Description
下記の用語は、当業者によってよく理解されると考えられるが、下記の定義は、本願明細書において開示された主題の説明を容易にするために記載される。
PAPは、ヒスチジン酸性ホスファターゼ スーパーファミリーのメンバーである。ヒスチジン酸性ホスファターゼは、活性部位内に位置する、高度に保存された、RHGXRXP(配列番号:1)モチーフを含む。PAPは、例えば、熱変性を含む方法、および当該タンパク質と、ジエチルピロカーボネート(DEPC)とのインキュベート(これは、全てのヒスチジン残基を化学的に修飾する)、または、活性部位のヒスチジン残基(His12)のアラニンへの変異(McTigueおよびVan Etten、1978;Ostaninら、1994)によって、触媒的に不活性にすることができる。その名称が意味する通り、PAPは、主に前立腺で発現するが、本願明細書において開示された主題は、PAPがまた、高いレベルで、小径DRGニューロンにおいて発現することを示す(実施例3−5、図1および図2A)。PAPは、分泌型(可溶性)タンパク質、または触媒性ホスファターゼ領域が細胞外に位置する、1型 膜貫通型(TM)タンパク質のいずれかとして発現する(図1)。分泌型は、広範に研究されてきており、前立腺癌についての、血液の診断マーカーとして使用される(OstrowskiおよびKuciel、1994;Roikoら、1990)。
実施例10−11は、PAPが、脳脊髄液への注入から3日間、マウスにおいて鎮痛薬として機能することを示す。図10Aおよび図10Bは、活性なウシ PAPの髄腔内注入が、マウスにおけるLPAに誘起された機械的感作および熱感作を阻害することを示す。図11A−11D、13および14A−14Cは、活性なヒトまたはウシのPAPの髄腔内注入が、鎮痛薬として機能し、マウスにおける熱感受性を減少させることを示す一方、図12Aおよび図12Bは、別のホスファターゼ(ウシ アルカリホスファターゼ(ALP))は、熱感受性または機械的感受性を減少させないことを示す。図17A−17B、18、および19は、ウシおよびヒトのPAPが、マウスにおける慢性的な機械的炎症痛および熱炎症痛を減少させることができることを示す。図20−23は、神経損傷によるアロディニアおよび痛覚過敏は、脊髄におけるPAP活性を増加させることによって、防ぐことができることを示す。例えば、神経部分損傷(SNI)手術によって引き起こされた神経因性疼痛は、損傷した肢における熱刺激に対する痛覚過敏をもたらす。ヒトまたはウシのPAPのいずれかの注入は、約3日間、SNI−損傷した肢における痛覚過敏を顕著に減少させ、損傷していない肢における鎮痛を生じさせる。SNI手術によって引き起こされた機械的感受性(アロディニア)はまた、hPAPまたはbPAPの注入後、約3日間、顕著に減少する。hPAPおよびbPAPは、損傷していない肢における機械的感受性を変化させない。前述のデータは、PAPの単回用量が、マウスがほぼ完全に回復する時点まで、慢性疼痛を治療することを示す。実施例12は、PAPが、PAP ノックアウトマウスにおけるアロディニアおよび痛覚過敏(hyperanalgesia)を阻害することを示す。
本願明細書において開示された主題の実施態様における使用のためのPAPタンパク質の調製物は、様々な方法を使用して調製できる。ヒト PAPは、シグマアルドリッチおよび他の業者から市販されている。PAPの生成は、一般的に、調製物が無菌であり、エンドトキシンフリーであり、ヒトへの使用について許容できることを保証するために、品質管理を必要とする。
PAPは、動物における疼痛および嚢胞性線維症の治療のために、様々な方法によって投与できる。当該PAP、その活性な変異体、断片もしくは誘導体、および/またはPAP修飾因子は、注入、経口投与、座剤、外科的な埋め込み型のポンプ、肺へのエアロゾル化、幹細胞、ウイルス遺伝子療法、またはネイキッドDNA遺伝子治療のうち1つ以上を介して投与できる。注入としては、いずれのタイプの注入(静脈内注入、硬膜外注入または髄腔内注入など、しかしこれらに限定されない)を挙げることができる。
いくつかの実施態様では、対象の遺伝子型は、疼痛および/または疼痛の薬物療法に対する対象の反応を予測するための有用な情報を特定するために使用できる。本願明細書で使用される用語「遺伝子型」は、生物の遺伝子の構造を意味する。遺伝子型の発現は、生物の表現型、すなわち、生物の身体的特徴を生み出す可能性がある。用語「表現型」は、生物および環境の遺伝子型の相互作用によって生じる、生物のいずれかの観察可能な特性を指す。表現型は、表現型の可変の発現度および浸透率を包含する可能性がある。代表的な表現型としては、可視の表現型、生理的な表現型、感受性の表現型、細胞の表現型、分子の表現型、およびそれらの組み合わせが挙げられるがこれらに限定されない。当該表現型は、疼痛反応および/または疼痛の薬物療法に対する反応に関連する可能性がある。特定の対象の遺伝子型は、参照遺伝子型、または1人以上の他の対象の遺伝子型と比較でき、現在の、または予測的な表現型に関連する有用な情報を与える。
