Description
The present invention relates to a method for producing an implant having a coating layer with excellent surface properties, and further to an implant produced thereby.
It is known that various diseases occur when a stenosis occurs in a blood vessel, which is a flow path for circulating blood in the body, and the circulation of blood is delayed. It is known that when a coronary artery that supplies blood to the heart, which is the source of blood circulation, is stenotic, it can cause serious diseases such as angina pectoris and myocardial infarction, resulting in an extremely high risk of death. Yes. One of the methods for treating such a stenotic portion of a blood vessel is angioplasty (PTA, PTCA) in which the stenotic portion is expanded using a balloon catheter, and is minimally invasive without requiring a thoracotomy such as a bypass operation. It is widely used because it is a therapy. However, in the case of angioplasty, restenosis occurs in a stenosis portion expanded at a frequency of about 40%, which has been pointed out as a major problem. As a treatment method for reducing the frequency of occurrence of restenosis (restenosis rate), stent placement is widely performed instead of angioplasty.
A stent is a medical device that is placed for the purpose of expanding a stenotic site and maintaining the state when a living body lumen such as a blood vessel, a bile duct, or a urethra is stenotic. Generally, a stent is composed of a metal, a polymer, or a composite thereof, and is most commonly composed of a metal such as SUS steel, a Co—Cr alloy, or a Ni—Ti alloy.
The expansion mechanism of the stent is roughly classified into a self-expandable type that expands due to shape memory property and superelasticity of the stent itself and a balloon expandable type that expands by a balloon catheter. A balloon expansion type is mainly used for treatment of a coronary artery stenosis.
When treating a stenotic portion of a coronary artery with a balloon expandable stent, the stent is inserted while being held by a balloon catheter, and is expanded by expanding the balloon. The rate of restenosis after such stent placement is about 20% to 30%, which is significantly lower than that after angioplasty using only a balloon catheter, but restenosis still occurs frequently. Has occurred.
The cause of this is thought to be that the placement of the stent causes physical damage to the stenosis, and excessive neointimal thickening that occurs as a repair reaction of this damage results in restenosis after the stent placement. The neointimal thickening is caused by proliferation of smooth muscle cells in the vascular media, migration of the proliferated smooth muscle cells to the intima, migration of T cells and macrophages to the intima, and the like.
In order to reduce restenosis that occurs after treatment of a stenosis, it has been studied to hold various resin materials, drugs, and the like on the surface of an implant such as a stent placed in the body. In particular, in recent years, it has been studied to hold a plurality of materials on the surface of an implant. However, even in such a case, the present invention is excellent in surface properties and exfoliation without requiring a large-scale device. It is an object of the present invention to provide a method for manufacturing an implant with low risk.
As a result of intensive investigations in view of the above problems, the present inventor is a method for producing an implant formed by applying and drying a coating liquid containing a polymer material and a solvent , wherein the polymer material is highly biodegradable. It is a lactic acid-glycolic acid copolymer that is a molecule, the solvent is a dehydrated amphiphilic solvent, and the amphiphilic solvent is at least one solvent selected from acetone and tetrahydrofuran. The manufacturing method of the implant characterized by this was provided. According to this, it is possible to manufacture an implant having excellent surface properties and less risk of peeling without requiring a large-scale device.
In addition, the present invention provides a method for manufacturing the implant, wherein the coating liquid includes a polymer material and one or more drugs.
Further, the present invention provides a method for manufacturing the implant, wherein the coating liquid is configured to include two or more drugs.
Further, the present invention provides a method for producing the implant, wherein the drug contains tacrolimus.
Further, the combination of the polymer material and one or more drugs, or the combination of two or more drugs includes a substance having an octanol / water partition coefficient (LogPow) of 0.89 or more and a substance of -0.32 or less. The method for manufacturing the implant is provided.
In addition, the present invention provides a method for producing the implant, wherein the coating liquid is once applied and then the solvent is sprayed to smooth the surface of the coating layer.
Further, the present invention provides a method for producing the implant, wherein the solvent contained in the coating liquid and the solvent sprayed after the coating liquid is applied have the same composition.
According to the present invention, it is possible to form a coating layer of an implant that is becoming complicated with good surface properties without requiring a large-scale device. In particular, it is possible to form an implant having a coating layer with a smooth surface, high uniformity, and reduced occurrence of peeling during coating.
