本発明は、中空糸型熱交換器に関するものである。

【従来の技術】
熱交換器には、ベローズ管(蛇腹形状)タイプ、パイプ形状タイプ、中空糸(細径チューブ)タイプのものがあり、このうち、中空糸タイプの熱交換器は、血液充填量に対して伝熱面積を大きくできるので、熱交換率を向上させるのに有利である。また、中空糸タイプの熱交換器を用いる場合、泡抜けが良いことから、血液を内部に流し、伝熱媒体を外部に流す内部潅流型の熱交換器が用いられている。

一方、血液充填量を小さくするため、熱交換器を人工肺と一体化したものが製品化されている。人工肺においては、泡抜け性能を考慮する必要がないので、ガス交換能に優れる外部潅流型人工肺を選択する方が有利である。

しかしながら、中空糸タイプの内部潅流型熱交換器を外部潅流型人工肺と一体化しようとすると、部品点数が多くなり大型化することから、熱交換器を人工肺と一体化する際には、熱交換器には、ベローズ管(蛇腹形状)タイプやパイプ形状タイプのものが用いられていた。

【発明が解決しようとする課題】
したがって、本発明の目的は、新規な外部潅流型熱交換器を提供することにある。

本発明の他の目的は、外部潅流型でありながら、泡抜け性が良く、同時に低血液充填量の熱交換器を提供することにある。

【課題を解決するための手段】
上記諸目的は、下記(1)〜(6)により達成される。

(1) 筒状コアと、該筒状コアの外表面に複数回巻き付けられた熱交換用中空糸からなる熱交換層を有する外部潅流型熱交換器であって、該熱交換器の外周に巻き付けられたガス交換用中空糸膜からなる人工肺層を備え、前記熱交換層は、前記熱交換用中空糸が交差する複数の交差部を有し、少なくとも1つの前記交差部は、前記熱交換層の厚さ方向に隣接する他の交差部と重なり合わないことを特徴とする外部潅流型熱交換器。

【発明の実施の形態】
つぎに、図面を参照しながら本発明による外部潅流型熱交換器について説明する。

図1は、本発明による外部潅流型熱交換器の断面図であるが、該外部潅流型熱交換器1は、伝熱媒体流出ポート2および血液流入ポート11を備えた筒状コア3と、その外周部に設けられた熱交換層4と、該熱交換層4を収納する内部ハウジング5と、該熱交換層4の外周部に設けられたガス交換用中空糸膜よりなる人工肺層6と、該熱交換層4および人工肺層6を収納する筒状の外部ハウジング9と、ガス流入ポート7を備えた第1のヘッダー20と、ガス流出ポート8を備えた第2のヘッダー21とからなっている外部潅流型熱交換器である。該外部ハウジングは、さらに血液流出ポート10を備えている。また、内部ハウジング5は、伝熱媒体流入ポート19を備えている。

前記熱交換層4は、ステンレス鋼、アルミニウム等の金属製または樹脂製の中空糸が用いられ、その両端はポッティング材等による隔壁12,12で固定されるとともに、該隔壁12,12の両端部で内部ハウジング5内の空間13,14に連通している。前記中空糸は、内径が0.1〜0.4mm、好ましくは0.15〜0.3mmで外径が0.2〜0.5mm、好ましくは0.25〜0.4mmのものである。その材質としては、後述のとおりである。

該熱交換層4ならびに内部ハウジング5の外周部には、ガス交換用中空糸膜が巻回されて筒状のガス交換用中空糸膜束よりなる人工肺層6が形成されている。筒状のガス交換用中空糸膜束は、中空糸が多層に重なった、言い換えれば、渦巻き状に重なった、もしくは、筒状コアを芯としてリール状に巻き取られた状態となっている。

熱交換用中空糸層は、図2に示すように、筒状コア3の長手方向中央付近に熱交換用中空糸14aが交差する交差部14bを備えるとともに、交差部14bは、交差部14bの上に他の交差部14bが直接重なり合わないようにもしくは交差部14bの上に他の交差部14bが直接重なることが連続しないように、各熱交換用中空糸層の部位により位置が異なっている。

