Description
本願は、発明者がMathias Ehrichであり、代理人整理番号がSEQ−6022−PVである、「PROCESSES AND COMPOSITIONS FOR METHYLATION−BASED ENRICHMENT OF FETAL NUCLEIC ACID FROM A MATERNAL SAMPLE USEFUL FOR NON INVASIVE PRENATAL DIAGNOSES」と題された、2008年9月16日に出願された米国仮特許出願第61/192,264号の利益を主張する。この米国仮特許出願の全体の内容は、あらゆる文章、表および図面を含めて、本明細書中に参考として援用される。
バイオマーカーが、ある特定の実施形態において提供される。いくつかの実施形態において、提供されるバイオマーカーは、胎児の遺伝形質の非侵襲的検出に有用である。ある特定の胎児の遺伝形質としては、胎児核酸の存在または非存在が挙げられるがこれらに限定されない。
非侵襲性の出生前検査は、急速に関心が寄せられている分野になっている。妊娠関連の状態(妊娠中の合併症および胎児の遺伝的欠陥を含む)の早期発見によって、母体と胎児の両方の安全にとって必要な早期の医学的介入が可能になるので、その早期発見は、非常に重要である。出生前診断は、絨毛生検(chorionic villus sampling)(CVS)または羊水穿刺などの手技によって胎児から単離された細胞を用いて行われている。しかしながら、これらの従来法は、侵襲性であり、母体と胎児の両方に対してかなりのリスクをもたらす。National Health Serviceは、現在、侵襲性の羊水穿刺検査後および絨毛生検(CVS)検査後の1〜2パーセントの流産率を言及している。
本技術は、とりわけ、胎児の遺伝形質の非侵襲的検出に有用なヒト後成的バイオマーカーを提供し、その遺伝形質としては、胎児核酸の存在または非存在、胎児核酸の絶対量または相対量、胎児の性別、および異数性などの胎児の染色体異常が挙げられるがこれらに限定されない。本技術のヒト後成的バイオマーカーは、胎児と母体との間で差次的なCpGメチル化パターンを示すゲノムDNAである。本技術の組成物およびプロセスは、母体サンプル中の胎児核酸のメチル化状態に基づいて前記サンプル中の胎児核酸の検出および定量を可能にする。より詳細には、母体サンプル由来の胎児核酸の量が、存在する核酸の総量に対して測定され、それにより、そのサンプル中の胎児核酸のパーセンテージが提供され得る。さらに、胎児核酸の量は、配列特異的(または遺伝子座特異的)様式で、かつ十分な感度で、測定され得ることにより、正確な染色体量決定(chromosomal

本発明の好ましい実施形態では、例えば以下が提供される:
(項目1)
胎児核酸を調製するための方法であって、該方法は:
a)妊婦からサンプルを得る工程;
b)胎児核酸と母体核酸対応物との間の異なるメチル化状態に従って、該妊婦のサンプル由来の該母体核酸から該胎児核酸を分離する工程であって、ここで、該胎児核酸は、配列番号1〜89のポリヌクレオチド配列の1つ以上からの1つ以上のCpG部位を含む、工程;および
c)パート(b)において分離された胎児核酸を鋳型として利用するプロセスによって、胎児核酸を含む核酸を調製する工程
を包含する、方法。

緒言
母体血漿中の胎児核酸の存在は、1997年に初めて報告され、単に母体の血液サンプルの解析による非侵襲性出生前診断の可能性をもたらした(Loら、Lancet 350:485−487,1997)。現在までに、数多くの潜在的な臨床応用法が開発された。特に、母体血漿中の胎児核酸、例えば、DNAの濃度の量的異常が、子癇前症、早産、分娩前出血、侵襲性の胎盤形成、胎児のダウン症候群および他の胎児の染色体異数性をはじめとしたいくつかの妊娠関連障害と関連することが見出された。ゆえに、母体血漿中の胎児核酸解析は、母子の福祉をモニターするための強力な機構である。
本技術の実施は、分子生物学の分野における日常的な手法を利用する。本技術において有用な一般的な方法を開示している基礎的な教科書としては、Sambrook and Russell,Molecular Cloning,A Laboratory Manual(3rd ed.2001);Kriegler,Gene Transfer and Expression:A Laboratory Manual(1990);およびCurrent Protocols in Molecular Biology(Ausubelら、eds.,1994))が挙げられる。
本技術は、妊娠に関連する状態もしくは障害の存在を検出するためおよび/またはその進行をモニターするための非侵襲性手段としての、母体血液中に見られる胎児DNAの分離、濃縮および解析に関する。したがって、本技術を実施する第1の工程は、妊婦から血液サンプルを得ることおよびそのサンプルからDNAを抽出することである。
本技術の方法を用いる検査に適した在胎期間に、血液サンプルを妊婦から得る。その適当な在胎期間は、以下で述べられるように、検査される障害に応じて異なり得る。女性からの血液の回収は、病院またはクリニックが通常従う標準的なプロトコルに従って行われる。適切な量、例えば、代表的には5〜50mlの末梢血が回収され、さらなる調製の前に標準的な手順に従って保存され得る。血液サンプルは、サンプル中に存在する核酸の品質の低下を最小にする当業者に公知の様式で回収され得るか、保存され得るか、または輸送され得る。
