【発明の属する技術分野】
本発明は内視鏡による観察下で患者の外科手術を行う内視鏡下外科手術装置に関する。

【従来の技術】
一般に、内視鏡による観察下で患者の体内の外科手術を行う内視鏡下外科手術装置として特開平6−269391号公報がある。ここでは、処置具の先端に触覚センサを設け、この処置具によって得られる触覚情報がTVモニターの内視鏡画像に重ねあわせて表示される体腔内診断処置装置が開示されている。そして、この場合には、外科手術中に、TVモニターの画面を目視することにより、内視鏡画像と触覚情報とを同時に確認することができる。

【発明が解決しようとする課題】
しかしながら、上記特開平6−269391号公報の体腔内診断処置装置の構成では処置具の触覚センサによって得られる触覚情報をTVモニターの画面の内視鏡画像に重ねあわせているため、本来の内視鏡画像が持っている視覚情報(色、明るさ等)が多少なりとも失われている。そのため、TVモニターの画面が見にくくなるので、適切な処置が行えているかの確認、或いはどのような処置をするべきか判断をするには情報が不足する問題がある。

なお、伝統的な開腹手術では、一般に、患部を目で直接確認し、手先の触覚によって患部の感触を確認して手術を進めている。しかし、上記特開平6−269391号公報の体腔内診断処置装置の構成では前述のごとくTVモニターの画面に表示される患部の視覚情報の一部が失われているため、TVモニターの画面を目視した術者が患部の処置に関して正確な判断を行い難い問題がある。

また、術者が患部の処置に関してより正確な判断をするためには、患部に関する情報は視覚情報や、触覚情報だけではなく、他の情報(生体インピーダンスやMRI等)も必要となるので、TVモニターの画面から多くの情報を確認できるようにすることが要望されているのが実状である。

本発明は上記事情に着目してなされたもので、その目的は、術者が処置を進める上で必要な情報を1つのモニターから全て入手することができ、術者が現在行っている処置が適切か否かを容易に判断でき、手術を安全確実に進めることができる内視鏡下外科手術装置を提供することにある。

【課題を解決するための手段】
請求項1の発明は、生体内組織の画像をモニタの画面に表示可能な観察手段と、生体に切除や、凝固などの処置を行う処置手段とを有する内視鏡下外科手術装置において、前記処置手段は、体内に挿入される細長い挿入部の先端部に生体組織を挟む一対の刃を有する処置部が設けられ、前記処置部の一方の刃における生体組織との接触部に配置され、生体情報を検知する第1の生体情報検知手段と、前記処置部の他方の刃における生体組織との接触部に配置され、生体情報を検知する第2の生体情報検知手段とを有する検知手段が設けられるとともに、前記検知手段で得られた生体情報を前記モニタの画面上に画像情報として表示する生体情報表示手段と、前記モニタの画面上に、前記観察手段による前記生体内組織の観察画像の表示領域と、前記生体情報表示手段の画像情報の表示領域とを分割する表示領域分割手段とを設けたことを特徴とする内視鏡下外科手術装置である。
そして、本請求項1の発明では、内視鏡下外科手術中は、処置部における生体組織との接触部の検知手段によって生体情報を検知し、この検知手段で得られた生体情報を生体情報表示手段によってモニタの画面上に画像情報として表示するとともに、表示領域分割手段によってモニタの画面上に、生体内組織の画像の表示領域と、生体情報の画像情報の表示領域とを分割することにより、術者が処置を進める上で必要な全ての情報を1つのモニタの画面から入手できるようにしたものである。
請求項2の発明は、前記処置手段は、超音波メスまたは電気メスであることを特徴とする請求項1に記載の内視鏡下外科手術装置である。
請求項3の発明は、前記検知手段は、前記第1の生体情報検知手段が触覚情報を検知するセンサであり、前記第2の生体情報検知手段が生体の電気情報を検知するセンサであることを特徴とする請求項1に記載の内視鏡下外科手術装置である。
請求項4の発明は、前記触覚情報を検知するセンサは、硬さセンサまたは圧力センサであることを特徴とする請求項3に記載の内視鏡下外科手術装置である。
請求項5の発明は、前記電気情報を検知するセンサは、インピーダンス検知センサであることを特徴とする請求項3に記載の内視鏡下外科手術装置である。
請求項6の発明は、前記検知手段は、前記第1の生体情報検知手段が生体の温度を測定する温度センサであることを特徴とする請求項1に記載の内視鏡下外科手術装置である。
請求項7の発明は、前記温度センサは、熱電対または赤外線温度センサであることを特徴とする請求項5に記載の内視鏡下外科手術装置である。