Hereinafter, the manufacturing method of the implant concerning the present invention and the implant manufactured by this are explained based on an embodiment. Here, the term “implant” refers to a member placed in the body for the purpose of treatment, such as a pacemaker, a stent, an artificial tooth root, or an artificial bone. For example, a stent, which is an embodiment of an implant, is a member that is formed in a substantially tubular body and can be placed in a blood vessel or the like that can extend radially outward of the tubular body.
1. Implant base material As a material forming the implant base material, for example, stainless steel, Ni-Ti alloy, Cu-Al-Mn alloy, tantalum, Co-Cr alloy, iridium, iridium oxide, niobium metal materials, ceramics, An inorganic material such as hydroxyapatite can be preferably used. In particular, when forming a stent, a cylindrical tube made of these materials can be prepared and cut into a stent design by laser cutting or the like. Further, electrolytic polishing or the like may be performed after laser cutting.
In addition to inorganic materials, the materials for forming the implant base material are polymers such as polyolefins, polyolefin elastomers, polyamides, polyamide elastomers, polyurethanes, polyurethane elastomers, polyesters, polyester elastomers, polyimides, polyamide imides, polyether ether ketones, etc. It is possible to use materials. As a method for producing an implant base material using these polymer materials, a processing method suitable for each material can be arbitrarily selected.
2. Solvent In order to coat an arbitrary substance on the surface of the implant base material, a coating solution in which the substance is dispersed in a solvent is usually used. Here, the coating liquid is not necessarily in a uniformly dissolved state, and a suspension or the like can be suitably used in addition to the dissolving liquid. On the other hand, examples of optional substances include substances that are included to protect the implant base material from contact with body fluids, temperature, humidity, friction, and the like, or substances that are included to improve or maintain the operation and performance of the implant. For example, a structural material (for example, a polymer material) forming a layer, an antibacterial material, a drug, and the like can be given. These substances are required to be included in the coating layer in a controlled dispersion state in a necessary amount in a necessary portion of the implant, and further to be arranged with good surface properties.
In the method for producing an implant of the present invention, the coating liquid to be used needs to contain an amphiphilic solvent. As the coating solution contains an amphiphilic solvent, the coating layer can be formed to cope with the substances held on the surface of implant bases that are becoming more complex, and to meet the advanced control required. It becomes possible to do. In particular, it is effective when it is necessary to immobilize a hydrophilic substance and a hydrophobic substance in the same layer on the surface of the implant base material. In addition, when immobilizing a hydrophilic substance and a hydrophobic substance in the same layer in this way, instead of an amphiphilic solvent, a hydrophilic solvent and a hydrophobic solvent that are soluble in each other are mixed. However, in this case, when applying and drying the coating liquid, the volatilization of one of the solvents proceeds due to the difference in the volatilization rate of the solvent, and one of the substances (solute) in the coating liquid first. Precipitation proceeds and finally a coating layer in which the distribution of the substance (solute) in the thickness direction is non-uniform is obtained. In a severe case, a polymer material that often functions as a structural material in the coating layer is deposited first, and a drug-rich layer is formed on the outside, which may cause peeling and cannot be used.
In the present invention, the amphiphilic solvent to be used needs to be dehydrated. When dehydration is not performed, moisture is absorbed due to the characteristics of the amphiphilic solvent and the coating liquid contains moisture. When drying, the surface of the coating layer is roughened, or The distribution of the substance held on the surface becomes non-uniform. In particular, as the amphiphilic solvent used in the coating liquid, a solvent having a relatively low boiling point is often selected because of its ease of drying, and the water contained in the drying remains until the end, and the coating layer is formed. Use a rough surface. In the present invention, the amount of water remaining in the coating solution is preferably 100 ppm or less, and more preferably 50 ppm or less, for the reasons described above. In addition, it is preferable that the amount of water is small from the viewpoint of surface properties, but on the other hand, it is preferably 10 ppm or more from the viewpoint of difficulty in preparation.
