Description
本出願は、米国特許仮出願第61/308,681/号(2010年2月26日出願)、第61/359,188号(2010年6月28日出願)、第61/378,793号(2010年8月31日出願)、及び第61/382,280号(2010年9月13日出願)の「35 U.S.C.§119(e)」の下での恩典を請求するものである。
本発明は、国立衛生研究所(NIH)により付与されたGM0779及び国立眼学研究所により付与されたFEY019454Aの下で米国政府の支援で行われたものである。米国政府は、本発明においてある一定の権利を有する。
エンコーダは、網膜内の細胞(例えば、神経節細胞又は双極細胞)の入出力モデルである。それは、刺激/応答の関係をもたらす。エンコーダは、アルゴリズムとして作動し、アルゴリズムは、本明細書に説明するように、専用回路による処理デバイスにより及び/又はコンピュータ可読媒体を使用して実行することができる。
これは、再スケーリング段階であり、再スケーリング段階は、処理デバイスの前処理モジュール内で実行することができ、前処理モジュールは、時空間変形の作動範囲である数量Xに実際の世界画像Iをマップする。尚、I及びXは、時変数量であり、すなわち、I(j,t)は、各位置j及び時間tでの実像の強度を表し、X(j,t)は、前処理段階の対応する出力を表している。前処理段階は、以下のようにマップすることができ、すなわち、I(j,t)は、X(j,t)=a+bI(j,t)によりX(j,t)にマップされ、ここで、a及びbは、時空間変形の作動範囲に実際の世界像強度の範囲をマップするように選択された定数である。
一実施形態では、変形は、線形、非線形カスケードを使用して実施され(Chichilnisky EJ、2001年、Simoncsaelli他、2004年においてレビュー)、ここで、各神経節細胞mの発火率λmは、以下の数1の式によって示され、
スパイク発生段階において、神経節細胞発火率は、神経節細胞スパイク列に対応するパルスのパターン(流れとも呼ばれる)に変換される。この段階は、処理デバイスの時空間変形モジュールによって実行することができる。
前の節に示した通り、一実施形態では、時空間変形は、数1の式に示すように、線形/非線形(LN)カスケードを通じて実施される。本節では、その式のLm及びNmのパラメータを判断する1つの方法に対して説明する。最初に、正常な生物学的網膜が、2種類の刺激、すなわち、白色ノイズ(WN)及び自然情景動画(NS)を用いて示される。図3〜図9、図12、図13及び図14に示すデータに使用されるエンコーダを生成するために、刺激を各々10minにわたって示し、神経節細胞応答を両方を通じて連続的に記録したが、データセットは、両方の刺激に対する応答を含むものである。呈示は、少なくとも各5分、少なくとも各約10分、少なくとも各約15分、又は少なくとも各約20分続くことができるが、他の時間間隔を用いることができる。測定値時間の長さの判断は、不相応な実験なしに当業者により行うことができる。パラメータ、Lm及びNmの値は、次に、数1の式の速度関数の下での観測されたスパイク列の対数尤度を最大にするように選択され、ここで、対数尤度Zは、以下によって示される(数2の式)。
上述のように、トランスデューサは、神経節細胞をターゲットにする。ここでは、双極細胞をターゲットにするトランスデューサに対して説明する。特に、ChR2は、一例として使用している。
この式で得られるものは、望ましい出力の関数λ(t)として次の入力S(t)、及び以前の時間入力S(t−aΔt)で供給された入力である。aにわたる合計は、フィルタ関数Lがゼロ以外である範囲に及ぶ。逆転アルゴリズムは、個別の合計として畳み込みを表して簡単な代数を実施することによって以下から続く。
トランスデューサは、入力信号を受信し、この信号を受け取ると瞬時に発火又は電圧変化するようにニューロンを駆動することができる。好ましい実施形態では、トランスデューサは、単一の細胞をターゲットにし、例えばかつ以下に限定されないが、感光性蛋白質又は1つの細胞をターゲットにする電極である。他の実施形態では、トランスデューサは、小規模群の細胞をターゲットとし、小規模群の細胞は、1つの細胞、1群の細胞、又はほぼ100の細胞から構成される場合がある。好ましい実施形態では、1組のトランスデューサが使用され、各トランスデューサは、上述のように単一の細胞又は小規模群細胞をターゲットにする。この1組のトランスデューサは、高解像度トランスデューサと呼ばれる。1つよりも多いトランスデューサは、特定の細胞又は小規模群の細胞にターゲットを定めることができ、例えば、チャンネルロドプシン−2及びハロロドプシンは、単一の細胞にターゲットが定めることができる。
