Description
以下に記載される研究は、いくつかの自然食品成分およびサプリメントによって植えつけられた(inculcated)認知増強に対して高感度であると示されているコンピュータ化された認知要求バッテリー(CDB)を利用した(Kennedy et al.2008;Kennedy and Scholey 2004;Reay et al.2005;2006)。このアプローチの基礎をなす作業仮説は、神経活動が、局所的な神経資源(例えば、代謝基質)に関して犠牲の大きい活動であり、ゆえに脳が短期間、不完全な状態に陥りやすいということである。ゆえに、被験物質の任意の精神賦活特性は、この強く認知的に要求される期間および長時間の課題遂行によって誘発される「精神的疲労」状態においてより容易に明らかになりやすい。したがって、このパラダイムは、特に介入が代謝基質の送達または利用に影響を与える場合、それほど要求が厳しくない認知的課題を使用するときよりも高感度であり得る。
デザイン
本研究は、無作為化、二重盲検、プラセボ対照、平衡クロスオーバーデザインを使用した。
先の4週間以内に処方薬もしくは違法薬物または任意の栄養補助食品を摂取していなかった20人の健常な成人(10人の男性、10人の女性、18〜25歳)を採用した。試験セッション前の12時間以内にアルコールまたはカフェインを摂取していた者と同様に、喫煙者およびカフェインの過剰消費者は除外した。
各参加者は、ラテン方格へのランダムな割り当てによって相殺される順序で各処置を受けた。参加者が各日において割り当てられた条件に応じて、参加者に、500mgのLCまたは500mgのALCまたはプラセボを含む4つのカプセルを投与し、それらのカプセルは、以下のうちのいずれかの総用量が投与されるように組み合された:
1)プラセボ
2)500mgのL−カルニチン(LC;750mgのCarnipureTM酒石酸塩,Lonza AGとして)
3)2000mgのL−カルニチン(LC;3000mgのCarnipureTM酒石酸塩,Lonza AGとして)
4)1000mgのアセチル−L−カルニチン(ALC;CarnipureTMALC,Lonza AGとして)
認知要求バッテリー(CDB)の前に、3つの課題によって、単純な精神運動機能および注意力を評価した。
参加者は、スクリーンに上向きの矢印が現れるたびに、できるだけ速く応答ボタンを押した。50回の刺激を、刺激間の時間を1〜3.5秒にランダムに変動させて提示した。結果は、応答の速さ(msec)だった。
左または右に向いた矢印を1回につき1つ、スクリーンに提示した。参加者は、その矢印の向きに対応する左または右のキー押しボタンを押すことによって応答する。50回の刺激を、刺激間の時間を1〜3.5秒にランダムに変動させて提示した。結果は、応答の速さ(msec)および正確性(%正確)だった。
上、下、左および右を指す矢印の視覚的表示をスクリーンに提示した。その矢印は、ランダムに「点灯」し、参加者は、対応する応答ボタンを用いて応答する。合計48回の刺激を、刺激間の時間を1〜3.5秒にランダムに変動させて提示した。結果は、応答の速さ(msec)および正確性(%正確)だった。
このバッテリーの目的は、認知要求課題の連続遂行において速さ/正確性および精神的疲労に対する処置の影響を評価することである。その60分の形態におけるこのバッテリーの適用(すなわち、10分間のバッテリーの6反復)は、「精神的疲労」の自己評価を確実に増加させ、ならびにいくつかの薬草および自然食品の介入に対して高感度であると示されている(Kennedy et al.2008;Kennedy and Scholey 2004;Reay et al.2005;2006)。この10分間のバッテリーは、以下を含む:
コンピュータ化された改変バージョンの2分間の連続3(Serial Threes)および連続7(Serial Sevens)テストを使用した。両方の課題において、参加者に、各応答を入力するための直線上に並んだ数字のキーを使用して、できるだけ速くかつ正確に、与えられるランダムに生成された数字から3または7を引き算するように標準的な指示スクリーンによって知らせた。参加者には、間違った場合には新しい間違った数字から減算を続ければよいことも、口頭で指示した。キーボードの直線上に並んだ数字のキーを使用して、各3桁の返答を入力した(ここで各桁は、アスタリスクによってスクリーン上に表されている)。エンターキーを押すことにより、各返答の終わりを知らせ、3つのアスタリスクがスクリーンから取り除かれる。この課題を、減算の総数および誤りの数についてスコア付けした。間違った返答の場合、その後の返答は、その新しい数字に対して正しい場合、正解としてスコア付けした。