本願明細書において開示された主題は、これから、さらに十分に、代表的な実施態様が示される下記の実施例への参照とともに記載される。しかし、本願明細書において開示された主題は、異なる形式で具体化でき、本願明細書に記載された実施態様に制限されるように解釈されるべきではない。むしろ、これらの実施態様は、この開示を詳細かつ完全にし、当業者に実施態様の範囲を完全に伝達するように提供される。
(方法)
(分子生物学)
ACPP−膜貫通型アイソフォーム(マウス PAP)の全長の発現コンストラクト(ジェンバンク(登録商標)受入番号 NM_207668からのnt 64−1317;配列番号:2)を、鋳型としてのC57BL/6 マウスの三叉神経のcDNA、およびPhusion ポリメラーゼ(New England BioLabs、米国、マサチューセッツ州、ベバリー)を使用して、RT−PCRの増幅によって作製した。PCR産物を、pcDNA3.1(インビトロジェン、米国、カリフォルニア州、カールズバッド)にクローン化し、完全に配列決定した。ACPP、分泌型変異体(ジェンバンク(登録商標)受入番号 NM_019807からのnt 1544−2625;配列番号:3)およびACPP、膜貫通型変異体(ジェンバンク(登録商標)受入番号 NM_207668からのnt 1497−2577;配列番号:4)の、アイソフォーム特異的な、in situ ハイブリダイゼーションのプローブを、PCR増幅によって、鋳型としてのC57BL/6 マウスのゲノムDNAおよびPhusion ポリメラーゼを使用して作製した。プローブをpBluescript−KS(Stratagene、米国、カリフォルニア州、ラ・ホーヤ)にクローン化し、完全に配列決定した。
in situ ハイブリダイゼーションを、ジゴキシゲニンで標識されたアンチセンスリボプローブおよびセンス(対照)リボプローブを使用して、Dongらが記載したように行った。
HEK 293細胞を、37℃、5% CO2で、ダルベッコ変法イーグル培地(DMEM)、高グルコース(1% ペニシリン、1% ストレプトマイシンおよび10% ウシ胎児血清を添加した)中で培養した。形質移入のため、ウェルあたり6×105の細胞を、6−ウェルのディッシュ中に播種した。細胞を、Lipofectamine Plus(インビトロジェン、米国、カリフォルニア州、カールズバッド)を使用して、0.5μg ACPP−膜貫通型アイソフォームおよび0.5μg ファルネシル化されたEGFP(EGFPf)で同時形質移入した。形質移入の24時間後に、試料を、全ての細胞が形質移入されたことを確かめるために、内在性EGFPfの蛍光についてイメージ化した。次いで、細胞をリン酸緩衝食塩水(PBS)中4% パラホルムアルデヒドで固定し、FRAP組織化学を使用して染色した。
脊椎動物が関与する全ての手順は、ノースカロライナ大学(チャペルヒル)およびオウル大学での、動物管理使用委員会(Institutional Animal Care and Use Committees)によって承認された。
FRAP/チアミン モノホスファターゼ(TMPase)組織化学を、基本的に、Shieldsら(2003)が記載したようにして、SilvermanおよびKruger(1988)によって提案された改変をして行なった。細胞または組織の切片を、40mM Trizma−マレイン酸(TM)バッファー(pH 5.6)で2度洗浄し、次いで、8%(w/v)ショ糖を含むTMバッファーで1度洗浄した。FRAPを有する細胞および軸索上に鉛を沈殿させるため、試料を、37℃で2時間、TMバッファー(8% ショ糖(w/v)、6mM チアミン一リン酸クロリド、2.4mM 硝酸鉛を含む)中でインキュベーションした。硝酸鉛は、使用の直前に、調製したてのものでなければならない。非特異的なバックグラウンド染色を減少させるため、試料を2% 酢酸で1分間、1回洗浄した。次いで、試料をTMバッファーで3回洗浄し、10秒間、1% 硫化ナトリウムで現像し、PBS(pH 7.4)で数回洗浄し、Gel/Mount(Biomeda社、米国、カリフォルニア州、フォスターシティ)に取り付けた。画像は、Zeiss Axioskopおよびオリンパス DP−71 カメラを使用して得た。

