Description
本発明は、個体における敗血症またはその進行段階を診断または予測する方法に関する。本発明はまた、個体における全身性炎症反応症候群を診断する方法にも関する。
疾病状態の早期発見は、一般に、より効果的な治療上の処置を可能にし、それに相応して臨床転帰がより好転する。しかしながら、多くの場合、病徴の早期発見は難しい問題であり、そのため、診断が可能となる前に疾患が相対的に進行してしまうことがある。全身性炎症状態はこのような疾患クラスの1つである。これらの病的状態、特に敗血症は、一般に、宿主において過剰で無調節な炎症反応を引き起こす病原微生物と宿主の防御系との間の相互作用によって起こる。全身性炎症反応の際の宿主の反応の複雑性が疾患の病原性の理解に向けた取り組みを込み入らせてきた。(Healy, Annul. Pharmacother. 36: 648-54 (2002)における総説。) 疾患の病原性の理解が不十分であることが、診断用バイオマーカーの発見を難しくしている一因である。しかしながら、敗血症は生命を脅かすような状態へと著しく急速に進行するため、早期の信頼性の高い診断が強く要請されている。
敗血症の早期診断における困難性が、この疾患に関する罹患率と死亡率の高さに反映されている。敗血症は、現在、米国の死因の第10位であり、さらに、非冠動脈疾患用集中治療室(ICU)にいる入院患者の間で特に有病率が高く、そこでの最も一般的な死因である。全体的な死亡率は35%という高さであり、米国だけで年に推定750,000症例が発生している。敗血症治療の年間コストは、米国だけでおよそ数十億ドル程度である。
本発明は、単一時点で生体サンプルから採取した2つ以上のバイオマーカーを測定することによって、敗血症を正確に、迅速に、かつ、感度良く予測および診断することを可能にする。これは、ある個体、特に敗血症を発現する危険性がある個体、敗血症に罹患している個体または敗血症に罹患している疑いがある個体から単一時点でバイオマーカープロフィールを得ること、およびその個体のバイオマーカープロフィールを参照バイオマーカープロフィールと比較することにより、実現できる。参照バイオマーカープロフィールは、例えば、敗血症に苦しんでいるか、または敗血症を発症しているかもしくは敗血症の特定の進行段階にある個体の集団(「参照集団」)から得られる。その個体からのバイオマーカープロフィールが、参照集団からのバイオマーカープロフィールの適切な固有の特徴を含んでいる場合には、その個体はその参照集団と同様に敗血症になる可能性が高いか、敗血症に苦しんでいるか、または敗血症の特定の進行段階にあると診断される。参照バイオマーカープロフィールは、SIRSを患っている個体、または感染しているがSIRSを患っていない個体をはじめとする個体の種々の集団からも得られる。よって、本発明は、とりわけ、SIRSに罹患していない患者、SIRSに罹患しているが試験期間内に敗血症を発現しそうにない患者、敗血症に罹患している患者、または最終的に敗血症になる危険性がある患者を医師が判定できるようにする。
本発明は、個体から単一時点で(「スナップショット」)または疾患の進行経過の間に得た1以上の生体サンプルを用いて、敗血症を迅速に、感度良く、かつ正確に診断または予測することを可能にする。有利には、敗血症は臨床症状の発症前に診断または予測され、それにより、より効果的な治療的介入が可能になる。
・体温が38℃(100.4゜F)より高いかまたは36℃(96.8゜F)より低いこと;
・心拍数(HR)が90拍/分を越えること;
・呼吸数(RR)が20呼吸/分を越えるか、またはPCO2が32 mmHg未満であるか、または人工呼吸器を必要とすること;および
・白血球数(WBC)が12.0×109細胞/Lを超えるかもしくは4.0×109細胞/L未満であるか、または未熟顆粒球(immature form (bands))が10%を超えていること。
1つの実施形態によれば、本発明の方法は、個体から採取した生体サンプルからバイオマーカーのプロフィールを得ることを含む。生体サンプルは、血液、血漿、血清、唾液、痰、尿、脳脊髄液、細胞、細胞抽出物、組織サンプル、組織生検材料、糞便サンプルなどであってよい。参照バイオマーカープロフィールは、例えば、SIRS陰性個体、SIRS陽性個体、敗血症の発症に苦しむ個体、およびすでに敗血症に罹患している個体からなる群から選択される個体の集団から得られうる。すでに敗血症に罹患している個体の参照バイオマーカープロフィールは、感染、菌血症、重度敗血症、敗血性ショックまたはMODなどの敗血症の任意の進行段階で得られる。
