〜72時
闇後、好ましくは投与の約24〜48時間後に患者に注射する。第1抗体の投与
が静脈内でなかったならば、第2抗体の少なくとも一部は同じ非経口経路で投与
することが有利となり得る。しかしながら、循環系に拡散している第1抗体のク
リアランスを促進するために、第2抗体の少なくとも一部は静脈内に注入するこ
とが有利である。
循環している標識第1抗体を除去し、第1抗体の局在比を増強するための第2抗
体の使用は、前出のGoldenbe rgの特許及びそこに引用されている参
考文献に記載されているような画像増強減色法(image−enhancin
g 5ubtraction techniques)を使用することにより更
に増強される。これは、無関係の抗体またはフラグメントを、独立に検出し得る
放射性核種で標識する認定技術である。この抗体は、画像化に必要とされる時間
の間、第1抗体と実質的に同じ分散速度及び代謝を有する。このような抗体を注
入する方がTc−99m標識血清アルブミンのような通常の減色剤よりも好まし
いが、通常の減色剤も、バックグラウンドを補償することにより画像処理を増強
することに使用するのに適している。減色剤として無関係な標識抗体を使用する
と、循環系から抗体を除去する臓器からの非到達バックグラウンド放射に対する
補正を計算することができる。第1キメラ抗体及び減色剤として使用される無関
係抗体は、第2抗体が第1抗体及び無関係抗体の免疫グロブリンを非ターゲント
領域から実質的に同じ速度で除去するように、同じ種または骨髄腫/ハイブリド
ーマ由来のものであるのが好ましいことは当業者には理解されるであろう、更に
は、第2抗体は第1及び無関係免疫グロブリン種の定常領域に特異的であること
が好ましい。
第2抗体の導入量は一般に、第1抗体の循環量を2〜72時間以内に10〜85
%だけ減少させる量であろう。
クリアランスに影響を与える第2抗体と第1抗体との比は、対を成す第1抗体と
第2抗体との結合特性に左右されるであろう、適当な比の初期推定値を与えるた
めに、患者血液の予備的なin vitroスクリーニングを使用し得る。
スクリーンは、例えばゲル内拡散法で沈降素バンドを得るために必要な第2抗体
と第1抗体との比を決定するために使用されるであろう、これは、第2抗体と第
1抗体とのモル比のほぼ下限と見なされる概括的な範囲を示す、何故なら、in
vivo使用の場合の第2抗体と第1抗体とのモル比は、in vitroテ
ストで示された値よりも高い値を必要とするからである。
実際には、第2抗体と第1抗体とのモル比は概して約5〜50の範囲であろうが
、この範囲に限定されると考えてはならない、第1抗体及び第2抗体の双方が全
IgGの場合、第2抗体と第1抗体との有利なモル比は、15〜25、好ましく
は20〜25であることが判明した。
キメラ抗体は、ターゲットである感染部位または炎症部位に治療薬を到達させる
ために使用され得る。このようにして放射性同位体または薬剤/毒素をターゲッ
トに到達させ得る。適当な治療用放射性同位体は通常は、β放射体、α放射体及
び/またはオージェ−電子放射体であり、非限定例として、fR−67、ヨウ素
−125,ヨウ素−131、レニウム186、レニウム188、ビスマス−21
2、アスタチン211などがある。当業界で公知の技術によってイメージング用
結合体と同様の方法でこれらの放射性同位体との結合体を調製できる。これらの
結合体は、薬剤耐性であり且つ放射線療法を要する極めて急性且つ衰弱性の感染
症の治療に極めて有利に使用される0本発明のターゲット到達方法は、全身性治
療に比べて放射性医薬の治療係数を原著に増加させ、これまで容認されていなか
ったこの種の介入が容認される。
ヒトに感染し病変を生じさせる細胞または微生物に対して細胞毒性効果を有する
多くの薬剤が公知である。これらの薬剤は、Merck Indexのように当
業界で認知され容易に入手し得る薬剤及び毒素の目録に収録されている。かかる
抗生物質薬は、キメラ抗白血球抗体と結合して本発明の治療薬を形成する。また
、感染性病変部位における抗生物質薬の有効濃度を増加させるべく、この病変部
位ターゲットへの抗生物質薬の到達率を改良するためにかかる結合体を使用する
ことも本発明の一部である。治療目的で使用するためには、1種またはそれ以上
の抗生物質薬をポリマー担体に結合させ、次いで該担体をキメラ抗体に結合させ
る。いくつかの場合には、薬剤の全身性投与が許容されないときでも該薬剤の全
身性副作用を軽減することによってこの薬剤を使用できるようにするために、抗
体結合体の一部を成す薬剤の毒性を循環中に部分的または完全に除去することも
可能である。薬剤をポリマーに結合させ、該ポリマーを更に抗体に結合させて使
用すると、治療用の薬剤分子の投与量を増加でき、しかも全身性毒性は軽減され
る。
第1キメラ抗体を抗生物質薬に結合させることによって、本発明の方法論を感染
性病変の治療処置に応用できる。抗生物質薬をイムノグロブリンに結合させる当

