Description
睡眠中の患者の呼吸パターンを観察するステップ、
前記観察される呼吸パターンが失調性呼吸の発現を含んでいるかを検出するステップ、及び
失調性呼吸の発現が検出される場合、患者の個人的に関連付けられたしきい値より上である、供給される呼吸ガスの分時量で、呼吸ガスのフローを患者の気道に供給することにより、第1の既定される時間の期間にわたり患者を換気する及び患者の自発呼吸を除去にするステップ
を含む。
−睡眠中の患者の呼吸パターンを観察するセンサであり、呼吸信号を生成するセンサ、
−呼吸ガスのフローを発生させる圧力発生器、
−前記発生した呼吸ガスのフローを患者の気道に誘導する患者インタフェースを含む圧力回路、及び
−制御器
を有し、前記制御器は、
前記観察される呼吸パターンが失調性呼吸の発現を含んでいるかを検出するために、前記呼吸信号を評価する、並びに
前記呼吸パターンにおいて失調性呼吸の発現が検出される場合、第1の既定される時間の期間にわたり患者を換気する及び患者の自発呼吸を除去するように、患者の個人的に関連付けられるしきい値より上である分時量で、呼吸ガスのフローを前記第1の既定される時間の期間にわたり供給するように前記圧力発生器を制御する、
ように構成される。
−呼吸信号に基づいて、患者の分時換気量、呼吸頻度及び呼吸振幅の1つ以上を決定する、
−前記分時換気量、前記呼吸頻度及び前記呼吸振幅の1つ以上の時間を通じての変化を評価する、並びに
−前記患者の前記分時換気量、前記呼吸頻度及び前記呼吸振幅の1つ以上の時間を通じての前記変化が既定されるしきい値よりも大きい場合、前記患者の観察される呼吸パターンの一部を失調性呼吸の発現として特定する
ように構成される。
(i)前記既定される時間の期間の後、少なくとも第2の既定される時間の期間にわたり呼吸ガスの供給を中断するステップ、
(ii)前記第2の既定される時間の期間中に患者の呼吸パターンを観察するステップ、
(iii)前記第2の時間の期間中に、失調性呼吸の発現が検出される場合、第3の既定される時間の期間にわたり再び患者を換気する及び患者の自発呼吸を除去するステップ、
第2の時間の期間中に、失調性呼吸の発現がもはや検出されなくなるまで、ステップ(i)−(iii)を繰り返すステップを有する。
(i)前記既定される時間の期間の後、少なくとも第2の既定される時間の期間にわたり呼吸ガスの供給を中断するように前記圧力制御器を制御する、
(ii)前記第2の既定される時間の期間中に患者の呼吸パターンを観察するように前記センサを制御する、及び
(iii)前記第2の時間の期間中に、失調性呼吸の発現が検出される場合、前記患者を換気する及び患者の自発呼吸を除去するように、患者の個人的に関連付けられるしきい値より上である分時量で、第3の既定される時間の期間にわたり再び呼吸ガスのフローを供給するように、圧力発生器を制御する
ように構成され、ここで制御器は、前記第2の時間の期間中に、失調性呼吸の発現がもはや検出されなくなるまで、制御ステップ(i)−(iii)を繰り返すように構成される。
Claims (6)
- 失調性呼吸を患う患者の呼吸パターンを再同期させる装置において、前記装置は、
−睡眠中の前記患者の呼吸パターンを観察するセンサであり、呼吸信号を生成する前記センサ、
−呼吸ガスのフローを発生させる圧力発生器、
−前記患者の気道に前記発生した呼吸ガスのフローを誘導するための患者インタフェースを含む患者回路、及び
−制御器
を有し、前記制御器は、
−前記観察される呼吸パターンが失調性呼吸の発現を含んでいるか検出するために、前記呼吸信号を評価する、及び
−前記呼吸信号において失調性呼吸の発現が検出される場合、前記患者の個人的に関連付けられるしきい値より上である分時量で、前記呼吸ガスのフローを第1の既定される時間の期間(Δt1)にわたり供給するように前記圧力発生器を制御し、前記第1の既定される時間の期間(Δt 1 )の間、前記患者を換気し、前記患者の自発呼吸を除去している
ように構成される、装置。 - 前記センサは、フローセンサ、圧力センサ、カメラ、レーダーセンサ、加速度計、圧電センサ及び電気化学ガスセンサの少なくとも1つを含む、請求項1に記載の装置。
- 前記制御器は、
−前記呼吸信号に基づいて、前記患者の分時換気量、呼吸頻度及び呼吸振幅の1つ以上を決定する、
−前記分時換気量、前記呼吸頻度及び前記呼吸振幅の1つ以上の時間を通じての変化を評価する、並びに
−前記患者の前記分時換気量、前記呼吸頻度及び前記呼吸振幅の1つ以上の時間を通じての前記変化が既定されるしきい値より大きい場合、前記観察される患者の呼吸パターンの一部を失調性呼吸の発現として特定する
ように構成される、請求項1に記載の装置。 - 前記制御器は、
(i)前記第1の既定される時間の期間の後、少なくとも第2の既定される時間の期間(Δt2)にわたり前記呼吸ガスの供給を中断するように前記圧力発生器を制御する、
(ii)前記第2の既定される時間の期間(Δt2)中に前記患者の呼吸パターンを観察するように前記センサを制御する、並びに
(iii)前記第2の既定される時間の期間(Δt2)中に失調性呼吸の発現が検出される場合、前記患者の前記個人的に関連付けられるしきい値より上である分時量で、再び前記呼吸ガスのフローを第3の既定される時間の期間(Δt 3 )にわたり供給するように前記圧力発生器を制御し、前記第3の既定される時間の期間(Δt 3 )の間、前記患者を換気し、前記患者の自発呼吸を除去している
ように構成され、前記制御器は、前記第2の既定される時間の期間(Δt2)中に失調性呼吸の発現がもはや検出されなくなるまで、制御ステップ(i)−(iii)を繰り返すように構成される、請求項1に記載の装置。 - 前記制御器により前記制御ステップ(iii)が繰り返されるたび、前記制御器は、以前の制御ステップ(iii)において供給した前記呼吸ガスのフローに比べ増大した分時量で前記呼吸ガスのフローを供給するように前記圧力発生器を制御する、請求項4に記載の装置。
- 前記制御器は、前記観察される呼吸パターンにおいて失調性呼吸の発現が検出されないとき、前記呼吸信号に基づいて、前記患者の分時換気量を決定することにより、前記患者の前記個人的に関連付けられるしきい値を決定するように構成される、請求項1に記載の装置。
