[3]ロイシンの代謝産物はケトイソカプロン酸(KIC)だけである。KIC代謝産物の少量生成物はβ−ヒドロキシ−β−メチル酪酸塩(HMB)である。HMBが様々な用途に有用なことが分かっている。具体的には、米国特許第5,360,613号(Nissen)には、HMBが総コレステロール及び低比重リポタンパク質コレステロールの血液レベルを減少させるのに有用であることが記載されている。米国特許第5,348,979号(Nissenら)には、HMBがヒトの窒素保持の促進に有効であることが記載されている。米国特許第5,028,440号(Nissen)には、動物の赤身を増加させるためのHMBの有効性が考察されている。米国特許第4,992,470号(Nissen)にも、HMBが哺乳動物の免疫反応の増強に有効であると記載されている。米国特許第6,031,000号(Nissenら)には、疾患に関連する衰弱を治療するためにHMBと少なくとも1つのアミノ酸を使用することが記載されている。
[7]アデノシン−5’−三リン酸(ATP)は、筋肉などの組織にとっての化学的なエネルギー源であることは長いこと知られてきた(19)。細胞内のATP濃度(1〜10mM)は細胞外の濃度(10〜100nM)よりもかなり高いため、赤血球や筋肉など、細胞からのATPの放出は厳密に制御されている。より最近になって、大部分の細胞型で見られるプリン受容体を介したATPの細胞外作用が発見された(20)。血管拡張(21)、疼痛認知の低減(22)、および神経伝達補助伝達因子として(23、24)などの、ATPのいくつかの細胞外性生理学的機能については記述がある。重要なことは、筋肉における血管ATPの僅かな一過性の上昇が、血管拡張および筋肉への血流の増加を引き起こす可能性があるということである(25)。そのため、ATPが筋肉への血流を増加させる場合、特に激しい筋肉トレーニングの最中には、基質利用能が改善され、代謝性老廃物の除去がより促進されるだろう。Ellisらは最近、プリン作動性シグナル伝達や神経伝達を介する筋肉での血流増加におけるATPの役割を支持する研究結果の概要について論評している(25)。

HMB
[28]β−ヒドロキシ−β−メチル酪酸、すなわちβ−ヒドロキシ−イソ吉草酸は、遊離酸の形態である(CH3)2(OH)CCH2COOHとして表すことができる。「HMB」という用語は、前述の化学式を有する、遊離酸および塩の形態両方の化合物、およびその誘導体を指す。どのような形態のHMBも本発明においては使用可能であるが、好ましくは、HMBは、遊離酸、塩、エステル、およびラクトンを含む群より選択される。HMBエステルには、メチルエステルおよびエチルエステルが含まれる。HMBラクトンには、イソバラリルラクトンが含まれる。HMB塩には、ナトリウム塩、カリウム塩、クロム塩、カルシウム塩、マグネシウム塩、アルカリ金属塩、および土類金属塩が含まれる。
[30]20年以上前に、HMBのカルシウム塩はヒト用の栄養補給剤として開発された。数多くの試験から、筋力トレーニングと併せてCaHMBを補給することで、筋肉量および強度が増大すること、また、癌やAIDSの状態にある人の筋肉量の減少が軽減されることが分かっている(1〜5)。NissenとSharpは、筋力トレーニングと併せて使用される補給剤のメタ解析を行い、HMBが、筋力トレーニングと併用することで強度および除脂肪筋肉量の有意な増加という臨床結果を有するたった2つの補給剤のうちの1つであることを見いだした(1)。複数の試験から、平均的なヒトにおけるCaHMBの有効量が、体重1キログラムあたり38mgと考えられることが分かっている(6)。
[33]多くの場合、HMBは、カルシウム塩の形態で強壮剤として臨床試験で用いられ、また、市販されてきた(3、16)。近年の進歩からHMBを栄養補助剤として使用するための遊離酸の形態で製造することができるようになった。最近になって、HMBの新たな遊離酸の形態が開発された。この新たなHMB遊離酸は、CaHMBよりも迅速に吸収され、その結果、より早くより高い血清HMBの最大レベルおよび組織への血清排出能の改善をもたらすことが示された(18)。
