本技術の実施形態は、システム、デバイス、分類器及び/又は方法を含むことができる。本明細書では、実施形態例を、添付図面、特に図1〜図13及び図15を参照して説明する。
チェーンストークス呼吸(CSR)は、通常、鼻腔内流量記録又は気道流量記録等、肺機能の直接測定により観察される周期的な呼吸の形態である。心臓系と肺系との結合に起因して、CSRを、酸素測定信号により交互の飽和低下期間及び再飽和期間として特定することも可能である。したがって、酸素測定信号は、チェーンストークス呼吸の解析のために利用可能な情報の情報源を提供することができる。この手法の利益は、血中酸素飽和度を非侵襲的に測定するオキシメータの使用を含むことができ、血中酸素飽和度は、被験者の健康状態の重要な決定要因である。酸素測定記録は、CSR、又は閉塞性睡眠時無呼吸(OSA)の状態等、酸素測定信号に同様に反映され得る他の呼吸異常の発生の兆候を提供することができる。これは、OSAからCSRを識別するために分類器の訓練中に考慮されることが好ましい。
導出された酸素測定(SpO2)信号104から、信号が−10%を下回る負の値から10%よりも大きな正の値になるアーティファクト期間の開始を特定することができる。導出された酸素測定信号は、アーティファクト期間の開始及び終了の表示を提供し、この表示は、最初の急な負のスパイクとそれに続く急激な正のスパイクとによってマークされている。アーティファクトを、アーティファクトの領域にわたって線形補間することによって除去することができる。
OSAの検出に酸素測定値を採用したが、それら検出方法は、CSRを検出する問題に移行可能ではない。CSRの存在は、換気調節における中枢の不安定性を示す。純粋なチェーンストークス呼吸では、流量は、中枢性無呼吸及び中枢性低呼吸に関連することが多い。閉塞性無呼吸とは対照的に、CSRにおける呼吸の再開は、通常、非常に緩やかであり、それにより再飽和の速度は低速になる。本技術は、平均再飽和期間と、本発明者らによる統計的解析によりCSRのみが10秒間よりも長い再飽和を明示することが分かったという事実とを利用することにより、OSAとCSRとの上記差を考慮する。
アーティファクトが特定されると、それらのアーティファクトをデータから除去することができる。残っているデータの信号をローパスフィルタリングしてフィルタリングされた信号を導出することもできる。信号を、まず望ましくなくかつ関心のない高周波数成分を除去するようにフィルタリングすることができる。例えば、使用されるフィルタは、矩形窓を有するフーリエ法を用いて設計されたデジタル有限インパルス応答(「FIR」)フィルタとすることができる。いくつかの実施形態では、フィルタは、0Hz〜0.1Hzの通過帯域と、0.1Hz〜0.125Hzの遷移帯域と、0.125Hzより高い阻止帯域とを有することができる。フィルタの項の数はサンプリング周波数によって変化する。時系列を点毎にフィルタベクトルで畳み込むことによって、信号をフィルタリングすることができる。
無呼吸/低呼吸と過呼吸との間の周期的交代により、飽和度低下及び再飽和が遅延するが呼吸と同期するようになることが多い。SpO2において観察される振動は複数の要因によって決まり、そのうちの1つは無呼吸の持続時間である。長い無呼吸は大きい飽和度低下に関連する。図2及び図3は、秒単位で測定された時間の関数として、OSAの平均飽和持続時間(図3)と比較したCSRの平均飽和持続時間(図2)の分布を示す。さまざまなCSR酸素測定パターンの観察により、閉塞性無呼吸の連続した期間中における酸素測定パターンの一過性の特徴とは対照的に、より高い規則性が見つけられる。フーリエ変換を用いて、スペクトル特徴は、0.083Hz〜0.03Hzに近い領域においてピークの存在を測定することができる。

