Description

超急性拒絶は、移植後数分〜数時間以内に生じ、移植された組織抗原に対する前形成された抗体に起因する。それは、出血および移植脈管構造の血栓閉塞により特徴付けられる。抗体の内皮への結合は、補体を活性化し、抗体および補体は、血管内の血栓症を促進し、血管の閉塞を生じる、移植内皮における多数の変化を誘導し、その結果として、移植された臓器は、不可逆的な虚血性損傷を罹患する(Abbasら、2000)。超急性拒絶は、多くの場合、予め存在するIgM同種抗体(例えば、赤血球に発現したABO血液群抗原に対して指向されたIgM同種抗体によって媒介される。この型の拒絶は、自然抗体によって媒介されるが、異種移植物の拒絶に対する主要な理由である。自然IgM抗体に起因する超急性拒絶は、通常は同種移植片がドナーおよびレシピエントのABO型に適合するように選択されるので、もはや同種移植片に関する主要な問題ではない。ABO適合同種移植片の超急性拒絶はなお生じ得るが、通常タンパク質同種抗原(例えば、異物MHC分子)に対して、または血管内皮細胞上に発現するあまりよく明らかになっていない同種抗原に対して指向されるIgG抗体により媒介される。このような抗体は、移植前または多胎妊娠前の輸血を介して同種抗原に対する事前の暴露(この事前の暴露は、「前感作(presensitization)」と言われる)の結果として生じ得る。Abbasら、2000。
移植片拒絶を遅延させる(すなわち、その生存を延長する)ために使用される多くの薬物は、種々の方法で機能する。免疫抑制剤は、広範に使用される。幾つかの免疫抑制薬物の作用の機構の概説については、Stepkowski,2000を参照のこと。シクロスポリンAは、移植片拒絶を阻害するために最も広範に使用される免疫抑制薬物の1種である。これは、インターロイキン−2すなわちIL−2のインヒビターである(インターロイキン−2のmRNA転写を防止する)。より直接的に、シクロスポリンは、カルシニューリン活性化を阻害する。カルシニューリン活性化は、通常、T細胞レセプター刺激の際に起こる。カルシニューリンは、NFAT(活性化T細胞の核性因子)を脱リン酸化して、NFATが核に侵入してインターロイキン−2プロモーターに結合することを可能にする。このプロセスを遮断することにより、シクロスポリンAは、CD4+T細胞の活性化を阻害し、そして活性化から生じる(阻害がなければ起こるはずである)事象のカスケードを阻害する。タクロリムスは、別の免疫抑制剤であり、インターロイキン−2の産生を阻害することによって作用する。
移植片拒絶における補体の役割は、周知である。これは、異種移植の場合に特に当てはまるが、補体は、同種移植においてもまた、役割を果たす。概説については、PlattおよびSaadi,1999を参照。補体の役割の1つの局面は、臓器移植片がレシピエントの血液で再灌流される時点で虚血−再灌流障害が起こり得ることである。補体はまた、同種移植片拒絶の幾つかの兆候を引き起こし得る。
本明細書中で開示される方法は、同種移植片生存を延長させるために使用される。この方法は、一般に、補体活性のインヒビターを1以上の免疫抑制剤と組み合わせて投与する工程を包含する。
補体活性のインヒビターの投与は、当業者に公知の方法に従って実施される。これらのインヒビターは、好ましくは、同種移植片移植の前に投与されるか、または移植の時点で投与され、この投与は、長期的様式で継続する。これらのインヒビターは、拒絶エピソードが起こる事象において拒絶エピソードの間、さらに投与され得る。
(方法)
(動物および免疫抑制剤)
雄性かつ成体のC3H(H−2k)マウスおよびBALB/c(H−2d)マウス(Jackson Labs,Bar Harbor,Maine)(体重25〜30g)を、それぞれドナーおよびレシピエントとして選択した。免疫抑制を受ける群において、レシピエントに、CsA(15mg/kg/日、s.c.、0日目から拒絶の終点もしくは100日目まで毎日)、もしくはCyP(40mg/kg/日、i.v.、0日目および1日目)、または抗C5 mAb(クローンBB5.1、Alexion Pharmaceuticals Inc.、40mg/kg/日、i.p.、0日目〜2日目、その後0日目〜60日目に週2回)を注射した。動物を、Animal Care Facility,University of Western Ontarioにて従来の条件下で収容し、the Canadian Council on Animal Careによって確立されたガイドライン(Olfertら,1993年)に従って世話をした。
C3Hドナーから摘出した皮膚移植片の全体の厚みを、1×1cm正方の小片に切断してBALB/cレシピエントの胸郭の背上に移植し、その1週間後、同じドナーからの心臓移植を行った。拒絶を、皮膚移植片の完全な壊死と規定した。
