Description
を有し、グリア芽腫異種移植におけるαvβ3インテグリンの光学イメージング検出や乳癌の早期診断のためのその使用が、Contrast Media & Molecular Imaging 2011;6:449-458およびContrast Media & Molecular Imaging 2013; 8,350~360に開示されている。
−血液系における最適な持続時間、薬剤と標的との十分な接触時間を確保するのに十分な長さ(したがって、標的部位での効果的な蓄積)および同時に循環システムからの非結合薬剤の完全な除去;
−病理的な領域への薬剤の選択的取り込みと均一な分布の両方を含む、標的化腫瘍部位における選択的蓄積、および(腫瘍部位において)手術中の腫瘍領域全体の明確な画定を可能にする十分な保持;
−腫瘍マージンの汚染または浸潤組織の認識が損なわれるのを防ぐための、病理的な領域から正常組織に向かっての緩徐な放出速度および無視できる程度の(蛍光剤の)抽出および拡散;
を示し、加えて
−蛍光プローブは、励起光への長期間の暴露で退色しないまたは損なわれずに信号を提供しなければならない。
i)前記癌が疑われる腫瘍を含む目的領域(ROI)の術中光学イメージングを生成すること、および
ii)蛍光下で癌が疑われる腫瘍のマージンを検出すること
を含む。
i)個人の患者に適切な量の蛍光プローブDA364を提供すること;
ii)根治手術の下で癌が疑われる腫瘍を含む目的の領域の術中蛍光ベースの光学イメージングを生成すること;
iii)蛍光下の腫瘍のマージンを特定すること;および
iv)腫瘍の根治手術を行うこと
を含む方法に関する。
a)画像ガイド手術の前の、例えば1週間以内、好ましくは少なくとも24時間以内、より好ましくは24〜48時間の時間枠内に、DA364蛍光プローブの有効量を個人の患者に投与すること;
b)場合により切開(関連する身体領域の)し、および場合により介在している皮膚および組織を除去することにより、目的の腫瘍を含む個人の患者の身体領域を外科医の目視検査に曝露すること;
c)蛍光下で前記個人の患者の身体領域の術中イメージングを生成し、腫瘍マージンを検出すること;
d)検出されたマージンに従って腫瘍の治癒的切除を行うこと、
e)切除されたマージンの状態を蛍光下で評価し、切除した腫瘍周辺の陰性マージンの存在を確認するか、
f)腫瘍周囲のより広いマージンに従って切除を繰り返すか、あるいは
g)切除された腫瘍周辺の陰性(非蛍光)マージンに達するまで工程e)およびf)を繰り返すこと
を含み、これは本発明の追加の実施形態を構成する。
実施例1 NIR剤DA364の調製
NIR剤DA364の調製は、上記で引用した文献Contrast Media Mol. Imaging 2011, 6, 449-458に記載されているように、上記で例示した合成手順を用いることによって行った。
1)インテグリン標的化環状ペンタペプチドcRGD−NH2
インテグリン標的化ペプチド模倣体部分cRGD−NH2の調製は、実質的にWO2006/095234に記載されており、より詳細にはAngew. Chem. Int. Ed., 2010, 49, 7111-7115, J. Org. Chem. 2005, 70, 4124-4132およびChem Med. Chem. 2009, 4, 615-632において、それぞれ以下のスキームによる、個々の中間体および環状cRGD−NH2インテグリン標的骨格の調製が記載されている。
a.中間体5の合成

c.cRGD−NH2の合成
ホウ酸塩緩衝液pH9(4mL)中のCy5.5−NHSエステル(13mg、0.011mmol)(GE Healthcareから購入、製品コードPA15602)の溶液に、ホウ酸塩緩衝液pH9(3.5mL)中の環状cRGD−NH2骨格(8.8mg,0.011mmol)の溶液を加えた。反応混合物を室温で24時間撹拌し、光から保護した(pHは8.1になった)。反応終了時(HPLC−MSで評価)、粗反応物を5%CH3COOH水溶液(1.5mL)で希釈し、残留した反応溶媒を凍結乾燥により除去した。
粗生成物を分取HPLC(溶出液A:H2O+0.1%TFA、溶出液B:CH3CN+0.1TFA、流速:15mL/分、固定相:Waters Atlantis Prep T3 OBD5μm、19×100mm、検出 :UV、λ1:210nm、λ2:254nm)で精製して、凍結乾燥後、所望の生成物を暗青色の固体(11.8mg、0.0082mmol)として得た。収率:74.6%、HPLC純度:92.96%(面積%として)。
得られたNIR剤の分析的キャラクタリゼーションは、HPLC−MSおよび1H−NMRにより行った。
核磁気共鳴
1H−NMRスペクトルは、ロックチャネルとして重水素を有するBruker Avance-III(1H、600.13MHz)を用いて、DMSO−d6またはD2O中で298Kにて記録した。