遺伝子コード化、例えば、光感受性の蛋白質は、ウイルスベクター及び非ウイルスベクター及び方法を通じて網膜細胞内に導入することができる。ウイルスベクターには、アデノウイルス、アデノ随伴ウイルス、レトロウイルス、レンチウイルス、ヘルペスウイルス、ワクシニアウイルス、ポックスウイルス、バキュロウイルス及び牛乳頭種ウイルス、及び組み換え型アデノ随伴ウイルス(AAV)、組み換え型アデノウイルス、組み換え型レトロウイルス、組み換え型ポックスウイルスのような組み換え型ウイルス、及び当業技術での他の公知のウイルスがあるがこれらに限定されない(Ausubel他、1989年、Kay他、2001年、Walther及びStein、2000年、Martin他、2002年、van Adel他、2003年、Han他、2009年、米国特許公開第20070261127号明細書)。組み換え型ベクター組み付け方法は、公知である(例えば、公開PCT出願WO2000015822及び本明細書で引用の他の参考文献を参照されたい)。
ベクターは、転写開始、終了、プロモーター及びエンハンサアレイ、スプライシング及びポリアデニル化信号のような効率的なRNAプロセシング信号、細胞質のmRNAを安定化するシーケンス、転換効率を高めるシーケンス(すなわち、Kozakコンセンサスアレイ)、蛋白質安定性を高めるシーケンス、及び必要な時には蛋白質処理及び/又は分泌を高めるシーケンスを含むがこれらに限定されない適切な発現制御シーケンスを含むことができる。例えば、固有、構成的、誘導性及び/又は組織特異の多くの異なる発現制御シーケンスは、当業技術で公知であり、望ましい発現のタイプにより、遺伝子の発現を駆動するのに利用することができる。不相応な実験なしに適切な発現シーケンスの選択を当業者によって達成することができる。
この考え方は、逆に実施することができ(本出願人が有するプロモーターに基づいて有用)、例えば、Thy1を使用して神経節細胞内でCHR2を駆動することができる。それを正しい向きに置きにおいて、loxシーケンスをその側に位置させる。次に、別のプロモーター、例えば、GABA A受容体プロモーターを使用して神経節細胞の何らかの部分集合内のCreリコンビナーゼを活性化する。Creは、それらの細胞内のChR2を逆転し、ChR2は遮断され、従って、ChR2は、Thy−1を発現させ、かつ他方のプロモーターを確かに発現させる細胞においてのみ活性になる。Creが他の細胞部類内でも活性化されるか否かは重要ではなく、その理由は、ChR2が他の細胞部類内にはないので、OFFになるChR2はないからである。
本明細書に説明する方法を実施する人工器官デバイスは、以下の要素を含み、要素は、物理的に、無線で、光学的に、又は当業技術で公知の他の手段により相互接続することができる。
カメラは、高忠実度で画像を取得する。一実施形態では、カメラは、Point Grey Firefly MV(60コマ/秒で752x480ピクセル、8ビット/ピクセルが可能)のような電荷結合デバイス(CCD)ベースである(Point Grey Research、カナダ国ブリティッシュコロンビア州リッチモンド44)。リアルタイムでカメラから処理デバイスまで画像を送信するには、広帯域接続が必要である。例えば、20MB/秒を超えるデータ転送は、カメラと処理デバイスの間のUSB2.0インタフェースを使用して達成することができる。
処理デバイスは、リアルタイムで画像からコードへの変換を実行するエンコーダを実行する。
トランスデューサは、出力インタフェースを通じてデバイスから信号を受け取って、エンコーダにより指定されたようにターゲットとされた細胞を活性化する。トランスデューサは、節「トランスデューサ」において先に詳細している。
