用語「M1細胞」は、本明細書で用いられるとき、対象の組織内部に存在するM1マクロファージおよび/または対象の末梢において循環するM1単球を包含する。
本明細書で用いられるとき、用語「約」は、数値に先行するとき、10%の範囲を足した値または引いた値を示す。例えば、「約100」は90および110を包含する。

典型的には、腫瘍細胞は移植前に照射される、例えば細胞は、約1Gy、約2Gy、約4Gy、約5Gy、約6Gy、約10Gy、もしくは最大15Gyの量のγ線照射で治療されてもよい。特定の実施形態では、細胞は、少なくとも1回、少なくとも2回、少なくとも3回、少なくとも4回、または少なくとも5回照射されてもよい。
実施例1:多形神経膠芽腫標本におけるIGF-1Rについての免疫組織化学

上記の通り、ノーベル(NOBEL)は、ホスホロチオエート骨格を有する18-merオリゴデオキシヌクレオチドであり、開始メチオニンコドンから下流の6つのヌクレオチドで始まる、インスリン様成長因子1型受容体に特異的なアンチセンス(IGF-1R
AS ODN)に相当する。遊離酸における分子量は5708.71ダルトンである。ナトリウム塩における分子量は6082.40ダルトンである。5’末端でのIGF-1R遺伝子の相補配列として誘導されたノーベル(NOBEL)の配列は、5’-TCCTCCGGAGCCAGACTT-3’である。
この試験の目的は、試験物質ノーベル(NOBEL)の、マウスに静脈内注射を介して単回用量として投与するときの毒性を評価することであり、可逆性、持続性、または効果出現の遅れを評価するため、投与後の約48時間(3日目、中間屠殺)または約14日間(15日目、最終屠殺)、動物を観察した。この試験は、優良試験所基準に従い、食品医薬品局によって確立された基準を遵守して実施した。下表に示すように、雄および雌Crl:CD1(ICR)マウスを数群に割り当て、諸用量を投与した。動物に、溶媒/希釈剤[注射用の0.9%塩化ナトリウム、USP(滅菌生理食塩水)]またはノーベル(NOBEL)(滅菌生理食塩水中、100mg/mLとして提供)を100μLの容量で静脈内注射を介して尾静脈に1回投与した(表2を参照)。
毒性の評価は、死亡率、臨床的観察、体重、摂食量、ならびに臨床および解剖病理学に基づいた。すべての動物が、投与期間の3日目または15日目のその予定された剖検まで生存した。摂食量における試験物質に関連した臨床的観察または変化は全く生じなかった。体重に対する統計学的に有意な効果は全く認められなかった。しかし、0.04mg/マウスを与えた雄において、わずかな体重減少(対照の93.8%)が投与期間の15日目までに生じた。この傾向は、これらの雄において投与期間の早くも8日目に認められたが、身体状態または水分補給状態における臨床的変化が全く生じなかったことから、試験物質に関連しても有害でないと考えられた。0.01mg/マウスを与えた雄かまたは雌においては、試験物質に関連した体重効果は全く認められなかった。
ノーベル(NOBEL)は、ヒトとマウスの双方においてCD163+細胞を選択的にノックダウンする。悪性神経膠腫、ならびに限定はされないが、星状細胞腫、乳がん、頭頸部扁平上皮細胞がん、乳頭状腎細胞がんタイプII、肺がん、膵がん、胆嚢がん、直腸がん、古典的ホジキンリンパ腫、卵巣がん、および結腸直腸がんを含む種々の他のがんを有する患者に由来する血清は、単球を、低マイクロモル濃度でノーベル(NOBEL)を吸収するCD163+表現型に分化させ、この表現型のノックダウンをもたらすことになる。
現行の試験から、本発明者は、以下の投与スケジュール:0.025g/kg、0.05g/kg、0.1g/kg、0.15g/kg、および0.2g/kgにおいて、再発性神経膠腫コホートの神経膠腫における投与に適した用量を決定した。