本技術に従う母体血液中に見られる胎児DNAの解析は、例えば、全血、血清または血漿を用いて行われ得る。母体の血液から血清または血漿を調製するための方法は、当業者に周知である。例えば、妊婦の血液は、血液凝固を防ぐために、EDTAを含むチューブまたはVacutainer SST(Becton Dickinson,Franklin Lakes,N.J.)などの専門の製品に入れられ得、次いで、遠心分離によって全血から血漿が得られ得る。他方、血清は、血液凝固後の遠心分離を用いて、または用いずに、得られ得る。遠心分離が用いられる場合、遠心分離は、代表的には、適切な速度、例えば、1,500〜3,000×gで行われるが、これに限らない。血漿または血清は、DNA抽出に向けて新しいチューブに移される前に、追加の遠心分離工程に供され得る。
血液を含む生物学的サンプルからDNAを抽出するための公知の方法が数多く存在する。DNA調製の一般的な方法(例えば、Sambrook and Russell,Molecular Cloning:A Laboratory Manual 3d ed.,2001に記載されている方法)に従い得る;商業的に入手可能な様々な試薬またはキット(例えば、QiagenのQIAamp Circulating Nucleic Acid Kit、QiaAmp DNA Mini KitまたはQiaAmp DNA Blood Mini Kit(Qiagen,Hilden,Germany)、GenomicPrepTMBlood DNA Isolation Kit(Promega,Madison,Wis.)およびGFXTMGenomic Blood DNA Purification Kit(Amersham,Piscataway,N.J.))もまた、妊婦由来の血液サンプルからDNAを得るために使用され得る。これらの方法の2つ以上の組み合わせも使用され得る。
本明細書中に提供される方法は、差次的にメチル化されたDNAのメチル化特異的分離に基づいて胎児DNAを濃縮するための代替アプローチを提供する。発生の制御に関与する多くの遺伝子が、胚性幹細胞におけるエピジェネティクスによって調節されることが最近、発見された。その結果として、複数の遺伝子が、胎児起源の核酸と母体起源の核酸との間で差次的なDNAメチル化を示すと予想され得る。いったん、これらの領域が同定されると、メチル化DNAを捕捉する手法を用いて、胎児DNAを特異的に濃縮することができる。差次的にメチル化された領域を同定するために、新規アプローチを用いて、メチル化DNAが捕捉された。このアプローチは、MBD2のメチル結合ドメインが抗体のFcフラグメントに融合されたタンパク質(MBD−FC)(Gebhard C,Schwarzfischer L,Pham TH,Schilling E,Klug M,Andreesen R,Rehli M(2006)Genomewide profiling of CpG methylation identifies novel targets of aberrant hypermethylation in myeloid leukemia.Cancer Res 66:6118−6128)を用いる。この融合タンパク質は、従来のメチル化特異的抗体よりもいくつかの利点を有する。MBD−FCは、メチル化DNAに対してより高い親和性を有し、二本鎖DNAに結合する。最も重要なことには、この2つのタンパク質は、それらがDNAに結合する方法が異なる。メチル化特異的抗体は、確率論的にDNAに結合し、これは、二成分の答えだけを得ることができることを意味する。他方、MBD−FCのメチル結合ドメインは、メチル化の状態に関係なく、DNA分子に結合する。このタンパク質−DNA相互作用の強度は、DNAのメチル化のレベルによって定義される。ゲノムDNAに結合した後、溶出液の塩濃度を徐々に高くすることにより、非メチル化DNAおよびメチル化されたDNAを分画することができ、これにより、より管理された分離が可能になる(Gebhard C,Schwarzfischer L,Pham TH,Andreesen R,Mackensen A,Rehli M(2006)Rapid and sensitive detection of CpG−methylation using methyl−binding(MB)−PCR.Nucleic Acids Res 34:e82)。その結果として、メチル−CpG免疫沈降(MCIP)と呼ばれるこの方法は、メチル化レベルに従ってゲノムDNAを濃縮することができるだけでなく、ゲノムDNAを分画することができ、これは、非メチル化DNA画分も同様に調査されるべきであるときに、特に役立つ。
本技術は、母体サンプル由来の胎児核酸の量を測定するための組成物およびプロセスも提供する。本技術は、少なくとも1つの胎児核酸領域について母体サンプルを濃縮するために、母体核酸を選択的かつ完全にまたは実質的に消化する酵素を用いて母体サンプル由来の核酸を選択的に消化することによって前記母体サンプル中の胎児核酸領域の濃縮を可能にする。好ましくは、消化効率は、約75%、76%、77%、78%、79%、80%、81%、82%、83%、84%、85%、86%、87%、88%、89%、90%、91%、92%、93%、94%、95%、96%、97%、98%または99%超である。濃縮の後、多型配列または亜硫酸水素塩処理を必要としないことにより、女の胎児に対してもおよび様々な民族性に対しても等しく十分機能する溶液がもたらされ、そしてサンプル中に存在するコピー数の少ない胎児核酸を保存する定量方法によって、胎児核酸の量が測定され得る。