以下では、エンコーダ及び人工器官の性能を測定する手順に対して説明する。性能は、少なくとも3つの異なる方法、すなわち、強制選択視覚弁別タスクに関する性能、又はベイズ的刺激再構成試験に関する精度、又は誤差パターン試験に関する性能により測定することができる。「試験刺激」という本明細書で使用する用語は、エンコーダ又はエンコーダ+トランスデューサ(すなわち、人工網膜)の性能の評価に向けて動物に呈示された刺激又は刺激を指すものである。「再構成された刺激」という本明細書で使用する用語は、本明細書に説明する方法を使用した刺激の再構成を指すものである。「活性化された網膜」という用語は、エンコーダ+トランスデューサで治療した網膜を指し、これは、神経節細胞又は双極細胞にターゲットが定められたトランスデューサを含む。
視力
世界保健機構(WHO)は、低視力を20/60未満、ただし20/400又はそれよりも大きい良好な方の眼の最高矯正遠見視力、又は20°未満、ただし10°を超える角度に内在する視野の最も広い直径、また、盲目を20/400又はそれ未満の良好な方の眼の最高矯正遠見視力、又は10°未満の角度に内在する視野の最も広い直径として定義している。米国において、法的盲は、良好な方の眼で20/200又はそれ未満の最高矯正遠見視力、又は代替的に20°未満に内在する視野の最も広い直径と定義され、殆どの北米の州では、制限なしの運転免許証には20/40の両眼での最高矯正視力が必要である(Riordan−Eva、2010年)。
ETDRS視力表1及び2を視力測定の標準化に使用する。全ての被験者の視力試験は、視力と無関係に4メートルで始まる。各々、文字順序が異なる2つのETDRS視力表を視力測定に使用する。常に、右眼を視力表1で、左眼を視力肥表2で試験することになる。
各診療所では、EVAが機能しない場合に、あらゆる研究視力試験中にETDRS視力表のためのETDRSライトボックスを有する/使用するべきである。ライトボックスは、3列目の文字(0,8 LogMA)の上部が床から49+2インチ(124.5+5.1cm)になるような高さに壁に吊り下げるか又はスタンド(ニューヨークのLighthouse for the Blindから購入することができる)上に配置すべきである。部屋の照明は、ほぼ50フィートカンデラであるべきであり、かつ被験者とライトボックス間は均一であるべきである。試験のための椅子の中心部から視力表までの距離は、4.0メートルであるべきである。
まず右眼、次に、左眼を試験する。試験椅子の中心部からETDRS視力表まで距離が4.0メートルになるように被験者を着座させる。たとえ被験者の屈折力を測定することができなくても、常に最初にこの試験距離を取る。検眼のためのフレーム内のオクルージョンに加えて、眼帯又はアイパッドが検眼のためのフレームの下に置かれた状態で左眼を塞ぐ。レンズ修正を検眼のためのフレーム内の自覚的屈折検査により行った状態で、被験者に右眼で一番上からETDRS視力表1を読むように求める。各々の答えが採点されるので最良の識別をもたらすために各々の文字に適切な時間を掛けるように被験者に念を押す。読まなければならない形状の全ては文字であり番号がないことを被験者に伝える。
被験者が眼の視力試験で文字を識別することができなかった場合(すなわち、文字得点=0)、倒像検眼鏡を光源として眼の光覚を試験する。試験手順は、光覚検査担当者の通常のルーチンに従って実行することができる。以下の手順が示されている。
各視力測定後に、その診察に対する視力得点を計算する。以下のように、正しく読まれる文字の数により視力得点を定義する。
線形−非線形ポアソン(LNP)カスケードを使用するエンコーダの構成
エンコーダのためのパラメータは、2組の刺激、すなわち、バイナリ時空白色ノイズ(WN)及びニューヨーク市のセントラルパークで録画したグレースケールの自然情景動画(NS)に対する応答から作成した。両方の刺激を15Hzのフレーム率で呈示し、同じ平均値輝度(網膜上で0.24μW/cm2)及びコントラストであった(二乗平均平方根値根(RMS)コントラストは、0.087μW/cm2であった)。前処理段階に対して、視覚刺激が数値範囲0〜255にマップされるように、(「エンコーダ」という名称の節で上述したように)a=0及びb=255/0.48μW/cm2を選択した。
時空間変形に向けてパラメータを判断する手順を説明する。説明するパラメータは、上述の節「エンコーダ」の場合と同様にある。