皮膚の前感作の7日後、C3Hマウス心臓を、前感作したBALB/cレシピエントの腹部に、ドナーの大動脈とレシピエントの大動脈とを吻合し、そしてドナーの肺動脈とレシピエントの下大動脈とを吻合することによって移植した。再移植を行った群においては、未処置のCH3マウスもしくは前感作した長期間生存BALB/cレシピエントのどちらかから収集した第二の心臓移植片を、長期生存の第一の腹部心臓移植片を保有するレシピエントの頚部領域内に、ドナー大動脈とレシピエントの頚動脈とを吻合し、そしてドナーの肺動脈とレシピエントの外頚動脈とを吻合(端側吻合)することによって移植した。心臓移植片を、他に示さない限り拒絶まで毎日モニタリングした。拒絶を、拍動の完全な停止として規定した。
前感作したレシピエントを、各群8匹の動物を含む8つの群に無作為に割り当てた:第1群(未処置のマウス);第2群(CsA処置マウス);第3群(CyP処置マウス);第4群(CsA+CyP処置マウス)、第5群(抗C5 mAb処置マウス);第6群(抗C5 mAb+CsA処置マウス);第7群(抗C5 mAb+CyP処置マウス);第8群(抗C5 mAbのCsAおよびCyPとの併用処置マウス)。心悸動が明確でなくなった時もしくはPOD 100の時、移植片を慣用的組織学、免疫組織化学およびウェスタンブロット分析のために摘出し、血清サンプルをフローサイトメトリー分析および補体溶血アッセイのために収集した。第6群および第8群にさらに5匹の動物を入れ、POD3(第1群〜第5群、第7群についてのMST)で屠殺し、同時点で比較した。血清サンプルを、抗ドナー抗体レベルおよび補体活性の連続的な変化を検出するために、第8群においてPOD 11、21、28および60で収集した。さらに、3重処置した前感作レシピエントが第一の心臓移植片を100日間保持した場合、これらに第二の心臓を再移植した。未処置C3H心臓もしくは別の前感作BALB/cレシピエントからの100日間生存C3H心臓を、第二の心臓として使用した。8匹の動物を、各再移植群に含めた。
組織サンプルを、10%緩衝化ホルムアルデヒド中で固定した。次いで、生検を、パラフィン中に包埋し、そしてH&E染色のために切片にした。この顕微鏡切片を、拒絶の重症度について目隠し様式で、病理学者が検査した。移植片拒絶の判定基準は、脈管炎、血栓症、出血およびリンパ球浸潤の存在を含んだ。これらの変化を、以下のように評価した:0(変化なし);1(最小限の変化);2(穏やかな変化);3(中程度の変化);4(著しい変化)。
4μm切片を、Tissue−Tek O.C.Tゲル(Optimum Cutting Temperature,Skura Finetek,Torrance,CA)中に入れ、ゼラチンコートした顕微鏡用スライドガラス上にマウントし、そしてElite Vectastain ABC kit(Vector Laboratories Inc.,Burlingame,CA)を用いて標準間接的アビジン−ビオチン免疫ペルオキシダーゼ染色方法によって染色した。生検を、ビオチン結合体化ラット抗マウスCD4 mAb(クローンYTS 191.1.2,Cedarlane Laboratories Ltd.,Homby,Ontario,Canada)およびビオチン結合体化ラット抗マウスCD8 mAb(クローン53−6.7,Pharmingen,Franklin Lakes,NJ)を用いてCD4+細胞およびCD8+細胞について、それぞれ染色した。移植片内単球/マクロファージ浸潤を、ビオチン結合体化ラット抗マウスMac−1 mAb(Cedarlane Laboratories Ltd.,Homby,Ontario,Canada)で染色することによって検出した。マウスIgGおよびマウスIgMの移植片内の沈着を、ビオチン結合体化ヤギ抗マウスIgGおよびヤギ抗マウスIgM(Cedarlane)を用いて検出した。補体沈着の同定について、切片を、ヤギ抗C3ポリクローナルAbもしくはヤギ抗C5ポリクローナルAb(Quidel,San Diego,CA)、ビオチン化ウサギ抗ヤギIgG(Vector Laboratories)、ならびにHRP結合体化ストレプトアビジン(Zymed Laboratories,South San Francisco,CA)と共に連続的にインキュベートした。スライドを、工程毎の間にリン酸緩衝化生理食塩水で洗浄し、そして光学顕微鏡下で検査した。ネガティブコントロールを、一次抗体を省略することによって実施した。この免疫染色を、各切片につき5つの高出力視野で評価し、そして5回の独立した実験を行った。免疫ペルオキシダーゼ染色の切片を、染色強度に従って0から4+までに等級をつけた:0(ネガティブ)、1+(不明瞭)、2+(弱い染色)、3+(中程度の染色)および4+(非常に強い染色)。
循環する抗ドナー特異的なIgG抗体およびIgM抗体を、FACScanフローサイトメトリー(Becton Dickinson,Mountain View,CA)により、レシピエント血清において評価した。Glotzら(1993);Tyanら(1994)。簡潔にいうと、C3Hマウス脾臓細胞を単離し、そして未処置コントロールおよび実験群に由来する血清とともに、37℃で30分間インキュベートした。全IgG、全IgG1、全IgG2a、全IgG2bおよび全IgMについて染色するために、細胞を洗浄し、そしてマウスIgGのFcタンパク質に特異的なFITC結合体化ヤギ抗体、もしくはマウスIgMのμ鎖に特異的なフィコエリトリン結合体化ヤギ抗体(Jackson ImmunoResearch Laboratories,West Grove,PA)、もしくはFITC結合体化ヤギ抗マウスIgG1(CALTAG Laboratories,Burlingame,CA)、もしくはFITC結合体化ヤギ抗マウスIgG2a(CALTAG)、もしくはFITC結合体化ヤギ抗マウスIgG2b(CALTAG)と共にインキュベートした。4℃で1時間の染色の後、細胞をPBSで洗浄し、5×106mLで再度浮遊させ、そして平均チャネル蛍光強度(抗体結合反応性を表す)についてフローサイトメトリーによって分析した。