アデノシン−5’−三リン酸(ATP)

[40]どのような好適な用量のHMBも本発明において使用することができる。正確な用量の算出方法は当該分野においてよく知られている。HMBの投与量は、対応するCa−HMBのモル量を単位として表すことができる。HMBを経口的にまたは静脈内に投与する場合の用量範囲は、体重1キログラム当たり、24時間で0.01〜0.5グラムHMB(Ca−HMB)の範囲となる。成人に対しては、体重を約100〜200ポンド(約45〜約90キログラム)と仮定した場合の経口または静脈内投与量は、対象1人当たり、24時間で0.5〜30グラムのHMB(Ca−HMBを基準として)となるだろう。

[55]本試験は、12週間の期分け筋力トレーニングからなる、無作為化、二重盲検、偽薬および食事を管理した試験として実施した。トレーニングプロトコールは3期に分けた(表1、2、および3)。第1期は、Kraemerら(36)を改変した、非線形の期分けされた筋力トレーニングプログラム(1週間に3回)からなるものであった(表1)。
[57]筋力トレーニングを行っていた40人の男性が本試験に参加した。平均年齢は23.0±0.9才で、スクワット、ベンチプレス、およびデッドリフトの平均はそれぞれ1.7±0.04、1.3±0.04および2.0±0.05×体重であった。対象の特徴を表4に示す。その時点で抗炎症剤、他の運動成績向上用補給剤のいずれかを摂取している場合、または喫煙している場合には参加者から除外した。試験に参加する前には全員が、タンパ大学施設内倫理委員会(University of Tampa Institutional Review Board)によって認可された同意書(インフォームドコンセント)に署名した。
[58]手順を良く理解した後で、筋肉強度を、バックスクワット、ベンチプレス、およびデッドリフトの1RM試験で評価した。リフト(持ち上げ)はいずれも、国際パワーリフティング協会規則(International Powerlifting Federation rules、44)に記載されているように行った。体組成(除脂肪体重、脂肪量、および全体重)を二重X線吸収法(DXA;Lunar Prodigy encore 2008、マジソン、ウィスコンシン、米国)で決定した。骨格筋肥大は、超音波検査で決定した外側広筋(VL)および中間広筋(VI)の厚みの変化の合計によって決定した。筋肉厚測定の試験−再試験に関する級内相関係数(ICC)はr=0.97であった。
[61]試験の前に、参加者を無作為に4群に振り分けた。それぞれの群の参加者は、1日当たり3gのHMB遊離酸(HMB)(食用のオレンジ香料および甘味料と合わせたもの)、1日当たり400mgのATP(PEAK ATP(登録商標);TSI,Inc.)、1日当たり3gのHMBと1日あたり400mgのATP両方の組み合わせ、または偽薬(食用のオレンジ香料と甘味料)のいずれかを、均等に3分割したものを、最初の1分割を運動の30分前、そして残りの2分割を昼食および夕食と共に摂取した。トレーニングを行わない日には、3回の食事毎にそれぞれ1分割ずつを摂取するよう、参加者は説明を受けた。補給はトレーニングおよび試験プロトコールを実施している間毎日続けられた。十分量と排出量の個人差を考慮し、かつ、有効な最低用量である0.800グラムが達成されるように、1分割の用量は1グラムのHMB遊離酸を使って処方した。この用量は1グラム用量のCa−HMBと同等である。
[64]試験期間中の採血は全回、12時間絶食した後に、訓練を受けた静脈採血士による静脈穿刺によって行われた。全血を採取し、血清と血漿を得るのに適したチューブに移し、その後、4℃、1,500gで15分間遠心した。次いで得られた血清と血漿を分割量に分け、その後の解析まで−80℃で保存した。