異なるIGF-1Rアンチセンス配列は、ノーベル(NOBEL)配列(5’-TCCTCCGGAGCCAGACTT-3’(配列番号1))の多様式効果の一部もしくは全部において生物活性がある。18-merのノーベル(NOBEL)配列は、IGF-1R受容体の下方制御活性とTLRアゴニスト活性の双方を有し、マウスにおけるさらなる実験によると、両方の活性がインビボでの抗腫瘍免疫活性にとって必要であることが示唆される。AS ODN分子が抗腫瘍活性を有する一方、相補的なセンス配列は、やはりCpGモチーフを有するにもかかわらず、抗腫瘍活性を有しない。IGF-1Rエクソンの全オープンリーディングフレーム(4104塩基対)を調査し、IDT1220を含む10のさらなるCpGモチーフを同定した(配列番号2~11)(表3)。さらに、CpGモチーフを含まない幾つかのさらなるIGF-1Rアンチセンス配列(配列番号12~14)も同定した(表3)。
実施例7:ノーベル(NOBEL)によるToll様受容体9の活性化
侵入する病原体に対する免疫学的応答におけるフロントライン防御は、病原体構造と自然免疫系を活性化するToll様受容体(TLR)を含む一連の受容体との間の相互作用を含む。免疫系のこのアームは、細菌、ウイルス、真菌、および寄生虫を含む種々の病原体によって産生される一般的クラスの分子(それらのすべては侵入する病原体の生存にとって必須である)を認識する。これらの病原体に関連する分子パターン、すなわちPAMPSは、免疫エフェクター細胞、特に樹状細胞において発現される病原体関連受容体(pathogen related receptors)、すなわちPRRによって認識される。これらのPRRは、1991年にニュスライン-フォルハルト(Nusslein-Volhard)によって最初に特徴づけられたショウジョウバエ(Drosophila)において特徴づけられたtollタンパク質に対する相同性に基づいて、Toll様受容体と称される(スタイン(Stein)ら著、セル(Cell)、第65巻:p.725-35(1991))。TLRの活性化効果は、後続する適応免疫応答のタイプを指示するのに重要である。現在、公知のヒトTLRが10存在し、TLR9が特に興味深い。TLR9は、細菌およびウイルスに固有の非メチル化シトシン-グアノシン配列を含むDNAの断片に結合する。ヒトCpGジヌクレオチドは、常にメチル化され、免疫系に曝露される場合、自家ヒトDNAを免疫寛容原性にする。非メチル化CpGモチーフは、特に好ましい隣接ヌクレオチド六量体配列内で入れ子状態であるとき、強力な自然免疫応答を誘導する(図9)。応答は、アンチセンス遺伝子翻訳を停止させる薬物濃度と比べると、1000倍低めの濃度で認められる。
単球由来樹状細胞のノーベル(NOBEL)での処理は、著しい用量依存性の成熟応答を呈する(図11)。未熟DCがCD14+PBMCをrGM-CSFおよびrIL-4中で4日間培養することによって得られた。未熟DCをノーベル(NOBEL)で36時間処理した。DC成熟における正の対照としてポリI:Cを用いた。処理したDCを収集し、蛍光タンパク質とともにインキュベートし、フローサイトメータを用いて分析した。高いエンドサイトーシス能力は、成熟シグナル時に迅速かつ劇的に低下する未熟DCの特徴である。ノーベル(NOBEL)処理により、DCにおけるエンドサイトーシス能力が用量依存性様式で低下した。
さらなるIGF-1R AS ODN配列をスクリーニングするための指針として、標的化mRNA転写物の5’末端の配列は、ワトソン・クリック塩基対律に従ってmRNA配列に結合する最高の可能性を有する。この生物学的活性は、体温で安定な5’ヘアピンループを形成する可能性がより高いDWA配列とは異なり、体温で線状分子をより高い確率で生成する好ましい配列に起因し得る(図13)。上述の通り、生物学的活性は、体温で分子、理想的には線状分子の5’末端から到達可能である必要がある、標的化されたmRNA配列および非メチル化CpGジヌクレオチドへの到達可能性を含むことに起因し得る。これらの指針にもかかわらず、IGF-1Rの下方制御する能力を有しないDWA配列は、DC成熟にてより優れているように思われた(図12)。異なるIGF-1R AS ODNの生物学的活性を以下の表4にまとめる。
実施例10:IGF-1Rの下方制御:ノーベル(NOBEL)の生物学的特異性および生物活性のまとめ
AS ODN配列の生物学的特異性を評価するため、2つのアッセイを設計した。
