Description
腫瘍形成、疾患進行および転移の過程を通じて、局所宿主組織の微小環境は、能動的な関与因子であり、癌細胞増殖、血管形成、浸潤および生存の程度を決定する。癌の腫瘍形成における肥満細胞の役割はよく理解されていないが、仮定ではあるものの、肥満細胞活性化が、腫瘍侵襲性を促進する分子を産生することによって一部の癌の成長および拡大を促進するという可能性がある。例えば、肥満細胞が膵臓癌発現に関与する正確な機序は明らかではなかったが、肥満細胞は、マウスモデルにおいて膵臓癌腫瘍形成の発現に直接関連付けられており、腫瘍微小環境への高レベルの肥満細胞浸潤が臨床転帰不良を予測するものであったことが示される[Chang DZら、Clin Cancer Res 2011;17:7015−7023]。それ故に、肥満細胞機能の阻害は、十分な肥満細胞の関与がある癌の成長を抑制することにおいて治療的に有用であることが証明され得る。しかし、正確に肥満細胞活性の標的化がどの癌に有効であるかは殆ど分かっていないかまたは議論の的となっている。局所的間質状態および放出されるメディエーターが腫瘍細胞の増殖を促進するかまたは悪性細胞のアポトーシスを誘導するか否かに依存して、肥満細胞が腫瘍形成にとって利益となるかまたは妨げになるか否かについて相反するデータが存在する。[(Theoharides TCら、Trends Immunol 2004;25:235−11);(Samoszuk Mら、BMC Cancer 2005;21:121);(Almholt Kら、Recent Results Cancer Res 2003;162:31−42);(Gooch JLら、Cancer Res 1998;15:4199−205)]。さらに、腫瘍成長の阻害または促進における肥満細胞の二元的役割と一致して、肥満細胞数が多いことは、乳癌、非小細胞肺癌および卵巣癌における良好な予後指標に相当することが示されているが[(Galinsky DSら、Crit Rev Oncol Hematol 2008;68:115−30);(Ribatti Dら、Int Rev Cell Mol Biol 2009;275:89−131)]、これらは皮膚癌(メラノーマおよび非メラノーマの両方およびメルケル細胞癌)[Grimbaldeston MAら、Br J Dermatol 2004;150:895−903);(Grimbaldeston MAら、J Invest Dermatol 2000;115:317−20)]、口腔扁平細胞癌、一部のタイプのリンパ腫および前立腺癌[(Galinsky DSら、Crit Rev Oncol Hematol 2008;68:115−30);(Ribatti Dら、Int Rev Cell Mol Biol 2009;275:89−131)]では予後不良と関連付けられる。肥満細胞密度または肥満細胞活性化の度合いが肥満細胞関連症状において肝要な考慮事項に相当するか否かも未だ明らかではなく、これらの2つの点は必ずしも互いに相関しない[Hermine Oら、PLoS ONE.2008;3:e2266]。
癌疼痛の病因は複雑であり、未だあまり理解されていない。癌疼痛は、重篤および消耗性であり得、既に余命が短くなっている患者において大幅にクオリティー・オブ・ライフを低下させる。特に膵臓癌を考慮すると、腹痛および背痛は重大な合併症であり、切除不能な膵臓癌患者のうち75%近くが診断時に疼痛があり、進行性疾患では患者の90%超まで増加する。[Hameed Mら、Cancer 2011,3,43−60]。膵臓癌における疼痛は、元来、内臓性、体性または神経性であり得、組織損傷、炎症、導管閉塞および浸潤により生じる。内臓性侵害受容性疼痛は、特に伸張、虚血および炎症に感受性がある構造である上腹部臓器に対する損傷により引き起こされ、一般的には場所がはっきりしない拡散痛を生じる。体性および神経障害痛は、腫瘍の伸長から生じ、周囲の腹膜、後腹膜、骨および、後者の場合、腰仙骨神経叢などの神経へと至り得る。
癌疼痛を評価し、記録するために、臨床家は、適切な評価機器および手順を選択しなければならい。しかし、このようなツールが評価すべきであることに対しても、現在も広く一般に許容される癌疼痛評価ツールまたはコンセンサスはない。結果として、既存のツールにおいて非常に多様な次元および項目が使用されており、これは、概して疼痛評価の妥当性に影響し得、研究間の比較も困難になり得る。疼痛評価ツールは、一次元または多次元であり得る。文献レビューおよび専門家ワーキンググループの意見に基づき、単項目一次元ツールが癌患者における最もよく使用される疼痛評価ツールの1つであることは一般に同意される。さらに、疼痛強度の変化の単純な評価および臨床設定における疼痛強度の評価のために、視覚的アナログ尺度(VAS)に基づくツールは、精神測定学的に満足できるものであることが証明されている[Jensen,MPら、J Pain 2003;4(1):2e21]。