記録された1H−NMRは、予想された構造と一致した。

実施例2:吸光係数の測定
材料と方法
デュアルビームパーキンエルマーラムダ40 UV-VIS分光光度計を使用して、NIR剤DA364の吸光モル係数を測定した。
1000μM(sol I:1.27mg、0.884mL)および1250μM(sol II:7.03mg、3.915mL)のPBS中でNIR剤を含む2つの標準ストック溶液を調製した。Table Aに示す希釈標準溶液は、PBSを用い、ストック溶液を適当なメスフラスコ中で希釈することにより調製した。各標準希釈液の吸光度をさらに希釈することなく測定した。
結果
各希釈標準溶液の吸光度を測定し、DA364のモル吸光係数を図1に示す線形回帰の傾きから計算した。この条件で測定された吸光係数は、208500M-1・cm-1(Cy5.5色素に関して報告された理論値250000M-1・cm-1の83%に相当)。
また、線形回帰の信頼性を検証するために、統計分析も行った(データ示さず)。
材料と方法
蛍光量子収率(Φ%)測定は、積分球アクセサリを備えたFluoroLog-3 1IHR-320(HORIBA-JobinYvon)により行った。
測定は、励起波長625nmを用い、450Wキセノン光源を用いて行った。検出は、645〜850nmのTBX-04検出器(励起および発光スリットは1.8nm)で行った。
DA364溶液の蛍光量子収率を、ヒト血清(Seronorm(商標) Human、NycomedPhama、バッチ1109514)およびリン酸緩衝生理食塩水(PBS)の両方で測定した。
NIR剤の溶液は、PBSおよびヒト血清中で0.1未満の吸光度で調製した。室温(RT)で15分間NIR剤をインキュベートした後、蛍光(Φ%)測定を行った。
結果
以下のTable Bに示す、得られたΦ%値は、DA364がPBSで36.5%(±2.37)、Seronorm(商標)で57.5%(±1.26)の量子収率を示すことを示している。
PBS(ex/em:674/694; ストークスシフト:20nm)およびSeronorm(商標)(694/704、ストークスシフト:10nm)におけるNIR剤の正規化された励起および発光スペクトルを図2に示す。図に示すように、DA364造影剤と血清アルブミンとの相互作用は、694nmから704nm(それぞれPBSおよびSeronorm(商標)において)の発光最大のシフトおよびストークスシフトの減少を促進する。
実施例4:αvβ3、αvβ5およびαvβ1インテグリンに対するin vitro競合結合アッセイ
DA364を、その特異的標的(インテグリンαvβ3)に対する親和性、および他のインテグリン(例えば、αvβ1、αvβ5)に対するその交差反応性について、インビトロ結合試験によって評価した。
材料と方法
96ウェルマイクロタイタープレート(MediSorp、Nunc)を、試験するインテグリン(0.5μg/mL、コーティング/洗浄緩衝液(20mM Tris HCl pH7.4;150mM NaCl;1mM MnCl2)で希釈)で4℃にて一晩コーティングした。コーティング後、プレートを洗浄緩衝液ですすぎ、次いで、室温(RT)(サーモミキサー内で24℃、振とう下)でブロッキング緩衝液(コーティング緩衝液中3%BSA)中2時間インキュベートした。次いで、プレートを洗浄緩衝液ですすぎ、DA364(5000〜0.005nMの濃度範囲)で3時間室温でインキュベートした。DA364溶液は、1%BSA中NIR剤のストック溶液を、ビオチン化ビトロネクチン(1μg/mL、cod HVN-U605、Molecular Innovations)の存在下、コーティング緩衝液中で連続的に希釈することによって調製した。次いでプレートを再びすすぎ、ストレプトアビジン-HRP(cod.EG RPN1051、GE Healthcare)(PBS中1:10000)と共に室温で1時間インキュベートし、ビオチン化ビトロネクチンを結合させた。PBSでさらに洗浄した後、プレートをTMB溶液(3,3’,5,5’−テトラメチルベンジジン)(cod.T-0440、Sigma-Aldrich)と共に5〜10分間インキュベートした後、2N硫酸の添加により比色反応を止めた。DA364についてのIC50の定量は、450nmでの吸光度を読み取ることにより競合物(ビオチン化ビトロネクチン)を測定することによって行った。IC50値は、最小二乗非線形回帰分析によって決定した。
αvβ3の天然基質であるビトロネクチンとの競合的結合アッセイは、DA364のその特異的標的、すなわちαvβ3に対する高い親和性、およびアナログαvβ5に対する低い親和性を示し;さらにαvβ1に対する交差反応性が明らかになった。
インテグリンαvβ3、αvβ5およびαvβ1について得られた親和性の結果を以下のTable Cに示す。
