GRPR-アンタゴニストは、非特異的結合(NSB)によりガラス及びプラスチック表面に固着する傾向があり、これは、医薬組成物を配合するための課題である。安定した組成物を提供するために、いくつかの界面活性剤を試験した。本発明者らは、すべての試験済みの界面活性剤の中でもとりわけ、(i)ポリエチレングリコール鎖及び(ii)脂肪酸エステルを有する化合物を含む界面活性剤が最良の結果を得たことを予想外に見出した。
Rは、脂肪酸鎖、好ましくは、置換されていてもよい脂肪族鎖である]の化合物を含む。
第2の態様では、本開示はまた、それを必要とする対象におけるがんの治療又は予防における使用のための放射性標識されたGRPR-アンタゴニストを含む組成物に関し、放射性標識されたGRPR-アンタゴニストは、2000~10000MBqの間に含まれる治療上効率的な量で、前記対象に投与される。

配合物キットの開発中、本発明者らは、ペプチドが、ガラス及びプラスチック表面に固着するある特定の傾向を有することを実感した。
NeoBOMB1の標識化は、Castaldiら(Castaldi E、Muzio V、D’Angeli L、Fugazza L. 68GaDOTATATE lyophilized ready to use kit for PET imaging in pancreatic cancer murine model、J Nucl Med 2014年;55巻(suppl 1):1926)による以前に発表したキット手法に基づいた。
本明細書中で開示されるのは、177Lu-NeoBOMB1の3種の異なる用量による、周知のGRPR-発現前立腺がん細胞系統PC-3を異種移植された動物の治療を伴う、177Lu-NeoBOMB1の治療有効性の前臨床試験の模範的な、非限定的な例である。更に、腫瘍を有さない動物の小グループにおいて、腎臓及び膵臓に対する177Lu-NeoBOMB1治療の効果を、治療後の病理組織学的検査により試験した。
放射性標識
NeoBOMB1(ADVANCED ACCELERATOR APPLICATIONS社)(WO2014052471)を、超純水に希釈し、濃度及び化学純度を、自社開発の滴定方法でモニターした(Breeman WA、de Zanger RM、Chan HS、de Blois E.Alternative method to determine specific activity of 177Lu by HPLC.Curr Radiopharm.2015年;8巻:119~122頁)。ペプチドの固着を防ぐために、放射活性を、すべての必要な添加剤、例えば、緩衝液、抗酸化剤、及び張力活性剤(Kolliphor HS15)を含めたペプチドを含有する、バイアルに加えた(177Lu100MBq/nmol)。高速液体クロマトグラフィを、放射化学的純度を決定するために、メタノール及び0.1%トリフルオロ酢酸の勾配で行った。前述した通り、シリカゲルの即時薄層クロマトグラフィ(instant thin-layer chromatography)により測定された放射性金属の取り込み(de Blois E、Chan HS、Konijnenberg M、de Zanger R、Breeman WA.Effectiveness of quenchers to reduce radiolysis of(111)In- or(177)Lu-labelled methionine-containing regulatory peptides. Maintaining radiochemical purity as measured by HPLC. Curr Top Med Chem.2012年;12巻:2677~2685)は、SPECT/CT、並びに有効性及び毒性試験の場合、それぞれ、>67%及び>90%であった。
すべての動物試験を、Erasmus Medical Centerの動物福祉委員会(Animal Welfare Committee)要件に合致し、認められているガイドラインに従って行った。接種培地(1/3がマトリゲル高濃度(Corning社)+2/3がハンクス平衡塩類溶液(Thermofisher Scientific社))中の、4×106 PC-3細胞(American Type Culture Collection)200μLを、雄のbalb c nu/nuマウスの右肩に皮下接種した。腫瘍細胞接種の4週間後、平均の腫瘍のサイズが543±177mm3に達した場合、動物を、4群、すなわち、対照群(n=10)及び第1~3の療法群(1群当たりn=15)に分けた。177Lu-NeoBOMB1の有効性を決定するために、動物に、イソフルラン/O2麻酔下で、3種の模擬注射(対照群)、177Lu-NeoBOMB1 3×30MBq/300pmol(第1群)、177Lu-NeoBOMB1 3×40MBq/400pmol(第2群)又は177Lu-NeoBOMB1 3×60MBq/600pmol(第3群)を投与した。注射を、静脈内投与し、注射を、1週間の間隔を空けて接種した。
