に接続された状態で第1および第2の電極用導電
層を形成すると共に、前記入出力端子に対応する
少なくとも2個の貫通孔を形成したプリント基板
を前記誘電体ブロツクの開放面上に配置し、前記

する第3の電極用導電層と、前記第2の電極用導