DRG中の、免疫蛍光組織化学とFRAP組織化学との間の重複を示すため、隣接する15μmの切片を、抗PAP抗体で染色し、かつFRAP組織化学のために染色した。同様の方法は、FRAPを、抗体およびレクチンマーカーと共局在化させるために、以前に使用されており、隣接する切片の使用により、共発現細胞の数を過小評価している(Doddら、1983;NagyおよびHunt、1982;SilvermanおよびKruger、1988;SilvermanおよびKruger、1990)。技術的な制限によって、免疫蛍光およびFRAP組織化学のための、同様のDRG切片の連続的な処理が阻まれた。
C57BL/6 オスマウス(2−3ヶ月齢)を、Jackson Laboratories(米国、メイン、バーハーバー)から、PAPタンパク質の注入に関連する全ての行動実験のために購入した。全てのマウスは、行動試験の1日前に、試験室、設備および実験者に順化させた。実験者は、行動試験の間、遺伝子型および薬物治療について知らされていなかった。
hPAP、bPAPおよび溶媒対照を、記載されたように(Fairbanks、2003)、無麻酔のマウスの腰部に注入した。
(組み換えPAPの調製)
C末端にトロンビン切断部位およびヘキサヒスチジン精製タグを含む、マウス PAP(分泌型アイソフォーム;ジェンバンク(登録商標)受入番号 NM_019807からのnt 64−1206;配列番号:5)バキュロウイルス発現コンストラクトを、実施例1に記載されたクローンおよび当該技術分野において標準的な手順を使用して作製した。組み換えマウス PAPを、個別払いのタンパク質精製コア施設を使用して精製した。トロンビン−ヘキサヒスチジン C末端タグを有する、hPAP(分泌型アイソフォーム;ジェンバンク(登録商標)受入番号 NM_001099からのnt 43−1200;配列番号:6)発現コンストラクトを同様に構築した。大量の組み換えhPAPタンパク質は、当該技術分野で公知の手順を使用して、このコンストラクトによって産生できた。組み換えhPAPタンパク質は、ヒトの臨床治験における薬物として有用であり、ヒトにおける髄腔内 hPAPの安全性を評価するために使用できる。
(PAPとしてのFRAPの分子同定)
約50年間、多くの小径DRGニューロンが、酸性ホスファターゼ(一般的に、FRAPまたはチアミン モノホスファターゼと呼ばれる)を含むことが知られてきた(CsillikおよびKnyihar−Csillik、1986;Knyihar−Csillik、1986;Colmant、1959)。FRAPは、脊髄のラミナ IIにおける非ペプチド作動性 DRGニューロンおよびその無髄軸索末端、ならびにペプチド作動性(CGRP+、物質 P+)ニューロンの一部を標識するために使用した(HuntおよびMantyh、2001;Carrら、1990)。マーカーとしてのFRAPの使用は、特定のレクチン(Griffonia simplicifolia Isolectin B4(IB4)など)もまた、非ペプチド作動性ニューロンを標識し、FRAPと共局在化することが見出された際に(SilvermanおよびKruger、1988)、衰退した。さらに、FRAPをコードする遺伝子は、明確に同定されたことがなかった。
(疼痛感覚の機構におけるPAPの役割)
マイクロアレイ解析は、坐骨神経の切除(Costiganら、2002)および神経因性疼痛モデルにおける神経損傷(Davis−Taber、2006)から3日後に、ラット DRGにおいて多数の遺伝子が上方制御または下方制御されることを示した。Costiganらが示したマイクロアレイのデータセット(Costiganらが添付図2として示した)を再分析し、全ての241の遺伝子を発現の倍率変化(fold change)によって並べた(なぜなら、当該遺伝子はアルファベット順で記載され、これは生物学的に無意味だからである)。再分析は、PAP mRNAが坐骨神経の切除後に、3.5倍下方制御され、遺伝子全体で2番目に多く下方制御されることを示した。表3を参照。同様に、PAP mRNAは、神経因性疼痛モデルにおいて最も激しく下方制御される遺伝子のうちの1つである(Davis−Taber、2006)。PAP発現は、神経因性疼痛のこれらの動物モデルにおいて下方制御されるため、神経因性疼痛は、PAP活性を回復することによって治療できる可能性がある。
(DRGニューロンはLPA受容体を発現する)
