Description
D−グルコシル−N−アシルスフィンゴシン+H2O=D−グルコース+N−アシルスフィンゴシン。
一実施形態において、GBA関連障害の治療で用いられる、(+)−R−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドシトラート;(−)−S−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドシトラート;(+)−R−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドマレアート;および(−)−S−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドマレアートから選択される化合物が提供される。
−GBA活性を増加させる方法、
−GBAレベル(または量)を増加させる方法、
−活性変異体GBAの量を増加させる方法、
−活性野生型GBAの量を増加させる方法、
−ER保持変異体GBAの折り畳みを増強する方法、
−処理/成熟したGBAの量を増加させる方法、
−成熟(ポストER)GBAの量を増加させる方法、および/または
−リソソームに到達する成熟GBAの量を増加させる方法。
別の態様において、GBA酵素レベルおよび/または活性が低下している個体においてゴーシェ病以外のグルコセレブロシダーゼ(GBA)関連障害を発症する危険性を減少させる方法で用いる、N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド、その立体異性体とその酸付加塩から選択される医薬品有効成分が提供される。
グルコセレブロシダーゼ活性は、当技術分野で既知の方法によって評価されることができる。例えば、グルコセレブロシダーゼ活性は、哺乳動物の脳脊髄液から測定することができる。一部の実施形態において、哺乳動物はGBA遺伝子の野生型である。「野生型」という用語は、タンパク質のレベルおよび/または酵素活性に影響を及ぼすことが知られている検出可能な変異を含まない遺伝子またはタンパク質を指す。
本明細書に参照するアリモクロモルは、N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド(アリモクロモル)、その立体異性体およびその酸付加塩から選択される医薬品有効成分(API)を包含する。アリモクロモルは、さらに、例えば国際公開第00/50403号に記載されている。
医薬品有効成分を未加工化学物質として投与することは可能であるが、一部の実施形態において該成分を医薬製剤の形態で提供するのが好ましい。したがって、医薬組成物などの組成物、すなわち、本明細書で定義する医薬品有効成分を含む薬学的に安全な組成物を本明細書とともに提供する。一実施形態において、組成物は、薬学的および/または生理学的に許容される担体または賦形剤を含む。
本明細書で定義するものと同じものを含む医薬品有効成分または組成物は、一実施形態において、それを必要とする個体に薬学的有効用量または治療有効量で投与される。
好ましい投与経路は治療される対象の全身状態および年齢、治療される状態の性質、体内で治療される組織の位置および選択される活性成分に依存することを理解されたい。
一実施形態において、投与経路により、最終的に望ましい作用の部位を標的とするために血流に生理活性剤を導入することが可能になる。
非経口投与は、経口/経腸経路ではない任意の投与経路であり、それによって生理活性剤が肝臓での初回通過分解を回避する。したがって、非経口投与としては、任意の注射および注入、例えば、ボラス注射または持続注入、例えば静脈内投与、筋肉内投与または皮下投与が挙げられる。さらに、非経口投与としては、吸入法および局所投与が挙げられる。
一実施形態において、医薬品有効成分または組成物を局所治療、すなわち作用部位(複数可)に直接導入する治療として使用する。したがって、医薬品有効成分は直接皮膚または粘膜に適用されてもよく、または作用部位に、例えば病変組織、もしくは直接病変組織につながる終動脈に注入されてもよい。
一態様は、他の治療法と併せて、ゴーシェ病(GD)以外のグルコセレブロシダーゼ(GBA)関連障害を治療する方法で用いる、N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド、その立体異性体およびその(アリモクロモル)酸付加塩から選択される医薬品有効成分を提供することである。
実施例1:ゴーシェ病II型ヘテロ接合体(パーキンソン病遺伝子型)における用量依存反応−BiPおよびGBA誘導
材料および方法
細胞培養:
初代ヒト線維芽細胞株を、非必須アミノ酸(NEAA)、1%ペニシリン−ストレプトマイシンおよび12%ウシ胎児血清(FCS)を補充したDMEM中で、標準細胞培養条件下(37℃および5%CO2)で培養した。ヒト線維芽細胞株を1:2または1:3の分割比で、1週間に1〜2回継代した。16〜26継代頃に、複製老化の徴候が観察されない(目視検査)細胞を実験のために用いた。
細胞をPBSに回収し、4℃で5分間、3500rpmで遠心分離した。細胞ペレットを、プロテアーゼ阻害剤を含有する1×抽出緩衝液(Enzo Life Science)に溶解し、超音波処理して、4℃で10分間、13000rpmでの遠心分離により清澄化させた。タンパク質濃度をBCAアッセイで測定した。約10〜20μgのタンパク質を含有する試料を糖タンパク質変性緩衝液(New England Biolabs)で希釈し、100℃で10分間のインキュベーションにより変性させた。試料を37℃で1時間、EndoH(New England Biolabs)とともに、またはEndoHなしでインキュベートし、Laemmliサンプル緩衝液を加え、次いで試料を、TGXゲルシステム(Bio−Rad)を使用するSDS−PAGEにかけた。ニトロセルロース膜(Trans−Blot Turbo,Bio−Rad)に移した後に、この膜をPonceau Sで短時間、染色し、続いて0.1%Tweenを加えたPBS(PBS−T)に5%脱脂乳を溶解した液でブロックした。パラフィルム被覆ガラスプレート上で一次抗体(1:500から1:2000に希釈)とともにインキュベーションを4℃で終夜行った。膜をPBS−Tで洗浄し、PBS−Tに5%脱脂乳を溶解した液で1:10,000に希釈した二次抗体とともに1時間インキュベートした。SuperSignal(商標)West Dura Extended Duration Substrate(Life technologies)を使用してブロットを展開し、次いでG−boxシステム(Syngene)を使用して可視化した。
一次細胞におけるアリモクロモル誘導によるER Hsp70(BiP)の用量依存増加:
アリモクロモルは、ヒートショックタンパク質、例えばヒートショックタンパク質70(HSP70)の発現レベルを上昇させることが報告されている(Kieranら,Nature Medicine, 2004)。
GBAタンパク質レベルに対するアリモクロモルの効果も、複合GBA対立遺伝子L444P、A456P、V460Vに対してヘテロ接合のヒト線維細胞株で評価した。ERシャペロンBiPの発現増加に一致して、アリモクロモルで処置した細胞においてGBAの総レベルの用量依存増加が見られる。