参加者は、3つ連続した奇数の数字または3つ連続した偶数の数字の標的について、とぎれていない一続きの数字をモニターした。それらの数字は、1分あたり100個の速度で提示され、参加者は、できるだけ速く応答キーを押すことによって、標的の一続きの検出に応答した。この課題は、連続的であり、5分間続いた(1分につき8つの正しい標的の一続きが提示された)。この課題を、正しく検出された標的の一続きのパーセンテージ、正しい検出に対する平均反応時間および誤り警報(false alarms)の数についてスコア付けした。
各課題セットの終わりに、どれくらい精神的に疲労したと参加者が感じたかを、終点に「まったく感じない」および「非常に感じた」と表示された100mmの線に印をつけることによって指摘するよう参加者に求めた。
精神的疲労に起因する認知障害は実行機能の減衰を反映すると仮定されている(上記を参照のこと)。以下の従来の「実行」課題は、疲労に対して高感度であると示されている(van der Linden et al.2003)。
参加者に、一連の記載が2文字の順序を正しく説明しているかを判定するように求めた。参加者にいくつかの短文を示し、その各々の後に文字対を示した。それらの文は、2文字の順序を説明している、すなわち、どちらが最初にあったかを述べていると主張するものだった。参加者は、いくつかの異なる方法でこれを行うことができた。例えば、ABという順序は、(1)AはBの前にある、または(2)BはAの後ろにある、または(3)BはAの前にない、または(4)AはBの後ろにない、のいずれかと述べることによって正しく記載され得る。これらのすべてが、対ABの正しい説明であるが、他方の対BAに適用されるときは誤りである。参加者に、各文が、その後に続く文字対の真または偽の説明であるかを判定するように求めた。この課題を、正確性および反応時間についてスコア付けした。
これは、注意および中央実行系の力を必要とする、通常使用される古典的な課題のコンピュータ化されたバージョンである。この場合において、この課題は、10分間行われ、データは、2分間のエポックに分解される。4色(「赤」、「黄」、「緑」、「青」)のうちの1つを記載している語を、異なる色が付けられたフォントでコンピュータスクリーンの中央に提示した。参加者は、そのフォントの色を識別するために4色の応答ボタンのうち1つを押した(例えば、「緑」という語が青色のフォントで提示された場合、正しい応答は、青色のボタンで応答することになるだろう)。提示される語は、「一致」(語とフォントが同じ色である)または「不一致」(語とフォントが異なる色である)であり、ランダムな順序で提示された。この課題を、「一致」および「不一致」の語に対する応答の反応時間および正確性についてスコア付けした。
Bond−Lader気分スケール(BondおよびLader 1974)
課題の前および後に、BondおよびLaderスケールを用いて気分を評価した。この尺度は、数多くの薬理学的、精神薬理学的および医学的な試験において利用されている。このスケールは、それぞれの端に反意語が付けられた(例えば、覚醒−気だるさ、落ち着き−興奮)合計16本の線(スクリーン上のおよそ100mm)を含む。被験体は、その線上のそれらの反意語の間に、自身の現在の主観的な位置を指摘する。個別の項目のスコアを、その線における%距離として計算した。結果は、3因子分析に由来するスコアを含んだ:「覚醒」、「落ち着き」および「満足」。
参加者は、別個の5回において、検査室を訪れた。試験は、参加者が互いに視覚的に隔離されたひと続きの試験施設において行われた。
主要解析
上記バッテリーから得られた認知/疲労および気分の測定のすべてに対する統計解析を、ベースライン調整データ(投与前に対して計算されたデータ)の反復測定分散分析(ANOVA)によって解析した。投与後の単回反復による測定(注意力課題、実行課題)を1元配置ANOVA(条件)によって解析した。投与後の複数回反復による測定(気分、CDB)を2元配置ANOVA(条件×反復)によって解析した。次いで、必要に応じて、上記ANOVAからの平均二乗誤差を用いるt検定(Keppel 1991)を使用して、単回の完了、または複数回の完了の各々からのベースライン調整データに対して計画比較(プラセボ 対 各実薬処置)に着手した。
ANOVAの統計量を図4、5および6に示す。
反応時間課題