一次元疼痛評価ツールとしては、数値化スケール(0は「疼痛なし」であり、10は「想像可能な最悪の疼痛」);語句表現スケール(「疼痛なし」、「軽度疼痛」、「中等度疼痛」、「重度疼痛」);または患者が疼痛の強度を最もよく示す場所を線上で示す、視覚的アナログ尺度(左側に「疼痛なし」および右側に「想像可能な最悪の疼痛」などのアンカー付きの100mmの線)が挙げられる。各スケールは、その強度および脆弱性を有するが;疼痛強度の殆どの自己評価基準は、互いに強く関連し、特に明確な指示および練習のための機会が与えられている場合、多くの状況において同じように使用され得る。
様々な染色体異常につながるゲノムの変化は、全ての悪性腫瘍の特徴である。遺伝子突然変異に加えて、腫瘍成長は、遺伝子発現のレベル変化によっても維持されている。遺伝子発現プロファイリングは、特定の生理学的/病理学的試料の細胞機能または遺伝子「指紋」の全体像を描き出すための、数千種類の遺伝子の発現(すなわち活性)の同時測定である。癌との関連で、より正確に腫瘍を分類するために遺伝子発現プロファイリングが使用されており;さらに発現プロファイルの比較は、遺伝子が一貫して上方制御または下方制御されるサブ集団を同定し得る。それ故に、遺伝子発現プロファイリング由来の情報は、客観的診断を行う、生存と相関する遺伝子を同定する、前癌状態の病変のリスク評価を提供する、およびある種の処置に対する反応を予測する、可能性を有する。後者の例において、その患者の遺伝子指紋から、与えられた薬物に患者が反応する可能性など、直接的な臨床的意義の疑問に答え得る。
膵臓は、(食物消化に重要な酵素の産生に関与する)外分泌細胞および(インスリンなどのホルモンを産生する)内分泌細胞を含有する。外分泌および内分泌細胞の両方とも腫瘍を形成し得るが、外分泌膵臓により形成されるものがはるかに一般的であり、予後が非常に不良である。外分泌膵臓腫瘍の大部分は腺癌である。内分泌膵臓の腫瘍(膵島細胞腫瘍としても知られる。)は非常に稀であり、本来、殆ど良性である。
膵臓腺癌の処置
膵臓癌の処置は、表1に記載のように癌のステージに依存する。疾患が膵臓に限定され、周囲の血管から明らかに分離される(すなわちそれが局所的で切除可能である)場合、最適な処置は、術後の化学療法および/または放射線療法を伴う手術である。疾患が周囲血管を包み込むかまたは圧迫するか、または隣接構造に広がっている(すなわち局所進行および切除不能)場合、化学療法および/または放射線療法が提案される。稀に、患者が処置によく反応する場合、その後手術により腫瘍を切除し得る。疾患が遠隔臓器に広がっている(すなわち転移性)場合、化学療法が提案される。殆どの場合、これらの処置により治癒しない。

(iii)ハロゲン、1から10個の炭素原子を含有するアルキル基、トリフルオロメチルおよびアルコキシなどの1以上の置換基の何らかの組み合わせをさらに有し得る、2−チエニル、3−チエニル、2−チアゾリル、4−チアゾリル、5−チアゾリルなどの5員環芳香族複素環基、である。);または医薬的に許容可能なその塩または溶媒和物を有する。
本明細書中で使用される場合、「アリール基」という用語は、炭素および水素原子を含む、単環式または多環式−芳香族基を意味する。適切なアリール基の例としては、フェニル、トリル、アンスラセニル、フルオレニル、インデニル、アズレニルおよびナフチルならびにベンゾ縮合炭素環部分、例えば5,6,7,8−テトラヒドロナフチルなどが挙げられるが限定されない。アリール基は、未置換であるかまたは1以上の置換基で置換されたものであり得る。ある実施形態において、アリール基は、6個の炭素原子を含む単環式環であり、本明細書中で「(C6)アリール」とする。

・患者が、少なくとも1回、VAS疼痛強度>20または同等の疼痛強度の評価基準の「疼痛」サブ集団基準に合致する場合、処置が指示される。
・膵臓癌と関連しない疼痛を取り除くために少なくとも1つの質問を行い得、例えば,患者は、その疼痛の位置を示すように求められ得る。
・何らかの他の独立予測因子なしで「疼痛なし、モルヒネなし」のサブ集団を処置しないという規則の厳守によって、マシチニブ+ゲムシタビン併用が有害であることが示されている患者が保護される。
−≦4カ月生存したマシチニブ+ゲムシタビン処置群の患者
−≦4カ月生存したプラセボ+ゲムシタビン処置群の患者
−>15カ月生存したマシチニブ+ゲムシタビン処置群の患者

−この分析では、治験AB07012に対して無作為化された全部で119名の患者から採取した末梢血細胞試料中の多数の遺伝子の発現レベルを同時に測定した(1:1比のマシチニブおよびプラセボ処置患者)。
−分析試料は第0週(ベースライン)にのみ1回採取した。