In addition, even if an amphiphilic solvent that has been dehydrated is used when preparing the coating solution, moisture in the air may be taken in before use or during coating or drying. It is preferable to keep the storage environment and work environment at low humidity. Also, until actual use, the solvent and coating solution are charged with a substance that promotes dehydration, such as molecular sieve, to remove moisture. It is preferable.
On the other hand, these solvents are preferably removed as much as possible in the finished implant, unless it is necessary to leave solvents to improve the function and the like of the implant. Therefore, it is preferable to remove the solvent promptly or to make it harmless by any method by applying an appropriate treatment after coating. Examples of the method for removing the solvent in the coating solution include a method for forcibly removing volatilization by adding natural drying or heat. Examples of the method for detoxifying the solvent include decomposition by energy such as heat and light. However, detoxification by decomposition may leave unnecessary substances in the coating layer, and it is generally preferable to remove the solvent. Moreover, vibration, rotation, pressure reduction, etc. are mentioned as another solvent removal method, You may combine these two or more.
Further, in order to protect the implant base material from body fluid, humidity, and friction, the coating layer is required to have a certain thickness or a surface property with less unevenness. For example, if the coating layer is not thick, body fluid and moisture will penetrate and easily reach the implant and its substrate, which may cause mechanical problems and rust. Further, when the surface of the coating layer is uneven and uneven, external pressure and friction applied to the coating layer and the implant base material are also unevenly applied, which may damage the coating layer. In order to obtain a surface property with a certain thickness and less unevenness in this way, the coating solution does not repel, the coating solution spreads sufficiently, and the substance is sufficiently soluble in the solvent. In addition, it is necessary that the uniformity of dispersion is high, and these depend greatly on the volatility, viscosity, surface tension and the like of the solvent.
The progress of drying or volatilization depends on the boiling point and vapor pressure of the solvent. When it is desired to obtain a coating layer having a certain thickness or more, it is also required that the solvent does not easily volatilize from the coating solution. When a solvent that readily and quickly volatilizes from the coating layer is used, the thickness of the coating layer may be reduced. When the solvent does not easily volatilize easily, the coating solution is dried little by little from the outside, so that a coating layer having a layer thickness is easier to form than the quick-drying solvent. Therefore, the boiling point of the solvent is preferably higher than the air temperature or the working environment temperature, since the solvent is easily volatilized in the coating operation at normal temperature and does not hinder the operation. However, from the viewpoint of efficiency, it is preferable to use a highly volatile solvent, and it is preferable that the solvent can be volatilized at a temperature at which the physical properties of the coating layer and the substance contained therein are not changed. For the above reasons, the boiling point of the solvent used is preferably 40 ° C. or higher and 66 ° C. or lower. In addition, it is preferable to perform the coating operation and before and after (drying operation, etc.) under manageable conditions. The conditions that can be managed here are preferably environments in which the solvent or the coating liquid can be sealed or handled under conditions close thereto.
In order to easily form a coating layer with less unevenness on the implant, the viscosity of the coating liquid or the solvent constituting the coating liquid has an important influence. When the viscosity of the coating liquid is too low, unevenness occurs when a part of the coating layer becomes thicker or thinner. If the viscosity of the coating solution is too high, the surface of the implant coating layer is not smooth and uniform, and defects in the coating layer such as pinholes are likely to occur. The viscosity of the coating liquid depends on the amount of the substance dissolved in the liquid, but depends on the viscosity of the solvent when the amount of the substance to be dissolved is very small. In addition, even if the solvent in the coating solution is slightly volatilized after coating onto the implant, there is a possibility that uneven viscosity will occur. Therefore, coating using a solvent having a too low viscosity may cause a problem during operation. Therefore, the viscosity of the solvent used is preferably 3.2 × 10 −4 Pa · s or more and 5.6 × 10 −4 Pa · s or less for the above reasons.
In order to adjust the thickness of the coating layer of the implant, the surface tension of the coating liquid or the solvent itself used can be used. If the surface tension of the coating liquid is too low, the coating layer thickness after drying may be thin, and the coating layer may be broken due to insufficient strength. Further, if the surface tension of the coating liquid is too high, the coating liquid may not flow over the entire area to be coated, resulting in structural problems such as unevenness and pinholes. Therefore, for the above reasons, the surface tension of the solvent used is preferably 2.37 × 10 −2 Pa · m or more and 2.81 × 10 −2 Pa · m or less.