ィルス、菌類及び種々の寄生虫のような種々の病因性微生物に対して有効な薬剤
を、これらの微生物に対する抗体、その産生物または夫々の病変と関連した抗原
に結合させるために使用される方法と全く同様である。
このような抗菌性、抗ウイルス性、抗寄生虫性、抗菌類性薬剤及び関連薬剤、例
えば、スルホンアミド、ペニシリン及びセファロスポリン、アミノグリコシド、
テトラサイクリン、クロラムフェニコール、ピペラジン、クロロキン、ジアミノ
ピリジン、メトロニアシト、イソニアシト、リフアンビン、ストレプトマイシン
、スルホン、エリスロマイシン、ポリミキシン、ナイスクチン、アンホテリシン
、5〜フルオロシトシン、5−ヨード−2°〜デオキシウリジン、1〜アダマン
タンアミン、アデニンアラビノシド、アマニチン及びアジドチミジン(AZT)
は、適当な特異的抗体/フラグメント及び抗体//フラグメント複合体(com
−posite)に好適に結合する0本発明で使用することが可能なその他の種
々の抗菌薬は、当業界の知見を概説したGoodman et al、、rTh

ゲット部位に薬剤を到達させるために望ましい適当な種々の条件は、例えばTr
ouet et al、、rTargetingof Drugs」、G、Gr
egoriadis etal、、eds、、PIenum Press、Ne
wYork and London、1982.PP、19〜30に概説されて
いる。この文献は、このようなターゲット療法が感染性病変に罹患した患者にと
ってどれほどの福音であるかに関する臨床経験を示している。
上記のごとき第2抗体の使用は、診断用イメージング結合体の場合と同様に本発
明の治療薬の有効性を増加させるであろう、治療薬の有効性は治療係数で表され
る。従来の治療係数なる用語は、治療効果と不都合な副作用との比であると定義
されている。治療係数はしばしば、標準モデル系における薬効対毒性の定量的測
定値であると定義され、例えば、50%致死量(L D s。)対50%有効量
(ED5゜)の比である0本文中に記載のごとき第2抗体の使用は、ターゲット
に到達しない第1抗体及び/または分離した治療用成分を除去することによって
抗白血球キメラ抗体結合体の治W!係数を向上させる。治療用製剤の場合には、
第2抗体が、上記のごとく第1のキメラ抗体に特異的であってもよく、治療薬に
特異的であってもよく、または、治療薬の担体に特異的であってもよい。
本文中で考察した治療用製剤は、上記のごとく定義したキメラ抗白血球抗体と該
抗体に結合した治療有効量の放射性同位体または薬剤とを、適当な非経口投与ビ
ヒクル中に含む、治療用製剤はまた、別個にパッケージされた上記のごとき第2
抗体を含む、適当なビヒクルは当業界でよく知られており、その例としては、上
記のごとき診断用イメージング剤の投与に使用される溶液と同様の滅菌PBS溶
液がある。
本発明のイメージング用及び治療用のキメラ抗白血球抗体結合体はまた好丈しく
は、白血球の巣を含む感染性または炎症性の病変ターゲットに抗体を到達させる
治療用または診断用キットとして提供される。このようなキットは典型的には、
本発明のキメラ抗体結合体を凍結乾燥製剤として収容しているかまたは注射用ビ
ヒクル中に収容しているバイアルから成る9シンチグラフイツクイメージングま
たはラジオイムノセラビーのために結合体を使用する場合には、結合体は一般に
、放射性標識用の試薬及び補助成分と共に、別の容器に入れた低温結合体として
福供される。また、MRI剤及び治療用薬剤の結合体は一般に、キメラ抗体に既
に結合した常磁性種または抗生物質と共に提供されるであろう6キツトは更に、
キメラ抗体または治療薬に特異的な非標識の第2抗体もしくは抗体フラグメント
、その担体、または、放射性核種もしくは常磁性イオンのキレート剤を別個のパ
ッケージとして含んでもよい。
本発明のイメージング剤及びイメージング方法は、In−111−標識白血球と
使用した従来の潜在性膿瘍診断用薬剤と少なくとも同程度に有効であり、コスト
、薬剤の潜在的毒性、使用容易性、及び最も重要なターゲット特異性増加に間し
ては従来の薬剤を明らかに凌駕している0本発明の治療薬及び治療方法は、放射
性同位体または薬剤の治療係数を改良するために、微生物感染したターゲット部
位に放射性同位体または抗生物質薬を到達させる手段を提供する。
更に詳細に説明しなくても、上記の記載に基づいて、当業者が本発明を十分に利
用し得ることは理解されよう、従って、以下の好ましい特定実施例は、本発明の
代表例であって本明細書の記載がこれらの実施例に限定されないことを理解され
たい、以下の実施例において、特に注釈がない限り、温度はすべて非補正の℃で
あり、部及びパーセンテージはすべて重量部または重量%である。