−患者の血液試料を回収するためにPAXgene(商標)Blood RNA Systemを使用し、製造者の推奨に従い、PAXgene Blood RNA Kit V.2(PreAnalitix)を用いてRNAを抽出した。Eukaryotic Total RNA 6000ナノチップ(Agilent Technologies)を用いて2100 Bioanalyzer(Agilent Technologies,Palo Alto,USA)でRNA完全性の調節を行った。NanoDrop ND−1000分光光度計を用いてRNA量を調節した。−80℃で精製RNAを保存した。
−一連の推定バイオマーカーを選択するために、デジタル遺伝子発現(DGE)実験を行った。COBASプラットフォーム(LC480、ROCHE Diagnostics)上でリアルタイムPCRを用いてバイオマーカー検証を行い、最良のバイオマーカーを選別するために適切な生物統計学的アプローチを使用している。
−Roche Diagnosticsのプロトコールに従い、RNAを逆転写した。リアルタイムPCRによって推定バイオマーカーの遺伝子発現レベルを調べた。
−DGE分析の結果、119名の修正治療企図患者のゲノムデータベースから得られた169種類の遺伝子が選択された。
−各遺伝子に対して、DCtに対する3つのカットオフを指定した:中央値、Q1(第一四分位、P25)およびQ3(第三四分位、P75)。「中央値DCt」は、被験患者の全てのDCtの中央値を意味する。各カットオフに対して(カットオフ未満/カットオフ超)、処置群間の全生存期間の差異を説明するために多変量モデルを使用した。処置群の効果が有意であった場合(p<5%)、試験中の遺伝子は、処置群に依存して生存において異なる効果を有し、遺伝子がさらなる分析に対して確保されたと結論付け得る。遺伝子KITは重要なので、その有意水準が如何なるものでもこの遺伝子を選択した。
−多変量分析によって、続いて遺伝子選択を関連カットオフ値である全部で64種類の遺伝子まで絞った。
−これらの64種類の遺伝子からの全ての可能性のある二遺伝子組み合わせおよびそれらの個々のカットオフ値を多変量モデルにもう一度供した。
−Coxモデルにおいて提供されるカイ二乗検定のp−値により測定される組み合わせの判別力に従って、各組み合わせを分類した。ある一定の二遺伝子組み合わせを確保するためのさらなる基準は、絞られるサブ集団が少なくとも40名の患者(総試料の1/3)を含有すべきであるということである。
−6種類の同定された遺伝子発現予測因子中で見出される10種類の個別の遺伝子(すなわちp−値<0.00001である制御遺伝子のペア)は、ACOX−1、TNFRSFS10B、RPS23、ABCC3、LYN、HIF1ALPHA、ABCC1、IGJ、UBE2HおよびPARP−2であった。
・ACOX−1遺伝子の場合、プライマーは、ヌクレオチド73,938,893とヌクレオチド73,939,007との間の染色体17上に位置する配列を増幅する(2009年2月にアセンブリー GRch37/hg19、UCSCソース)。
・TNFRSF10B遺伝子の場合、プライマーは、ヌクレオチド22,877,657とヌクレオチド22,877,728との間の染色体8上に位置する配列を増幅する(2009年2月にアセンブリー GRch37/hg19、UCSCソース)。
・RPS23遺伝子の場合、プライマーは、ヌクレオチド81,571,951とヌクレオチド81,572,049との間の染色体5上に位置する配列を増幅する(2009年2月にアセンブリー GRch37/hg19、UCSCソース)。
・ABCC3遺伝子の場合、プライマーは、ヌクレオチド48,762,132とヌクレオチド48,762,221との間の染色体17上に位置する配列を増幅する(2009年2月にアセンブリー GRch37/hg19、UCSCソース)。

−これらの10種類の遺伝子および6種類の遺伝子発現予測因子を同定し、最も一般的な/特異的な遺伝子発現プロファイルを同定して、(例えば試料操作エラーゆえの)個々の変動または外れ値を捨てるために、プールストラテジーを最終的に実行した。試料プールは、小さすぎて個々の研究が何らかの確定的な結論を下せない場合、疫学において頻繁に使用される方法である。
−それ故に、最も重要な二遺伝子組み合わせを最初に選択した(および、次いで次の情報を記録した:サブ集団中の患者数、ハザード比(HR);Coxモデルのp−値。次に、試料サイズおよびまた分析力も向上させるために、第二の最重要クラスの二遺伝子組み合わせを加えた。上記と同じ情報を記録した。新しい組み合わせの付加後、試料サイズにさらなる患者が加えられなかった(または少数しか加えられなかった)場合、ハザード比および/またはp−値が維持された状態で、選択過程を停止した(実施例2、表5参照)。