1つの実施形態では、レーザー脱離イオン化飛行時間型質量分析法を利用してバイオマーカーのプロフィールを作成してもよい。この場合、バイオマーカーはタンパク質またはタンパク質断片であり、これらを入射レーザー光によりイオン化し、固定している担体から気化させた。その後、その質量対電荷(「m/z」)比率に応じた各タンパク質固有の飛行時間によりプロフィールを得る。種々のレーザー脱離/イオン化技術は当技術分野で公知である。(例えば、Guttman ら, Anal. Chem. 73: 1252-62 (2001)およびWei ら, Nature 399: 243-46 (1999)を参照。)
レーザー脱離イオン化飛行時間型質量分析法では、比較的短時間に大量の情報を得ることが可能である。生体サンプルを、サンプル中の全てのバイオマーカーまたはそのサブセットと結合する数種類の担体のうちの1つに適用する。細胞溶解物またはサンプルを、事前に精製もしくは分画して、または事前に精製もしくは分画せずに、0.5μL程度の少ない量でこれらの表面に直接適用する。担体表面に適用する前に溶解物またはサンプルを濃縮または希釈してもよい。その後、レーザー脱離/イオン化を利用して、3時間ほどでサンプルの質量スペクトルが得られる。
1つの実施形態では、個体のバイオマーカープロフィールと参照バイオマーカープロフィールとの比較は、判定ルールを適用することを含む。この判定ルールは、コンピューターのパターン認識アルゴリズムなどのデータ解析アルゴリズムを含んでもよい。他の好適なアルゴリズムとしては、限定されるものではないが、ロジスティック回帰または特徴値の分布における差異を検出するノンパラメトリックアルゴリズム(例えば、ウィルコクソンの符号付き順位検定)が挙げられる。判定ルールは、1つ、2つ、3つ、4つ、5つ、10、20、またはそれ以上の特徴に基づくものであってよい。1つの実施形態では、判定ルールは数百以上の特徴に基づくものである。判定ルールの適用が分類ツリーアルゴリズムを使用することも含んでよい。例えば、参照バイオマーカープロフィールは少なくとも3つの特徴を含み、それらの特徴が分類ツリーアルゴリズムにおける予測指標となる。データ解析アルゴリズムは、集団(またはクラス)の構成員を、少なくとも約60%、少なくとも約70%、少なくとも約80%および少なくとも約90%の精度で予測する。
本発明の方法は、敗血症またはSIRSを診断または予測するためのバイオマーカープロフィールを得ることにより実施することができる。プロフィールの作成は本発明を実施するのに十分であるため、プロフィールを構成するバイオマーカーが既知のものである必要はないし、または後に同定される必要もない。
バイオマーカーとしては、炎症プロセスに関与することが分かっているか、または炎症プロセスに関与することが明らかになったシグナル伝達因子も含んでよい。シグナル伝達因子は、受容体結合、受容体の活性化、細胞内キナーゼの活性化、転写因子の活性化、遺伝子転写および/または翻訳のレベルにおける変化、ならびに代謝プロセスにおける変化などをはじめとする事象の細胞内カスケードを開始するものである。シグナル伝達分子およびこれらの分子によって活性化されるプロセスは、本発明においては、まとめて「敗血症経路に関与する生体分子」と定義される。関連した予測バイオマーカーとしては、敗血症経路に関与する生体分子を含んでよい。
有用なバイオマーカーとしては、まだ同定されていないバイオマーカーまたは該当する生理学的状態に関連するバイオマーカーがあると思われる。本発明の1つの態様において、有用なバイオマーカーは生体サンプルのバイオマーカープロフィールの構成要素として同定される。このような同定は、イムノアッセイまたは自動微量配列分析をはじめとする当技術分野で周知のいずれかの手法によって行える。
1つの実施形態では、現在記述されている方法を使用して、特に敗血症を発現する危険性があるSIRS患者をスクリーニングする。生体サンプルをSIRS陽性患者から採取し、サンプル中のバイオマーカーのプロフィールを最終的に敗血症へと進行したSIRS陽性個体の参照プロフィールと比較する。患者のバイオマーカープロフィールが、敗血症へと進行したSIRS陽性集団の参照プロフィールと合致するとして分類されることにより、SIRS陽性患者が同様に敗血症へと進行するであろうとの診断がなされる。その後、敗血症の進行を予防または回避するために治療計画が開始され得る。