Morrison他、前出、の方法を用い、顆粒球に高度に特異的なマウス(m
u r i ne)モノクローナル抗体を産生ずるハイブリドーマから単離した
DNAと、ヒト単球のFcレリセブターに高い結合親和性を有するFc部分を有
するヒトI gG、をコードするDNAとから組換えキメラ抗体を調製する。
キメラを大腸菌細胞中で発現させ、結合顆粒球と入れたアフィニティカラムで単
離する。
キメラ抗体を任意に、放射性同位体またはMRI増強剤に対するキレート剤に結
合させるか、または、かかるキレート剤または薬剤分子を抗体に結合させる補助
試薬によって処理する。

シン ブーツインクイメージング ブト診断用イメージングキットは、ゴム隔壁
を備えており実施例1のキメラ抗体溶液の凍結乾燥物を入れた第1の滅菌バイア
ルを含み、また任意に、局在化後に循環する標識複合体を速やかに除去する第2
抗体、例えばアフイニティ精製ウサギ抗ヒトIgGの溶液を入れた第2の滅菌(
隔壁密封)バイアルを含み、更に、例えばl−123、In−111またはTc
−99m−ペルテクネテートでS識した標識結合体を注射するための追加の隔壁
密封滅菌バイアル及び滅菌注射器を含む。

餐 イメージング
1週間前に帝王切開で女児を出産した30歳の婦人患者が発熱及び腹痛の症状を
示した。患者に対して抗生物質靜注泊療を2週間続けたが、熱及び腹痛は続いて
いた。CATスキャンでは異常集団(mass)の表示に成功しなかった。従来
の放射性ヨード化及びリアクター産生ヨウ化ナトリウムに特に適応した実施例2
のキット成分を使用し、l−123放射性同位体で直接標識した実施例1のキメ
ラ抗顆粒球抗体を使用してイムノシンチグラフィー試験を行なった。20mC1
の放射性標識キメラを注射し、患者をγカメラで5PECTモードでスキャンし
た。!!者の腹部のスキャンによって、r−123の蓄積巣が表示された。
外科手術を行なって、l−123活性の部位に膿瘍を発見した。膿瘍をドレーン
して病理状態を観察すると、化膿性物質中に多数の顆粒球、単球、Bリンパ球及
び活性化Tリンパ球が存在していた。2日後に患者の熱及び痛みが軽快した。

° イメージング
萎縮性腎炎の治療を受けた62歳の男性患者が発熱及び急性を髄圧痛の症状を示
した。を椎骨の骨を髄炎の疑いがあったが、を椎のレントゲン写真は正常であっ
た。キメラ抗体をメタロチオネイン末端ペプチドに結合させ塩化第1スズ及びゼ
ネレータ産生ナトリウムペルテクネテートで前処理した実施例2のキット成分を
使用し、Tc−99m放射性同位体で直接標識した実施例1のキメラ抗顆粒球抗
体を使用してイムノシンチログラフィー検査を行なった。
20mC1の放射性キメラを注射し、標識複合体の投与24時間後に患者をガン
マカメラで平面イメージングモードでスキャンした。このスキャンで、を髄の尾
端の直ぐ上にTc−99mの集中巣が表示された。ターゲット部位で椎弓切除及
び硬膜外腔のドレーンを行なった。ドレーン流体中の化膿性物質の病理状態を観
察すると、多数の単核リンパ球と少数の顆粒球とを含む主として浸出性の肉芽組
織が観察された。
Gd(III)イオンをロードした平均100個の改質ジエチレントリアミン五
酢酸(DTPA)キレート剤と結合したアミノデキストランに結合させた同じキ
メラを使用して、MRIスキャンを行なうと、炎症病巣が明確な輪郭の硬膜外領
域として表示された。