−最終的選択による6種類の組み合わせ(全部で66名の患者)後、本過程を停止し、二遺伝子組み合わせ(個々に本明細書中で以後、「遺伝子発現予測因子」またはまとめて「遺伝子/転写指紋」と呼ぶ。)の最終選択は、次のものであった:
・ACOX−1に対する3.05以下およびTNFRSF10Bに対する6.1以下の患者デルタサイクル閾値での、同時に起こる、遺伝子ACOX−1およびTNFRSF10Bの上方制御;(HR=0.19、p−値=0.0091)。
・RPS23に対する0.35より大きい、およびACOX−1に対する3.05以下の患者デルタサイクル閾値での、同時に起こる、遺伝子RPS23の下方制御および遺伝子ACOX−1の上方制御;(HR=0.20、p−値=0.00046)。
・ABCC3に対する4.3以下およびLYNに対する1.65以下の患者デルタサイクル閾値での、同時に起こる、遺伝子ABCC3およびLYNの上方制御;(HR=0.19、p−値=0.00025)。
・HIF1Aに対する3.95以下およびTNFRSF10Bに対する5.65以下の患者デルタサイクル閾値での、同時に起こる、遺伝子HIF1AおよびTNFRSF10Bの上方制御;(HR=0.19、p−値=0.00013)。
・ABCC1に対する3.5より大きい、およびIGJに対する7.05以下の患者デルタサイクル閾値での、同時に起こる、遺伝子ABCC1の下方制御および遺伝子IGJの上方制御;(HR=0.19、p−値=0.000011)。
・UBE2Hに対する3.7より大きい、およびPARP−2に対する7.1より大きい患者デルタサイクル閾値での、同時に起こる、遺伝子UBE2HおよびPARP−2の下方制御;(HR=0.192、p−値=0.000004)。
この適用におけるゲムシタビンとの併用処置でのマシチニブの有効性および安全性を評価するためのAB Scienceにより行われた膵臓癌における開発計画は、次の臨床試験に基づいた:治験AB07012「進行性/転移性膵臓癌の患者の処置における、ゲムシタビンと組み合わせたプラセボおよび、ゲムシタビンと組み合わせた9mg/kg/日のマシチニブの有効性および安全性を比較するための、プロスペクティブ多施設無作為化二重盲検プラセボ対照、2平行群、第3相試験(A prospective,multicenter,randomized,double−blind,placebo−controlled,2−parallel group,phase 3 study to compare efficacy and safety of masitinib at 9mg/kg/day in combination with gemcitabine,to placebo in combination with gemcitabine, in treatment of patients with advanced/metastatic pancreatic cancer)」。有効性および安全性分析のためのカットオフ日は、盲検を解いた日付に対応する、2012年3月1日であった。
治療企図(ITT)集団は、試験処置を受けたことがあるかないかにかかわらず、全無作為化患者として定めた。mITT(修正治療企図)集団には、十分に裏付けられた処置と関連しない原因のために治験から早期に撤退させられた患者を除き、全ITT患者が含まれた。治験AB07012のITT集団は、2008年1月11日から2010年7月6日(最終の患者が含まれた。)まで登録された353名の患者からなり:マシチニブ+ゲムシタビン処置群の患者が175名およびプラセボ+ゲムシタビン処置群の患者が178名であった。5名の患者を除くITT集団として定義されるmITT集団において臨床効果を分析した。マシチニブ+ゲムシタビン処置群からの2名の患者は、マシチニブ投与もゲムシタビン投与も受けなかった。プラセボ+ゲムシタビン処置群からの2名の患者のうち、1名はプラセボでもゲムシタビンでも処置せず、他の1名はゲムシタビンで処置したがプラセボでは処置しなかった。除外すべき第五の患者は、プラセボ+ゲムシタビン処置群に割り当てられたが、膵臓癌を有しなかった。
−173名の患者はマシチニブ+ゲムシタビンで処置し、
−175名の患者はプラセボ+ゲムシタビンで処置した。
ベースライン特性、処置タイプおよび有効性の分析から3つのサブ集団が現れ、次のとおり定義された:
−「疼痛」:視覚的アナログ尺度(VAS)疼痛強度スコア>20として定義される疾患関連疼痛のある患者(でN=137:マシチニブ+ゲムシタビンおよびプラセボ+ゲムシタビン処置群それぞれ64および73)。
−「疼痛なし、モルヒネなし」:VAS[0−5]として定義される疾患関連疼痛があり、オピオイド鎮痛剤を必要としない患者(N=68:マシチニブ+ゲムシタビンおよびプラセボ+ゲムシタビン処置群において各34名の患者)。