最初のANOVAも計画比較も、単純反応時間、選択反応時間または4選択反応時間課題に対していかなる有意な効果も明らかにしなかった。
最初のANOVAも計画比較も、ストループ課題または論理的推論課題に対していかなる有意な効果も明らかにしなかった。
図2は、それらの課題の各反復におけるプラセボに対する各実薬処置のデータの計画比較における有意な効果を立証したCDBの結果を示している。
連続3:最初のANOVAは、連続3課題の遂行に対して、条件または条件を含む相互作用の有意な主効果を示さなかった。しかしながら、計画比較は、参加者が、4回目[t[270]=2.74,p=0.006]および6回目[t[270]=2.41,p=0.016]の反復において、プラセボと比べて500mgのLCの後により多くの減算を行ったことを示した(2回目[t[270]=1.74,p=0.08]および3回目[t[270]=1.67,p=0.09]の反復においても同じ効果の統計学的傾向があった)。さらに、5回目の反復では、プラセボと比べて2000mgのLCの後により多くの減算が行われた[t[270]=2.06,p=0.04]。この後者の効果は、他の反復において再現されていないので、第1種の過誤に起因する可能性がある。遂行の正確性(誤りの数)に対して有意な効果はなかった。
ANOVAは、RVIP課題における応答の速さに対する条件の主効果を明らかにした[F(3,57)=2.78,p<0.05]。しかしながら、処置手段の事後比較は、この効果が、プラセボとの差異ではなく実薬処置間の差異に起因すると示した。計画比較は、参加者が、各反復において500mgのLCの後により速く行った一方で、この効果は、課題の3回目の反復においてのみ有意に達したことも明らかにした[t[285]=2.51,p=0.013]。4回目の反復では、2000mgのLCの後により遅い反応の傾向も存在した[t[255]=1.71,p=0.089]。
予想どおり、ANOVAは、課題の反復全体を通して疲労が増加するにつれて、反復の主効果を明らかにした[F(5,95)=42.32,p<0.001]。しかしながら、条件、または条件×反復の相互作用の主効果は無かった。計画比較は、疲労の評価が、6回目の反復[t[285]=2.14,p=0.033]においてプラセボと比べて500mgのLCの後に低かったことを明らかにした(4回目の反復[t[285]=1.85,p=0.065]においても同じ効果の傾向があった)。対照的に、疲労の評価は、5回目の反復[t[285]=2.32,p=0.02]の後では、プラセボより2000mgのLCの後が高かった。
結果を図3に示す。最初のANOVAは、Bond−Lader気分スケールに対して有意な処置関連効果を示さなかった。しかしながら、500mgのLCの後、参加者は、最初(課題前)の評価において自身を、あまり落ち着かない[t[57]=2.12,p=0.037]およびあまり満足していない[t[57]=3,p=0.004]と評価したが、この効果は、課題後の評価によって消え、その課題後の時点では、この群は自身をより落ち着いていると評価した[t[57]=2.47,p=0.016]。1000mgのALCの後においては、参加者は自身を、課題に着手する前に、あまり覚醒していないと評価し[t[57]=2.7,p=0.009]、投与後の課題の前[t[57]=3.6,p=0.001]と後[t[57]=2.5,p=0.015]の両方において、あまり満足していないと評価した。
本研究は、互いに独立して行われた2つの統計解析;各評価におけるプラセボと3つの各実薬処置との間の任意の差異を評価する集中的な計画比較(ANOVAからの平均二乗誤差を使用するt検定)、ならびに処置全体および評価全体における主効果および相互作用効果(必要に応じて)を評価するより一般的なANOVAを使用した。これらの解析のうちの2つ目(ANOVA)では有意差は無かったが、計画比較では明らかな効果のパターンが存在した。以下は、プラセボとの比較における各処置に対する明らかな効果を要約している。
各CDB課題の遂行では、計画比較解析における有意な利点が見られた。参加者は、2反復においてより多くの連続3減算を終え、さらなる2時点において改善に向かう傾向があった。参加者は、2時点においてより多くの連続7減算も終え、RVIP課題の1反復においてより迅速に応答した。この後者の効果は、2時点のRVIPでは正確性の改善に伴うものであり、さらなる1反復では改善に向かう傾向があった。この条件における参加者は、課題の各反復後に、自身を数値的により低く「精神的に疲労した」とも報告したが、この効果は、最後の課題セットの後に有意に達しただけで、他の1時点では同じ効果の傾向があるだけだった。