【発明の実施の形態】
以下、本発明の第1の実施の形態を図1(A),(B)を参照して説明する。図1(A)は本実施の形態の内視鏡下外科手術装置1全体の概略構成を示すものである。本実施の形態の内視鏡下外科手術装置1は生体に切除や、凝固などの処置を行う処置手段としての超音波メス2と、超音波駆動装置3と、硬さ検出ユニット4と、インピーダンス検出ユニット5と、カメラコントロールユニット(CCU)6と、モニター7と、カメラヘッド8と、内視鏡(観察手段)9と、光源装置10とから構成されている。

ここで、生体内組織を観察するための内視鏡9には体内に挿入される細長い挿入部9aと、この挿入部9aの基端部に配設された手元側の操作部9bとが設けられている。さらに、操作部9bには接眼部9cと、ライトガイド接続口金9dとが設けられている。そして、ライトガイド接続口金9dには光源装置10から送られる照明光を導くライトガイドケーブル11が接続されている。

また、接眼部6にはカメラヘッド8が装着されている。このカメラヘッド8の出力側はカメラコントロールユニット6に接続されている。さらに、カメラコントロールユニット6にはモニター7が接続されている。そして、内視鏡9による観察時には光源装置10から送られる照明光が内視鏡9内に導かれ、内視鏡9の挿入部9aの先端部から外部に出射されるようになっている。このとき、照明光によって照らされた生体組織の内視鏡9による観察像は内視鏡9の内部の図示しないレンズ系を介してカメラヘッド8に結像され、カメラヘッド8に内蔵された図示しないCCDによって得られた電気信号がカメラコントロールユニット6に入力されるようになっている。

また、超音波メス2には体内に挿入される細長い挿入部12と、この挿入部12の基端部に配設された手元側の操作部13とが設けられている。ここで、超音波メス2の挿入部12は予め体内に穿刺された図示しないトラカールに挿入され、このトラカールを通して体内に挿入されるようになっている。さらに、挿入部12の先端部には生体組織を挟むハサミ状の処置部14が設けられている。この処置部14には一対の刃(固定刃15および可動刃16)が設けられている。

また、手元側の操作部13には超音波振動子17が内蔵される基体部18と、この基体部18に固定された固定ハンドル19と、この固定ハンドル19に対して連結ピン20を介して開閉自在に連結された可動ハンドル21とが設けられている。ここで、ハンドル21は、図示しない連動機構を介して可動刃16と連結されている。そして、固定ハンドル19に対して可動ハンドル21を開閉操作することにより、処置部14の可動刃16を開閉操作可能になっている。

また、処置部14の固定刃15には生体組織Hとの接触部に硬さセンサ(検知手段)22,22が装着されている。さらに、処置部14の可動刃16には生体組織Hとの接触部にインピーダンス検知部(検知手段)23,23が装着されている。

また、超音波振動子17の発生する超音波振動は固定刃15に伝達可能になっている。そして、処置部14の固定刃15と可動刃16との間で生体組織Hを挟み、この状態で超音波振動子17からの超音波振動を固定刃15に伝達することにより、この処置部14の超音波振動によって生体組織Hを凝固及び切離することができ、出血を非常に少なくできる。

