Description

ビフィドバクテリウム・ロンガムNCIMB42020株を、健康なヒト対象に由来する糞便試料から単離した。
密度勾配遠心分離を使用して、健康なドナーから末梢血単核球(PBMC)を単離した。PBMCを洗浄し、ダルベッコ変法イーグル培地-Glutamax(DMEM)(商標)(Glutamax(グルタミン代替物)+ピルビン酸塩+4.5g/lグルコース(Gibcoカタログ10569-010)10%ウシ胎児血清(SigmaカタログF4135)および1%ペニシリン/ストレプトマイシン(SigmaカタログP0781)に再懸濁した。PBMCを、37℃、5%CO2で、補足DMEM中で24時間、様々な用量のビフィドバクテリウム・ロンガム((全細菌:PBMC)高(100:1)および中(50:1))で刺激した。
ウイルス感染に対する樹状細胞による過剰な免疫応答は、肺で炎症促進性応答を引き起こす。1型IFNは、Th1細胞の化学誘引物質であるケモカイン、IP-10の産生を刺激し得る。B.ロンガムAH0106がヒト単球由来樹状細胞に対する有益な抗ウイルス効果を有するかどうかを決定するために、MDDCを、ヒトライノウイルス16(HRV16)に曝露する前にB.ロンガムAH0106で刺激し、HRV16に対するインターフェロンおよびIP-10応答を監視した。MDDCを、複数用量のB.ロンガムAH0106(100:1、50:1、25:1、10:1、1:1)による前処理(1時間)後に、37℃、5%CO2でcRPMI中24時間、HRV16(MOI)25:1)で、またはHRV16単独で刺激した。IFN-α、IFN-β、IFN-λおよびIP-10を測定するBio-Plex多重懸濁アレイ(Bio-Rad Laboratories)によってサイトカイン分泌を調べた。驚くべきことに、RVに対するIP-10応答が、用量依存的様式でB.ロンガムAH0106株によって減弱された(図5)。同様に驚くべきことに、低用量では、B.ロンガムAH0106株とのコインキュベーションは、III型インターフェロン応答(インターフェロンλ)の増強をもたらしたが、IP-10応答と同様に、高用量では、インターフェロンαおよびβ応答が抑制された(図6)。このデータは、損傷性応答を弱めながら防御免疫応答を支持することにおいて、B.ロンガムAH0106がRVに対する免疫応答を変化させることを示している。
別のグラム陽性菌株が、ヒト単球由来樹状細胞に対する有益な抗ウイルス効果を有するかどうかを決定するために、黄色ブドウ球菌で刺激したMDDCをヒトライノウイルス16(HRV16)に曝露し、IP-10応答を監視した。手短に言えば、MDDCを、複数用量の黄色ブドウ球菌(100:1、50:1)による前処理(1時間)後に、37℃、5%CO2でcRPMI中24時間、HRV16(MOI)25:1)で刺激し、B.ロンガムAH0106(100:1)またはHRV16単独と比較した。黄色ブドウ球菌株は、HRV16に応じてDC IP-10分泌を減少させなかった(図7)。
炎症を起こした粘膜内では、IP-10およびTNF-α分泌を誘導するのがウイルス自体だけではなく、他の(toll様受容体)TLRリガンドもその産生を誘導し得る。そのため、MDDCを、LPSに曝露する前にB.ロンガムAH0106またはB.ロンガム35624で前処理した。LPS、TLR-4アゴニストに対するIP-10およびTNF応答を監視した。手短に言えば、MDDCを、(100:1)の用量のB.ロンガムAH0106もしくは35624による前処理(1時間)後に、37℃、5%CO2でcRPMI中24時間LPS(50ng/ml)、またはLPS単独で刺激した。驚くべきことに、LPSに対するIP-10およびTNF-α応答はB.ロンガムAH106によって減弱された一方、B.ロンガム35624への曝露後、LPSに対するIP-10応答はほんのわずかしか減少せず(図8)、B.ロンガム35624は同じ用量でTNF-αを増加させる(図9)。B.ロンガム35624は周知の抗炎症性株であり、仮定される最高の候補であったので、これは非常に驚くべき結果である。B.ロンガムAH0106に関するサイトカインデータのあるものは、これが一般化された免疫刺激反応を引き起こすことを示唆していたが、実際には、B.ロンガムAH0106は、この細胞系で優れた、特異的免疫応答を有していた。
