Description
The present invention relates to an image diagnosis support apparatus that performs image diagnosis support using nuclear medicine data of a subject obtained based on radiation generated from a subject to which a radiopharmaceutical is administered.
However, in a nuclear medicine diagnosis apparatus that obtains nuclear medicine data of a subject based on radiation generated from a subject to which a radiopharmaceutical is administered, that is, an ECT (Emission Computed Tomography) apparatus, the nuclear medicine data (for example, There is no computer diagnosis support device for PET (Positron Emission Tomography) images.
Furthermore, as an ECT device (nuclear medicine diagnostic device), taking a PET (Positron Emission Tomography) device as an example, there is no computer diagnosis support device for distinguishing between abnormal accumulation and normal accumulation in a PET image.
In general, when detecting a lesion from a medical image, if threshold processing is simply performed, sensitivity and specificity tend to be in a pair relationship. When the difference between the pixel value of the target location and the pixel value of the exception location is taken and falls below a preset pixel value threshold, the pixel value of the target location is the pixel value of the lesion location. Instead, the case where the pixel value is detected as a normal location and exceeds the threshold value of the pixel value will be described as an example in which the pixel value at the target location is detected as the pixel value at the lesion location.
When detection is performed with a lower pixel value threshold, the number of detected initial lesion candidates increases and the sensitivity increases, but false positives (which are judged to be abnormal although normal) are increased among the initial candidates. , The specificity goes down. On the other hand, when the pixel value threshold is increased and detected, the possibility that the detected initial candidate for the lesion is a lesion increases and the specificity increases, but false negatives (other than the initial candidate are normal but normal) The sensitivity decreases.
Here, the sensitivity is a rate (%) at which a test value performed on a population having a specific disease is positive (abnormal), and the sensitivity is represented by A / (A + B). The specificity is a rate (%) at which the test value performed on a population having no specific disease is negative (normal), and the specificity is represented by D / (C + D). A to D are sampling numbers of a group as shown in Table 1 below.
A PET image always has physiological normal accumulation in addition to abnormal accumulation. The point is how much this normal accumulation can be removed from the accumulation in consideration of the threshold value of the pixel value described above.
The present invention has been made in view of such circumstances, and an object thereof is to provide an image diagnosis support apparatus capable of providing image diagnosis support even in the field of nuclear medicine diagnosis.
In order to achieve such an object, the present invention has the following configuration.
That is, the invention described in
[Operation / Effect] According to the invention described in
In the field of nuclear medicine diagnosis, there is physiological normal accumulation in addition to abnormal accumulation as described above. Accordingly, not all of the accumulations detected by the above-described accumulation detection means are abnormal accumulations. Therefore, in the above-described invention, the part extraction means for extracting the data for nuclear medicine for each part of the subject, and the judgment standard for the part to which the target accumulation belongs among the judgment standards characterized for each part. And an accumulation determination unit that determines normal accumulation or abnormal accumulation from the accumulation detected by the accumulation detection unit for each region extracted by the region extraction unit based on the selected determination criterion .
In this case, the part extraction means extracts the nuclear medicine data for each part of the subject, and selects the judgment criterion of the part to which the target accumulation belongs among the judgment standards characterized for each part. Then , based on the selected determination criterion , the accumulation determination means determines normal accumulation or abnormal accumulation from the accumulation detected by the accumulation detection means for each part extracted by the part extraction means. In this way, data for nuclear medicine is extracted for each part of the subject, and among the judgment criteria characterized for each part, the judgment standard for the part to which the target accumulation belongs is selected and selected. Therefore, normal accumulation or abnormal accumulation is determined from the accumulation detected for each part based on the determination criteria , so that normal accumulation or abnormal accumulation is not determined only by the pixel value threshold as in the conventional case. Each characteristic normal physiological accumulation can be removed. As a result, normal accumulation that is false positive can be removed from the accumulation that is the initial candidate, and the specificity can be increased.
According to the image diagnosis support apparatus according to the present invention, it is possible to use the accumulation detection result for image diagnosis support by including the accumulation detection means for detecting accumulation in the subject by the radiopharmaceutical from the data for nuclear medicine. Thus, image diagnosis support can also be provided in the field of nuclear medicine diagnosis.
In addition, data for nuclear medicine is extracted for each part of the subject, and among the judgment criteria characterized for each part, the judgment standard for the part to which the target accumulation belongs is selected, and the selected judgment Based on the standard, normal accumulation or abnormal accumulation is determined from the accumulation detected for each region, so that each region is characterized without determining normal accumulation or abnormal accumulation only with the threshold value of the pixel value as in the past. Physiological normal accumulation can be removed.

