【発明の属する技術分野】
本発明は角膜の反射光量比の測定装置に関する。さらに詳しくは、角膜の厚さ方向の各層における反射光量相互の比を測定するための装置に関する。

【従来の技術】
図7には角膜Cの断面が模式的に示されている。符号Tは涙液層である。符号Uで示すのは上皮であり、符号Jで示すのは実質部であり、符号Iで示すのは内皮である。角膜Cに光を照射したとき、角膜の厚さ方向における各層の組織の密度の相違によってその反射光量は異なる。すなわち、角膜の厚さ方向に沿って反射光量の分布は一定ではない。一方、医療界では糖尿病等の疾病の初期段階で上記反射光量分布が変化するのではないかと言う研究が行われている。成人病の早期発見の手がかりが期待される重要な研究であると言われている。

【発明が解決しようとする課題】
しかし、従来、角膜の特定位置での厚さ方向に沿って反射光量比を正確に測定する装置は知られていない。スリット照明光によって角膜内皮細胞を撮影する装置は公知であるが、この装置による撮影範囲は比較的広いため、光量を測定しても広い測定範囲の光量の平均値が得られるだけである。しかも、広い範囲の撮影であるため各層の反射光を分離することが困難である。したがって、特定位置における正確な反射光量を測定することは困難である。

本発明はかかる課題を解消するためになされたものであり、被検眼の角膜の狭い特定位置について、測定しようとする層以外からの反射光を効果的に分離して正確な反射光量比を測定することができる角膜の反射光量比の測定装置を提供することを目的としている。

【課題を解決するための手段】
本発明の角膜の反射光量比の測定装置は、
第一スリットと第二スリットとが同一円周上に形成され、この円周の中心回りに回転し且つ停止しうるスリット板と、
上記第一スリットと照明光源とを有し、上記第一スリットを通過したスリット照明光によって被検眼の前眼部をその斜め前方から照明するための照明光学系と、
上記第二スリットと、上記スリット照明光の前眼部で反射された反射光を、停止した第二スリットを通して受光して当該光の光量を測定するための光量測定手段とを有する光量測定光学系と、
この光量測定光学系に配設された、光量測定光学系の焦点位置をその光軸方向に移動させるフォーカシング機構とを備えている。

かかる測定装置によれば、スリット板の回転に伴い、一の第一スリットを通過したスリット照明光は被検眼の角膜で反射されつつその上を移動する。一方、第二スリットは第一スリットと同一速度で移動するので上記スリット照明光は一の第二スリットを通過するようにされ得る。幅の狭いスリット照明光であっても、その角膜における反射光は測定しようとする層以外の層での反射光も若干含む。しかし、第二スリットを通過るときに測定対象層以外の層からの反射光は遮られるために、測定対象層での反射光が効果的に切り取られる。そして、スリット板の回転を停止させることにより、光量測定手段が被検眼の角膜上のある位置における幅狭のスリット反射光を受光する。この状態でフォーカシング機構が角膜の厚さ方向の各層に焦点を移動させることにより、各層での反射光量を測定して正確な反射光量比を得ることができる。

そして、上記スリット照明光による前眼部の像光線を上記第二スリットを通して観察および撮影するための撮影手段を有する撮影光学系をさらに備えている測定装置が好ましい。スリット板の回転に伴い、スリット照明光がスキャン(走査)した角膜の範囲の反射光を撮影手段が受光することができ、その範囲の角膜像を得ることができる。また、この像は前述した光量測定手段が得ることのできるスリット反射光の集合であるため、スリット板の回転を停止させることによってこの角膜像の内の一部に反射光量を測定する位置を設定することが可能である。換言すれば、角膜の象を観察しながら反射光量を測定する位置を選択することが可能となる。

また、上記スリット板に形成されたスリットのうちの一を検出するための検出器をさらに備えており、検出器によるスリットの検出信号に基づいてスリット板の回転が停止するように構成されてなる測定装置にあっては、常にスリットが停止する角度位置が定まる。換言すれば、常に角膜上の特定位置の反射光を受光し得るので正確な反射光量比を測定することができるので好ましい。