精製抗C5 mAbを、GVB2+緩衝液(ゼラチンVeronal緩衝化生理食塩水:0.1%ゼラチン、141mM NaCL、0.5mM MgCl2および1.8mMバルビタールナトリウム)中で2倍で連続希釈し(175〜0.1μg/ml)、そして96ウェルプレートに3連で(50μl/ウェル)加えた。BALB/cマウス血清を、GVB2+緩衝液で40% v/vに希釈し、そして同じ96ウェルプレートの列に加え(50μl/ml)、各ウェルにおけるBALB/cマウス血清の終濃度が20%になるようにした。このプレートを、室温で約30分間インキュベートし、この間にニワトリ赤血球を調製した。ニワトリ赤血球を、5回×1mlのGVB2+緩衝液で洗浄し、そして終濃度5×107/mlでGVB2+緩衝液中に再度浮遊させた。4mlのニワトリ赤血球を、抗ニワトリRBCポリクローナル抗体(Intercell Technologies,Hopewell,NJ,0.1% v/v)を加え、そしてこの細胞を、4℃で15分間、頻繁にボルテックスしながらインキュベートすることによって感作させた。次いで、この細胞を、でGVB2+で2回×1ml洗浄し、そしてでGVB2+中に終容量2.4mlで再度浮遊させた。このニワトリ赤血球(30μl/ウェル、2.5×106細胞)を、血清および抗C5 mAbを上述のように含むプレートに加え、よく混合し、そして37℃で30分間インキュベートした。次いで、このプレートを、1000×gで2分間遠心分離し、そして85μlの上清を、新しい96ウェルマイクロタイタープレートに移した。このプレートを、マイクロプレートリーダーを用いてOD415nmで読み取り、溶血の百分率を以下の式を用いて決定した:
凍結心臓サンプルの音波破砕を、RIPA溶解緩衝液(Santa Cruz Biotechnology,Inc.)中4℃で1分間、10秒間の間隔で行い、その後、4℃、13,000rpmで10分間の微小遠心分離を行った。澄んだ上清を、Detergent−compatible protein assay kit(BIO−RAD)を用い、直ちに3連でタンパク質含有量について定量した。心臓溶解液(10μgタンパク質/ウェル)を、NuPAGE、4〜12%勾配Bis−TrisゲルおよびMES緩衝液系(Invitrogen)上で分離し、そして半乾燥トランスファー装置(BIO−RAD)を用いて、ポリビニリデンジフルオリド(PVDF)膜(孔サイズ0.45μm;Invitrogen)に移した。膜を、正しい分子量において適切に切断し、ブロット1枚につき2種類の異なる一次抗体で発色させた(各ブロットを、試験抗体および(等量のサンプルをローディングしたことを保証するために)内部コントロール抗体に曝した)。抗Bcl−3(N−19)ウサギポリクローナル血清(Santa Cruz Biotechnology,Inc.)および抗Bcl−XS/L(M−125)ウサギポリクローナル血清(Santa Cruz Biotechnology,Inc.)を含む試験一次抗体を、Bcl−2タンパク質およびBcl−xlタンパク質の移植片内発現を検出するために用いた。抗カルセケストリンウサギポリクローナル血清(Calbiochem)を、内部コントロール一次抗体として使用した(Kobayashiら,1999)。一次抗体結合の検出を、以前に記載された(Arpら,1996)ように、洗浄したインキュベート後のブロットを西洋ワサビペルオキシダーゼ(HRP;Roche Laboratories)に結合体化したポリクローナルヤギ抗ウサギIgG画分に曝し、次いで化学発光を増強させるためにHRP結合体化抗体(Roche Laboratories)に曝すことによって適切に発色させることにより、実施した。
データを、平均±SDとして報告した。実験群の間の同種移植片生存を、rank−log試験を用いて比較した。組織学的知見および免疫組織学的知見を、Mann−Whitney U試験を用いて分析した。フローサイトメトリーデータおよびウェスタンブロットデータを、一元ANOVAを用いて分析した。0.05未満のp値を有する相違を、有意であるとみなした。
(C3Hドナー皮膚移植片による前感作は、BALB/cレシピエントの心臓同種移植片における抗体媒介性ACHRを誘導する。)