様々なメチル化解析手順が、当該分野で公知であり、本技術とともに使用され得る。これらのアッセイは、DNA配列内の1つまたは複数のCpGアイランドのメチル化の状況の判定を可能にする。さらに、それらの方法は、メチル化された核酸を定量するために使用され得る。そのようなアッセイは、他の手法の中でも、亜硫酸水素塩で処理されたDNAのDNA配列決定、PCR(配列特異的増幅のため)、サザンブロット解析およびメチル化感受性制限酵素の使用を含む。
メチル化差次的様式で核酸を分離した後、その核酸は、配列ベースの解析に供され得る。さらに、胎児起源の1つの特定のゲノム配列が、母体の対応物と比べて高メチル化されているかまたは低メチル化されていることが判明すると、この胎児ゲノム配列の量が、測定され得る。続いて、この量は、標準コントロール値と比較され得、ある特定の妊娠関連障害の可能性についての指標として機能し得る。
多くの例において、当該分野で周知のいくつかの核酸増幅手順(上に列挙したものおよび以下でさらに詳細に記載されるもの)のうちのいずれかを用いて本技術の核酸配列を増幅することが望ましい。具体的には、核酸増幅は、増幅される核酸配列に相補的な配列を含む核酸アンプリコン(コピー)の酵素的合成である。核酸増幅は、サンプル中に存在する標的配列の量が非常に少ないときに特に有益である。目的の生物またはウイルスに属するサンプル中の核酸の検出をより確実にするためにアッセイの開始時にはより少ない標的配列が必要とされるので、このアッセイの感度は、標的配列を増幅し、合成されたアンプリコンを検出することによって、大幅に改善され得る。
ポリヌクレオチド配列を決定するための手法もまた、十分に確立されており、関連性のある研究分野において広く実施されている。例えば、ポリヌクレオチドを配列決定するための基本原理および一般的な手法は、分子生物学および組換え遺伝学に対する様々な研究報告および学術論文(例えば、Wallaceら、前出;SambrookおよびRussell,前出およびAusubelら、前出)に記載されている。手動または自動的に研究室において日常的に行われているDNA配列決定方法は、本技術を実施するために使用され得る。本技術の方法を実施するためのポリヌクレオチド配列の変化の検出に適したさらなる手段としては、質量分析、プライマー伸長、ポリヌクレオチドハイブリダイゼーション、リアルタイムPCRおよび電気泳動が挙げられるがこれらに限定されない。

胎児の異数性を検出するために、いくつかの方法が、母親から遺伝した対立遺伝子と父親から遺伝した対立遺伝子との比の測定に依存する。しかしながら、染色体の変化を定量する能力は、無細胞核酸の母体の寄与によって損なわれ、それにより、測定の前に母体DNA由来のサンプルを枯渇させる必要がある。有望なアプローチは、胎児および母体のDNAの異なるサイズ分布を利用するか、またはもっぱら胎児によって発現されるRNAを測定する(例えば、US20060252071として公開され、本明細書中で参考として援用される米国特許出願番号11/384128を参照のこと)。胎児DNAが、母体の血漿中のすべての無細胞DNAの約5%しか構成しないと仮定すると、トリソミーサンプルと健常コントロールとの間には1.6%からたった約1.2%への比の差の減少しか存在しない。その結果として、対立遺伝子比の変化の信頼度が高い検出には、例えば、本技術の組成物および方法を用いて無細胞DNAの胎児画分を濃縮することが必要である。
染色体異常の「検出」という用語は、本明細書中で使用される場合、増幅された核酸種、ヌクレオチド配列種、または前述のものから生成された検出可能な産物(集合的に「検出可能な産物」)のセットを検出することから生じたデータを処理することによる、染色体の不均衡の同定のことを指す。任意の適当な検出デバイスおよび検出方法が、本明細書中で扱われる検出可能な産物の1つ以上のセットを識別するために使用され得る。染色体異常の有無に関係する成果は、被験体またはサンプルに対する染色体異常の存在に関連する、確率(例えば、オッズ比、p値)、尤度、パーセンテージ、閾値に対する値または危険因子を含むがこれらに限定されない任意の適当な形態で表され得る。成果は、ある特定の実施形態において、感度、特異性、標準偏差、変動係数(CV)および/もしくは信頼水準、または前述のものの組み合わせの1つ以上によって提供され得る。
下記の実施例では、10個の母体DNAと胎盤DNAとのサンプル対を用いることにより、差次的にメチル化された領域を同定した。これらの結果は、質量分析ベースの定量的メチル化アッセイを用いて確認された。第1に、母体のバフィーコート由来および対応する胎盤組織由来のゲノムDNAをまず抽出した。次に、MBD−FCを用いることにより、各DNAサンプルのメチル化された画分を捕捉した。図1〜3を参照のこと。2つの組織画分を異なる蛍光色素で標識し、Agilent(登録商標)CpG Islandマイクロアレイにハイブリダイズさせた。図4を参照のこと。これを行うことにより、出生前診断に利用され得る差次的にメチル化された領域が同定された。それゆえ、2つの基準を使用することにより、潜在的な濃縮マーカーとしてのゲノム領域が選択された:観察されたメチル化の差は、試験されたすべてのサンプル対に存在しなければならないし、その領域は、200bpを超える長さでなければならなかった。