または、上記スリット板に形成された回転停止用の被検出マークと、この被検出マークを検出するための検出器とをさらに備えており、検出器による被検出マークの検出信号に基づいてスリット板の回転が停止するように構成されてなる測定装置にあっては、常にスリット板がその回転を停止する角度位置が定まることはもとより、常に特定のスリットが特定の位置に至ったときにスリット板が停止するので好ましい。なぜなら、複数個のスリットに製作誤差があっても、特定のスリットが常に角膜上の特定位置の反射光を受光し得るので正確な反射光量比を測定することができる。

この測定装置は、光学系を被検眼に接触させるいわゆる接触型の測定装置として構成したものである。すなわち、被検眼にたとえばコーンレンズと呼ばれる対物光学部材を当接させることによって光学系と被検眼との位置合わせを行い、そのうえで角膜上の特定位置について反射光量を測定することができる。対物光学系は接触している被検眼の動きに応じてその光軸方向に移動することができ、しかもこの対物光学系の後方は平行光が形成されるアフォーカル系であるため、測定位置や焦点位置が被検眼の動きに影響されない。接触している被検眼の動きに応じてその光軸方向に移動することができる対物光学系はフローティング系と呼ばれる。

この測定装置は、光学系を被検眼に接触させない、いわゆる非接触型の測定装置として構成したものである。すなわち、装置を被検眼に接触させることなく、XYZの各方向に移動しうる対物光学系を用いて光学系を被検眼に位置合わせする。その上で合焦光用スリットをその光軸に対して実質的に垂直方向に移動させるか、または、合焦光検出器における合焦用スリット光検出位置を変化させることによってフォーカシングを行う。すなわち、角膜の厚さ方向に焦点位置を移動させるものである。この構成により、被検者に煩わしさを与えることなくその被検眼の反射光量比を測定することができる。合焦光検出器における合焦用スリット光検出位置を変化させることは、たとえばラインセンサやエリアセンサ等の合焦光検出器において、その光検出範囲のうちの合焦したと判定する位置を変更することにより可能となり、また、合焦光検出器に検出用スリットを設けておき、このスリットをその光軸に対して横方向に移動させることにより可能となる。

【発明の実施の形態】
添付図面に示される実施形態に基づいて本発明の角膜の反射光量比の測定装置を説明する。

図1は本発明の測定装置の一実施形態としての接触型測定装置を示す光路図である。図2は図1の測定装置の要部斜視図である。

図1および図2に示す測定装置1は、被検眼Eに向かって前進後退するZ方向、並びにZ方向に垂直で且つ互いに垂直なX方向およびY方向に移動させうる機枠2上に各光学系が搭載されている。この機枠2を移動させることによって被検眼Eに対する光学系の各方向の位置決めを行う。

これら光学系は、被検眼Eに対向する対物光学系12、対物光学系12を通して被検眼Eの前眼部をその斜め前方からスリット光によって照明するための照明光学系4と、被検眼Eの前眼部表面で反射された上記スリット光を対物光学系12を通して受光し、その反射光量を測定する測定光学系3、および、この反射光の像を観察、撮影するための撮影光学系5とを含んでいる。また、照明光をスリット光とするため、および、前眼部表面で反射されたスリット状の反射光が通過するための複数のスリット16が形成されたスリット板6を備えている。

照明光学系4は照明用光源としてキセノンランプ7を有している。照明光学系4における対物光学系12とキセノンランプ7との間に、後述する傾斜ミラー群18a、18b、18c、18d、照明レンズ9およびスリット板6がその順に配設されている。キセノンランプ7からの照明光は照明用リレーレンズ19および上記スリット板6のスリット16を通過し、このスリット光が照明レンズ9、ミラー群および後述の対物光学系12を通って被検眼Eの角膜に収束させられる。

撮影光学系5は角膜細胞を観察、撮影するための撮像素子(CCD)10を有している。撮影光学系5における対物光学系12と撮像素子10との間には、撮影レンズ11、スリット板6、視野絞り20、撮影用リレーレンズ群21がその順に配設されている。角膜で反射された上記スリット光は、対物光学系12、撮影レンズ11、スリット板6のスリット16、視野絞り20、および後述のリレーレンズ群21を通過して撮像素子10に至る。撮像素子10は、スリット板6の回転に伴って角膜面をスキャンしたスリット光を視野絞り20の範囲で受光して角膜細胞の像を得る。

