Description
前記態様及び前記実施態様のいずれかと組み合わせて使用可能ないくつかの実施形態において、前記装置は、前記生体層の上に生体侵食性材料の層をさらに備えていてもよい。
前記態様及び前記実施態様のいずれかと組み合わせて使用可能ないくつかの実施形態において、前記長尺状のコア部は、チューブ状部材(例えば、中実壁を有するチューブ又は螺旋状に巻かれたワイヤ等)であってもよい。
本発明の他の態様は、(a)前記態様及び実施形態のいずれかによる閉塞装置と、(b)前記閉塞装置に対して着脱可能に構成された長尺状の装入部材とを備えていてもよい。
本発明の他の態様は、前記態様及び実施形態のいずれかによる閉塞装置が体液(例えば、血液等)にさらされ、それによって当該閉塞装置が自己膨張し、流れを阻止することになる、体内管腔(例えば、精管、卵管、並びに正常な血管、及び動脈瘤、動静脈瘻、及び動静脈奇形等を含む異常な血管から選ばれる)の部位に、当該閉塞装置を導入することを含む治療方法に関する。いくつかの態様において、体内管腔は、胃動脈、脾動脈、胃十二指腸動脈、及び***静脈若しくは卵巣静脈から選択される血管である。
本発明のより完全な理解は、本発明の多くの態様及び実施形態の以下の詳細な説明を参照することによって可能である。以下の詳細な説明は、本発明を説明することを意図しているが、本発明を限定することを意図しているものではない。
これらの図1A〜図1Bから分かるように、閉塞装置100は、ほぼ円筒形であり、コア部110を備え、該コア部110は、近位端110pと、遠位端110dと、厚さtを有する生体層120によって少なくとも部分的に被覆されている軸110aとを有している。コア部110は、長さに対する幅の比率が、2対1から1600対1以上まで、例えば2対1から、5対1まで、10対1まで、25対1まで、50対1まで、100対1まで、250対1まで、500対1まで、1000対1まで、1600対1まで(すなわち、前記の値のうちの任意の2つの範囲まで)の範囲を有するという点で、好ましくは、長尺状のコア部110である。図示の実施形態における閉塞装置100は、以下でさらに説明するように、装入部材に対する着脱用のオプションの取付機構116を備えている。本実施形態の場合、取付機構116は、コア部110と一体の構造であってもよく、又は取付機構116は、コア部110に対し、例えば、はんだ付け、溶接、溶融、接着、圧着、又は他の方法で連結可能にコア部110と取り付けられる別個の構成要素の形態であってもよい。
既述の通り、本発明による閉塞装置は、様々なサイズのカテーテルを介して装入するように構成可能である。カテーテル装入に伴う悪影響を低減するために、閉塞装置は、例えば、シース内に配置された状態でカテーテルを介して装入され、それにより、カテーテルを介して閉塞装置を押圧する際に摩擦により失われる親水性天然重合体の量を最小にする。シース材料の例としては、他の材料のうち、ポリテトラフルオロエチレン(PTFE)といったフルオロ重合体を含む低摩擦重合体が含まれる。一旦、閉塞装置及びシースが装入部位でカテーテルから送出されると、シースは引き抜かれることとなり、親水性天然重合体が体液(例えば血液)にさらされることで、生体層が膨張する結果となる。
摩擦を低減するための別の施策は、親水性天然重合体が配置されている部位よりも小さい直径の隘路領域の、単一のセクション又は複数個所繰り返すセクションをコア部に設けることである。閉塞装置の直径全体において、隘路領域(コア部+親水性天然高分子)における直径がコーティングされていない領域の直径以下であることを確保することにより、親水性天然重合体と装入カテーテルとの間の摩擦を低減することができ、閉塞装置の摩耗を減少させたり、押し込み性を改善したりすることが可能となる。
図7A〜図7Dを参照して、装入のために、閉塞装置100は、開口している遠位端160dを有する装入カテーテル160等のチューブ状装置のルーメンを介して導入されてもよい。装入シャフト150のような長尺状の装入部材もまた、装入カテーテル160の内部に配置可能であり、閉塞装置100が所望のタイミングでカテーテル160に対して相対的に前進及び後退し、最終的には身体内に送出できるように、取付具116の近位のところで閉塞装置100に着脱可能に接続されていてもよい。
Claims (14)
- (a)長手軸を有する長尺状のコア部と、
(b)長尺状のコア部に配置され、体内管腔内において体液と接触して膨張する親水性天然重合体を備えた生体層と、
(c)前記長尺状のコア部の遠位端から長手軸に沿って遠位側に延出して径方向外方に延び、前記体内管腔の壁に係合するように構成された複数の爪と、
を備える閉塞装置。 - 前記親水性天然重合体は、コラーゲンである請求項1に記載の閉塞装置。
- 前記生体層の上に生体侵食性材料の層をさらに備える請求項1又は2に記載の閉塞装置。
- 前記長尺状のコア部は、1つ以上の縮径された領域を備えており、前記親水性天然重合体は、前記1つ以上の縮径された領域内に設けられている請求項1から3のいずれか一項に記載の閉塞装置。
- 前記長尺状のコア部は、中実ロッドである請求項1から4のいずれか一項に記載の閉塞装置。
- 前記長尺状のコア部は、チューブ状部材である請求項1から4のいずれか一項に記載の閉塞装置。
- 前記チューブ状部材は、螺旋状に巻かれたワイヤである請求項6に記載の閉塞装置。
- 前記長尺状のコア部は、非拘束時に三次元スパイラルとなる形状を記憶する請求項1から7のいずれか一項に記載の閉塞装置。
- 前記三次元スパイラルは、螺旋及び円錐スパイラルから選択されるものである請求項8に記載の閉塞装置。
- 前記複数の爪は、非拘束時に三次元スパイラルとなる形状を記憶する請求項1から9のいずれか一項に記載の閉塞装置。
- 取付機構をさらに備えている請求項1から10のいずれか一項に記載の閉塞装置。
- 請求項1から11のいずれか一項に記載の閉塞装置と、前記閉塞装置に対し着脱可能に構成された長尺状の装入部材とを備えているアセンブリ。
- 前記閉塞装置は、チューブ状装置に予め装填されている請求項12に記載のアセンブリ。
- (a)請求項1から11のいずれか一項に記載の閉塞装置と、
(b)少なくとも1つの物品であって、(i)前記閉塞装置に対し、着脱可能に構成された長尺状の装入部材、(ii)前記閉塞装置を閉塞部位に装入するのに適したカテーテル若しくはシース、又は(iii)カテーテルイントロデューサから選択されるものと
を備えているキット。