Examples of the amphiphilic solvent include various alcohols such as methanol, ethanol, isobutanol, isopropanol, and tert-butanol, dimethylformamide, dimethyl sulfoxide, tetrahydrofuran, acetone, methyl ethyl ketone, polyethylene glycol, propylene glycol, and ethyl acetate. Can be mentioned. Among these, acetone, tetrahydrofuran, ethyl acetate, methyl ethyl ketone, and ethanol are preferable, and acetone, tetrahydrofuran, and ethyl acetate are particularly preferable because they satisfy the above various characteristics and can be obtained relatively inexpensively. In particular, when polylactic acid such as lactic acid-glycolic acid copolymer (hereinafter referred to as PLGA) or tacrolimus is used as the solute, the solvent is preferably acetone or tetrahydrofuran. Alternatively, as an amphiphilic solvent, a substance having an octanol / water partition coefficient (LogPow) of 0.89 or more, a solvent capable of dissolving a substance of −0.32 or less, and a substance of 2.50 or more , A solvent capable of dissolving a substance of −0.50 or less is preferable.
Besides these, solvents that can be used for coating various implants include chloroform, dichloromethane, acetonitrile, chlorobenzene, cyclohexane, 1,2-dichloroethene, 1,2-dimethoxyethane, N, N-dimethyl. Acetamide, N, N-dimethylformamide, 1,4-dioxane, 2-ethoxyethanol, ethylene glycol, formamide, hexane, methanol, 2-methoxyethanol, methylbutylketone, methylcyclohexane, N-methylpyrrolidone, nitromethane, pyridine, Sulfolane, tetralin, toluene, 1,1,2-trichloroethene, xylene, acetic acid, anisole, 1-butanol, 2-butanol, n-butyl acetate, t-butyl methyl ether, cumene, diene Methyl sulfoxide, diethyl ether, ethyl formate, formic acid, heptane, isobutyl acetate, isopropyl acetate, methyl acetate, 3-methyl-1-butanol, methyl isobutyl ketone, 2-methyl-1-propanol, pentane, 1-pentanol, 1 -Propanol, 2-propanol, propyl acetate and the like.
3. Structural material that forms the coating layer Implants are taken into the body and play a role in supporting life activity and movement. Therefore, in particular, when a structural material such as a polymer is used without forming the coating layer only with a drug, it is preferable to use a biocompatible substance for the structural material. In the case of a coating layer that is required to disappear in vivo, it is more preferable to use a biodegradable substance for the coating layer. Biodegradable substances include substances composed of metals and polymers.
When the biodegradable substance is a biodegradable polymer, for example, a monomer component selected from lactic acid, glycolic acid, γ-butyrolactone, δ-valerolactone, ε-caprolactone, tetramethylene carbonate, dioxanone, etc. Among them, a polymer containing one or several kinds as a constituent component can be mentioned. In particular, considering the biocompatibility of the biodegradable polymer itself and the safety of the degradation products produced by biodegradation, polylactic acid composed only of lactic acid or lactic acid-glycolic acid composed of lactic acid and glycolic acid. It is preferably one of polymers.
Lactic acid is known to have two types of optical isomers, which are referred to as D-lactic acid and L-lactic acid, respectively. Therefore, polylactic acid includes poly-D-lactic acid composed only of D-lactic acid, poly-L-lactic acid composed solely of L-lactic acid, and poly-D composed of D-lactic acid and L-lactic acid. , L-lactic acid (poly-D-lactic acid and poly-L-lactic acid are crystalline polymers, and poly-D, L-lactic acid is an amorphous polymer). In particular, in the present invention, it is preferable that both D-lactic acid and L-lactic acid are contained, and it is further preferred that they are contained in equimol%.

5. Coating layer Implants such as stents are configured to include a structural material such as a polymer material and one or more drugs on at least a part of the surface of the base material, or include two or more drugs. Is preferable (in other words, the coating solution is preferably configured to contain such a substance). Moreover, it is preferable that the coating layer is arrange | positioned at the substantially whole surface of the outer surface of a base material, an inner surface, and a side surface. For example, in the case of a stent substrate, by providing a coating layer on almost the entire surface, there is an advantage that cracking of the coating layer and peeling from the surface of the stent substrate are less likely to occur during stent expansion.