連続ウェーブレット変換を適用することにより、信号の持続時間にわたって時間周波数情報を与えることも可能である。図6は、CSRがCSRエポック等の代表的なエポックE1Inにおいて発生している酸素飽和度を示し、ウェーブレット変換されたデータにより、多くの場合、2次元データにおいて見つけるか又は検出することができるリッジになる。ウェーブレットスペクトルを、使用されるウェーブレット変換のタイプに応じて、スケール領域(無次元)からフーリエ等価周波数(Hz)に変換することができる。図7は、ウェーブレット関数としてモルレー(Morlet)ウェーブレットを用いるフーリエ等価周波数の関数としてのグローバルウェーブレットスペクトルを示す。CSRの存在が強いエポックは、0.02Hzフーリエ等価領域の周囲にスペクトルピークを見つけることが多い。これは、図7に示すように、フーリエベースのスペクトルピークに十分に対応する。したがって、本技術のいくつかの実施形態では、グローバルウェーブレットスペクトルのピークを、酸素測定信号におけるCSRの解析のためのスペクトル特徴として用いることも可能である。
無呼吸/低呼吸と過呼吸との周期的交代により、飽和度低下及び再飽和が遅延するが呼吸と同期することになることが多い。この飽和の遅延(「DoS」)度応答は、複雑な心臓・呼吸ダイナミクスの結果である。いくつかの実施形態において、以下の方法のステップのうちのいくつか又はすべてを用いて、遅延をアルゴリズムによって抽出することができる。
1.流量信号を二乗する
2.二乗した流量信号をローパスフィルタリングする
3.結果の信号の平方根をとる
4.信号を酸素測定信号の等価周波数にダウンサンプリングして換気信号を得る
5.換気信号を絶対最大値によって正規化する
6.100%から酸素測定信号を減算する

任意選択で、上記ステップ1及びステップ3において流量信号に対し上述した二乗演算及び平方根演算を実行する代りとして、流量信号に対する絶対値演算を実施することができる。
本技術の分類器を訓練するように、事象特徴及びフーリエベースのスペクトル特徴を選択することができる。実施形態例に対する訓練を、臨床診断検査の90のEmbletta記録を用いて行った。

分類器が患者毎にCSRをいかによく識別するかを理解するために、分類器を、各エポックに対して2値出力(CSR又は非CSR)を単純に決定するように実装することができるが、そうする代わりに、各エポックセグメントに対して0と1との間の確率値を生成するように実装することができる。各導出された平均再飽和持続時間及びスペクトル特徴に対して、特徴空間のデータ点から決定境界まで垂直な距離を計算する。その後、この垂直距離を、以下のように確率が決定線からの距離の関数である確率値にマッピングする。

プログラムされたプロセッサ又は他の処理デバイスによって実装される場合のこうした分類器の1つの応用は、臨床医が、コンピュータ支援診断ツールとしてCSRの兆候がないか多数の患者をスクリーニングするのを可能にするというものである。こうした応用の一例を、在宅睡眠検査の環境で用いることができ、そこでは、睡眠医が、オキシメータを備えたApneaLink(商標)等の携帯型SDBスクリーニングデバイスを患者に支給する。好ましくは、睡眠データを、医師が後に解析するために一晩収集することができる。内科医又は臨床医によるこの解析を、オフラインで、すなわち1回又は複数回の睡眠セッションにおける測定デバイスの使用の後に行うことができる。例えば、分類器を具現化するアルゴリズムを、Somnologica(商標)(Emblaと言う名称の会社製)又はApneaLink(商標)(ResMed Limited製)等、睡眠検査解析ソフトウェア用のモジュールとして実装することができる。これにより、CSRの自動スコアリングを、酸素測定信号トレース又はグラフに対してマークすることができる。実施形態例を図13の概略図に示す。相補的特徴は、アルゴリズムによって計算される分類結果に基づいてレポートを自動に生成するモジュールである。その後、臨床医は、自身の意思決定プロセスを支援するために要約として該レポートを用いることができる。任意選択で、こうした分類器アルゴリズムを、SDBスクリーニング装置内で実施することにより、上述したようにCSの分類を有する表示メッセージ用のデータを生成することができる。
なお、出願当初の特許請求の範囲は以下の通りである。
(請求項1)
酸素測定信号によって測定される血中酸素飽和度からチェーンストークス呼吸の存在を示す方法であって、前記酸素測定信号からアーティファクト酸素測定期間を特定し除去して第2の信号を生成することと、プロセッサにより、前記第2の信号における連続的な再飽和の期間の平均長さを求め、該求められた平均長さの程度に基づいてチェーンストークス呼吸の陽性表示を生成することと、を含む方法。
(請求項2)
前記陽性表示は、前記求められた平均長さの程度が所定の閾値よりも大きい場合に生成される、請求項1に記載の方法。
(請求項3)
前記第2の信号をフィルタリングして高周波数を除去することを更に含む、請求項1に記載の方法。
(請求項4)
前記第2の信号に対して周波数解析を行って、前記酸素飽和度における振動の程度を求めることを更に含み、チェーンストークス呼吸の陽性表示はより長い周期時間にわたる振動に対して生成される、請求項3に記載の方法。
(請求項5)
前記第2の信号は、前記酸素飽和度における振動の程度を求めるようにフーリエ解析され、チェーンストークス呼吸の陽性表示は、約0.02Hzのフーリエベースのスペクトルにおけるピークに対して生成される、請求項4に記載の方法。