本発明者らは、治療利益を示すよう設計された前臨床モデルで、B.ロンガムAH0106株の有効性を試験した。ライノウイルスマウスモデルは、マウスがICAM、細胞に侵入するための主要なグループのライノウイルスの受容体を有さないために、ヒト感染症の優れた代用と考えられない。そのため、本発明者らは、優れたモデルと考えられるマウスにおける致死性インフルエンザモデルを利用した(Bartlettら、2015)。H1N1インフルエンザ株A/RP8/34(100PFU/50ul)株を使用してマウスを感染させた。
0~10日目 罹患率についての動物の監視(体重、体温および臨床スコア、1~3群)。

肺組織のウイルス力価の測定
単離した肺葉を、定量的PCRによる肺組織のウイルス量の定量化のために調製した。RNAをTRI試薬(Molecular Research Center)で調製し、次いで、DNアーゼ(Invitrogen)で処理してゲノムDNA汚染を回避した後、SuperScript III(Invitrogen)を使用して逆転写によってRNAをcDNAに変換した。SYBR Green(Stratagene)を使用してリアルタイムPCR(iCycler;Bio-Rad)によってcDNAを定量化し、試料をGAPDH発現レベルで正規化した。使用したプライマー配列(それぞれ、順方向および逆方向)は、インフルエンザRP8 Mタンパク質、5’-GGACTGCAGCGTAGACGCTT-3’(配列番号4)および5’CATCCTGTATATGAGGCCCAT-3’(配列番号5)であった。
群(1~3):
1.プラセボ-PBSに再懸濁した凍結保護物質による処理。
2.B.ロンガムAH0106 109個全細胞による処理。
3.B.ロンガム35624 109個全細胞による処理。

BALへの細胞浸潤物の測定
細胞をBAL液から単離し、コールターカウンター(IG Instrumenten-Gesellschaft AG、Basal、スイス)を使用して、気管支肺胞洗浄(BAL)液中の全細胞数を測定した。Diff-Quik溶液(Dade Behring、Siemens Healthcare Diagnostics、Deerfield、IL)で染色したサイトスピンでの標準的な形態学的および細胞化学的基準に基づいて、微分細胞計数を行った(200個細胞数/試料)。
細菌収穫/洗浄
方法:
1.遠心分離(14000rpm、4℃、20分;ローターJA-20(Avanti J-26×P Beckman Coulter)によって、元の細菌バイオマス(全細胞数=1.5×1011)250ml当量を収穫する。細菌ペレットを滅菌PBSで洗浄し、上清を廃棄し、再度洗浄する(さらに2回繰り返す)。
2.生および非生凍結乾燥細菌(3.0×1011粉末0.5gまたは全細胞数=1.5×1011)を50mlに再懸濁し、遠心分離(14000rpm、4℃、20分;ローターJA-20(Avanti J-26×P Beckman Coulter)によって収穫した。細菌ペレットを滅菌PBSで洗浄し、上清を廃棄し、再度洗浄する(さらに2回繰り返す)。
3.最後に、ペレットを滅菌PBS 50mlに再懸濁し、細菌溶液を2つの25ml分割量に分ける。
細胞破壊
この手順の目的は、細菌全体から細胞壁画分を作製し、細胞質画分および他の成分を除去することであった。これは、以下から選択される1つまたは複数のステップを含む:
・溶解を助けるために酵素処理を使用する場合にDNアーゼまたはプロテアーゼ活性を防ぐために、場合により凍結融解手順と合わせた、キレート剤による処理
・グリコシドヒドロラーゼおよび/またはNアセチルムラミダーゼによる酵素処理
・超音波処理またはフレンチプレス使用などの高圧の印加などの剪断力の印加。
・遠心分離を使用することによる、細胞質画分からの細胞壁画分の分離
・細胞壁画分の濾過
材料:
・EDTA(0.5M Fluka)は、細胞壁溶解のために酵素を使用する場合にDNアーゼまたはプロテアーゼ活性を防ぐために金属イオン(カルシウムまたはマグネシウム)を除去するためのキレート剤である