また、挿入部12と手元側の操作部13との間には処置部14と連結された回転操作部24が配設されている。この回転操作部24は手元側の操作部13および挿入部12に対して回動自在に支持されている。そして、回転操作部24によって処置部14を挿入部12の軸回り方向に回転操作可能になっている。

また、操作部13の基体部18には端子25が突設されている。この端子25には超音波駆動装置3と、硬さ検出ユニット4と、インピーダンス検出ユニット5とがそれぞれ接続されている。ここで、超音波駆動装置3は端子25を介して操作部13の基体部18の内部の超音波振動子17を駆動可能になっている。

さらに、硬さ検出ユニット4は端子25を介して固定刃15の硬さセンサ22,22に接続されている。そして、硬さセンサ22から得られる生体組織Hの硬さ情報をこの硬さ検出ユニット4によって映像信号に変換して出力可能になっている。したがって、この硬さ検出ユニット4によって硬さセンサ22で得られた生体情報である生体組織Hの硬さ情報をモニター7の画面上に画像情報として表示する生体情報表示手段が形成されている。ここで、硬さ検出ユニット4にて出力される映像信号は、生体組織Hの硬さを示す曲線や、色でもよいし、数値でもよい。

また、インピーダンス検出ユニット5は端子25を介して可動刃16のインピーダンス検知部23,23に接続されている。そして、インピーダンス検知部23から得られる生体組織Hのインピーダンスをこのインピーダンス検出ユニット5によって映像信号に変換して出力可能になっている。したがって、このインピーダンス検出ユニット5によってインピーダンス検知部23から得られる生体組織Hのインピーダンスをモニター7の画面上に画像情報として表示する生体情報表示手段が形成されている。ここで、インピーダンス検出ユニット5にて出力される映像信号は、生体組織Hのインピーダンスを示す曲線や、色でもよいし、数値でもよい。

さらに、カメラコントロールユニット6には上述したようにカメラヘッド8の出力側が接続されているとともに、硬さ検出ユニット4およびインピーダンス検出ユニット5の出力側がそれぞれ接続されている。そして、内視鏡9による観察像がカメラヘッド8内の図示しないCCDによって電気信号に変換され、このカメラヘッド8から出力された電気信号がカメラコントロールユニット6に入力されるとともに、硬さ検出ユニット4およびインピーダンス検出ユニット5にて出力される各映像信号(生体組織Hの硬さ情報および生体組織Hのインピーダンス)もカメラコントロールユニット6に入力されるようになっている。このとき、カメラコントロールユニット6ではカメラヘッド8から入力された電気信号を映像信号に変換するとともに、硬さ検出ユニット4およびインピーダンス検出ユニット5から入力された各映像信号をモニター7に同時に表示すべく制御可能になっている。

例えば、図1(B)に示すようにモニター7の画面をその大半を占める主画面26と、モニター7の画面の右上に表示される第1の副画面27aと、モニター7の画面の右下に表示される第2の副画面27bとに3分割し、主画面26にカメラヘッド8にて得られた内視鏡9の観察画像を表示するとともに、モニター7の右上の第1の副画面27aに硬さ検出ユニット4から入力された生体組織Hの硬さを示す画像、モニター7の右下の第2の副画面27bにインピーダンス検出ユニット5から入力された生体組織Hのインピーダンスを示す画像をそれぞれ表示可能である。なお、図1(B)では一例として生体の硬さ及びインピーダンスを波形として表示している。これにより、カメラコントロールユニット6によってモニター7の画面上に、生体内組織の観察画像の表示領域である主画面26と、生体情報の画像情報の表示領域である右上の第1の副画面27aおよび右下の第2の副画面27bとを分割する表示領域分割手段が形成されている。

