Description

本発明は、手術用スクリュー及びこれを用いた融合装置に関し、さらに詳細には、セルフロック（ｓｅｌｆ−ｌｏｃｋｉｎｇ）が可能な手術用スクリュー及びこれを用いた融合装置に関する。
様々な種類のファスナー（ｆａｓｔｎｅｒ）が、骨に移植体及びその他の装置を締結するために使用される。脊椎分野では、通常、骨ねじが、移植体及び器具の板、ロッド及びその他のものを一つまたはそれ以上の椎体に付着させるために使用される。
骨ねじを椎体に定着するために、或いは移植体及び器具の板、ロッドから分離されることを防止するために、様々な技術が開発されている。例えば、米国特許第７８４６２０７号明細書には、椎体の間にケージを挿入し、骨ねじを椎体に装着してケージを固定させることができる技術が開示されている。ここで、骨ねじの緩み（ｐｕｌｌｉｎｇ ｏｕｔ）を防止するために、さらに安全プレート（ｓｅｃｕｒｉｎｇ ｐｌａｔｅ）を骨ねじのヘッド側に設け、安全ねじ（ｓｅｃｕｒｉｎｇ ｓｃｒｅｗ）を安全プレートを貫通させてケージに結合する方法が提示されている。
また、米国特許第６３９８７８３号明細書では、プレートを椎体に固定させるとき、骨ねじの緩み（ｐｕｌｌｉｎｇ ｏｕｔ）を防止するために、骨ねじが挿入されるホールの周りに回転するロックエレメント（ｌｏｃｋｉｎｇ ｅｌｅｍｅｎｔ）をプレートに設置した。
また、米国特許第７６２５３７９号明細書では、仙骨（Ｓａｃｒｕｍ）と腸骨（骨盤、Ｉｌｉａｃ）との間の関節に問題が生じるときに使用する仙腸関節癒合インプラント（ＳＩ ｊｏｉｎｔ ｆｕｓｉｏｎ ｉｍｐｌａｎｔ）において、骨ねじの緩みを防止するために２つのナットを使用している。
このように、いろいろの領域で骨ねじの緩みを防止する様々な技術が検討されているが、そのためには、別のロックエレメントを使用しなければならないという欠点がある。よって、別のロックエレメントなしで簡単にロック状態を維持することが可能なねじの開発が求められる。

本発明は、上述した問題点を解決するためになされたもので、その目的は、別のロックエレメントなしでセルフロック（ｓｅｌｆ−ｌｏｃｋｉｎｇ）が可能な手術用スクリュー及びこれを用いた融合装置を提供することにある。
上記の目的を達成するための本発明のある観点によれば、外周面に本体メインねじが形成され、前記本体メインねじとオフセットを介して離隔して本体ロックねじが形成され、上部に本体駆動溝が形成されたスクリュー本体と、内周面に前記本体ロックねじと螺合されるスリーブねじが形成され、前記スリーブねじの下部に隔設されて前記本体ロックねじを支持することが可能なスリーブ止め顎を有するスリーブとを含んでなる、手術用スクリューを提供する。
前記スリーブ止め顎の下側は、前記オフセットの外周面に一致する内周面を有することを特徴とする。
また、前記本体ロックねじは、前記本体メインねじと同一のピッチ及び外径を有し、前記オフセットは、前記本体メインねじのピッチの整数倍の長さを有することを特徴とする。
また、前記本体ロックねじまたは前記スリーブねじのリード高さは、前記本体ロックねじのピッチの１倍以下であることを特徴とする。
本発明の他の観点によれば、上述した手術用スクリューと、前記手術用スクリューが装着されるプレートとを含む融合装置を提供する。
本発明の別の観点によれば、上述した手術用スクリューと、前記手術用スクリューが貫通する貫通孔を有するケージとを含む融合装置を提供する。
本発明によれば、別のロックエレメントなしで簡単にロック状態を維持することが可能なスクリュー及び融合装置を提供することができる。また、別のロックエレメントがないので、スクリューをロック状態にするためのツールの数が減少するだけでなく、施術者の手間及び施術時間が減少するという利点がある。