・リゾチーム(Sigma 10mg/ml)*エンドトキシンフリーは、N-アセチルムラミン酸とN-アセチル-D-グルコサミン残基との間の1,4-β-結合の加水分解を触媒するグリコシドヒドロラーゼである。この加水分解は、細菌細胞壁の完全性を損ない、細菌の溶解を引き起こす。
・ムタノリシン(1ml H2Oに希釈した10KU Sigma)は、細菌細胞壁のβ-N-アセチルムラミル-(1→4)-N-アセチルグルコサミン結合を切断するムラリティック酵素である、N-アセチルムラミダーゼである。この切断は、細菌細胞壁の完全性を損ない、細菌の溶解を引き起こす。
・ガラスビーズ90~150μm粒径(VWR)は溶解を助けるために超音波処理と合わせて使用した
方法:
1.250μlのEDTA(0.5Mストック Fluka)を25ml分割量の各々に添加することにより、最終濃度を5mM EDTAにした。
2.凍結するまで2つの分割量を液体窒素に入れることによって分割量を凍結し、次いで、分割量を水中で融解する;この手順を2回繰り返す。
3.細菌分割量それぞれ25mlを、時折穏やかなボルテックスで、37℃で1時間、250μlのリゾチーム(10mg/ml)および250μlのムタノリシン(2.5KU)と共にインキュベートする。このステップから、超音波処理器またはフレンチプレスによる剪断力を使用して、細菌細胞を破壊する。
超音波処理
・小さじ2分の1のオートクレーブ処理したガラスビーズ90~150μm粒径(VMR)を、超音波処理直前に細菌分割量に添加した。
・25mlの再懸濁した細菌材料を一度に超音波処理し、次いで、氷上に置き、50mlプローブを含む超音波処理器(VibraCell SONICS)を使用して他の分割量を超音波処理する。(設定チューン=50、周波数=60)。この手順を氷上で10分間、4回繰り返す。超音波処理後、4℃で10分間の1000rpmでの遠心分離によって、ガラスビーズ、壊れていない細胞および細胞片を除去する。
フレンチプレスによる高圧
・あるいは、フレンチプレス(Thermo Electron Corporation FA-078A)を使用した高圧設定(137.93MPa(20,000psi)当量)で、10.34MPa(1500psi)の圧力によって、細菌細胞を破壊する。この手順を氷上で3回繰り返す。
・フレンチプレスを使用した剪断力の印加は、3回目の実行後に混濁した細菌細胞の清澄化をもたらす。フレンチプレス破壊後、4℃で10分間の1000rpmでの遠心分離によって、壊れていない細胞および細胞片を除去し、細胞壁材料および細胞質画分を含有する上清が保持される。
4.剪断力(超音波処理または高圧)の印加後、細胞壁材料および細胞質画分を含有する上清が保持された。この上清を4℃で20分間、14000rpmで遠心分離して、細胞質材料から細胞壁材料を分離する。上清を廃棄する。
5.細胞壁材料を含有するペレットを、元の細菌バイオマス250ml当たり5mlのPBSに再懸濁する。細胞壁材料のペレットを1000rpmで10分間遠心分離して、ペレット中の黒色残渣を除去した。細胞壁材料を含有するペレットを-80℃で保存した。

1.細胞壁材料を含有する再懸濁した細菌ペレット(5ml)を融解し、ポリビニリデンジフルオリド(PVDF)膜フィルター(0.45μm孔径、Millipore、Bedford、MA;使用前にPBSで徹底的にすすぐ)を通して濾過する。膜フィルターを通り抜ける材料は、無傷細菌を含まず、サイズが0.45μm未満の細胞壁材料である。この0.45μmフィルターステップによって、4mlの細胞壁材料を産生する。元の250mlの細菌培養物から、4mlの懸濁細胞壁材料が残る。