1.1. 取得及び分析するサンプル
2つの患者集団についての参照バイオマーカープロフィールを確立した。第1集団(「SIRS群」)は、SIRSを発現していた患者であって、「第1日」に本試験を開始したが患者の入院期間には敗血症へと進行しなかった患者20人であった。第2集団(「敗血症群」)は、同様にSIRSを発現していた患者であって、「第1日」に本試験を開始したところ試験開始の少なくとも数日後には敗血症へと進行した患者20人であった。およそ24時間おきに血液サンプルを各試験群から採取した。従来の技術によって確定された、敗血症群における臨床的な敗血症の疑いは、「時点0」に生じたものとする。「−24時間時点」および「−48時間時点」は、それぞれ、敗血症群における敗血症の発症の臨床的疑いが生じるよりも約24時間および約48時間前に採取されたサンプルを表す。すなわち、敗血症群からのサンプルとしては、試験開始日(第1日)に採取されたサンプル、敗血症の臨床的疑いが生じるよりも約48時間前に採取されたサンプル(−48時間時点)、敗血症の臨床的疑いが生じるよりも約24時間前に採取されたサンプル(−24時間時点)および敗血症の発症の臨床的疑いが生じた日に採取されたサンプル(時点0)があった。敗血症群の患者20人からの80サンプルとSIRS群の患者20人からの80サンプルとの合計160個の血液サンプルを分析した。
血漿中では、かなりの数の小分子がタンパク質と結合している可能性があり、その結果、パターン生成法により検出される小分子の数が減少する可能性がある。そのため、タンパク質と結合し得る小分子を放出した後に血漿サンプルから大部分のタンパク質を除去した。タンパク質の除去に好適な方法としては、限定されるものではないが、氷冷メタノール、アセトニトリル(ACN)、ブタノールもしくはトリクロロ酢酸(TCA)での血漿の抽出、または熱変性および酸加水分解が挙げられる。この実施例では、血漿を氷冷メタノールで抽出した。最も多数の小分子が検出されたため、メタノール抽出が好ましい方法であった。各血漿サンプルの50μLを100μL氷冷100%メタノールと混合し、メタノールの最終容量パーセントを67%とした。その溶液を60秒間ボルテックス攪拌した。次いで、サンプルを4℃にて20分間インキュベートし、12,000rpmにて10分間の遠心分離によりタンパク質を沈降させた。上清を除去し、乾燥させ、50μLの水に再懸濁した。LC/MS分析の前に、抽出した血漿サンプルに、2種類の低分子量分子、スルファクロロピリダジンおよびオクタデシルアミンを添加した。これらの分子を内部標準として用いて、イオン強度および保持時間を正規化した。スルファクロロピリダジンは、MS測定ではm/zが285.0 Daであり、LC測定では44%のACNで溶出する。オクタデシルアミンは、m/zが270.3 Daであり、89%のACNで溶出する。
再懸濁した上清10μLを2.1×100 mm C18 Waters Symmetry LCカラム(粒径=3.5μm;内部孔径=100Å)に注入した。次いで、そのカラムを0.1%ギ酸中のACNの3段階の線形勾配を用いて25℃の温度にて300μL/分で溶出した。t=0〜0.5分の間は、ACN濃度を9.75%〜24%とし、t=0.5〜20分の間は、ACN濃度を24%〜90.5%とし、そして、t=20〜27分の間は、ACN濃度を90.5%〜92.4%とした。上記の試験条件を本明細書において「LC試験条件」という。LC試験条件下では、スルファクロロピリダジンは44% ACNで保持時間6.4分にて溶出し、オクタデシルアミンは、89% ACNで保持時間14.5分にて溶出した。次いで、LCにより分画されたサンプルを、Agilent MSD1100 四重極質量分析計をLCカラムと縦一列に連結して使用するESI-MS(LC/ESI-MS)に付した。キャピラリー電圧を4000Vとし、陽イオンモードで、質量/電荷比(m/z)が100または150〜1000Daに及ぶイオンについての質量スペクトルデータを得た。各サンプルにつき3回、LC/ESI-MS分析を実施した。データは、各イオンのm/z(単位:ダルトン)と保持時間(単位:分)(「m/z、保持時間」)として表されるが、ここで、イオンの保持時間とは、線形ACN勾配中での逆相カラムからの溶出に要する時間である。しかしながら、稼動毎の保持時間のわずかな変動を考慮して、データを、実験上の変動により大きな影響を受けないイオンの本来的性質である、m/zとC18カラムからイオンが溶出するACNの%として表すこともできる。保持時間と溶出時のACN %との関係は、以下の方程式により表される:
0<t<0.5については、 ACN % = 28.5t + 9.75;
0.5<t<20については、 ACN % = 3.4103 (t-0.5) + 24;および
20<t<27については、 ACN % = 0.27143 (t-20) + 90.5。
各血漿サンプルからの数百のスペクトル上の特徴を分析した。スペクトル間で類似する特徴を並べた。アラインメントアルゴリズムの選択は本発明にとって重要ではなく、当業者はこの目的のために使用することができる種々のアラインメントアルゴリズムについて認識している。合計で4930のスペクトル上の特徴を分析した。この実施例において「特徴」とは、特定のイオンに対応する「ピーク」と互換的に使用される。異なる5人の個体から得たサンプル由来の代表的なピークを表1に示す。第1列では、各イオンのm/zと溶出時のACNのパーセンテージのそれぞれを、括弧内に列挙している。残りの列には、各患者由来の対応するイオンの正規化された強度を示す。その正規化強度は、それらの強度を2つの内部標準の強度に対して正規化することにより決定した。400を超えるピークの平均正規化強度は0.1を上回っていた。
平均イオン強度の比較により、SIRS患者と敗血症患者との間での各イオン強度における差異が効果的に強調される。1800を超える正規化イオン強度を敗血症群とSIRS群について別々に平均した。敗血症群またはSIRS群のいずれかにおいて平均正規化強度が0.1未満のイオンは、両群のプロフィールにおける正規化強度が0.1を上回るイオンとは別に分析した。正規化強度が0.1を上回る約400のイオンについて、平均正規化強度のSIRS群に対する敗血症群の比率を決定した。これらのイオンの相対強度比率の分布を図3に示す。
調べたバイオマーカープロフィールから、個体が敗血症の発症に向かって進むにつれてますます高いレベルでそしてより低いレベルで現れた特徴が、示された。これらの特徴に対応するバイオマーカーは、個体における感染および/または炎症に対する生理学的応答の指標であると予測される。上述の理由で、これらのバイオマーカーは、個体における敗血症状態またはSIRSを判定するための特に有用な予測指標を提供するものと思われる。すなわち、個体由来の異なる生体サンプルから得られるプロフィールにおけるこれらの特徴を比較することにより、個体が重度敗血症へと進んでいるのかどうか、またはSIRSが正常な状態へと向かっているのかどうかが明らかになると考えられる。
判定ルールが比較的少数のバイオマーカープロフィールからの多数の特徴に基づくものである場合には、選択のバイアスが、判定ルールをもたらす特徴の同定に影響を及ぼし得る。(Ambroise ら, Proc.Nat'l Acad. Sci. USA 99: 6562-66 (2002)を参照。)データを利用して特徴を選択する際には、選択のバイアスが生じる可能性があり、そして、選択プロセスにおける変動を考慮することなしに、選択された特徴に条件付けされたパフォーマンスが算出される。その結果、分類精度が過大評価されることとなる。判定ルールがランダムな入力パラメーターに基づいたものである場合でさえも、選択のバイアスについて補正しなければ、分類精度は100%に達することがありうる(上掲の文献)。パフォーマンス算出プロセスに特徴の選択を含めることにより、そのパフォーマンス算出プロセスが10分割交差検定であろうとブートストラップ法であろうと、選択のバイアスを避けることができよう。(例えば、参照により本明細書に組み入れられる、Hastie ら, 上掲、第7.10節-第7.11節を参照。)

