Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は量大人工器官装置／連結具に関する。
従来の装置として、種種の理由から人工器官の頭部と首
部との間にある構造体を介入するのが一般的である。し
かし、本発明の出願人は本発明のような構造および特定
目的をもつものを知らない。
〔従来の技術〕
本発明の出願人が知っている従来例は次の通りである。
しｏｃｋｅ他のＵ、　Ｓ、特許第４　、００５，４９５
はセラミックキャップ骨の人工器官およびその移植法を
開示している。この発明は首部２上に取りつけ自在のキ
ャップ１から成り、該首部は該キャップ１を外科的に受
けることのできる形状の生体骨の大腿頭部を構成する。
スラスト板４は内部に形成された開口部を介して骨１０
上に取りつけられる。スラスト板４は首部２上にキャッ
プ１を係止する助けをする。本発明は、生体骨組織以外
の人工器官の首部およびキャップと環状に係合する量大
人工器官装置／連結具を含む等のの理由からＬｏｃｋ他
のものと異なる。
Ｌａｎｇｌａｉｓ他のＵ、　Ｓ、特許第４　、１７３，
７９７は横スカートを有する薄い金属カップ１６を有し
、該カップ１６は生体大腿１の頭部２２上に取りつけ自
在に設計され、該頭部２２は該カップ１６の形状に合致
するように機械加工されている。また、この発明は、人
工器官装置／連結具が人工器官のキャップと首部との間
に介入される本発明の技術内容と異なる。
ＦｒｅｙのＵ、Ｓ、特許第４．２２７．２６５は要素の
異なる機械特性の人工器官間に挿入するプラスチック骨
移植片を開示している。このプラスチック挿入はステム
１の首部上に取りつけ自在に設計されかつその表面で頭
部２内の空腔の内面と係合する。頭部２をステム１に固
定するのにねじ３が使用される。
本発明は、金属製量大装置／連結具を必要とし、量大装
置／連結具の内外面を異なる角度で形成しかつ内外面を
異なる面組織にする等、Ｆｒｅｙと種種の理由で異なる
。
Ａｎａｐｌｉｏｔｉｓ　　他の［１，Ｓ、特許第４．６
７６、７９７は複数の部分１例えば、頭部材１と他の中
間部材９とを連結するように設計された中間部材５から
形成された人工器官で構成した切除人工器官用ユニット
を開示している。この発明はとりわけ人工器官の頭部と
首部間に直接内設されるように設計された量大装置／連
結具を含む本発明と明らかに異なる。
ＦｒｅｙのＵ、　Ｓ、特許第４．６８７．４８８は溶接
手段１２により溶接されたスリーブを有する連結頭部５
を含む大腿頭部人工器官を開示している。これは、頭部
が人工器官の首部を挿入することのできる挿入自在の人
工器官装置／連結具と係合する予め存在する内面を含む
本発明と異なる。
Ｗｅｉｇｈｔｍａｎ他のＥＰＡ第１４４，２０９は内部
人工器官骨結合構成装置を開示しており、これは人工器
官の首部１２と頭部２０との間に挿入する設計のプラス
チック部３０を有し、該頭部２０は好ましくはセラミッ
ク製であり、首部Ｉ２は好ましくは金属で形成されてい
る。本発明は、このＢＰとは、異なる面を形成する内外
面を有する金属製量大装置／連結具を提供し、かつこれ
ら内外面が異なる面組織を有する点で異なる。
Ｅｌｌｏｙ他のＥＰＡ第１９３，６８１は人工器官の頭
部と首部との間に挿入するように設計されたプラスチッ
クスリーブ７または１３を有する連結人工器官を開示し
ている。本発明は上記Ｕ、Ｓ、特許第４．２２７，２６
５およびＷｅｉｇｈｔ＋ｎａｎ他のＥＰ八へ１４４．２
０９の場合と同様理虫からεｌ１ｏｙの発明と異なる。
Ｆｌｅｇｅａｕ他のＥＰＡ　　第２０２，１４１はスリ
ーブ装置を取りつける頭部を含む腰連結人工器官用の大
腿頭部を開示している。このスリーブ装置は頭部１１の
支持提１３を有する。本発明はこのＦ４ｅｇｅａｕ他と