前記第2の信号は、前記酸素飽和度における振動の程度を求めるようにウェーブレット解析され、チェーンストークス呼吸の陽性表示は、より長い周期時間にわたる振動に対して生成される、請求項4に記載の方法。
(請求項7)
酸素測定信号及び換気流量信号によって測定される血中酸素飽和度からチェーンストークス呼吸の存在を示す方法であって、
プロセッサにより、無呼吸又は低呼吸のいずれか及び過呼吸を有する換気流量レベルデータと比較された血中酸素飽和度データの遅延を求めることと、
所定の閾値を上回る求められた遅延に対してチェーンストークス呼吸の陽性表示を生成することと、
を含む方法。
(請求項8)
酸素測定信号によって測定される血中酸素飽和度からチェーンストークス呼吸を識別するように分類器を訓練する方法であって、
睡眠ポリグラフデータを、各々が優勢な事象の指定されたクラスを有する重ならないエポックを取得するように事前分類することと、
プロセッサにより、酸素測定記録及び流量記録を、所定の長さの時間を有するデータの重ならないエポックにセグメント化することと、
プロセッサにより、前記エポックによって事象のチェーンストークス呼吸クラスと非チェーンストークス呼吸クラスとを識別するように、決定境界を形成することと、
を含む方法。

前記所定の長さの時間が5分間よりも長い、請求項8に記載の方法。

前記酸素測定信号は第1の分類器によって第1の閾値のセットと比較され、前記換気流量信号は、第2の分類器により第2の閾値のセットと比較される、請求項15に記載のデバイス。
(請求項18)
1つ又は複数のプログラムされたプロセッサによりチェーンストークス呼吸の発生を検出する、コンピュータにより実行される方法であって、
測定された血液ガス信号を表す血液ガスデータにアクセスすることと、
前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求めることと、
前記求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値との比較から、チェーンストークス呼吸の発生を検出することと、
を含む方法。

飽和度が変化する前記1つ又は複数の連続した期間は再飽和期間を含み、前記測定された血液ガス信号は酸素測定信号を含む、請求項18に記載の方法。

前記メモリに結合され、(a)前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求め、(b)前記求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値との比較から、チェーンストークス呼吸の発生を検出するように構成されたプロセッサと、
を具備する装置。