42歳の男性エイズ患者が左右対称肺炎に罹患していたが、この肺炎は従来の広
スペクトル抗生物質治療には反応l甜4ズ
しなかった、臨床&!及び喀痰細胞誇は、患者が重症の呼吸困難を生じさせる進
行したニューモシスチス・カリニ肺炎Pneumocystis carini
i pneumoniaであることを示唆した。前出の5hin他の特許に記載
の方法論を応用し、顆粒球抗原及び単球のFcレセプターに結合する実施例1の
キメラ抗体を、平均約25分子のトリメトプリン(We 11 c one )
を担持し分子量約15.000MWe有する平均1個のアミノデキストラン担体
に部位特異的に結合させ、メトトレキセート誘導体を担体に結合させた。約25
mgの抗体結合体をゆっくりと靜注し、治療用量の抗生物質を肺病変にデリバリ
−し、この処置を3日間継続した。2日後に、患者の症状が軽減され、熱も下が
った。これは、3日後の胸部X線写真の改善によって確認された。はとんどの肺
炎が抗体薬剤結合体投与の2週間以内に解決され、この期間中に患者には総合的
支持療法を与える。
上記実施例に使用した反応体及び/または処理条件に代替して、本文中に包括的
または特殊的に記載した本発明の反応体及び/または処理条件を使用した実施例
でも上記と同様の結果が得られる。
当業者は、上記の記載より本発明の本質的な特徴を容易に理解し、本発明の要旨
及び範囲を逸脱することなく、種々の用途及び条件に適応させるべく種々の変更
及び修正を行なうことができよう。

白血球癒着巣に到達するために使用されるキメラ抗体−薬剤結合体は、顆粒球に
特異的に結合する抗原結合性超可変領域と、ヒト単核リンパ球上のレセプターに
高い親和性を有するFc部分を有するヒトイムノグロブリンの定常領域とを有す
る組換えキメラを含んでおり、前記キメラが、少なくとも1種の診断薬または治
療薬に結合していることを特徴とする。炎症性または感染性の病変にイメージン
グ剤または治療薬を到達させる方法では、到達すべく有効な量の上記キメラ抗白
血球結合体を哺乳票に非経口的に注入する。