GBA活性は、4−メチルウンベリフェリルβ−D−グルコピラノシド(4−MUG)基質を用いる「無傷細胞」GBAアッセイ(Muら,Cell,2008)を使用して測定した。手短に言うと、線維芽細胞を12ウェルプレートに播種し、表示濃度のアリモクロモルを使用して4週間、生物学上の三つ組で処置した。培地は2〜3日ごとに新鮮な化合物を補充し、実験期間中、細胞を2回分割した。4週間の処置の後、細胞を96ウェルプレートに移し、基質としてpH4.0の4−MUGを用いてGBA活性を測定した。放出された4−MUフルオロフォアを蛍光単位(FLU)として定量化し、平行プレートのクリスタルバイオレット染色を用いて細胞密度に正規化した。正規化データは、任意単位(平均値±SD)として記録する。
GBA変異を有する一次細胞においてGBA活性に対するアリモクロモルの効果を、遺伝子型L444P/L444P、A456P、V460Vを有するゴーシェ病II型患者由来の線維芽細胞において評価した。アリモクロモル処置は用量依存的にGBA活性を増加させることが観察された。注目すべきことに、50μMのアリモクロモルによって誘導されたGBA活性の増加は、無症候の個体由来のL444P、A456P、V460V対立遺伝子(図3において灰色の線で印がついている)に対するヘテロ接合細胞の活性レベルに対応する。

GBAのさらなる変異を有する一次細胞においてGBA活性に対するアリモクロモルの効果を、表示された遺伝子型の細胞をアリモクロモルで処置することによって評価した。N370S/V394LおよびN370S/1−BP ins 84Gの2種類のGDI型細胞株においてアリモクロモルがGBA活性を用量依存的に増加させることを我々のデータは示している。重要なことに、1−BP ins 84Gはヌル対立遺伝子であると考えられるので、アリモクロモルがN370S変異の活性を増加させることをこれらの結果は示している。

パーキンソン病においてN370S変異に対するアリモクロモルの効果を調べるために、N370S変異を有するパーキンソン病患者由来の一次細胞をアリモクロモルで処置した。アリモクロモルがパーキンソン病患者由来の細胞においてN370S GBA活性を用量依存的に増加させることを我々のデータは示している。

野生型GBAタンパク質に対するアリモクロモルの効果を調べるために、GBA変異のない健常者由来(+/+)の一次細胞をアリモクロモルで処置した。アリモクロモル処置によって、大きさが異なる3種類の細胞株すべてにおいて野生型GBA活性が用量依存的に増加したことが観察された。