DA364のHSAに対する親和性は、例えば、Connors、K.A. (1987) Binding Constants The Measurement of Molecular Complex Stability, John Wiley & Sons, New York(特に第4章参照)に開示された手順に従って、吸光度の分光測定によって決定した。
材料と方法
デュアルビームパーキンエルマーラムダ40 UV−VIS分光光度計を使用して、HSAと共にインキュベートしたDA364の吸収スペクトルを得た。HSAを含有する2つの標準溶液を、75mMおよび500mMのPBS中で毎日新たに調製し、それぞれ約30分および1時間攪拌し続けた。
DA364のストック溶液(845±5mM)を1:20に希釈し、HSA(HSA Sigma Aldrich、バッチ049K7535、項目A1653)と混合して濃度の傾斜を得た(0、1、10、20、40、60、30、80、100、200、400mM)。0.99mMのNIR剤および異なる量のHSAを含む最終溶液を、測定前に室温で1時間インキュベートした。各溶液の吸光度を、同じ濃度のHSAの参照溶液に対して測定した。689nm波長(HSAに結合したDA364の最大吸収に対応する)で測定した吸光度値を、以下の式を用いて当てはめた。
DA364について測定したHSAについての会合定数(ka(M−1))は、2.8670・104±0.1140・104(R2=0.9949)であった。
この結果は、本発明により同定されたNIR剤によって示される血清アルブミンに対する親和性の存在を示し、これは、それが示すインビボのハイブリッド挙動と一致し、インテグリン受容体に対する高い特異性、および例えば血管外(漏出)アルブミンを含む細胞外成分との非特異的結合によって媒介される非特異的な保持の結果として、腫瘍へのその蓄積および拡散を可能にする。
NIRFプローブDA364の血漿動態を、CD−1(ICR)BRマウスにおける単回静脈内投与後に評価した。
プロトコール
蛍光プローブは、イメージング実験で使用される有効用量の約10倍に相当する、0.4μモル/kgの用量で投与した(平均のマウス体重25gとして1nmol/マウスに相当)。処置後14日まで異なる時点で動物を屠殺し、蛍光定量法検出器と組み合わせたHPLC法を用いて行うDA364含有量測定のために血液サンプルを採取した。
製剤の調製
DA364は、655μMの青色溶液として供給された。動物投与の数日前に解凍後、DA364の655μM溶液を0.04μmol/mLの最終濃度にまで希釈した。このために、DA364の655μM溶液0.8mLを、0.9%NaCl生理食塩水12.2mLで希釈した。0.04μmol/mL製剤を投与まで−80℃で保存した。
動物
実験は、Charles River Laboratories Italia(Calco (LC), Italy)から購入したCD−1(ICR)BRマウス(40匹の雄;到着時の体重:22〜30g;齢:4〜5週齢)を用いて行った。動物を含むすべての手順は、実験動物の保護および使用に関する国内および国際法および政策(L.D. 116/92; Authorization n.19/2008-A、2008年3月6日発行、イタリア保健省;EEC理事会指令86/609CEE、およびEEC理事会指令2010/63/EU)に従って実施された。
薬物動態試験でDA364の血漿動態を測定した。動物の到着と治療の間に少なくとも3日間の期間をおいた。投与日に、動物を薬物動態学的時点および較正群とにランダムに割り当てた。動物は、尾部に作られた着色されたマークで識別した。各ケージのラベルには、研究番号、化合物、投与経路、種、投与量、治療日および犠牲の日付および動物番号について一意の番号が付された。
予め室温にしたNIR剤DA364を、ハーバード注入ポンプを用いて1mL/分の注入速度で0.4μmol/kgの用量で尾静脈を介して静脈内に注射した。投与される用量は、イメージング実験で使用される有効用量の約10倍に相当する。静脈内投与経路は、ヒトに可能な曝露経路として選択された。
実験スキーム:
DA364の投与前および投与後5分、10分、20分、30分、60分、4時間、24時間、48時間、7日および14日に動物(各時点で3匹)を犠牲にした。
較正曲線のためのブランク血漿を採取するために、さらに7匹の動物を使用した。
屠殺の日に、誘発時3〜4%、維持時2〜3%の呼気濃度にてセボフルランで麻酔をかけ、次に断頭により犠牲にした。HPLC測定のための血液(少なくとも0.5mL)を、ヘパリン処理した試験管を用いて採取した。遠心分離(2100g、10分間)後に得られた血漿を分析日まで−80℃で保存した。
血漿サンプル中のDA364の量の決定は、以前に用いた蛍光量検出器と組み合わせたHPLC法によって行った。
DA364の酢酸緩衝溶液の調製
酢酸緩衝液を、1gの酢酸塩緩衝液を1LのMilli-Q水に溶解することによって調製した。次いで、適切な量のNIR剤を、得られた溶液に添加した。
血漿「CAL」サンプルの調製