診療所において前感作された患者を模倣する安定な小動物モデルを開発するため、ならびにABMRを研究するため、新規な完全MHC不適合マウスABMRモデルを、マウスレシピエントの前感作を通して開発してきた。このモデルにおいて、BALB/cレシピエントを、CH3ドナー皮膚移植片によって前感作し、その1週間後に同じドナーからの心臓移植を行った。ドナー皮膚前感作の7日後、抗ドナーIgG抗体の血清レベルは著しく上昇し、前感作BALB/cレシピエントにおけるピークのレベルに達したが、IgM抗体レベルは上昇しなかった(図1A)。次いで、同じドナーからの心臓移植を、これらの高度に感作されたマウスにおいて実施した。免疫抑制剤なしで、CH3心臓移植片は、ACHRによって急速に(3.1±0.4日以内)拒絶され、重篤な血栓症、出血および梗塞によって特徴付けられた(図1B−a)。対照的に、感作されていないBALB/cレシピエントにおける同じ心臓移植片(8.2±0.8日の平均生存時間(MST)を有する)は、術後3日目(POD3)に正常な組織学を示した(図1B−b)。未感作BALB/cレシピエントと同じ日に比較した場合、前感作された動物における心臓移植片は、多量のIgG抗体および補体(C3およびC5)の沈着を有したが、最小限のCD4+浸潤
およびCD8+浸潤を有したことを明らかにした。その上、前感作レシピエントにおける循環する抗ドナーIgGレベルは、POD3の心臓移植片を与えた未感作の同じレシピエントのIgGレベルより有意に高かった(P<0.01、図1C)。しかし、抗ドナーIgMは、循環(図1C)および心臓移植片(表1)において非常に低いレベルで維持され、未感作レシピエントと前感作レシピエントとの間に有意な相違がなかったことを示した。さらに、未処置の前感作心臓レシピエントおよび未感作心臓レシピエントの両方において、正常レベルの補体溶血活性が示された(図1D)。これらのデータは、このモデルが、前感作レシピエント(前感作レシピエントで補体が病因において重要な役割を果たす)におけるABMRを研究するための理想的な移植モデルであることを示す。
(POD3での未感作BALB/cレシピエントおよび前感作BALB/cレシピエントにおけるC3H心臓同種移植片の免疫組織学的変化の比較)
補体は、ABMRにおいて重要な役割を果たすことが示されている。しかし、高度に感作されたレシピエントにおける、終末補体カスケードのC5レベルでの機能的遮断の阻害効果は、未知である。本明細書中で提示される研究において、前感作モデルを、抗C5 mAbの効力を研究するために、ABMRの存在下で、単独でまたはCsAおよび/もしくはCyPと組み合わせて用いた。図2Aに示されるように、CsAもしくはCyPまたは組み合わせたこの2つの薬物のいずれかによる処置は、ABMRを防止せず、移植片は、それぞれ3.0±0.0日以内、3.3±0.5日以内、および3.5±0.6日以内に拒絶され、ACHRの代表的な病理学的不全(血管内血栓症および間隙性出血を含む)を有した(図2B−b、c、d)。これらの不全は、未処置の前感作BALB/レシピエントにおける心臓移植片(図2B−a)と区別不能であった。抗C5単一治療もしくは抗C5とCyPとの併用治療は、移植片生存を改善せず、心臓移植片はそれぞれ3.5±0.6日以内および3.2±0.4日以内(図2A)にACHRによって拒絶された(図2B−e、f)。抗C5 mAbとCsAとの併用治療(このプロトコールは、未感作動物において長期心臓同種移植生存を誘導し得る)は、この前感作モデルにおいて移植片生存をわずかに延長したが、やはり心臓移植片は、11.9±1.8日以内(図2A)に重篤な体液性拒絶(脈管炎、血栓症、出血および最小限の細胞浸潤を伴う)によって拒絶された(図2B−g)。対照的に、抗C5 mAbのCsAおよびCyPとの併用の3重の治療は、前感作動物において、拒絶の兆候なしに(図2B−h)、100日間を超える(図2A)無期限の心臓移植片生存を達成した(未処置動物または単独治療で処置されたかもしくは2つの薬物の併用治療で処置された動物に対してP<0.01)。この前感作マウスモデルにおいて、図2に示されるように、3日以内に心臓移植片が拒絶されたレシピエントにおいて、僅かな移植片内CD4+細胞浸潤および移植片内CD8+細胞浸潤しか観察されなかった。しかし、心臓移植片がより長く生存した場合、これらのT細胞の数は、抗C5 mAb+CsA処置レシピエントにおいて拒絶の時(POD11)に僅かに増加し、そして3重治療処置レシピエントにおいて移植片生存の初期段階(例えば、POD11)において僅かに増加した。その上、CsAの継続処置を受けた3重治療群においてCD4+細胞浸潤およびCD8+細胞浸潤は、長期生存心臓移植片において、POD60およびPOD100で阻害された。さらに、中程度の移植片内Mac−1+細胞浸潤(単球およびマクロファージを含む)が、未処置マウスおよびCsA処置マウス、CyP処置マウスもしくはCsA+CyP処置マウスにおいて見出された。このとき、これらの細胞の浸潤は、抗C5 mAb処置マウスにおいて有意に減少した(表2)。これらの結果は、機能的遮断性抗C5 mAbは、従来の免疫抑制剤の使用および効力により、前感作動物においてABMRを予防しそして無期限の心臓移植片生存を達成することを示す。
(剖検での前感作マウスレシピエントにおける心臓同種移植片の免疫ペルオキシダーゼ染色)
