Description
TECHNICAL FIELD The present invention relates to a disease prevention support method, a disease prevention support device, and a disease prevention support system that support prevention of diseases in advance.
It is now known that there is a close relationship between disturbance of life rhythm and health. In particular, it is known that elderly people are more likely to experience changes in their life rhythms due to a decline in the ability to maintain homeostasis with age. In addition, it has been clarified that the modulation of life rhythms is associated with dementia and depressive symptoms.
From the above, there is a technique for grasping the health condition of a subject by knowing the life rhythm and its modulation. As such a technique, for example, there is a living activity monitoring system for a solitary person described in
However, the monitoring system determines that the subject has "moved" by stepping on the home care mat. Therefore, the above monitoring system can detect the movement of the subject indoors, but cannot detect the movement of the subject outdoors. For this reason, for example, it is not possible to detect actions taken by the subject, such as taking a walk outdoors or in a facility other than his/her home. In addition, the subject's behavior includes not only movement, but also gymnastic exercises that move the body on the spot. Therefore, the monitoring system cannot detect movements performed by the subject without moving. Furthermore, the monitoring system detects the movement route of the subject from the time-series data, and the life rhythm includes the element of the time zone of movement in addition to the movement route. For this reason, in the field of detecting the health condition of a subject from the life rhythm, it is desired to detect the modulation of the life rhythm of the subject in more detail.
The present invention has been made in view of the above points, and relates to a disease prevention support method, a disease prevention support device, and a disease prevention support system capable of detecting the life rhythm of a subject with higher accuracy.

DESCRIPTION OF THE PREFERRED EMBODIMENTS A first embodiment and a second embodiment of the present invention (hereinafter collectively referred to as "this embodiment") will be described below with reference to the drawings. In addition, in all the drawings, the same components are denoted by the same reference numerals, and overlapping descriptions are omitted as appropriate.
[overview]
The disease prevention support device of the present embodiment acquires information (hereinafter referred to as "activity information") regarding activities of a person (hereinafter referred to as "subject") from whom information for disease prevention is to be obtained. Here, the activity means that the subject moves the body regardless of the presence or absence of a change in the position of the subject. The information related to the activity is, for example, the speed of body movement, information on the time of movement, and information indicating the time period during which the activity was performed. Focusing on the fact that acceleration acts on the body when the body moves, this embodiment acquires activity information by attaching an acceleration sensor to the body of the subject. That is, in this embodiment, information based on the acceleration applied to the subject's body is acquired as activity information.
In the present embodiment in which information based on acceleration is used as activity information, the activity information acquisition unit includes an acceleration sensor that acquires information based on acceleration applied to the subject's body. In recent years, many mobile communication devices such as smart phones have built-in acceleration sensors, so in this embodiment, a smart phone in which an acceleration sensor application is installed is applied to the information acquisition device. and When a smart phone is used as an information acquisition device, the smart phone is attached to a position such as the waist or arm of the subject where it can detect the application of acceleration to the body of the subject. The acceleration sensor detects uniaxial or triaxial acceleration every 0.1 seconds, for example. Also, the acceleration sensor may have a function of a data logger that outputs the detected acceleration in association with time. Furthermore, in this embodiment, the existing functions of a smartphone such as an acceleration sensor, a CPU (Central Processing Unit), a memory device, a display screen, and a voice generation function are used for the disease prevention support device.
However, the present embodiment is not limited to such a configuration, and for example, it is possible to configure the acceleration sensor and the disease prevention support device as dedicated equipment that can be worn on the body. As such a device, for example, a wearable sensor or the like can be considered.
Furthermore, the present embodiment can also be configured as a disease prevention support system in which a server device performs processing with a relatively large amount of calculation and data among the functions of the disease prevention support device, and the results are transmitted to the subject side. can be done. The disease prevention support system will be described later.