較正標準(CAL)を、10μLのDA364酢酸緩衝液(0.04〜4nmol/mL)、および100μLのメタノールおよびアセトニトリル溶液(50:50)を90μLのブランク血漿に添加することによって調製した。試験管を攪拌し、続いて4℃にて13000gで10分間遠心分離した。10マイクロリットルの透明な上清をHPLC装置に注入した。
血漿「QC」サンプルの調製
10μLのDA364酢酸緩衝溶液(0.06〜3nmol/mL)、および100μLのメタノールおよびアセトニトリル溶液(50:50)を90μLの血漿に添加することによって、品質管理(QC)を調製した。試験管を攪拌し、続いて4℃にて13000gで10分間遠心分離した。10マイクロリットルの透明な上清をHPLC装置に注入した。
血漿「S」サンプルの調製
100μLの酢酸緩衝液および100μLのメタノールアセトニトリル溶液(50:50)を100μLの希釈した血漿に添加することによって、血漿サンプル(S)を調製した。試験管を撹拌し、続いて4℃にて13000gで10分間遠心分離した。10マイクロリットルの透明な上清をHPLC装置に注入した。血漿サンプルが濃過ぎる場合、適切に希釈を行った。

HPLC測定のための血液(少なくとも0.5mL)をヘパリン処理した試験管に集めた。DA364の血漿濃度は、蛍光検出器と組み合わせたHPLC法によって測定した。
全身暴露を評価するために、コンピュータプログラムWIN-NONLIN 6.3(WinNonlin 6.3 SCI Software、Lexington、KE、USA)を用いて、ノンパラメトリック法により、Cmaxおよび時間の関数とする血漿DA364濃度曲線下の面積(AUC)を計算した。試験した用量については、各時点で3匹の平均血漿濃度を用いて非コンパートメント分析を行った。Cmaxおよび対応するtmaxについて、観察された値が報告された。最後の観察可能な血漿濃度についての血漿濃度−時間曲線の下の面積(AUC(0 to t))を、対数台形法を用いて観測データから計算した。実際には、t1/2、Vd、およびClを計算した。
臓器中のDa364含量は、μg/mL(血液)またはμg/g組織として表される。データは、各臓器の注射された投与量のパーセント(%ID)に変換される。次に、各サンプリング時間について平均および標準偏差を計算する。
HPLC分離後の蛍光によって測定されたDA364の血漿濃度は、以下のTable Dおよび図6のグラフで示される。得られた結果は、遊離の循環剤の迅速なクリアランスを確認し、試験動物への投与の4時間後には血中濃度が無視できる程度に達し、機器の検出限界未満(LOD=0.0012nmol/mL)となることが確認される。
注射後5分(tmax)で1.629nmol/mLのCmaxを測定した。最終消失期に関する半減期の値は64分であった。AUC(0 to ∞)値は77分*nmol/mLであった。総血漿クリアランス値は5mL/分/kg(0.15mL/分)であった。クリアランス値は、肝臓および腎臓の血流(1.8および1.3mL/分)よりも低く、身体(他の組織)に蓄積していることが示唆される。定常状態での体積分布値(Vdss)は329mL/kg(10mL)であり、マウスの体内総水分量(14.5mL)よりもわずかに低く、血漿区画からのDA364の溢出が示唆される。
本実施例は、ラットの同所性前立腺腫瘍モデルMat Ly-Lu(以下に示すように、αvβ3インテグリンの発現が減少した腫瘍)を用いた腫瘍塊の認識におけるNIR RGD環状プローブDA364の感受性を評価することを目的としている。
術中イメージングは、照射およびプローブから放出された信号の選択のための適切なフィルタを含む修正された外科用顕微鏡であるZeissシステムで行った。ラットの腫瘍性MatLyLu前立腺細胞を前立腺の右側葉に接種した。接種の3〜4日後、動物に、NIR蛍光プローブのラット5〜20nmol/用量の範囲の用量を静脈注射し、24時間後に画像化した。次いで腫瘍を蛍光シグナルガイドを通して切除した。前立腺左葉由来の組織を含む蛍光領域を取り囲む組織を組織学的比較のために切除した。すべてのサンプルを、通常の組織学的処理(ヘマトキシリンおよびエオシン染色)およびさらなる免疫標識によって分析した。