2000mgのLCに関するパターンは、いくつかの孤立した差異から構成された。参加者は、1時点においてより多くの連続3を終え、1時点において連続7減算の増加の傾向があった。対照的に、RVIPではより遅い応答および1反復では精神的疲労の評価の有意な増加の傾向があった。結果のパターンが単一の矛盾する知見から構成される場合、それはおそらく、この処置に関係する結果が偶然に起因するとみられる。
ALCは、気分に対する負の効果を伴った(より低レベルの覚醒が投与後の2回目の時点にあり、低レベルの「満足」が投与後の両方の時点にあった)。
ANOVAではいかなる有意な処置関連効果が無かったにもかかわらず、計画比較では、プラセボと比較したとき、500mgのLCの後のCDB遂行の有意な改善の一貫したエビデンスが存在した。しかしながら、2000mgのLCおよび1000mgのALCによる処置の後には、有利なエビデンスは存在しなかった。
本研究の知見から、500mgのLCが、健常な若年成人の認知能力を高めると証明される。認知要求バッテリー課題の3つすべてにおいて、(6回のうち)2回以上の課題反復において改善が見られ、課題に関連する精神的疲労の処置関連減弱のいくつかのエビデンスがあった。ここで、より高用量(2000mg)のLCまたは1000mgのALCが有益な効果を有するという一貫したエビデンスは無かった。
認知機能に対するカルニチンの効果を、CogStateTM12分間早期認知テストバッテリーを用いて調査した。このテストは、CogState,USによって開発されたものであり、米国特許第7163513(B2)号明細書に記載されている。このテストは、実行機能、精神運動機能、視覚的注意力および視覚学習の認知ドメインを測定するためにトランプ札およびコンピュータ迷路を使用する。課題は、以下のとおり認知ドメインに対応する:

Claims (12)
- L−カルニチンおよび/またはL−カルニチンの塩を含有する、60歳未満のヒト個体における精神的疲労を減少させるためもしくは予防するための、および/または認知機能を改善するための組成物であって、
前記個体が健常な個体であり、前記個体が、慢性疲労および/または慢性L−カルニチン欠乏を経験しておらず、
前記L−カルニチンおよび/またはL−カルニチンの塩の投与の前に、前記個体が、一時的な疲労を経験しており、
前記L−カルニチンおよび/またはL−カルニチンの塩が、250〜750mgのL−カルニチンの同等量に対応する量で投与され、
前記L−カルニチンおよび/またはL−カルニチンの塩が、疲労を減少させるかもしくは予防するのに有効な別の物質と併用して投与されず、
前記L−カルニチンおよび/またはL−カルニチンの塩が、1日を超えて投与されない、前記組成物。 - 前記L−カルニチンおよび/またはL−カルニチンの塩が、250〜500mgのL−カルニチンの同等量に対応する量で投与される、請求項1記載の組成物。
- 前記塩が、酒石酸塩、クエン酸塩、コハク酸塩、フマル酸塩または塩酸塩である、請求項1又は2記載の組成物。
- 前記L−カルニチンおよび/またはL−カルニチンの塩が、単回投与で投与される、請求項1〜3のいずれか1項記載の組成物。
- 前記L−カルニチンおよび/またはL−カルニチンの塩が、50%(w/w)超のL−カルニチンを含む固体組成物の一部として投与される、請求項1〜4のいずれか1項記載の組成物。
- 前記L−カルニチンおよび/またはL−カルニチンの塩が、経口的に液体の形態で、または錠剤、カプセル、散剤のような固体調製物で、またはゲルもしくはペーストで投与される、請求項1〜5のいずれか1項記載の組成物。
- 前記L−カルニチンおよび/またはL−カルニチンの塩が、錠剤、カプセル、散剤またはゲルもしくはペーストの形態のような液体、あるいは固体調製物で、投与される、請求項1〜6いずれか1項記載の組成物。
- L−カルニチンおよび/またはL−カルニチンの塩を含む組成物が、精神的疲労の減少または予防のために投与される、請求項1〜7のいずれか1項記載の組成物。
- L−カルニチンおよび/またはL−カルニチンの塩を含む組成物が、認知機能改善のために投与される、請求項1〜8のいずれか1項記載の組成物。
- 短時間の認知機能が改善する請求項1〜9のいずれか1項記載の組成物。
- 前記組成物が食品である、請求項1〜10のいずれか1項記載の組成物。
- 前記精神的疲労の減少が治療的でない、請求項1〜11のいずれか1項記載の組成物。