The disease
(Activity information)
The activity information includes activity amount information and time period information. The activity amount information may include information on at least one of the number and intensity of activities performed by the subject. The fact that the activity amount information includes the number of times of activity means that, for example, when acceleration within a predetermined range is continuously detected repeatedly for a predetermined number of times, the
Further, in the first embodiment, the activity
Note that the activity
The time period information refers to the time when the acceleration is detected. For example, when the acceleration is continuously detected, it refers to the acceleration detection start time to the detection end time. According to the activity information including the activity amount information and the time zone information, it is possible to find information such as "what time and how much activity was performed", "the amount of activity during the day is greater than the amount of activity at night", etc. , etc. can also be obtained.
The
(classification department)
In addition, in the disease
The actual
However, the first embodiment is not limited to using a clustering technique when classifying activity information into clusters (groups). Any technique for creating one or more groups from a plurality of pieces of activity information may be used as long as it generates a pattern of activity information based on the similarity of a plurality of pieces of activity information. In the first embodiment, "based on similarity" refers to data such as numerical values representing a plurality of activity information, or similarity or dissimilarity between data and reference data. It means that the data to be configured is determined.
The disease
(cluster)
Here, the cluster will be explained. FIGS. 2(a) to 2(d) are diagrams exemplifying the contents of a plurality of clusters stored in the
More specifically, the clusters illustrated in FIGS. 2(a) to 2(d) are created by, for example, the following method. First, in the first embodiment, a large number of elderly people are targeted, and activity information detected by an acceleration sensor is acquired from each subject. The acquired activity information is, for example, data in which the intensity of acceleration detected every 0.1 seconds and the detection time are associated with each other. At this time, the acceleration sensor can divide the intensity every 0.1 seconds into, for example, every 4 seconds, and record the integrated value of each division as one value corresponding to 4 seconds by threshold determination. The recorded intensities are classified into six clusters CL1 to CL6 as a result of the above clustering. Data representing CL1 to CL6 are stored in the
FIGS. 2(a) to 2(d) respectively illustrate CL1, CL2, CL4, and CL5 among the six clusters CL1 to CL6. As shown in FIGS. 2(a)-(d), each cluster is shown as a curvilinear pattern each having a different shape. It should be noted that such CL1 to CL6 are clusters obtained when activity information is acquired for elderly people. A plurality of clusters stored in the
For example, CL1 shown in FIG. 2A indicates that the subject walks about 1000 steps per 3 hours from morning to evening, and the amount of activity decreases before and after that. Such an image of the subject's life rhythm is that they mainly do housework at home and go out for shopping and the like during the day. CL2 shown in FIG. 2B indicates that the subject always walks 1000 steps or less, and the number of steps decreases slightly during the daytime. The image of the subject's life rhythm is that they spend all day at home (at home) and do not engage in any particular activity during the day. CL3 indicates that the subject walks 2000 or more steps in the morning and afternoon, and the number of steps decreases slightly around noon. Such an image of the subject's life rhythm is that he goes out during the day for work or the like and returns home in the evening.
CL4 shown in FIG. 2C indicates that the subject walks 1500 steps or less in the morning and about 1000 steps in the afternoon, and the number of steps thereafter decreases. The image of such a subject's life rhythm is that he goes out in the morning and afternoon. CL5 shown in FIG. 2D indicates that the subject walks 1500 steps or less in the morning and about 1800 steps in the evening, and the number of steps thereafter decreases. Such an image of the subject's life rhythm is that he goes out in the morning and in the evening. CL6 indicates that the subject walks about 2,500 steps in the morning, about 1,000 to 1,300 steps from the morning to the evening, and the number of steps decreases after the evening. Such an image of the subject's life rhythm is to exercise early in the morning and then go out.
Figure 3 shows the percentage of subjects with suspected dementia and the percentage of subjects with suspected dementia in CL3, which had the highest percentage of subjects judged to be cognitively healthy, for CL1 to CL6. Fig. 10 is a graph showing the percentage of subjects in CL6; According to the graph in FIG. 3, CL6 is a cluster with 1.1 times higher percentage of subjects suspected of having dementia than CL3. Thus, according to the first embodiment, by classifying the subject's activity information into any one of CL1 to CL6, it is possible to obtain information regarding the possibility of deterioration of the subject's cognitive function.