ラット前立腺腺癌モデルG-Dunning R-3327の亜系である前立腺腫瘍Mat-Ly-Lu細胞株は、European Collection of Cell Culture(ECACC)から供給された。細胞を、10%ウシ胎仔血清(FBS)、2mMグルタミン、100IU/mLペニシリン、100μg/mLストレプトマイシンおよび250nMデキサメタゾン(DXMT)を補充したRPMI 1640培地で培養した。トリパンブルー排除試験を用いて、細胞接種前の細胞生存率を評価した。生存率は%(全細胞に対する生存細胞の割合×100)で表した。生存率<70%の細胞培養物を廃棄した。ヒト前立腺腺癌PC−3細胞株は、American Type Culture Collection(ATCC)から供給された。細胞を、10%ウシ胎仔血清(FBS)、2mMグルタミン、100IU/mLペニシリン、100μg/mLストレプトマイシンを補充したHam's F-12培地で培養した。トリパンブルー排除試験を用いて、細胞接種前の細胞生存率を評価した。生存率は%(全細胞に対する生存細胞の割合×100)で表した。生存率<70%の細胞培養物を廃棄した。ヒトメラノーマ細胞株WM−266は、American Type Culture Collection(ATCC)から供給された。細胞を、10%ウシ胎仔血清(FBS)、2mMグルタミン、100IU/mLペニシリン、100μg/mLストレプトマイシンを補充したEagle最小必須培地で培養した。
MatLyLu細胞系におけるαvβ3インテグリンの発現を、インテグリン過剰発現が高いことが知られているWM266メラノーマ細胞株(Anticancer Research 33:871-880(2013))との比較によりウェスタンブロットにより測定した。全タンパク質を抽出するために、細胞ペレットを溶解緩衝液(Tris-HCl pH8 50mM、NaCl 150mM、EDTA 1mM、NaF 100mM、グリセロール10%、MgCl2 1mM、TritonX-100 1%、プロテアーゼインヒビター)中に溶解し、3回の凍結融解(−80℃/37℃)を行い、氷中で20分間放置した。得られた溶液を10秒間超音波処理し、14000g、4℃で15分間遠心分離した。次いで上清のタンパク質含有量をBCA法で定量した。ここでは、全抽出物を10%ポリアクリルアミドゲルに負荷し、変性条件(20%SDS)において、それらの分子量に基づいてタンパク質を分離するために電気泳動を適用した。次いで、ゲル内のタンパク質をニトロセルロース膜上に移し、続いて抗β3インテグリン抗体でブロットした。特定のシグナルを、化学発光検出法によって単一または複数のバンドとして視覚化した。標識されたニトロセルロースの画像をChemiDoc MPイメージングシステムにより取得し、β3発現レベルに対するバンドの面積をソフトウェアImage Lab v.4.1で定量化した。
MatLyLu細胞におけるαvβ3の発現は、フローサイトメトリー(FACS)法によっても測定し、PC3細胞株の発現と比較した。
ここでは、細胞を回収し、0.2%BSAおよび0.01%アジ化ナトリウムを補充したPBS中で洗浄した。非特異的部位をウサギIgG(Sigma-Aldrich)でブロックした。次いで、細胞を抗αvβ3インテグリン抗体(希釈率1:100;クローンLM609、Millipore)またはアイソタイプ適合ネガティブコントロールと共に4℃で30分間インキュベートし、続いてフルオレセインイソチオシアネート(FITC)を結合したウサギ抗マウスIg(DakoCytomation)とインキュベーションした。サンプルを回収し、CyAn ADPフローサイトメーターおよびSummit 4.3ソフトウェア(DakoCytomation)で分析した。細胞破片を排除するため細胞をその光散乱特性に従って集め、死細胞を排除するためのヨウ化プロピジウム(Sigma-Aldrich)陰性に従って細胞を集めた。
ウエスタンブロット分析の結果、WM266メラノーマ細胞による発現と比較して、MatLyLu細胞によるαvβ3インテグリンの発現は無視できる程度であった。FACS実験において、PC3細胞の比較的高いαvβ3発現(45%の蛍光シグナルを示す細胞)が確認され、引用文献で引用された値と一致しており、Mat-Ly-Lu細胞上のαvβ3の発現は無視できる程度(0.1%)であった(図8)。
実験は、Harlan Laboratories Srl, S.Pietro al Natisone (UD), Italyから購入した11週齢のコペンハーゲンラット(雄)10匹で行った。ラットの前立腺腺癌を、イソフルラン麻酔下でのMatLylu腫瘍細胞の同所性注射によってコペンハーゲンラットにおいて誘導した。腫瘍移植の約1時間前に、鎮痛薬(Rymadil)をラットに皮下投与(5mg/kg)した。下腹部を剃毛して消毒した(ベタジン)。腹白線に沿った腹壁の12mm切開を、尿口から3〜4cm上で実施した。動物の両側の脂肪組織を押しのけた後、前立腺を露出させた。
0.5×106個の腫瘍細胞を含む細胞懸濁液(30μL)に、27G針を前立腺の右腹側葉に注射した。筋肉および皮膚の計画は、滅菌された非吸収性の糸(3-0黒の絹、Distrex S.p.a.)で縫い合わせた。傷口にはベタジン消毒液を塗布した。鎮痛剤は、手術後少なくとも1回(約24時間後)投与した。動物を含むすべての処置は、実験動物の保護および使用に関する国内および国際法および政策(EEC理事会指令86/609CEE; LD 116/92およびEEC理事会指令2010/63/EU; L.D. 26/2014. イタリア保健省により2006年10月27日に発行されたAuthorization n.149/2006-A)に従って実施した。