母体のバフィーコート由来および胎盤組織由来のゲノムDNA(gDNA)をそれぞれ、Qiagen(登録商標)(Hilden,Germany)製のQIAamp DNA Mini KitTMおよびQIAamp DNA Blood Mini KitTMを用いて調製した。MCIpのために、NanoDrop ND 1000TM分光光度計(Thermo Fisher(登録商標),Waltham,MA,USA)を用いてgDNAを定量した。Branson Digital Sonifier450TM(Danbury,CT,USA)を以下の設定:振幅20%、超音波処理時間110秒、パルスオン/パルスオフ時間1.4/0.6秒を用いて、500μlのTE緩衝液中で2.5μgのDNAを300〜500bpの平均フラグメントサイズに超音波処理した。フラグメントの範囲を、ゲル電気泳動を用いてモニターした。
1サンプルあたり、56μgの精製MBD−Fcタンパク質および150μlのProtein A Sepharose 4 Fast Flowビーズ(Amersham Biosciences(登録商標),Piscataway,NJ,USA)を15mlのTBS中において4℃で一晩回転させた。次いで、そのMBD−Fcビーズ(150μl/アッセイ)を移し、2mlのUltrafree−CL遠心濾過デバイス(Millipore(登録商標),Billerica,MA,USA)中で分散させ、BufferA(20mM Tris−HCl,pH8.0、2mM MgCl2、0.5mM EDTA 300mM NaCl、0.1%NP−40)で3回、遠心分離によって洗浄した。2mlのBufferA中の洗浄されたMBD−Fcビーズに超音波処理されたDNA(2μg)を加え、4℃で3時間回転させた。ビーズを遠心分離することにより、未結合のDNAフラグメント(300mMの画分)を回収し、続いて増加性のNaCl濃度(400、500、550、600および1000mM)を含む600μlの緩衝液で2回洗浄した。各洗浄工程のフロースルーを別個のチューブに回収し、MinElute PCR Purification KitTM(Qiagen(登録商標))を用いて脱塩した。並行して、MinElute PCR Purification KitTM(Qiagen(登録商標))を用いて、200ngの超音波処理された投入DNAをコントロールとして処理した。
マイクロアレイハイブリダイゼーション用の蛍光標識DNAを生成するために、各サンプルについての600mMおよび1MのNaCl画分(濃縮されたメチル化DNA)を併せ、BioPrime Total Genomic Labeling SystemTM(Invitrogen(登録商標),Carlsbad,CA,USA)を用いてAlexa Fluor 555−aha−dCTP(母体)またはAlexa Fluor 647−aha−dCTP(胎盤)で標識した。この標識反応は、製造者のマニュアルに従って行われた。対応する母体/胎盤対の異なって標識されたゲノムDNAフラグメントを、50μgのCot−1DNA(Invitrogen(登録商標))、52μlのAgilent10×ブロッキング試薬(Agilent Technologies(登録商標),Santa Clara,CA,USA)、78μlの脱イオンホルムアミドおよび260μlのAgilent2×ハイブリダイゼーション緩衝液が補充された最終体積80μlになるように併せた。そのサンプルを3分間95℃に加熱し、混合し、続いて37℃において30分間インキュベートした。次いで、Agilent CpG Island Microarray KitTMにおけるハイブリダイゼーションを、Agilent SureHybTMチャンバーおよびAgilentハイブリダイゼーションオーブンを使用して67℃において40時間行った。スライドを、室温において5分間、Wash I(6×SSPE、0.005%N−ラウロイルサルコシン)中で洗浄し、37℃においてさらに5分間、Wash II(0.06×SSPE)中で洗浄した。次に、そのスライドをアセトニトリルおよびAgilent Ozone Protection SolutionTM中に、それぞれ30秒間浸漬した。直ちにAgilent DNA Microarray ScannerTMを用いて像をスキャンし、解析した。Feature Extraction Software v9.5および標準的なCGHプロトコルを用いて、マイクロアレイ像を処理した。
ゲノムDNAの亜硫酸水素ナトリウム変換を、EZ−96 DNA Methylation KitTM(ZymoResearch,Orange County,CA)を用いて行った。製造者のプロトコルに従って、1μgのゲノムDNAおよび交互変換プロトコル(2温度のDNA変性)を用いた。
SequenomのMassARRAY(登録商標)Systemを用いることにより、定量的メチル化解析を行った。このシステムは、飛行時間型マトリックス支援レーザー脱離イオン化(MALDI−TOF)質量分析をRNA塩基特異的切断(Sequenom(登録商標)MassCLEAVETM)と組み合わせて利用する。次いで、検出可能なパターンをメチル化の状態について解析した。Sequenom(登録商標)EpiDESIGNERTM(www.epidesigner.com)を用いて、PCRプライマーを設計した。85個の標的領域を網羅する合計261個のアンプリコンを、確認のために使用した(増幅長中央値=367bp、最小値=108、最大値=500;1アンプリコンあたりのCpG数の中央値=23、最小値=4、最大値=65)。