LPA受容体の発現をDRGニューロンで分析し、LPA受容体の情報伝達の調節におけるPAPについての役割を確かめた。これらの研究を開始した時点で、RT−PCRの実験は、LPA1は、DRGにおける唯一のLPA受容体であることを示した(Inoueら、2004;Renbackら、2000)。より詳細にこれらの受容体の発現を試験するため、in situ ハイブリダイゼーションを、アンチセンスのLPA1リボプローブおよびLPA3 リボプローブで行った。これらの実験は、LPA1が、全てのマウス DRGニューロンにおいて発現し、一方、LPA3は小径DRGニューロンの一部で発現することを明らかにした。LPA3が、Mrgprdと共発現しているかどうかを特定するため、二重蛍光(fluorescent double)のin situ ハイブリダイゼーションを、アンチセンスのMrgprdリボプローブおよびLPA3リボプローブで、以前に公表された方法(Zylkaら、2003)を使用して行った。当該実験は、全てのMrgprd+ ニューロンがLPA3を発現することを明らかにした。逆に、ほぼ全てのLPA3+ 細胞は、Mrgprdを発現していた(LPA3+ のみを発現する少数の細胞があったが)。要約すると、全てのDRGニューロンは、LPA1を発現しており、一方、Mrgprd+ ニューロンは、LPA1およびLPA3を共発現している。これらのデータは、全てのDRGニューロンが、LPA受容体を介して信号を送る可能性を有することを示唆する。Mrgprd+ ニューロンはまた、PAPを発現するため(表2を参照)、LPA受容体の情報伝達は、PAPタンパク質レベルを増加および減少させることによって調節できる。
(溶液中のPAP活性の測定のための定量的蛍光定量アッセイ)
PAP活性を定量化する方法は、再現性のある量の、活性なPAPタンパク質を培養細胞に添加し、または、下記の実験のために生きたマウスに注入できるようにするために必要だった。これを達成するため、2つのよく確立された方法を、PAP活性を測定するために試験した:1)パラ−ニトロフェニルリン酸エステル(p−NPP)の加水分解を使用する比色分析;および2)ジフルオロ−4−メチルウンベリフェリルリン酸エステル(DiFMUP)の加水分解を使用する蛍光定量アッセイ(EnzChek Acid Phosphatase kit(インビトロジェン(米国、カリフォルニア州、カールズバッド)製)のように市販されている)。直接的な比較に基づき、蛍光定量アッセイは、PAP活性の定量化のためには、p−NPPよりもずっと感度がよいことを特定した。精製されたウシ PAP(bPAP、分泌型アイソフォーム)およびマウス PAP(mPAP)についての代表的なデータは、図3に示される。PAP ホスファターゼ活性は、L−酒石酸塩(よく特徴付けられたPAP阻害剤)によって阻害される(OstrowskiおよびKuciel、1994)。重要なことには、このアッセイは、標準曲線を作成することによって、酵素活性(ユニット/mg タンパク質)を特定するために使用できる。従って、当該蛍光定量アッセイは、純粋なPAPタンパク質および細胞可溶化物からのPAPのホスファターゼ活性を定量化するために使用できる。
(ウシ PAPは、LPAを脱リン酸化し、LPAに誘起された情報伝達を阻害する)
以前の研究は、ヒト PAPが、試験管中でLPAを脱リン酸化することを見出した(HiroyamaおよびTakenawa、1999;Tanakaら、2004)。脱リン酸化されたLPAは、LPA受容体をもはや活性化できないと仮定されるが、これは、より生物学的に有意義な、細胞ベースのアッセイを使用して、正式に示されたことがなかった。PAPが、LPAを不活性化させることを証明するため、1μm LPAを、過剰な(0.2mU)ウシ PAPと、試験管中で1.5時間、37℃でインキュベーションした(図4中の「a」)。平行して、1μm LPA(bPAPは含まない)を含む第2の試験管を、1.5時間、37℃でインキュベーションした(図4中の「b」)。Rat1 細胞に、カルシウム感受性色素 Fura2−アセトキシメチル(AM)エステルを添加し(Dongら、2001)、(LPA+bPAP)を、これらの細胞に1分間かけて投与した(図4中の「a」を参照)。短い洗い流し時間の後、(LPA)を細胞に1分間かけて投与した(図4中の「b」を参照)。図4において見出すことができる通り、細胞内カルシウムレベルは、Rat1 細胞をLPA+bPAPで刺激した場合、あまり変化しなかった。しかし、細胞内カルシウムレベルは、これらの同様の細胞をLPAで刺激した場合に、劇的に変化した。これらのデータは、bPAPが、LPAを脱リン酸化し、かつ、不活性化することを明白に示す。このような不活性化は、LPAに誘起された情報伝達を効果的に阻害する。bPAPおよびhPAPは、市販されているため(シグマ、米国、ミズーリ州、セントルイス)、これらの純粋なタンパク質を、非遺伝的アプローチで、下記の実験においてPAP活性を増加させるために使用した。