実際のクラスについてクラス確率が高い場合には逸脱度を最小にする。2つのモデルが所与のデータに対して同じ予測を与えることがあり、さらに、好ましいモデルの予測逸脱度はより小さいであろう。10分割交差検定での10回の繰り返しの各回について、その繰り返しの回において当て嵌めるモデルから外れるケースについての予測逸脱度が算出される。その結果として、10のバイアスのない逸脱度が得られる。一般に、これらの10の逸脱度を合計して、全データセットに対するモデルパフォーマンス(すなわち、精度)の全体の概要を作成する。実際には10の異なるモデルを適合させたため、交差検定では特定モデルのパフォーマンスは判明しない。むしろ、10のモデルは共通のモデリングプロセスによって作成されたものであることから、交差検定はこのプロセスのパフォーマンスを立証するものであった。このプロセスで生じた11番目のモデルは、最初の10のモデルの予測パフォーマンスと類似の予測パフォーマンスを有すると思われる。10分割交差検定を用いることにより、通常、モデルパフォーマンスは100%未満となる。しかし、トレーニングセット以外のサンプルから得られたバイオマーカープロフィールに判定ルールを適用する場合、10分割交差検定後に得られたパフォーマンスは、その判定ルールの生物学的に重要な予測精度をより厳密に反映していると考えられる。
このデータを解析するための1つのアプローチが、大きなデータセットにおいてパターンと関係性を検索する分類ツリーアルゴリズムを使用することである。「分類ツリー」とは、特定の患者をクラスの1つに正確に位置づけるように設計された一連の質問を用いて、その特定の患者を特定クラス(例えば、敗血症またはSIRS)に分類するための再帰分割(recursive partition)である。各質問では患者の状態が所与の予測指標を満たしているかどうかを尋ね、その各回答により、その患者が分類されるクラスが決定するまで、ユーザーは分類ツリーの下方へと誘導される。本明細書において、「予測指標」とは、固有のm/zとACN中でのC18カラムからの溶出プロフィールを有する一イオンの特徴値(本実施例では、イオン強度)の範囲である。「状態」とは、その個体のバイオマーカープロフィールにおいて測定される特徴の単一の特定の値である。この実施例では、「クラスネーム」は敗血症とSIRSである。すなわち、分類ツリーユーザーは、まず、個体のバイオマーカープロフィールにおいて測定される第1のイオン強度が第1イオンの予測範囲の所定の範囲内であるかどうかを尋ねる。最初の質問に対する回答は、個体がSIRSまたは敗血症に罹患しているかどうかを判定する上で方向を決定するかもしれない。また、最初の質問に対する回答は、ユーザーをさらに誘導し、個体のバイオマーカープロフィールにおいて測定される第2のイオン強度が第2のイオンの予測範囲の所定範囲内であるかどうかを尋ねるかもしれない。さらに、2番目の質問に対する回答により方向が決定するかもしれないし、患者の分類が最終的に決定されるまで、2番目の質問に対する回答はユーザーをさらに分類ツリーの下方へ誘導するかもしれない。
多重加法回帰ツリー(MART)を用いる自動化フレキシブルモデリング技法を利用して、特徴のセットを2つの集団のうちの一方に属するように分類した。MARTモデルでは、全ての予測に適用する定数を指定する開始オフセット、続いて、一連の回帰ツリーを使用する。そのフィッティングは、各ツリーにおける判定ポイントの数、フィッティングするツリーの数、および特定のツリーがMARTモデルに根本的に影響を及ぼし得る程度を特定する「精度定数」によって特定される。各繰り返しでは、回帰ツリーを適用して、フィッティング基準の最も急降下する方向を推定する。精度定数により指定される長さを有する工程がその方向で選択される。次いで、MARTモデルは、開始オフセットと、回帰ツリーによりもたらされる工程で構成される。実測値と予測値との間の差異が再計算され、そのサイクルが再び進行し、それにより予測の改良がますます進む。このプロセスは、設定した回数のサイクルについて、またはなんらかの停止基準に当てはまるまで、継続する。
ロジスティック回帰は、上述のLC/MS分析からのデータストリームを解析するさらに別の手段を提供する。「ピーク強度」は、所与のm/z位置においてスペクトルに現れるピークの高さで示される。所与のm/z位置にピークが存在しなければ、割り当てられるピーク強度は「0」となる。次いで、SIRS集団と敗血症集団を合わせたスペクトルから、所与のm/z位置からのピーク強度の標準偏差(SD)を得る。SIRS集団と敗血症集団との間にピーク強度の変動がない(すなわち、SD=0)場合には、それ以上ピーク強度を考慮しない。回帰分析の前に、当技術分野で周知の方法を用いてピーク強度をスケーリングする。スケーリングアルゴリズムは、Hastie ら, 上記、第11章に概説されている。