この熱交換層4の両端は、ポッティング材12で固定されているとともに、該ポッティング材等による隔壁12,12の両端部で内部ハウジング5内の空間である第1の伝熱媒体室13および第2の伝熱媒体室21に直通している。

熱交換層4を形成する熱交換用中空糸14aは、図2に示すように、筒状コア3の外用部3に順次巻回されることにより熱交換用中空糸束が多層に重なった、言い換えれば、渦巻き状に重なった、もしくは、筒状コア3を芯としてリール状に巻き取られた状態となっている。さらに、中空糸層は、筒状コア3の長手方向中央付近に熱交換用中空糸14aが交差する交差部14bを備えるとともに、交差部(クロスワインド部)14bは、中空糸層の部位により位置が異なっている。このように、交差部の位置を変化させることにより、図2に示すように、重なり合う層における交差部が重ならず、交差部の重なりによる血液の短絡を防止できる。交差部は、例えば、ほぼ平行に巻回された2〜6本の熱交換用中空糸が、交互に交差することにより連続して形成されている。

本実施態様では、交差部14bは、交差部の上に他の交差部が直接重なり合わないように、中空糸層の部位により位置が異なっている。言い換えれば、直接交差部が重ならなければ、重ならない交差部(言い換えれば、中空糸層)を介して、間接的に交差部の位置が重なっていてもよい。具体的には、熱交換層4(中空糸層)を展開した状態を説明する図3に示すように、熱交換層4の長手方向の中心を中央として、交差部14bの位置は連続して変化している。図3に示す各N+1層といった順に積層される。そして、この例では、N層からN+7層の全8層により1セットとなるように交差部14bの位置は連続して変化し、その後は、その繰り返しとなる。セット数は、熱交換面積によって相違するが、2〜15セット程度が一般的であり、層数としては、3〜20程度が一般的である。

この例では、N層当初においてほぼ熱交換用中空糸束の長手方向中央に位置していた交差部14bは、徐々に熱交換用中空糸束の一端側(右側)に移行し、N+1層の終わり(言い換えれば、N+2層の当初)には、最も一端側(右側)に移行した状態となる。そして再び、交差部は、熱交換用中空糸束の長手方向中央に移行し、N+3層の終わり(言い換えれば、N+4層の当初)には、N層当初と同じ位置である熱交換用中空糸束の長手方向中央に位置する。N層当初の交差部とN+4層当初の交差部は、N+1層、N+2層およびN+3層を介して重なっているが、直接は重なっていない。続いて、交差部は、熱交換用中空糸層の長手方向他端側(左側)に移行し、N+5層の終わり(言い換えれば、N+6層の当初)には、最も他端側(左側)に移行した状態となる。そして再び、交差部は、熱交換用中空糸層の長手方向中央方向に移行し、N+7層の終わりには、N層当初と同じ位置である熱交換用中空糸層の長手方向中央に位置する。N+4層当初の交差部とN+7層終わり(次にN層の当初)交差部は、N+5層、N+6層およびN+7層を介して重なっているが、直接は重なっていない。

また、熱交換層4における交差部14bの位置は、図4に示すようにステップ的に変化しているものであってもよい。具体的には、熱交換層4を展開した状態を説明する図4に示すように、熱交換用中空糸層の長手方向の中心を中央として、交差部14bの位置はステップ的に変化している。図4に示す各層は、熱交換層4における一周する熱交換用中空糸層(熱交換用中空糸束)を示しており、N層の上にN+1層といった順に積層される。そして、この例では、N層からN+3層の全4層により1セットとなるように交差部3bの位置は変化し、その後は、その繰り返しとなる。セット数は、人工肺の膜面積によって相違するが、2〜15セット程度が一般的であり、層数としては、3〜20程度が一般的である。

なお、上述の実施例では、全く交差部が重ならないものとなっている。このような形態が好ましいが、交差部は、交差部の上に他の交差部が重なることが連続しないように、熱交換用中空糸層の部位により位置が異なっているものであってもよい。