(mPAPは、Rat1 線維芽細胞におけるLPAに誘起されたカルシウムの反応を急激に減少させる)
外因性bPAPは、LPAに誘起された情報伝達を遮断できるため、LPAに誘起された情報伝達を、PAPを過剰発現するRat1 細胞において急激に減少させることができる、と仮定した。この仮説を検証するため、黄色蛍光タンパク質 VenusをTM−PAPのC末端に融合することによって、蛍光で標識したmPAP コンストラクトを作製した(Nagaiら、2002)。これによって、PAP−Venusを形質移入された生細胞を直接的に視覚化することができた。PAP−Venusがホスファターゼ活性を有することを、FRAP組織化学を使用して、形質移入された細胞の染色によって示した。次いで、触媒的に活性な融合コンストラクトを、Rat1 細胞に形質移入し、細胞内カルシウムにおけるLPAに誘起された変化を、カルシウム感受性色素 Fura2−AMで測定した。図5において見出すことができる通り、LPAに誘起されたカルシウム反応の振幅および持続時間は、PAP−Venusを形質移入された細胞において、形質移入されていない細胞と比較して急激に減少する。これは、マウス PAPは、細胞ベースの条件におけるLPAに誘起された情報伝達を急激に減少させることを示す。これらの発見は、公表された結果と合わせて、マウス、乳牛およびヒトのPAPが、LPAを脱リン酸化することを示す。これは、PAPについての高度に保存された機能を示唆する。
(LPA反応は、PAPホスファターゼ活性に依存する)
PAPが、LPAの脱リン酸化によって、LPA情報伝達を調節するという仮説を裏付けるため、ホスファターゼデッドのマウス PAP発現コンストラクト(PAP変異体)を、活性部位残基ヒスチジン 12をアラニンに変異させ、次いで、蛍光タンパク質 VenusをC末端に融合させること(このPAP変異体を形質移入された細胞の視覚化を可能にするため)によって開発した。第1に、野生型 PAP−Venusと同程度に効率的に、PAP変異コンストラクトが発現し、膜に局在していることを確かめた。第2に、フッ化物抵抗性酸性ホスファターゼ(FRAP)組織化学を使用し、PAP変異コンストラクトにホスファターゼ活性が欠けていることを確かめた。次いで、Rat1 線維芽細胞を、PAPまたはPAP変異体で形質移入し、細胞にカルシウム感受性色素 Fura2−AMを添加した。次いで、細胞を100nM LPAで刺激した。PAPを形質移入された細胞について、形質移入されていない細胞と、同一の視野で、カルシウムの反応を比較した。図6Aおよび図6Cにおいて見出すことができる通り、LPAに誘起されたカルシウム反応は、PAPを形質移入された細胞で顕著に減少し、図5に示した結果を再現した。対照的に、LPAに誘起されたカルシウムの反応は、ホスファターゼデッドのPAP変異体を形質移入された細胞においては変化しなかった。図6Bおよび図6Dを参照。これらの結果は、図6Aおよび図6Cに示される、PAPを形質移入された細胞における減少したLPA反応が、PAPホスファターゼ活性に依存することを示す。
(疼痛治療のためのPAPの使用)
異常な量のLPAは、侵害受容システムを刺激し、神経因性疼痛(アロディニアおよび痛覚過敏を含む)を惹起する。図7を参照。神経因性疼痛は、LPA ホスファターゼ活性を増加させることによって治療できるだろう(図7)。本願明細書に記載されたデータは、PAPが、LPAを分解し、かつLPAに誘起された情報伝達を低下させることができることを示す。従って、PAP注入は、神経因性疼痛に関連する、いくつかの細胞型(ニューロン、ミクログリア細胞、シュワン細胞)におけるLPAに誘起された情報伝達を制御でき、さらなる効果(シュワン細胞におけるLPAに誘起された情報伝達の遮断および脱髄の遮断など)を有する。これらの可能性は、((Zylkaら、2005)において行われたように)電子顕微鏡観察を使用して坐骨神経をイメージングし、次いで、対照および処置された動物におけるミエリンの厚さを測定することによって検証できる。
(in vivoにおけるアロディニアおよび痛覚過敏のPAP阻害)
(用量の選択)