異なる組織の内外面を含む。
Ｆｒｅｙのドイツ公告公報第２５２４９２３＋：５ステ
ム５と頭部１との間に挿入する金属スリーブを有する腰
連結人工器官を開示している。本発明は、このＦｒｅｙ
の発明とスリーブの内外面の面組織が異なる。
Ｒｉｂｏｎｅｔ他のドイツ公開公報２６１８７６３は相
対的並進運動を防ぐためにステム６に蝋付するスリーブ
４を介して該ステム６へ取りつけられる頭部を有する連
結人工器官を開示している。本発明は量大装置／連結具
が蝋付することなく首部に摩擦係合する点および内外面
の面組織が異なる点でこの発明と異なる。
〔発明の課題〕
従って、本発明は改良された人工器官量大装置／連結具
を提供することを第１課題とする。
本発明の他の課題は人工器官の頭部と首部とが相互に摩
擦係合する改良された人工器官量大装置／連結具を提供
することである。
更に、本発明の他の課題は異なる傾斜および異なる組織
を有する内外面をもった改良された人工器官量大装置／
連結具を提供することである。
更に、本発明の他の課題は１材料の頭部と他の材料の首
部との間に挿入できる改良された人工器官量大装置／連
結具を提供することにある。
上記本発明のこれらの課題、態様および特徴を添付図面
を参照して以下に詳述する。
〔発明の概要〕
本発明は次の態様および特徴を有する。
イ）本発明の量大装置／連結具の好ましい態様をここに
開示する。第１態様において、本発明の装置は内外面お
よび開口端部を有する傾斜スリーブから成る。第２およ
び３態様は第１態様と同様であるが、小さい貫通口を有
する端壁部を有するｌ端部を有する。
口）本発明の各態様において、本発明の量大装置／連結
具は金属製として設計されている。この量大装置／連結
具の好ましい材料は２ｉｍｍｅｒ、　Ｉｎｃ。
の登録商標ＴＩＶＡＮＩＵＭ　Ｔｉ　６Ａ１−４Ｖ合金
として既知のチタン合金である。
ハ）本発明の量大装置／連結具は１材料の人工器官頭部
と該頭部材料とは異なる材料の人工器官首部との間の相
互面に使用されるのが好ましい。
本発明の量大装置／連結具は連結する頭部と首部とに合
致するように格別使用に適した注意深い設計がなされて
いる。この観点から、本発明の量大装置／連結具の外壁
の傾斜は量大装置／連結具が設置される頭部の内腔の傾
斜に合致するように形成されるのが好ましい。同様に、
量大装置／連結具の内面の傾斜は内設される首部の外面
の傾斜に合致するように設計されている。これら傾斜は
特定使用の他のものと異なる。
ニ）他の態様において、本発明の好ましい態様の量大装
置／連結具の内外面の表面は係合する材料の表面と最良
に合致するように特別に設計されている。従って、本発
明の量大装置／連結具の外面がセラミック製の人工器官
頭部の内面と係合する状態で、量大装置／連結具の外壁
の表面組織は比較的荒い。この目的に適した荒さ程度は
１１６゜７マイクロインチ・（約２９．６μｍ）数値平
均の表面仕上げを基準とできる。この例は量大装置／連
結具の内面が金属首部の外面と係合する場合に適用でき
る。このような場合、量大装置／連結具の内面は量大装
置／連結具の外面の表面組織の荒さより細かい荒さ程度
の表面組織にできる。この例に適した量大装置／連結具
の内面の荒さ程度として、例えば、３２マイクロインチ
（約８．１μｍ）数値平均の表面仕上げにできる。
ホ）本発明の量大装置／連結具の内外面は両者間の摩擦
相互作用を利用して人工器官頭部を人工器官首部にのみ
取りつけることを意図している。
該内外面の面組織は本発明の量大装置／連結具を挿入す
る該首部と該頭部との係合摩擦を最高に増すことのでき
る特別設計になっている。
へ）他の態様において、本発明の量大装置／連結具は該
量大装置／連結具が他の人工器官頭部および首部に適合