本発明は、別のロックエレメントなしでセルフロック（ｓｅｌｆ−ｌｏｃｋｉｎｇ）が可能な手術用スクリュー及びこれを用いた融合装置を提供しようとする。
以下に添付図面を参照しながら、本発明の好適な実施形態を詳細に説明する。
下記の各図面の構成要素に参照符号を付するにあたり、同一の構成要素に限っては、たとえ他の図面上に表示されても、できる限り同一の符号を有するようにし、本発明の要旨を無駄に不明確にするおそれがあると判断される公知の機能及び構成についての詳細な説明は省略する。
図１から図４において、図面符号２００は本発明の実施形態による手術用スクリューを示す。前記手術用スクリュー２００は、基本的に、スクリュー本体２０２と、前記スクリュー本体２０２に締結されるスリーブ２０４とを含んでなる。
前記スクリュー本体２０２は、外周面に本体メインねじ２０６が形成され、前記本体メインねじ２０６とオフセット２０８を介して離隔して本体ロックねじ２１０が形成される。前記本体ロックねじ２１０は、前記本体メインねじ２０６と同一またはそれより大きい外径を有することができる。ただし、前記スクリュー本体２０２が回転するとき、前記本体ロックねじ２１０と前記本体メインねじ２０６が同時に螺合されるようにするために、前記本体ロックねじ２１０は前記本体メインねじ２０６と同一の外径を有する。
同様の理由から、前記オフセット２０８は、前記本体メインねじのピッチの整数倍の長さを有することが好ましい。
前記スクリュー本体２０２の上部には、ドライバなどの器具から回転力の伝達を受けるための本体駆動溝２１２が形成される。
前記スリーブ２０４は、内周面に前記本体ロックねじ２１０と螺合されるスリーブねじ２１６が形成され、前記スリーブねじ２１６の下部に隔設されて前記本体ロックねじ２１０を支持することが可能なスリーブ止め顎２１８を有する。
すなわち、前記スリーブねじ２１６は、前記本体ロックねじ２１０と少ない回転量でのみ螺合され、直ちに前記スリーブねじ２１６の下側に前記本体ロックねじ２１０を移動させることによりスリーブねじ２１６の下面が前記本体ロックねじ２１０の緩みを防止するストッパーの役割を果たす。また、前記スリーブ止め顎２１８は、前記スリーブねじ２１６の下側に抜け出た本体ロックねじ２１０が、もはや下側に進むことを防止する役割を果たす。
さらに、前記スリーブねじ２１６と前記本体ロックねじ２１０との結合区間を長くないようにすることにより、前記スリーブ２０４の全体高さを減らすことができるうえ、前記スリーブねじ２１６と前記本体ロックねじ２１０との螺合の際に発生する摩擦力により、施術者に与える労働の負担を軽減することができる。このため、前記本体ロックねじ２１０または前記スリーブねじ２１６のリード高さは、前記本体ロックねじ２１０のピッチの１倍以下であることが好ましい。
したがって、前記スリーブ止め顎２１８の下側は、前記オフセット２０８の外周面と一致する内周面を有するようにして、前記オフセット２０８が前記スリーブ止め顎２１８の下側によって案内されるようにする。その結果、前記スリーブ２０４に対して前記スクリュー本体２０２の直進性を確保することができる。
前記スリーブ２０４のスリーブボディ２１４は、必要に応じて様々な形状に形成でき、外側を球面とした場合にはプレート、ケージなどにおける回転が可能となる。
図５及び図６は、本発明の実施形態による手術用スクリュー２００を用いた融合装置２２０を示す。前記融合装置２２０は、椎体の間に挿入されるケージ２２２と、前記ケージ２２２に形成される貫通孔２２４に挿入される手術用スクリュー２００とを含んでいる。前記手術用スクリュー２００は、外表面が曲面であるスリーブボディ２１４を有するスリーブ２０４が装着されているため、前記スリーブ２０４の内部で一定の範囲で回転可能である。前記手術用スクリュー２００が、前記ケージ２２２にセルフロックされながら椎体に螺合されるので、追加のロック装置は必要としないという利点がある。
図７は、本発明の実施形態による手術用スクリュー２００を用いた融合装置２２６を示す。前記融合装置２２６は、椎体などの骨に接するプレート２２８と、前記プレート２２８に設置される手術用スクリュー２００とを含んでなる。よって、前記手術用スクリュー２００が、前記プレート２２８に安着されながらセルフロックされるので、追加のロック装置は必要としないという利点がある。前記手術用スクリュー２００は、外表面が曲面であるスリーブボディ２１４を有するスリーブ２０４が装着されているため、前記プレート２２８に対して一定の範囲で回転可能である。
上述の如く、本発明の好適な実施形態を参照して説明したが、当該技術分野における通常の知識を有する者であれば、下記の特許請求の範囲に記載された本発明の思想及び領域から逸脱しない範囲内で、本発明に多様な修正及び変更を加え得るのを理解することができるであろう。
本発明によって、別のロックエレメントなしで簡単にロック状態を維持することが可能なスクリュー及び融合装置を提供することができる。また、別のロックエレメントがないので、スクリューをロック状態にするためのツールの数が減少するだけでなく、施術者の手間及び施術時間が減少するという利点がある。

Claims (5)

  1. 外周面に本体メインねじが形成され、前記本体メインねじとオフセットを介して離隔して本体ロックねじが形成され、上部に本体駆動溝が形成されたスクリュー本体と、
    内周面に前記本体ロックねじと螺合されるスリーブねじが形成され、前記スリーブねじの下部に隔設されて前記スリーブねじの下側に抜け出た本体ロックねじがもはや下側に進むことを防止するように前記本体ロックねじを支持することが可能なスリーブ止め顎を有するスリーブと、
    を含み、
    前記本体ロックねじを移動させることにより、スリーブねじの下面が前記本体ロックねじの緩みを防止するとともに、
    前記スリーブ止め顎の下側は、前記オフセットの外周面と一致する内周面を有することを特徴とする、
    手術用スクリュー。
  2. 外周面に本体メインねじが形成され、前記本体メインねじとオフセットを介して離隔して本体ロックねじが形成され、上部に本体駆動溝が形成されたスクリュー本体と、
    内周面に前記本体ロックねじと螺合されるスリーブねじが形成され、前記スリーブねじの下部に隔設されて前記スリーブねじの下側に抜け出た本体ロックねじがもはや下側に進むことを防止するように前記本体ロックねじを支持することが可能なスリーブ止め顎を有するスリーブと、
    を含み、
    前記本体ロックねじを移動させることにより、スリーブねじの下面が前記本体ロックねじの緩みを防止するとともに、
    前記本体ロックねじは、前記本体メインねじと同一のピッチ及び外径を有し、前記オフセットは、前記本体メインねじのピッチの整数倍の長さを有することを特徴とする、
    手術用スクリュー。
  3. 前記本体ロックねじまたは前記スリーブねじのリード高さは、前記本体ロックねじのピッチの１倍以下であることを特徴とする、
    請求項１または２に記載の手術用スクリュー。
  4. 請求項１から３のいずれか１項に記載の手術用スクリューと、
    前記手術用スクリューが装着されるプレートと、
    を含む融合装置。
  5. 請求項１から３のいずれか１項に記載の手術用スクリューと、
    前記手術用スクリューが貫通する貫通孔を有するケージと、
    を含む融合装置。