- 1.顆粒球に特異的に結合する抗原結合性超可変領域と、ヒト単核リンパ球上の レセプターに高い親和性を有するFc部分を有するヒトイムノグロブリンの定常 領域とを有するキメラ抗体を含み、前記キメラが、少なくとも1種の診断薬また は治療薬に結合していることを特徴とする白血球癒着病単に到達するためのキメ ラ抗体−薬剤結合体。
- 2.前記キメラが、診断用イメージング剤に結合していることを特徴とする請求 項1に記載の結合体。
- 3.前記イメージング剤が、50〜500KeVの範囲のγ線またはポジトロン を放射する放射性同位体であることを特徴とする請求項2に記載の結合体。
- 4.前記放射性同位体が、Tc−99m、I−123、Ga−67、In−11 1またはI−131であることを特徴とする請求項3に記載の結合体。
- 5.前記イメージング剤が、磁気共鳴映像増強剤であることを特徴とする請求項 2に記載の結合体。
- 6.前記イメージング剤が、ガドリニウムイオンまたはマンガンイオンであるこ とを特徴とする請求項5に記載の結合体。
- 7.前記超可変領域が、Ia(DR)抗原に特異的に結合することを特徴とする 請求項1に記載の結合体。
- 8.前記キメラが、少なくとも1種の治療薬に結合していることを特徴とする請 求項1に記載の結合体。
- 9.前記治療薬が、放射性同位体であることを特徴とする請求項8に記載の結合 体。
- 10.前記治療薬が、抗菌薬であることを特徴とする請求項8に記載の結合体。
- 11.前記治療薬が、放射性同位体と抗菌薬との組み合わせであることを特徴と する請求項8に記載の結合体。
- 12.前記治療薬が、ポリマー担体を用いて前記キメラ抗体に結合していること を特徴とする請求項8から11のいずれか一項に記載の結合体。
- 13.前記ポリマーが、デキストランまたはアミノデキストランであることを特 徴とする請求項12に記載の結合体。
- 14.前記抗菌薬が、抗細菌薬であることを特徴とする請求項10、12または 13のいずれか一項に記載の結合体。
- 15.前記抗菌薬が、抗ウイルス薬であることを特徴とする請求項10、12ま たは13のいずれか一項に記載の結合体。
- 16.前記抗菌薬が、抗菌類薬であることを特徴とする請求項10、12または 13のいずれか一項に記載の結合体。
- 17.前記抗菌薬が、抗寄生虫薬であることを特徴とする請求項10、12また は13のいずれか一項に記載の結合体。
- 18.到達すべく有効な量の請求項2から6のいずれか一項に記載の結合体を哺 乳類に非経口的に注入することを特徴とする炎症性または感染性病変に診断用イ メージング剤を到達させる方法。
- 19.前記イメージング剤が放射性同位体であり、前記方法が更に、前記結合体 が前記病変部位に局在するための十分な時間の経過後に、シンチグラフィックイ メージを得る段階を含むことを特徴とする請求項18に記載の方法。
- 20.前記イメージング剤が磁気共鳴映像増強剤であり、前記方法が更に、前記 結合体が前記病変部位に局在するための十分な時間の経過後に、前記病変の磁気 共鳴像を得る段階を含むことを特徴とする請求項18に記載の方法。
- 21.到達すべく有効な量の結合体を哺乳類に非経口的に注入する方法において 、炎症性または感染性の病変に治療薬を到達させるべく使用される薬剤を調製す るための請求項8から17のいずれか一項に記載の結合体の使用。
- 22.キメラ抗体の超可変領域が、顆粒球のIa(DR)抗原に特異的に結合す ることを特徴とする請求項21に記載の使用。
- 23.前記治療薬が、放射性同位体であることを特徴とする請求項21または2 2に記載の使用。
- 24.前記放射性同位体が、銅−67、ヨウ素−125、ヨウ素−131、レニ ウム186、レニウム188、ビスマス−212またはアスタチン211である ことを特徴とする請求項23に記載の使用。
- 25.前記治療薬が、抗菌薬であることを特徴とする請求項21または22に記 載の使用。
- 26.更に、前記結合体の投与から、前記病変部位の白血球による前記結合体の 選択的吸収が最大値に達する十分な時間が経過した後で、前記結合体に特異的に 結合する非標識の第2抗体または抗体フラグメントを、前記結合体の局在比を2 〜72時間以内に約20%以上増加させる十分な量で患者に非経口投与する段階 を含むことを特徴とする請求項18から20のいずれか一項に記載の方法。
- 27.前記方法において、前記キメラ結合体に加えて前記結合体に特異的に結合 する非標識の第2抗体または抗体フラグメントを使用して調製した前記方法に使 用する薬剤を使用し、結合体の投与から、前記病変部位の白血球による結合体の 選択的吸収が最大値に達する十分な時間の経過後に、前記治療薬の治療係数を約 10%以上増加させるに十分な量の前記非標識の第2抗体または抗体フラグメン トを、患者に非経口投与することを特徴とする請求項21から25のいずれか一 項に記載の使用。
- 28.白血球を含む感染性または炎症性の病変にイメージング剤または治療薬を 到達させるために、医薬として許容される減菌注射用ビヒクル中に、イメージン グまたは治療に有効な量の請求項1から17のいずれか一項に記載の結合体を含 むことを特徴とするヒト減菌注射用製剤。
- 29.請求項1から17のいずれか一項に記載の結合体が適当な容器に収容され ていることを特徴とする白血球を合む感染性または炎症性病変のinvivo検 出または治療の目的で使用されるキット。
- 30.更に、前記結合体に特異的に結合する非標識の第2抗体または抗体フラグ メントが第2の容器に収容されていることを特徴とする請求項29に記載のキッ ト。
- 31.到達させるに有効な量の前記結合体を哺乳類に非経口注入する、診断用イ メージング剤を炎症性または感染性の病変に到達させる方法において使用される 薬剤を調製するための請求項2から6のいずれか一項に記載の結合体の使用。
- 32.キメラ抗体の超可変領域が、顆粒球のIa(DR)抗原に特異的に結合す ることを特徴とする請求項21または31に記載の使用。
- 33.前記方法において、前記キメラ結合体に加えて前記結合体に特異的に結合 する非標識の第2抗体または抗体フラグメントを使用して調製された前記方法に 使用する薬剤を使用する結合体の投与から、前記病変部位の白血球による結合体 の選択的吸収が最大値に達する十分な時間が経過した後で、前記結合体の局在比 を2〜72時間以内に約20%以上増加させるべく十分な量の非標識の第2抗体 または抗体フラグメントを、患者に非経口投与することを特徴とする請求項21 に記載の使用。