C57/BL6マウスは、ジャクソン・ラボラトリー(Jackson Laboratory)(バーハーバー、メイン州)およびタコニック・ファームズ・インコーポレーテッド(Taconic Farms,Inc.)から入手し、8~10週齢を用いた。マウスは、イソフルランを含有するチャンバ内で麻酔し、100μLのPBS中、106個のGL261を側腹に1mLのビーディー・ファルコン(BD Falcon)シリンジおよび21Gのビーディー(BD)針(フィッシャー(Fisher))を用いて注射した。AS ODN GL261細胞製剤を左側腹に注射する一方、2週間後、野生型GL261細胞を右側腹に注射した。マウスは、腫瘍産生について少なくとも週2回検査した。これらの試験においては、GL261細胞が類遺伝子マウスの側腹に置かれるとき、かかる動物の約50%が介入の不在下で抗腫瘍細胞免疫を発生させることが想定される程度に免疫刺激性であることが周知であることは注目すべきである。図16に見られるように、AS-ODNで治療されるGL261細胞を伴う前処理では、WT GL261腫瘍成長が対照マウスにおける53%から13%に低下する一方、GL261 AS ODN混合物(4mgのノーベル(NOBEL)/106個のGL261)を伴う前処理では、WT GL261腫瘍が0%に低下した。興味深いことに、GL261とノーベル(NOBEL)をマウスの反対の側腹に注射したとき、ワクチンの有効性は失われた(図17)。これらのデータは、AS ODNで処理されるGL261とアンチセンス分子が抗腫瘍応答に寄与するが、最も有効なワクチンが自家腫瘍細胞とIGF-1R AS ODNとの同時注射を含むことを示唆する。さらに興味深いことに、CpGモチーフ周囲のパリンドロームであるノーベル(NOBEL)センス配列は抗腫瘍免疫の刺激時に有効でなく、これはCpGモチーフの生物学的有効性がCpGモチーフ単独を超えるIGF-1R AS ODNの生物活性に関連することを示唆している。
U118細胞をノーベル(NOBEL)(4mg/107個の細胞)の存在下または不在下で24時間インキュベートした。細胞を収集し、照射し、クリック-イテドゥ(Click-iTedu)試薬(10μMの最終濃度)の存在下で(新たなノーベル(NOBEL)を伴う場合または伴わない場合)培養物に戻した。72時間のインキュベーション後、製造業者のプロトコルに従って培養を展開した。図18に示すように、ノーベル(NOBEL) AS ODNは、5Gyを超える等効果のプラトーを伴う1~15Gyの放射線量の範囲全体にわたり、U118ヒト神経膠芽腫細胞の放射線感受性を引き起こした。
WHOグレードIV星状細胞腫(神経膠芽腫)は、18か月の生存中央値を有する一様に致死性のある原発性頭蓋内悪性腫瘍である。良好な手術候補者であると判断された再発性神経膠芽腫と診断された患者12名を治療に登録した。すべての患者では、手術、テモゾロミド化学療法および原体照射療法を含む標準治療が奏功していなかった。登録患者と彼らの疾患経過のまとめを表5に含める。すべての患者を、40mg/日での皮下エノキサパリンで3か月にわたり治療した。
自家腫瘍細胞からなる混合剤は、手術時に除去し、次にIGF-1R AS ODNで一晩処理した後、半透性チャンバに添加し、照射した。ワクチン製品では、配列5’-TCCTCCGGAGCCAGACTT-3’を有する18-merのIGF-1R AS ODNを用い、先行配列から下流に1つのフレームシフトを設け、その免疫刺激特性に基づき、各チャンバ(C-v)に2μgの外因性アンチセンスを付加した。プロトコルはまた、PBSを含有するチャンバ(C-p)を含むように改訂した。最大で10のチャンバを腹直筋鞘に移植した。ワクチン製剤のプレーティング段階中に得られる自家腫瘍細胞上清と外植されたチャンバ内容物を、探索研究を目的として急速冷凍した。