なお、本実施の形態では第1の副画面27aおよび第2の副画面27bはモニター7の画面上で主画面26の右側に表示しているが、左側でも良く、術者の好み、或いは状況に応じて表示場所を切り替えられる構成としても良い。

次に、上記構成の作用について説明する。本実施の形態の内視鏡下外科手術装置1の使用時には術者はモニター7の画面を目視して主画面26の内視鏡9による生体内組織の観察画像を確認しながらハンドル21を操作する。そして、処置部14の固定刃15と可動刃16との間で生体組織Hを挟んだ状態で、生体組織Hをつかむ。このとき、生体組織Hと接触する固定刃15の硬さセンサ22によって生体組織Hの硬さが検出され、同様に生体組織Hと接触する可動刃16のインピーダンス検知部23によって生体組織Hのインピーダンスが検出される。

さらに、硬さセンサ22から得られる生体組織Hの硬さ情報は硬さ検出ユニット4によって映像信号に変換され、同様にインピーダンス検知部23から得られる生体組織Hのインピーダンスはインピーダンス検出ユニット5によって映像信号に変換される。そして、硬さ検出ユニット4およびインピーダンス検出ユニット5から出力される各映像信号(生体組織Hの硬さ情報および生体組織Hのインピーダンス)はカメラコントロールユニット6に入力される。このカメラコントロールユニット6ではカメラヘッド8から入力された電気信号を映像信号に変換するとともに、硬さ検出ユニット4およびインピーダンス検出ユニット5から入力された各映像信号をモニター7に同時に表示すべくモニター7を制御する。

このとき、モニター7の画面上におけるその画面の大半を占める主画面26に内視鏡9による生体内組織の観察画像が表示され、モニター7の画面の右上の第1の副画面27aに硬さ検出ユニット4から入力された生体組織Hの硬さを示す画像、モニター7の右下の第2の副画面27bにインピーダンス検出ユニット5から入力された生体組織Hのインピーダンスを示す画像がそれぞれ表示される。

そして、外科手術中、術者は生体組織Hの凝固・切離の作業中にモニター7の画面を目視して生体組織Hの凝固・切離の作業状態を確認しながら生体組織Hの凝固及び切離などの作業を行う。ここで、生体組織Hの凝固及び切離の作業の際に、モニター7の副画面27a,27bに表示される生体組織Hの硬さ及びインピーダンスの画像には、まさに生体組織Hが凝固・切離される過程が可視化されて表示されている。そのため、術者は生体組織Hの凝固・切離の作業中にモニター7の画面を目視することにより、主画面26に現れる内視鏡9による観察像内の生体組織Hの表面の色、明るさとともに、処置している生体組織Hの硬さとインピーダンスを全てリアルタイムで確認することができる。

そこで、上記構成のものにあっては次の効果を奏する。すなわち、本実施の形態ではモニター7の画面上におけるその画面の大半を占める主画面26に内視鏡9による生体内組織の観察画像が表示され、この主画面26から分離されたモニター7の画面の右上の第1の副画面27aに硬さ検出ユニット4から入力された生体組織Hの硬さを示す画像、モニター7の右下の第2の副画面27bにインピーダンス検出ユニット5から入力された生体組織Hのインピーダンスを示す画像がそれぞれ表示される。そのため、モニター7の主画面26に表示される内視鏡9による生体内組織の観察画像の上にこの部分の生体組織Hの硬さの情報などが画像として重ねて表示されることがないので、モニター7の主画面26に表示される画像における患部の視覚情報の一部が失われるおそれがなく、患部の切除や、凝固が適切に行われているか判断するための情報を、術者によりわかりやすく表示することができる。したがって、従来のように処置している部分の生体組織Hの画像の上にこの部分の生体組織Hの硬さの情報を画像として重ねて表示する場合に比べてモニター7の画面を目視する術者に供給される情報量が多く、手術を進める上で処置が常に適切になる。