抗C5 mAbは、補体タンパク質C5の炎症促進性分子であるC5aとC5b−9とへの切断を遮断すること(Kroshusら,1995)、ならびにマウスにおいて完全にかつ継続的に終末補体活性を遮断すること(Wangら,1999)が、以前に示された。現在の研究において、レシピエントマウス血清を、抗体によって前感作されたニワトリ赤血球を溶解する能力を評価し、そして同時点(POD3)において比較することによって、終末補体活性を測定した。CsAもしくはCyPまたはこの2つの薬物の併用のいずれかによるマウスの処置は、終末補体活性に影響を有さなかった。一方で、抗C5 mAbの単独処置またはCsAもしくは/およびCyPとの併用処置は、この活性を完全に阻害した(図3;ナイーブかつ未処置の動物ならびにCsA処置動物、CyP処置動物もしくはCsA+CyP処置動物に対して、P<0.01)。さらに、抗C5 mAb処置動物から幾つかのより初期の時点で得られた血清は、同様に低減された溶血活性を示した。このことは、血清終末補体が、処置期間を通じて阻害されたことを示唆する。さらに、心臓移植片における局所的C5沈着は、抗C5 mAb処置前感作レシピエントにおいて完全に予防されたが、未処置前感作動物またはCsA処置前感作動物、CyP処置前感作動物およびCsA+CyP処置前感作動物においては予防されなかった(表2)。予測されたように、抗C5 mAbによる処置は、移植片におけるC3沈着を予防しなかった(表2)。これらの結果は、抗C5治療は、高度に感作されたレシピエントにおける心臓同種移植後の補体活性全体を完全に遮断することを示唆する。
体液性拒絶における抗C5 mAbのさらなる役割を調べるために、異なる群において、レシピエント血清中の抗ドナー同種抗体レベルをフローサイトメトリーによって測定し、そして移植片内抗体沈着を免疫染色技術を用いて測定した。図4Aは、POD3で、未処置前感作BALB/cレシピエントが高レベルの循環抗ドナーIgG抗体を有したことを示す。単独治療もしくは2つの薬物の併用治療のいずれかを与えたレシピエントにおいて、CsAおよび/またはCyPは、循環する抗ドナーIgGレベルを部分的にダウンレギュレートした。一方、抗C5 mAb単独または抗C5 mAbとCsAもしくはCyPとの併用での処置は、同日に抗ドナー抗体レベルにさらなる影響を及ぼさなかった。対照的に、抗C5 mAb、CsAおよびCyPの3重治療により、高レベルの循環抗ドナーIgGが、徐々にダウンレギュレートされ、そしてPOD60で低レベルに達し、その後、100日目までこのレベルに留まり続けた(図4B)。異なる処置群における循環抗体のレベルと同様に、表2は、未処置の前感作動物または単独治療もしくは2つの薬物の併用治療で処置された前感作動物の、急速に拒絶された心臓移植片において、抗マウスIgGの強い沈着が存在したことを示す。興味深いことに、3重治療によって、IgG抗体沈着は、POD100の長期生存心臓移植片において、徐々に穏やかなレベルにまで弱まった(図4C−a、表2)。このモデルにおいて、IgMは、処置したかまたは処置しなかった前感作レシピエントにおける循環(図4A、B)もしくは移植された心臓移植片(図4C−b、表2)のどちらかにおいて、非常に低いレベルに留まった。さらに、抗C5 mAbによる処置は、60日目までに補体活性を検出不可能なレベルにまで除去し、その後、3重治療を与えた前感作マウスレシピエントにおける抗C5治療の中断後のPOD100に、枯渇前レベルにまで段階的に回復した(図4D)。その上、移植片内C5沈着もまた、100日間生存前感作動物において検出された(表2)。これらのデータは、抗移植片抗体の存在および補体活性化にもかかわらず、進行中の移植片適応が、3重治療処置前感作レシピエントにおいて起こることを実証する。
抗C5 mAbベースの3重治療がIgGサブクラスにおけるシフト(このシフトは、適応に関連して起こり得る)を誘導するか否かを決定するために、抗ドナーIgG1サブクラス、抗ドナーIgG2aサブクラス、および抗ドナーIgG2bサブクラスの血清レベルを、未処置レシピエントと心臓移植片適応化レシピエントとの間で比較した。未処置レシピエント由来の血清は、主にIgG1イソ型を含み、IgG1/IgG2aの高い比によって示された(図5A)。対照的に、適応化移植片を保有するレシピエントにおいて、IgG1/IgG2aの比における有意な低下が観察された(図5A、P<0.01)。さらに、適応化心臓移植片を有する前感作レシピエントは、拒絶した移植片を有する同じレシピエントと比較した場合、抗ドナーIgG2bの増大したレベルを示した(図5B、P<0.01)。さらに、単独治療もしくは2つの薬物の併用治療で処置されたレシピエントにおけるIgGイソ型のパターンは、未処置動物におけるIgGイソ方のパターンと区別がつかなかった。これらのデータは、抗ドナーIgG1イソ型は、移植片拒絶と関連し得、一方、抗ドナーIgG2bサブクラスの産生は、保護抗体として機能し得、そして適応の誘導において重要な役割を果たし得ることを示す。