各逆方向プライマーについてはインビボ転写用の追加のT7プロモータータグ、ならびに順方向プライマーには融解温度の差について調整する10merのタグが付加された。MassCLEAVE(tm)生化学を以前に報告されているように行った(Ehrich M,ら、(2005)Quantitative high−throughput analysis of DNA methylation patterns by base specific cleavage and mass spectrometry.Proc Natl Acad Sci USA 102:15785−15790)。MassARRAYTMCompact MALDI−TOF(Sequenom(登録商標),San Diego)を用いて質量スペクトルを取得し、メチル化の比をEpiTYPERTMソフトウェアv1.0(Sequenom(登録商標),San Diego)によって生成した。
すべての統計的計算を、R統計学的ソフトウェアパッケージ(www.r−project.org)を用いて行った。まず、アレイプローブをそれらのゲノム位置に基づいてグループ分けした。続いて、1000bp以上離れていないプローブを一緒にグループ分けした。差次的にメチル化された領域を同定するために、コントロールサンプルを参照として使用した。コントロールサンプルにおいて、血液由来コントロールDNAのメチル化画分を、それ自体に対してハイブリダイズさせた。理想的には、このサンプルは、およそ0の2色チャネルlog比を示すべきである。しかしながら、ハイブリダイゼーション挙動のばらつきが原因で、それらのプローブは、0.02という平均log比および0.18という標準偏差を示す。次に、本発明者らのサンプルにおいて観察されたlog比をコントロールサンプルと比較した。二元配置の(two way)対応のあるt検定を用いることにより、それらのグループが同一であるという帰無仮説を試験した。4つ未満のプローブを含むグループをこの解析から除外した。4または5つのプローブを含むグループについては、すべてのプローブを対応のあるt検定に使用した。6つ以上のプローブを含むグループについては、1回につき5つのプローブからなるスライディングウィンドウ検定(sliding window test)を用い、ここで、そのウィンドウを、プローブ1つ分ずつ移動させた。各試験サンプルをコントロールサンプルと比較し、p値を記録した。10個中8個のサンプルがp値<0.01を示すか、または10個中6個のサンプルがp値<0.001を示す場合に、ゲノム領域を、差次的にメチル化されているとして選択した。そのグループの8個未満のサンプルがp値<0.01を示し、6個未満のサンプルがp値<0.001を示すとき、そのゲノム領域を差次的にメチル化されていないと分類した。いずれのカテゴリーにも入らなかったサンプルは、この解析から除外した。差次的にメチル化されていると同定されたゲノム領域のサブセットについて、その結果を、定量的メチル化解析を用いて確かめた。
差次的にメチル化された領域を同定するために、単球のメチル化DNA画分をそれ自体に対してハイブリダイズさせた標準的なサンプルを使用した。この標準物質は、あるゲノム領域における蛍光測定値のばらつきについての参照を提供した。次いで、この標準に対する10個の胎盤/母体サンプルの各々のlog比を比較することによって、差次的にメチル化された領域を同定した。本研究の目的は、母体DNAと胎児DNAとの信頼度の高い分離を可能にするマーカーを同定することであるので、標的の選択は、ある連続した一続きのゲノムDNAにわたって安定して一貫したメチル化の差を示す遺伝子に限定された。これにより、その解析の焦点が、複数のプローブが差次的なメチル化を示すゲノム領域に当てられた。その選択は、個体間の差が大きいサンプルを除くすべてのサンプルが差次的なメチル化を示す標的領域にも限定された。これらのサンプルのうちの2つが、このマイクロアレイ解析において概して低いlog比を示した。対応のある検定を標的選択に使用したので、これは、その結果に対して否定的に影響しなかった。
マイクロアレイの知見を確認するために、13、18および21番染色体由来の63個の領域ならびに他の常染色体由来の追加の26個の領域を、異なる技術によって確かめるために選択した。母体および胎盤のサンプル中のDNAメチル化を定量的に測定するために、Sequenom EpiTYPERTM技術を使用した。EpiTYPERTM法の説明については、Ehrich M,Nelson MR,Stanssens P,Zabeau M,Liloglou T,Xinarianos G,Cantor CR,Field JK,van den Boom D(2005)Quantitative high−throughput analysis of DNA methylation patterns by base specific cleavage and mass spectrometry.Proc Natl Acad
Sci USA 102:15785−15790)を参照のこと。標的領域内の各個別のCpG部位について、すべての母体DNAサンプルおよびすべての胎盤サンプルにおける平均メチル化値を計算した。次いで、母体のメチル化の平均と胎盤のメチル化の平均との差をマイクロアレイの結果と比較した。この2つの技術による結果は、よく一致していた(図7を参照のこと)。85個の標的領域について、定量的結果が、マイクロアレイの結果を裏付ける(95%の確認率)。すべてが18番染色体上に位置する4つの標的領域については、結果を確認できなかった。この矛盾の理由は、現在のところ不明である。