−「中央値疼痛以下」:「疼痛」および「疼痛なし、モルヒネなし」サブ集団に属しない全ての他の患者(N=107:マシチニブ+ゲムシタビンおよびプラセボ+ゲムシタビン処置群においてそれぞれ、57および50名の患者)。
−治験AB07012での全生存期間における多変量分析から、疼痛強度が、全生存期間に影響を与える主要因子(可変要素)として同定された。さらに、疼痛強度の可変要素と患者に投与される併用処置との間で相互関係が見られた。
−全生存期間において治験AB07012から得られた結果から、マシチニブ+ゲムシタビンが、プラセボ+ゲムシタビンと比較して、VAS>20mmの疼痛強度を伴った膵臓癌の患者の全生存期間を有意に上昇させたことが示された。
−20mmのVAS疼痛強度は、死亡に対するハザード比におけるプラトー(または水平漸近線)の出現と一致した。
Mooreらによる膵臓癌療法における同様の試験において、VAS疼痛評価カットオフとして20mmのVASが報告された。その試験において、エルロチニブ+ゲムシタビン併用は、VAS>20;ハザード比1.00(95%CI[0.78;1.27])で、患者に対して、プラセボ+ゲムシタビンの投与を受けた患者と比較した場合、全生存期間においてさらなるベネフィットを全く明示しなかった[Moore MJら、J Clin Oncol.2007 May 20;25(15):1960−6]。
−科学文献において、一部の他の刊行物は、20mmのカットオフでのVASスコアを含む臨床結果を報告した[(Marineo G.J Pancreas 2003;4(1):1−10);(Zaza C、ら、J Pain Symptom Manage,2002;24:526−542)]。
全生存期間は、この治験の主要エンドポイントであった。無作為化の日付から確認された死亡の日付までの間、OSを測定した。死亡が観察されなかった場合、OS上のデータは、患者が生存していることが分かっている最後の日付に打ち切った。処置とは独立に全生存期間に影響を与え得る可変要素を決定するために、プラセボ+ゲムシタビン処置を受けていた患者における一変量分析を通じて、各ベースライン特性でOSを調べた。OSの結果における主要な相違(5%で統計学的有意性、データは示されない。)は、疼痛強度に対するVASスケール;局所的進行性/転移性癌;アルブミンレベル(正常/異常);および膵臓体部における原発性腫瘍の位置のベースライン特性において観察された。全生存期間に対して最大の影響があるパラメーターは疼痛強度であった。疾患関連疼痛が以前から全生存期間に関連付けられてきた一方で、全生存期間に対して疼痛強度が最も重要な因子であることは明らかになっていない。したがって、膵臓癌の患者の全生存期間における疼痛強度の影響は、大きな発見と考えられる。これらの可変要素および特に疼痛強度はプラセボ+ゲムシタビン併用処置で処置した患者において全生存期間に対して明らかに影響を示したので、両併用処置群間のベースライン特性の何らかの差もまた全生存期間に影響を与えると予想された。一変量モデルは、2つの可変要素(ここで、国および転移性/局所進行性)に対して層別される場合でも適切ではなく、したがって全生存期間に対する併用療法の効果を同定するために、代わりに多変量Coxモデルを使用した。全集団および3つのVAS疼痛強度サブ集団(「疼痛」;「疼痛なし、モルヒネなし」;および「中央値疼痛以下」)において、全生存期間に対して多変量Cox分析から得られた結果を下記で与える。
可変要素の加入および維持の両方のために5%閾値を使用して可変要素が段階的手順を通じて選択された、多変量モデルの構築を介して、全生存期間および治療効果における各可変要素の影響を調べた。最終的な多変量モデルには次の因子が含まれた:
−その有意水準が如何なるものであれ、処置群
−「段階的」多変量モデルとともに選択される因子:局所進行性/転移性癌、膵臓の本体における原発性腫瘍の位置、アルブミンレベル(正常/異常)および3つのVAS疼痛強度サブ集団に割り当てられるVAS疼痛評価(上記で定義されるように)。
−(処置群によって、およびファクター・モダリティー(factor modality)によって)カプランマイヤー推定を介して図式的に相互関係が確認された。

「疼痛」(VAS>20)がある患者のサブ集団において、有害事象(AE)の頻度は、両処置群で同様であった(100%)。重篤なAEおよび重大なAEの頻度は、マシチニブ処置群(それぞれ患者の68.8%および85.9%)においてプラセボ処置群(それぞれ56.2%および71.2%)よりも高かった。ゲムシタビン中止または中断につながるAEは、マシチニブ+ゲムシタビン処置群において、プラセボ+ゲムシタビン群よりも頻度が高かった。これらの傾向は全集団で繰り返され;すなわちAEの頻度は両処置群で同様であり、重篤および重大なAEの頻度、ならびにゲムシタビン処置の中止または中断は、マシチニブ+ゲムシタビン処置群においてプラセボ+ゲムシタビン群よりも多かった。