飽和度が変化する前記1つ又は複数の連続した期間は再飽和期間を含み、前記測定された血液ガス信号は酸素測定信号を含む、請求項26に記載の装置。

前記プロセッサは、アーティファクトデータを除去するように前記血液ガスデータを処理するように更に構成される、請求項31に記載の装置。

前記プロセッサに結合された、前記血液ガス信号を生成するオキシメータを更に具備する、請求項32に記載の装置。

酸素測定信号によって測定される血中酸素飽和度からチェーンストークス呼吸の存在を示す装置であって、
前記酸素測定信号からアーティファクト酸素測定期間を特定し除去して第2の信号を生成する手段と、
前記第2の信号における連続的な再飽和の期間の平均長さを求め、該求められた平均長さの程度に基づいてチェーンストークス呼吸の陽性表示を生成する手段と、
を具備する装置。
(請求項35)
酸素測定信号及び換気流量信号によって測定される血中酸素飽和度からチェーンストークス呼吸の存在を示す装置であって、
無呼吸又は低呼吸のいずれか及び過呼吸を有する換気流量レベルデータと比較される血中酸素飽和度データの遅延を求める手段と、
所定の閾値を上回る求められた遅延に対してチェーンストークス呼吸の陽性表示を生成する手段と、
を具備する装置。
(請求項36)
チェーンストークス呼吸の発生を検出する装置であって、
測定された血液ガス信号を表す血液ガスデータにアクセスする手段と、
前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求める手段と、
前記求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値と、の比較から、チェーンストークス呼吸の発生を検出する手段と、
を具備する装置。
Claims (24)
- 1つ又は複数のプログラムされたプロセッサによりチェーンストークス呼吸の発生を検出する、コンピュータにより実行されるデータ解析方法であって、
測定された血液ガス信号を表す血液ガスデータにアクセスするステップと、
前記血液ガスデータ内の急激な正のスパイクに先行する最初の急な負のスパイクを特定することにより、前記血液ガスデータ内のアーティファクトを決定するステップと、
前記血液ガスデータから前記アーティファクトを除去するステップと、
前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求めるステップと、
該求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値と、の比較から、チェーンストークス呼吸の発生を検出するステップと、
を含む方法。 - 飽和度が変化する前記1つ又は複数の連続した期間は再飽和期間を含み、前記測定された血液ガス信号は酸素測定信号を含む、請求項1に記載の方法。
- 求められた持続時間は平均期間長を含み、前記検出するステップは、該平均期間長が前記閾値を超える場合に発生を示す、請求項2または3に記載の方法。
- 前記閾値は判別関数を含む、請求項1乃至3のいずれか一項に記載の方法。
- 前記発生を検出するステップは、前記閾値からの距離を求めることと、該距離を更なる閾値と比較することと、を更に含む、請求項1乃至4のいずれか一項に記載の方法。
- 前記血液ガスデータの飽和度低下周期及び再飽和周期に対し所定周波数範囲においてピークの存在を判断するステップと、該判断された存在を判別関数と比較するステップと、を更に含む、請求項1乃至5のいずれか一項に記載の方法。
- 前記プロセッサは、アーティファクトデータの領域にわたって線形補間することにより該アーティファクトデータを除去する、請求項1乃至6のいずれか一項に記載の方法。
- 前記チェーンストークス呼吸の発生の検出に関連付られた信頼度を求めるための品質指標(QI)を決定するステップをさらに含む、請求項1乃至7のいずれか一項に記載の方法。
- チェーンストークス呼吸の発生を検出する装置であって、
測定された血液ガス信号を表す血液ガスデータのためのメモリと、
前記メモリに結合されたプロセッサであって、
(a)前記血液ガスデータ内の急激な正のスパイクに先行する最初の急な負のスパイクを特定することにより、前記血液ガスデータ内のアーティファクトを決定し、
(b)前記血液ガスデータから前記アーティファクトを除去し、
(c)前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求め、
(d)該求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値と、の比較から、チェーンストークス呼吸の発生を検出する、
ように構成されたプロセッサと、
を具備する装置。 - 飽和度が変化する前記1つ又は複数の連続した期間は再飽和期間を含み、前記測定された血液ガス信号は酸素測定信号を含む、請求項9に記載の装置。
- 前記求められた持続時間は平均期間長を含み、前記検出することは、該平均期間長が前記閾値を超える場合に発生を示す、請求項9または10に記載の装置。
- 前記閾値は判別関数を含む、請求項9乃至11のいずれか一項に記載の装置。
- 前記プロセッサは、更に判別関数からの距離を求め、該距離を更なる閾値と比較することにより、前記発生を検出するように構成される、請求項9乃至12のいずれか一項に記載の装置。
- 前記プロセッサは、前記血液ガスデータの飽和度低下周期及び再飽和周期に対し所定周波数範囲においてピークの存在を判断し、該判断された存在を判別関数と比較するように更に構成される、請求項9乃至13のいずれか一項に記載の装置。
- 前記プロセッサは、アーティファクトデータの領域にわたって線形補間することにより、該アーティファクトデータを除去する、請求項9乃至14のいずれか一項に記載の装置。
- 前記プロセッサに結合された、前記血液ガス信号を生成するオキシメータを更に具備する、請求項9乃至15のいずれか一項に記載の装置。
- 前記プロセッサは、所定の割合の閾値よりも前記測定された血液ガス信号が低下するサンプルの割合を求めることにより、品質指標(QI)を決定する、請求項9乃至16のいずれか一項に記載の装置。
- 前記所定の割合の閾値は10%である、請求項17に記載の装置。
- 前記品質指標は、前記測定された血液ガス信号が所定の割合の閾値よりも低下するサンプルの割合の関数である、請求項18に記載の装置。
- チェーンストークス呼吸の発生を検出する装置であって、
測定された血液ガス信号を表す血液ガスデータにアクセスする手段と、
前記血液ガスデータ内の急激な正のスパイクに先行する最初の急な負のスパイクを特定することにより、前記血液ガスデータ内のアーティファクトを決定する手段と、
前記血液ガスデータから前記アーティファクトを除去する手段と、
前記血液ガスデータから血液ガスの飽和度が変化する1つ又は複数の連続した期間の持続時間を求める手段と、
該求められた持続時間と、チェーンストークス呼吸に起因した飽和度変化と閉塞性睡眠時無呼吸に起因した飽和度変化とを区別するように導出された閾値と、の比較から、チェーンストークス呼吸の発生を検出する手段と、
を具備する装置。 - プロセッサが、前記チェーンストークス呼吸の発生の検出に関連付られた信頼度を求めるための品質指標(QI)を決定するように構成される、請求項20に記載の装置。
- 前記プロセッサは、所定の割合の閾値よりも前記測定された血液ガス信号が低下するサンプルの割合を求めることにより、前記品質指標を決定する、請求項21に記載の装置。
- 前記所定の割合は10%である、請求項22に記載の装置。
- 前記品質指標は、前記測定された血液ガス信号が前記所定の割合の閾値よりも低下する前記サンプルの割合の関数である、請求項22に記載の装置。
