Description
本願は、2017年1月9日付で出願した米国特許仮出願第62/443,850号表題「フローサイトメトリーによる特有のペクトル強度比(SIR)分析に基づく血液培養から直接的な迅速抗菌薬感受性試験」ならびにそれぞれ2017年5月10日および2017年5月11日付で出願された米国特許仮出願第62/504,205号および同第62/504,903号、どちらも表題「単一蛍光膜色素染色およびフローサイトメトリーによる特有のスペクトル強度比分析に基づく陽性血液培養から直接的な迅速グラム陰性抗菌薬感受性試験」の利益を主張する。
本願はさらに、2017年1月9日付で出願された、表題「フローサイトメトリーによる特有のスペクトル強度比(SIR)分析に基づく臨床単離株からの迅速抗菌薬感受性試験」の米国特許仮出願第62/443,854号、およびそれぞれ2017年5月10日および2017年5月11日付で出願されたどちらも表題「単一蛍光膜色素染色およびフローサイトメトリーによる特有のスペクトル強度比分析を利用した高速抗生物質感受性試験」の米国特許仮出願第62/504,152号および同第62,504,851号の前記利益を主張する。
前記仮出願は全体として参照により本明細書中に組み込まれる。
細菌感染の強度は前記感染を引き起こす前記微生物および前記微生物の前記病原性に依存して変わり得る。その結果、前記微生物の特定が適切な治療には必須である。例えば、敗血症は毎年世界中で2600万人を超える人々が罹患し、子どもの前記最大の死因であって、毎年500万人以上が死亡している(例えば、https://www.sepsis.org/faq/“Sepsis Fact Sheet”を参照)。集中治療室での敗血症による死亡率は、世界中で40%~60%の範囲であり、死因のひとつは患者が最初に不適切な抗生物質療法で治療されたことである。
(https://www.medicinenet.com/sepsis/article.htm)。これは主に、病原体の型の厳密な診断を得るために最先端の臨床微生物学研究室でさえも数日かかるためである。さらに、最初に間違った治療を受けた患者は、前記疾患の過程で早期から最適の療法で治療された者よりも低い生存率を示し、平均余命が短くなり、生活の質が損なわれている可能性が高い(https://www.sepsis.org/faq/“Sepsis Fact Sheet”)。したがって、微生物に感染した患者における病原体の診断の前記スピードは、患者の予後に著しく影響を及ぼし得る。
既存の抗菌薬感受性試験(AST)技術は冗長なプロセスである。概して、抗生物質耐性遺伝子を用いてまたは用いないで細菌株を特定、単離、および分化するための最近の慣例は、多くの場合、微生物学実験室での複雑かつ冗長なプロセスを必要とする。前記現行のプロセスでは、細菌を含む、生物学的サンプルは、まず前記実験室に受理される。前記BD PhoenixおよびbioMerieux Vitex 2システムのようなシステムを使用して、前記当該技術分野で公知の方法で細菌株を検出することができる。別のプロセスでは、前記生物学的サンプルを次に、滅菌ループを使用して、栄養的にリッチな培地(例えば、溶原性ブロスまたは任意の他の好適なブロス)を含む寒天プレート上に画線した。この寒天プレートは抗生物質で処理したスポットを含む。一旦前記検体を前記プレート上に画線したら、前記寒天プレートを専用のインキュベータ中に最低12時間入れる。前記寒天プレートを次いで細菌のコロニー成長について定期的にチェックする。当業者には理解されるように、前記生物学的サンプルが細菌を含む場合、細菌のコロニー成長は前記抗生物質を含まない前記スポット上で予想される。前記細菌が抗生物質耐性遺伝子を獲得しなかった場合、前記抗生物質を含む前記スポット上の成長は予想されない。しかしながら、前記細菌株が抗生物質耐性遺伝子を獲得した場合、コロニー成長が前記抗生物質で処理された前記スポット上で起こる。例えば、共同所有の米国特許出願公開第2008/0220465号明細書を参照。
前記本発明は、サンプル中の1以上の細菌の最小阻止濃度を決定するための方法であって:
a.複数の容器中で複数の細菌懸濁液を調製し、
b.ステップa.に様々な濃度の抗菌薬を添加し、それによって細菌と抗菌薬との組み合わせを含む複数の懸濁液を作製し、
c.ステップb.の細菌と抗菌薬との組み合わせを含む前記複数の懸濁液を好適な温度で好適な時間インキュベートし、それによって細菌と抗菌薬との組み合わせを含む複数のインキュベートされた懸濁液を作製し、
d.ステップcの前記新しい懸濁液に単一の膜結合色素を添加し、
e.ステップd.の前記新しい懸濁液を1以上の励起波長の入射光で照明し、