免疫パラメータのベースライン評価のため、血漿・白血球フェレーシスを手術の1週間前に実施した。血液はまた、先に述べられたように1、手術後に得た。血清と細胞画分とを遠心分離によって分離し、細胞を赤血球溶解緩衝液で処理し、白血球は、フローサイトメトリーによって定量化するか、または血清試料と同様、DMSO中に-80℃で貯蔵した。フローサイトメトリーを、前述のように、イージーサイト8HT(EasyCyte 8HT)(ミリポア(Millipore))と、ヒトCD4、CD8、CD11b、CD14、CD16、CD20、CD45、CD56、CD80、CD83、およびCD86(すべてがビーディー・バイオサイエンシーズ(BD Biosciences)製)、およびCD163(R&D Systems)に特異的な蛍光的にコンジュゲートされたmAbとを用いて実施した。収集後の分析は、フロウジョ(FlowJo)ソフトウェア(トリースター・インコーポレーテッド(Tree Star Inc)、アシュランド、オレゴン州)を用いて実施した。可溶性サイトカイン因子を、ルミネックス(Luminex)ビーズアレイ(ミリポア(Millipore)製のヒトサイトカイン/ケモカインパネルI、II、およびIII)を用いて定量化した。治療前と手術後にT細胞の多機能性を評価するため、患者および正常対照からのPBMCを、インビトロで13-酢酸12-ミリスチン酸ホルボール(PMA)およびイオノマイシン(シグマ・アルドリッチ(Sigma Aldrich))で刺激し、培養液中に放出されたサイトカインおよびケモカインをルミネックス(Luminex)によって定量化した。腫瘍組織切片を、IGF-1R、CD163、CD14、CD206、CD204、CD3、CD4、およびCD8について免疫組織化学により評価した。可能であれば、免疫陽性細胞のアペリオ(Aperio)定量化を用いた。さもなければ、免疫染色は、熟練した神経病理学者(LEK)により、0(染色なし)~6(強い拡散染色)の順序尺度を用いて定性的に評価した。
記録された54の重度有害事象の中で、ただ1つのSAEは、血漿・白血球フェレーシスのために用いられる大腿ポートからの血栓を含むプロトコルに関連した。試験におけるDVTの発生率は8.3%であった。患者9名が腫瘍増殖のために死亡したが、患者3名が頭蓋内出血および敗血症(カンジダ・グラブラータ(candida glabrata)、クレブシエラ・ニューモニエ(Klebsiella pneumonia))を含む他の原因で死亡した。5回の剖検を実施した。全患者は、臨床的指示に従い、術後期間内にステロイド中止または一日量維持がなされた。全生存中央値は91.4週であり、初期手術と再発のための手術との間の間隔に高度に相関した。再発性腫瘍手術および自家細胞ワクチン接種の後、48.2週および10週の2つの有意に異なるプロトコルの生存コホートがそれぞれ、より長期の生存コホートおよび短期の生存コホートとして同定された(図19a-c)。1つの異常値(TJ03)を除去し、我々は、登録時のプロトコルの生存とリンパ球減少症の程度との間の有意な相関を記録した(図19d)。初期診断時とプロトコル登録時との値の比較によると、標準治療後、平均リンパ球数が有意に減少していた(65%)ことが示された(8つの利用可能なペア試料、p=.012、対応t検定)。登録時のリンパ球数とワクチン接種後の最終の利用可能なリンパ球数との間の有意差は全くなかった(データは示さず)。
解剖学的な腫瘍応答をスコア化した。例を図20aおよび図20bに示す。標準MRIでの解剖学的改善は生存と相関しなかったが、追加的なイメージング判定基準は生存と相関した。4つの内の3つのより長期の生存コホート(TJ03、TJ06、およびTJ09)は、増加する見かけの拡散係数(ADC、図20c参照)に高度に相関するrCBVにおいて逆説的な増加を有した。これらの患者が、灌流データが疾患の進行を示唆しているにもかかわらず、腫瘍内部での細胞消失を反映するADC値を有したことから、これは逆説的であると考えられた。TJ06の症例では、CD163+マクロファージ集団における有意な持続的減少が、剖検の前に行った2回目のワクチン接種時に認められた(図20dおよび下記参照)。これらの内の2つの症例で、炎症に合致するPET/CT判定基準が認められ、これらの知見を実証している(図24)。サイトカインプロットの概要は、これら3名の患者における炎症促進性過程を支持した(図20e)。