できるモジュールシステムに使用できる。
〔実施例と作用〕
第１図を参照して、大腿人工器官は参照番号１で全体を
示し、ステム５と該ステ５の１端部上の首部７を有する
第１要素を含む。また、該人工器官１は盲ボアを１側部
上に有する頭部４を含む。
第１図の実施例において、人工器官１の頭部４と首部７
との間には量大装置／連結具１０が挿入されている。第
１図において、量大装置／連結具１０はこの明細書に記
載した実施例の１態様にすぎない。第１図のこの特定量
大装置／連結具は第２および３図において更に詳細に表
されている。
第２および３図を参照して、量大装置／連結具１０は第
１端部１１、第２端部１３、外面１５および内面１７を
含む。第３図は第１図のより良い人工器官１の首部７と
頭部４との間に挿入された本発明の量大装置／連結具１
０を示す。
上述のごとく、人工器官は１材料のステム／首部と他の
材料の頭部とで構成されうる。具体的には、チタン合金
は生体骨組織との相容性からステム／首部構成に適した
材料であり、他方セラミック材、は人工器官の頭部製造
にチタン合金を含む金属材料よりも実益がある。他の理
由から、人工器官の頭部はベアリングとして作用しかつ
セラミック材はこの目的に非常に有利であることは知ら
れている。
既知のごとく、セラミック体の表面組織は金属体の表面
組織と異なっているので、人工器官を設計する場合この
事実を考慮することが重要である。
第２および３図から明らかなように、量大装置／連結具
０１の好ましい態様において、外面１５の表面組織は内
面１７の表面組織よりも荒い。本発明の量大装置／連結
具１０の１好適例において、量大装置／連結具１０の外
面１５の表面組織は１１６．７マイル口インチ（約２９
．６μｍ）計算平均であってよく、内面１７の表面組織
は３２マイクロインチ（約８．１μｍ）計算平均であっ
てよい。当然のことながら、この荒さ程度は連結具１０
の表面１５．１’７の荒さ程度の１例にすぎない。
量大装置／連結具１０の外壁１５の比較的高い荒さ程度
はセラミック頭部４の空腔の内面との摩擦係合および保
持をより良くする。同様に、量大装置／連結具１０の内
面の荒さ程度が低い場合は金属首部７の摩擦係合および
保持により適している。
他の態様において、人工器官の頭部的空腔の傾斜角度は
その首部の傾斜角度としばしば異ってよい。この傾斜角
の相違は接触面よりも接触線に沿った該首部と頭部との
摩擦係合を生じさせる。このような状態は摩擦内部係合
の表面積を縮小して首部に摩擦保持される頭部の能力を
減少することになる。この観点から、量大装置／連結具
１０の外面は盲ボア６の傾斜程度に一致する傾斜に設計
し、量大装置／連結具１０の内面は首部７の傾斜程度に
一致した傾斜にする。このようにして、量大装置／連結
具１０はその摩擦保持を増加する大きな接触表面積で量
大装置／連結具１０と頭部４および首部７とを係合させ
る。例えば、外面１５の傾斜は５゛　４４′、内面７１
の傾斜は６゛であってよい。上記頭部盲ボアの傾斜と首
部傾斜との傾斜程度に依存して所望により傾斜程度は変
化できる。
第４および５図は本発明による量大装置／連結具の他の
態様を示す。各態様は以下に詳述するごとく、第１から
３図の上記量大装置／連結具１０に共通するいくつかの
態様を有する。第１に、第４および５の量大装置／連結
具１０は第１および３図の量大装置／連結具の内外面の
荒さ程度に関して説明した関係を有する。更に、第４お
よび５図の量大装置／連結具は上記量大装置／連結具１
０と同様に、連結された人工器官のステムと頭部の各傾
斜を一致させるために上記内外面の傾斜角度間の関係に
する。
第４図に関して、量大装置／連結具の第２態様は参照番
号１０″で表し、同様要素を同様番号で示した。第４図