f.各懸濁液の2つの発光波長の放射光の強度を測定し、
g.各懸濁液のステップfに基づいた前記スペクトル強度比を決定し、そして
h.前記抗菌剤濃度の関数として前記スペクトル強度比(SIR)を決定することによって、ステップg.に基づいて前記最小阻止濃度を決定する、前記ステップを含む方法である。
a.複数の細菌懸濁液を複数の容器中で調製し、
b.ステップaに、様々な濃度の抗菌薬を添加し、それによって細菌と抗菌薬との組み合わせを含む複数の懸濁液を作製し、
c.ステップbの細菌と抗菌薬との組み合わせを含む前記複数の懸濁液を好適な温度で好適な時間インキュベートし、それによって、細菌と抗菌薬との組み合わせを含む複数のインキュベートされた懸濁液を作製し、
d.ステップcの前記懸濁液に単一の膜結合色素を添加し、
e.ステップdの前記懸濁液を1以上の励起波長の入射光で照明し、
f.一つの発光波長の放射光の強度を測定し、
g.ステップfに基づいて前記スペクトル強度比を決定し、
h.前記抗菌剤濃度の関数として前記スペクトル強度比(SIR)を決定することによって、前記サンプル中の細菌がステップg.に基づいた抗菌治療に対して感受性であるか、耐性であるか、または中間であるかを決定する前記ステップを含む方法も含み;前記第二の方法は、細菌と抗菌薬との組み合わせを含む前記複数のインキュベートされた懸濁液の各々の部分を除去し、ステップc後に前記部分を新しい容器に入れる前記ステップを含み得る。
前記以下の説明は事実上単なる例示であり、前記発明、その応用、または使用を何ら制限するものではない。前記説明は当業者が前記発明を作成し使用することを可能にするように設計され、その最後に具体例を提示しているが、それらは全く限定的と解釈されるべきではない。前記以下に対する様々な修飾は前記添付のクレームの前記範囲内にあることは当業者には明らかであろう。前記本発明は、前記実施例で提供されているかまたは本明細書中の他の箇所で提供されているかに関わらず、前記現在開示されている態様を限定するとみなされるべきではない。
この実施例では、抗生物質曝露の2~4時間後、15分の所要時間で血液培養物から直接ASTおよびMICを決定するための迅速な方法を提示する。
31の陽性血液培養物は、Sheba Medical Center(Ramat-Gan、Israel)から入手し、ゲンタマイシンおよびアンピシリン抗生物質(Sigma-Aldrich、USA)を使用して抗菌薬感受性について試験した。合計62の陽性血液サンプル-抗生物質の組み合わせを試験した。
b)抗菌剤ストック溶液をCLSI勧告にしたがって調製し、そしてCAMHB中、抗菌剤組み合わせの各々について推奨される前記最高濃度より2倍高い濃度に希釈した(「クラスII特別管理指針書:抗菌薬感受性試験(AST)システム」、2009年8月28日、FDA)。
c)サンプル/抗菌剤組み合わせの各々について、試験管のセットを調製し、それに対して、前記第一試験管を除いて、1mlのCAMHB培地を添加した。
d)前記セットの各第一試験管に、前記抗菌剤ストック溶液(b)から2mlを分配した。
e)CLSIによって前記必要最低限の濃度になるまで前記第一試験管から、1mlの前記溶液を前記次の試験管に移すことによって、2倍の段階希釈を調製した。試験管の各セットの前記最後の試験管から1mlを捨てた。
f)前記セット中の各試験管に、1mlの細菌サンプル溶液(a)を添加した。
g)各段階希釈セットについて、2つの対照を添加した。負の対照として、2mlのCAMHBを含む試験管。正の対照として、抗菌薬を含まない細菌サンプル溶液(a)を含む試験管。
h)すべての試験管を37℃でインキュベートした。
i)インキュベーション開始から2~4時間、各試験管からの200μlを新しい試験管に移し、2μlのSynaptogreenまたはFM1-43(それぞれ、Sigma-Aldrich;Molecular Probes)で染色し、フローサイトメーターによって測定した。前記サンプルの前記残りを16~20時間さらにインキュベートし、次いで成長について視覚的に評価した。
62のサンプル(31の陽性血液培養と2種類の抗生物質の組み合わせ)を三連で分析した。各サンプルを前記フローサイトメーターによって測定し、前記MICは前記スペクトル強度比(SIR)計算およびステップ関数評価を用いて算出した。前記サンプル内で同定された前記細菌は;大腸菌(15サンプル)、肺炎桿菌(6サンプル)、A.baummanii(4サンプル)、P.mirabilis(2サンプル)、E.cloacae(2サンプル)、Aeromonass spp.(1サンプル)、およびK.oxytoca(1サンプル)であった。