実施例2では、胎児の形質(例えば、胎児の性別またはRhD適合性)を検出するかもしくは確かめるため、またはトリソミー21などの染色体異常を診断するため(両方が、本明細書中で「メチル化に基づく胎児の診断法」と呼ばれる)に使用され得る、母体サンプル中に存在する胎児核酸の量を検出するための非侵襲性のアプローチ(本明細書中で「胎児の定量方法」と呼ばれる)が記載される。図10は、胎児の定量方法の1つの実施形態を示しており、図11は、メチル化に基づく胎児の診断法の1つの実施形態を示している。両方のプロセスが、母体サンプルから得られた胎児DNAを使用する。そのサンプルは、差次的にメチル化されている母体核酸および胎児核酸を含む。例えば、そのサンプルは、母体の血漿または血清であり得る。胎児DNAは、母体血漿中に約2〜30%の全DNAを含む。サンプル中に存在する全核酸に対する胎児の寄与の実際の量は、妊娠ごとに異なり、在胎期間、母体の健康状態および胎児の健康状態を含むがこれらに限定されないいくつかの因子に基づいて変化し得る。
母体サンプルから非侵襲的に単離された胎児DNAを正確に検出および定量する必要がある。本技術は、母体の血漿中または血清中の循環無細胞胎児核酸(ccfDNA)の存在を利用する。商業的および臨床的に実施するために、本技術の方法は、利用可能な少量の限られた胎児DNAだけを消費すべきである。例えば、サンプルの50%、40%、30%、25%、20%、15%、10%、5%未満またはそれ以下。さらに、このアプローチは、好ましくは、以下のアッセイの1つ以上(好ましくは、すべて)が含まれる多重アッセイ形式で開発されるべきである:
・サンプル中に存在するゲノム等価物の全量を検出するためのアッセイ、すなわち、母体と胎児の両方のDNA種を認識するアッセイ;
・男児を妊娠している女性から単離された胎児DNA、すなわち、Y染色体に特異的な配列を検出するためのアッセイ;
・胎児と母体との間で差次的にメチル化されていると同定された領域に特異的なアッセイ;または
・調査されるすべての組織において低メチル化されていると知られている領域に特異的なアッセイ(制限効率に対するコントロールとして機能し得る)。
・各アッセイについて、標的配列に対する1つ以上のヌクレオチド差などの競合物質の識別性の特徴ではなく、標的配列と同一または実質的に同一である、標的特異的な競合オリゴヌクレオチド。このオリゴヌクレオチドは、PCR反応物に加えられると、標的と同時に増幅され、これらの2つのPCRアンプリコンから得られる比は、母体サンプル中に存在する標的特異的なDNA配列の数(例えば、特定の遺伝子座由来の胎児DNA)を示唆し得る。
・アンプリコンの長さは、好ましくは、より短いフラグメントに対する増幅に偏らないように同様の長さであるべきである。しかしながら、増幅効率がほぼ等しい限り、様々な長さが使用され得る。
・差次的にメチル化された標的は、表1、または母体と胎児との間で差次的にメチル化されていると知られている他の任意の標的から選択され得る。これらの標的は、非妊婦から単離されたDNAでは低メチル化され得、胎児サンプルから得られたサンプル中では高メチル化され得る。これらのアッセイは、制限効率のコントロールとして機能し得る。
・様々なアッセイから得られた結果は、以下の1つ以上を定量するために使用され得る:○サンプル中に存在する増幅可能なゲノムの総数(ゲノム等価物の総量);
○増幅可能なゲノムのうちの胎児画分(胎児の濃度またはパーセンテージ);または
○胎児から得られたDNA配列間の(例えば、胎児の21番染色体と3番染色体などの参照染色体との間の)コピー数の差。
下記は、例えば、図10に提供されているような、本技術の方法を行うために用いられる反応工程の概要である。この概要は、本技術の範囲を限定すると意図されない。むしろ、この概要は、Sequenom(登録商標)MassARRAY(登録商標)技術を用いる本技術の1つの実施形態を提供する。
1)血漿サンプルからのDNAの単離。
2)メチル化感受性制限酵素(例えば、HhaIおよびHpaII)を用いたDNA標的の消化。
各反応について、利用可能なDNAを水と混合して、25μlの最終容積にした。10単位のHhaI、10単位のHpaIIおよび反応緩衝液からなる10μlの反応混合物を加えた。そのサンプルを制限酵素に対する最適温度においてインキュベートした。HhaIおよびHpaIIは、非メチル化DNAを消化する(そして、ヘミメチル化DNAまたは完全にメチル化されたDNAを消化しない)。消化の後、加熱工程を用いて、それらの酵素を変性させた。
3)ゲノム増幅−PCR試薬(緩衝液、dNTP、プライマーおよびポリメラーゼ)を加えることによって50μlの総体積中でPCRを行った。例示的なPCRおよび伸長プライマーは、以下に提供される。さらに、既知濃度の合成競合オリゴヌクレオチドを加えた。
4)複製(任意)−本試験のPCR後の工程中に導入されるばらつきを最小にするために、PCRの後、50μlの反応物を5μlの対応反応物に分けた(複製物)。PCR後の工程は、SAP、プライマー伸長(MassEXTEND(登録商標)技術)、樹脂処理、スペクトロチップ(spectrochip)の調製およびMassARRAYを含む。