専用の外科用顕微鏡(Axio Zoom v16, Carl Zeiss Beteiligungs GmbH, Jena, Germany)を用いて腫瘍を取り出した。顕微鏡にPLANAPO Z0,5X/0.125 FWD 114 MM対物レンズを装着した。手術中、約2.5×2.5cm2の領域を可視化した。手術野への照明は、蛍光信号が必要でないときは冷光源(CL 9000 LED CAN(D))によって、腫瘍除去中はフィルター光(照明:HXP 200C(D)、フィルターセット:32 Cy 5.5シフトフリー(E))によって行った。
腫瘍移植後3〜4日に相当する手術前日に、5〜20nmol/ラットの範囲の用量でDA364試験薬剤を動物に投与した。プローブ注射の24時間後、動物をイソフルランガス0.5%(O2 99.5%)で麻酔し、手術台に仰向けに固定し、前立腺腫瘍を露出させた。
前立腺被膜を除去した後、前立腺腫瘍をフィルター光に曝露し、原発腫瘍の質量および病理的マージンの概要をみるため蛍光画像を取得した(図3、パネルAおよびD参照)。次いで、蛍光を用いて切除をガイドすることにより、腫瘍塊の除去を腫瘍領域を取り囲む領域に蛍光シグナルがなくなるまで反復して行った(図3、パネルB、EおよびC、F参照)。蛍光シグナルの大部分は、移植領域、すなわち前立腺右側葉に閉じ込められていた。蛍光シグナルを囲む領域から組織サンプルを切除し、前立腺左葉、背葉および前葉を収集し、エクスビボでの蛍光画像を取得して、フルオロフォアの蓄積の原因となる組織学的パターンを調べた(図4)。注目すべきは、より安全な手術戦略のために少なくとも最小の非蛍光性の、陰性マージンを含むより大きな容積の切除が推奨されるものの、この実験的試験では、組織のサンプルを高強度の蛍光シグナルによって同定された腫瘍マージンに従い厳密に切除した。最終的に、収集した組織サンプルはすべて以下の組織学的分析のために処理した。実験の最後に、動物は過剰用量の麻酔によって犠牲にされた。
切り出したサンプルをKillik化合物(クリオスタット包埋媒体)に直接包埋し、液体窒素中で冷却したイソペンタン中で直ちに凍結させた。
切り出した組織ブロックを、パラレルラベリングのために3つの連続する10μmの厚さのスライスのグループに切断した。次いで、各組織サンプルを組織病理学的分析のための標準的な手順で処理した。特に、手術による腫瘍の根治的切除を評価および確認するため、収集したサンプルの組織学的染色をヘマトキシリンおよびエオシンで行い、切り出したサンプル中の解剖学的な腫瘍のマージンを検出および特徴付け、周囲の正常な前立腺組織における任意の腫瘍細胞の存在を確認した。
切り出した組織サンプル中の蛍光剤DA364分布の検出は、室温(RT)で解凍し、PBS中で洗浄してKillikを除去した後、組織スライド上で行った。細胞核を4’,6’−ジアミジノ−2−フェニルインドール(DAPI, cod. BML-AP402, Enzo Life Sciences)で染色した(PBS中5μg/mL、5分間)。DA364の内在化した画分を同定するために、形質膜を2μg/mLのFITC結合小麦胚芽凝集素(WGA)(cod. W11261 Life Technologies)で染色した。ここでは、染色された切片全体をAperioデジタルスキャナScanScope FL、さらなる詳細についてはLeica DM2500蛍光顕微鏡を用いて、組織を視覚化した。
腫瘍細胞におけるDA364の内在化を蛍光顕微鏡で観察した。例えば、図7に示されるように、細胞膜および細胞外間隙の同定が可能な原形質膜染色である、2μg/mLのFITC結合小麦胚芽凝集素(WGA)で染色されたMat-Ly/Lu腫瘍組織スライドから得られたシグナルにDA364によって生成された蛍光シグナルを重ね合わせた図7に示されるように、Mat-Ly/Lu腫瘍細胞の白黒画像が得られた。
手術直後に採取したエクスビボ蛍光画像(図4)を用いて、切除された組織サンプルを2つの蛍光クラス、すなわち蛍光(FLUORESCENT)および非蛍光(NON FLUORESCENT)、に視覚的に分類した。2つの蛍光クラス間の分類は、組織サンプルの解剖学的起源を考慮することなく、蛍光シグナルのみに基づいた。組織サンプルが少なくとも強度が強く明るい蛍光発光によって特徴付けられた領域を含む場合に、蛍光クラスに割り当てた(例えば、図4のサンプル3、4および5参照)。それ以外の組織サンプル、すなわち蛍光を示さない(図4のサンプル1など)または腫瘍領域から離れた正常組織(図4のサンプル2など)で観察される自己蛍光と一致する蛍光を示すサンプル、はすべて非蛍光クラスとした。各画像について、蛍光シグナルを、全ピクセル強度範囲を用いピクセル強度でマッピングした。この分類手順は、外科医が腫瘍除去中に行う必要がある「正常な」(非蛍光)と「病理的な」(蛍光)組織の蛍光ベースの視覚的選択を再現することを意図した。