活性GBAは、蛍光活性ベースのプローブ(ABP)ME569によって選択的に標識されることが可能である(Witteら,2010)。手短に言うと、線維芽細胞をシャーレに播種し、表示濃度のアリモクロモルを使用して5日間、生物学的二つ組で処置した。培地は2〜3日ごとに新鮮な化合物を補充した。細胞はPBSで回収し、タンパク質を抽出し、次いでBCAアッセイを用いて濃度を決定した。同量の総タンパク質をME569とともに37℃で30分間インキュベートした。ローディング緩衝液を添加し、試料を98℃で5分間インキュベートし、次いでTGXゲルシステム(Bio−Rad)を使用するSDS−PAGEにかけた。ゲル電気泳動の後、赤色LED/705Mフィルター(G−box,Syngene)を用いて蛍光を検出した。Syngene製ソフトウェアGeneTools 4.03.01.0版を使用して標識GBAの量を定量化した。正規化データは、模擬処置対照細胞と比較して倍率変化(平均値±SEM、n=3〜4)として記録する。
蛍光ABPで標識したGBAの量に対するアリモクロモルの効果を、表示された遺伝子型のゴーシェ病患者由来の一次細胞で評価した。アリモクロモルがGDI型細胞株(N370S/V394L)およびGDII型細胞株(G325R/C342G)においてGBA標識を用量依存的に増加させることを我々のデータは示している。高用量のアリモクロモルだけをGDII型細胞株(L444Pに対してホモ接合体)において評価した。さらに、アリモクロモルは、この細胞株において蛍光ABPで標識することができるGBAの量を増加させる。
T1、I型、TII,II型およびTIII,III型)由来の一次細胞において、活性変異体GBAの量を増加させることをこれらのデータは示している。

GDI型一次細胞におけるアリモクロモル誘導によるER Hsp70(BiP)の用量依存増加:
アリモクロモルは、ヒートショックタンパク質、例えば、ヒートショックタンパク質70(HSP70)の発現レベルを増加させることが報告されている(Kieranら,Nature Medicine,2004)。一次細胞においてER Hsp70(BiP)発現レベルに対するアリモクロモルの効果を評価するために、GDI型患者由来のヒト線維芽細胞を0、25、100、200もしくは400μMのアリモクロモル(N370S/V394L)または0、100、200もしくは400μMのアリモクロモル(N370S/1−BP ins 84G)を用いて5日間処置した。次いで細胞をウェスタンブロット解析のために回収した。結果は、アリモクロモルがゴーシェ病I型個体由来のヒト線維芽細胞株においてBiP発現レベルを用量依存的に増加させることを実証する。これは、BiP発現増加によってアリモクロモルがER保持変異体GBAの折り畳みの増強をもたらし得ることを示唆する。
GBAタンパク質レベルに対するアリモクロモルの効果もN370S/1−BP ins 84G遺伝子型を有するゴーシェI型患者由来のヒト線維芽細胞株で評価した。ERシャペロンBiPの発現増加に一致して、GBAの総レベルの用量依存的増加は、この個体由来のアリモクロモル処置細胞で見られる。さらに、EndoH耐性画分の増加は、アリモクロモルが細胞内の処理/成熟したGBAの量を増加させることを示す。
材料および方法

GBAタンパク質レベルおよび成熟化に対するアリモクロモルの効果もGDII型一次細胞株において評価した。ERシャペロンBiPの発現増加に一致して、GBAの総レベルの用量依存的増加は、これらの個体由来のアリモクロモル処置細胞で見られる。さらに、EndoH耐性画分の増加は、アリモクロモルがGDII型細胞において成熟(ポストER)GBAの量を増加させることを示す。