データセットを構成するために、この実験に向けてチェッカー盤、自然の情景、及びドリフト格子であった自然の刺激及び人工の刺激を含む広範囲の刺激に対する数百個のマウス網膜神経節細胞(515の細胞)の応答を記録した。各細胞に対して、エンコーダ(仮想網膜細胞又はモデル細胞とも呼ばれる)を構成した。これは、以下のように達成された。網膜にWN(白色ノイズ)及びNS(自然の又は自然のままの刺激)刺激を呈示して、神経節細胞の応答を記録し、上述のように、各細胞に対して刺激/応答関係をパラメータ化した。次に、付加的な自然情景及びドリフト格子応答を使用してエンコーダを試験した。このようして、新しい刺激を使用して全ての試験が行われた。
図4は、仮想細胞及び実際の細胞により担持された情報の質も同じであることを示している。図4内の各細胞に対して、仮想細胞の応答によって生成されたポステリアを実際の細胞の応答によって生成されたポステリアと比較した。図4(4−1,4−2,4−3)のAは一部の実施例を示し、図4(4−1,4−2,4−3)のBはデータセット内の全ての細胞に関する結果のヒストグラムを示している。
本出願人のエンコーダを使用して、神経節細胞部類の挙動に対して1組の予想を提供し、次に、予想を検証した。この場合の予想では、ON細胞及びOFF細胞の動き情報の取り出し方、特に、スローモーションの差異が中心であった。
最後に、挙動に関する予想に移った。これに対して、視運動タスクを用い、その理由は、a)簡単であり、b)容易に定量化可能であり、かつc)視運動タスクにより、単一の細胞部類自体、すなわち、この挙動を駆動するON細胞(ON細胞のみが副視索系(AOS)に投影する)を徹底調査することができるからである(Dann及びBbuhl 1987年、Giolli他、2005年)。このタスクにおいて、動物(wtマウス)にドリフト格子を呈示すると、動物は、ドリフト格子を追跡するか、又はドリフト格子を追跡することができない。従って、挙動に関する予想を行うために、エンコーダに動物にするのと同じ質問をした。格子は存在するのか、又は不在か?本出願人は電気生理学的実験において予想を検証するのに使用される手法と平行した手法を使用した。すなわち、最大尤度を使用して応答を復号した。唯一の差異は、挙動との比較に向けて、エンコーダの応答をちょうど2つの代替物(格子存在ウイルスペア不在格子)に復号したという点である。その理由は、これは、挙動的タスクの代替に対応するからである。最後に、動物及びエンコーダに対して、明順応(日光)又は暗順応(常夜灯)の状況を表す刺激を呈示して、2つの代替強制選択心理物理学に対して標準的であるように、刺激の75パーセントが正しく復号されたコントラストと定義されたコントラスト感度を測定した。図6に示すように、エンコーダは、視運動性能のシフトを正しく予想している。
本出願人は、自然情景の動画を呈示して、動物の3つの群から切り取った網膜から神経節細胞応答を記録した。すなわち、a)正常動物(簡潔にいうと、網膜を野生型(WT)マウスから抽出し、次に、網膜に自然情景の動画を呈示し、神経節細胞の発火パターンを記録した)(図7上)、b)人工網膜で治療した盲目の動物(簡潔にいうと、網膜は、網膜変性を有し、かつ網膜神経節細胞内でチャンネルロドプシン−2も発現する研究室で育てた商業的供給元から購入した二重遺伝子組み換えマウスから抽出し、網膜にコード化された自然情景の動画を呈示して、神経節細胞の発火パターンを記録した)(図7中)、及びc)現在の光遺伝子人工器官の手法で治療した盲目の動物(簡潔にいうと、網膜を上述のように同じ二重遺伝子組み換えマウスから抽出し、次に、網膜に自然情景の動画(コード化なし)を呈示して神経節細胞発火パターンを記録した)(図7下)である。
3通りの方法で、すなわち、弁別タスク法(図8)、画像再構成(図9)、挙動的タスク(視運動)、及びに関する性能(図10)を用いてエンコーダ及び人工器官の性能を評価した。尺度及び結果を以下に示す。
弁別タスクから開始した。簡潔にいうと、刺激のアレイを呈示し、神経節細胞(又はエンコーダ)の応答に基づいて、互いから区別することができる範囲を測定した。神経節細胞の記録に対して、刺激をコンピュータモニタ上に示して、Pandarinath他、2010年の場合と同様に神経節細胞応答を多電極アレイで記録した。
次に、刺激再構成が行われた。刺激再構成では、1組のスパイク列を前提として呈示される最も見込みのある刺激を判断するために標準的な最大尤度手法を使用する(Paninski、Pillopw、及びLewi、2007年にレビュー)。脳では刺激は再構成されないが、再構成は、人工器官方法を比較して各手法で可能な視力の回復のレベルの近似値が得られる有利な方法として機能する。