On the other hand, the drug / biodegradable polymer weight ratio in the coating layer is preferably 0.50 or more and 1.60 or less. If it is less than 0.50, it is difficult to efficiently increase the amount of drug retained, which is not preferable. On the other hand, when it is larger than 1.60, it is not preferable because there is a high possibility that the coating layer is cracked or peeled off as the stent expands.
In the method for producing an implant of the present invention, the combination of the polymer material and one or more drugs in the coating solution, or the combination of two or more drugs is 0.89 or more in terms of octanol / water partition coefficient (LogPow). And a substance of −0.32 or less, more preferably 2.50 or more and −0.50 or less of a substance. Originally, in such a system, the coating liquid is adjusted by mixing the solvent. However, in the present invention, the coating liquid can be adjusted with a single solvent even in such a system. As a result, when the coating liquid is applied to the surface of the implant and dried, it is possible to reduce deterioration of the surface property of the coating layer due to drying of the non-uniform solvent, and further, peeling of the coating layer caused thereby. .
OECD GUIDELINE FOR THE TESTING OF CHEMICALS lO7, Adopted by the Council on 27th July 1995, Partition Coefficient(n-octanol/water),Shake FIask Methodに記載の方法により個々の薬物について測定した値をいい、logPowの対数の底は10である。 In addition, logPow is an index representing hydrophobicity / hydrophilicity, which is also described in the product safety data sheet, etc .. / water), the value measured for each drug by the method described in Shake FIask Method. The logarithmic base of logPow is 10. More specifically, the drug is distributed to the water layer and the n-octanol layer, and the drug concentration in each layer is determined by an appropriate method. The quantification of the drug is not particularly limited as long as it is a method for confirming the quantitative property by confirming the linearity of a calibration curve, but usually an absorbance method, a gas chromatography method or a high performance liquid chromatography method is used. When measuring a drug having no UV absorption, other methods such as a titration method may be used as long as quantitativeness can be confirmed. Further, when the drug is a dissociable substance, logPow is measured by suppressing dissociation according to this guideline. That is, logPow is measured in a free acid or free base state using a buffer solution having an appropriate pH as the aqueous layer. When the drug is an acidic drug, a buffer solution having a pH one or more lower than the pKa value of the drug is used. When the drug is a basic drug, a buffer solution having a pH one or more higher than the pKa value of the drug is used. In this guideline, the logPow measurement range is usually −2 to 4, but the sensitivity of quantitative analysis is increased by using a high-sensitivity analysis method such as high performance liquid chromatography, and the logPow measurement range is −2 to 5 can be set.
On the other hand, multiple coating layers are provided for the purpose of further increasing the amount of drug retained, imparting sustained release of drug elution (or controlling the release time and speed), and suppressing cracking and peeling of the coating layer during stent expansion. Also good.
Hereinafter, the coating layer will be exemplified and described in detail (the present invention is not limited to this, and various modifications can be used). When the biodegradable polymer according to the present invention is a lactic acid-glycolic acid copolymer, lactic acid is 75 mol% to 100 mol% (especially 85 mol%), glycolic acid is 25 mol% to 0 mol% (especially 15 mol). %) Is preferably included. When the lactic acid-glycolic acid copolymer contains less lactic acid than the lower limit and more glycolic acid than the upper limit, the induction of an inflammatory reaction accompanying biodegradation of the biodegradable polymer is unfavorable. When the lactic acid-glycolic acid copolymer contains more than 90 mol% of lactic acid and less than 10 mol% of glycolic acid, the lactic acid-glycolic acid copolymer becomes less flexible and the coating is associated with the expansion of the stent. Care must be taken in this case because the layer is likely to crack or peel off. The lactic acid component contained in the lactic acid-glycolic acid copolymer may be only D-lactic acid, L-lactic acid alone, or both D-lactic acid and L-lactic acid. -It is preferable that lactic acid is contained equimol% each.
As a method of arranging the coating layer on the implant base material, the coating liquid is attached to the surface of the implant base material to remove the solvent, or the coating liquid is applied on a separately prepared plate and dried to prepare a film in advance. In addition, the method of sticking this on an implant base material etc. can be utilized.