髄腔内(i.t.)への初回量100mUのPAPは、1μmolの蛍光定量用基質が、1分間あたり1Uのウシ PAPによって分解されるという発見に基づいて選択した。bPAPは、LPAと同程度に効率的に蛍光定量用基質を加水分解すると仮定すると、これは、1μmolの加水分解されたLPA/U bPAP/分の速度と等しい。LPA(1nmol、髄腔内)は、神経損傷後に見られるものと大きさが等しい、行動性アロディニアおよび痛覚過敏を引き起こした(Inoueら、2004)。同様の量のLPAが、神経損傷後に、血小板によって放出されると仮定すると、1分間に1nmolのLPAを分解するために、1mUのbPAPが必要とされるだろう。従って、100mUの用量のPAPは、100倍の過剰を意味し、CSFおよび脊髄実質における、拡散および希釈を説明する。
ウシ PAPタンパク質(bPAP)(シグマ(米国、ミズーリ州、セントルイス)から購入した)が、髄腔内に注入された場合に、無毒性であることを証明するため、かつ、bPAPが、LPAに誘起された情報伝達をin vivoで調節できることを証明するため、4群の野生型 C57BL/6 オスマウスに、下記を(髄腔内)注入した:1)媒体、2)20μU bPAP、3)1nmol LPA、または4)1nmol LPA+20μU bPAP。20μU bPAPは、37℃で、10分間インキュベーションした場合、1nmol LPAを脱リン酸化できることを見出した。従って、全ての試料を、37℃で10分間で、注入の前にインキュベーションした。
PAPについて疼痛に関連する機能を特定するため、ウシ bPAPを野生型マウスの脊髄に注入した。次いで、注入前および注入後5日まで、Hargreave法(後肢の放射加熱)を使用し、熱感受性についてこれらのマウスを試験し、電気的von Frey装置を使用し、機械的感受性についてこれらのマウスを試験した。bPAPを注入したマウスは、媒体を注入された対照と比較して、有意に増加した、熱刺激からの後肢の退避潜時を最大3日間示した。図11Aを参照し、点線を実線と比較すること。対照的に、機械的感受性においては、有意差がなかった(6時間の時点を除く)。図11Bを参照。図11Aおよび図11Bにおけるデータは、図10Aおよび図10Bから得て、hPAP行動性の結果との比較を容易にするために再度プロットしたことに注意すること。当該データは、bPAP注入が麻痺または嗜眠を引き起こさなかったという事実と合わせると、PAPが鎮痛性であり、麻痺性または催眠性ではないことを強く示唆する。さらに、ヒト hPAPの髄腔内注入もまた、注入後3日間、有意な熱鎮痛を引き起こすが、機械的鎮痛は引き起こさない。図11Cおよび図11Dを参照。hPAPの調製物を、注入前に0.9% 生理食塩水で透析し、そのため、この鎮痛効果は、タンパク質調製物中の小分子の混入物によるものではないようだった。さらに、ウシおよびヒトのPAPが、同様の鎮痛効果を、同様の持続時間で生じさせたという事実は、この効果がPAPに特異的であることをさらに示唆する。ウシ血清アルブミン(BSA)を注入した場合、鎮痛は認められなかった。図11Cおよび図11Dを参照。BSAは、分子量がPAPと似ているが、ホスファターゼ活性を欠いているタンパク質である。さらに、熱感受性または機械的感受性の変化は、異なる分泌型ホスファターゼ、すなわち、ウシ アルカリホスファターゼの髄腔内注入後には認められなかった。図12Aおよび図12Dを参照。
完全フロイントアジュバント(CFA)炎症痛モデルを、PAPが慢性的な機械的炎症痛および熱炎症痛を逆転できるかどうかを特定するために使用した。成体(2−3ヶ月齢)の、月齢をマッチさせ、体重をマッチさせたオスのC57BL/6 マウスのベースラインの機械的感受性を、無毛皮膚(右後肢)を電気的von Frey装置(IITC)で調査することによって定量化した。Hargreave法(これは、後肢の放射加熱(IITC Plantar Test Apparatus)を必要とする)を、同じ群のマウスにおける熱感受性を試験するために使用した(Hargreavesら、1988)。ベースラインの熱感受性および機械的感受性を、試験化合物の注入の前に特定した。次いで、マウスに、20μL CFAを注入した。1日後、全てのマウスは、CFAを注入された後肢において、深刻な熱過敏症および機械的過敏症を示した。次いで、マウスの半数に、1.3mg/mLのBSA(対照)を髄腔内注入し、残りの半分にはbPAPを注入し(図17Aおよび図17Bを参照)、または、半数に活性なhPAPを、半数に不活性なhPAPを注入した。図18および図19を参照。次いで、マウスを、注入から最大7日後まで、von FreyおよびHargreaves試験を使用して、機械的感受性および熱感受性について試験した。平均的な感受性をプロットし、統計検定(対応t検定)を、PAPが、過敏症(アロディニア;痛覚過敏)、感受性低下(鎮痛)を引き起こすかどうか、または効果がないかどうかを特定するために使用した。