この実施例では、ASTの迅速な方法および臨床単離株からのMIC決定。
JMI Labs(LA、USA)から購入した、大腸菌、肺炎桿菌、E.cloacae、P.mirabilis、およびC.freundiiの30の臨床単離株を、ゲンタマイシン、アンピシリン、およびシプロフロキサシン抗生物質(Sigma-Aldrich、USA)を用いて抗菌薬感受性について試験した。合計90の単離株-抗生物質の組み合わせを試験した。
b)前記調節した細菌懸濁液をカチオン調節ミュラーヒントンブロス(CAMHB、Hylabs Israel)で1:100希釈して、約1×106CFU/mlの濃度にした。
c)抗菌剤ストック溶液をCAMHB中で、CLSIの推奨に従い、各細菌/抗菌剤の組み合わせの前記最高濃度よりも2倍高い濃度から始めて調整した(「抗菌薬感受性試験の性能基準」、M100-S17、V.27、No.1、CLSI)。
d)各細菌/抗菌剤組み合わせについて、1セットの試験管を調製し、それに対して、前記第一試験管を除いて、1mlのCAMHB培地を添加した。
e)前記セットの各第一試験管に、前記抗菌剤ストック溶液(c)から2mlを分配した。
f)各細菌/抗菌剤組み合わせについてCLSIによる前記必要最低限の濃度まで、1mlの前記溶液を前記次の試験管に移すことによって、前記第一試験管から、2倍希釈の段階希釈を調製した。試験管の各セットにおける前記最後の試験管から1mlを廃棄した。
g)各段階希釈セットについて、2つの対照を添加した。負の対照として、2mlのCAMHBを含む試験管。正の対照として、抗菌薬を含まない細菌接種材料を含む試験管。
h)前記セット中の各試験管に、1mlの細菌溶液(b)を添加し、その結果、約5×l05CFU/mLの最終接種材料濃度となった。
i)全ての試験管を37℃でインキュベートした。
j)インキュベーション開始から2~4時間、各試験管から200μlを新しい試験管に移し、2μ1のSynaptogreenまたはFM1-43(それぞれSigma-Aldrich;Molecular Probes)で染色し、フローサイトメーターによって測定した。前記サンプルの前記残りを16~20時間さらにインキュベートし、次いで成長について視覚的に評価した。
90のサンプル(30株および3種類の抗生物質組み合わせ)を三連で分析した。各サンプルを前記フローサイトメーターによって測定し、前記MICは前記スペクトル強度比(SIR)計算およびステップ関数評価を用いて算出した。
Claims (23)
- 抗菌薬の、サンプル中の1以上の細菌に対する最小阻止濃度を決定する方法であって:
a.複数の容器中で複数の細菌懸濁液を調製し、
b.ステップaの懸濁液に、様々な濃度の抗菌薬を添加し、それによって細菌と抗菌薬との組み合わせを含む複数の懸濁液を作製し、
c.ステップbの細菌と抗菌薬との組み合わせを含む前記複数の懸濁液を、好適な温度で好適な時間インキュベートし、それによって細菌と抗菌薬との組み合わせを含む複数のインキュベートされた懸濁液を作製し、
d.ステップcの懸濁液に、単一の膜結合色素を添加し、
e.ステップdの懸濁液を1以上の励起波長の入射光で照明し、
f.各懸濁液の2つの発光波長での放射光の強度を測定し、
g.各懸濁液のステップfに基づく前記2つの発光波長での放射光の強度比であるスペクトル強度比と抗菌薬の濃度との対応関係を決定し、
h.ステップgにおいて決定した前記対応関係を、ステップ関数に近似できる関数で近似し、当該関数に基づいて前記最小阻止濃度を決定する方法。 - 前記単一の膜結合色素がスチリル色素またはシアニン色素である、請求項1に記載の方法。
- 前記単一の膜結合色素がFM1-43である、請求項1に記載の方法。
- 前記1つの励起波長が360nm~570nmの範囲で選択される波長である、請求項1に記載の方法。
- 前記一つの発光波長が520nm~850nmの範囲で選択された波長である、請求項1に記載の方法。
- 前記サンプルが体液である、請求項1に記載の方法。

g.各懸濁液のステップfに基づく前記2つの発光波長での放射光の強度比であるスペクトル強度比と抗菌薬の濃度との対応関係を決定し、
h.ステップgにおいて決定した前記対応関係を、ステップ関数に近似できる関数で近似し、当該関数に基づいて前記サンプル中の細菌がステップgに基づく抗菌治療に対して感受性であるか、耐性であるか、または中間であるかを判定する前記ステップを含む、方法。 - 前記方法が、細菌および抗菌薬の組み合わせを含む前記複数のインキュベートされた懸濁液の各々の部分を除去し、前記部分をステップc後に新しい容器に入れる前記ステップを含む、請求項18に記載の方法。