腫瘍取込みを定量するために、SPECT/CTイメージングを、PC-3異種移植された動物の追加の群(1群当たりn=2)で行った。腫瘍のサイズが、すべて477±53mm3であった場合、動物に、有効性及び毒性試験に含まれる動物と同じペプチド量を注射した。1回目の治療的注射の4時間及び24時間後、2回目及び3回目の治療的注射の4時間後、全身SPECT/CT走査を、ハイブリッドSPECT/CTスキャナー(VECTor5、MILabs、Utrecht、The Netherlands)で行った。SPECTを、報告された空間分解能が0.85mmである2.0-mmピンホールコリメータを用いて、40のベッド位置で、30分で行った(Ivashchenko O、van der Have F、Goorden MC、Ramakers RM、Beekman FJ.Ultra-high-sensitivity submillimeter mouse SPECT.J Nucl Med.2015年;56巻:470~475頁)。SPECT画像を、光電ピークの両側にバックグラウンドウインドウを有する(幅が、対応する光電ピークの20%である)、光電ピークウインドウ113及び208keV、並びにSR-OSEM再構成法(Vaissier PE、Beekman FJ、Goorden MC. Similarity-regulation of OS-EM for accelerated SPECT reconstruction. Phys Med Biol.2016年;61巻:4300~4315)、ボクセルサイズ0.8mm3を用いて再構成し、CTデータに登録した。再構成後の3次元ガウシアンフィルターを適用した(1mm fwhm)。CTを、次の設定で行った。0.24mA、50kV、周角走査、1ポジション。CTを、100μm3で再構成した。
病理学的分析のために収集した膵臓及び腎臓組織をホルマリン固定し、パラフィン包埋した。ヘマトキシリン及びエオシン染色を、Ventana Symphony(商標)H&Eプロトコール(Ventana)を用いて、4μM厚の組織切片で行って、4つの治療群間の組織構造の差を決定した。合計4個の組織切片において、各臓器50μMを互いに別々に評価した。ヘマトキシリン及びエオシン染色は、経験豊かな病理学者により評価された。
体重25gで、先に公開された体内分布及び薬物動態試験から得られたデータ(Dalm SU、Bakker IL、de Blois Eら、68Ga/177Lu-NeoBOMB1、a Novel Radiolabeled GRPR Antagonist for Theranostic Use in Oncology. J Nucl Med.2017年;58巻:293~299頁)を有する、RADARリアリスティックマウスモデル(Keenan MA、Stabin MG、Segars WP、Fernald MJ. RADAR realistic animal model series for dose assessment. J Nucl Med.2010年;51巻:471~476頁)を用いて、動物を、177Lu-NeoBOMB1 3×30MBq/300pmol、4×40MBq/400pmol又は3×60MBq/600pmolで治療した場合、腫瘍、膵臓及び腎臓への用量を算出した。本発明者らの先に公開した論文(Dalm SU、Bakker IL、de Blois Eら、68Ga/177Lu-NeoBOMB1、a Novel Radiolabeled GRPR Antagonist for Theranostic Use in Oncology.J Nucl Med.2017年;58巻:293~299頁)の体内分布データを、指数曲線に適合させて、腫瘍及び臓器における時間放射能曲線を定義した。177Luについての時間積分活動(time integrated activities)を、177Lu崩壊曲線(T1/2=6.647d)で重ねたこれらの指数曲線を積分することにより得られた。投与された活性当たりの吸収線量を、Keenanら(Keenan MA、Stabin MG、Segars WP、Femald MJ. RADAR realistic animal model series for dose assessment. J Nucl Med.2010年;51巻:471~476)から得られた臓器S値で乗じることにより又は腫瘍340mgの場合の球状のノードS値(Stabin MG、Konijnenberg MW. Re-evaluation of absorbed fractions f