外植されたチャンバは、構造的に無傷であり、トリパンブルー排除によると生細胞を全く含有しなかった。チャンバから得た膜の組織学的分析によると、CD15+好中球およびCD163+マクロファージがC-pおよびC-vチャンバの双方から得た膜の外表面をコーティングしているが、C-v上で劇増することが示された(図3a)。チャンバ内容物は、被包細胞および周囲環境から内向きに拡散する因子の生成物を反映し、制御C-pチャンバは後者に対して制御した。C-vにおけるC-pと比べて上昇したケモカインは、CCL21、CCL20、およびCCL19を含み、それらのいずれも血清レベルを超えて有意に上昇した。CXCL12は、C-vおよび血清においてC-pと比べて上昇した(図25および表6)。また、C-vにおけるHSP-70およびグランザイムBの血清と比べての有意な上昇が認められた(各々、3826pg/ml対327pg/ml、p=.0015、および37pg/ml対12pg/ml、p=.01)。これらの結果は、本明細書で開示される方法が抗がん効果を増強する炎症促進性免疫応答を誘導することを示す。
剖検を通じた外科的介入由来のパラフィン切片は免疫組織化学分析のために利用可能であった。すべての評価可能な症例において、初期診断に対してワクチン接種時に有意に増加した腫瘍浸潤性CD163+マクロファージの数が剖検時に有意に減少したことが認められた(図21bおよび26)。
C-vにおけるサイトカイン/ケモカインでの任意の有意な増加が、腫瘍微小環境(TME)または血清のいずれかにおけるそれらの存在の増加を反映し得ると仮定された。この問題へのさらなる洞察を得るため、腫瘍縮小術がこれらのサイトカインの血清レベルを低減するか否かを探索した。調査したすべての血清サイトカイン/ケモカインから2つの異常値を除外し、血清CCL21は、手術後2日目に有意に減少したが(表6)、これはTMEからのCCL21の産生を支持した。興味深いことに、T細胞のワクチン接種後レベルが、より長期生存の対象におけるCCL21とCXCL12の双方のレベルと酷似する傾向があった(図4)。それに対して、短期生存コホートにおけるT細胞、単球またはサイトカインの間に関連するパターンが存在しないことを我々は認めた(図28)。
インビトロでのT細胞活性化
-7日目および14日目に得られたPBMC試料をPMA/イオノマイシンで非特異的に刺激し、ケモカイン/サイトカインレベルについて上清を評価した。リンパ球減少性が著しい1つの異常値(TJ03)を除外後、14日目、2つの生存コホートにおける有意差を古典的Th-1およびTh-2応答に関連する6つの推定上のサイトカインについて認めた(図5および表8)。
実施例14:M2細胞でのIGF-1Rの過剰発現に極性化された単球
IL-4およびIL-13による基準のM2分化によりM2極性化に誘導される未熟な未分化ヒト単球が、M1極性化に誘導されるマクロファージと比べてIGF-1Rを過剰発現させる。さらに、IGF-1R AS ODNによる治療は、極性化されたM2細胞の出現および既存のM2細胞の生存を選択的に遮断する(図29)。これらの観察は、免疫系についての新たな情報を表し、種々のがんを有する患者における予後不良に関連したM2細胞の標的化された除去を含む治療的介入を支持する。図29aは、IGF-1R AS ODNの取り込みの大多数が単球および好中球の場合に生じることを示す。M1およびM2に分化されたマクロファージにおいてIGF-1R AS ODNの同様の取り込みが生じるが、IGF-1R AS ODNの濃度増加により、M2 CD163+細胞の選択的除去が標的化され、IGF-1Rのみが上方制御される(図29b)。CD163+細胞のアポトーシス細胞死の速度は、IGF-1R AS ODNの濃度と直接的に関係する(図29c)。
異なるタイプのがんを有する患者について、彼らの血清がCD163+分化の能力があるか否かを観察するための分析を実施した。図30に示すように、頭頸部扁平上皮細胞がん(N=2)、非小細胞肺がん(N=2)、および前立腺がん(N=5)に由来する血清と同時インキュベートした未分化単球からCD163+マクロファージ分化が認められた。すべての症例で、IGF-1R AS ODNによる治療により、この細胞集団がノックダウンされた。これにより、種々のがんを有する患者の血清中に存在する因子が、単球の、CD163および/または種々の他の表現型マーカ、例えばCD204およびCD206を差次的に発現するM2単球への分化を誘導するという確証が得られる。