治験AB07012の1つの目的は、切除不能な局所進行性および/または転移性膵臓癌の患者の処置におけるプラセボ+ゲムシタビンの有効性とマシチニブ+ゲムシタビンの有効性を比較することであった。全集団において、マシチニブ+ゲムシタビン処置は、患者の中央値全生存期間において統計学的に有意な改善を示さなかった。しかし、異なるベースライン特性における多変量分析から、全生存期間に対して予測力がある単一の最も重要な因子として疼痛強度が同定された。次の基準に従い、3つのサブ集団が現れた:
「疼痛」:VAS>20
「疼痛なし、モルヒネなし」:VAS<5
「中央値疼痛以下」:VAS=]5−20]
ゲノムデータから有効性の予測基準を定めるために、補助的薬理ゲノム学試験を行った。すなわち、治験AB07012から取り出した膵臓癌患者の無作為化サブセットにおいて下方制御または上方制御され、全生存期間および治験中の処置の臨床的利益に相関させ得る遺伝子の同定である。遺伝子発現予測因子を介して定義される患者サブ集団の処置に関するこの補助的試験からの主要な知見は、上記セクション「発明を実施するための形態」で与えられる。ここで、使用される技術に対する付加的または補足的な詳細を提供する。
ゲノム分析は、ハイスループット法を用いたマシチニブでの処置前に回収された末梢血細胞試料の包括的トランスクリプトーム分析からなり、Next Generation SequencingはSkuldtech(Montpellier、France)により行われた(独立に3回実施)。発現が全生存期間および処置タイプと相関した遺伝子の同定は、多段階過程によるものであった。Skuldtechプロトコール(段階1から7)から得られた抽出とそれに続く差次的遺伝子発現に対するある一定の方法的態様についての全般的考察を下記で与える。
−ドライアイス中のPAXgene試験管中の患者からの全血試料(荷送人:LabConnect,USA)を受け取り、−80℃で保存した。
−回収した試験管は、処置前の119名の患者に属し、第0週と名付ける。
−処置前に119名の患者の血液試料から全RNAを抽出し、第0週と名付けた。この時点でのみトランスクリプトーム分析(バイオマーカー試験)を行った。
−119名のRNA試料全てを分析した。受け取った一部の試料が、不十分な品質の物質であるがゆえに分析不能であった場合、これらは使用しなかった。
−一連の推定バイオマーカーを選択するために、デジタル遺伝子発現(DGE)実験を行った。

患者のプール血液RNA試料から12個のデジタル遺伝子発現(DGE)ライブラリを構築した。4つの処置群のそれぞれ(すなわちプラセボ/ゲムシタビンPまたはマシチニブ+ゲムシタビンMおよび第4月前の死亡、M4または第15月後の生存、M15)に対して、同じプール血液RNA試料を用いて3つのDGEライブラリを構築した(3つの技術的複製物)。このライブラリは、5μgの全RNA(各RNA試料間でプール中のRNA等量)を用いて、製造者のプロトコール(version 2.1B)に従い、IlluminaのDGE Tag Profilingキットにより構築した。Illumina Pipelineを用いて配列決定分析およびベースコーリングを行い、純度フィルタリング後に配列タグを得た。使用したプラットフォームはMGX(Montpellier,France)である。タグ検出、タグカウンティングについて、およびDGEライブラリ品質を評価するために、BIOTAGソフトウェア(Skuldtech,Montpellier,France)で、各DGEライブラリからのデータを分析した(Piquemal Dら、Genomics.2002 Sep;80(3):361−71)。
UniGeneクラスターのよくアノテーションされている配列(Built#232,March 2012,NCBI)からホモサピエンス配列および関連情報を収集するローカルデータベースを作成した。このデータベースの各配列に対して、配列(R1)の3’−の最も近いNlaIII制限部位(CATG)の上流に位置する、予想されるDGEタグ(カノニカルタグ)ならびに内部位置にある推定タグ(転写産物の3’末端から始まるR2、R3およびR4として標識)を抽出した(Piquemal Dら、Genomics.2002 Sep;80(3):361−71)。この仮想のタグの一群を用いてDGEライブラリから得られた実験的タグを一致させ、アノテーションした(17bpについて完全一致)。最初に、仮想カノニカルタグ(R1)との各実験的タグに対する対応を探った。次に、R2タグと、次にR3およびR4と一致しない実験的タグをアノテーションした。edgeR法(バージョン2.6.9、Bioconductor)を用いてDGE実験の分析を行った。