具体的には、図5に示す実施態様では、交差部14bは、交差部の上に他の交差部が1つ直接重なり合っているが、交差部の重なりは2つのみであり、3以上にはならないように、言い換えれば、交差部の重なりが連続しないように、中空糸層の部位により位置が異なっている。また、直接交差部の重なりが連続しなければ、重ならない交差部を介して、間接的に交差部が重なっていてもよい。具体的には、熱交換層4を展開した状態を説明する図5に示すように、熱交換用中空糸層の長手方向の中心を中央として、交差部14bの位置はステップ的に変化している。図5に示す各層は、熱交換層における一周する中空糸層を示しており、N層の上にN+1層といった順に積層される。そして、この例では、N層からN+7層の全8層により1セットとなるように交差部14bの位置は変化し、その後は、その繰り返しとなる。セット数は、熱交換面積によって相違するが、2〜15セット程度が一般的であり、層数としては、3〜20程度が一般的である。

この例では、N層およびN+1層において、交差部14bは、ほぼ熱交換用中空糸層の長手方向中央に位置しているため、N層およびN+1層においては、交差部14bは重なり合っている。しかし、N+2層およびN+3層では、熱交換用中空糸層の一端側(右側)に移行し、N+4層およびN+5層では、再び、熱交換用中空糸層の長手方向中央に方向に移行し、N+6層およびN+7層では、熱交換用中空糸層の長手方向他端側(左側)に移行し、次のセットのN層では、再び、熱交換用中空糸層の長手方向中央に方向に移行する。接触する2つの層における交差部は重なるが3層にわたり交差部は重なり合わない。また、N層およびN+1層の交差部は、N+2層およびN+3層を介して、N+4層およびN+5層と位置的には重なるが、直接は重なっていない。

なお、上記のいずれの実施態様においても、すべての交差部は、筒状コアの長手方向中央付近の幅(図3のX、図4のY、図5のZ)100mm以内に位置していることが好ましい。好ましくは、80mm以内である。幅(図3のX、図4のY、図5のZ言い換えれば)、交差部間の最大離間距離は、3mm〜80mmであることが好ましい。特に、4〜40mmであることが好ましい。また、幅(図3のX、図4のY、図5のZ)つまり交差部間の最大離間距離は、熱交換層の長手方向の長さの2〜75%の範囲内、好ましくは、3〜40%であることが好ましい。

そして、熱交換層は、熱交換用中空糸が、1本あるいは複数本同時に、かつすべての中空糸がほぼ一定の間隔となるように前記筒状コアに巻き付けられることにより形成されている。また、中空糸と実質的に平行となっている隣り合う中空糸との距離は、中空糸の外径の1/10〜1/1となっていることが好ましい。

また、中空糸は、1本あるいは複数本同時に、実質的に平行でかつ隣り合う中空糸が実質的に一定の間隔となるように筒状コア3に巻き付けられることが好ましい。これにより、血液の偏流がより抑制できる。また、中空糸は、隣り合う中空糸との距離が、中空糸の外径の1/10〜1/1となっていることが好ましい。さらに、中空糸は、隣り合う中空糸との距離が、30μm〜200μmが好ましく、特に、好ましくは50μm〜180μmである。

熱交換用中空糸に使用される材質としては、ステンレス鋼、アルミニウム等の金属類、ポリウレタン、ポリプロピレン、ポリエチレン、ポリエステル等の高分子材料が用いられる。熱交換層4の外径は、20〜100mmが好適であり、厚さは、1〜4.5mmであることが好ましい。熱交換層4は、筒状コアの外側面と筒状の熱交換層4の内側面間により形成される筒状空間に対する熱交換用中空糸の充填率が60%〜80%であることが好ましい。より好ましくは、65〜70%である。すなわち、80%を超えると、気泡が抜けにくくなり、一方、60%未満では熱交換率が低下するからである。

そして、筒状コア3に熱交換層4の中空糸を巻回した後、両端を隔壁12,15により筒状コア3と筒状の外部ハウジング9に固定し、そして、熱交換層4の両端が切断される。なお、上述したような熱交換層形成装置により作製される熱交換層4は、トラバース位置が層により異なるため、両端部が揃っていない。このため、形成された熱交換層4の両端部は、すべての層が重なり合っている部分において切断することが必要である。すべての層が重なり合っている部分において切断しないと端部が開口しない中空糸が存在するものとなる。