さらに、GBAのEndoH耐性画分の増加は、アリモクロモルがGDIII型細胞において成熟GBAの量を増加させることを示す。

パーキンソン病GBA一次細胞におけるアリモクロモル誘導によるER Hsp70(BiP)の用量依存増加:
GBA欠損パーキンソン病(PD−GBA)を有する個体由来の一次細胞においてER Hsp70(BiP)発現レベルに対するアリモクロモルの効果を評価するために、PD−GBA(N370S/N370S)を有する個体由来のヒト線維芽細胞を表示濃度のアリモクロモルで5日間処置した。次いで細胞をウェスタンブロット解析のために回収した。
材料および方法
細胞培養:
ヒト多分化能成体幹細胞(MASC)を、GD個体由来の皮膚生検から単離した。MASCの遺伝子型を以下に示す。細胞は、Bergaminら.Orphanet Journal of Rare Diseases 2013に記載のニューロン運命に沿って分化するよう誘導した。幹細胞の表面免疫表現型は、FACSによって分析した。幹細胞および神経マーカー発現を免疫蛍光法で評価した。
アリモクロモルはGDI型およびGDIII型の個体からの皮膚由来ヒト多分化能成体幹細胞の神経分化に影響を及ぼさない:
神経分化に対するアリモクロモルの効果を評価するために、模擬またはアリモクロモルで処置する一方で、GD個体由来のMASCを分化するよう誘導した。結果は、神経マーカーのチューブリンβ3およびNeuNの発現がアリモクロモルに影響を受けなかったことを示す。
アリモクロモルがGDI型(N370S/Y133*)を有する個体由来の神経において、およびGDIII型(F213I/L444P、L444P/L444PまたはIVS2+1G>A/N188S)を有する3個体において変異体GBA活性を増加させることを我々は見出した。

Claims (15)
- グルコセレブロシダーゼ(GBA)関連パーキンソン病(PD)またはGBA関連パーキンソン症候群を治療する方法に使用するための、N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド、その立体異性体、およびその酸付加塩から選択される化合物を含む医薬組成物。
- GBA関連パーキンソン病を治療する方法に使用するための、請求項1に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群が、GBA酵素レベルの低下および/またはGBA酵素活性の低下に関連する、請求項1または2に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群が、1または複数の個別のGBA遺伝子変異に関連する、請求項1〜3のいずれか1項に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群が、1または複数のヘテロ接合GBA遺伝子変異に関連する、請求項1〜4のいずれか1項に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群を有する個体が、ゴーシェ病により臨床的に影響を受けない状態を存続している、請求項1〜5のいずれか1項に記載の医薬組成物。
- 前記1または複数の個別のGBA遺伝子変異が、a)軽度(GD I型に関連する)であるか、b)重度(GD II型およびIII型に関連する)である、請求項4〜6のいずれか1項に記載の医薬組成物。
- 前記GBA遺伝子変異が、L444P、D409H、D409V、E235A、E340A、E326K、N370S、N370S/1−BP ins 84G、V394L、A456P、V460V、C342G、G325R、P415R、Y133*、F213I、N188SおよびIVS2+1G>A/N188Sからなる群から選択される、請求項4〜7のいずれか1項に記載の医薬組成物。
- 前記GBA関連パーキンソン病が、1または複数のホモ接合または複合ヘテロ接合GBA遺伝子変異に関連する、請求項1〜8のいずれか1項に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群を有する個体が、L444P、A456P、V460Vのヘテロ接合体である、請求項1〜9のいずれか1項に記載の医薬組成物。
- 前記GBA関連パーキンソン病またはパーキンソン症候群を有する個体が、
a)GBA変異保因者、例えば絶対保因者、例えばゴーシェ病に関して臨床的に影響を受けない保因者である、および/または、
b)臨床的に影響を受けないゴーシェ病患者の親または同胞である、
請求項1〜10のいずれか1項に記載の医薬組成物。 - 前記GBA関連パーキンソン病またはパーキンソン症候群が、
GBA酵素活性の低下および/またはGBA酵素レベルの低下に関連し、前記GBA遺伝子は野生型であり、かつGBA活性の低下がタンパク質活性の抑制または遺伝子/タンパク質の転写もしくは翻訳の阻止に起因する、または特発性である、
請求項1〜3のいずれか1項に記載の医薬組成物。 - 前記化合物が、
a)N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドのラセミ化合物である、
b)N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドの光学活性立体異性体である、
c)N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドの鏡像異性体である、
d)(+)−R−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド、および
(−)−(S)−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリド
からなる群から選択される、
e)N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドの酸付加塩である、
f)N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドシトラート、および
N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドマレアート
からなる群から選択される、または、
g)(+)−R−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドシトラート、
(−)−(S)−N−[2−ヒドロキシ−3−(1−ピペリジニル)−プロポキシ]−ピリジン−1−オキシド−3−カルボキシイミドイルクロリドシトラート、

- 前記治療が予防、治癒または改善のためである、請求項1〜14のいずれか1項に記載の医薬組成物。