ではステム７°および盲ボア６′を有する頭部４°で示
した。
量大装置／連結具１０″は第１端部１１′、小さい中心
開口部２３を除いて閉鎖した壁２１を有する第２端部、
外面１５′および内面１７′　を有する。量大装置／連
結具１０′　は量大装置／連結具１０より長い。
開口部２３は量大装置／連結具１０°内に設けられてい
るので、ステム７′が人工器官１０°内に取りつけられ
るとステム７°の端部と壁２１の内面との間に溜まった
空気その他の物質を除去して固定を確実にする。更に、
開口部２３は首部７′と頭部４°に量大装置／連結具１
０°を組立てまたは解体するに際して器具（図示せず）
に連結して使用されてよい。
第５図に関して、第１から４図と同様の要素は同様番号
に”を付けた。量大装置／連結具１０”は人工器官１”
の首部７”と頭部４”との間で連結されている。量大装
置／連結具１０”は外面１５”、内面１７”、および第
１端部１１”および量大装置／連結具１０”を貫通口２
７を除いて横切って延びる壁２５で構成される第２端部
を有する。
量大装置／連結具１０”は量大装置／連結具１０°に類
似し、主な相違は量大装置／連結具１０”が量大装置／
連結具１０°の壁２１よりも厚い壁２５を有することで
ある。量大装置／連結具１０”の壁２５の大きな厚みは
量大装置／連結具１０”を長くするためである。量大装
置／連結具１０”の壁２５を貫通する開口部２７は量大
装置／連結具１０°の対応開口部と同一目的で設けられ
ている。本発明の好ましい態様において、量大装置／連
結具１０′　は量大装置／連結具１０よりも長く、かつ
量大装置／連結具１０”よりも短い。この長さの相違は
転用性を増すためである。
人工器官を組立てるには、量大装置／連結具１０．１０
°または１０″の１態様を用いて、これをまず人工器官
の頭部へ挿入する、または人工器官の首部上に組立てる
。いづれの場合においても、量大装置／連結具は摩擦係
合により隣接構成要素上に組立てられた関係で量大装置
／連結具が保持されるように隣接構造に付勢する。本発
明の量大装置／連結具を使用して、人工器官頭部と対応
人工器官ステム／首部とを更に確実に組立てるには、特
に異種材料で形成した異種構成要素を使用するのがよい
。本発明の量大装置／連結具はこれらの構成要素間の相
互面として作用し、確実に組立体を組立てるために所望
の表面荒さおよび壁傾斜を有する。
当然のことながら、本発明の量大装置／連結具態様は異
なる大きさおよび異なる人工器官設計の人工器官頭部お
よびステムをモジニールシステムで（組合せて）使用で
きる。
従って、本発明による量大装置／連結具の態様は上記本
発明の各目的を達成して人工器官要素の質、対応性、お
よび保持性を改善して従来技術にみられない必要性を実
施する装置を提供する。
更に、本発明の精神および範囲内で種種変更が可能であ
る。従って、本発明は請求の範囲の表現に限定されない
。

    【特許請求の範囲】
  1. １．第１材料で形成された第１傾斜をもつ首部、第２材
    料で形成された第２傾斜を有する盲ボアを具備する頭部
    、および上記頭部と上記首部とを連結する連結具から成
    り、上記連結具は上記第２傾斜に対応する傾斜を有する
    外面および上記第１傾斜に対応する傾斜を有する内面を
    有する本体から成り、上記外面は第１荒さの表面組織を
    有し、上記内面は上記第１荒さと異なる第２荒さの表面
    組織を有することを特徴とする人工器官。
  2. ２．上記第１傾斜は上記第２傾斜と異なる、請求項１の
    人工器官。
  3. ３．上記第１荒さ程度は上記第２荒さ程度よりも荒い、
    請求項１の人工器官。
  4. ４．上記第１材料は上記第２材料と異なる、請求項１の
    人工器官。