本装置1では、上記対物光学系12はいわゆるフローティング式として構成されている。すなわち、対物光学系12は微弱な弾性力によって被検眼に当接するように構成されており、当接している被検眼のZ軸方向の変位に応じてZ軸方向に移動できるようにされている。対物光学系12は、被検眼Eの角膜面に当接させるためのコーンレンズ13と、コーンレンズ13の後方(被検眼とは反対側)のフォーカシングレンズ(後部レンズ群)14とを有しており、これらは鏡筒12aに収容されている。

フォーカシングレンズ14は、駆動装置22aと移動距離検出器(エンコーダ)22bとを有するフォーカシング装置22によって光軸方向(Z方向)に移動させられる。この移動によって対物光学系12の焦点が移動する(フォーカシングする)。対物光学系12の焦点はすなわち測定光学系3の焦点であり、撮影光学系5の焦点でもある。フォーカシングレンズ14の特定の基準位置からの移動距離はエンコーダ22bによって検出される。この基準位置は、図1に示すごとく対物光学系12の焦点がコーンレンズの先端より若干寸法だけ内方にある点F(原点)にある位置とされている。フローティングおよびフォーカシングのためにフォーカシングレンズ14から後方に、照明光学系4では照明レンズ9まで、撮影光学系5では撮影レンズ11までが光束が平行となるアフォーカル光学系15とされている。かかる構成により、対物光学系12は当接している被検眼Eの動きに応じてZ方向に追随することができ、この追随作動の最中にもフォーカシングが可能である。すなわち、上記構成により、コーンレンズ13を被検眼Eに当接させると対物光学系12のフローティング動作によって被検眼EとのXYZ各方向の位置関係が定まり、これが維持され、その状態でフォーカシングレンズ14がZ方向に移動することによって焦点の移動がなされる。この機能を使用して後述する反射光量比の測定を行う。

上記スリット板6は対物光学系12の焦点に対して共役位置に配設されており、モータ17によって正逆方向に回転させられる。図3に示すように、スリット板6上にはその回転中心Oを中心とする円周上に、半径方向に延びる複数個のスリット16が等間隔で形成されている。本実施形態では、一のスリット(以下、第一スリットと呼ぶ)16aを通過したスリット状の照明光は、被検眼Eの前眼部で反射され、スリット板6上に結像する。また、このスリット反射光はスリット板6の回転中心3を挟んで第一スリット16aに対して180゜方向に位置する他のスリット(以下、第二スリットと呼ぶ)16bを透過して撮像素子10に至るように、照明光学系4と撮影光学系5とが配置されている。なお、スリット板6上にはこのスリット反射光以外の反射光も来ているが、第二スリット16bによって上記スリット反射光のみが切り取られ、他の反射光は遮られる。第一スリット16aおよび第二スリット16bは特定のスリットではない。多数あるスリットのうちの一のスリット16aを通過した照明光が被検眼で反射したうえで通過するスリットが第二スリット16bだというだけのことである。

両スリット16a、16bは同一幅に形成されているのが好ましい。スリット板6の背後から照明されてスリット16を通過したスリット照明光をレンズ系9、12によって角膜上で約2.5μm程度の幅のスリット像となるように縮小する。そして、この反射光をレンズ系12、11によって拡大する。また、スリット板6の回転によって第一スリット16aが移動するのに伴ってスリット照明光は被検眼Eの前眼部上を移動する。すなわち、スキャニングが行われる。そして、前眼部におけるスリット反射光も同様に移動するが、この移動するスリット反射光の移動方向及び移動速度に一致して第二スリット16bが移動している。このように移動するスリット16aを通過したスリット照明光の反射光がスリット16bを通過しうるように、照明光学系4におけるアフォーカル光学系15の光軸上には、適切に傾斜させた複数個のミラー18a、18b、18c、18dが配設されている。これらミラーの傾斜を適宜微変更することにより、スリット反射光が第二スリット16bを通過するようにスリット反射光の移動方向および傾斜方向を調整することができる。

スリット板6には停止マーク23が付されている。さらに、停止マーク23を検出するための検出器24が配設されている。この検出器24が停止マーク23を検出した時点でスリット板6の回転を停止するためである。こうすることにより、検出器24を作動させれば常にスリット板6をその所定の回転角度位置で停止させることができる。換言すれば、特定のスリットが特定の位置に来たときにスリット板6を停止させ得る。また、公知の手段により、上記モータ17の回転速度を連続的に変化させることができ、また、モータ17の出力を手動制御することによってスリット板6を任意角度に停止させることができる。このように構成することにより、操作者が任意に角膜上のスリット反射光の照射位置を決定することができる。換言すれば、角膜上の任意の位置を反射光量比の測定位置として選択することができる。