最後に、視運動タスクを使用して1組の挙動実験が行われた。結果を図10に示している。簡潔にいうと、動物にモニタ上で運動正弦波格子を呈示し、動物の眼の位置は、ISCAN PCI瞳孔/角膜反射追跡システム(ISCAN Corp.、マサチューセッツ州ウォーバーン)で記録した。後で記録を解析し、運動を刺激の運動と相関させた。図10Aの左のパネルは、基線ドリフト(刺激なし)を示している。盲目の動物は、盲人に対して観測されたものと類似の眼の位置のドリフトを示している。図10B(中心の欄)は、網膜変性を有し、かつ網膜神経節細胞内でチャンネルロドプシン−2も発現する本出願人の研究室で育てた商業的供給元の二重遺伝子組み換えマウスの結果を示している。これらのマウスには生刺激を示している。それによって標準的な光遺伝子の方法がモデル化される。図10C(右の欄)は、本出願人の人工器官のモデルの結果を示している。網膜変性を有し、かつ網膜神経節細胞内でチャンネルロドプシン−2も発現する研究室で育てた商業的供給元から購入した二重遺伝子組み換えには、生刺激の代わりに本出願人のエンコーダの出力を示している。図に示すように、追跡は、標準的な光遺伝子の方法をモデル化するマウスによって生成されはしなかったが、本出願人の人工器官をモデル化するマウスによって生成されている。画像が神経節細胞によって使用されるコードに変換された時に、動物は、コードを追跡することができるようになる。
図12の概略図は、例示的なエンコーダのための画像から光パルスへの変換を示している。図12Aは、例示的な動画(セントラルパークからの情景)を示している。図12Bは、予め処理した動画を示している。平均値強度及びコントラストをスケーリングして時空間変形の作動範囲と適合させる。この例示的な動画において、平均値又はコントラストの再スケーリングは不要であった。図12Bは、図12C〜図12Eの出力を生成する例示的なエンコーダの位置も示している。図12Cは、時空間変形段階の出力を示している。予め処理した動画を実施例細胞の時空的なカーネルに畳み込んでその非線形性を通過させて発火率を生成する。図12Dは、スパイク発生段階の出力を示している。時空間変形によって生成された発火率をスパイク発生器に通して一連の電子パルスを生成する。図12Eは、スパイク発生段階によって生成された出力に対応する光パルスを示している。
この実施例では、本出願人は、2つのサンプルエンコーダ、すなわち、マウスエンコーダ及びサルエンコーダに対するパラメータセットを提供する。パラメータセットは、空間パラメータ、時間パラメータ、及び非線形性(スプライン)パラメータから構成される。更に、時間関数を構成するのに使用される基底関数を提供する(見出し「時空間変形段階」の場所の節「エンコーダ」に詳述)。

弁別タスク法(図14)を使用して1組のサルエンコーダの性能を評価した。このタスクでは、実施例8(「弁別タスク内の性能」のセクションを参照されたい)内に概説した方法に従った。
図15は、エンコーダ+トランスデューサによって生成された神経節細胞応答が高忠実度でコード化された出力に追従することを示している。エンコーダを上述のように作成した。刺激、すなわち、赤ちゃんの顔面の画像をエンコーダを実行する処理デバイスに入力してコードを発生させる。コードをインタフェースに通して、盲目でありかつChR2を発現させる二重遺伝子組み換えマウスから切り取った網膜より上方に位置決めされたLEDを駆動する。電極により、網膜応答が記録される。図15Aは、光パルス及び対応する神経節細胞出力を示している。各対の列に対して、一番上の行は、光パルスの時間を示し、一方、一番下の行は、Ch2を表す神経節細胞によって生成された活動電位の時間を示している。次に、図15Bは、光パルスと活動電位間の1対1の対応を示す図15Aからの丸で囲った領域の拡大を示している。図が示すように、活動電位は、光パルスに、従って、高忠実度でエンコーダに追随した。
53才の男性患者は、黄斑変性を示している。EVA試験を行うと、得点は48点であり、視力は20/200で、低視力と診断される。視力は、着実に悪化しており、彼は完全に失明するのではないかと心配している。内科医は、網膜人工器官を使用する治療を患者と話し合い、本発明の網膜人工器官で患者を治療することを判断する。

Claims (15)
- エンコーダで、一連の画像に相当する画像データを受け取る段階と、