PAPタンパク質の髄腔内注入が、神経因性疼痛の維持を遮断できる程度を特定した。神経因性疼痛の惹起の遮断と、神経因性疼痛の維持の遮断との間の主な相違は、PAPが神経部分損傷(SNI)手術に関連していつ注入されたか、ということと関連する。図8を参照。神経損傷前のPAPの注入によって、神経因性疼痛の惹起の遮断における有効性を測り、一方、損傷から4−5日後のPAPの注入によって、維持された疼痛の遮断における有効性を検証した。末梢神経損傷は、症候および薬物に対する反応性の観点から、ヒト 神経因性疼痛の最も近いモデルとなるため、SNI モデルを使用した(Abdiら、1998;LaBudaおよびLittle、2005)。
(PAP ノックアウトマウスにおけるアロディニアおよび痛覚過敏のPAP阻害)
PAPは、一般的に、前立腺においてのみ機能すると考えられていた(OstrowskiおよびKuciel、1994)。しかし、本願において開示されたデータは、PAPが侵害受容性ニューロンにおいても機能できることを示唆する。PAPについて疼痛に関連する機能をさらに評価するため、年齢をマッチさせた野生型C57BL/6 オスマウスおよびPAP−/− オスマウス(10世代にわたって、C57BL/6に戻し交配した)を、急性および慢性の疼痛行動アッセイを使用して評価した。機械的感受性の基準(電気的von Frey)または急激な有害な熱感受性のいくつかの異なる基準を使用したところ、遺伝子型の間の有意差は見出されなかった。表4を参照。
(アデノシンのPAP生成)
PAPの抗侵害受容性効果は、触媒活性を必要とする。いずれかの1つの理論に結び付けられることなく、これは、PAPが、脱リン酸化を介して、脊髄中の侵害受容性神経伝達を制御する分子を生成することを示唆する。PAPおよびTMPaseは、多くの異なる基質を脱リン酸化できる(Dziembor−Gryszkiewiczら、1978;SanyalおよびRustioni、1974;SilvermanおよびKruger、1988b;Vihko、1978b)。1つの可能性がある基質はAMPである。AMPの脱リン酸化は、脊髄片における侵害受容性神経伝達を阻害し、哺乳類におけるよく研究された鎮痛特性を有する分子である、アデノシンを生成する(LiおよびPerl、1994;LiuおよびSalter、2005;Post、1984;Sawynok、2006)。
(PAPの小分子修飾因子を同定するための高処理スクリーニング)
高処理生化学的アッセイを、PAP活性を調節する薬物を同定するために開発した。このアッセイは、純粋なhPAPタンパク質、および蛍光定量的PAP基質(ジフルオロ−4−メチルウンベリフェリルリン酸エステル(DiFMUP);インビトロジェンから市販されている)の使用に依存する。hPAPによるDiFMUPの脱リン酸化を、蛍光定量的マイクロプレートリーダー(FLIPRまたはFlexstationなど)を使用してモニターした。第1に、hPAPタンパク質およびDiFMUP基質の適切な濃度を、96ウェルプレートにおける使用のために同定し、次いで、2,000の化合物(NCI Diversity Set)を、hPAPの反応速度を促進(活性化剤)または抑制(阻害剤)させる小分子を同定するためにスクリーニングした。このスクリーニングからのデータを使用し、Z因子は、0.86であると算出された(この数字は、0−1の範囲に及ぶ可能性があり、0.5は有用なHTSについてのカットオフである。0.86は、非常に高い値であり、当該アッセイの再現性が高く、大きなシグナル・ノイズ比を有することを示す)(Zhangら、1999)。当該スクリーニングから、6つの候補hPAP阻害剤および3つの候補hPAP活性化剤を同定した。未使用の化合物(NCIから注文した)を入手し、用量−反応実験を行った。これらの実験によって、全ての9つの候補は実際に活性化剤または阻害剤であることを確認した。これらの化合物がhPAPに対して特異的である程度を、hPAP、bPAP、ジャガイモ酸性ホスファターゼおよびウシ アルカリホスファターゼへのこれらの化合物の効果を試験することによって評価した。従って、hPAPの活性化剤および阻害剤は、再現性のある、小型化された、経済的なHTSを使用して、同定することができる。当該アッセイは、PAPのさらなる小分子修飾因子を同定するために有用である。

Claims (19)
- 動物における疼痛の治療における使用のための組成物であって、治療的有効量のPAP、またはその酵素的に活性な変異体、断片もしくは誘導体、を含み、