実施例7で作製される、B.ロンガムAH0106由来の細胞壁画分が有益な抗ウイルス効果を有するかどうかを決定するために、画分を、HRV16に曝露したヒトMDDCでインキュベートし、1型インターフェロンおよび細胞IP-10応答を監視した。単球由来樹状細胞作製について:密度勾配遠心分離を使用して、健康なドナーから末梢血単核球(PBMC)を単離した。MACSシステム(Miltenyi Biotec、130-050-201)によるCD14陽性単離を使用して、ヒト末梢血単球を単離した。細胞を、インターロイキン4 1000U/ml(Novartis)および顆粒球マクロファージコロニー刺激因子(PeproTech、300-03)1000U/mlを含むcRPMI培地(Life Technologies、21875-091)で6日間培養して、MDDCに分化させた。細胞をcRPMI培地(RPMI(Life Technologies、21875-091)+10%ウシ胎児血清(SigmaカタログF4135)および1%ペニシリン/ストレプトマイシン(SigmaカタログP0781)で培養した。
実施例4に記載される方法論を使用して、別のビフィドバクテリウム属菌、ビフィドバクテリウム・ブレーベ(Bifidobacteria breve)(Bif UCC2003)由来の細胞壁画分も試験したが、同様の有意な効果は示さなかった。Bif UCC2003画分は、ウイルス刺激後にIP-10産生を減少させたが、24時間アッセイ中、B.ロンガムAH0106細胞壁画分と同程度ではなかった(図17(a))。
炎症を起こした粘膜内では、炎症促進性TNF-α分泌を誘導するのがウイルス自体だけではなく、他の(toll様受容体)TLRリガンドもその産生を誘導し得る。そのため、本発明者らは、TLRリガンドPoly:IC(TLR-3)およびLPS(TLR-4)に応じたTNF-αの分泌を調べた。MDDCを、LPS、TLR-4アゴニストに曝露する前に細胞壁B.ロンガムAH0106、B.ロンガム35624およびBif UCC2003で前処理し、IP-10およびTNF応答を監視した。MDDCを、(30mg/ml)の用量のビフィドバクテリウム属菌由来の細胞壁画分による前処理(1時間)後に、37℃、5%CO2でcRPMI中24時間、TLR-4アゴニストLPS(50ng/ml)もしくはTLR-3アゴニストPoly:IC(5μg/ml)でまたはTLRアゴニスト単独で刺激した。Quantikine ELISA(R&D systems)によってTNF-α分泌を調べた。B.ロンガムAH0106株全体の結果と一致して、LPSおよびPoly:ICに対するTNF-α応答はB.ロンガムAH106由来の細胞壁画分によって減弱された一方、B.ロンガム35624由来の細胞壁画分とBif UCC2003由来の細胞壁画分の両方とも、同じ用量でTNF-αを増加させる(図18aおよび図18b)。B.ロンガム35624は周知の抗炎症性株であるので、これは非常に驚くべき結果であり、B.ロンガムAH0106はこの系で優れた応答を有していた。
考察
要約すると、実施例2~10に例示されるように、本発明者らは、有益なIII型インターフェロン応答増強が存在し、AH0106由来の細胞壁画分の添加がこの望ましい応答を引き起こすことを示した。後期宿主免疫系応答(DC)の一部である細胞では、細胞壁画分が、細胞損傷および二次感染症をもたらし得る過剰な1型インターフェロン応答を遮断する。この標的化効果は、小児と成人の両方、および肥満個体においてインフルエンザ、感冒(ライノウイルス)およびRSVによって引き起こされる感染症、喘息、COPDおよびARDなどの慢性呼吸器疾患のウイルス性憎悪で利益がある。
カスケード応答
ウイルス感染症に対する異常な応答のカスケードを図19(a)に例示する。
主な抗ウイルス応答はIFNによって制御される。最もよく定義されたI型IFNはIFN-αおよびIFN-βである。ほとんどの細胞型はIFN-βを産生する一方で、造血細胞、特に形質細胞様樹状細胞はIFN-αの優勢な産生者である(IvashkivおよびDonlin 2014)。前述のように、IFN-αおよびIFN-βなどのインターフェロンI型応答は、インフルエンザ感染症のモデルで罹患率および死亡率増加と直接相関することが示されている(Davidsonら、2014)。1型IFNは、IP-10(CXCL-10とも呼ばれる)、その主な機能がTh1細胞の化学誘引物質である、CXCR3に結合するケモカインの産生を刺激し得る。λIFN(IFN1、III型IFNまたはIL28およびIL-29)は、I型IFNと相同性、発現様式および抗ウイルス機能を共有するインターフェロンの新たなクラスを構成する(Lazearら、2015b;Wackら、2015)。これらは、I型IFNのものと著しく類似しているように思われる下流シグナル伝達を誘導し、ISGの発現および抗ウイルス応答の誘導を駆動する(Durbinら、2013;Mendozaら、2017)。しかしながら、III型インターフェロンは、炎症促進性応答または免疫病理学を制限するのに重要な役割を果たす。
