Description
The present invention relates to a balloon catheter used for removing foreign matter in the gastrointestinal tract.
For the treatment of stones occurring in the bile duct and urethra, a balloon catheter or a basket catheter (basket forceps) made by knitting a shape memory alloy wire such as SUS stranded wire or Ni-Ti into a cage shape is used. In the procedure using a balloon catheter, for example, the balloon is inflated with air on the distal side of the foreign body in the bile duct, and the balloon catheter is moved to the proximal side to move the foreign body to the duodenum through the papilla. Since the inside of the bile duct gradually narrows toward the papilla side, the operation of moving the balloon catheter to the proximal side is performed while contracting the balloon according to the diameter of the bile duct. However, the expansion or contraction of the balloon did not follow the air injection or suction operation, and the balloon sometimes expanded or contracted rapidly. If the balloon contracts rapidly, foreign matter may slip through and remain. Therefore, a method of controlling the diameter of the balloon is being studied.
For example,
However, the catheter described in
The balloon catheter of the present invention that has solved the above problems is used for removing foreign substances in the gastrointestinal tract, and is connected to a shaft extending in the perspective direction and the distal side of the shaft. It has a balloon, and the shaft has a first lumen into which the liquid flowing into the balloon is injected and a second lumen into which the guide wire is inserted, in the first section proximal to the balloon. The second lumen is located within the first lumen, the shaft is proximal to the balloon and distal to the first section, and the second lumen is located outside the first lumen. It has a gist in that it has a section. Since the balloon catheter of the present invention expands and contracts the balloon using a liquid, it is easy for the change in the outer diameter of the balloon to follow the operation on the hand side, and it is easy to finely adjust the outer diameter of the balloon even when the balloon is contracted. Further, by arranging the second lumen in the first lumen in the first section of the shaft, the cross-sectional area of the first lumen can be increased, so that the contraction speed of the balloon can be increased. Further, by providing the second section in which the second lumen is arranged outside the first lumen on the distal side of the first section, the portion protruding out of the endoscope on the distal side of the first section. Since the outer diameter of the shaft can be reduced, the insertability of the catheter is improved.

1. 1. Balloon catheter The balloon catheter of the present invention is used for removing foreign substances in the gastrointestinal tract, and has a shaft extending in the perspective direction and a balloon connected to the distal side of the shaft. However, the shaft has a first lumen into which the liquid flowing into the balloon is injected and a second lumen through which the guide wire is inserted, and is contained in the first lumen in the first section proximal to the balloon. The second lumen is located and the shaft has a second section in which the second lumen is located outside the first lumen, proximal to the balloon and distal to the first section. There is. The cross-sectional area of the first lumen refers to the cross-sectional area of the portion where the liquid for balloon expansion can be injected, and the portion excluding other members (including the lumen) existing in the first lumen is cut off. The area.
The balloon catheter of the present invention is used for removing foreign substances in the gastrointestinal tract, for example, moving stones generated in the bile duct to the duodenal side through the papilla. The balloon catheter is delivered to the treatment site via the endoscopic forceps channel. As this endoscope, one having a forceps channel having a diameter of 3.2 mm or 3.7 mm is generally used. Hereinafter, the balloon catheter may be simply referred to as a "catheter". The catheter of the present invention can be applied to both the rapid exchange type and the over-the-wire type. The proximal side of the catheter refers to the direction toward the user's (operator's) hand side with respect to the extending direction of the catheter, and the distal side refers to the direction opposite to the proximal side (that is, the direction toward the treatment target side). .. Further, the direction from the proximal side to the distal side of the catheter is referred to as an axial direction or a perspective direction.
Hereinafter, the catheter according to the embodiment of the present invention will be described with reference to FIGS. 1 to 8. FIG. 1 is a side view (partial cross-sectional view) of the balloon catheter according to the embodiment of the present invention, and FIGS. 2 and 3 are along lines II-II and III-III of the balloon catheter shown in FIG. 1, respectively. It is a cross-sectional view. 4 to 5 are side views (partial cross-sectional views) of the distal side of the balloon catheter shown in FIG. 1, and FIG. 6 is a side view (partial cross-sectional view) showing a modified example of the balloon catheter shown in FIG. Is. 7 to 8 are modified examples of the cross-sectional view of the balloon catheter shown in FIG. 1 along the line II-II. Note that FIG. 4 shows the contracted state of the balloon, and FIGS. 1, 5 and 6 show the balloon expanded to the extent that it does not exceed the distal end and the proximal end of the operating portion of the balloon that is not joined to the shaft. It shows the state of being made.
As shown in FIG. 1, the