or photons and electrons in spheres of various sizes.J Nucl Med.2000年;4l巻:l49~160頁)を用いることにより得られた。
腫瘍の倍加時間を、対照群において、経時的に指数関数的な成長関数を腫瘍体積に適合することにより決定した。療法群では、指数関数的な腫瘍体積の減少を伴う間隔を、最下点時間後の再成長の開始と適合させた。成長曲線は、腫瘍(>2000mm3)が大きすぎて、平均成長統計を決定できないマウスについての打ち切り時点を越えて推定した。腫瘍の成長の遅延時間を、最大の腫瘍のサイズ2000mm3に達するのに必要とされる時間を対照群において判明した平均時間と比較することにより個別に決定した。
Prismソフトウェア(バージョン5.01、GraphPad Software社)を、統計解析のために用いた。P値>0.05は、統計的に有意であると考えられた。4つの群についての腫瘍体積成長及び遅延時間の差を、ボンフェローニの多重比較試験を含む一元配置ANOVA試験を用いて分析した。曲線当てはめを、ピアソンR2による最小/二乗当てはめに従って行って、当てはまりの良さを定量化した。
SPECT/CT
ほとんどの時点で、SPECT/CTにおいて定量化された平均放射活性取込みは、第3群が最も高く、続いて、第2群及び第1群であった。しかしながら、群間の差は、有意でなかった。図1Aは、1回目の注射の4時間後及び24時間後、並びに2回目及び3回目の注射の4時間後に得られた、各群の動物1匹の走査を示す。定量化された腫瘍取込みを、図1Bに図示する。
177Lu-NeoBOMB1による療法は、有効であることが証明された。対照群における動物は、腫瘍のサイズが2000mm3に達し、20.3±5.9dの範囲内であり、これは、第1群、第2群、及び第3群の場合、それぞれ97±59d、l03±66d及び95±26dであった(図2A)。更に、第1群から得られた動物2匹及び第2群の動物1匹は、完全寛解後にいかなる腫瘍再成長をも示さなかった。しかしながら、治療群内で腫瘍の成長の遅延時間に有意差はなく、対照群による差は、非常に有意であった(P<0.0001)。
毒性に含まれた動物は、追跡期間にわたって、体重の決定的な減少を示さなかった(図3)。動物の体重は、最初の週に増加し、経時的に、比較的安定したままであった。対照群中の1匹(ID:B)及び第1群から得られた1匹(ID:869)は、体重の減少を示したが、これは、48h以内で10%未満であった。膵臓の病理組織学的分析では、組織損傷又は他の異常を示さなかった(図4)。腎臓に関して(図5)、リンパ球の浸潤を伴う狭い領域が、最終の治療的注射後12週目及び24週目に腎臓において観察された。これは、対照動物並びに治療される動物の腎臓の症例であり、本知見は、療法に関連しないことを示した。療法の24週間後、萎縮及び線維症は、療法と関連しそうにない、最も低い治療用量を投与された動物1匹のみ(ID:864)の腎臓において観察された。療法の24週間後に安楽死させた第3群から得られた動物2匹の腎臓において、軽度の慢性炎症反応が観察された。
177Lu-NeoBOMB1 3×30MBq/300pmol、3×40MBq/400pmol又は3×60MBq/600pmolによる治療後の腫瘍、膵臓及び腎臓への放射活性用量を推定した(以下のTable 3(表3)を参照のこと)。これについて、腫瘍及び臓器取込みは、各注射後に類似したことが仮定された。

- 医薬組成物であって、
- 放射性標識されたGRPR-アンタゴニスト、並びに
- (i)ポリエチレングリコール鎖及び(ii)脂肪酸エステルを有する化合物を含む界面活性剤
を含み、
前記GRPR-アンタゴニストが、式(I):
の化合物である、医薬組成物。 - Mが、177Lu及び 68Gaから選択される、請求項1に記載の医薬組成物。

- 前記放射性標識されたGRPR-アンタゴニストが、少なくとも100MBq/mLの体積放射活性を提供する濃度で存在する、請求項1から8のいずれか一項に記載の医薬組成物。
- 前記放射性標識されたGRPR-アンタゴニストが、250MBq/mL~500MBq/mLの間の体積放射活性を提供する濃度で存在する、請求項9に記載の医薬組成物。