喘息は、しばしば自然にまたは治療中に可逆性の、通常は気道過敏および可変気流閉塞に関連する、気道の慢性炎症性障害である(WTO、2007)。小児、青年期およびあまり頻度は高くないが成人における喘息憎悪のおよそ80~85%がウイルス性上気道ウイルス感染症に関連しており、ライノウイルス(RV)がこれらのウイルス関連憎悪の約60~70%を占める。ウイルス感染症は全年齢の小児で喘鳴病と密接に関連している。RSVが細気管支炎またはクループの主原因物質である一方で、ライノウイルス(RV)はその後に喘鳴小児で最も一般的に検出される。これらのウイルスのいずれかによって誘発される重篤な呼吸器病はその後の喘息発症に関連し、RV感染症で喘鳴している若齢小児でリスクが最も大きい(JarttiおよびGern、2017)。
さらに、本発明の株を含むワクチンは、任意の適切な公知の方法を使用して調製され得、薬学的に許容される担体またはアジュバントを含み得る。
プレバイオティクス
プロバイオティクス生物の導入は、適切な担体中の微生物の摂取によって達成される。大腸内でのこれらのプロバイオティクス株の増殖を促進するような媒体を提供することが有利となると予想される。1種または複数のオリゴ糖、多糖または他のプレバイオティクスの添加は、胃腸管内の乳酸細菌の増殖を増強する。プレバイオティクスは、プラスの価値があると考えられる常在菌、例えばビフィドバクテリウム属菌、乳酸桿菌によって結腸内で特異的に発酵される任意の非生存食物成分を指す。プレバイオティクスの種類には、フルクトース、キシロース、大豆、ガラクトース、グルコースおよびマンノースを含有するものが含まれ得る。プロバイオティクス株と1種または複数のプレバイオティクス化合物の組合せ投与は、インビボで投与されたプロバイオティクスの増殖を増強し、より明白な健康上の利益をもたらし得るので、シンバイオティクスと呼ばれる。
他の有効成分
プロバイオティクス株は、それ自体で、あるいは上記のように他のプロバイオティクスおよび/またはプレバイオティクス物質と共に、予防的にまたは治療方法として投与され得ることが理解されると予想される。さらに、細菌は、他の活性物質、例えば炎症または他の障害、特に免疫学的関与を有するものを治療するために使用されるものを使用した予防的または治療体制の一部として使用され得る。このような組合せは、単一製剤で、または同時にもしくは異なる時に、同じもしくは異なる投与経路を使用して投与される別々の製剤として投与され得る。
医薬組成物

- 受託番号NCIMB42020を有するビフィドバクテリウム・ロンガム株。
- 生物学的に純粋な培養物の形態である、請求項1に記載のビフィドバクテリウム株。
- 請求項1に記載のビフィドバクテリウム株の単離株。
- 生細胞の形態である、請求項1~3のいずれか1項に記載のビフィドバクテリウム株。
- 非生細胞の形態である、請求項1~3のいずれか1項に記載のビフィドバクテリウム株。
- 細菌ブロスの形態又は凍結乾燥粉末の形態である、請求項1~5のいずれか1項に記載のビフィドバクテリウム株。
- 請求項1~6のいずれか1項に記載のビフィドバクテリウム株を含む、製剤。
- 別のプロバイオティクス物質及び/又は別のプレバイオティクス物質をさらに含む、請求項7に記載の製剤。
- 摂取可能な担体をさらに含む、請求項7又は8に記載の製剤。
- 摂取可能な担体が、カプセル、錠剤又は粉末のような薬学的に許容される担体である、請求項9に記載の製剤。

- ウイルスが、インフルエンザウイルス、ライノウイルス及び呼吸器合胞体ウイルスから選択される呼吸器ウイルスである、請求項22又は23に記載のビフィドバクテリウム株又は製剤。

炎症性肺疾患と診断されている;
呼吸器感染症に対する感受性が増加している;
肥満である;
急性呼吸窮迫症候群(ARDS)患者である;
喘息患者である;又は