図31は、異なるがんを有する患者由来の血清を用いる治療によってM2 CD163+表現型へ極性化される単球がCD163とPDL-1の双方の上方制御を示し、いずれの症例でも、AS ODNを用いる治療により、CD163とPDL-1の双方がこの細胞集団を選択的に標的化することによってノックダウンされることを示す。図31Aは、PBS対照に対するPDL-1を発現するCD163+マクロファージのIGF-1R AS ODN(ノーベル(NOBEL)、250μg)治療における手段の比較を示す。図31Bは、マッチドペア分析により、PDL-1の有意な減少として反映されるこの細胞集団の高度に有意な減少が示されることを示す。PDL-1を過剰発現する細胞集団の除去により、阻害の供給源から細胞傷害性T細胞が放出され、それによりこれらのがん患者における1型免疫が回復される。
正常な個体と星状細胞腫患者との間での循環CD163+単球における差異について試験した。正常な個体は、その循環中での約6%のCD14+単球とともに中間レベルのCD163を示した(図33A)。がん患者において、単球の数が増加しかつ単球はより高レベルのCD163を有するという2つの変化が認められる(図33A)。他の細胞(赤ボックス)はCD163を全く有しない。正常な個体は感染などに起因して広範囲の単球を有し得る(図33B、CD11b+CD14が陽性の細胞)が、これらは悪性星状細胞腫を有する患者において上昇する。図33Cにおけるヒストグラムは、患者の単球が不定にもそのCD14単球上に対照細胞(赤色ヒストグラム)よりも高レベルのCD163を有することを示す。
無作為に選択したWHOグレードIII星状細胞腫患者18名および正常ドナー24名から得たPBMCをCD11b、CD14、およびCD163に特異的な抗体で染色し、フローサイトメトリーによって評価した。フォワードスキャッター(FSC)およびサイドスキャッター(SSC)の特性を、ライブゲートを確立するために用い、単球をCD11bおよびCD14を発現する生細胞として規定した(図35A)。ライブゲートにおける代表的な等高線図ならびに正常およびAAドナーから得たPBMCにおけるCD11bおよびCD14陽性の分析を図35Aに示し、ここで軸はログスケールで示し、数はゲートされた細胞の頻度を示す。図35Bは、フローサイトメトリーによって測定された、AIIIを有する患者12名、AIII-Gを有する患者6名、および正常な個体24名から得たPBMCにおけるCD11b+CD14+単球の頻度を示す概要チャートである。正常な個体とAA患者サブセットとの間の細胞百分率における差の統計学的有意性をスチューデントt検定によって評価した(**,p<0.01)。CD11b+CD14+のゲートされた単球のCD163染色における中央値蛍光強度(MFI)が、図35CにおいてAIII、AIII-G、および正常血液標本の代表的なヒストグラムプロットから重ねてある。軸はログスケールとして示す。異なるドナー群から得たPBMC試料中のゲートされた単球サブセットのCD163染色におけるMFIを図35Dに示す。統計学的有意性は、分散分析により、続いてチューキーポストテストにより評価した(**,p<0.05)。CD11b+CD14+単球が試験患者全員の循環中に同様に上昇したレベルで存在する一方、GBM(多形神経膠芽腫;AIII-G)により酷似するAIII腫瘍を有する患者の循環から得た細胞は、悪性度が低めのAIII腫瘍を有する患者および正常対象からの場合よりも高レベルのCD163を進行性に発現する(図35、CおよびD)。想定通り、循環単球における増加に起因して、グレードIII星状細胞腫患者の血液中でのCD3+およびCD20+リンパ球の頻度は正常な個体と比べて減少する。