As shown in FIGS. 2 to 3, the
A plurality of first lumens L1 may be provided on the
The
Since the outer diameter of the
At the transition between the proximal shaft having the cross section shown in FIG. 2 and the distal shaft having the cross section shown in FIG. 3, the cross-sectional structure of the shaft gradually changes and switches. The transition portion is formed, for example, by heating the shaft. The proximal shaft and the distal shaft are connected, for example, by the following method. (1) Prepare the distal shaft, the proximal first shaft, the second shaft, and the third shaft, which are formed in a cylindrical shape. (2) The second shaft and the third shaft are arranged in the lumen of the first shaft on the proximal side. (3) For example, a core material along the inner diameter of each shaft is inserted into the second shaft and the third shaft. (4) A core material having the shape of the first lumen L1 of the distal shaft is inserted into the lumen of the first shaft on the proximal side. (5) The distal end face of the proximal first shaft and the proximal end face of the distal shaft are abutted against each other, or the distal portion of the proximal shaft is inserted into the lumen of the proximal portion of the distal shaft. , Or insert the proximal part of the distal shaft into the lumen of the distal part of the proximal shaft. At that time, the second shaft, the third shaft and all the core materials are arranged in the lumen of the distal shaft. (6) A heat-resistant member is placed in a portion where the first shaft on the proximal side and the shaft on the distal side abut or overlap each other, and heated to form a transition portion. (7) By arranging a heat-resistant member on the outside of the distal shaft and heating it, the distal shaft having the cross section shown in FIG. 3 is formed. For example, by the above method, the proximal shaft having the cross section shown in FIG. 2 and the distal shaft having the cross section shown in FIG. 3 do not have a step on the outer surface of the shaft or the first lumen. Is smoothly connected to.

Although not shown, a second tapered portion formed tapered toward the distal side may be provided on the proximal side of the

In the contracted state of the

As shown in FIG. 2, in the present invention, the second lumen L2 is arranged in the first lumen L1 in the
The length of the

The second lumen L2 preferably extends over the entire perspective of the
In order to adjust the followability of the change in the outer diameter of the
As shown in FIGS. 1 and 3, the
In the axial direction, the
The proximal end of the second section 2B is preferably located 50 mm or more proximally from the proximal end of the
The distal end of the second section 2B may overlap with the proximal end of the
As shown in FIGS. 2 to 3, it is preferred that the
When the
The
In the
As shown in FIG. 2, the
As another embodiment, as shown in FIG. 7, the
As yet another embodiment, as shown in FIG. 8, in the
Although not shown, in yet another embodiment, the
The
In the embodiments of FIGS. 2 and 7 to 8, one shaft and the other shaft may be made of the same material or may be made of different materials. For example, in the case of the embodiment shown in FIG. 2, the hydrophilicity of the material constituting the first shaft S1 can be made higher than that of the material constituting the second shaft S2. By making the wall surface of the first shaft S1 hydrophilic, it becomes difficult for contrast media, chemicals, and the like contained in the liquid to adhere to the wall surface of the first shaft S1.
In the wall surface of the
In order to improve the slidability of the guide wire, the wall surface of the second lumen L2 may be coated with a lubricant containing a fluororesin such as polytetrafluoroethylene (PTFE) or silicone oil.