次に、各組織サンプルからの2つの組織学的切片の、その次元にかかわらず腫瘍領域を含む切片の同定が可能なH&E染色によって、(ガイド手術中に採取した組織サンプルの)視覚的分類の結果を組織学的に確認した(図5パネルA)。腫瘍を含む組織サンプル(すなわち、試験した組織学的切片の少なくとも1つが腫瘍細胞を含んでいる組織サンプル)を陽性と分類し、腫瘍を含まないサンプルを陰性と分類した。次いで、本発明によって同定された化合物により可能な視覚的分類の感度の尺度と、腫瘍を含まない組織と腫瘍を含む組織の同定および分離におけるその信頼性を提供する分割表を用いて組織学的および視覚的分類を比較した。
採取した組織サンプルのさらなる分析もまた、核染色剤4’,6’−ジアミジノ−2−フェニルインドール(DAPI)の蛍光に基づいて行った。一般的に言えば、この分析は、正常な前立腺の形態が腫瘍浸潤によって破壊され、それが自動閾値ベースの画像分析手順で容易に区別できる、正常組織および腫瘍組織の形態に存在する差異に依存している(図5パネルC、D)。正常な前立腺組織は、実際には、線維性間質に埋没した腺および管で主に構成され、組織の空の領域は前立腺の広い内腔を示し、上皮腺細胞および間質は前立腺の実質を示す。それに対し、腫瘍組織は、より典型的には、均一に詰まった細胞で構成される。この自動的な手順は、セクションの面積に対する、組織(非空領域)によって占有されるサンプルの面積の比として定義される組織密度を計算するというものであった。正常な組織は病理的な組織よりも低い組織密度によって特徴付けられる。
外科手術の有効性の組織学的検証
分割表を用いて、組織学的および視覚的分類の結果を比較した。例えば、下の分割表Table Eに示される比較の結果は、検出された蛍光の強度と試験した組織サンプルにおいて組織学的に確認された病理学的状態との間に有意な相関が存在することを示す。特に、得られた結果は、DA634 NIR蛍光プローブが87%の感度と83%の特異度で腫瘍組織の視覚的分類を可能にしたことを示す(Table E)。
蛍光サンプルはまた、本質的には病理的組織で確認された前立腺の腺および管の不在に起因する統計的に高い組織密度を示すことによって、正常組織形態の実質的な破壊によっても特徴付けられた(マン・ホイットニーU検定、p値<0.05、図5、パネルB)。
腫瘍塊に関するαvβ3インテグリン発現の測定
αvβ3インテグリンの発現は、動物モデルからの3つの異なるタイプの腫瘍について、ウエスタンブロットによって測定した:
・U−87MG(ヒト膠芽細胞腫の異種移植マウスモデル)は、αvβ3インテグリンの相対的に高い発現を有することを示した。
・MatLyLu(ラット前立腺癌の同所性ラットモデル)は、αvβ3インテグリンの中〜低度の発現を示した。
・A431(ヒト表皮癌の異種移植マウスモデル)は、比較的低いαvβ3インテグリンの発現を示した。
方法
凍結した腫瘍組織をメスを用いて小片に切断し、次いで液体窒素の存在下で乳鉢と乳棒を用いて粉砕した。粉末を秤量し、適切な容量の溶解緩衝液(Tris-HCl pH8 50mM、NaCl 150mM、EDTA 1mM、NaF 100mM、グリセロール10%、MgCl2 1mM、TritonX-100 1%、プロテアーゼインヒビター)に溶解し、1分間ボルテックスし、氷上に45分間置いた。その後、溶液を15秒間超音波処理し、4℃にて14000rpmで15分間遠心分離した。
BCA法を用いて上清のタンパク質含量を定量した。総抽出物を10%ポリアクリルアミドゲルに負荷し、電気泳動を行い、変性条件(20%SDS)での分子量によってタンパク質を分離した。次いで、ゲル内のタンパク質をニトロセルロース膜上に移し、続いて抗β3インテグリン抗体でブロットした。特定のシグナルを、化学発光検出法によって単一または複数のバンドとして視覚化した。標識されたニトロセルロースの画像をChemiDoc MPイメージングシステムにより取得し、β3発現レベルに対するバンドの面積をソフトウェアImage Lab v.4.1で定量化した。
結果
インテグリンβ3の量は、腫瘍の質量(腫瘍のmg)で標準化した。実験は、腫瘍の種類ごとに少なくとも2つのサンプルを用いて行った。結果(インテグリン発現の平均値)をTable Fに示す。
腫瘍塊中のタンパク質総量に対するαvβ3インテグリン発現の測定
αvβ3インテグリンの発現は、実施例8で上に詳述した3つの異なる動物モデル、およびタンパク質抽出物として購入したさらなるヒト腫瘍について、ウェスタンブロットによって測定した。
腫瘍モデルからのタンパク質抽出物は、前記の抽出方法を用いて組織から得たが、ヒトタンパク質抽出物(以下のTable Gに示す)は、OriGeneより購入した。
αvβ3インテグリン(A431)の発現が減少した腫瘍のin vivo視覚化