3つの星状細胞腫患者の原発性腫瘍細胞株から単離したエキソソームを96ウェルプレート上にコーティングし、初期手術前に収集した患者血清(13AIII、8AIII-G)および正常対照血清(4)とともにインキュベートした。結合抗体を蛍光結合全IgG(図36A)またはIgGアイソタイプに特異的な二次抗体(図36B)で検出し、抗体結合の程度をMFIとして測定した。データは、実施例18に記載の箱髭図で個別対象からの値として示す。図36Aにおけるアスターリスクおよびバーは、分散分析、続いてダネット検定(p<0.05)による判定としての正常な対照値と有意に異なる値を示す。図36Bでは、分散分析およびチューキーポストテストにより正常対照値およびAIII患者値と統計学的に有意に異なるAIII-G患者から得た値の群が**(p=0.004)によって認められる。図36Aに示すように、これらのエキソソームと反応するIgG抗体もまた、グレードIII星状細胞腫患者の大部分に由来する血清中に彼らの予後カテゴリーと無関係に存在する。しかし、アイソタイプに特異的な抗体を検出に用いるとき、我々は、Th2関連IgG2アイソタイプのエキソソーム結合抗体がAIII-Gにおいてより長寿命のAIII患者の場合と比べて有意に上昇することを認めた(図36B)。IgG1のレベルが後者の患者においてやや上昇する傾向があった一方、IgG4のレベルはAIII-G患者においてやや上昇したものの、これらの差異のいずれも有意でなかった。
ガドリニウム増強MRIに基づいてAIII(n=17)およびAIII-G(n=13)サブセットに分離されたAA患者由来の血清は、可溶性因子のレベルについてルミネックス(Luminex)により評価した。個体標本における濃度は、実施例18に記載のように箱髭図で示す。2群間の差の統計学的有意性をスチューデントt検定により評価した(****,p≦0.001;**,p≦0.01;*,p≦0.05)。Th2サイトカインIL-10およびCCL4の血清濃度は、GBM特性を有するAIII腫瘍を有する患者において有意に上昇した一方、IL-9およびTh1に関連したIL-12 P40、CXCL10、およびFLT3Lは残りのAIII患者において有意に上昇した(図37)。
最初に、AIIIおよびAIII-G患者のPBMCを最良に分別する、実施例22に記載のように得られた遺伝子発現データを同定するため、判別分析を用いた(図39A)。次に、これらの遺伝子CCL3、CCR4、CCR5、CCR7、CXCL7、IL-15、IL-32、IL-15R、IL-21R、IL-23R、IL-31RA、およびCD163のいずれが2つの患者コホートを区別するのに最も有効であるかを判定するため、主成分分析を用いた(図39B)。判別および主成分プロットの双方は、各個体のAIIIおよびAIII-GのPBMC標本に対する遺伝子に特異的な発現特性を用いて作成した(各々がドットの青色および赤色の色付けによって表される)。(A)における緑色破線および(B)における緑色ベクトルは各々、基準および成分空間における遺伝子転写物の方向を表す。AIII-G患者からのAIIIを最も確実に描写する遺伝子を同定するため、PBMCからの検出可能なシグナルを用いて、93すべての遺伝子のmRNAレベルに対してLDAを実施した(図39A)。PCA分析用に表9に詳述する12の遺伝子を選択した(図39B)。第1の主成分によって明示される全分散が37%であった一方、第2の主成分は約20%の全分散を明示した。PCAに基づき、M2マーカCD163、炎症誘発性サイトカインIL-32、ならびに1型サイトカイン受容体IL-21RおよびIL-23RのPBMCにおける発現レベルの評価は、異なるクラスのAIII腫瘍を有する患者間での明確な分別を得るのに十分である。

Claims (32)
- 神経膠芽腫を患う対象におけるM2細胞の選択的低減における使用のための医薬組成物であって、インスリン様成長因子1受容体アンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)を含み、前記IGF-1R AS ODNが配列番号1の配列を有し、
前記IGF-1R AS ODNが、約0.025g/kg~約0.2g/kgの範囲の用量で前記対象に投与され、
a)前記M2細胞が、CD11b、CD14、CD15、CD23、CD64、CD68、CD163、CD204およびCD206からなる群から選択される1つ以上の細胞表面マーカを発現するM2マクロファージまたはM2単球である、または