とくに、スリットを特定せずに、いずれかのスリットが一定の位置に位置するようにスリット板6の回転を停止させるには、上記検出器24が不特定のスリットを検出するようにすればよい。しかし、スリット板6上に形成されるスリットはどうしてもその形状寸法や傾き等に製作誤差が生じるため、停止マーク23を配設して特定のスリットを特定の位置に停止させるのが好ましい。

図1に示すごとく、撮影光学系5におけるリレーレンズ群21はリレーレンズ間の光束を平行光とするアフォーカル光路25を構成している。このアフォーカル光路25から分岐して測定光学系3が配設されている。測定光学系3は、上記アフォーカル光路25上に配置されたハーフミラー26、絞り27および光量測定手段としての光電素子28がその順に配置されたものである。光電素子としてはフォトマルチプライヤが好適に用いられる。測定光学系5は、スリット板6の回転を停止させ、スリット反射光をそのまま光電素子28によって受光して反射光量を測定するものである。したがって、測定時には、スリット反射光が通過する第二スリット16bがちょうど視野絞り20の範囲内に停止するように設定される。

本測定装置1には如上の光学系の作動を制御する制御部31とこの制御部31を操作するための操作部32とを備えた制御装置33が配設されている。また、得られた角膜細胞像や反射光量比などを表示するためのモニタ34が配設されている。制御部31は、撮像や光量測定のためのキセノンランプ7の点灯、スリット板6の回転および回転停止、撮像素子10によって得た情報の画像処理、光電素子28によって得た情報の定量化による反射光量比の演算、エンコーダ22bからのフォーカシング位置情報の処理などを行う。

本測定装置1の作動を説明する。

まずキセノンランプ7を点灯しておく。つぎに、被検者に図示しない固視灯を固視させて被検眼を固定する。検査者は手動によって装置1を被検眼に対して位置合わせし(アライメント)、コーンレンズ13を被検眼の角膜面に当接する。ついで、スリット板6を高速回転する。第一スリット16aを通過したスリット照明光は被検眼の角膜面をスキャンし、移動するそのスリット反射光は同期して移動する第二スリット16bを通過する。図4(a)に示すように、その後スリット反射光Rは視野絞り20を通過して撮像素子10によって受光される。撮像素子10は高速移動するスリット反射光を視野絞り20の範囲で受光するので、図4(b)に示すように視野絞り20の範囲で角膜細胞を撮影する。この角膜細胞像Gはモニタ34の画面上で観察することができる。また、角膜細胞像Gは各種医療診断に用いることができる。

次に、角膜の反射光量比を測定する。測定者が反射光量比を測定すべく制御装置33の操作部32を操作すると、スリット板6はその回転を停止する。この場合、前述した検出器24がスリット板の停止マーク23を検出することによって回転が停止するので、図4(c)に示すごとく視野絞り20の範囲内の特定の位置に特定のスリット16bが停止する。上記特定の停止位置は、被検眼の角膜面における特定位置と対応している。この角膜面における特定位置が反射光量比を測定しようとする位置である。この位置は停止マーク23の位置を変えることによって任意に選択することができる。たとえば、対物光学系12の光軸に対応する位置としてもよい。また、検査者が視野絞り20の範囲の上記角膜細胞像Gをモニタ画面で観察(図5の表示A)してスリットの停止位置を大まかに決めておき、ついで、手動操作によってスリット板6をゆっくり回転させて所望の上記位置にスリットを停止させてもよい。

反射光量の測定に先立って対物光学系12の焦点の位置は原点Fに復帰している。ついで光電素子28が作動し、フォーカシングレンズ14が前進する。すなわち、光量を測定すべき角膜の各層に焦点Fが順次移動していき、各層におけるスリット反射光が受光される。このとき、フォーカシングレンズ14の移動距離はエンコーダ22bによって測定され、制御部31に位置情報として記憶される。制御部31においては、かかる位置情報と各位置に対応する反射光量が演算され、その結果がモニタ34の画面に表示される。

図5に示すのが、モニタ34の画面に表示される測定結果である。表示Aとして、視野絞り20の範囲内に角膜におけるある層の角膜細胞像Gが表示される。さらに、表示Aの枠には測定光学系3の光軸位置を示すマークDを付している。