人工肺層6が外面に巻き付けられた熱交換層4の両端は、隔壁15,17により、筒状コア3と筒状の外部ハウジング9の両端部に液密に固定され、筒状中空糸外面と該外部ハウジング9内面間に環状空間(筒状空間)である第2の血液室18が形成される。筒状の外部ハウジング9の側面に形成された血液流出ポート10は、第2の血液室18と連通する。隔壁12,15は、ポリウレタン、シリコーンゴムなどのポッティング材で形成される。

つぎに、本発明による外部潅流型熱交換器についてその作用を説明する。

すなわち、図1に示すように、伝熱媒体流入口19より流入した冷媒媒体、例えば水は、熱交換層4の熱交換用中空糸内部を流通して内部ハウジング5内の空間に流入したのち、伝熱媒体流出ポート2より系外に排出される。一方、血液流入ポート11より流入した血液は、熱交換層4を通過する間に、前記伝熱媒体により熱交換される。続いて人工肺層6を通過する間にガス交換が行なわれて血液中の二酸化炭素が除去されると同時に酸素が導入される。ついで、ガス交換された血液は、第2の血液室18に達し、さらに、血液流出ポート10より系外に排出される。

ガス流入ポート7より流入したガス、例えば純酸素または酸素含有ガスは、第1のガス室16より人工肺層6の各ガス交換用中空糸内を流通して第2のガス室17へ到達する。その間に血液中のガス交換を行なう。第2のガス室に達したガスは、ガス流出ポート8より系外に排出される。

さらに、血液接触面は、抗血栓性表面となっていることが好ましい。抗血栓性表面は、抗血栓性材料を表面に被覆、さらには固定することにより形成できる。抗血栓性材料としては、ヘパリン、ウロキナーゼ、ヒドロキシエチルメタクリレート−スチレン−ヒドロキシエチルメタクリレート共重合体、ポリ(ヒドロキシエチルメタクリレート)などが使用できる。

【実施例】
次に、本発明の外部潅流型熱交換器の具体的実施例および比較例について説明する。

実施例1
図1に示すようなプロピレン製の外部潅流型熱交換器において、外径84mmの筒状コア3の周囲に、内径0.26mm、外径0.32mm、熱交換用中空糸を図6または図7に示すような熱交換層形成装置を用いて、4本の中空糸間隔を100μmに保って巻回し、次に隣接する中空糸間隔も以前に巻回されている中空糸間隔と同じとなるようにし、隣り合う中空糸間隔が一定となるように中空糸を巻き回し、熱交換用中空糸束を作製した。中空糸を筒状コア上に巻き付ける際に、筒状コアを回転させるための回転体と中空糸を編み込むためのワインダーとが、下記式で動かすとともに、ワインダーを軸方向に継続的に微量動かし、図5に示すように、0.75層により1セットとなるように交差部の位置幅±2.5mm以内にて変化させ、全13.3セット、層数10、充填率68%、熱交換面積0.13m2の中空糸熱交換層を作製した。

このとき、熱交換層の層厚は3mm、血液充填量は10.5mlであった。

ついで、その外周部(外径87mm)に、内径260μm、外径320μm、空孔率約40%のポリプロピレン製の多孔質中空糸を、同様の装置により巻回し、全46.5セット、層数35、充填率65、ガス交換面積0.5m2の人工肺層を作成した。

そして、熱交換層および人工肺層の両端をポッティング材により筒状コアとともに筒状の外部ハウジング本体の両端に固定し、熱交換器部を中心にして回転させながら、固定された熱交換層および人工肺層の両端を切断した。そして、筒状ハウジングの両端に、上述した第1のヘッダーおよび第2のヘッダーを取り付け、図1に示すような構造の外部潅流式の熱交換器を作製した。このときの有効長は18mmであった。

【発明の効果】
以上述べたように、本発明による外部潅流型熱交換器は、筒状コアの上に熱交換用中空糸を巻回した熱交換用中空糸層よりなる熱交換層の上にガス交換用中空糸を巻回した人工肺層を設けてなるものであるから、熱交換体層内の気泡を速やかに人工肺層に接触させることにより除去することが可能となる。また、該熱交換器の厚みが5mm以下であるので気泡が抜けきらずにブロックを生じることはない。さらに、該中空糸の外径が0.25〜0.5mmであるので、充分な伝熱面積がとれる。