同じ日に、同じピペッターのセットおよび同じマニピュレーターで、製造者の説明書に従い、200単位のSuperscript II酵素(M−MLV RT Type,Invitrogen)および250ngのランダムプライマーを用いて、300ngの全RNAが入っている20μLの最終反応体積中の119種類のRNAのそれぞれに対して逆転写を行った(25℃で10分間、42℃で50分間、70℃で15分間)。
Roche DiagnosticsからのリアルタイムPCR(qPCR)プラットフォーム上で標的遺伝子の確認を行った。製造者の説明書に従い、Roche Diagnostics LightCycler1536(登録商標)qPCR装置上でLightCycler(登録商標)1536 DNA Green Master KitおよびRealTime ready DNA Probes Master Kit(Roche Diagnostics)を用いてqPCR実験を行った。Sybr Greenアッセイに対して、2μLの最終体積中で次のとおり反応混合物を調製した:0.4μLのLightCycler 1536 DNA Green Master 5X(Roche)、0.1μLのBright Green 20X(Roche)、0.1μLのSetup Control 20X(Roche)、0.04μLの50μMプライマーカップル(Eurogentec)、0.36μLのDNAse RNAse不含水および1μLのcDNAマトリクス(1/50最終希釈)。プローブアッセイに対して、次のとおり2μLの最終体積中で反応混合物を調製した:0.4μLのReal Time Ready DNA Probe Master 5X(Roche)、0.1μLのControl Setup 20X、0.1μLの4μMフォワードプライマー(Eurogentec)、0.1μLの4μMリバースプライマー(Eurogentec)、0.1μLの4μM FAM/TAMRAプローブ(Eurogentec)、0.2μLのDNAse RNAse不含水および1μLのcDNAマトリクス(1/50最終希釈)。Agilent Bravo自動液体操作プラットフォームを用いて全ピペット操作段階を行った。PCRプログラムは、95℃で1分間の第一の予備温置段階とそれに続く50回のPCRサイクル(95℃で2秒間、60℃で30秒間)からなる。非特異的な生成物およびプライマー二量体から特異的なものを区別するために、60から95℃への温度の漸増によって溶融曲線を得た。Delta.Ct(DCt)法を用いてqPCRデータを分析した(Livak KJおよびSchmittgen TD.Metohds.2001 Dec;25(4):402−8)。Ct値を2種類の参照遺伝子(ハウスキーピング)のCt値の平均から差し引くことによって、全標的遺伝子に対してDCt値を決定した。この2種類のハウスキーピング遺伝子は、B2M(NM_009735、ムス・ムスクルス(Mus musculus)β−2ミクログロブリン、mRNA)およびGAPDH(NM_002046、グリセルアルデヒド−3−ホスフェートデヒドロゲナーゼ、転写産物変異体1、mRNA+NM_001256799ホモサピエンスグリセルアルデヒド−3−ホスフェートデヒドロゲナーゼ、転写産物変異体2、mRNA)であった。
デジタル遺伝子発現(DGE)法を用いて、患者の全血のトランスクリプトミックプロファイルを行い、良好なマシチニブ反応者と不良な反応者との間で差次的に発現される遺伝子として、edgeR法で169個の遺伝子を選択した。(i)タイプI(α=5%)エラーに基づく、最大の差次的な倍単位の変化(>1.5)、FDR調整p−値基準(<10%)での数学的フィルターおよび(ii)特異的な過程および公知の代謝経路における標的遺伝子の関与による生物学的フィルターに従い、分析遺伝子を選択した。リアルタイムPCRアッセイにおいて、蛍光シグナルの蓄積によって陽性反応を検出する。Ct(サイクル閾値)は、蛍光シグナルが閾値を超える(すなわちバックグラウンドレベルを超える)のに必要なサイクル数として定義される。Ct値は、試料中の標的核酸の量と反比例する(すなわちCt値が低いほど、試料中の標的核酸の量が多くなる。)。
この補助的研究の目的は、(i)プラセボ+ゲムシタビン処置患者と比較した場合の、マシチニブ+ゲムシタビン処置患者に対する延命(すなわちOS上昇)の予測的なバイオマーカー;(ii)プラセボ+ゲムシタビン処置患者と比較した場合のマシチニブ+ゲムシタビン処置患者に対する早期死亡(すなわちOS低下)の予測的なバイオマーカーを明らかにすることであった。この分析によって、治験AB07012の119名の患者からマシチニブでの処置前に回収された末梢血細胞試料中の多数の遺伝子の発現レベルを同時に測定した(1:1比マシチニブ+ゲムシタビンおよびプラセボ+ゲムシタビン処置患者)。患者の血液試料を回収し、RNA(リボ核酸)を単離するためにPAXgene(商標)Blood RNA Systemを使用し、輸送のためにドライアイス中に詰め、−80℃で保存した。PAXgene試験管はFDA承認済みである(i.d.K042613)。分析試料は、第0週(ベースライン)でのみ1回採取した。