5)増幅可能なゲノムの定量−Sequenom MassARRAY(登録商標)技術を用いることにより、各アッセイに対する増幅産物の量を測定した。PCRの後、一塩基伸長アッセイを用いることにより、増幅された領域(工程3において導入された競合オリゴヌクレオチドを含む)を調べた。目的の部位に直接隣接してハイブリダイズするように設計された特異的な伸長プライマーを導入した。以下に提供される伸長プライマーを参照のこと。これらのDNAオリゴヌクレオチドは、iPLEX(登録商標)MassEXTEND(登録商標)プライマーと呼ばれる。伸長反応において、iPLEXプライマーは、相補DNA鋳型にハイブリダイズし、DNAポリメラーゼによって伸長された。酵素および緩衝液とともに、様々な組み合わせのデオキシヌクレオチド三リン酸およびジデオキシヌクレオチド三リン酸を含む特別な終結混合物が、iPLEXプライマーの限定的な伸長を指示した。相補的なジデオキシヌクレオチドが組み込まれるまで、プライマー伸長が起きる。伸長反応は、各々が独特の分子量を有する様々な長さのプライマー産物を生成した。結果として、そのプライマー伸長産物は、MassARRAY(登録商標)Analyzer Compactにおける飛行時間型マトリックス支援レーザー脱離/イオン化(MALDI−TOF)質量分析を用いて同時に分離および検出され得る。この分離および検出の後、SEQUENOM専売ソフトウェアが、データを自動的に解析する。
6)胎児核酸の量および濃度の計算−差次的にメチル化された標的に基づいて、サンプル中に存在するゲノム等価物の総量、男児を妊娠している女性から単離された胎児核酸の量(および濃度)、および胎児核酸の量(および濃度)を計算するための方法は、下記ならびに図18および19に提供される。
13、18、21番染色体、XまたはY染色体に位置しないハウスキーピング遺伝子において標的を選択した。その標的は、1コピー遺伝子として存在するべきであり、メチル化感受性制限酵素に対するいかなる認識部位も含まないべきである。
ゲノム内の他の位置に類似配列またはパラログ配列を有しない、Y染色体に特異的な標的を選択した。それらの標的は、好ましくは、1コピー遺伝子として存在するべきであり、メチル化感受性制限酵素に対するいかなる認識部位も含まないべきである。
母体DNAと胎児DNAとの間で差次的にメチル化されていると知られている領域において標的を選択した。メチル化感受性酵素に対するいくつかの制限酵素認識部位を含む配列を選択した。本研究の場合、HhaI(GCGC)およびHpaII(CCGG)酵素を使用した。
調査される任意の組織においてメチル化されていると知られていない領域において標的を選択した。使用される各制限酵素に対する1つ以下の部位を含む配列を選択した。
本技術の感度および正確度を、モデルシステムと臨床サンプルの両方を用いて測定した。様々なサンプルにおいて、総コピー数を定量するための2つのアッセイ、メチル化を定量するための3つのアッセイ、Y染色体に特異的な1つのアッセイ、および1つの消化コントロールアッセイを含む多重アッセイを行った。表X1およびX2を参照のこと。さらなるアッセイを含む別の多重スキームが、表Y1およびY2に提供されている。
サンプル中の増幅可能なゲノムコピーの総数を測定するときの方法の感度および正確度を測定するために、非妊婦の血液から単離された様々なDNAサンプルのサブセットを検査した。1反応物あたり約2500、1250、625または313コピーを含むように各サンプルを希釈した。3つの総コピー数アッセイから得られた平均DNA/競合物質比を取得することによって、増幅可能なゲノムコピーの総数を得た。4つの異なるサンプルからの結果を図12に示す。
臨床サンプルにおいて上記方法の感度および正確度を調査するために、男の胎児を妊娠している女性から得られた33個の血漿サンプルを、表Xの多重スキームを用いて調査した。全サンプルの一部だけを使用するという重要な要件を満たすために、各反応について、4mlの抽出物から得られたDNAの4分の1を使用した。
総コピー数の定量の結果は、図14AおよびBに見られる。図14Aでは、各サンプルに対するコピー数が示されている。2つのサンプル(25および26番)が、他のすべてのサンプルよりも著しく多い総コピー数を有する。概して、約1300個の増幅可能なコピー/ml血漿の平均値が得られた(766〜2055の範囲)。図14Bは、所与の値の箱ひげ図を示しており、結果を要約している。
図15AおよびBでは、各サンプルについての胎児のコピー数がプロットされている。すべてのサンプルが男児妊娠に由来した。得られたコピー数は、メチル化特異的マーカーまたはY染色体特異的マーカーを用いて計算され得る。図15Bに見られるように、所与の値の箱ひげ図は、2つの異なる測定値間の最小の差を示した。
Typer4(Sequenomソフトウェア製品)を用いて、質量スペクトル解析を行った。各個別のDNA被検体および競合物質アッセイに対するピークの高さ(ノイズに対するシグナル)を測定し、さらなる解析のためにエクスポートした。
21番染色体由来の20個のマーカーおよび1つ以上の他の常染色体由来の20個のマーカーを使用する測定システムを仮定する第1の単純な検出力計算を行った。100コピーの胎児DNA、25コピーという測定値標準偏差、および0.001より小さい第1種の過誤に対する確率から出発して、本技術の方法が、すべての場合の99.5%において、三倍体の染色体セットから二倍体を区別することができることが見出された。そのようなアプローチの実際の実行は、例えば、絶対コピー数の測定のために競合PCRアプローチを用いる系である質量分析を用いて達成され得る。この方法は、1つの反応において20のアッセイを行い得、繰り返された測定のおよそ3〜5%において標準偏差を有すると示されている。この方法は、メチル化核酸と非メチル化核酸とを区別するための既知の方法(例えば、核酸を分離するためにメチル結合剤を用いる方法、または母体核酸を消化するためにメチル化感受性酵素を用いる方法)と組み合わせて使用された。図8には、メチル化されていない過剰のDNAの存在下において、メチル化DNAを捕捉し、それにより分離するためのMBD−FCタンパク質(メチル結合剤)の有効性が示されている(図8を参照のこと)。

