Description
本発明は一般に、生体活性ガラスおよび担体を含む代用骨移植片に関する。また、本発明は、そのような代用骨移植片を含むインプラント、代用骨移植片を作製および使用する方法、ならびにそのような代用骨移植片を含むキットに関する。例示的な組成物としては、生体活性ガラスと、グリセロールと、ポリエチレングリコールとが挙げられる。他の組成物としては、生体活性ガラスと、コラーゲンと、場合によりヒアルロン酸などの血管新生促進剤と、が挙げられる。更なる組成物としては、生体活性ガラスと、ヒアルロン酸と、グリセロールと、が挙げられる。
自己移植片、即ち患者自身の組織の使用は、骨移植の「ゴールドスタンダード」と見なされている。しかしその使用は、採取部位の疼痛および罹患の問題により制限され続けている。結果として、自家移植片の骨移植は、数年間かけて普及してきた。しかし疾患の伝染および発生源に関する問題が、引き続き自家移植片製品の使用を妨げている。
本発明は一般に、生体活性ガラスと、合成再吸収性担体と、を含む代用骨移植片に関する。
本開示の実施形態は一般に、生体活性ガラスと、合成再吸収性担体と、を含む合成代用骨移植片、ならびにそれを含むインプラントおよびキットを対象とする。同じく、代用骨移植片を哺乳動物に投与する方法が包含される。
本明細書で用いられる「a」または「an」は、1つ以上を意味し得る。本明細書で用いられる「他の」は、少なくとも第二の、またはそれら以外を意味し得る。更に、文脈により他に必要とならない限り、単数の用語は複数を包含し、複数の用語は単数を包含する。
本発明の非限定的例としては、生体活性ガラスと、グリセロールと、ポリエチレングリコール(PEG)と、を含む代用骨移植片を挙げることができる。生体活性ガラスは、例えば繊維などの粒子の形態であってもよい。非限定的な例示的生体活性ガラス粒子としては、Bioglass粒子を挙げることができる。グリセロールおよびポリエチレングリコールは担体を形成し、特定の実施形態によれば、これが骨刺激性骨癒合のために、所望の速度で、例えば埋入後数日以内に、分解されて生体活性ガラスを露出し得る。PEGは市販されており、当業者に公知である。
他の例示的な代用骨移植片は、コラーゲンと、生体活性ガラスと、場合により血管新生促進剤と、を含んでもよい。本明細書における他の実施形態と同様に、生体活性ガラスは、例えば繊維などの粒子の形態であってもよい。非限定的な例示的生体活性ガラス粒子としては、Bioglass粒子を挙げることができる。コラーゲンは、当業者により決定することができるが、哺乳動物に挿入されるコラーゲンの適切な形態でなければならない。非限定的例としては、コラーゲンは、ウシコラーゲンI型であってもよい。
他の非限定的な例示的実施形態によれば、生体活性ガラスと、ヒアルロン酸と、グリセロールとを含む代用骨移植片が提供されてもよい。生体活性ガラスは、本明細書に示された通りであってもよく、例えば45S5生体活性ガラス粒子であってもよい。
本明細書において、本発明の代用骨移植片の1種以上を含み得るインプラントも提供される。例えば本発明の代用骨移植片は、椎体間スペーサーの内部もしくは内側の移植片として、または圧迫骨折の治療に用いられてもよい。
本発明の代用骨移植片を作製する方法が、更に提供される。該方法は、例えば最終的な所望の組成物に依存し得る。例示的方法は、例えばPEGを融解すること、グリセロールを添加すること、およびその後、生体活性ガラスを添加すること、を含んでもよい。他の実施形態によれば、例示的方法は、グリセロール中にヒアルロン酸を混合すること、およびその後、生体活性ガラスを添加すること、を含んでもよい。更なる例示的方法は、コラーゲンを酸で膨潤させること、生体活性ガラスを混合すること、および凍結すること、を含んでもよい。
本明細書において、本発明の代用骨移植片を必要とする哺乳動物に挿入することを含む方法も提供される。例示的方法は、例えば本発明の代用骨移植片の1つ以上を哺乳動物、例えば骨移植片を必要とする哺乳動物に外科的に挿入することにより、代用骨移植片を哺乳動物に移植することを含んでもよい。代用骨移植片は、例えば単独で、例えばストリップもしくはパテの形態で移植されてもよく、あるいは代用骨移植片は、インプラントと併せて、例えばインプラントのコーティングとして、またはインプラント内部もしくは上に混和して、使用してもよい。
本明細書において、本発明の代用骨移植片の1つ以上を含むキットも提供される。代用骨移植片は、例えばキット中で、既に配合された形態、例えばストリップの形態で、提供されてもよい。あるいは代用骨移植片は、代用骨移植片に再構成され得る、さもなければ形成され得る形態で、例えば水和させてパテを形成し得る粉末形態で、提供されてもよい。あるいは代用骨移植片は、インプラント内、またはインプラント上に提供されてもよい。
非限定的実施形態によれば、本発明の代用骨移植片は、BAGとグリセロールとPEG、またはBAGとグリセロールとヒアルロン酸、のいずれかを含む組成物であってもよい。そのような製品は、例えば図1に示されたシリンジ100などの投与デバイスの内側に存在する、例えば成形可能なパテまたは成形可能な生地状のクランチパテの形態であってもよい。
Claims (12)
- 半硬質のコラーゲンベースのスキャフォールドの形態であり、5重量%〜20重量%のコラーゲンと、0.1重量%〜10重量%の血管新生促進剤とを含み、かつ、グリセロールおよびポリエチレングリコールを含まない担体と、
84重量%の生体活性ガラス粒子と、を含み、
ストリップの形態であり、水和されると成形可能なパテとなり、水和されると多孔性が、150μm〜450μmに保持される、代用骨移植片。 - 上記血管新生促進剤が、ヒアルロン酸を含む、請求項1に記載の代用骨移植片。
- 上記コラーゲンが、ウシコラーゲンI型である、請求項1に記載の代用骨移植片。
- カルシウムセラミック成分を含まない、請求項1に記載の代用骨移植片。
- 上記血管新生促進剤が、1種以上の多糖類を含む、請求項1に記載の代用骨移植片。
- 上記コラーゲンの含有量が、17重量%〜20重量%である、請求項1に記載の代用骨移植片。
- 上記血管新生促進剤の含有量が、1重量%〜5重量%である、請求項1に記載の代用骨移植片。
- 上記生体活性ガラスが、45%のSiO2、24.5%のNa2O、24.5%のCaO、および6%のP2O5を含む、請求項1に記載の代用骨移植片。
- 血液または骨髄にて水和される、請求項1に記載の代用骨移植片。
- コラーゲンを8重量%含み、上記血管新生促進剤として8重量%のヒアルロン酸を含む、請求項1に記載の代用骨移植片。
- 上記生体活性ガラス粒子が繊維の形状を包含する、請求項1に記載の代用骨移植片。
- 上記ストリップは、水和により成形可能なパテに形成され得る耐圧縮性ストリップである、請求項1に記載の代用骨移植片。