このモデルにおいて、保護タンパク質の移植片内発現と体液性損傷に対する移植片抵抗性との間に因果関係が存在するか否かを決定するために、ウェスタンブロット分析を使用して、高度に感作されたマウスレシピエント由来の心臓移植片組織における目的のタンパク質を検出した。長期生存心臓移植片は、POD100において高レベルのBcl−2タンパク質およびBcl−xlタンパク質を発現することが見出され、そして、これらのタンパク質は、抗C5 mAbベースの3連治療を受けた高度感作レシピエントにおいて心臓移植の12日後という早さで検出された(図6)。対照的に、未処置動物(図6)または単独治療もしくは2つの薬物の併用治療で処置された動物の心臓移植片において、Bcl−2タンパク質発現およびBcl−xlタンパク質発現は存在しなかった。これらの結果は、この前感作モデルにおいて、無期限生存動物における体液性損傷に抵抗性の移植片は、Bcl−2タンパク質およびBcl−xlタンパク質によって提供される保護に関連することを示唆する。
適応化移植片の拒絶に抵抗する能力は、ナイーブ移植片が同種移植後に拒絶を受ける病態生理学的条件下で直接的に試験されていない。このモデルにおいて、適応性第一心臓移植片を有する前感作レシピエントが、第二の適応化移植片を許容するが第二のナイーブ移植片を拒絶するか否かを決定するために、本発明者らは、再移植シナリオを実施した。抗C5 mAbベースの3重治療によって処置した前感作BALB/cレシピエントにおいて、適応化C3H心臓移植片が100日目時点を生存した後、低レベルの同種抗体が検出された時(図4B)ならびに補体活性が処置前レベルに戻った時(図4D)、これらのレシピエントに、第二の心臓移植片を与えた。具体的には、ナイーブ心臓(図7A)もしくは別の前感作BALB/cレシピエント由来の100日間適応化C3H心臓(図7B)を、適応性第一C3H心臓を保有する前感作レシピエントの頚部に移植した。これらのレシピエントは、第2のナイーブ心臓を、6.6±1.1日目に重篤なAVRを伴って拒絶した(図8B−a)。同時に、第一の心臓は生存し続けた。対照的に、既に別の前感作マウスにおいて100日間生存している適応化心臓を第二の移植片として用いた場合は、これらの移植片は、適応性第一心臓移植片を保有する前感作レシピエントによって許容された(図8A)。これらの適応化第二心臓移植片において、第二移植後の90日間、拒絶の兆候は存在しなかった(図8B−b)。これらのデータは、適応化移植片は、抗ドナー抗体および補体の効果(通常、これらの前感作レシピエントにおいて同種移植片拒絶を媒介する)に対して抵抗性になることを示す。さらに、適応化移植片の宿主は新しい移植片を拒絶するという事実は、この適応化は、移植片に対する変化に関与することを示唆する。
この前感作モデルにおける適応が、移植片および/またはレシピエントにおける変化によって引き起こされるか否かを決定するために、別の再移植を実施した。具体的には、C3H心臓移植片を、前感作BALB/cマウスにおいて100日間にわたって適応させ、次いで、適応化心臓移植片を、CsAのみで処置した第2の前感作BALB/cレシピエント内に再移植した(図7C)。CsAのみの治療は、細胞性拒絶を予防し得るが、新しいC3H心臓の急速体液性拒絶を予防できない。この適応化C3H心臓移植片は、CsA処置前感作BALB/cレシピエントにおいて、急速に拒絶された。再移植後、適応化心臓移植片における病理学は、正常(図9A)から重篤なACHRに変化した。このACHRは、大量の間隙出血を伴ったが、細胞浸潤はほとんどなかった(図9B)。さらに、適応化C3H心臓を与えたレシピエントにおける高レベルの抗ドナーIgGおよび正常レベルの補体溶血活性は、ナイーブC3H心臓を与えたCsA処置前感作レシピエントのこれらのレベルと同様であった。この結果は、抗C5 mAbベースの3重治療によって誘導される適応化は、移植片に対する変化のみでなく、レシピエントに対する変化にも関与する機構に起因し得ることを、さらに示す。
(心臓移植モデルにおける急性脈管拒絶)
終末補体の形成のインヒビターを含むことが急性脈管拒絶を弱めるか否か、ならびにこのようなインヒビターを免疫抑制剤と組み合わせた使用は長期同種移植片生存を達成するか否かを決定するために、実験を実施した。この一組の実験において、抗C5モノクローナル抗体を、シクロスポリンと組み合わせて使用した。使用したモデルは、C3HマウスからBALB/cマウスへの同種移植片異所心臓移植であった。このモデルは、強くMHC不適合であるC3HマウスおよびBALB/cマウスによる、緊縮急性脈管拒絶モデルである。移植および他の方法を、Wangら(2003)によって記載されたように実施した。
腹部内異所心臓移植を、以前にWangら(2003)によって記載されたように実施した。簡潔に言うと、ドナーにおいて胸骨正中切開を実施し、そして心臓移植片を、下大動脈および大動脈を通してインシチュでゆっくりと1.0mlの冷ヘパリン化リンガー乳酸溶液で灌流し、その後、上大動脈および肺静脈を繋ぎ、そして切り離した。上行大動脈および肺動脈を横に切断し、そして移植片をドナーから摘出した。次いで、この移植片を、ドナーの肺動脈とレシピエントの下大動脈との間、ならびにドナーの動脈とレシピエントの腹部動脈との間で、11−0ナイロン縫合糸を用いて端側吻合によって血管移植した。移植した心臓の拍動を、直接腹部触診によって毎日モニタリングした。