−PAXgene(商標)血液RNA回収システムの使用により、RNAの分解および部位間の回収標準が異なることによる試料品質の差が回避される。
−edgeR Bioconductor分析およびRNA試料のプールを通じて、生来の遺伝子特異的なおよび個体間の発現変動性を考慮に入れた。
−DGE法は、関心のあるトランスクリプトームの既存の知識に依存せず、したがって関心のあるあらゆる患者群に適用され得る。
−qPCR実験に対して、産業および研究の標準と適合する、現在の最先端のプラットフォームを使用した。
DGE分析の結果、119名の修正治療企図患者のゲノムデータベースから得られた169種類の遺伝子を選択した。これらの遺伝子の選択は、全生存期間における遺伝子、例えばマシチニブ+ゲムシタビン処置と全生存期間の相関に関連がある遺伝子、の影響に基づいた。定量的ポリメラーゼ連鎖反応(qPCR)によって選択遺伝子のそれぞれを増幅し、個々の正規化後にある患者におけるある遺伝子の発現レベルを評価した。分析は、一連の参照遺伝子(B2M、GAPDH)に対する、各qPCR実験に伴うCt値の変動に基づいた。Ct値をこれらの参照遺伝子のCt値の幾何平均から差し引くことによって、デルタCt値(DCt)を試験下の各遺伝子に対して定義した。各遺伝子に対して、3つのカットオフ:中央値、Q1(第一四分位、P25)およびQ3(第三四分位、P75)を指定した。OSを説明するために、各カットオフ(カットオフ未満/カットオフ超)に対して、第一の判断基準に対して使用された多変量モデルは、ハザード比、カイ二乗統計値およびCoxモデルのp−値(遺伝子あたり1)を計算し、ベースラインでの処置群、腫瘍の状況、腫瘍の位置およびアルブミンレベルの因子を使用した。

ゲノム情報が入手可能な患者の完全コホートを考慮すると(n=119;60名のマシチニブおよび59名のプラセボ患者)、中央値OSは両処置群において7.7カ月であり、死亡に対するハザード比は、0.55のp−値で0.89[0.60;1.31]であった。したがって、遺伝子発現によるさらなる分析なしでは、このサブ集団におけるマシチニブ処置の有益または有害効果に関して何ら結論を引き出すことはできなかった。

Claims (13)
- ゲムシタビンと共にマシチニブまたは医薬的に許容可能なその塩と1種以上の医薬的に許容可能な担体とを含む、ヒト患者における膵臓癌の処置に使用するための医薬組成物であって、前記患者が、20%超の一次元疼痛強度評価ツールスコアの少なくとも1回の発生の報告として定義される疾患関連疼痛強度の予測因子に基づいて処置に対して最初に選択される、医薬組成物。
- 医薬的に許容可能なマシチニブの塩がメシル酸塩である、請求項1に記載の医薬組成物。
- 前記疾患関連疼痛強度は、100mmスケール上で20mm超の視覚的アナログ尺度(VAS)疼痛強度スコアの少なくとも1回の発生の報告として定義される、請求項1または2に記載の医薬組成物。
- マシチニブまたは医薬的に許容可能なその塩が、4.5から12.0mg/kg/日(1日あたり、体重1kgあたりのmg)の1日用量で投与される、請求項1〜3のいずれか1項に記載の医薬組成物。
- マシチニブまたは医薬的に許容可能なその塩が、6.0から7.5mg/kg/日の開始用量で投与される、請求項1〜4のいずれか1項に記載の医薬組成物。
- マシチニブまたは医薬的に許容可能なその塩の用量が、12.0mg/kg/日という最大量に到達するように1.5mg/kg/日の割合で漸増させられる、請求項1〜5のいずれか1項に記載の医薬組成物。
- マシチニブまたは医薬的に許容可能なその塩が、経口投与される、請求項1〜6のいずれか1項に記載の医薬組成物。
- マシチニブまたは医薬的に許容可能なその塩が、1日2回投与される、請求項1〜7のいずれか1項に記載の医薬組成物。
- 前記ヒト患者が、ゲムシタビンでは未処置であるか、またはゲムシタビンでの処置に反応しているかの何れかである、請求項1〜8のいずれか1項に記載の医薬組成物。
- 前記ヒト患者が、ゲムシタビンに対して不応性または抵抗性の何れかである、請求項1〜8のいずれか1項に記載の医薬組成物。
- 前記膵臓癌が、切除不能な膵臓腺癌である、請求項1〜10のいずれか1項に記載の医薬組成物。
- 前記膵臓癌が、転移性の膵臓腺癌である、請求項1〜10のいずれか1項に記載の医薬組成物。
- 請求項1〜12のいずれか1項に記載の医薬組成物を含むキット。