[65]試料を1回融解し、各解析を2つ組にして解析を行った。採血は全回、一日の中の同じ時間に予定し、時間毎のホルモン変動が解析結果を複雑にする影響を回避した。血清中の総テストステロンおよび遊離テストステロン、コルチゾール、およびC反応性タンパク質(CRP)をDiagnostic Systems Laboratories(ウェブスター、テキサス)から入手したELISAキットで検定した。ホルモン値の測定は全て、同じアッセイ系を使って同じ日に行い、アッセイ毎の複雑な差を回避した。アッセイ内での差は全標本について3%未満だった。血清クレアチンキナーゼ(CK)は呈色的な手法を用い、340nm(Diagnostics Chemicals、オックスフォード、コネティカット)で測定した。24時間の採尿を実施し、既に記載されている方法(Rathmacherら1992およびWilsonら、2013)によって3−メチルヒスチジンを定量した。
[66]自覚される回復状況(PRS)の尺度を第0、4、8、9、10、および12週に測定し、トレーニング期における主観的な回復を評価した。PRS尺度は0〜10の値からなり、0〜2がほとんど回復せず成績の衰退が予期される、4〜6が多少回復し同程度の成績が予期される、また8〜10が非常によく回復し成績の向上が予期される、を表している。
[67]SASに組み込まれているProc GLM手法(Version 9.1、SAS Institute、カリー、ノースカロライナ)1(SAS Institute,Inc.(1985)SAS利用者ガイド:統計(SAS User’s Guide:Statistics)、第5版。カリー、ノースカロライナ:SAS Institute,Inc.)を用いた対応のないANOVAモデルにより、基本特性データを解析した。処置の主な効果(Trt)はこのモデルに含まれていた。筋肉の強度および性能、体組成、筋肉損傷、ホルモンの状態、および自覚される回復状況(PRS)の12週間の試験期間にわたる変化は、SASに組み込まれているProc Mixed手法を用い、2×2要因多群、反復測定ANOVAで解析した。このモデルでは第0週の初期値を、HMB、ATP、および時間の主な効果、ならびにHMB*時間、ATP*時間、およびHMB*ATP*時間の相互作用と共に共変量として使用した。本試験の過負荷のサイクルも、SASに組み込まれているProc Mixed手法を用い、2×2要因多群、反復測定ANOVAで評価した。しかしながら、この場合には、第8週の時点での測定値を、HMB、ATP、および時間の主な効果、ならびにHMB*時間、ATP*時間、およびHMB*ATP*時間の相互作用と共に共変量として使用した。次いで最小二乗法によって、各時点での処置の平均を比較した(事後t検定)。統計的な有意差はp≦0.05で、傾向ありをp>0.05〜p≦0.10の範囲で決定した。
参加者の特徴
[68]試験開始時には、各処置群の間で、年齢(偽薬=23.0±1.2、ATP=23.7±0.9才、HMB=22.3±0.6、HMB−ATP=22.4±0.5)、身長(偽薬=180.6±2.3、ATP=179.0±1.0cm、HMB=179.3±2.1、HMB−ATP=180.0±1.4)、または体重(偽薬=87.4±4.3、ATP=85.7±1.7、HMB=83.1±1.6、HMB−ATP=84.6±2.2)に差は無かった。