前記酵素的に活性な変異体、断片もしくは誘導体は、下記からなる群から選択される1つ以上の修飾を含むPAPである、組成物:アミノ酸付加、アミノ酸欠損、保存的アミノ酸置換、非天然アミノ酸置換、D−またはD,L−ラセミ混合物異性体型アミノ酸置換、アミノ酸化学置換、カルボキシ末端修飾またはアミノ末端修飾、生体適合性の分子への結合、および生体適合性の支持構造への結合。 - 前記疼痛は、慢性疼痛、慢性炎症痛、神経因性疼痛、慢性神経因性疼痛、アロディニア、痛覚過敏、手術の後の疼痛、神経損傷、外傷、組織損傷、炎症、癌、ウイルス感染、帯状疱疹、糖尿病性神経障害、変形性関節症、熱傷、関節痛、腰痛、内臓痛、片頭痛、群発性頭痛、頭痛、線維筋痛症または分娩に関連する疼痛である請求項1記載の組成物。
- 前記疼痛は、過剰量のリゾホスファチジン酸によって特徴付けられる請求項1記載の組成物。
- 前記疼痛は、アデノシンまたはアデノシン受容体の機能における欠損によって特徴付けられる請求項1記載の組成物。
- 前記動物はヒトである請求項1記載の組成物。
- 前記PAPは、ヒト PAP、ウシ PAP、ラット PAPおよびマウス PAP、ならびにそれらの酵素的に活性な断片、変異体および誘導体からなる群から選択される請求項1記載の組成物。
- 前記PAPは、組み換えPAPである請求項1記載の組成物。
- 前記PAP、またはその酵素的に活性な変異体、断片もしくは誘導体は、注入、経口投与、外科的な埋め込み型のポンプ、幹細胞、ウイルス遺伝子療法またはネイキッドDNA遺伝子治療を介して投与される請求項1記載の組成物。
- 前記投与は、静脈内注入、硬膜外注入、または髄腔内注入を介する請求項8記載の組成物。
- 前記投与は、PAPを発現している胚性幹細胞の髄腔内注入を介する請求項9記載の組成物。
- 前記投与は、3日に約1回の髄腔内注入による請求項9記載の組成物。
- 前記投与は、アデノシン、アデノシン一リン酸(AMP)、AMP類似体、アデノシンキナーゼ阻害剤、5’−アミノ−5’−デオキシアデノシン、5−ヨードツベルシジン、アデノシンデアミナーゼ阻害剤、2’−デオキシコホルマイシン、ヌクレオシド輸送体阻害剤、ジピリダモールのうち1つ以上と組み合わされる請求項8記載の組成物。
- 前記投与は、1つ以上の鎮痛薬と組み合わされる請求項8記載の組成物。
- 前記鎮痛薬はオピエートである請求項13記載の組成物。
- 前記投与は、PAPまたはその酵素的に活性な変異体、断片もしくは誘導体をコードする核酸配列を含む、レトロウイルス、アデノウイルスまたはアデノ随伴ウイルスのベクター移入カセットを使用したウイルス遺伝子療法を介している請求項8記載の組成物。
- 動物における嚢胞性線維症の治療における使用のための組成物であって、治療的有効量のPAP、またはその酵素的に活性な変異体、断片もしくは誘導体を含み、
前記酵素的に活性な変異体、断片もしくは誘導体は、下記からなる群から選択される1つ以上の修飾を含むPAPである、組成物:アミノ酸付加、アミノ酸欠損、保存的アミノ酸置換、非天然アミノ酸置換、D−またはD,L−ラセミ混合物異性体型アミノ酸置換、アミノ酸化学置換、カルボキシ末端修飾またはアミノ末端修飾、生体適合性の分子への結合、および生体適合性の支持構造への結合。 - 前記PAP、またはその酵素的に活性な変異体、断片もしくは誘導体は、肺へのエアロゾル化または肺中への経肺投与によって投与される請求項16記載の組成物。
- 動物の肺におけるアデノシンのレベルを増加させる組成物であって、治療的有効量のPAP、またはその酵素的に活性な変異体、断片もしくは誘導体を含み、
前記酵素的に活性な変異体、断片もしくは誘導体は、下記からなる群から選択される1つ以上の修飾を含む、組成物:アミノ酸付加、アミノ酸欠損、保存的アミノ酸置換、非天然アミノ酸置換、D−またはD,L−ラセミ混合物異性体型アミノ酸置換、アミノ酸化学置換、カルボキシ末端修飾またはアミノ末端修飾、生体適合性の分子への結合、および生体適合性の支持構造への結合。 - 動物における疼痛の治療における使用のためのキットであって、治療的有効量のPAP、またはその酵素的に活性な変異体、断片もしくは誘導体を含む、組成物、および局所組織へのPAPの送達のための外科的な埋め込み型のポンプ装置を含み、
前記酵素的に活性な変異体、断片もしくは誘導体は、下記からなる群から選択される1つ以上の修飾を含むPAPである、キット:アミノ酸付加、アミノ酸欠損、保存的アミノ酸置換、非天然アミノ酸置換、D−またはD,L−ラセミ混合物異性体型アミノ酸置換、アミノ酸化学置換、カルボキシ末端修飾またはアミノ末端修飾、生体適合性の分子への結合、および生体適合性の支持構造への結合。