さらに別の方法では、ウィルコクソンの符号付き順位検定などのノンパラメトリック検定を利用して、目的のバイオマーカーを各々同定することができる。バイオマーカープロフィールにおける特徴には、バイオマーカーを使用して個体を特定の参照集団に属するものとして分類することができることについてその確実性の程度を示す「p値」が割り当てられる。一般に、予測的価値を有するp値は約0.05未満である。個体を分類するためにp値の低いバイオマーカーを単独で使用することができる。あるいは、個体を分類するために2つ以上のバイオマーカーの組合せを使用してもよく、この際、その組合せはバイオマーカーの相対p値に基づいて選択される。一般に、p値の低いバイオマーカーが、バイオマーカーの所与の組合せに好ましい。このようにして、個体を分類するために少なくとも3つ、4つ、5つ、6つ、10、20もしくは30またはそれ以上のバイオマーカーの組合せを使用することもできる。当業者ならば、所与の任意のバイオマーカーの相対p値が参照集団の大きさに応じて異なることは分かるであろう。
2.1. 取得及び分析するサンプル
上述のように、参照バイオマーカープロフィールを、15人の患者からなる第1集団(「SIRS群」)と、SIRSを発現していた患者で敗血症に進行した15人の患者からなる第2集団(「敗血症群」)とから得た。第1日、時点0および−48時間時点で患者から採血した。この際、患者由来の血漿サンプル50〜75μLを4バッチ分(SIRS陽性である5個体分と10個体分の2バッチ分、そして敗血症陽性である5個体分と10個体分の2バッチ分)、プールした。プールした各バッチからの6サンプルをさらに分析した。
血漿サンプルについて、まず、多量のタンパク質、具体的に言えば、全体でサンプル中のタンパク質の約85%(wt%)を構成するアルブミン、トランスフェリン、ハプトグロブリン、抗トリプシン、IgGおよびIgAを除去するために、免疫枯渇させた。マルチプルアフィニティー除去システムカラム(Agilent Technologies、Palo Alto、California)を製造業者の教示に従って用い、免疫枯渇を行った。このシステムを使用して、血漿サンプルから上記の6種類のタンパク質の少なくとも95%を除去した。例えば、枯渇させたサンプル中にはアルブミンは約0.1%しか残留しなかった。サンプル中に残ったおよそ8%に過ぎないタンパク質は、IgMおよびα-2マクログロブリンなどの存在量の多いタンパク質の残存物であった。次いで、分画した血漿サンプルを当技術分野で周知の方法を用いて、変性させ、還元し、アルキル化し、トリプシンで消化した。プールした各サンプルから、消化されたタンパク質約2mgを得た。
次に、トリプシン消化後のペプチド混合物をLCカラムを用いて分画し、LC/MS/MSの配置で構成したAgilent MSD/トラップESI-イオントラップ質量分析計により分析した。消化したタンパク質1mgを10μL/分にてマイクロフローC18逆相(RP1)カラムに通した。RP1カラムを強陽イオン交換(SCX)分画カラムに縦列に連結し、次に、このカラムをC18逆相トラップカラムに縦列に連結した。サンプルを第1の0〜10%ACN勾配中のRP1カラムに通し、そのRP1カラムでペプチドを分画した。ACN勾配の後、10mM塩バッファー溶出を行い、それにより、ペプチドを、SCXカラムに固定されている画分とトラップカラムに固定された溶出画分とにさらに分画した。その後、トラップカラムを、SCXカラムとのその機能的連結から取り外し、別のC18逆相カラム(RP2)と機能的連結をするように配置した。トラップカラムに固定された画分を0〜10% ACN勾配を用いて300nL/分にてトラップカラムからRP2カラムへと溶出した。RP2カラムはスプレー電圧1000〜1500Vで操作するAgilent MSD/トラップESI-イオントラップ質量分析計と機能的に連結した。その後、このサイクル(RP1-SCX-トラップ-RP2)を繰り返して、全体で0〜80%の範囲のACN %および最大1Mの塩濃度を用いて残留ペプチドを分画し、分離した。本発明に有用なバイオマーカープロフィールを得るためにLC/MS/MSに好適な他の構成を使用してもよい。質量スペクトルは200〜2200 Daの範囲のm/zにおいて得た。データ依存スキャンおよびダイナミック・エクスクルージョン(dynamic exclusion)を適用してより高いダイナミックレンジを達成した。図6はLC/MSおよびLC/MS/MSにより得られた代表的なバイオマーカープロフィールを示す。