この実施例は、それぞれ低いおよび高いαvβ3過剰発現によって特徴づけられる、ヒト腫瘍のA431およびU87−MGマウスモデルにおけるDA364の取り込みを示す。
腫瘍モデル
A431細胞を集めPBSで2回洗浄した。5百万個の細胞を0.1mLのDMEM高グルコース培地に再懸濁し、全てのマウスの右脇腹に皮下注射した。A431接種の11日後、U−87MG細胞を回収しPBSで2回洗浄した。2百万個の細胞を0.1mLのEMEM培地に再懸濁し、予めA431細胞を注射した同じ動物の左脇腹に皮下注射した。
接種後、触診およびキャリパー測定を1週間に1回行ない、腫瘍の発達を追跡した。腫瘍体積は、式:(L×W2)/2(ここで、LおよびWは腫瘍の最大長および幅である)に従って計算した。実験期間の終わり、または両方の腫瘍の合計が約2000mm3の体積に達したときに、動物を過剰麻酔により犠牲にした。
A431腫瘍移植の3週間後、5匹のマウスに1nmolのDA364を全投与量0.2mL(注入速度1mL/分)で投与した。インビボOI取得は、IVISスペクトル光学イメージングシステム(Perkin Elmer)を用い、蛍光プローブの静脈内投与の5分後、15分後、30分後、1時間後、2時間後、3時間後、4時間後、6時間後、24時間後および48時間後に実施した。OI実験中、動物を3〜4%セボフルランガス麻酔下(O2 97.0〜96.0%)に保った。麻酔の間、動物の体温を加熱ベッドを用いて約37℃に安定化した。
インビボでのOI実験に続いて、プローブ注射から48時間後、動物を犠牲にした。腫瘍、肝臓、腎臓、心臓および脾臓を、切除した腫瘍または器官全体に対する生体外蛍光シグナル測定のために切除した。
U−87MGおよびA431腫瘍からの蛍光強度を、正常な組織からのシグナル(バックグラウンド)と共にインビボで測定した。
注射後3時間から24時間までの時間枠において、U−87MGおよびA431腫瘍は、バックグラウンドに関して安定したシグナル比を示した。図9は、マウスの蛍光イメージングであり、DA364が皮膚下の低インテグリン発現腫瘍A431の明確な視覚化を可能にすることを示している。
IVISスペクトル光学イメージングシステム(Perkin Elmer)を用いて、摘出した生体外サンプルについて蛍光シグナルを測定した(放射効率(RE):p/s/(μW/cm2)として示す、pは光子カウント)。
以下のTable Iに示すように、U−87MG腫瘍におけるDA364の蛍光シグナルは、A431腫瘍で測定されたシグナルの強度の約2倍であった。しかし、A431腫瘍で測定された蛍光強度は、肝臓および腎臓(DA364が蓄積する***器官)で測定されたものと同等であったが、心臓および脾臓で測定されたシグナル(正常組織、バックグラウンドを代表して)に対して約10倍高かった。
Claims (11)
- 個人の患者のガイド手術における腫瘍マージンの検出および画定のための薬学的診断組成物の製造のための、次式で示される化合物DA364の使用であって、該腫瘍がαvβ3インテグリン受容体の減少または変化した発現を示す腫瘍である、使用。
- 前記検出および画定が、根治手術下での病理的部分の外科的境界をリアルタイムに同定することを含んでなる、請求項1記載の化合物DA364の使用。
- 腫瘍マージンの検出および画定が蛍光下で行われる請求項1または2記載の化合物DA364の使用。
- 光の波長が630〜690nmの範囲である、請求項3記載の化合物DA364の使用。
- 前記検出および画定が、
i)有効量のDA364が適切に前投与された患者における疑わしい腫瘍を含んでいる目的領域に、化合物DA364を励起することが可能な光源を照射すること;および
ii)蛍光下で病理的部分または領域のマージンを同定すること
を含んでなる、請求項1〜4のいずれかに記載の化合物DA364の使用。 - 0.05〜20mgの量の化合物DA364が個人の患者に前投与される、請求項5記載の化合物DA364の使用。
- DA364の投与がガイド手術の48時間以内に行われる、請求項5または6記載の化合物DA364の使用。
- 化合物DA364を含んでなる、腫瘍マージンをリアルタイムに検出および画定するための術中光学イメージングのための薬学的診断組成物の製造のための化合物DA364の使用であって、該腫瘍がα v β 3 インテグリン受容体の減少または変化した発現を示す腫瘍である、使用。
- 前記光学イメージングが、根治切除下で腫瘍部分のマージンをリアルタイムに検出および画定するための個人の患者の腫瘍のガイド手術において行われる、請求項8記載の薬学的診断組成物の製造のための化合物DA364の使用。
- メラノーマ、神経膠腫、膠芽細胞腫、神経芽細胞腫、肉腫、卵巣癌、乳癌、肺癌、肝臓癌、結腸癌、頭頚癌および前立腺癌からなる群から選択される腫瘍の検出および画定のための、請求項2〜7のいずれか記載の薬学的診断組成物の製造のための化合物DA364の使用。
- 前記腫瘍が前立腺癌である請求項10記載の化合物DA364の使用。