(Information output part)
Prior to describing the specific configuration of the information output unit, first, the relationship between the cluster types (CL1 to CL6) and the estimated disease state of the subject will be described. Table 1 shows, for each of CL1 to CL6, the average number of steps per day, the percentage of all subjects (%), the percentage of subjects with low back pain (%), the percentage of subjects with knee pain (%), and urine leakage. The proportion (%) of subjects and the proportion (%) of subjects who have experienced a fall are shown. In Table 1, the symbol "*" indicates a significant difference, and the significant difference between the numerical values marked with two ** is greater than the numerical value marked with one *. According to Table 1, CL1 is a cluster with a larger number of subjects who experienced knee pain, urinary incontinence, and falls than other clusters. CL2 is a cluster with a large number of subjects with low back pain compared to other clusters. CL4 is the cluster with the second largest of all clusters of subjects with urinary incontinence. CL5 is the second largest cluster of all clusters with a history of falls.
From the results shown in Table 1, in the first embodiment, it can be inferred that, for example, subjects whose activity information is classified as CL1 have experience of knee pain, urinary incontinence, and falling. Further, in the first embodiment, it is presumed that a subject whose activity information is classified as CL4 has urinary leakage. Therefore, the clustering of the first embodiment can contribute to estimating the subject's health condition.
The
Here, an example is shown and demonstrated about the message corresponding to a cluster. The
Note that the first embodiment is not limited to the one in which the
Table 2 is a diagram exemplifying the messages for each disease state of knee pain, lumbago, urinary incontinence, and experience of falling. The messages shown in Table 2 include "suggestion of lifestyle" and "suggestion of product". "Suggestion of lifestyle habits" is information that proposes lifestyle habits that are effective in improving the state of each disease, and "Proposal of products" is information that proposes products that are effective in improving the state of each disease. Information.
According to Table 2, for example, a lifestyle suggestion for knee pain corresponding to CL1 is "do exercises to strengthen the muscles that support the knee", and a product suggestion is "a bath agent that efficiently warms the whole body". . Also, for example, a lifestyle proposal for low back pain corresponding to CL2 is "exercise exercises to strengthen the abdominal muscles and back muscles", and a product proposal is "a thermal product for warming the lower back".
In addition, for example, a lifestyle proposal for CL4 related to urinary incontinence is "exercising exercises to strengthen the pelvic floor muscles", and a product proposal is "a urine absorption pad for use when going out". In addition, for example, a lifestyle proposal related to a fall experience corresponding to CL5 is "perform one-leg standing exercises to prevent weakening of the muscles of the lower body", and a product proposal is "walking while supporting the movement of the legs". Supplements that help maintain function."

<Disease prevention support system>
As described above, the first embodiment is not limited to the configuration in which the disease
As shown in FIG. 5, the disease
In the disease