[69]試験を行っている間の第0、4、8、9、10、および12週に、筋肉の強度(スクワット、ベンチプレス、およびデッドリフトの1RM)および筋肉のパワー(垂直跳びおよびウィンゲートの最大パワー、PP)を測定した。筋肉の強度およびパワーはいずれも、12週間の試験の間に向上した(時間、p<0.001)。HMB、ATPおよびHMBとATPの組み合わせを補給すると、総強度はそれぞれ、77.1±5.6、55.3±6.0、および96.0±8.2kgに増加し、一方、偽薬を補給された参加者は12週間の試験の間に総強度は22.4±7.1kgまでにしか増加しなかった(t検定、p<0.05)。図2および3a〜cは、強度に対するHMBとATPの相乗効果を示している。図2に、第8週から第12週にかけての総強度の変化を示す。図3a〜cは第4〜8週、および第4〜12週にかけての相乗的な組み合わせ(スクワット強度およびベンチプレス強度を含む)に関する個々の指標を示している。
[72]筋力トレーニングの結果、第0、4、8、および12週の時点で、除脂肪体重(LBM)および大腿四頭筋の厚み(時間、p<0.001)が増加したが、一方で、脂質のパーセンテージはトレーニングを行うことで低下した(時間、p<0.001)。HMBを補給すると、体重、LBM、および大腿四頭筋の厚みが増加し、体脂肪率が減少した(HMB*時間、それぞれ、p<0.03、p<0.001、p<0.001、およびp<0.001)。ATPを補給するとLBMと大腿四頭筋の厚みが増加した(ATP*時間、それぞれp<0.01および0.04)。除脂肪体重は偽薬、HMB、ATP、およびHMB−ATPの補給を受けた参加者でそれぞれ2.1±0.5、7.4±0.4、4.0±0.4、および8.5±0.8kgと、相加的に増加した(t検定、p<0.05、表5)。また、脂質のパーセンテージはHMBおよびHMB−ATPの補給を受けた参加者でそれぞれ、7.0±0.6および8.5±0.9%に低下した(t検定p<0.05)。脂質のパーセンテージに対してはHMBの補給のみが有意な効果を示した(HMB*時間p<0.001)。試験を行っている間、体重についてはATP*時間に主な効果は見られなかった。しかしながら、ATPだけの補給を受けた群では偽薬補給群に比べて、試験12週目の体重が重かった(t検定、p<0.05)。12週間の大腿四頭筋の厚みの増加は、偽薬、HMB、ATP、およびHMB−ATPの補給を受けた参加者でそれぞれ2.5±0.6、7.1±1.2、4.9±1.0、および7.8±0.4mmであり、HMB、ATP、およびHMB−ATPの補給は、偽薬の補給と比較して、12週間の間に大腿四頭筋の厚みを大幅に増加させた(t検定、p<0.05、表5)。
[73]筋肉損傷は、トレーニングによって上昇する、特にトレーニング量を変化させた後に上昇する血液CKで、試験開始時、および過負荷のサイクルを行っている第9週と第10週に評価した(表6;時間、p<0.001)。偽薬補給群のCKレベルは、初期のトレーニングで342±64%上昇し、2週間の過負荷サイクルで159±55%上昇した。HMBの補給は、初期のトレーニング(第0〜1週)および過負荷サイクルの間(第9週および第10週)(HMB*時間、p<0.001)の両方で、CKの上昇を有意に弱めた。ATPのみの補給では偽薬の補給と比較してCKの上昇は弱まらなかった。しかしながら、HMB−ATPの補給では、第1、4、9、および10週の時点で、偽薬と比較してCKの上昇がHMBのみを補給した場合の効果と同様、有意に弱まった(t検定、p<0.05)。

- 投与経路が経口、非経口、舌下、局所、経皮、筋内および吸入からなる群より選択される、請求項1〜3のいずれか一項の組成物。
- 前記経口投与における送達形態が、錠剤、カプセル、粉末、顆粒、微粒剤、小丸薬、軟質ゲル、制御放出形態、液体、溶液、エリキシル剤、シロップ、懸濁液、乳剤および泥膏からなる群より選択される、請求項4の組成物。
- 動物の強度を向上させるための、請求項1〜5のいずれか一項の組成物。
- 動物のパワーを向上させるための、請求項1〜6のいずれか一項の組成物。
- 動物の筋肉量を増加させるための、請求項1〜7のいずれか一項の組成物。
- 動物の過負荷の成績特性における衰退を軽減するための、請求項1〜8のいずれか一項の組成物。