b)前記対象が循環中のM2細胞、腫瘍微小環境中のM2細胞、または、未分化単球をM2細胞へと極性化する血清を有する、医薬組成物。 - 前記対象における前記M2細胞が、FACSによる測定によると、前記医薬組成物の前記対象への投与から約24時間以内に少なくとも約40%減少する、請求項1に記載の使用のための医薬組成物。
- 前記医薬組成物の投与が前記対象におけるM1細胞の数に影響しない、請求項1に記載の使用のための医薬組成物。
- 前記IGF-1R AS ODNが1つ以上のホスホロチオエート結合を含む、請求項1に記載の使用のための医薬組成物。
- 前記IGF-1R AS ODNが、修飾されたリン酸骨格を含む、請求項1に記載の使用のための医薬組成物。
- 前記IGF-1R AS ODNが、約0.025g/kg、約0.05g/kg、約0.1g/kg、約0.15g/kg、または約0.2g/kgの用量で前記対象に投与される、請求項1に記載の使用のための医薬組成物。
- 前記医薬組成物が前記対象における1型免疫を回復させる、請求項1に記載の使用のための医薬組成物。
- 前記IGF-1R AS ODNが約37℃でヘアピンループ構造を形成しない、請求項1に記載の使用のための医薬組成物。
- 前記M2細胞が、CD11b、CD14、CD15、CD23、CD64、CD68、CD163、CD204およびCD206からなる群から選択される1つ以上の細胞表面マーカを発現するM2マクロファージである、請求項1に記載の使用のための医薬組成物。
- 前記M2細胞が、CD11b、CD14、CD15、CD23、CD64、CD68、CD163、CD204およびCD206からなる群から選択される1つ以上の細胞表面マーカを発現するM2単球である、請求項1に記載の使用のための医薬組成物。
- 前記M2細胞が、CD163を発現するM2マクロファージまたはM2単球である、請求項1に記載の使用のための医薬組成物。
- 前記対象が循環中のM2細胞、腫瘍微小環境中のM2細胞、または、未分化単球をM2細胞へと極性化する血清を有する、請求項1に記載の使用のための医薬組成物。
- 前記対象における前記神経膠芽腫の退縮を誘導する、請求項1に記載の使用のための医薬組成物。
- 前記対象における前記神経膠芽腫の産生の低下を引き起こす、請求項13に記載の使用のための医薬組成物。
- 前記対象における前記神経膠芽腫の産生の除去を引き起こす、請求項14に記載の使用のための医薬組成物。
- 経口、腹腔内、または静脈内投与のために製剤化される、請求項1に記載の使用のための医薬組成物。
- 前記医薬組成物が、前記対象に全身投与されるものであり、
前記医薬組成物が、前記対象におけるM2細胞を選択的に除去する、請求項1に記載の使用のための医薬組成物。 - 対象のがん治療における使用のための医薬組成物であって、第1のインスリン様成長因子1受容体アンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)を含み、前記第1のIGF-1R AS ODNが配列番号1の配列を有するとともに修飾されたリン酸骨格を有し、
少なくとも1つの拡散チャンバが前記対象にさらに投与され、
前記拡散チャンバが、腫瘍細胞と、第2のインスリン様成長因子1受容体アンチセンスオリゴデオキシヌクレオチド(IGF-1R AS ODN)とを含み、前記第2のIGF-1R AS ODNが、配列番号1の配列を有するとともに修飾されたリン酸骨格を有し、
前記医薬組成物が、前記対象に全身投与されるものである、医薬組成物。 - 放射線療法が、前記医薬組成物の投与後に前記対象にさらに施される、請求項18に記載の使用のための医薬組成物。

- 前記がんが、神経膠腫、神経膠芽腫、星状細胞腫、乳がん、頭頸部扁平上皮細胞がん、乳頭状腎細胞がんタイプ2、肺がん、膵がん、胆嚢がん、直腸がん、古典的ホジキンリンパ腫、卵巣がん、および結腸直腸がんからなる群から選択される、請求項18に記載の使用のための医薬組成物。
- 前記がんが神経膠腫である、請求項31に記載の使用のための医薬組成物。