[Disease prevention support method]
6 and 7 are flowcharts for explaining the disease prevention support method of the first embodiment. The flowchart shown in FIG. 6 is for explaining the process of creating data to be stored in the
The flowchart shown in FIG. 6 includes an activity information acquisition step (step S801) for acquiring activity information related to the activity performed by the subject based on the acceleration applied to the subject; an information extraction step (step S802) of extracting activity amount information about the amount of activity and time period information about the time period in which the activity was performed from the activity information of and a matching step ( Step S804) and .
In the above description, "based on similarity" means, as described above, data such as numerical values representing a plurality of activity information, or data and reference data similar, dissimilar, and one group based on the degree It means that the data that make up the Here, the data included in each cluster is determined based on the distance (dissimilarity) between each activity amount information (number of steps) from a plurality of set centers.
That is, in the first embodiment, when creating data to be stored in the
In step S803, the extracted plurality of activity amount information and time period information are clustered and classified into a plurality of clusters. A plurality of classified clusters are represented by curve patterns, such as the clusters shown in FIG. 2, for example. The
In step S804, the activity amount of the activity information indicated by the actual cluster and the time zone corresponding to the peak of the activity amount are considered to estimate the subject's life rhythm and disease state. Then, each cluster is associated with a message including a lifestyle proposal and a product proposal corresponding to the estimated life rhythm and disease state. Data in which clusters and messages are associated constitutes a database (D·B). At least part of this database is stored in the
In step S803, the plurality of pieces of activity information are classified into 2 or more and 10 or less clusters. Thus, the number of clusters is within a desirable range for increasing the accuracy of pattern classification of the subject's lifestyle rhythms more than conventionally, and for securing significant differences in the differences between clusters.
The processing of the flowchart of FIG. 6 described above may be performed as a series of processing, or a plurality of processing may be performed separately and integrated. Also, the processing may be performed automatically, or may be performed at least partially manually.
The flowchart shown in FIG. 7 includes an activity information acquiring step (step S901) for acquiring activity information based on the acceleration applied to the subject, activity amount information relating to the amount of activity from the acquired activity information, and an information extraction step (step S903) of extracting time zone information related to the time zone obtained; and an information output step (steps S906 and 907) of outputting disease prevention support information corresponding to the cluster. Furthermore, the flowchart shown in FIG. 7 includes step S902 for determining whether or not the activity information acquired in step S901 has been accumulated for seven days. done. According to such processing, by acquiring the activity information of the subject over a plurality of days, it is possible to improve the accuracy of the actual cluster indicating the activity information.

When the activity information for seven days is accumulated, the actual
Subsequently, the
However, the first embodiment is not limited to representing the active mass information by the number of steps such as CL1 to CL6. In other words, the activity amount information extracted in the process of step S903 may be any information that includes at least one of the number and intensity of activities performed by the subject. The number of times of activity is determined based on the number of times acceleration is detected by the activity
In the first embodiment, when activity information is acquired for a predetermined period (7 days) for one subject, the activity information to be extracted may include variations in the amount of activity acquired during the predetermined period. Specifically, the cluster classification in FIG. Then, in creating the real clusters shown in FIG. 7, the variation in the activity information is used to create the real clusters, and the real clusters are compared with the clusters in the
Furthermore, in the first embodiment, in step S803 shown in FIG. 6, the activity information is clustered using each of a plurality of variables such as the number of steps, activity intensity, and variation in activity intensity, and the variables obtained as a result of clustering are clustered. Each cluster may be stored in the
[Second embodiment]
The disease prevention support device of the second embodiment can perform the clustering of activity information described above a plurality of times for one subject at intervals. Then, a change in the state of the subject can be detected depending on whether the clusters obtained as a result of clustering performed multiple times are the same or have changed.
FIG. 8 is a functional block diagram of the disease
In the disease

The
In the second embodiment described above, it is possible to presume that there has been a change in the subject's disease state based on differences in clusters into which the subject's activity information is classified, and to call attention to it early.
As described above, in the first embodiment and the second embodiment of the present invention, the example in which the amount of activity information is indicated by the number of steps of walking has been described. For example, it is represented by activity intensity or daily walking speed. FIGS. 9(a) to 9(d) show examples in which the activity amount information is represented by factors other than the number of steps. FIGS. 9A to 9D are graphs showing examples of activity information in which activity amount information is represented by activity intensity, and show activity intensities of CL1, CL2, CL4 and CL5 subjects, respectively. As shown in FIGS. 9A to 9D, this embodiment can classify activity information into a plurality of groups regardless of physical quantities used as activity amount information.