It is preferable that the cross-sectional area of the second lumen L2 is larger than the cross-sectional area of the third lumen L3 in at least a part of the

It is preferable that the cross-sectional area of the first lumen L1 is larger than the cross-sectional area of the third lumen L3 in at least a part of the second section 2B of the

Next, it is preferable to remove the air accumulated inside the
In the degassing step, it is preferable that another syringe (fourth syringe 40) is connected to the proximal end of the

(Expansion process)
The expansion of the
In the expansion step of the

(Balloon contraction test I: Change in outer diameter of balloon)
After performing the balloon contraction test I in an environment of a temperature of 26.0 degrees, the outer diameter of the balloon was measured using a laser outer diameter measuring instrument LS-7600 manufactured by KEYENCE. The results are shown in FIG. 14 and Table 2. A syringe SS-05Lz (lock type) manufactured by Terumo Corporation was used as the first syringe. RO water having a water temperature of 24.8 degrees was used as the water for balloon expansion. The details of the test are as described in "1. Balloon catheter".
[Balloon contraction test I]
(1) A first syringe having a first cylindrical body having an inner diameter of 15 mm and a first pusher that moves in the perspective direction in the first tubular body is connected to the proximal side of the shaft to form a first lumen. Communicate the lumen of the first cylinder.
(2) Water exists in the first tubular body and the first lumen, and the position of the first pusher in the contracted state of the balloon is set to the initial position (0 mm).
(3) By moving the first pusher from the initial position to the distal side, water is injected into the inside of the balloon until the outer diameter of the balloon becomes 15 mm.
(4) Move the first pusher to the proximal side to the initial position at 2 mm / sec.
In (3) above, Table 3 shows the distance from the initial position of the first pusher when the balloon is expanded to an outer diameter of 15 mm, and the amount of water injected.
The balloon catheter of Example 1 had a larger cross-sectional area of the first lumen at the second position than that of Comparative Example 1, and had a larger cross-sectional area of the first lumen at the first position than that of Comparative Example 2. In Example 1, the outer diameter of the balloon changes significantly following the movement of the first pusher toward the proximal side as compared with Comparative Examples 1 and 2, and the rate of change in the outer diameter increases particularly as it approaches the initial position. I grew up. In Comparative Examples 1 and 2, even if the first pusher is returned to the initial position, the outer diameter is reduced only to 11.5 to 11.6 mm, whereas in Example 1, the outer diameter of the balloon at the initial position is It was 6.2 mm. From this result, it was found that the balloon catheter of the present invention easily follows the change in the outer diameter of the balloon to the operation on the hand side, and it is easy to finely adjust the outer diameter of the balloon even during contraction.

A balloon contraction test II was performed in an environment of 25 degrees Celsius, and the contraction rate of the balloon was calculated. The results are shown in Table 4. The contraction rates of Comparative Examples 1 and 2 shown in Table 4 show the average value measured three times. As the pressure sensor, a high-precision type pressure sensor for medium and high pressure (model number: VPRT-A3-5MPa-5) manufactured by Balcom Co., Ltd. was used. RO water having a water temperature of 25 degrees was used as the water for expanding the balloon. The details of the test are as described in "1. Balloon catheter". The results are shown in Table 4.

(3) By moving the second pusher to the distal side, at least 95% or more of the amount of water in the second syringe is injected into the inside of the balloon.
(4) Operate the three-way stopcock to shut off the second syringe from the balloon, create a negative pressure in the back-locked state, and then operate the three-way stopcock to connect the second syringe and the balloon to operate the back lock. The time from the start of suction of water at -100 kPa to the completion of the contraction of the balloon is measured.

(Inner diameter of the cylindrical body of the syringe and the load of the pusher)
For the catheters of Examples 1 and Comparative Examples 1 and 2, the load of the pusher (unit: kgf) when the type of the syringe connected so as to communicate with the first lumen was changed was measured. The measurement was performed in an environment with a temperature of 25 degrees. When 3 mL of RO water with a water temperature of 25 degrees was placed in the tubular body of each syringe and the tubular body of the syringe was moved to the distal side with a force gauge at a speed of 1 mm / sec to expand the balloon with RO water. The maximum load was measured. A digital force gauge (FEP-10) manufactured by Nidec-Shimpo Corporation was used to measure the maximum load. The results are shown in Table 5.
It was found that the balloon catheter of Example 1 can reduce the load of the pusher as compared with Comparative Examples 1 and 2.