表示Bとして、角膜の厚さ方向に沿った各層の検出光量が表示される。縦軸は検出光量を示しているが、その数値は無単位であるので、表示Bはいわば反射光量の分布を示すものといえる。反射光量比は各測定点の光量データを選択して演算される。表示Bにおける横軸は位置情報であり、焦点の原点Fを0として250μmまでを表示範囲としている。一般的な人間の角膜厚さは平均的に500μm程度であり、測定範囲もこれに応じて増減することができる。250μmを超える測定結果は画面を横方向にスクロールすることによって表示が可能である。原点Fは記憶されており、また、原点Fからコーンレンズの先端面(被検眼の角膜表面の位置)までの距離も既知であるため、角膜の深さ位置とその反射光量比とを対応させることができる。

本実施形態ではいわゆる接触型の装置を例にとって説明したが、本発明では接触型には限定されず、下記のいわゆる非接触型の撮影装置に反射光量比測定機能を備えたものも含む。

図6に概略的に示すように、非接触式の反射光量比測定装置41においても制御装置33およびモニタ34を備えている。一方、各光学系はXYZ軸の各方向に移動させうる機枠42上に搭載されており、後述するように測定動作に入る前に装置41と被検眼Eとの位置合わせ(アライメントと合焦)を行うことができる。さらに、対物光学系43がXYZの三軸方向それぞれに移動しうる追随架台43aに搭載されており、後述するように、反射光量の測定中に被検眼Eの変位に応じてXYそれぞれの方向の位置合わせを維持しながらZ方向へ移動して対物光学系43の焦点を移動させることができる。このように移動対象を軽量化することにより、反射光量比を測定する際に位置合わせの維持および焦点移動が迅速になされ得る。

図1の装置1と同様に、照明光学系44と撮影光学系45とにまたがる同様のスリット板6が配設されている。スリット板6の対物光学系側に照明レンズ9と撮影レンズ11とが配設されており、対物光学系43から照明レンズ9および撮影レンズ11にいたる光路46a、46bはそれぞれアフォーカルにされている。すなわち、対物光学系43と両レンズ9、11の間の光学系がアフォーカル光学系である。符号40で示すのは、スリット反射光が前述の第二スリットを通過するようにスリット反射光の移動方向および傾斜方向を調整するためのミラー群である。

機枠42上には図示しないアライメント光学系が配設されている。また、上記の各アフォーカル光路46a、46bからそれぞれ分岐して合焦光学系48が配設されている。アライメント光学系の作動によって機枠42をXY方向に移動させ、対物光学系43の光軸を被検眼Eの頂点(被検眼の装置41に最も近い点)に一致させるのがアライメントである。また、対物光学系43は固定した焦点を有している。そして、合焦光学系48の作動によって機枠42をZ軸方向に移動させ、上記焦点を自動的に被検眼の表面に一致させるのがZ軸方向の位置合わせ、すなわち合焦である。

アライメント光学系は、たとえば、アライメント指標光を照射する光源と、前眼部を観察撮影するテレビカメラ等の観察手段を備えたものである。そして、上記アライメント用光源からの近赤外光が被検眼の前眼部にその正面から照射される。アライメント指標光の被検眼の角膜における反射像たる輝点(プルキンエ像)は、上記テレビカメラにおいて前眼部像に重ねて撮影される。フィードバック制御により、被検眼Eの変位に応じてこのプルキンエ像を撮影光軸、たとえば対物光学系の光軸に対応する位置に至らせるように上記機枠42をXY方向に移動させる。こうすることによって撮影光軸を被検眼の角膜頂点に一致させる。以上がアライメント機構およびアライメント動作の一例である。

合焦光学系48は赤外光を発光する合焦用ランプ49と合焦光用の可動スリット50とを主要構成要素としている。図中の符号53はミラーである。合焦光学系48は、合焦用ランプ49から可動スリット50を通過したスリット光の被検眼Eにおける反射光(合焦用スリット光)を検出する光センサ51を主要構成要素としている。光センサ51としてはラインセンサやエリアセンサが用いられる。可動スリット50を通過したスリット光が所定の基準位置にある対物光学系43の焦点で反射され、この合焦用スリット光が光センサ51における所定部位に受光されたとき、制御部31において合焦されたと判断される。この合焦状態を維持すべくフィードバック制御により、被検眼Eの変位に応じて機枠42がZ軸方向に移動する。