結果は、図20に示されている。x軸における変動係数(CV)と、単純なt検定を用いてアッセイ集団を識別する能力(y軸)との間の関係が、示されている。このデータから、すべての場合の99%において、5%またはそれ以下のCVを提供する0.001の有意水準で2つの集団(正倍数性 対 異数性)が識別され得ることが示唆される。このシミュレーションに基づくと、本方法は、性別に依存せず、すべての民族性において機能する(すなわち、対立遺伝子の偏りがない)胎児異数性の出生前検出のための強力な非侵襲性の診断法である。
Claims (28)
- サンプル中の胎児核酸の量を決定するための方法であって、該方法は:
(a)妊婦からのサンプル中に存在する核酸を、メチル化ヌクレオチドに特異的に結合する作用物質と接触させ、それにより該作用物質に結合した核酸を産生する工程であって、該サンプルは、差次的にメチル化された胎児核酸および母体核酸を含み、メチル化ヌクレオチドに特異的に結合する該作用物質は、メチル化ヌクレオチドに特異的に結合するタンパク質を含む、工程;
(b)(a)の該作用物質に結合した該核酸を単離して、それにより該胎児核酸を濃縮する工程;および
(c)配列番号1〜59から選択される複数の遺伝子座における(b)の胎児核酸の量を分析することによって、胎児核酸の量を決定する工程
を包含する、方法。 - メチル化ヌクレオチドに特異的に結合する前記タンパク質が、メチル−CpG結合タンパク質(MBD)またはそのフラグメントである、請求項1に記載の方法。
- 前記MBDまたはそのフラグメントは、抗体のFcフラグメントに融合されたものである、請求項2に記載の方法。
- 前記作用物質に結合した前記核酸は、塩濃度を徐々に増加させた溶液と接触させることにより単離される、請求項1に記載の方法。
- メチル化ヌクレオチドに特異的に結合する前記作用物質は、高親和性および高アビディティーで結合する、請求項1に記載の方法。
- 前記サンプル中に存在する核酸の総量を決定する工程を包含する、請求項1〜5のいずれか1項に記載の方法。
- 前記サンプル中の胎児核酸の量を決定する工程が、(c)の胎児核酸の量と、核酸の総量とを比較する工程を含む、請求項6に記載の方法。
- (b)の後、既知濃度の1つ以上の競合物質を導入する工程を含む、請求項1〜7のいずれか1項に記載の方法。
- 前記サンプル中の胎児核酸の絶対量を決定する工程を包含する、請求項8に記載の方法。
- 前記複数の遺伝子座が、配列番号1〜33および配列番号36〜59の遺伝子座から選択される、請求項1〜9のいずれか1項に記載の方法。
- 前記複数の遺伝子座が、配列番号42の遺伝子座を含む、請求項1〜10のいずれか1項に記載の方法。
- 前記複数の遺伝子座が、配列番号52の遺伝子座を含む、請求項1〜10のいずれか1項に記載の方法。
- 前記複数の遺伝子座が、配列番号33の遺伝子座を含む、請求項1〜10のいずれか1項に記載の方法。
- 胎児核酸の前記量が、胎児の形質の指標となる、請求項1〜13のいずれか1項に記載の方法。
- 前記胎児の形質が、胎児の異数性である、請求項14に記載の方法。
- 前記方法が、ある特定の臨床的感度または臨床的特異性の必要条件を満たす濃度の胎児核酸を必要とする、請求項14または15に記載の方法。
- 前記胎児核酸の量を決定する工程が、質量分析法、RT−PCR、デジタルPCR、アレイに基づく方法、配列決定法、ナノポアに基づく方法、核酸に結合したビーズに基づくカウント方法および競合物質に基づく方法から選択される1以上のプロセスの使用を含む、請求項1〜16のいずれか1項に記載の方法。
- 前記胎児核酸の量を決定する工程が、質量分析法の使用を含む、請求項17に記載の方法。
- 前記胎児核酸の量を決定する工程が、配列決定法の使用を含む、請求項17に記載の方法。
- 前記配列決定法は、合成により配列決定することを含む、請求項19に記載の方法。
- 妊婦からのサンプル中のY染色体核酸の有無を判定する工程を包含する、請求項1〜20のいずれか1項に記載の方法。
- 妊婦からのサンプル中に存在するY染色体核酸の量が、男の胎児に対して決定される、請求項21に記載の方法。
- 前記胎児核酸の量が、前記Y染色体核酸の量と比較される、請求項22に記載の方法。
- 3以上の遺伝子座における胎児核酸の量が決定される、請求項1〜23のいずれか1項に記載の方法。
- 10以上の遺伝子座における胎児核酸の量が決定される、請求項24に記載の方法。
- 前記胎児核酸の量が、前記消化された母体核酸の平均長より大きいアンプリコンを生成する増幅反応により、該胎児核酸がさらに濃縮されることによって決定される、請求項1〜25のいずれか1項に記載の方法。
- 標的染色体由来の胎児核酸の量が決定され、そして参照染色体由来の胎児核酸の量が決定される、請求項1〜26のいずれか1項に記載の方法。
- 前記標的染色体由来の胎児核酸の量と前記参照染色体由来の胎児核酸の量とを比較することにより、胎児異数性の有無を決定する工程をさらに包含する、請求項27に記載の方法。