The projection data deriving unit 9, the
The

The image information sent to the projection data deriving unit 9 is sent to the

Next, a specific series of processing for image diagnosis support according to the present embodiment will be described with reference to the flowchart of FIG. 3 and the explanatory diagrams of FIGS. FIG. 3 is a flowchart showing a series of flow of image diagnosis support according to the present embodiment, and FIG. 4 is an explanatory diagram schematically showing a PET image when there is physiological normal accumulation. FIG. 5 is an explanatory diagram schematically showing a case where the PET image of FIG. 4 is extracted for each part of the subject M, and FIG. 6 is a criterion for physiological normal accumulation in the chest of the subject M. It is explanatory drawing which showed typically an example.
In this embodiment, a cross-sectional image (coronal sagittal image) based on a tomographic image obtained by the
(Step S1) Writing / Reading PET Image A PET image (in this case, a coronal sagittal image) processed by the projection data deriving unit 9 or the
(Step S2) Accumulation Detection From the PET image read from the
(Step S3) Part Extraction In parallel with step S2, the
Specifically, when imaging was performed with both arms raised, both arms were above the neck, so it moved from the right area to the left area or from the left area to the right area. At this time, if there are three groups of image information (pixel values) intermittently, it can be considered that there is a group of pixel values for one arm, a group of pixel values for the neck, and a group of pixel values for the other arm. Thus, if there are three groups of pixel values intermittently, it can be recognized as a region above the neck, and can be recognized as the head and neck portion R 1 . Conversely, if there is only one group of pixel values, it can be recognized as an area below the neck, and any one of chest R 2 , upper abdomen R 3 , and lower abdomen R 4. Can be recognized. Then, the region between the diaphragm and the neck is recognized as the chest R 2 using the diaphragm as an index, or the region below the neck by a predetermined distance from the neck is recognized as the chest R 2. can do. The upper abdomen R 3 and the lower abdomen R 4 may be similarly recognized and extracted based on the index. The position information extracted in this way is sent to the
(Step S4) Accumulation Determination The accumulation detected by the
The criterion stored in advance in the
In addition, normal accumulation may be observed in the muscles of the whole body about 1 hour and a half after eating. Therefore, the criteria for determining that accumulation in the muscles in such a case is normal accumulation are the respective head and neck R 1 , This is a judgment standard characterized by the chest R 2 , the upper abdomen R 3 , and the lower abdomen R 4 .
Integrated detected by the
The PET image is output to the
In this embodiment, the integration detection in step S2 and the site extraction in step S3 are performed in parallel. However, the site extraction in step S3 may be performed after the integration detection in step S2, or conversely in step S3. The accumulation detection in step S2 may be performed after the part extraction.

In the field of nuclear medicine diagnosis, there is physiological normal accumulation in addition to abnormal accumulation as described above. Accordingly, not all of the accumulations detected by the
In this case, the
The present invention is not limited to the above-described embodiment, and can be modified as follows.
(1) In the above-described embodiments, the PET apparatus has been described as an example. However, the present invention is a SPECT (Single Photon Emission CT) apparatus that reconstructs a tomographic image of a subject by detecting a single γ-ray. It can also be applied.
(2) In the above-described embodiment, the
(3) The above-described embodiments can be applied to an apparatus provided with an external radiation source in the vicinity of the subject in order to perform absorption correction. That is, a radiopharmaceutical to be administered to the subject M, that is, radiation of the same type as the radioisotope (RI) is irradiated from an external radiation source, absorption correction data (transmission data) is obtained, and absorption is performed using this absorption correction data. It is also possible to provide image diagnosis support using the corrected diagnosis data.
(4) In the Example mentioned above, it can apply also to the apparatus which combined the nuclear medicine diagnostic apparatus and X-ray CT apparatus like PET-CT provided with PET apparatus and X-ray CT apparatus.
(5) In the above-described embodiment, the nuclear medicine diagnostic apparatus typified by the PET apparatus and the image diagnosis support apparatus typified by the CAD apparatus are configured separately, and data such as a PET image is transmitted between the apparatuses. Although it was the structure which transmits / receives, you may integrate and comprise as an apparatus by incorporating an image diagnosis assistance apparatus in a nuclear medicine diagnostic apparatus. In this case, for example, the configuration is as shown in FIG. In FIG. 7, the
(6) In the above-described embodiment, the criterion for accumulation determination is stored in advance in the