MS/MSモードで分析した各サンプルについて、約1.5ギガバイトの情報に相当する約150,000スペクトルを得た。合計で、およそ50ギガバイトの情報を収集した。スペクトルをスペクトルミル(Spectrum Mill)v 2.7 ソフトウェア((C) Copyright 2003 Agilent Technologies、Inc.)を使用して解析した。MS/MSスペクトルとヒト非冗長タンパク質の米国国立バイオテクノロジー情報センター(National Center for Biotechnology Information)(NCBI)データベースに対して、MS-Tag データベース検索アルゴリズム(Millennium Pharmaceuticals)を使用し、MS/MSスペクトルのマッチングを行った。95%信頼に相当するカットオフスコアを使用して、一致したペプチドを検証し、次いで、これらを集めてサンプル中に存在するタンパク質を同定した。本方法を用いて検出可能であったタンパク質は血漿中に約1ng/mLの濃度にて存在し、これは約6桁のオーダーの血漿濃度でのダイナミックレンジをカバーする。
Claims (23)
- SIRS患者における敗血症の発症を予測するためのデータを得る方法であって、
(a) SIRS患者から採取した第1の血液サンプルから得られたバイオマーカープロフィールにおけるイオンの質量分析(MS)イオン強度を測定すること;および
(b) 該バイオマーカープロフィールにおけるイオンのMSイオン強度を、敗血症に進行するSIRS−陽性患者集団において敗血症発症の0−48時間前に採取した血液サンプルから得られた第1の参照プロフィールの特徴と比較することを含み、
ここで、該特徴は第1の参照プロフィールに存在するイオンのMSイオン強度に対応するものであり、
比較される第1の参照プロフィールにおけるMSイオン強度は敗血症に進行しないSIRS陽性患者集団から採取した血液サンプルから得られた第2の参照プロフィールに存在するイオンのMSイオン強度とは異なっており、
該バイオマーカープロフィールにおけるMSイオン強度が第1の参照プロフィールに対応するとき、敗血症が該SIRS患者に予測される、上記方法。 - MSイオン強度が、チロキシン結合グロブリン前駆体、イソクエン酸デヒドロゲナーゼ、
α2-HS糖タンパク質、血漿ゲルソリン前駆体、アポリポタンパク質A-II前駆体、SAAB1β、アポリポタンパク質H前駆体、ロイシンリッチなα-2-糖タンパク質、プラスミノーゲン前駆体、または補体成分C1q鎖B前駆体を含むタンパク質からのイオンを含み、第1の参照プロフィールにおけるMSイオン強度が第2の参照プロフィールに存在するイオンのMSイオン強度よりも大きいか、または、
MSイオン強度が、ヒスチジン−プロチンリッチな糖タンパク質、セリン(またはシステイン)プロテイナーゼ阻害剤、クレードC、インスリン様増殖因子結合タンパクの酸不安定性サブユニット、ペプチドグリカン認識タンパク質L前駆体、ケラチン硫酸プロテオグリカンルミカン、アポリポタンパク質C-III、アポリポタンパク質C-I、フォークヘッドホモログ、nGAP様タンパク質、またはUDP-GlcNAc:α-1,3-D-マンノシドβ-1,4-N-アセチルグルコサミニルトランスフェラーゼIVを含むタンパク質からのイオンを含み、第1の参照プロフィールにおけるMSイオン強度が第2の参照プロフィールに存在するイオンのMSイオン強度よりも小さい、請求項1に記載の方法。 - MSイオン強度が以下のタンパク質:
チロキシン結合グロブリン前駆体、イソクエン酸デヒドロゲナーゼ、α2-HS糖タンパク質、血漿ゲルソリン前駆体、アポリポタンパク質A-II前駆体、SAAB1β、アポリポタンパク質H前駆体、ロイシンリッチなα-2-糖タンパク質、プラスミノーゲン前駆体、補体成分C1q鎖B前駆体、ヒスチジン−プロチンリッチな糖タンパク質、セリン(またはシステイン)プロテイナーゼ阻害剤、クレードC、インスリン様増殖因子結合タンパクの酸不安定性サブユニット、ペプチドグリカン認識タンパク質L前駆体、ケラチン硫酸プロテオグリカンルミカン、アポリポタンパク質C-III、アポリポタンパク質C-I、フォークヘッドホモログ、nGAP様タンパク質、またはUDP-GlcNAc:α-1,3-D-マンノシドβ-1,4-N-アセチルグルコサミニルトランスフェラーゼIVの少なくとも5種からのイオンを含む、請求項2の方法。 - バイオマーカープロフィールにおけるイオンが陽イオンモードでエレクトロスプレーイオン化質量分析により検出される約100ダルトン〜約1000ダルトンの質量対電荷比(m/z)を有している、請求項1に記載の方法。