マウス(雄性、8〜12週齢マウス、体重25〜30g)を、各群6〜8匹のマウスを含む6つの実験群に分けた。移植を、0日目に行った。組織学的変化を、終末点(この終末点は、移植片不全の時点である)において調べた。または、ある場合は、マウスを、移植片不全の前に屠殺した。投与したBB5.1の投薬量(週に3回、40mg/kg体重)が、完全に終末補体活性を阻害することは、先行研究から公知である。
(同種移植片生存)

この結果は、補体阻害薬物を免疫抑制剤に加えて使用することの相乗効果を示す。未処置マウスにおいて、移植片を、約8日目に拒絶した。免疫抑制剤シクロスポリンAのみの、毎日の長期的な使用は、移植後役15日間の移植片生存の延長をもたらした。終末補体の形成を阻害するための抗C5抗体BB5.1の使用は、それ自体では効果を有さず、移植片拒絶は、コントロール群(第1’群)と同様に、移植後8日目に起こった。移植後28日目までのBB5.1+シクロスポリンAの併用は、移植片生存において相乗効果を示し、移植片生存を約80日間まで延長した。より驚くべき結果は、BB5.1およびシクロスポリンAをそれぞれ移植後長期的に投与した第5’群の結果である。この場合、移植片生存は、100日間を超えた(ここで得られたデータの限り)。さらに、表4に示される組織学的結果は、BB5.1およびシクロスポリンAの投与は、BB5.1単独もしくはシクロスポリンA単独のどちらかよりもずっとよく移植片を変化から保護し、移植後100日目において移植した心臓に組織学的変化が存在しなかったほどであった。これらのモデルにおける100日間の生存時間を、金基準とみなす。このモデルにおける100日間の生存は、同種移植片が無期限に生存することを示すと考えられる。BB5.1投与を28日目に停止した場合、移植片は保護されるが、約80日目に、最低限の組織学的変化から穏やかな組織学的変化までの変化を開始し、この時に移植片不全が起きた。
(前感作心臓移植モデルにおける急速拒絶)
実施例3に類似の第二の組の実験を行った。しかし、レシピエントマウスを、ドナー器官に前感作した。これらの実験において、この前感作を、皮膚移植片の事前の移植によって起こした。一般的に、前感作は、より初期の同種移植片を与えた結果としてのみでしか起こり得ないのではなく、多数回の血液輸血によって、もしくは女性においては妊娠することによってもまた、起こり得る。このような前感作方法に加えて、ABO不適合を有する同種移植片は、ABO抗原に対して機能する抗体ゆえに迅速に攻撃されかつ拒絶され、このような攻撃を防止するための工程を行うまで攻撃される。
(同種移植片生存)
(剖検における心臓同種移植片の組織学的変化の等級中央値)

表5に示される結果は、前感作マウスモデルと実施例3において使用したような非前感作マウスモデルとの間の相違を示す。この結果は、前感作の非存在下において、移植片は、いかなる薬物の処置も非存在の下で、約8日目に拒絶されることを示す。動物の前感作は、より迅速な拒絶を引き起こし、前感作した動物における移植片の拒絶は、いかなる薬物の処置も非存在の下で、約3日以内である。BB5.1、シクロスポリンAもしくはシクロホスファミドのいずれかによる処置は、移植片生存になんら効果を及ぼさず、各群の動物において、移植片は約3〜4日以内に拒絶された。BB5.1とシクロスポリンAとの併用は、拒絶にいくらか効果を有し、約12日目に拒絶が起こった。BB5.1とシクロホスファミドとの併用は、拒絶に対して保護的効果を有さず、拒絶は約3日目に起こった。同様に、シクロスポリンAとシクロホスファミドとの併用は、拒絶に対して本質的に保護的効果を有さず、拒絶は3〜4日目に起こった。非常に驚くべきことに、3種全ての薬物(BB5.1およびシクロスポリンの長期的投与+移植時のシクロホスファミドの投与)は、全てのマウスに高い相乗効果を示し、これらのマウスは、100日を超えて生存した。やはり、このモデルにおける100日の生存を、金基準とみなし、無期限生存と考える。

Claims (46)
- レシピエントのヒトにおける同種移植片の生存を延長するための医薬であって、
a)補体成分C5に結合し、かつC5のフラグメントC5aおよびフラグメントC5bへの切断を阻害する抗体、ならびに
b)少なくとも一種の免疫抑制薬物
を含み、該抗体が長期投与されることにより、該ヒトにおいて、終末補体活性を遮断するのに有効な量の該抗体が少なくとも14日間維持されることを特徴とする、医薬。 - 前記同種移植片が、MHC不適合同種移植片である、請求項1に記載の医薬。
- ヒトにおける同種移植片の生存を延長するための医薬または医薬パッケージの製造における、(a)補体成分C5に結合し、かつC5のフラグメントC5aおよびフラグメントC5bへの切断を阻害する抗体、ならびに(b)免疫抑制薬物の使用。
- 一種よりも多くの免疫抑制薬物が、前記医薬または医薬パッケージに含まれる、請求項3に記載の使用。
- 前記抗体および前記免疫抑制薬物が、前記ヒトへの同時投与に適した処方物中に存在する、請求項3または4に記載の使用。