特に、本実施の形態の超音波メス2では処置部14の固定刃15と可動刃16との間で生体組織Hを把持する力によって処置部14の切れ味が変わるため、以上の構成とすることで、術者は常に最適な状態、すなわち生体組織Hを最もよく切れる状態に調節しながら手術を進めることができる。そのため、手術時問の短縮が可能である。

なお、本実施の形態ではモニター7の副画面27a,27bに生体情報の画像情報として生体組織Hの硬さ及びインピーダンスを表示する構成のものを示したが、代わりにMRIの断層像、圧力、温度などを生体情報の画像情報としてモニター7の副画面27a,27bに表示する構成にしても良い。例えば、副画面27a,27bにMRIの断層像を表示する場合には、硬さセンサ22に代えてMRIアンテナを使用し、硬さ検出ユニット4はMRIの画像処理ユニットに変更すれば良い。

すなわち、本実施の形態では処置部14の固定刃15における生体接触面側の部分に鋸歯31が形成されている。さらに、この鋸歯31の途中には半導体式圧力センサを利用した硬さセンサ32が配設されている。

さらに、本実施の形態ではモニター7には温度センサで測定した温度の数値をそのまま画像化した数値データ38が表示される。なお、温度の数値データ38に代えて温度に応じて色の表示を変化させる図3(B)に示す色表示部37を設けても良い。例えば、30℃の時には色表示部37に青色を表示させ、120℃の時には色表示部37に赤色を表示させるように設定し、これらの間は色表示部37に表示させる色を連続的に変化させる構成にしても良い。

そこで、本実施の形態ではモニター7に温度センサで測定した温度の数値をそのまま画像化した数値データ38を表示させるようにしたので、第1の実施の形態と同様に、患部の切除や凝固が適切に行われているか判断するための情報を、術者によりわかりやすく表示することができる効果が得られるうえ、本実施の形態では特に、モニター7の画面の数値データ38を目視することにより、生体の温度を確認しながら処置できるので、生体に過度な熱を加えること無く手術を進めることができ、より安全な治療が可能である。

また、図5は本発明の第4の実施の形態を示すものである。本実施の形態は第1の実施の形態(図1(A),(B)参照)の超音波メス2の代わりにバイポーラ方式の電気メス41を使用し、超音波駆動装置3の代わりに高周波発振器42を使用したものである。

そこで、本実施の形態では第1の実施の形態と同様にモニター7の画面上におけるその画面の大半を占める主画面26に内視鏡9による生体内組織の観察画像が表示され、この主画面26から分離されたモニター7の画面の右上の第1の副画面27aに硬さ検出ユニット4から入力された生体組織Hの硬さを示す画像、モニター7の右下の第2の副画面27bにインピーダンス検出ユニット5から入力された生体組織Hのインピーダンスを示す画像がそれぞれ表示されるので、モニター7の主画面26に表示される内視鏡9による生体内組織の観察画像の上にこの部分の生体組織Hの硬さの情報などが画像として重ねて表示されることがない。そのため、モニター7の主画面26に表示される画像における患部の視覚情報の一部が失われるおそれがなく、患部の切除や、凝固が適切に行われているか判断するための情報を、術者によりわかりやすく表示することができるので、従来のように処置している部分の生体組織Hの画像の上にこの部分の生体組織Hの硬さの情報を画像として重ねて表示する場合に比べてモニター7の画面を目視する術者に供給される情報量が多く、手術を進める上で処置が常に適切になる。

なお、本実施の形態では硬さセンサ22及びインピーダンス検知部23のみを記述したが、第1〜第3の各実施の形態に示したMRIアンテナ、圧力センサ、温度センサなどはすべて適用可能である。

また、図6は本発明の第5の実施の形態を示すものである。本実施の形態の内視鏡下外科手術装置51には内視鏡52と、2つのカメラヘッド53A,53Bと、カメラコントロールユニット54と、モニター55とから構成されている。