- 血液サンプルが血清または血漿である、請求項1に記載の方法。
- さらに、
(c)SIRS患者から採取した第2の血液サンプルから得られた第2のバイオマーカープロフィールにおけるイオンのMSイオン強度を測定すること;および
(d)第2のバイオマーカープロフィールにおけるイオンのMSイオン強度を第1の参照プロフィールの特徴と比較すること;
を含む、請求項1に記載の方法。 - SIRS患者から第1の血液サンプルを採取した約24時間後に該SIRS患者から第2の血液サンプルを採取する、請求項6に記載の方法。
- 第1の参照プロフィールが、敗血症に進行するSIRS-陽性患者集団において敗血症発症の約24時間前に採取された血液サンプルから得られたものである、請求項1に記載の方法。
- 第1の参照プロフィールが、敗血症に進行するSIRS-陽性患者集団において敗血症発症の約48時間前に採取された血液サンプルから得られたものである、請求項1に記載の方法。
- 比較が判定ルールを適用することを含む、請求項1に記載の方法。
- 判定ルールの適用がデータ解析アルゴリズムを使用することを含む、請求項10に記載
の方法。 - データ解析アルゴリズムが分類ツリーの使用を含む、請求項11に記載の方法。
- データ解析アルゴリズムがノンパラメトリックである、請求項11に記載の方法。
- データ解析アルゴリズムが特徴値の分布における差異を検出する、請求項13に記載の方法。
- ノンパラメトリックアルゴリズムがウィルコクソンの符号付き順位検定を使用することを含む、請求項14に記載の方法。
- データ解析アルゴリズムが多重加法回帰ツリーを使用することを含む、請求項11に記載の方法。
- データ解析アルゴリズムがロジスティック回帰である、請求項11に記載の方法。
- データ解析アルゴリズムが少なくとも10個の入力パラメーターを含む、請求項11に記載の方法。
- 判定ルールが10分割交差検定を受けたものである、請求項11に記載の方法。
- 敗血症が少なくとも約60%の精度で予測されるものである、請求項1に記載の方法。
- 敗血症に進行するSIRS-陽性患者集団が少なくとも20個体を含んでいる、請求項1に記載の方法。
- バイオマーカープロフィールにおけるイオンのMSイオン強度が、イオン化質量分析(ESI-MS)、ESI-MS/MS、ESI-MS/(MS)n、マトリクス支援レーザー脱離/イオン化飛行時間型質量分析法(MALDI-TOF-MS)、表面増強レーザー脱離/イオン化飛行時間型質量分析法(SELDI-TOF-MS)、シリコン上脱離/イオン化(DIOS)、二次イオン質量分析法(SIMS)、四重極飛行時間型(Q-TOF)、大気圧化学イオン化質量分析法(APCI-MS)、APCI-MS/MS、APCI-(MS)n、大気圧光イオン化質量分析法(APPI-MS)、APPI-MS/MSおよびAPPI-(MS)n、四重極型質量分析法、フーリエ変換質量分析法(FTMS)、ならびにイオントラップ質量分析法からなる群から選択される質量分析法によって測定され、ここで、nは1以上の整数である、請求項1に記載の方法。
- 敗血症を発症する可能性のないSIRS患者を予測するためのデータを得る方法であって、
(a) SIRS患者から採取した第1の血液サンプルから得られたバイオマーカープロフィールにおけるイオンの質量分析(MS)イオン強度を測定すること;および
(b) 該バイオマーカープロフィールにおけるイオンのMSイオン強度を、敗血症に進行しないSIRS−陽性患者集団から採取した血液サンプルから得られた第1の参照プロフィールの特徴と比較することを含み、
ここで、該特徴は第1の参照プロフィールに存在するイオンのMSイオン強度に対応するものであり、
比較される第1の参照プロフィールにおけるMSイオン強度は敗血症に進行するSIRS−陽性患者から敗血症発症の0−48時間前に採取した血液サンプルから得られた第2の参照プロフィールに存在するイオンのMSイオン強度とは異なっており、
該バイオマーカープロフィールにおけるMSイオン強度が第1の参照プロフィールのMSイオン強度に対応するとき、該SIRS患者が敗血症を発症する可能性がないと予測される、上記方法。