- 前記抗体および前記免疫抑制薬物が、前記ヒトへの連続投与に適した処方物中に存在する、請求項3または4に記載の使用。
- 前記抗体が、前記ヒトへの長期投与に適した処方物中に存在する、請求項3または4に記載の使用。
- 前記免疫抑制薬物が、前記ヒトへの長期投与に適した処方物中に存在することにより、該ヒトにおいて、終末補体活性を遮断するのに有効な量の前記抗体が少なくとも14日間維持される、請求項3〜7のいずれか一項に記載の使用。
- レシピエントのヒトにおいて、同種移植片の生存を延長するための医薬であって、該医薬は:
a)補体成分C5に結合し、かつC5のフラグメントC5aおよびフラグメントC5bへの切断を阻害する抗体、ならびに
b)少なくとも二種の免疫抑制薬物
を含み、該抗体が該ヒトに長期投与されることにより、該ヒトにおいて、終末補体活性を遮断するのに有効な量の該抗体が少なくとも14日間維持され、該レシピエントのヒトは、該同種移植片に関してMHC不適合であるか、該同種移植片に対して前感作されているか、または該同種移植片に関してABO不適合であることを特徴とする、医薬。 - 前記抗体が、全抗体またはそのC5結合フラグメントである、請求項1、2または9のいずれか一項に記載の医薬。
- 前記全抗体またはそのC5結合フラグメントが、ヒト抗体、ヒト化抗体、キメラ化抗体もしくは脱免疫化抗体、またはそのC5結合フラグメントである、請求項10に記載の医薬。
- 前記C5結合フラグメントが、Fab、F(ab’)2、Fv、および単鎖抗体からなる群より選択される、請求項10または11に記載の医薬。
- 前記C5結合フラグメントが、ペキセリズマブである、請求項10に記載の医薬。
- 前記全抗体が、エクリズマブである、請求項10に記載の医薬。
- 前記エクリズマブが、2週間毎に1回投与されることを特徴とする、請求項14に記載の医薬。

- 前記抗体が、前記ヒトの残りの生存期間の間、長期投与されることを特徴とする、請求項19に記載の医薬。
- 前記少なくとも一種の免疫抑制薬物が、シクロスポリンA、タクロリムス、シロリムス、OKT3、コルチコステロイド、ダクリズマブ、バシリキシマブ、アザチオプリン、ミコフェノール酸モフェチル、メトトレキサート、6−メルカプトプリン、抗T細胞抗体、シクロホスファミド、レフルノミド、ブレキナル、ATG、ALG、15−デオキシスペルグアリン、およびブレディニンからなる群より選択される、請求項1または2に記載の医薬。
- 少なくとも二種の免疫抑制薬物が前記レシピエントのヒトに投与されることを特徴とする、請求項1、2または21のいずれか一項に記載の医薬。
- 前記免疫抑制薬物のうちの少なくとも一つが、T細胞活性またはB細胞活性を阻害する、請求項9または22のいずれか一項に記載の医薬。
- 前記免疫抑制薬物のうちの少なくとも一つがT細胞活性を阻害し、かつ該免疫抑制薬物のうちの少なくとも一つがB細胞活性を阻害する、請求項9または22に記載の医薬。
- 前記免疫抑制薬物が、シクロスポリンAおよびシクロホスファミドである、請求項24に記載の医薬。
- 前記免疫抑制薬物が、シクロスポリンA、タクロリムス、シロリムス、OKT3、コルチコステロイド、ダクリズマブ、バシリキシマブ、アザチオプリン、ミコフェノール酸モフェチル、メトトレキサート、6−メルカプトプリン、抗T細胞抗体、シクロホスファミド、レフルノミド、ブレキナル、ATG、ALG、15−デオキシスペルグアリン、およびブレディニンからなる群より選択される、請求項9に記載の医薬。

- レシピエントのヒトにおける同種移植臓器の生存を延長するための医薬であって、該医薬は、補体成分C5に結合し、かつC5のフラグメントC5aおよびフラグメントC5bへの切断を阻害する抗体を含み、ここで、該医薬が該レシピエントのヒトに長期投与されることにより、該ヒトにおいて、終末補体活性を遮断するのに有効な量の該抗体が少なくとも14日間維持され、そして、該医薬が、該レシピエントのヒトにおける同種移植臓器の生存を延長するための少なくとも一種の免疫抑制薬物と組み合わせて該レシピエントのヒトに投与されることを特徴とする、医薬。
- レシピエントのヒトにおける同種移植臓器の生存を延長するための医薬であって、該医薬は、補体成分C5に結合し、かつC5のフラグメントC5aおよびフラグメントC5bへの切断を阻害する抗体を含み、ここで、該レシピエントのヒトは、該同種移植片に関してMHC不適合であるか、該同種移植片に対して前感作されているか、または該同種移植片に関してABO不適合であり、かつ、該医薬が該レシピエントのヒトに長期投与されることにより、該ヒトにおいて、該抗体の有効量が少なくとも14日間維持され、そして、該医薬が、該レシピエントのヒトにおける同種移植臓器の生存を延長するための少なくとも二種の免疫抑制薬物と組み合わせて該レシピエントのヒトに投与されることを特徴とする、医薬。
- 前記抗体が、全抗体またはそのC5結合フラグメントである、請求項32または33に記載の医薬。
- 前記全抗体またはそのC5結合フラグメントが、ヒト抗体、ヒト化抗体、キメラ化抗体もしくは脱免疫化抗体、またはそのC5結合フラグメントである、請求項34に記載の医薬。
